Determining ideal staple size for small intestinal surgery in catsHiebert, Elizabeth C. 08 March 2022 (has links)
Background: The use of stapling equipment for intestinal surgery in cats is rarely reported, and appropriate staple sizes for cat intestine are unknown.
Objective: To determine staple cartridge sizes for thoracoabdominal (TA) and endoscopic gastrointestinal anastomosis (EndoGIA) that will simultaneously prevent leakage of small intestinal contents while also allowing for sufficient vascular permeability past the staple lines for intestinal healing.
Methods: Two sizes of EndoGIA cartridges (2.0/2.5/3.0mm and 3.0/3.5/4.0mm) and two sizes of TA cartridges (2.5mm and 3.5mm), applied in a transverse manner across fresh cadaveric cat jejunum, were evaluated via intestinal burst pressure testing for maximum intraluminal pressure prior to leaking, and via infusion of an intravascular dye at normal arterial pressures to determine percentage of vascular patency past the staple lines. Vascular patency was compared not only from pre-and post-staple segments of the same intestinal sample, but also EndoGIA vascular patency was evaluated against TA vascular patency.
Results: Two cats met study criteria. All samples had intraluminal burst pressures over twice the chosen minimum (of 30mmHg). Vascular patency post- staple line ranged from 0-90.8%, with the most consistently high numbers noted with the TA 3.5mm cartridges. No EndoGIA cartridge had a post- staple line vascular patency higher than 31.1%, and no intravascular dye was noted in any post- staple line sample in the EndoGIA 2.0/2.5/3.0mm group.
Conclusions: While statistical analysis of the dataset was unable to be performed due to low numbers of samples for comparison, both intestinal intraluminal burst pressure trends and intravascular dye patterns suggested both the TA 3.5mm cartridge and (to a lesser extent) the 3.0/3.5/4.0mm EndoGIA cartridge could provide the ideal combination of intraluminal seal without restriction of vascular access for healing. The intravascular dye infusion technique, developed during this research, shows promise as a future instrument to determine vascular patterns around intestinal implants in cadaveric cat specimens. / Master of Science / Despite the regularity of feline small intestinal surgery, few reports exist of stapled anastomoses in cats, in part due to stapler size limitations. However, the recently developed endoscopic gastrointestinal anastomosis (EndoGIA) stapler shows promise as a future surgical tool for cats because it fits into cat intestine.
In dogs, 3.5mm staples are often chosen for intestinal surgery; however, dog intestine is considerably thicker than cat intestine. The study goal was to evaluate not only intestinal burst pressures (the pressure at which repaired intestine leaks), but also the ability of fluids to flow through blood vessels that cross the staple lines of four stapler cartridge types from two staple lineages (EndoGIA 2.0/2.5/3.0mm, EndoGIA 3.0/3.5/4.0mm, TA 2.5mm, and TA 3.5mm).
The central hypothesis was twofold. First, smaller stapler cartridge sizes (the EndoGIA 2.0/2.5/3.0mm and TA 2.5mm) would have higher intraluminal burst pressures when compared to the larger sizes (the EndoGIA 3.0/3.5/4.0mm). Second, larger stapler cartridges (the EndoGIA 3.0/3.5/4.0mm and the TA 3.5mm) would allow for increased flow of fluids in blood vessels past the stapler lines compared to the smaller cartridges (the EndoGIA 2.0/2.5/3.0mm and the TA 2.5mm).
Two cats were included in the study. Trends in the data suggested that all components of the hypothesis might be proven with further data. However, due to the low number of cats acquired during the study period, the hypotheses could not be verified with statistics. The dye infusion technique to evaluate flow of fluids in blood vessels, developed during this research, shows promise as a future instrument to determine vascular patterns around intestinal implants. Future research should focus on acquiring more cats to have the ability able to perform statistical analyses (and prove the hypothesis), before proceeding with additional related studies.

méthodes ont prouvé leur efficacité. Cependant, malgré des caractéristiques positives
évidentes, ces méthodes peuvent causer un nombre important d'effets indésirables sur la
santé du patient. Les techniques sans fusion pour le traitement de la scoliose semblent être
une alternative perspective de traitement traditionnel, car ils apportent moins de risques et
des complications chirurgicales que les méthodes conventionnelles avec la conservation de
la mobilité du disque intravertébral. Cependant, l'utilisation de techniques mentionnées
exige une connaissance profonde de la modulation de croissance vertébrale. L'objectif
principal de la présente étude est d'estimer le potentiel d'agrafes à l’AMF de moduler la
croissance des vertèbres porcines en mesurant la croissance osseuse sur la plaque de
croissance de vertèbres instrumentées en comparaison avec le groupe contrôle.
La méthode est basée sur la loi de Hueter-Volkmann. Nous avons choisi NiTi agrafes à
l’AMF pour notre étude et les porcs de race Landrace comme un animal expérimental. Les
agrafes ont été insérés sur 5 niveaux thoracique de T6 à T11. En outre, les radiographies ont
été prises toutes les 2 semaines. La présence d'agrafes en alliage à mémoire de forme a
produit la création de courbes scoliotiques significatives dans 4 de 6 animaux chargés et le
ralentissement considérable de la croissance osseuse (jusqu'à 35,4%) comparativement aux
groupes contrôle et sham. L'étude a démontré in vivo le potentiel d'agrafes en alliage à
mémoire de formes de moduler la croissance des vertèbres en créant des courbes
scoliotiques sur les radiographies et en ralentissant le taux de croissance sur les plaques de
croissance instrumenté. La position précise de l'agrafe est essentielle pour la modulation de

study and Landrace domestic pigs as an experimental animal. The staples were inserted on
5 thoracic levels from T6 to T11. Additionally, radiographs were taken every 2 weeks.
The presence of shape memory alloy staples led to creation of significant scoliosis curves in
4 of 6 loaded animals and considerable slowing of bone growth (up to 35.4.%)
comparatively to control and sham groups.
The study has demonstrated in vivo the potential of the shape memory alloy staples to
modulate the growth of vertebrae by creating scoliotic curves on radiographs and by
slowing the growth rate on the instrumented growth plates. Accurate position of the staple
is essential for the bone growth modulation and the development of experimental scoliosis.

Shadows and chivalry : pain, suffering, evil and goodness in the works of George MacDonald and C.S. LewisMcInnis, Jeff January 2004 (has links)
This thesis argues that George MacDonald's literary influence upon C. S. Lewis-concerning the themes of pain, suffering, evil and goodness-was transforming and long-lasting. It is argued in the opening chapter that MacDonald's work had a great deal to do with the change in young Lewis's imagination, helping to convert him from a romantic doubter to a romantic believer in God and his goodness. A review of both writers' first works suggests that such influence may have begun earlier in Lewis's career than has been noticed. The second chapter examines how both authors contended with the problems that pain and suffering present, and how both understood and presented the nature of faith. Differences in their treatment of these subjects are noted, but it is argued that these views and depictions share fundamental elements, and that MacDonald's direct influence can be demonstrated in particular cases. The view that MacDonald was primarily a champion of feelings is challenged, as is the idea that either man's later writing displays a loss of faith in God and his goodness. The third chapter, in specifically refuting the assertion that MacDonald's view of evil was inclusive in the Jungian or dualistic sense, shows how both authors' work maintains an unmistakable distinction between evil fortune and moral evil. The next two chapters examine fundamental similarities in their treatment of evil and goodness. Special care is taken in these two chapters to trace MacDonald's direct influence, especially regarding the differences they believed existed between hell's Pride and what they believed God to be. The fifth chapter reviews their ideas and depictions of heaven in summing up the study's argument concerning the overall influence of MacDonald's writing upon Lewis's imagination-in particular the change in Lewis's understanding of the relations between Spirits, Nature, and God.

Previous issue date: 2013 / The Lion, the Witch and the Wardrobe (1950), by C. S. Lewis, is a typical fantasy narrative. Thus, its form and contents are shaped by characteristics which have traditionally marked the genre, such as the appeal to a medieval atmosphere. The story was adapted to the Hollywood cinema by Andrew Adamson in 2005 keeping the same title. The main goal of this dissertation is to discuss the strategies employed by the director to produce an action blockbuster in the format of the classical Hollywood narrative through the exploration of the fantastic and medieval elements of the novel. It starts from the idea that the film adaptation, although it comes from a text which occupies a peripheral position in the British literary system, manages to stand out in the Hollywood cinematographic system. This research is descriptive and has a qualitative approach, which consists of the reading of both the novel and the film to analyse how fantasy and medievalism are configured in these narratives. The analysis led us to conclude that the adaptation was able to resignify the elements of the novel which are responsible for its marginalization. The theoretical bases for this work are the principles of the descriptive translation studies, especially the concept of rewriting, by Lefevere (2007), and the premises of the polysystem theory, by Even-Zohar (1990). Theoretical references still include Todorov (2010, 2006) and Propp (2006), to characterize the novel as a fantasy narrative; Cecire (2009) and Hobsbawm (1997), to approach the matter of medievalism; Compagnon (1999) and Wellek and Warren (2003), to discuss the literary canon; and Bordwell (1985), to delimitate the properties of the classical Hollywood narration pattern. / O Leão, a Feiticeira e o Guarda-Roupa (1950), de C. S. Lewis, é uma típica narrativa de fantasia. Assim, apresenta características de forma e conteúdo que têm sido tradicionalmente empregadas pelo gênero, entre elas o apelo a uma atmosfera medieval. A história foi adaptada para o cinema de Hollywood em 2005 por Andrew Adamson, com título homônimo. O objetivo principal desta dissertação é discutir as estratégias utilizadas pelo diretor para produzir um blockbuster de ação no formato da clássica narrativa hollywoodiana, explorando os elementos fantásticos e medievais da obra. Parte-se da ideia de que a adaptação, embora oriunda de um texto que ocupa um espaço periférico no sistema literário britânico, consegue se destacar no sistema cinematográfico de Hollywood. A pesquisa tem caráter descritivo, com abordagem qualitativa, que consiste na leitura da obra literária e do filme para analisar como a fantasia e o medievalismo são configurados nessas narrativas. A análise levou-nos a concluir que a adaptação foi capaz de ressignificar os elementos da obra literária responsáveis por sua marginalização. Como base teórica, apoia-se nos princípios dos estudos descritivos de tradução, sobretudo no conceito de reescritura, de Lefevere (2007), e nos pressupostos da teoria dos polissistemas, de Even-Zohar (1990). The discoursal construction of female physical identity in selected works in children's literatureHunt, Sally Ann 20 September 2013 (has links)
This thesis reports on an analysis of the discursive construction of female and male physical identity in children’s literature and explicitly combines corpus linguistic methods with a critical discourse approach. Based on three novels from each of the Chronicles of Narnia and the Harry Potter series, it shows clear gendering of body parts, not only in terms of the purely quantitative preferences for certain body parts to be associated with one or other gender, but in terms of discourse prosody, or the uses to which the body parts are put. Human body parts in these series are mostly used in the following four ways, all of which show differences in realisation in terms of gender: · to describe individuals, physically, in order to distinguish one from the other; · to convey emotion, unintentionally as well as consciously; · for physical interaction between people and · for interaction with the world more broadly: responses to danger and agency, i.e. the ability to act on the world and the nature of what is achieved. The use of body parts by characters to express emotion and act agentively on the world is revealed to be strongly gendered in the two series. I characterise the most prominent patterns in terms of the bodily products blood, sweat and tears, of which the last is strongly connected to female characters, who are generally associated with emotion. The other two, referring to active participation in fighting and injury, as well as agency, are almost exclusively reserved for males, with female characters rendered unable to act on the physical world as a result of overwhelming feelings. The females’ response to danger suggests stereotyped discourses of inequality which see women and girls as requiring protection and being physically incapable. Thus gender is still a particularly salient aspect in these widely-read examples of children’s literature, despite plots which appear to be fairly positive towards women. The strength of the inclusion of a corpus approach in this study lies in its capacity to reveal objective, and often fairly covert, trends in language use. These in turn enrich the critical analysis of discourses in these influential texts, which facilitates social change through linguistic analysis.

Arboreal thresholds - the liminal function of trees in twentieth-century fantasy narrativesPotter, Mary-Anne 09 1900 (has links)
Trees, as threshold beings, effectively blur the line between the real world and fantastical alternate worlds, and destabilise traditional binary classification systems that distinguish humanity, and Culture, from Nature. Though the presence of trees is often peripheral to the main narrative action, their representation is necessary within the fantasy trope. Their consistent inclusion within fantasy texts of the twentieth century demonstrates an enduring arboreal legacy that cannot be disregarded in its contemporary relevance, whether they are represented individually or in collective forests. The purpose of my dissertation is to conduct a study of various prominent fantasy texts of the twentieth century, including the fantasy works of J.R.R. Tolkien, C.S. Lewis, Robert Holdstock, Diana Wynne Jones, Natalie Babbitt, and J.K. Rowling. In scrutinising these texts, and drawing on insights offered by liminal, ecocritical, ecofeminist, mythological and psychological theorists, I identify the primary function of trees within fantasy narratives as liminal: what Victor Turner identifies as a ‘betwixt and between’ state (1991:95) where binaries are suspended in favour of embracing potentiality. This liminality is constituted by three central dimensions: the ecological, the mythological, and the psychological. Each dimension informs the relationship between the arboreal as grounded in reality, and represented in fantasy. Trees, as literary and cinematic arboreal totems are positioned within fantasy narratives in such a way as to emphasise an underlying call to bio-conservatorship, to enable a connection to a larger scope of cultural expectation, and to act as a means through which human self-awareness is developed. / English Studies / D.
