En stor och viktig del av behandlingen utgörs av egenvård och då patienterna är införstådda med vikten av denna. Syfte: Att belysa sjuksköterskans påverkan på kroniska hjärtsviktspatienters egenvård, med fokus på patientundervisning.   Metod: Systematisk litteraturstudie där åtta artiklar valdes ut, kvalitetsgranskades och sedan analyserades till ett resultat. Resultat: Det huvudsakliga resultatet visar att de viktigaste interventionerna för att påverka hjärtsviktspatienters egenvård är kontinuerlig utbildning där patient och sjuksköterska närvarar. En viktig del efter att ha informerat patienterna är att kontrollera deras förståelse för egenvården. Centralt är även sjuksköterskans bemötande och patientens medverkan. Stor vikt läggs vid att patienterna ska motiveras till egenvården och därmed öka följsamheten. Resultatet ger ny kunskap om hur viktig sjuksköterskans påverkan är för egenvården och har hjälpt oss att förstå vilka delar som är väsentligast för att stötta patienter i egenvården. Slutsatser: Sjuksköterskan har en stor påverkan på patientens egenvård. Relationen mellan sjuksköterska och patient bör grundas på en ömsesidig kommunikation, förtroende och engagemang. A novel workflow management system for handling dynamic process adaptation and compliance

Modern enterprise organisations rely on dynamic processes. Generally these processes cannot be modelled once and executed repeatedly without change. Enterprise processes may evolve unpredictably according to situations that cannot always be prescribed. However, no mechanism exists to ensure an updated process does not violate any compliance requirements. Typical workflow processes may follow a process definition and execute several thousand instances using a workflow engine without any changes. This is suitable for routine business processes. However, when business processes need flexibility, adaptive features are needed. Updating processes may violate compliance requirements so automatic verification of compliance checking is necessary. The research work presented in this Thesis investigates the problem of current workflow technology in defining, managing and ensuring the specification and execution of business processes that are dynamic in nature, combined with policy standards throughout the process lifycle. The findings from the literature review and the system requirements are used to design the proposed system architecture. Since a two-tier reference process model is not sufficient as a basis for the reference model for an adaptive and compliance workflow management system, a three-tier process model is proposed. The major components of the architecture consist of process models, business rules and plugin modules. This architecture exhibits the concept of user adaptation with structural checks and dynamic adaptation with data-driven checks. A research prototype - Adaptive and Compliance Workflow Management System (ACWfMS) - was developed based on the proposed system architecture to implement core services of the system for testing and evaluation purposes. The ACWfMS enables the development of a workflow management tool to create or update the process models. It automatically validates compliance requirements and, in the case of violations, visual feedback is presented to the user. In addition, the architecture facilitates process migration to manage specific instances with modified definitions. A case study based on the postgraduate research process domain is discussed.

Ensuring Sustainability Beyond Tier-One : The Issue of Second-Tier Suppliers

Background: Exposure of non-sustainable practices put MNCs in bad light, hurting their brand image. Therefore, it is of great importance to ensure supplier sustainability upwards the chain. Vast focus has been put on the first-tier suppliers. However, there are few studies that have investigated the issue of second-tier supplier sustainability, and there are many real life examples of firms’ who failed to ensure second-tier supplier sustainability. Hence, the need for further investigation within the subject.   Purpose: The purpose of this study was to investigate how two Scandinavian-based MNCs ensure second-tier supplier sustainability within their supply chain.   Methodology: This thesis is conducted with an inductive approach, collecting qualitative data through semi-structured interviews and archival research, at two independent companies. Thus, a multiple case study. Empirical findings were analyzed through a template analysis. Based on the theoretical framework and formulated research questions, categories have been derived, constructing the outline for our empirical findings and analysis.   Conclusions: (I) Social and cultural distance are major determinants settling which second-tier suppliers being regarded as critical. Conclusively, these two factors are the major determinants identifying whom to prioritize. Furthermore, IT is recognized as a vital and beneficial tool to utilize when mapping the network of second-tier suppliers. (II) In order to achieve second-tier supplier compliance of CoC, “the grey area approach” is identified as the used approach by investigated companies. Further, long-term relationships with first-tier suppliers are a major key in order to ensure second-tier sustainability in long-term perspective. (III) Last but not least, first-tier supplier network investigation is regarded prior to choosing a first-tier supplier. However, it is nothing decisive since other factors are more important in the process.

Vårdpersonals följsamhet till handhygienrutiner inom hälso. och sjukvården : En litteraturstudie om faktorer som främjar eller hindrar

Bakgrund: Utövande av handhygienrutiner är en av de viktigaste åtgärderna inom vården för att förhindra vårdrelaterade infektioner. Årligen orsakar vårdrelaterade infektioner stort lidande för patienter och kan leda till bestående skador och döden. All personal inom hälso- och sjukvården är enligt lag ålagda att följa de regler kring basala hygienrutiner som fastställts av Socialstyrelsen. Syfte: Syftet med litteraturstudien var att belysa vad som främjar eller hindrar vårdpersonalens följsamhet till handhygienrutiner inom hälso- och sjukvården, samt äldrevården. Metod: Inklusionskriterierna för litteraturstudien var all vårdpersonal och studenter på sjukhus och äldreboenden. Nio artiklar valdes för att belysa vårdpersonalens upplevelser, perspektiv och erfarenheter. Artiklarna skulle vara skrivna på engelska och peer reviewed samt etiskt granskade. I litteraturstudien har de nio kvalitativa artiklarna kvalitetsbedömts efter SBU:s modell. Artikelsökningarna har gjorts på databaserna: Cinahl, Scopus och PubMed. Exklusionkriterierna var all personal inom tandvården. Analysmetoden innebar att texten kondenserades ned utan att innehållet förlorades. Tio underkategorier framkom i analysen som sedan delades in i två huvudkategorier. Resultat: Resultatet visar att organisatoriska och individuella faktorer har betydelse för utövandet av god handhygien. Information och utbildning höjde följsamheten tillfälligt. Att ha bra förebilder inom yrkesområdet sågs som viktigt och höjde vårdpersonalens benägenhet till att utföra hygienrutiner. Slutsats: Denna litteraturstudie visar att utförande av handhygien är beroende både av organisatoriska- och individuella faktorer. Det finns många metoder för att öka följsamheten, men författarna har inte funnit någon metod som gör att bra rutiner bibehålls. Orsaker som nämnts är bland annat tidsbrist och attityd till att utföra handhygienrutiner.

Bakgrund: Vårdrelaterade infektioner är vanligt förekommande över hela världen.Det innebär en hög kostnad, ökat lidande och medför fler vårddygn för patienten.God handhygien är den viktigaste påverkbara faktorn för att minska vårdrelateradeinfektioner. Det är sjuksköterskans ansvar att säkerställa rutiner för en godhandhygien. Syfte: Syftet är att beskriva vårdpersonalens följsamhet till riktlinjer för handhygien. Metod: En litteraturstudie där datainsamlingen valdes att göra från 10 olikakvantitativa studier. Genom analysen gjordes en sammanställning av de resultat somsvarade på vårt syfte och frågeställningar. Resultat: Resultatet redovisas i tre stapeldiagram med tillhörande text. Förstarubriken är följsamhet till riktlinjer för handhygien medel %. De två andradiagrammen är följsamhet till riktlinjer för handhygien före och efter patientkontaktsamt sjuksköterskor och läkares följsamhet till riktlinjer för handhygien. Huvudfynden i litteraturstudien visade att följsamhet till handhygien varierar i allastudier och att det är statistiskt säkerställt att fler vårdpersonal utför handhygienefter patientkontakt än före samt att fler sjuksköterskor än läkare följde riktlinjer förhandhygien. Konklusion: Det finns brister i följsamhet till handhygien. Kunskapen omsmittspridning och handhygien mellan olika arbetsmoment i den patientnära vårdenär otillräcklig. Ytterligare forskning behövs och alla länder måste sträva efter sammamål.

Treatment Adherence in Internet-Based CBT : The Effects of Presentation, Support and Motivation

Homework assignments that patient work with between sessions is a key component in both face-to-face and Internet-based Cognitive Behavior Therapy (CBT). However, adherence to assignments is often low and it is largely unclear what factors predict or affect treatment adherence, and in the end, treatment outcomes. The overall aim of this thesis was to investigate if treatment presentation and therapist support can affect adherence and treatment outcome in internet-based CBT, whether adherence can be predicted by motivation variables and to compare differences in face-to-face and online conditions in this regard. A randomized controlled trial with a brief online relaxation program for people with stress and anxiety symptoms was conducted (n = 162). Participants in the enhanced support conditions completed a larger proportion of the online treatment but adherence was not affected by enhanced treatment presentation (Study I). Participants reported reduced symptoms of stress and anxiety after the relaxation program but there were no significant additional effects of enhanced presentation or support (Study II). Participants who adhered to the prescribed assignments reported lower symptom levels at study end, regardless of treatment conditions. Adherence to the online treatment was predicted by subject factors such as treatment credibility prior to the treatment and intrinsic motivation during the treatment (Study III). To further elucidate how motivation may affect adherence, an experiment with a one-session psychotherapy model was subsequently conducted (n = 100). Participants who were randomized to the face-to-face condition reported higher motivation for the assignment and completed significantly more of the homework compared to participants in the online condition (Study IV). Self-reported intrinsic motivation could predict adherence in both conditions while new motivational variables were identified specifically for the online condition. The results from these studies confirm that adherence to assignments in Internet-based CBT is difficult to affect with treatment features but can be predicted early in treatment by subject factors such as treatment credibility and motivation. How such motivational variables can be affected to improve treatments is still unclear.

Applying the theory of planned behaviour and the commonsense model of self-regulation to fitness, activity and treatment adherence in elderly patients with congestive heart failure

Congestive heart failure (CHF) is one of the leading causes of hospitalisation, morbidity and mortality in the UK. The incidence and prevalence of CHF is expected to increase due to the aging population and improved survival in heart disease. Exercise has been recognised as a valuable treatment and has proved to be beneficial in CHF. The present study applied the Theory of Planned Behaviour (TPB) and the Commonsense Model of Self-regulation to fitness, daily activity and medication-adherence in elderly patients with CHF. The study was in parallel with a randomised controlled trial of a 3-month exercise programme. A TPB questionnaire was used at baseline and at 3 months. Illness representations were assessed by IPQ-R at baseline. Fitness (measured by 6 minute walk test) and daily activity (measured by an accelerometer) were assessed at baseline, 3 and 6 months. ACEI-adherence (assessed by measuring serum ACE level) was assessed at baseline. The main findings from 81 CHF patients (mean age = 81 years) showed that Subjective norm (SN) was the only predictor of Intention (IN1) at baseline; Attitude, Perceived behavioural control (PBC) were predictors of INT at 3 months; fitness was predicted by PBC at 3 months. The exercise intervention led to significant changes in Attitude and PBe. The finding also indicated that the participants were more likely to attribute their illness to Chance, BiolOgical factors and God. Identity and Illness coherence predicted fitness, and Consequences predicted daily activity. Participants who believed that their illness was chronic or serious were less likely to adhere to ACEI medication. Conclusion: both the TPB and IPQ-R were useful instruments to predict behaviours in elderly patients with CHF. The IPQ-R had a greater predictive power than the TPB in this population. Illness representations may play a role in influencing the formation of intention as background factors.

Increasing child compliance: Fathers flying solo

This study investigated the effectiveness of the Triple P Programme to reduce non-compliant behaviour in three solo father families. Using behavioural monitoring, observational coding, and self-report questionnaires, outcome measures included non-compliant behaviour, the quality of the parent-child relationship, parenting efficacy, parental mental health, and parenting practices. A measure of change was also included to identify change points in the therapeutic process. Results suggest that parent training is effective in the reduction of non-compliant behaviour, as positive changes were found across all the measures employed. This early intervention has the potential to increase child compliance with solo fathers, and contributes to the knowledge base about this under-reported population. Limitations of the study and directions for future research are discussed.
