Factors Influencing Bariatric Patients’ Level of Compliance with Supplement Recommendations and Bioavailability of Iron Supplement Formulations in Roux-en-Y Gastric Bypass Patients

<p>In our first study, we explored the barriers to complying with iron supplement recommendations using focus groups. We recruited adults, ages 18-75 years, who have had bariatric surgery at least two months previously to participate in one of four 90-minute focus groups. Participants filled out a survey asking for information on demographics and supplement use, and a facilitator asked a set of pre-determined questions to each group. Responses were written, recorded, transcribed using TranscribeMe (San Francisco CA), and analyzed using NVivo (QSR International Pty Ltd, Doncaster, Victoria). The focus groups contained nineteen participants, five of which had sleeve gastrectomy (SG) and fourteen had Roux-en-Y gastric bypass (RYGB). The average age of the participants was 49.3 ± 9.4 years, and they had undergone surgery 3.9 ± 3.6 years previously. The key factors that influenced participants’ adherence to supplement guidelines were cost, tolerability, and palatability of the supplement, level of knowledge and support from healthcare providers, and convenience of the supplementation regime.</p> <p>The second study was a prospective observational study to determine the bioavailability of ASP compared to FS. Iron deficient RYGB patients ages 18-65 years, who had surgery at least 6 months previously, participated in 8-hour iron absorption tests. Participants received a low-iron breakfast with 65 mg ASP (N=7) or FS (N=3). We assessed serum iron every 30 minutes for 8 hours following the supplementation using a colorimetric assay (South Bend Medical Foundation, South Bend, IN). In participants administered FS, serum iron increased 96.0 ± 27.2 µg/dL compared to baseline, whereas with ASP, serum iron increased 5.8 ± 4.7 µg/dL compared to baseline (<i>P</i> = 0.02). These data indicate that ASP is not as bioavailable as FS in RYGB patients.</p>

Evaluating Different Approaches for the Delivery of Probiotics to Broilers at Different Life Stages

Probiotics are an alternative to increase broiler productivity and flock health. For this reason, the objective of the first study was to evaluate commercially available Bacillus spp. based probiotic products on their ability to reduce Salmonella Heidelberg, in vitro. Results showed that most products reduced the lower concentrations of S. Heidelberg and maintained a reduction for up to 8 h. The second study was conducted to determine if bacterial colonization occurred in a broiler chick after in ovo injection. Using bioluminescence, it was demonstrated that bacteria could migrate and colonize the embryo within 2 h after in ovo injection into the amniotic fluid. The results of these studies demonstrated that probiotics could reduce pathogenic microorganisms and can potentially have a positive impact on the chick before it even hatches and is placed in a broiler house.

Protein Metabolism and Energy Utilization in Females Participating in Endurance Activity: The Effect of Nutritional Supplement Timing

Ten healthy young females (age 22.3 ± 0.2 y, V0₂ₘₐₓ > 40 ml·kg⁻¹·min⁻¹) volunteered to participate in a randomized, double-blinded study that examined the effect of both (A) the timing of PRO/CHO/FAT supplementation, pre- and post-endurance exercise, and (B), extra-energy supplementation ( ~ 400 kcal) on indices of protein metabolism. Each subject completed each of three, 7-day supplementation trials conducted at least 1 week apart: a pre-exercise trial (PRE), a post-exercise (PO) trial, and a post-exercise with extra energy (POE) trial. All females were eumenorheic and tested during the early (days 4-7), middle (days 8-11 ), and late (days 12-14) periods of the follicular phase of their menstrual cycle. Subjects were weighed prior to and following each trial to determine weight loss. During each trial, subjects would consume a checklist diet (days 1-3) and a prepackaged diet (days 4-6) that were isoenergetic, isonitrogenous, and matched for diet composition (carbohydrates, protein, and fat) to the individual's habitual dietary intake. Subjects cycled at 65% V0₂ₘₐₓ for 1 hr, according to their target heart rate on Days 1,3, and 4 and for 1.5 hrs on Day 6 with Days 2 and 5 being rest days. Expired gas and blood samples were collected on Day 6 during exercise (t=0,30,60,90 min) and post exercise for 2 hrs (Blood t=30,60,90,120, Gas t=10,20,30,45,60,90,120 min). On Day 5, expired gas samples were collected post- supplement consumption at the same timepoints as Day 6. 24 h-urine collections were performed on Days 5 and 6. A 75% V0₂ₘₐₓ performance ride to exhaustion was performed in a fasted state on the morning of Day 7. During each trial, subjects had consumed their daily meals at regular intervals by 1500 h prior to the exercise bout (1600 h). In the PRE trial, a PL can+ PRO/CHO/FAT Supplement (Results® + 0.5 g·kg CHO powder[Gatorade®]) was consumed with breakfast (BKFT), and a PL Supplement (I can+ powder) was consumed immediately following the cessation of exercise. In the PO trial, a PL Supplement (2 cans+ powder) was taken at BKFT and the PRO/CHO/F AT Supplement was consumed post-exercise. The POE trial was identical to the PO trial with the exception that subjects received an extra 400 kcal of energy·d-1 at BKFT (Boost®-250 kcal and an extra 150 kcal CHO powder). During the POE checklist diet (Days 1-3), the extra energy was given by the addition of extra items to their daily food consumption. Although plasma estrogen levels were significantly higher during the PO trial (P <0.05), plasma progesterone levels were not significantly different between trials, and both hormones indicated that subjects were in the follicular phase of their menstrual cycle. Significant main effects for time occured in which hematocrit, sodium, potassium, and lactate were observed to increase during exercise and decrease post-exercise. Glucose concentration increase was signiciant for time (P < 0.01) increasing post-exercise after consumption of the supplement. Significant main effects for insulin were observed across time (P < 0.000001) and for trial (P < 0.01) with the POE trial having the largest post-exercise insulin response. No significant differences were observed between the three trials for urinary creatinine (g·24 h⁻¹) and urea (g·24 h⁻¹ and g·g Cr⁻¹). Day 6 urinary 3-methylhistidine (μmol·g Cr⁻¹) was significantly different between trials (POE>PO>PRE)(P < 0.01). Similarly, no significant differences were observed between trials for resting metabolic rate, thermic effect of food, thermic effect of exercise, and excess post-exercise oxygen consumption. However, on Day 5, Apparent Nitrogen Balance trended toward signifcance for trial (P=0.086) becoming significant on Day 6 (P <0.005) wherein POE and PO trials were positive and PRE trial was negative. There was a trend towards increased performance times when comparing the POE and PO trials versus the PRE trial (P =0.074). Furthermore, weight loss was significantly lower with post-exercise supplementation (POE<PO<PRE)(P < 0.01). This study suggests that the protein balance of females regularly engaging in endurance activity may benefit from immediate nutritional supplementation following exercise. / Thesis / Master of Science (MSc)

Prenatal food and micronutrient supplementation to malnourished women in Bangladesh : Effects, Equity, and Cost-effectiveness

Maternal nutrition is closely linked to child health and survival. In Bangladesh there is a high prevalence of undernutrition in the form of chronic energy deficiency [CED, Body Mass Index &lt;18.5 (kg/m2)] in women and low birth weight. The aims of this thesis are to explore women’s perceptions of maternal undernutrition, to analyse the association between prenatal food supplementation and birth weight (BW), to analyse whether food- and multiple micronutrient interventions generate pro-disadvantaged equity in child survival, and to examine whether an early prenatal invitation to food supplementation and multiple micronutrient supplements (MMS) represents value for money in infant survival compared to invitation to food supplementation at usual time combined with 60 mg iron and 400 µg folic acid (standard practice). The study on women’s perceptions (n=236) was nested into the MINIMat randomized trial where women (n=4436) were allocated to early (E), or usual (U) time of invitation to prenatal food supplementation and 30 mg iron with 400 µg folic acid, or 60 mg iron with 400 µg folic acid, or MMS. Live births (n=3625) were followed-up. The analyses of equity and cost-effectiveness were based on this trial. A cohort design (n=619) was employed for the analysis of food supplements and BW. Women perceived maternal undernutrition as a serious health problem and attached very low scores to CED in pregnancy. An average of four months of prenatal food supplementation increased BW by 118 g. An early invitation to prenatal food supplementation and MMS lowered mortality in children before the age of five years and reduced social disparity in child survival chances. An increment from standard practice to E-MMS averted one extra infant death at a cost of US$797 to US$907, and saved one extra life year at a cost of US$27 to US$30. High priority should be given to the nutritional status of pregnant women in societies where undernutrition and food insecurity occurs. Prenatal food supplementation has the potential to significantly increase BW, and an early initiation of prenatal food supplementation combined with MMS was considered cost-effective in lowering infant mortality and increase social equity in child survival chances.

Esta dissertação busca compreender a representação da vida cultural local proposta pelo suplemento semanal Cultura, de Zero Hora, considerando que o jornalismo tem o poder de instituir valores, reforçar consensos e registrar tendências de uma época, e que o suplemento é um espaço que foge da lógica diária, concentrando supostamente o que de mais relevante acontece na área cultural. Para atingir os objetivos propostos, foram utilizados análise de conteúdo e um corpus de 52 edições do caderno publicadas em 2010, totalizando 422 textos. Buscou-se identificar critérios de noticiabilidade, referências espaciais e temporais, temas, gêneros e colaboradores presentes no suplemento; apontar os valores que a publicação estabelece para definir o que é ser culto; e vislumbrar o mapa do sistema cultural proposto por ela. Também foi realizada uma entrevista em profundidade com seu atual editor, Luiz Antônio Araujo, a fim de melhor compreender o contexto de produção. Os resultados indicaram a predominância de atualidade, notoriedade e proximidade entre os valores-notícia; de textos opinativos entre os gêneros; de literatura e livros entre os temas, seguidos por música e história; e professores universitários, agentes culturais e jornalistas entre os colaboradores. No desenho da vida cultural delineado pelo Cultura são valorizados temas e discussões atuais e próximos do leitor, buscando uma identificação entre público e jornal. Um aspecto essencial da cobertura é o prestígio construído pelo caderno ao reafirmar a crença do campo em certos agentes notórios. Mesmo tendo apresentado uma predominância de assuntos internacionais, o veículo mostrou seguir a linha editorial de Zero Hora ao considerar a perspectiva local o norte da cobertura, já que a origem da maioria dos colaboradores é o RS. O caderno parte de um conceito de cultura que privilegia as manifestações artísticas tradicionais, enfatizando suas versões clássicas, ao mesmo tempo em que expande seu escopo acolhendo outros temas. Também funciona seguindo a lógica do consumo de produtos culturais, abundantes em um mercado de fluxo contínuo, relacionando-o ao aperfeiçoamento intelectual do leitor e à distinção que pode ser alcançada por meio dele. No Cultura, o jornalismo atua como espaço de mediação entre o público e o sistema cultural, aproximando leitor e campos especializados. Nesse processo atua a figura do editor que, ao julgar e hierarquizar temas e discussões, define o que o suplemento deixará para a história. In order to reach the goals proposed by this research, Content Analysis (CA) was used with a corpus of the 52 editions of the supplement published in 2010, totalizing 422 texts. The intention was to identify news values, references of time and space, type of texts, subjects and authors; indicate values the supplement establishes to define what means to be cultured; and to catch a glimpse of the intended map of the cultural system. An in-depth interview was made with the current editor of the supplement, Luiz Antônio Araujo, in order to know better its production context. The results indicated the prevalence of present time; notoriety and proximity among the news values; of opinative texts among genres; of literature and books among themes, followed by music and history; and university professors, cultural agents and journalists among the recurrent authors. In the map of cultural life proposed by Cultura are valued current and close to the reader subjects and discussions, seeking to promote and identification between newspaper and its audience. An essential aspect of its coverage is the prestige the supplement built when reiterates the field’s belief in certain notorious agents. Even though it has presented a predominance of international subjects, the supplement follows the editorial line of Zero Hora when considers local perspective as its coverage guide, once the major part of the authors is from RS. The supplement uses a culture concept that privileges subjects related to the traditional artistic expressions, emphasizing its classic versions, in the same time as expands its scope accepting others themes. The supplement follows the consumption’s logic, abundant in a market of continuous flow, relating it to the reader’s intellectual improvement and the distinction that can be reached through it. In Cultura, journalism works also as a mediator between public and the cultural system, approaching reader and specialized fields. In this process acts the central figure of the editor. THE ACUTE IMPACT OF A SINGLE DOSE OF RESVERATROL ON INSULIN SENSITIVITY, WHOLE BODY FAT OXIDATION, AND INTRACELLULAR SIGNALING IN SKELETAL MUSCLE AND ADIPOSE TISSUE IN OVERWEIGHT AND OBESE MEN

Resveratrol (RSV) is a natural compound that improves mitochondrial function and metabolic health in animal models. Thus far, RSV’s effects on metabolic outcomes in humans are controversial, and RSV’s acute mechanism has not yet been confirmed in vivo. This study aimed to evaluate the effect of an acute dose of RSV on insulin sensitivity and fatty acid oxidation, and to determine RSV’s mechanism of action in human skeletal muscle and adipose tissue. Overweight males (n=8; BMI, 30.5±3.6; VO2peak, 34.0±7.3 ml/kg) reported to the lab on 2 occasions and were provided a breakfast supplemented with 0.3g of RSV or a placebo pill. Experiments were performed in random order using a double blind crossover design. Gas exchange measures, blood samples, and skeletal muscle and adipose tissue biopsies were obtained before and 2 hours after the supplement meal. RSV acutely improved insulin sensitivity, but had no effect on fatty acid oxidation. Additionally, RSV supplementation had no effect on the intracellular signaling of key proteins proposed to mediate its effects in skeletal muscle and adipose tissue. Taken together, these results suggest a single dose of RSV can acutely enhance insulin sensitivity, but its mechanism of action is not conserved across species, and its intracellular signaling pathway is different in humans than previously thought. Evaluation of resources for analyzing drug interactions

Objective: The research sought to evaluate seven drug information resources, specifically designed for analyzing drug interactions for scope, completeness, and ease of use, and determine the consistency of content among the seven resources. Methods: A cross-sectional study was conducted where 100 drug-drug and drug-dietary supplement interactions were analyzed using 7 drug information resources: Lexicomp Interactions module, Micromedex Drug Interactions, Clinical Pharmacology Drug Interaction Report, Facts & Comparisons eAnswers, Stockley's Drug Interactions (10th edition), Drug Interactions Analysis and Management (2014), and Drug Interaction Facts (2015). The interaction sample was developed based on published resources and peer input. Two independent reviewers gathered data for each interaction from each of the 7 resources using a common form. Results: Eighty-two drug-drug and 18 drug-dietary supplement interactions were analyzed. Scope scores were higher for Lexicomp Interactions (97.0%), Clinical Pharmacology Drug Interaction Report (97.0%), and Micromedex Drug Interactions (93.0%) compared to all other resources (p<0.05 for each comparison). Overall completeness scores were higher for Micromedex Drug Interactions (median 5, interquartile range [IQR] 4 to 5) compared to all other resources (p<0.01 for each comparison) and were higher for Lexicomp Interactions (median 4, IQR 4 to 5), Facts & Comparisons eAnswers (median 4, IQR 4 to 5), and Drug Interaction Facts (4, IQR 4 to 5) compared to all other resources, except Micromedex (p<0.05 for each comparison). Ease of use, in terms of time to locate information and time to gather information, was similar among resources. Consistency score was higher for Micromedex (69.9%) compared to all other resources (p<0.05 for each comparison). Conclusions: Clinical Pharmacology Drug Interaction Report, Lexicomp Interactions, and Micromedex Drug Interactions scored highest in scope. Micromedex Drug Interactions and Lexicomp Interactions scored highest in completeness. Consistency scores were overall low, but Micromedex Drug Interactions was the highest.

Activité physique et produits dérivés du soja : intérêts dans la prise en charge du stress oxydant associé au diabète de type 1 / Physical activity and soybean products : interests in management of oxidant stress associated to type 1 diabetes

Le Stress Oxydant (SO) est bien connu pour être impliqué dans la survenue et le développement des complications du diabète. Il est causé, au moins en partie, par une élévation importante de la glycémie. Chez les sujets diabétiques, lathérapie par l’insuline est vitale et constitue la pierre angulaire de la thérapie. Mais elle ne permet pas d’éviter à long terme l’apparition de complications, résultant du SO induit par les épisodes hyperglycémiques persistants.Chez le sujet sain, des prises en charge nutritionnelles et/ou par l’activité physique ont démontré avec succès leur capacité à diminuer le SO. L’activité physique bien conduite présente de plus l’avantage d’exercer un effet glucorégulateur.L’objectif de ces travaux de thèse était donc de déterminer si l’association de différentes prises en charge pouvait permettre de mieux lutter contre le SO, en agissant par des mécanismes complémentaires, démontrant alors des effetsadditifs. En s’appuyant sur différents protocoles de supplémentation, de traitement à l’insuline et/ou d’entraînement en endurance, nos travaux ont mis en évidence démontré l’intérêt d’un composé alimentaire dérivé du soja, le PFS, dansla prise en charge du SO associé audiabète. Nos travaux ont également mis en évidence que l’insuline et l’entraînement en endurance améliorait le SO induit par l’hyperglycémie, via des mécanismes partiellement différents. Leur association dans la prise en charge du diabète leur permet de potentialiser mutuellement leurs effets respectifs / Oxidant stress (OS) is well known to contribute to the onset and the development of diabetic complications. OS mainly result from marked rises in blood glucose level. Insulin treatment is required to keep patients alive, but fails to prevent long term complications progression, which result from hyperglycemic induced OS.In healthy people, diet and/or physical activity succeed to significantly reduce OS. Physical activity exhibit moreover glucoregulatory properties.Consequently, the aim of this thesis was to determine if combination of some therapeutic managements could better counteract OS, acting by complementary mechanisms, and so demonstrating additional effects. We used different protocols of supplementation, insulin therapy and endurance training to achieve these objectives. Our research demonstrated the value of soy derived compound, FSP, in management of diabetes-induced OS. We highlighted the abillity of insulin therapy and endurance training to improve hyperglycemic-induced OS, by differents mechanisms. Combination of both in therapeutic management of diabetes allow then to improve each other
