Long-term pain and psychosocial outcomes in children following major orthopedic surgery

Chronic pain is a significant public health problem. A large portion of those with chronic pain have had their acute postsurgical pain transition into a chronic postsurgical pain state. The mechanisms contributing to pediatric persistent postsurgical pain is not well understood; however, there is empirical support in the adult literature to suggest that psychosocial factors play a significant role in the maintenance and exacerbation of post-surgical pain. Recent research by our group found high pain prevalence rates up to 5-years post-surgery among children undergoing spinal fusion surgery, particularly among those reporting poor pre-surgical mental health. The current study aims to extend this research by exploring psycho-social functioning and pain among children (10-21 years) who underwent major orthopedic surgery and their parents (n=21 dyads; data collection is ongoing). Measures administered 1-3 years post-surgery included pain ratings, the Bath Adolescent Pain Questionnaire (Child; Parental Impact), Fear of Pain Questionnaire, Functional Disability Inventory, and the Adult Responses to Child Symptoms. Preliminary results found that 52% of patients reported pain in the moderate-severe range in the past 6 months. Additionally, increased child pain was associated with greater child-reported functional disability (p<.01), pain-specific anxiety (p<.01), and fear of pain (p<.05), as well as worse overall emotional functioning (p<.05). Parents of children with increased pain reported worse parental strain (e.g., "found my relationship with my child difficult," p<.05). Identifying correlates of poor long-term outcomes in children with postsurgical pain may prevent the development of chronic pain into adulthood. With recent economic costs of adult chronic pain estimated to be between $560-$635 billion per year research on the role of persistent pain in children is of upmost importance in order to positively impact pre-surgical preparation, postsurgical care, and in potentially preventing disabling pain into adulthood for a population at considerable risk. Introdução: a analgesia multimodal é amplamente usada para o controle da dor perioperatória em um esforço para reduzir o uso de opioides. A duloxetina é um inibidor seletivo da recaptação da serotonina e noradrenalina com eficácia para estados de dor crônica. O objetivo principal deste estudo foi avaliar a eficácia de duas doses orais de 60 mg de duloxetina em termos de consumo de fentanil durante o período pós-operatório em pacientes submetidos à cirurgia eletiva de artrodese de coluna lombar. Método: este estudo foi um ensaio clínico prospectivo, duplo encoberto, aleatório e controlado com placebo. Os pacientes receberam 60 mg de duloxetina ou placebo idêntico uma hora antes da cirurgia e 24 horas depois. Os sujeitos do estudo foram divididos em dois grupos: grupo C (controle) de indivíduos que receberam o placebo; e grupo D (duloxetina) de indivíduos que receberam 60 mg de duloxetina. O consumo total de fentanil administrado pelo próprio paciente em 24 e 48 horas após a cirurgia foi mensurado. Os desfechos secundários foram os escores de dor e a presença ou ausência de efeitos adversos, tais como cefaleia, náuseas, vômitos, prurido, tonturas e sonolência. Resultados: as características demográficas não diferiram entre os grupos. Houve uma diferença significativa no consumo de fentanil nas primeiras 24 horas entre os grupos C e D (diferença média, 223,11 ± 39,32 ?g; p < 0,001). O consumo de fentanil também diferiu entre os grupos C e D após 48 horas (diferença média, 179,35 ± 32,55 ug; p < 0,00). Os escores de dor em mais de 48 horas não diferiram significativamente entre os grupos. A incidência de efeitos colaterais foi semelhante nos dois grupos. Avaliação clínica e radiográfica de pacientes com estenose do canal lombar submetidos a técnica de artrodese lateral minimamente invasiva / Clinic and radiological evaluation of patients with lumbar canal stenosis submitted to lateral minimally invasive technic

Introdução: O acesso lombar transpsoas extremo lateral (LLIF) é uma abordagem retroperitoneal para artrodese intersomática que constitui num implante (cage) o qual se sustenta nas duas margens laterais do anel epifisário, maximizando o apoio na placa terminal. Há também a descompressão indireta das estruturas nervosas através da discectomia e restauro da altura discal pelo cage. Objetivo: Avaliar a eficácia clínica e radiológica da descompressão indireta após o LLIF para estenose do canal lombar. Métodos: Estudo prospectivo clínico e radiológico em único centro, com 20 pacientes portadores de estenose do canal lombar, sem resposta ao tratamento clínico, submetidos à cirurgia de artrodese por via lateral transpsoas minimamente invasiva. Foram coletados dados clínicos e radiológicos no período pré-operatório, em 6 semanas, em três meses, seis e doze meses após a cirurgia. Dados radiológicos medidos em RNM incluíram área e altura dos forâmens, área do canal, diâmetro do canal e altura anterior e posterior do disco intervertebral. Questionários clínicos incluíram EVA e ODI. A incidência de complicações e re-operações foram descritas. Resultados: A idade média do grupo foi de 57,8 anos. Foram tratados ao todo 25 níveis. As cirurgias decorreram sem complicações intra-operatórias. Um caso (5%) recebeu re-operação. A área do canal aumentou de 109 mm para 149mm (p < 0,001) e de 9,3mm para 12,2mm (p < 0,001) em diâmetro anteroposterior. Os valores de área dos forâmens também demonstraram o efeito da descompressão indireta em ambos lados (p < 0,001). A altura do forâmen mostrou ganhos significativos, de 2,8mm de média (p < 0,001). A altura discal foi aumentada em média 25% (p < 0,001). Os resultados de questionários EVA e ODI respondidos pelos pacientes confirmaram o efeito clínico da descompressão indireta demonstrada radiologicamente. Conclusão: Observamos que a descompressão indireta pelo método de LLIF apresenta resultados que comprovam a melhora tanto radiológica quanto clínica com baixa taxa de complicações e re-operações / Background: Lateral transpsoas lateral access (LLIF) is a retroperitoneal approach for intersomatic arthrodesis that constitutes an implant, which is supported on the two lateral margins of the epiphyseal ring, maximizing the support in the terminal plate. Clinical and radiological data were collected in the preoperative period, at 6 weeks, at three, six and twelve months after surgery. Radiological data measured on MRI included area and height of foramen, canal area, canal diameter and anterior and posterior height of the intervertebral disc. Clinical questionnaires included VAS and ODI. Complications and re-operations were described. Results: The mean age of the group was 57.8 years. A total of 25 levels were treated. Surgeries were performed without intraoperative complications. One case (5%) received reoperation. The canal area increased from 109 mm to 149 mm (p < 0.001) and from 9.3 mm to 12.2 mm (p < 0.001) in the anteroposterior diameter. The values of the foramen area also demonstrated the effect of indirect decompression on both sides (p < 0.001). The height of the foramen showed significant gains, of 2.8 mm of mean (p < 0.001). Disc height was increased by an average of 25% (p < 0.001). The results of VAS and ODI questionnaires answered by the patients confirmed the clinical effect radiologically demonstrated of indirect decompression. Conclusion: We observed that indirect decompression by the LLIF method is feasible radiologically and clinically with a low rate of complications and reoperations

Evolution de la posture rachidienne au cours de la croissance normale et modifications dans la scoliose idiopathique de l'adolescent / Spinal posture evolution with normal growth and changes in adolescent idiopathic scoliosis

L’évaluation classique de la posture rachidienne chez les patients porteurs d’une scoliose idiopathique de l’adolescent (SIA) se fait habituellement dans une position contrainte et statique en radiographie standard. Les conséquences sur la fonction rachidienne dynamique de ces déformations sont mal connues. Par ailleurs, les changements de la posture rachidienne au cours de la croissance et avec l’acquisition d’une marche stable n’ont encore jamais été explorés. L’analyse quantifiée du mouvement (AQM) d’une cohorte d’enfants sains a permis de mettre en évidence des modifications de la posture rachidienne dynamique au cours de la croissance, avec une augmentation de la gîte du tronc vers l’avant. Ces modifications s’accompagnaient de modifications anatomiques, en particulier au niveau des facettes articulaires cervicales. En AQM, les patients porteurs d’une SIA avaient des modifications du schéma de marche avec un décalage de phase à la marche entre la rotation des épaules et du bassin chez les patients scoliotiques. Il n’y avait pas de différence dans le schéma de marche entre les patients ayant une courbure thoracique droite et ceux ayant une courbure lombaire gauche. L’analyse de ces patients à 11 mois postopératoire a montré une restauration de certains paramètres grâce à la fusion vertébrale. Notamment, l’arthrodèse rachidienne postérieure de la courbure scoliotique a pour effet de faire disparaître le décalage de phase dans le plan transversal. L’AQM permet de mettre en évidence des modifications de la posture rachidienne chez ces patients et apparait comme un outil d’évaluation fondamental, qui pourrait nous permettre de mieux évaluer les traitements de la SIA. / In adolescent idiopathic scoliosis (AIS) patients, spinal posture is usually assessed in a constraint position with radiographic evaluation. However, the consequences of spinal deformity in these patients on the daily functioning of the spine remains unclear. On the other hand, spinal posture changes with normal growth and mature gait achievement have never been explored.A gait analysis was performed on a cohort of healthy children and highlighted changes in dynamic spinal posture with growth, showing that the trunk was increasingly leaning forward with mature gait achievement. These modifications were associated with anatomical changes, especially in the cervical spine.Changes in gait pattern were also observed in AIS patients thanks to gait analysis. In particular, there was a modification of upper trunk and pelvic rotation during gait. There was no difference in gait pattern according to major curve location. Eleven month postoperatively, our results showed that spinal fusion allowed restoration of a normal gait pattern, especially in the transversal plane.Gait analysis was able to highlight changes in dynamic spinal posture that occur in AIS patients, and thus appears as a major tool for spinal posture assessment in these patients. It could help us to improve the evaluation of the treatments that are proposed for spinal deformity correction.

L'efficacité de l'imagerie guidée sur la douleur, l'anxiété, le coping et le retour aux activités d'adolescents ayant subi une chirurgie correctrice de scoliose

De nombreuses études ont démontré que les adolescents qui subissent une chirurgie correctrice de scoliose éprouvent des douleurs postopératoires intenses et cela pendant plusieurs jours après leur chirurgie. L’originalité de cette étude de type quasi-expérimental, consistait à réaliser un DVD sur une technique d’imagerie guidée et de relaxation combinée à une intervention éducative (gestion de la douleur) sur la douleur, l’anxiété, le coping et le retour aux activités d’adolescents ayant subi une chirurgie correctrice de scoliose. Prenant appui sur la théorie du portillon (Melzack & Casey, 1968) et sur la théorie transactionnelle du stress et coping (Lazarus & Folkman, 1984), quatre hypothèses ont été retenues en vue de vérifier l’effet de cette intervention. Trente et un adolescents, âgés entre 13 et 19 ans ont été répartis en deux groupes de façon aléatoire. Les participantes du groupe expérimental devaient visionner un DVD sur une technique d’imagerie guidée et de relaxation combinée à de l’information sur la gestion de la douleur en période préopératoire, au moment du congé de l’hôpital et ce, au moins trois fois par semaine sur une période d’un mois post congé. De plus, ce groupe continuait à recevoir les soins usuels. Les participants du groupe contrôle ne recevaient que les soins usuels. Les instruments de mesure utilisés furent le questionnaire concis de la douleur (QCD) (Larue F, Colleau, Brasseur, & Cleeland, 1995), l’inventaire d’anxiété situationnelle et de trait d’anxiété (IASTA-Y) (Gauthier & Bouchard, 1993) et l’inventaire du coping face à la douleur pédiatrique (Spicher, 2003). Quatre temps de mesure ont été planifiés, soit la journée avant la chirurgie, au congé de l’hôpital, au 14ième jour post congé et lors de la visite à la clinique d’orthopédie à un mois post congé. Les résultats montraient que les deux groupes étaient équivalents en ce qui concernait les caractéristiques sociodémographiques et les variables dépendantes. Les participants du groupe expérimental présentaient une douleur générale ressentie dans le dernier 24 heures plus basse au 14ième jour (p = 0,012) et un mois après leur congé de l’hôpital (p = 0,049). Certaines activités quotidiennes ont démontré une différence significative au 14ième jour soit : les activités scolaires ou de travail (p = 0,046), et les activités de soutien vital (manger, dormir) (p = 0,024), Par contre, nous n’avons pas observé de différence significative au niveau de l’anxiété situationnelle et des stratégies adaptatives de coping entre les deux groupes. L’auteur conclut que le niveau de douleur postopératoire des adolescents qui ont subi une chirurgie correctrice de scoliose peut être réduit par un visionnement d’un DVD sur une technique d’imagerie guidée et de relaxation combinée à une intervention éducative (information sur la douleur). De plus, ceci semble avoir un effet positif sur le retour plus rapide de certaines activités quotidiennes. / Several studies have shown that a spinal fusion surgery to repair scoliosis is a distressing experience for adolescents. This type of surgery involves extensive surgical lesions to soft tissues and bones that result in severe post-operative pain levels which can last several days after the surgery. Recent studies have reported significant results from interventions testing cognitive-behavioral therapies to help children and adolescents cope with acute pain experiences. The present study was designed to develop and to verify the effectiveness of a home- based guided imagery and relaxation intervention, combined to an educational intervention, presented through a DVD, on adolescent’s postoperative pain, anxiety, coping and activities of daily living following a spinal fusion. The theory of stress and coping (Lazarus and Folkman, 1984) was the conceptual framework for this randomized controlled trial study. Between April and December 2010, thirty-one adolescents 13 to 19 years of age were assigned randomly to receive either standard medical care or standard medical care with the home- based guided intervention (DVD). Participants from the experimental group were introduced to the intervention before the surgery and before discharge home. Postoperative recalls regarding the intervention were offered by telephone two weeks after the patient was discharged home. The Brief Pain Questionnaire (French version) was used to assess adolescents’ usual activities and pain. The anxiety was measured using the child/adolescent version of Spielberger’s State-Trait Anxiety Inventory (French version), and the coping strategies were measured by using the French version of the Pediatric Pain Coping Inventory. Data collection, using these questionnaires, was performed before the surgery, before discharge home, two weeks post-discharge and one month following discharge from the hospital. Results have shown that there were no differences among the groups before the surgery on gender, age, daily activities, anxiety, adaptive strategies on coping and pain. Participants of the experimental group presented a lower mean on pain intensity two weeks (p = 0,012) and one month after being discharged home (p = 0,049). Significant results were obtained in the experimental group at two weeks post-discharge regarding some of the activities of daily living: school or working activities (p = 0,046) and vital support activities (eg. eat, sleep) (p = 0,024), On the other hand, we did not observe significant differences between both groups on all other variables. Guided imagery and relaxation intervention combined to an educational intervention was superior to standard care for the treatment of postoperative pain two weeks and one month after returning home. Furthermore, this intervention seems to have a positive effect on the resuming by adolescents of some of their activities at home.

Cifosectomia em pacientes com mielomeningocele: uma série de casos prospectiva / Kyphectomy in patients with myelomeningocele: a prospective case series

INTRODUÇÃO: A cifose em pacientes com mielomeningocele representa uma deformidade complexa, cujo tratamento é eminentemente cirúrgico, com altas taxas de complicação. OBJETIVOS: O objetivo deste estudo é relatar os resultados e complicações observados no tratamento da cifose associada à mielomeningocele, assim como avaliar se a intervenção tem algum impacto na qualidade de vida relacionada à saúde desses pacientes. MÉTODO: Este é um estudo prospectivo de série de casos operados em três instituições em São Paulo nos anos de 2012 e 2013. A técnica utilizada foi a que é conhecida como \"de Dunn-McCarthy\", e consiste na cifosectomia e fixação posterior, utilizando-se hastes moldadas em \"S\" através dos forâmens de S1 associadas a parafusos pediculares na coluna torácica. Os resultados foram registrados como ângulo da cifose (pelo método de Cobb) antes e depois da cirurgia, além de tempo de cirurgia e de internação e volume de sangue transfundido. Os pacientes foram todos operados pela mesma equipe de cinco cirurgiões e responderam a dois módulos do questionário de qualidade de vida PedsQL, o módulo principal (questionário genérico, QG) e o módulo neuromuscular (MNM). Todos os eventos adversos foram registrados. RESULTADOS: No período do estudo, 31 pacientes foram incluídos no estudo, mas 9,7% (3 pacientes) não completaram o período mínimo de seguimento de 2 anos, sendo excluídos da análise, resultando em 28 pacientes ao final. O tempo médio de seguimento dos pacientes que completaram o estudo foi de 3 anos (± 9 meses). Dos 28 pacientes, 28,6% (8) eram do sexo masculino. A média de idade no ato cirúrgico foi de 10 anos e 7 meses ( ± 20 meses). As cirurgias duraram em média 247 ( ± 68) minutos de pele a pele. Foram retirados em média 1,7 corpos vertebrais. Os pacientes receberam em média 0,6 concentrados de hemácias durante o procedimento. O tempo de internação médio foi de 14 ( ± 27) dias. Os pacientes tinham em média cifose de 130º ( ± 36) no pré-operatório, que foi corrigida para 56º (± 29) em média no pós-operatório imediato e se manteve como 60º ( ± 30) após dois anos. Houve necessidade de reoperação em 67,8% (19) dos pacientes. Desses, 64,2% (18) pacientes necessitaram de reoperações para limpeza cirúrgica e desbridamento e 17,8% (5) dos pacientes necessitaram de retirada do material de síntese para controle da infecção. Houve perda de redução e pseudoartrose em 10,7% (3) dos pacientes. Houve melhora estatisticamente significante (p < 0,001) de 5 pontos em média na qualidade de vida utilizando-se o QG e 8 pontos utilizando-se o MNM, principalmente devido ao componente de saúde física das escalas. CONCLUSÃO: Apesar das altas taxas de reoperação, a cifosectomia demonstrou ser um método eficiente para melhora de qualidade de vida relacionada à saúde nesses pacientes / INTRODUCTION: Severe myelomeningocele-related kyphosis is a complex deformity that demands surgical treatment, with high complication rates. OBJECTIVES: The objective of this study is to evaluate the impact of the kyphectomy on the health-related quality of life of affected patients. METHODS: This is prospective study of a case series of patients operated by the authors at three institutions in São Paulo, Brazil, between 2012 and 2013. The surgical technique used was the one originally described by Dunn-McCarthy, in which posterior fixation is made using \"S-shaped\" rods, inserted through the foramens of S1, associated with pedicle screws in the thoracic spine. All patients were operated by the same team of five surgeons. Cobb angles before and after surgery, surgical time, hospital stay and transfused blood volume were registered. The patients\' caregivers answered both the generic (core) and the specific (neuromuscular module) PedsQL questionnaire before and 2 years after surgery. All adverse events were registered. RESULTS: In the study period, 31 patients were operated but 9,7% (3) were lost for follow up and therefore excluded from the analysis. They were followed-up for 3 years (± 9 months). The average age at surgery of the remaining 28 patients was 10 years and 7 months, and 28,6% (8) patients were males. The surgeries lasted on average 247 ( ± 68) minutes from skin to skin. The mean hospitalization time was 14 (± 27) days. On average, 1.7 vertebral bodies were removed per patient. The mean follow-up time of patients who completed the study was 3 years ( ±9 months). The patients had on average 130º (± 36) kyphosis in the preoperative period, that was corrected to 56º ( ± 29) in the immediate postoperative period and remained as 60º ( ± 30) after two years. Reoperation was needed in 67,8% (19 patients), 64,2% (18) of these for surgical cleaning and debridement. 17,8% (5) patients needed implant removal to control infection. There was loss of reduction and pseudoarthrosis in 10,7% (3) of the patients. There was a significant improvement (p < 0,001) of 5 points on average in the generic and 8 in the neuromuscular PedsQL questionnaires, mainly due to the physical health component of the scales. CONCLUSIONS: Kyphectomy improves health-related quality of life in these patients with myelomeningocele, despite the high reoperation rate

L'efficacité de l'imagerie guidée sur la douleur, l'anxiété, le coping et le retour aux activités d'adolescents ayant subi une chirurgie correctrice de scoliose

De nombreuses études ont démontré que les adolescents qui subissent une chirurgie correctrice de scoliose éprouvent des douleurs postopératoires intenses et cela pendant plusieurs jours après leur chirurgie. L’originalité de cette étude de type quasi-expérimental, consistait à réaliser un DVD sur une technique d’imagerie guidée et de relaxation combinée à une intervention éducative (gestion de la douleur) sur la douleur, l’anxiété, le coping et le retour aux activités d’adolescents ayant subi une chirurgie correctrice de scoliose. Prenant appui sur la théorie du portillon (Melzack & Casey, 1968) et sur la théorie transactionnelle du stress et coping (Lazarus & Folkman, 1984), quatre hypothèses ont été retenues en vue de vérifier l’effet de cette intervention. Trente et un adolescents, âgés entre 13 et 19 ans ont été répartis en deux groupes de façon aléatoire. Les participantes du groupe expérimental devaient visionner un DVD sur une technique d’imagerie guidée et de relaxation combinée à de l’information sur la gestion de la douleur en période préopératoire, au moment du congé de l’hôpital et ce, au moins trois fois par semaine sur une période d’un mois post congé. De plus, ce groupe continuait à recevoir les soins usuels. Les participants du groupe contrôle ne recevaient que les soins usuels. Les instruments de mesure utilisés furent le questionnaire concis de la douleur (QCD) (Larue F, Colleau, Brasseur, & Cleeland, 1995), l’inventaire d’anxiété situationnelle et de trait d’anxiété (IASTA-Y) (Gauthier & Bouchard, 1993) et l’inventaire du coping face à la douleur pédiatrique (Spicher, 2003). Quatre temps de mesure ont été planifiés, soit la journée avant la chirurgie, au congé de l’hôpital, au 14ième jour post congé et lors de la visite à la clinique d’orthopédie à un mois post congé. Les résultats montraient que les deux groupes étaient équivalents en ce qui concernait les caractéristiques sociodémographiques et les variables dépendantes. Les participants du groupe expérimental présentaient une douleur générale ressentie dans le dernier 24 heures plus basse au 14ième jour (p = 0,012) et un mois après leur congé de l’hôpital (p = 0,049). Certaines activités quotidiennes ont démontré une différence significative au 14ième jour soit : les activités scolaires ou de travail (p = 0,046), et les activités de soutien vital (manger, dormir) (p = 0,024), Par contre, nous n’avons pas observé de différence significative au niveau de l’anxiété situationnelle et des stratégies adaptatives de coping entre les deux groupes. L’auteur conclut que le niveau de douleur postopératoire des adolescents qui ont subi une chirurgie correctrice de scoliose peut être réduit par un visionnement d’un DVD sur une technique d’imagerie guidée et de relaxation combinée à une intervention éducative (information sur la douleur). De plus, ceci semble avoir un effet positif sur le retour plus rapide de certaines activités quotidiennes. / Several studies have shown that a spinal fusion surgery to repair scoliosis is a distressing experience for adolescents. This type of surgery involves extensive surgical lesions to soft tissues and bones that result in severe post-operative pain levels which can last several days after the surgery. Recent studies have reported significant results from interventions testing cognitive-behavioral therapies to help children and adolescents cope with acute pain experiences. The present study was designed to develop and to verify the effectiveness of a home- based guided imagery and relaxation intervention, combined to an educational intervention, presented through a DVD, on adolescent’s postoperative pain, anxiety, coping and activities of daily living following a spinal fusion. The theory of stress and coping (Lazarus and Folkman, 1984) was the conceptual framework for this randomized controlled trial study. Between April and December 2010, thirty-one adolescents 13 to 19 years of age were assigned randomly to receive either standard medical care or standard medical care with the home- based guided intervention (DVD). Participants from the experimental group were introduced to the intervention before the surgery and before discharge home. Postoperative recalls regarding the intervention were offered by telephone two weeks after the patient was discharged home. The Brief Pain Questionnaire (French version) was used to assess adolescents’ usual activities and pain. The anxiety was measured using the child/adolescent version of Spielberger’s State-Trait Anxiety Inventory (French version), and the coping strategies were measured by using the French version of the Pediatric Pain Coping Inventory. Data collection, using these questionnaires, was performed before the surgery, before discharge home, two weeks post-discharge and one month following discharge from the hospital. Results have shown that there were no differences among the groups before the surgery on gender, age, daily activities, anxiety, adaptive strategies on coping and pain. Participants of the experimental group presented a lower mean on pain intensity two weeks (p = 0,012) and one month after being discharged home (p = 0,049). Significant results were obtained in the experimental group at two weeks post-discharge regarding some of the activities of daily living: school or working activities (p = 0,046) and vital support activities (eg. eat, sleep) (p = 0,024), On the other hand, we did not observe significant differences between both groups on all other variables. Guided imagery and relaxation intervention combined to an educational intervention was superior to standard care for the treatment of postoperative pain two weeks and one month after returning home. Furthermore, this intervention seems to have a positive effect on the resuming by adolescents of some of their activities at home.
