Bariatric Surgery for Obesity: A Systematic Review and Meta-analysis

Obesity is the fifth leading cause of global deaths. The efficacy and safety of obesity treatment is still controversial. The objective of the thesis is to evaluate the efficacy and safety of bariatric surgery, through a systematic review of the current evidence and meta- analysis of important outcomes. Nineteen (19) randomized controlled trials (RCTs) with 1346 participants were included. Bariatric surgery resulted in greater weight loss when compared to non-surgical treatment. Weight loss was also associated with resolution and/or improvement of obesity related comorbidites such as diabetes, hypertension, hyperlipidemia, and sleep apnea. Weight loss and safety varied across the surgical procedures. Biliopancreatic diversion/duodenal switch had the greatest weight loss, followed by sleeve gastrectomy and Roux-en-Y gastric bypass, purely restrictive procedures such as vertical banded gastroplasty and adjustable gastric banding resulted in the least weight loss. Long term, high quality, and adequately powered trials are still needed to support the available evidence

It is noteworthy production for the domestic and foreign market, whose requirements demand the need for competitive development of new genetic material to the producer. In this case, the breeding program for cultivation in tropical conditions has employed the crossing of varieties of seedless grapes, using as support embryo rescue technique, which enables the acquisition of hybrid course. Moreover, the cytogenetic characterization of hybrids and parental emerges as a tool that can generate important data for the breeding programs of the grapevine. Counterpart, micropropagation, which is the wider applicability of the technique of tissue culture, has, as one of the many advantages, the large scale production of plants in less time than usual and pathogen-free. During the acclimatization phase of grapevine micropropagation significant losses occur. The various benefits they provide, mycorrhizal fungi are an alternative because they reduce losses, making the transition less stressful conditions in vitro to ex vitro. This study aimed to evaluate cytogenetically vine varieties and hybrids, as well as study the mycorrhizal association in ex vitro establishment of plantlets, trying to select isolates effective in promoting growth and nutrient uptake in apirenic hybrid grapesvines. / A videira representa uma das mais importantes culturas no Vale do S?o Francisco, por ser importante gerador de renda e desenvolvimento social. Destaca-se a produ??o para o mercado interno e externo, cujas exig?ncias em competitividade demandam a necessidade de desenvolvimento de novos materiais gen?ticos para o produtor. Nesse caso, os programas de melhoramento gen?tico para a cultura em condi??es tropicais tem empregado o cruzamento de variedades de uvas apir?nicas, utilizando como suporte a t?cnica de resgate de embri?es, que possibilita a obten??o de h?bridos. Por outro lado, a caracteriza??o citogen?tica de h?bridos e parentais surge como ferramenta que pode gerar dados importantes para os programas de melhoramento gen?tico da videira. Analyse systématique des données sur le control banding et proposition d’une méthode semi-quantitative selon des critères physico-chimiques et biologiques des nanomatériaux manufacturés

Parmi les méthodologies d’appréciation du danger ayant connu une forte croissance ces dernières décennies figurent le Control Banding (CB). Selon une estimée du NIOSH (National Institute for Occupational Safety and Health), cette approche qualitative a été développé dans l’industrie pharmaceutique dans un but concret de protéger près de 90% de travailleurs n’ayant pas accès à des services de santé, de sécurité et d’hygiène au travail. Actuellement, cette approche peut être adaptée aux spécificités des nanomatériaux (NMs) pour lesquelles peu de connaissances scientifiques sont disponibles sur les risques potentiels pour la santé et pour la sécurité des travailleurs. Au cours des dernières années, différentes études ont porté sur l’application de l’approche CB et plusieurs pays ont développé leur propre méthode adaptée aux NMs. Par contre, tous les modèles développés sont basés sur des critères qualitatifs du danger et de l’exposition. Une approche semi-quantitative permettrait de mieux gérer les dangers liés à la manipulation des NMs. L’objectif de ce mémoire était de développer une approche semi-quantitative d’évaluation des dangers sur la base d’une approche de CB. Pour ce faire, une analyse systématique de la littérature scientifique des 20 dernières années sur l’approche de CB a été effectué. Les bases de données Toxline, Pubmed et Google Scholar ont été consultées, couvrant la période de janvier 1996 à novembre 2016, en utilisant 4 mots-clés: control banding, nanomaterials, physicochemical, biological characteristics. Cette recherche a permis d’identifier 982 articles, dont 11% (91) furent retenus en première intention pour au final en garder 6 % (51/982). Il ressort de cette revue que le CB est une approche complémentaire aux méthodes d’évaluation quantitative et que les outils CB développés pour les NMs sont tous de nature qualitative, en réponse à une exposition. Une approche semi-quantitative où il y a maillage entre la caractérisation biologique avec les tests in vitro et la caractérisation physicochimique par des analyses chimiques pour mieux caractériser le niveau de dangérosité des NMs tel que présenté dans le présent mémoire permettrait une utilisation plus judicieuse du CB. Cette approche novatrice du CB dépasse largement l’utilisation que l’on en fait actuellement en hygiène du travail, devrait accroître la confiance dans le jugement des experts en santé au travail, afin d’orienter les travailleurs et la population générale vers une utilisation sécuritaire des NMs. / Among the hazards assessment methodology that has grown exponentially during the last decade, mention Control Banding (CB). According to the National Institute for Occupational Safety and Health (NIOSH), this qualitative approach originally developed in the pharmaceutical industry, aims to protect approximately 90% or 2.7 billion workers that do not have access to occupational safety, health, and hygiene. Currently, this approach can be adapted to the specificities of nanomaterials (NMs), considering the significant lack of scientific knowledge about their potential health and safety risks to workers. In recent years, several CB models have been developed, and many countries have created their own nano-specific CB instruments. However, a semi-quantitative approach would better managing the hazards of handling nanomaterials in the workplace. This thesis aimed to perform a systematic literature review over the past 20 years about the CB approach and then to suggest a semi-quantitative hazard assessment. The official Web pages of the databases Toxline, Pubmed and Google Scholar were used, covering the literature from January 1996 to November 2016, with use of the 4 keywords to locate relevant articles: Control Banding, nanomaterials, physico-chemical, biological characteristics. These searches yielded a total of 982 articles, 11% (91) were retained to eventually retain 6% (51/982). This review shows that the CB approach is an interesting and complementary methodology to quantitative evaluation methods and the CB tools developed for NMs are all qualitative in nature, in response to an exposure and adapted to different work environments. It is possible to make better use by proposing a semi-quantitative approach based on physicochemical and toxicity parameters of NMs to better characterize their degree of dangerousness. We propose here an original methodology proposing the interaction of in vitro tests and chemical analyzes. This innovative Control Banding approach should increase confidence in the judgment of experts and industry, as well as to guide both exposed workers and the uses of NMs in this industry.

Wally Olins (2008:6), points out that in contemporary culture “brands and branding are all-pervasive and ubiquitous”. As he says, one need only walk down the high street of any major foreign city in the world, be it San Francisco or Shanghai, to be embraced by so many familiar brands, including Coca-Cola, KFC, Apple MAC, Chanel and Toyota, that one could mistake it for home. In the last 15 years particularly, the ubiquitous influence of brands and branding has seen the field outgrow its commercial role and expand into more secular and political spheres. Thus, the focus of this study is the role and influence of brands and branding on modern nations and their reputations via intentionally constructed national identities, with specific reference to South Africa. According to the main objectives of this study, focus was placed on the critical examination of South Africa’s nation brand, It’s Possible, in the light of the theoretical ideals for an intentional constructed national identity as proposed by Bartholmé and Melewar, Anholt and Olins. The examination centred on the analysis and interpretation of the individual communiqués that constituted the South African pavilion at the world Expo 2010, as their content is the result of the implementation of the nation’s visual brand language. Investigating the impact of dose banding and oral formulations of paracetamol in pediatrics: A pharmacokinetic simulation-based safety assessment study / Formulerings- och doseringeringseffekter på paracetamol i barn: en farmakokinetisk simuleringsstudie

Paracetamol är ett vanligt använt läkemedel med analgesisk och antipyretisk effekt. Läkemedlet finns tillgängligt i ett flertal beredningsformer och doseringsstyrkor för användning både receptfritt och i sjukhusvården. Syftet med detta projekt var att undersöka påverkan av alternativ, off-label, dosering av paracetamol i pediatrisk vård, med hjälp av fysiologiskt baserad farmakokinetisk (PBPK) modellering. Modellen utvecklades först för en vuxen population genom integrering av in vitro, in vivo och in silico data för paracetamol. Efter detta extrapolerades concentrationskurvor till en pediatrisk population med hjälp av ontogeni-information. Modellen validerades i både vuxna och barn, och var tillförlitlig för både peroral och intravenös dosering. Efter valideringen utfördes simuleringar för nio olika åldersgrupper baserat på rekommenderade doseringsprotokoll i Sverige. Simuleringarna visade att perorala tablettdoseringen var jämförbar med formulering i lösningsform, med snarlika maximumkoncentrationer och area-under-kurvan (AUC) för exponering. Hastigheten av magtömning influerade maximumkoncentrationer men inte AUC. Ytterligare testades modellens förmåga att prediktera plasmakoncentrationer i blodet efter överdosering med paracetamol. Dessa prediktioner fungerade bättre när läkemedelsmetaboliserande enzymer lämnades oförändrade, eller ökade något i aktivitet. Slutligen, den utvecklade PBPK-modellen kan användas för att säkert undersöka olika doseringsprotokoll och för design av pediatriska kliniska studier. / Paracetamol, a widely used analgesic and antipyretic drug, can be found in various formulations and doses for both home and hospital use. The aim of this study was to investigate the impact of off-label dosing of paracetamol in pediatric clinical practice using physiologically based pharmacokinetic (PBPK) modeling. The model was initially developed for adults by integrating relevant in vitro, in vivo and in silico data of paracetamol, after which the model was extrapolated for pediatrics by adding ontogeny information. The model was successfully validated in both adult and pediatric populations, and it showed accuracy for both oral and intravenous administration routes. After validation, simulations were conducted across nine different age groups following the recommended doses in Sweden. These simulations showed that tablet dose is comparable to solution dosing, resulting in nearly identical maximum concentrations and area under the curve (AUC) values. Furthermore, it was observed that gastric emptying time, which reflects the fed state of individuals, significantly influences the maximum concentration, with longer gastric emptying times resulting in lower and delayed peak concentrations. However, the gastric emptying time had no effect on the AUC values. Lastly, the model’s performance on overdose data was evaluated, and it turned out that it performs better when liver enzymes were not affected, or they were only slightly elevated. Finally, the developed PBPK model can be further used for safe and effective way of exploring dose banding and designing clinical trials in pediatrics.

Untersuchung somatischer Chromosomenveränderungen bei amyotropher Lateralsklerose

Die ALS ist eine fortschreitende neurodegenerative Erkrankung, deren Symptome durch den Untergang der Motoneuronen bedingt sind. Neueste zytogenetische Untersuchungen zeigen ein vermehrtes Auftreten konstitutioneller Chromosomenveränderunge bei ALS-Patienten. Dies lässt eine Verbindung zwischen dem Ausbruch der Erkrankung und der auffälligen Zytogenetik vermuten. Das Auftreten spontaner Chromosomenveränderungen als Zeichen einer chromosomalen Instabilität wurde in der vorliegenden Arbeit an ALS-Patienten untersucht. METHODE: Neben der Karyotypisierung, der Bestimmung der SCE-Rate und der Bruchrate nach Behandlung mit Bleomycin kam die Fluoreszenz in situ Hybridisierung zum Einsatz. Die Untersuchungen wurden an Patienten mit sporadischer ALS (45), an Kontrollpersonen (38) und Verwandten (9) durchgeführt. ERGEBNISSE: Die Karyotypisierung ergab bei den Patienten eine spontane Translokationsrate von 0,02 Translokationen/Zelle (t/Z), bei den Kontrollen 0,04 t/Z. Weitere numerische oder strukturelle Auffälligkeiten waren nicht signifikant verschieden. Es wurden keine konstitutionellen Chromosomenaberrationen gefunden. Die Häufigkeit der Schwesterchromatidaustausche (SCE-Rate) bewegte sich mit 7-8 SCE/Z in der Patientengruppe innerhalb der Normwerte. Durch die Zugabe von Bleomycin in die Zellkultur stieg die Zahl der Chromatidbrüche von 0,0 auf 0,8 Brüche/Z an. Dabei zeigten die untersuchten Gruppen ähnliche Progredienzen in den Bruchraten. Mit der Fluoreszenz in situ Hybridisierung werden quantitative Aussagen über spontane Translokationsraten gemacht. Sie betrug in der Kontroll-und Patientengruppe 0,03 bis 0,04 t/Z. DISKUSSION: Die vorliegenden Ergebnisse liefern keinen Anhalt für eine chromosomale Instabilität als Risikofaktor für die Entstehung der sporadischen ALS. Indizien für eine chromosomale Instabilität wie erhöhte Bruchraten und SCEs als auch vermehrtes Auftreten somatischer Aberrationen konnten bei den ALS-Patienten nicht nachgewiesen werden. Über mögliche Auffälligkeiten in den Motoneuronen lässt sich allerdings mit den Untersuchungen an Blutlymphozyten keine hinreichend sichere Aussage machen. / Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease which is characterized by the degeneration of motor neurons. Recently, a high rate of constitutional structural chromosomal rearrangements has been reported in apparently sporadic ALS patients. It remains questionable whether or not these genomic rearrangements are caused by a chromosomal instability involved in the pathogenesis of the disease. Therefore, we performed different cytogenetic studies on chromosomal instability. METHOD: We performed chromosome analyses from patients (N=45), control subjects (N=38), and relatives (N=9) after culturing blood lymphocytes. Conventional chromosome analysis after GTG-banding, chromosomal breakage test after Bleomycin treatment, the rate of sister chromatid exchange (SCE), and whole chromosome painting were used for these analyses. RESULTS: Neither karyotyping nor whole chromosome painting revealed higher levels of structural or numerical aberrations in lymphocytes of patients with sALS. After karyotyping we found 0.02 t/cell in patients and 0.04 t/cell in controls. Whole chromosome painting revealed 0.04 t/cell in patients and 0.03 t/cell in controls. The chromosomal breaks increased likewise after Bleomycin treatment in the control group and the patient group as well. Cell cultures without Bleomycin did not show any breaks while the highest Bleomycin concentration induced up to 0.08 breaks/cell. The SCE rate in patients which corresponds to the chromatid repair activity did not rise to a higher level than in the control individuals. Both groups were in the normal range of 7 to 8 SCE/cell. DISCUSSION: The pathomechanism of neurodegeneration in ALS patients is still unknown. We tried to find a cytogenetic correlative being a risk factor for the development of ALS. However, so far there is no clue for chromosomal instability being involved in the neurodegenerative process.

Multi-Scale Approaches For Understanding Deformation And Fracture Mechanisms In Amorphous Alloys

Amorphous alloys possess attractive combinations of mechanical properties (high elastic limit, ~2%, high fracture toughness, 20-50 MPa.m1/2, etc.) and exhibit mechanical behavior that is different, in many ways, from that of the crystalline metals and alloys. However, fundamental understanding of the deformation and fracture mechanisms in amorphous alloys, which would allow for design of better metallic glasses, has not been established on a firm footing yet. The objective of this work is to understand the deformation and fracture mechanisms of amorphous materials at various length scales and make connections with the macroscopic properties of glasses. Various experimental techniques were employed to study the macroscopic behavior and atomistic simulations were conducted to understand the mechanisms at the nano level. Towards achieving these objectives, we first study the toughness of a Zr-based bulk metallic glass (BMG), Vitreloy-1, as a function of the free volume, which was varied by recourse to structural relaxation of the BMG through sub-Tg annealing treatment. Both isothermal annealing at 500 K (0.8Tg) for up to 24 h and isochronal annealing for 24 h in the temperature range of 130 K (0.65Tg) to 530 K (0.85Tg) were conducted and the impact toughness, Γ, values were measured. Results show severe embrittlement, with losses of up to 90% in Γ, with annealing. The variation in Γ with annealing time, ta, was found to be similar to that observed in the enthalpy change at the glass transition, ΔH, with ta, indicating that the reduction of free volume due to annealing is the primary mechanism responsible for the loss in Γ with annealing. Having established the connection between sub-atomic length scales (free volume) and macroscopic response (toughness), we investigated further the affects of relaxation on intermediate length scale behavior, namely deformation induced by shear bands, by employing instrumented indentation techniques. While the Vickers nano-indentation response of the as-cast and annealed glasses do not show any significant difference, spherical indentation response shows reduced shear band activity in the annealed BMG. Further, relatively high indentation strain was observed to be necessary for shear band initiation in the annealed glass, implying an increased resistance for the nucleation of shear bands when the BMG is annealed. In the absence of microstructural features that allow for establishment of correlation between properties and the structure, we resort to atomistic modeling to gain further understanding of the deformation mechanisms in amorphous alloys. In particular, we focus on the micromechanisms of strain accommodation including crystallization and void formation during inelastic deformation of glasses. Molecular dynamics simulations on a single component system with Lennard-Jones-like atoms suggest that a softer short range interaction between atoms favors crystallization. Compressive hydrostatic strain in the presence of a shear strain promotes crystallization whereas a tensile hydrostatic strain was found to induce voids. The deformation subsequent to the onset of crystallization includes partial re-amorphization and recrystallization, suggesting important mechanisms of plastic deformation in glasses. Next, a study of deformation induced crystallization is conducted on two component amorphous alloys through atomistic simulations. The resistance of a binary glass to deformation-induced-crystallization (deformation stability) is found to increase with increasing atomic size ratio. A new parameter called “atomic stiffness” (defined by the curvature of the inter-atomic potential at the equilibrium separation distance) is introduced and examined for its role on deformation stability. The deformation stability of binary glasses is found to increase with increasing atomic stiffness. For a given composition, the internal energies of binary crystals and glasses are compared and it is found that the energy of glass remains approximately constant for a wide range of atomic size ratios unlike crystals in which the energy increases with increasing atomic size ratio. This study uncovers the similarities between deformation and thermal stabilities of glasses and suggests new parameters for predicting highly stable glass compositions.

Μοντελοποίηση της ροής του αίματος σε στένωση προκαλούμενη από περίδεση της πνευμονικής αρτηρίας / Blood flow modeling in the stenosis induced by the pulmonary artery banding

Οι καρδιαγγειακές παθήσεις αποτελούν την κύρια αιτία θανάτου στις αναπτυγμένες χώρες. Η στένωση σε μία αρτηρία, είτε αυτή προκαλείται από μία πάθηση όπως το ανεύρυσμα, είτε προκαλείται από μία περίδεση, όπως στις περιπτώσεις των συγγενών καρδιοπαθειών, μπορεί να μεταβάλλει σε σημαντικό βαθμό τα χαρακτηριστικά της ροής του αίματος. Η μελέτη της φυσιολογικής παλλόμενης ροής μέσα από στένωση είναι ιδιαίτερα σημαντική για τη διάγνωση και αντιμετώπιση των αγγειακών νόσων. Το ιατρικό πρόβλημα το οποίο εξετάζουμε στην παρούσα εργασία, είναι η στένωση της πνευμονικής αρτηρίας από περίδεση. Η περίδεση γίνεται προφανώς για να μειωθεί η υψηλή αρτηριακή πίεση και τελικά η ροή του αίματος προς τους πνεύμονες. Πρόκειται για μία χειρουργική μέθοδο αντιμετώπισης συγγενών καρδιοπαθειών. Η περίδεση της πνευμονικής αρτηρίας (pulmonary artery banding - PAB) είτε με συμβατικό τρόπο, ή με την πλέον σύγχρονη μέθοδο μέσω της συσκευής FloWatchTM προκαλεί τη στένωσή της. Με τον συμβατικό τρόπο η στένωση μπορεί να θεωρηθεί αξονικά συμμετρική, ωστόσο με τη χρήση του FloWatchTM είναι μη αξονικά συμμετρική. Έχει αποδειχθεί ότι τόσο η αξονικά συμμετρική, όσο και η μη συμμετρική περίδεση δημιουργεί διαφόρου βαθμού ίνωση του τοιχώματος της πνευμονικής αρτηρίας. Η αναδόμηση της πνευμονικής αρτηρίας είναι πολύ ηπιότερη στην περίπτωση της περίδεσης με το FloWatchTM. Η διαφοροποίηση αυτή έγκειται κυρίως στο ότι η συμβατική περίδεση προκαλεί για συγκεκριμένη μείωση της διατομής ισχυρότερη μείωση της περιμέτρου της διατομής από εκείνης της περίδεσης με το FloWatchTM. Στην παρούσα εργασία γίνεται αναφορά και ανάλυση των διαφόρων περιπτώσεων ροής σε στενώσεις αρτηριών, των συγγενών καρδιοπαθειών και των τεχνικών περίδεσης της πνευμονικής αρτηρίας. Επιπρόσθετα, μελετήθηκαν και υπολογίστηκαν η μόνιμη και η παλλόμενη ροή σε αξονικά συμμετρική 25% στένωση προκαλούμενη από συμβατική περίδεση, καθώς και η μόνιμη και παλλόμενη ροή σε μη συμμετρική 25% στένωση της πνευμονικής αρτηρίας όπως προκαλείται από το FloWatchTM, μέσω των πακέτων Fluent και Gambit. Η υπολογιστική μελέτη του πεδίου ροής περιλαμβάνει την κατανομή ταχυτήτων, τον προσδιορισμό των περιοχών ανακυκλοφορίας, την κατανομή των πιέσεων και την σύγκριση των παραπάνω μεγεθών με τα αντίστοιχα αποτελέσματα της βιβλιογραφίας. Τέλος, με βάση τα αποτελέσματα γίνεται η σύγκριση των δύο μελετούμενων μεθόδων περίδεσης. Αριθμητικά ρεαλιστικά δεδομένα ελήφθησαν από την καρδιοχειρουργική κλινική του νοσοκομείου Παίδων «Αγία Σοφία». / Cardiovascular diseases are the leading cause of death in developed countries. A stenosis in an artery , caused either by a disease such as an aneurism or by a banding (such as in congenital diseases) can change the characteristics of the blood flow very seriously. The study of the physiological pulsatile flow through a stenosis is very important for the diagnosis and treatment of the arterial diseases. The medical problem which is examined in this study is pulmonary artery stenosis caused by a banding. The banding takes place to reduce the high arterial pressure and finally the blood flow from the heart to the lungs. It is a surgical method used for treatment of congenital heart diseases. The pulmonary artery banding either with the use of the conventional method or the most modern with the use of the FloWatchTM technology causes stenosis of the artery. With the conventional method, stenosis can be considered axially symmetrical while with the use of FloWatchTM it is asymmetrical. It has been proven that both the axially symmetrical and asymmetrical banding cause fibrosis of the pulmonary artery walls of different degrees. The reconstruction of the pulmonary artery is milder where there is banding with FloWatchTM. This differentiation is based mainly on the fact that the conventional banding causes, for a specific decrease of the cross-section, a decrease in the perimeter of the cross-section higher than that of banding with FloWatchTM. In this assignment there is a report of different cases of flow in arterial stenosis, in congenital heart diseases and pulmonary banding techniques. In addition what was studied and appreciated was the steady and pulsatile flow in axially symmetrical 25% stenosis caused by the conventional banding, as well as the steady and pulsatile flow in asymmetrical 25% stenosis of pulmonary artery caused by FloWatchTM with the use of Fluent and Gambit. The numerical study of flow distribution includes velocity distribution, designation of back flow area, distribution of pressure and comparison of these quantities with the results in bibliography. Finally, based on the results, there is a comparison of the two banding methods under study. The numerical realistic data were received from the cardio-surgical clinic of children’s hospital “Aghia Sophia”.

Many of these groups, especially those that exhibit great diversity, like Perciformes and Siluriformes, possess species with difficult morphologic characterization, called cryptic species, commonly detected through karyotypic analyses, which reveals outstanding interespecific variations with relationship to the number and its chromosomal structures. Thus, the present work intends to contribute for the cytogenetic knowledge of marine and brackish fish species, because they peculiar life habits and by lack of cytogenetic data of your genetic aspects. Therefore, cytogenetic studies were developed in a species of Apogonidae (Perciformes), two species of sea catfishes of the family Ariidae (Siluriformes) and brackish fish Paurachenipterus galeatus (Siluriformes, Auchenipteridae), through C banding, Ag-NOR, use of base-specific flourochromes (DAPI and CMA3), as well as FISH (Fluorescent in situ hybridization) using ribosomal DNA probes 5S and 18S. The present results contribute to a better understanding of the processes of differentiation patterns and chromosome evolution in these groups. The use of other approaches (the morphology and molecular tools) will allow a larger understanding of the genetic and biological diversity of the Brazilian ichthyofauna. / Estudos citogen?ticos t?m revelado uma grande diversidade at? ent?o n?o detectada em diversas fam?lias de peixes. Muitos destes grupos, sobretudo os que exibem grande diversidade, como Perciformes e Siluriformes, possuem esp?cies de dif?cil caracteriza??o morfol?gica, chamadas de esp?cies cr?pticas, muitas vezes s? detectadas atrav?s de an?lises cariot?picas, as quais revelam varia??es interespec?ficas marcantes quanto ao n?mero e estrutura cromoss?mica. Laparoscopic adjustable gastric banding for morbid obesity:primary, intermediate, and long-term results including quality of life studies

Abstract Morbid obesity is the most rapidly increasing health threat of developed countries, and the costs caused by it are already higher than those of smoking. In an increasing number of developing countries both starvation and morbid obesity are increasing simultaneously. Obesity in children and adolescents is also increasing rapidly. Conservative treatment almost invariably fails when treating morbid obesity. Results of pharmacotherapy have been disappointing after great expectations. Laparoscopic gastric banding has been used in the treatment of morbid obesity since 1993. The method was first used mostly in Europe. In the USA either an open or laparoscopic gastric bypass have been the most common methods of surgery. The aim of this study was to investigate the operation results of 280 patients operated in Vaasa Central Hospital during the 11 years after March 1996. Of these patients, 123 have been followed at least 5 years. The results have been analyzed with BAROS that measures the quality of life. Quality of life was measured prospectively 1 year after surgery with the 15D questionnaire that is validated in the Finnish population. The effect of gastric banding in esophageal motility and reflux was studied prospectively in 31 patients. Late results were analyzed in 123 patients 11 years after the first operation. Mean excess weight loss (EWL) was 56% in patients who had their band in place 7 years after surgery, and 46% in all patients. There was no mortality related to the operation, and there was only one serious complication. Disease-specific quality of life improved in 78.8% of the patients in 28 months of follow-up. Health-related quality of life was significantly improved 12 months after surgery, but improvement was not connected to the amount of weight loss. The band inhibited reflux 19 months after surgery. Complications, failures, and reoperations increase with longer follow-up. Weight loss is moderate 9 years after a gastric banding operation, and in carefully selected patients this operation is still a good option in the treatment of morbid obesity.
