Distriktssköterskors dokumentation i omvårdnadsjournal vid telefonrådgivning.

The aim of the study was to describe what district nurses, who work with telephone- advice, document in patient record, to be able to develop their documentation. The Authors have examined 50 patient records. The examination tool is collected from “Lokal anvisning för hälso- och sjukvården I Södra Älvsborg”. The audit areas was record keeping, review and planning, realization, individual nursing and the patients participation, information, education and agreement. The sample was stratified to avoid examining only one district nurses´ documentation. The sample was from, the five first and the five last telephone-advices that was documented in patient record, the five first days in July 2006. The result of the audit shows a lack of documentation in nursing record with telephone-advice, for example the patients problems is missing, it can lead to a poorer security of the patient. The Authors conclusion is that the district nurses who works with telephone-advice needs education in documentation, to increase and improve security of the patient in nursing and treatment. / Syftet med studien var att beskriva vad distriktssköterskor dokumenterar i samband med telefonrådgivning för att kunna utveckla distriktssköterskors omvårdnadsdokumentation vid telefonrådgivningssituationer. Författarna har granskat totalt 50 patientjournaler. Granskningsdokumentet som använts är hämtat ur ”Lokal anvisning för Hälso- och sjukvården i Södra Älvsborg”. Den omfattar granskning av journalföring, bedömning och planering, genomförande, individuell omvårdnad och den vårdsökandes delaktighet och information, undervisning och överenskommelse. Urvalet var stratifierat, de fem första och de fem sista telefonrådgivningarna de fem första dagarna i juli månad 2006, för att undvika att granska enbart en distriktssköterskans dokumentation. Resultatet av granskningen visar på bristande dokumentation i omvårdnadsjournalen i samband med telefonrådgivning inom samtliga granskningsområden t ex så saknas dokumentation om den vårdsökandes aktuella behov. Det kan leda till att patientsäkerheten försämras. Författarnas slutsats är att distriktssköterskor som arbetar med telefonrådgivning har behov av utbildning vad gäller dokumentation i samband med telefonrådgivning för att förbättra och öka patientsäkerheten i vård och behandling.

This essay concerns the site of Pompeii and its preservation. The aim is to investigate what causes the decay of the ruins, how to prevent it and what consequences the tourism has on the site. The material consists of interviews with three Swedish scientists who are currently working with documenting a block in Pompeii. The result reveals a large number of threats against the city, such as climate, tourism and an inadequate economy situation. Examples of solutions are more investments which can be used for restorations and reconstructions, and guards to prevent vandalism. Another alternative is to stop the preservation work and let the ruins decay.

AOPS : an abstraction oriented programming system for literate programming

The practice of literate programming is not widespread because existing literate programming systems have some undesirable characteristics such as programming language and text processor dependence and lack of flexible tools for viewing and manipulation of the source file. This dissertation describes the literate programming system AOPS (Abstraction Oriented Programming System) which addresses both of these problems. AOPS is programming language and text processor independent literate programming system. AOPS tools include a hypertext browser, a lister with the ability to select what is presented and what is suppressed, and a filter to extract the program code from the AOPS source file. AOPS introduces the notion of a phantom abstraction that enhances the understandability of the literate program and when used in conjunction with the browser greatly extends the capabilities of AOPS. We also discuss how the design of AOPS supports extension of the concept of literate programming to encompass the entire software life cycle. Finally we describe an experiment which showed that literate programs contain more documentation than traditional programs. / Graduation date: 1993

The HABS Culture of Documentation with an Analysis of Drawing and Technology

The Historic American Buildings Survey (HABS) is one of the oldest federal programs in the United States. In 1933, the HABS culture of documentation started with the mission of creating a permanent record of the nation's architectural heritage. Since the inception of the program, the formal documentation methodology has been measured drawings, large-format photographs, and written histories. HABS documentation accentuates the act of drawing as a mediating conversation between the documenter and the historic environment. In a typical HABS project, the documenter is immersed in the historic setting by hand measuring the structure and creating field notes. The documenter's intimate access to the artifact develops his awareness of cultural heritage and helps cultivate an appreciation for the compositional sensibilities of the architectural precedents. However, the HABS culture of documentation has been fine-tuned to incorporate a number of digital technologies into documentation projects. When projects involve issues of logistics, time, and cost, HABS professionals utilize a host of digital methodologies to produce measured drawings. Although HABS prepares deliverables to meet the archival standards of the Library of Congress, the hardware and software necessary to recognize digital files have a limited lifespan that makes them unacceptable for use in the Library. Only measured drawings that use archival ink on stable translucent material, accompanied by negatives on safety film, can be submitted to the Library. Thus, if HABS pursued only digital technologies and deliverables, the effects of this approach on the quality of the documenter's engagement with cultural heritage would pose a significant question. This study addressed the question of how the HABS culture of documentation evolved in regards to drawing and technology, and how this relationship might be transformed in the future. Using HABS as a focus of inquiry is important in order to illuminate similar dynamics in heritage projects that utilize digital technologies. The methodology used in this study included a literature review, participant observations, and an analysis of documentation projects, as well as in-depth interviews with HABS staff, project participants, private practitioners, and academicians. The outcome of the study will be recommendations to heritage professionals for a future that resides in digital means without compromising the qualities that the HABS experience has offered to generation of documenters.
