Individual goal-oriented cognitive rehabilitation to improve everyday functioning for people with early-stage dementia: a multi-centre randomised controlled trial (the GREAT trial)

Yes / Objectives: To determine whether individual goal-oriented cognitive rehabilitation (CR) improves everyday functioning for people with mild-to-moderate dementia. Design and methods: Parallel group multi-centre single-blind randomised controlled trial (RCT) comparing CR added to usual treatment (CR) with usual treatment alone (TAU) for people with an ICD-10 diagnosis of Alzheimer’s, vascular or mixed dementia and mild-to-moderate cognitive impairment (MMSE score ≥ 18), and with a family member willing to contribute. Participants allocated to CR received ten weekly sessions over three months and four maintenance sessions over six months. Participants were followed up three and nine months post-randomisation by blinded researchers. The primary outcome was self-reported goal attainment at three months. Secondary outcomes at three and nine months included informant-reported goal attainment, quality of life, mood, self-efficacy, and cognition, and study partner stress and quality of life. Results: We randomised (1:1) 475 people with dementia; 445 (CR=281) were included in the intention to treat analysis at three months, and 426 (CR=208) at nine months. At three months there were statistically-significant large positive effects for participant-rated goal attainment (d=0.97, 95% CI 0.75 to 1.19), corroborated by informant ratings (d=1.11, 0.89 to 1.34). These effects were maintained at nine months for both participant (d=0.94, 0.71 to 1.17) and informant ratings (d=0.96, 0.73 to 1.2). The observed gains related to goals directly targeted in the therapy. There were no significant differences in secondary outcomes. Conclusions: Cognitive rehabilitation enables people with early-stage dementia to improve their everyday functioning in relation to individual goals targeted in the therapy. / National Institute for Health, Health Technology Assessment Programme, Grant/Award Number: 11/15/04

This thesis deals with the neuropsychological diagnosis of Alzheimer's disease. The aim is to evaluate the psychometric characteristics ofthe new Czech translation of the Montreal Cognitive Assessment (MoCA) by comparison with the Mini-Mental State Examination (MMSE), a method widely used by doctors inscreening diagnostics of Alzheimer's disease. The theoretical part deals with the diagnostics of Alzheimer's disease and mild cognitive impairment. We describe international diagnostic criteria of cognitive disorders and provide an overview of the screening neuropsychological methods most commonly used by Czech specialists. We summarize the current psychometric and psychodiagnostic findings on these methods and focuse on description of MMSE and MoCA. In the empirical part we compare Czech version of MMSE and MoCA-CZ (the new Czech translation of the test). We examined 38 patients with Alzheimer's disease and 70 cognitively healthy seniors. The results show that MoCA-CZ is sufficiently valid and reliable screening method that accurately distinguishbetween healthy subjects and patiens with Alzheimer's disease. Action des rétinoïdes et processus neurodégénératifs associés à la maladie d'Alzheimer

Un ensemble des données cohérentes de la littérature plaide en faveur d'une relation entre une baisse d'activité de la voie de signalisation de la vitamine A, des altérations de la plasticité synaptique et des déficits mnésiques spécifiques associés au vieillissement. Une diminution de l'activité de cette voie de signalisation est également évoquée dans les processus neurodégénératifs caractéristiques de la maladie d'Alzheimer. Dans ce contexte, les objectifs de ce travail étaient de mieux comprendre les conséquences neuro-anatomiques et fonctionnelles d'une baisse d'activité de la voie de signalisation de la vitamine A. Notre approche expérimentale a mis en œuvre 2 modèles animaux, un modèle de carence vitaminique A qui induit spécifiquement une baisse d'activité de sa voie de signalisation et un modèle d'hypothyroïdie dont il a été montré qu'il induit aussi une hypoactivité de la voie de signalisation de la vitamine A. La démarche expérimentale conduite chez les rats carencés en vitamine A comporte deux volets : (i) un volet mettant en œuvre l’imagerie et la spectroscopie RMN, (ii) un volet moléculaire consacrée à l’étude de l’expression de gènes cibles des rétinoïdes impliqués dans le processus amyloïdogène. Les mesures ont été réalisées, d'une part, chez des animaux soumis à un régime dépourvu en vitamine A pendant 10 semaines et d'autre part, chez des animaux soumis à ce même régime pendant une durée de 13 ou 14 semaines. Une partie des animaux carencés a été traitée par de l'AR. Les résultats montrent que dès 10 semaines de carence, les animaux présentent une altération du métabolisme et de son action cellulaire de la vitamine A qui se traduit par (i) une diminution significative du taux de vitamine A sérique, (ii) une diminution du taux d'ARNm codant pour les récepteurs RAR, dans le cerveau entier, le striatum, l'hippocampe et de manière moins prononcée le cortex des animaux. Après 10 semaines de régime dépourvu en vitamine A, des modifications métaboliques ont été mises en évidence essentiellement dans le cortex. Elles se traduisent par une hausse du (i) NAA/Cr, marqueur de la densité neuronale corrigée par une administration d'AR, et (ii) du GSH/Cr, indicateur du potentiel antioxydant cellulaire dans cette structure. Au plan anatomique, un ralentissement de la croissance cérébrale a été observé dés la 7ème semaine de régime. Une diminution du volume hippocampique et une augmentation des espaces ventriculaires ont été observées à partir de 11 semaines de carence. Au plan moléculaire, aucune modification de l'expression du gène codant pour APP, ou du rapport APP770-751/APP695, considéré comme un indicateur précoce de la MA n'a été observée après 10 semaines de carence. Après 14 semaines de régime dépourvu en vitamine A, de profondes modifications métaboliques sont observées dans les trois structures à savoir le cortex, l’hippocampe et le striatum. Au plan moléculaire, les principaux résultats suggèrent un basculement du processus biochimique de dégradation de la protéine APP en faveur de la voie amyloïdogénique dans le cortex, et par voie de conséquence en faveur de la formation du peptide Aß. Cependant, aucune modification du taux protéique des peptides Aß n'a été mise en évidence dans le cortex et l'hippocampe des rats carencés. Le modèle d'hypothyroïdie que nous avons mis en oeuvre entraine bien une hypoactivité de la voie de signalisation de la T3, observée dans l'hippocampe des animaux et une diminution du taux d'ARNm codant pour RARß observée dans le cortex des rats hypothyroïdiens. Au plan moléculaire, l'augmentation du rapport APP770-751/APP695 a été observée chez les rats rendus hypothyroïdiens par rapport aux rats témoins. Comme chez les rats carencés en vitamine A, les indicateurs de la voie physiologique ne sont que très faiblement affectés chez les rats rendus hypothyroïdiens. / Some data reveal that retinoid hyposignalling, presumably resulting from decreased bioavailability of retinoid ligands naturally, was shown to result in aging-related synaptic plasticity and long term potentiation (LTP) alterations as well as in aging-related decline of cognitive function. Moreover, genetic, metabolic and dietary evidence has been provided for a defective retinoid metabolism in Alzheimer disease (AD). Thus, key steps of the amyloid production process are under the control of proteins whose expression is positively regulated by RA in vitro. In this context, the aims of this work were to better understand neuro-anatomical and functionnal consequences of retinoid signaling brain hypoactivity. Our experimental method uses two animal models: a Vitamin A deficiency model which induce especially an hypoactivity of retinoid pathway, and an hypothyroid model which was also characterized by an hypoactivity of retinoid pathway. In the fisrt model, two main approch were used : (i) an NMR imaging and spectroscopy approach, (ii) a molecular approach to study expression of retinoid target genes implicated in amyloidogenic process. NMR results showed that VAD induces severe anatomic and metabolic disorders in particular a slowing of brain growth, hippocampus atrophy, and a decrease of NAA/Cr, marker of neuronal density which was observed in cortex, hippocampus and striatum. Molecular results reveal a vitamin A deficiency-related dysregulation of the amyloid pathway in the cortex of rats, which is known to be the first brain area altered by AD development. In this area, 14 weeks of deprived diet induces physiological dysregulation in the modulation of RA target genes leading to an increased amount of ADAM10, BACE and PS1, with some modifications in amyloidogenic pathway but without increased amount of Aß peptides. In hypothyroid model, molecular results suggests that adult onset-hypothyroidism may induce the amyloidogenic pathway of APP processing by increasing activity of ß and ?secretases and levels of amyloid peptides mainly in hippocampus. Together these data argue for the idea that hypoactivity of retinoid signalling which occurs naturally with aging could be a factor participating in accelerating aging and that hypothyroidism that become more prevalent with advancing age, could increase, via a hyposignaling of T3 pathway, the vulnerability of amyloidogenic pathway of APP processing as well as of other clinical symptoms of AD.

Approche écologique de l'évaluation de la mémoire épisodique et de la navigation spatiale dans la maladie d'Alzheimer / Ecological approach of episodic memory and spatial navigation assessment in Alzheimer disease

Ces dernières années, la prise en compte des difficultés quotidiennes des personnes a largement évolué pour être aujourd’hui au centre des préoccupations des cliniciens et chercheurs. Dans le vieillissement en particulier, les difficultés quotidiennes tiennent une place particulière car elles constituent un critère de diagnostic de la démence mais également un facteur de risque de celle-ci, même en l’absence de trouble cognitif avéré.Leur prise en compte relève donc d’un intérêt majeur mais pose la question de leur mesure, notamment au vu des relations non systématiques qu’elles entretiennent avec les troubles cognitifs évalués par les tests neuropsychologiques traditionnels et les plaintes des sujets. Les approches écologiques, la réalité virtuelle en particulier, dans lesquelles s’inscrivent nos travaux, tentent de proposer une évaluation de la cognition « quotidienne » en espérant par ce bais, fournir des mesures qui soient à la fois précises, spécifiques et reflétant le fonctionnement quotidien de la personne. L’utilisation de deux environnements virtuels, représentant un appartement et un quartier résidentiel nous a permis de dégager des profils cognitifs spécifiques associés au vieillissement normal et à la maladie d’Alzheimer dans le cadre de la mémoire épisodique et de navigation spatiale. Ces profils sont discutés en référence au modèle multifactoriel du vieillissement et au regard de l’objectivation des difficultés quotidiennes des personnes. L’application de ces recherches pour le développement d’aides à l’autonomie des âgés est proposé comme perspective de travail. / These last years, taking account of everyday difficulties has largely improved and is at the core of preoccupations of clinicians and researchers.Particularly, everyday difficulties hold a special place in aging because of their potential value as a diagnostic criterion, but also as a risk factor, for dementia, even in the absence of cognitive impairment. However, despite being taken into consideration and their major interest, their assessment is being questioned, especially regarding their inconsistent relationships with cognitive impairments assessed by traditional neuropsychological tests and subjects’ complaints. The purpose of ecological approaches, particularly those using virtual reality technologies, in which our work is framed, is to assess the “daily” cognition, by which we hope providing measures that are accurate, specific and reflecting the one’s daily functioning. The use of two virtual environments, an apartment and a residential district, has allowed us identifying specific cognitive patterns of episodic memory and spatial navigation associated with normal aging and Alzheimer's disease. These patterns are discussed according to the multifactorial model of aging and the objective assessment of daily difficulties.The application of this research to the development of aids for the autonomy of the elderly is proposed as a work perspective.

Biomarcadores na doença de Alzheimer: GSK3B e PLA2 na resposta aos inibidores de colinesterase / Biomarkers in Alzheirmer\'s disease: GSK3B and PLA2 in response to cholinesterase inhibitors

A Doença de Alzheimer (DA) é uma desordem neurodegenerativa progressiva que causa comprometimento cognitivo e demência. O diagnóstico é baseado em parâmetros clínicos, mas sua confirmação é post-mortem, após avaliação patológica durante a autópsia. Os tratamentos disponíveis para a DA são os inibidores da colinesterase (IChEs) e os antagonistas de receptores de N-metil-D-aspartato (NMDA), sendo que os IChEs compõe o principal grupo. Diversos estudos tem mostrado um efeito neuroprotetor dos IChEs, levando a alterações na patogênese da DA. Avaliar e mensurar essas alterações são papeis atribuídos aos biomarcadores. Neste sentido podemos destacar a fosfolipase A2 (PLA2), a principal responsável pelo metabolismo de fosfolípides de membrana, e que tem sido achada diminuída na DA, assim como a glicogênio sintase-quinase (GSK), responsável pela fosforilação da proteína Tau, que é um dos processos alterados na DA. O objetivo deste trabalho foi avaliar o efeito do tratamento com IChE sobre a atividade da PLA2 e expressão da GSK3B em plaquetas de 30 pacientes com DA após 3 e 6 meses de tratamento. Como grupo controle foram investigados 42 individuos idosos sem doença neurodegenerativa. Encontramos nos pacientes com DA antes do tratamento uma diminuição da atividade da iPLA2 quando comparada ao grupo controle. Após três e seis meses de tratamento a PLA2 aumentou, voltando ao nível dos controles. Os pacientes que apresentaram um aumento maior da iPLA2 apos 3 meses de tratamento apresentaram melhora cognitiva mais marcante após seis meses de tratamento, avaliado pelo CAMCOG. Apos 6 meses de tratamento encontramos um inativação da GSK3B, medida por um aumento em sua forma fosforilada. Nossos resultados sugerem que o donepezil apresenta propriedades modificadoras na doença de Alzheimer, e ainda que a medida da atividade da iPLA2 poderia ser usada como marcador de resposta terapêutica ao donepezil e, possivelmente, a outros IChEs, na doença de Alzheimer / Alzheimer\'s disease (AD) is a progressive neurodegenerative disorder that causes dementia and cognitive impairment. The Diagnosis is based on clinical parameters, but confirmation is post-mortem after pathologic evaluation during autopsy. The treatments available for AD are cholinesterase inhibitors (IChEs) and N-methyl-D-aspartate (NMDA) antagonists. The main group comprises the IChEs. Several studies have shown a neuroprotective effect of IChEs, leading to alterations in the pathogenesis of AD. Evaluate and measure these changes are assigned to biomarkers. In this regard we can highlight the phospholipase A2 (PLA2) the main enzyme in membrane phospholipids metabolism and that has been found decreased in AD as well as Glycogen Synthase kinase (GSK), a major responsible for tau phosphorylation which is one processes altered in AD. The objective of this study was to evaluate the effect of treatment with IChE on PLA2 activity and GSK3B expression in platelet of 30 AD patients after 3 and 6 months of treatment. The control group comprised 42 elderly individuals without neurodegenerative disease The results obtained were a decreased iPLA2 activity in patients with AD before treatment as compared to controls. After 3 and 6 months of treatment, we observed a significant increase in iPLA2 activity, restoring enzymatic activity similar to that observed among control. The patients who showed higher iPLA2 activity in the first three months were those showing cognitive improvement after six months of treatment, measured by CAMCOG. After 6 months of treatment a GSK3B inactivation were found, measured by an increase in its phosphorylated form. Our results suggest that donepezil present modifying properties in Alzheimer disease and that iPLA2 activity measurement could be used as a marker of therapeutic response to donepezil and possibly other IChEs in Alzheimer\'s disease

Análise comparativa do eletroencefalograma em pacientes com doença de Alzheimer e lesão axonial difusa / Comparative analysis of the electroencephalogram in patients with Alzheimer\'s disease and diffuse axonal injury

Introdução: Entre as lesões encefálicas adquiridas (LEAs) causadas por processos degenerativos, destaca-se a doença de Alzheimer (DA), que é uma demência que acomete uma grande parcela da população idosa e caracteriza-se pela presença de emaranhados neurofibrilares e placas senis. Exames de ressonância magnética (RM) mostram atrofia do córtex entorrinal, hipocampo, amígdala e da região para-hipocampal. Já a tomografia por emissão de pósitrons (PET) aponta redução do metabolismo cerebral de glicose em regiões como o lobo temporal e cíngulo posterior. O alentecimento do eletroencefalograma (EEG) pelo aumento das ondas teta e a diminuição da frequência alfa são mais evidentes em indivíduos com algum tipo de lesão encefálica. O traumatismo cranioencefálico (TCE) caracteriza-se por ser uma LEA não degenerativa e não congênita e é provocado por uma força mecânica externa. Espera-se um prejuízo, permanente ou temporário, nas funções cognitiva, física e psicossocial, com diminuição ou alteração do estado de consciência. Uma das principais causas de TCE é a lesão axonial difusa (LAD), causada por mecanismos de aceleração-desaceleração. Frequentemente as regiões ventral e lateral dos lobos frontal e temporal são danificadas. Objetivo: Entender as diferenças dos mecanismos funcionais entre os grupos - DA e LAD - com queixa de memória, sob o ponto de vista eletroencefalográfico. Métodos: Participaram deste estudo 85 indivíduos adultos. Destes, 34 haviam recebido o diagnóstico de DA, 32 de LAD e 19 eram adultos saudáveis. Foram aplicados o Mini-Exame do Estado Mental (MEEM), o teste do desenho do relógio (TDR) e o teste de fluência verbal (FV) para a categoria animais. Os indivíduos foram submetidos ao EEG de alta resolução com 128 canais. As fontes corticais dos ritmos do EEG foram estimadas pela análise por tomografia eletromagnética exata de baixa resolução (eLORETA). Resultados: Observou-se que a média da frequência registrada no EEG está dentro da normalidade nos grupos DA e LAD. A análise por eLORETA mostrou que, em comparação ao grupo controle (CTL), o grupo DA apresentou aumento da atividade teta nos lobos parietal e frontal e diminuição da atividade alfa 2 nos lobos parietal, frontal, límbico e occipital. Em comparação ao grupo CTL, o grupo LAD apresentou aumento da atividade teta nas áreas límbica, occipital sub-lobar e temporal. Conclusão: Os resultados sugerem que os indivíduos com DA e com LAD apresentam comprometimento da atividade elétrica em áreas importantes para a memória e aprendizagem / Introduction: Acquired brain injuries (ABI) caused by degenerative processes include Alzheimer\'s disease (DA), which is a dementia that affects a large part of the elderly population and is characterized by the presence of neurofibrillary tangles and senile plaques. Magnetic resonance (MR) imaging shows atrophy of the entorhinal cortex, hippocampus, amygdala and para-hippocampal area. Positron emission tomography (PET) points to a reduction in the cerebral metabolism of glucose in regions such as the temporal lobe and posterior cingulate. Traumatic brain injury (TBI) is a non-degenerative and non-congenital ABI and is caused by an external mechanical force. Impairment, permanent or temporary, is expected in cognitive, physical and psychosocial functions, with a decrease or alteration of the state of consciousness. One of the main causes of TBI is diffuse axonal injury (DAI), caused by acceleration-deceleration mechanisms. Often the ventral and lateral regions of the frontal and temporal lobes are damaged. Objective: To understand the differences in the functional mechanisms between the AD and DAI groups - with memory complaints, from the electroencephalographic point of view. Methods: The study included 85 adult subjects. Of these, 34 had received the diagnosis of AD, 32 of DAI and 19 were healthy adults. The Mini-Mental State Examination (MMSE), the clock drawing test (CDT) and the verbal fluency test (VF) for the animals category were applied. Subjects were submitted to high resolution EEG with 128 channels. Cortical sources of EEG rhythms were estimated by exact low resolution electromagnetic tomography (eLORETA) analysis. Results: The eLORETA analysis showed that, in comparison to the control (CTL) group, the AD group presented increased theta activity in the parietal and frontal lobes and decreased alpha 2 activity in the parietal, frontal, limbic and occipital lobes. In comparison to the CTL group, the DAI group presented increased theta activity in the limbic, occipital sublobar and temporal areas. Conclusion: The results suggest that individuals with AD and DAI have impairment of electrical activity in areas important for memory and learning

Teste de memória integrativa: comparação de desempenho entre demência frontotemporal variante comportamental e doença de Alzheimer / Memory binding test: comparisons between Alzheimer\'s disease and behavioural variant frontotemporal dementia

INTRODUÇÃO: O teste de memória integrativa (TMI) de curto prazo, nas modalidades de recordação espontânea (RE) e reconhecimento visual (RV), tem se mostrado promissor para apoiar o diagnóstico precoce da doença de Alzheimer (DA), com grande potencial para aplicação clínica. Até o momento, não foram encontrados estudos sobre o TMI junto à população idosa brasileira, nem tampouco pesquisas utilizando a tarefa de RV em pacientes com a variante comportamental da demência frontotemporal (DFTvc). OBJETIVOS: A presente pesquisa comparou o desempenho de pacientes com DA, pacientes com DFTvc e Controles nas duas modalidades de TMI e examinou a acurácia diagnóstica dos testes para a detecção da DA. MÉTODOS: Foram avaliados participantes de três centros de pesquisa, USP São Paulo, USP Ribeirão Preto e Universidade Federal de Minas Gerais, assim como do Instituto Paulista de Geriatria e Gerontologia. A amostra foi composta por 85 indivíduos, subdivididos em 32 Controles, 35 pacientes com DA e 18 pacientes com DFTvc. Foram geradas curvas ROC para calcular a acura?cia diagno?stica dos testes contrastando os grupos diagno?sticos. Realizou-se análises de regressão logística, de correlação e análises de interação entre diagnóstico e condição experimental (características integradas x não-integradas). RESULTADOS: No TMI com RE, Controles apresentaram resultados semelhantes aos pacientes com DFTvc e os pacientes com DA mostraram resultado significativamente menor em relac?a?o aos dois primeiros grupos (Controles = DFTvc > DA). No TMI com RV, Controles mostraram escores significativamente maiores que os pacientes com DFTvc e DA (Controles > DA = DFTvc). A acurácia diagnóstica do teste de RE para DA foi de 0,853, com sensibilidade e especificidade de 84,4% e 80%, respectivamente. A acurácia diagnóstica do teste de RV para DA foi de 0,809, com sensibilidade e especificidade de 65,4% e 72,2%, respectivamente. As modalidades do TMI mostraram correlação significativa com outros testes neuropsicológicos. A tarefa de RV apresentou correlação maior com testes relacionados as funções executivas e de atencão, enquanto que a tarefa de RE mostrou correlação com testes que avaliam memória episódica e funcões executivas. CONCLUSÕES: O presente estudo demonstrou a aplicabilidade do TMI na população brasileira. As tarefas apresentaram características adequadas para a prática clínica: são fáceis e rápidas de aplicar, necessitam de pouco tempo de aplicação, e podem contribuir para o diagnóstico da DA. Especialmente a tarefe de RE, que foi capaz de diferenciar DA de DFTvc / INTRODUCTION: The free recall (FR) and visual recognition (VR) modalities of the memory binding test (MBT) are promising tools to support the early diagnosis of Alzheimer\'s disease (AD). Until the present moment, no studies have been conducted with these tests among the elderly population in Brazil, and there are no studies with the VR modality of the MBT involving behavioural variant frontetemporal dementia (bvFTD) patients. OBJECTIVES: the present research examined the diagnostic accuracy of the two modalities of the MBT for the detection of AD, and compared the performance of patients with AD, bvFTD and controls. METHODS: Participants of three research centres were assessed, USP São Paulo, USP Ribeirão Preto and Federal University of Minas Gerais, and from the Paulista Institute of Geriatrics and Gerontology. The sample comprised 85 participants: 32 controls, 35 AD and 18 bvFTD patients. ROC analyses were used to examine the diagnostic accuracy of the MBT contrasting the different diagnostic groups. Logistic regression analysis, correlations were calculated. ANOVA analyses tested the interaction between diagnosis and experimental condition (features bound and unbound). RESULTS: In the FR task, controls and bvFTD showed similar results, and AD patients showed worse performance (controls = bvFTD > AD). In the VR task, controls showed better performance than the clinical groups (controls > AD = bvFTD). The diagnostic accuracy of the FR task for AD was 0.853, with 84.4% of sensitivity and 80% of specificity. The diagnosis accuracy of the VR task for AD was 0.809, with 65.4% of sensitivity and 72.2% of specificity. The MBT showed significant correlation with other neuropsychological tests. The VR task correlated with tests associated with executive functions and attention, while the FR task correlated with episodic memory and executive functions. CONCLUSIONS: The present study showed the applicability of the MBT for the Brazilian population. The tasks presented adequate characteristics for clinical use: they are quick to administer and can contribute to the diagnosis of AD. Especially the FR task, that could differentiate AD from bvFTD

A transferência de metais para os alimentos e sua possível relação com a saúde do consumidor / The transfer of metals for the foods and it possible relationship with the consumer's health

Com a preocupação voltada para a qualidade dos alimentos de consumo humano, a presente pesquisa tem a finalidade de avaliar a transferência dos metais alumínio (Al), cromo (Cr), ferro (Fe) e níquel (Ni) durante o cozimento dos alimentos arroz e feijão em recipientes de alumínio e aço inoxidável. Inicialmente foram preparadas águas padrões, sob diferentes condições de pH e salinidade, com a água proveniente do sistema de distribuição do município de Ribeirão Preto. Estas combinações tiveram o intuito de verificar qual a condição em que ocorreria uma maior transferência de metais, em especial o alumínio. As águas padrões foram levadas à fervura por um tempo determinado, em recipientes de alumínio, e várias alíquotas de amostras foram recolhidas no decorrer do aquecimento. Posteriormente, foi realizada a quantificação dos metais nas amostras de água e constatou-se que com o aumento do pH e da salinidade, ocorre um aumento da transferência de alumínio e ferro, principalmente, do recipiente para a água. A partir daí foram preparadas apenas águas padrões de pH básico e salinidades altas para o cozimento dos alimentos. No decorrer do preparo dos alimentos também foram retiradas alíquotas de amostras, a fim de mostrar uma possível elevação da transferência de metais com a prolongação do tempo de cozimento. As amostras recolhidas sofreram digestão via úmida (ácidos nítrico e clorídrico) e, finalmente, foram analisadas empregando-se a técnica de Espectrometria de Emissão Óptica em Plasma de Argônio Indutivamente Acoplado (ICP OES) através do uso do equipamento ICP da marca PerkinElmer, modelo Optima 2000. Os resultados analíticos dos metais supracitados estão expressos em gráficos e tabelas, tanto em base úmida quanto em base seca. O equipamento pertence à empresa Ambiental 2000 Ltda., sendo que a calibração do ICP foi efetuada com água proveniente por um sistema de purificação através de membrana de osmose-reversa. No final da pesquisa também foi realizada uma experiência com água contaminada com alumínio para cocção de arroz e feijão em recipientes de teflon para verificação do comportamento de migração do íon alumínio. Existiu uma preocupação especial com a transferência do alumínio, em virtude de haver inúmeros artigos publicados considerando este metal como potencial etiológico para a doença de Mal de Alzheimer, pois, apesar dos esforços sensíveis da ciência em busca da cura para esta moléstia, o tratamento atual permite apenas suavizar seus efeitos. Os resultados obtidos nesta pesquisa serão subsídios para posteriores pesquisas na área médica comparando-se, por exemplo, doenças como o Mal de Alzheimer, Anemia, etc com a prática do uso de recipientes específicos para a cocção de alimentos / With the preocupation gone back to the quality of the foods of human consumption, the present research has the purpose of evaluating the transfer of the metals aluminum (Al), chrome (Cr), iron (Fe) and nickel (Ni) during the cooking of the foods rice and bean in containers of aluminum and stainless steel. Initially they were prepared waters patterns, under different pH conditions and salinity, with the originating from water the system of distribution of the municipal district of Ribeirão Preto. These combinations had the intention of verifying which the condition in that it would happen a larger transfer of metals, especially the aluminum. The waters patterns were taken to the ebullition by a certain time, in containers of aluminum, and several brackets of samples were collected in elapsing of the heating. Later, the quantification of the metals was accomplished in the samples of water and it was verified that with the increase of the pH and of the salinity, it happens an increase of the transfer of aluminum and iron, mainly, of the container for the water. Since then they were prepared just waters standard of basic pH and high salinities for the cooking of the foods. In elapsing of the preparation of the foods brackets of samples were also removed, in order to show a possible elevation of the transfer of metals with the continuation of the time of cooking. The collected samples suffered digestion saw humid (acids nitric and hydrochloric) and, finally, they were analyzed being used the technique of Spectrometer of Optical Emission in Plasma of Argon Coupled Inductively (ICP OES) through the use of the equipment ICP of the mark PerkinElmer, model Optima 2000. The analytical results of the foregoing metals are expressed in graphs and tables, in humid base and in base it dries. The equipment belongs to the Ambiental 2000 company, and the calibration of ICP was made with coming water by a purification system through membrane of osmosis-reverse. In the end of the research an experience was also accomplished with polluted water with aluminum for cooking of rice and bean in teflon containers for verification of the behavior of migration of the aluminum ion. A special concern existed with the transfer of the aluminum, because of there being countless published goods considering this metal as etiological potential for the disease of Evil of Alzheimer, because, in spite of the sensitive efforts of the science in search of the cure for this disease, the current treatment just allows to soften their effects. The results obtained in this research will be subsidies for subsequent researches in the medical area being compared, for instance, diseases as the Evil of Alzheimer, Anemia, etc with the practice of the use of specific containers for the cooking of foods

INTRODUÇÃO: Julgamento é a capacidade de tomar decisões após cuidadosa consideração das informações disponíveis, soluções possíveis, resultados prováveis e fatores contextuais. Do ponto de vista neuropsicológico, o conceito de julgamento envolve memória, linguagem, atenção, raciocínio e principalmente funções executivas. Perda de julgamento tem sido descrita em Comprometimento Cognitivo Leve (CCL) e demência. O Test of Practical Judgment (TOP-J) é uma medida originalmente americana, desenvolvida para avaliar o julgamento prático em adultos mais velhos. É um questionário aberto de 15 itens (TOP-J/15) ou de 9 itens (TOP-J/9) no qual os participantes escutam breves cenários sobre os problemas cotidianos e relatam em voz alta as soluções propostas. Este estudo teve como objetivos a adaptação do TOP-J para uso no Brasil, a elaboração de uma versão reduzida deste instrumento e verificação da acurácia de ambas a versões em amostra da população brasileira composta de controles cognitivamente saudáveis e pacientes com Comprometimento Cognitivo Leve (CCL), doença de Alzheimer (DA) e demência frontotemporal variante comportamental (DFT). MÉTODOS: A amostra foi composta por 85 indivíduos, com idade mínima de 50 anos e escolaridade mínima de 4 anos, sendo 24 controles, 26 CCL, 20 DA e 15 DFT. Os participantes foram submetidos a avaliação neuropsicológica e ao TOP-J. RESULTADOS: No TOP-J/15 versão brasileira (TOP-J/15-Br) e no TOP-J/9 versão brasileira (TOP-J/9-Br) (versão reduzida), o desempenho de controles foi estatisticamente superior ao desempenho de pacientes com CCL, DA e DFT e o desempenho de CCL foi superior ao de pacientes com DA e DFT. No TOP-J/15-Br, a confiabilidade verificada pelo alfa de Cronbach foi de 0,69 e o melhor ponto de corte para distinção de controles e pacientes foi de 30 (sensibilidade de 91,7%; especificidade de 59% e área sob a curva de 0,80). No TOP-J/9-Br, o alfa de Cronbach foi de 0,68 e o melhor ponto de corte para distinção entre julgamento de controles e de pacientes foi 19, com sensibilidade de 79,2, especificidade de 72,1 e área sob a curva de 0,82. CONCLUSÕES: O TOP-J/15-Br e o TOP-J/9-Br apresentaram características psicométricas robustas para o uso pretendido com amostra da população brasileira. Ambos foram capazes de identificar prejuízo de julgamento já em pacientes com CCL e diferenciaram julgamento de controles do julgamento de pacientes com boa sensibilidade e especificidade / INTRODUCTION: Judgment is the ability to make sound decisions after careful consideration of available information, possible solutions, likely outcomes and contextual factors. From a neuropsychological perspective, the concept of judgment involves memory, language, attention, reasoning and specially the executive functions. Loss of judgment has been described in Mild Cognitive Impairment (MCI) and dementia. The Test of Practical Judgment (TOP-J) is an originally American measure created for evaluate practical judgment in older adults. It is a 15-item (TOP-J/15) or 9- item (TOP-J/9) open-ended questionnaire in which participants listen to brief scenarios about everyday problems and report aloud their proposed solutions. This study aimed the adaptation of the TOP-J for use in Brazil, the development of a reduced version of this instrument and the verification of accuracy of both the versions in Brazilian sample composed of cognitively healthy controls and patients with mild cognitive impairment (MCI), Alzheimer\'s disease (AD) and frontotemporal dementia behavioral variant (FTD). METHODS: The sample consisted of 85 subjects with a minimum age of 50 years and minimum education of 4 years, being 24 controls, 26 MCI, 20 AD e 15 FTD. The participants were submitted to a neuropsychological assessment and TOP-J. RESULTS: In the TOP-J/15 Brazilian version (TOP-J/15-Br) and in the TOP-J/9 Brazilian version (TOP-J/9-Br) (reduced version), performance of controls was statistically better than the performance of MCI, AD and FTD patients, and performance of MCI was statistically better than AD and FTD patients. In TOP-J/15-Br, the reliability verified by Cronbach\'s alpha was 0.69 and the best cutoff for distinction between controls and patients was 30 (sensibility of 91,7%; specificity of 59% and area under the curve of 0,8). In the TOP-J/9-Br, Cronbach\'s alpha was 0.68 and the best cutoff point for distinguishing between judgment of controls and judgment of patients is 19, with a sensitivity of 79,2, specificity of 72,1 and area under the curve of 0.82. CONCLUSIONS: The TOP-J/15-Br and the TOP-J/9-Br showed robust psychometric characteristics for the intended use with a sample of the Brazilian population. Both were able to identify deficits of impaired of judgment already in patients with MCI and were able to distinguish judgment of controls from judgment of patients with good sensitivity and specificity
