Functional failure sequences in traffic accidents

This thesis examines the interactions between road users and the factors that contribute to the occurrence of traffic accidents, and discusses the implications of these interactions with regards to driver behaviour and accident prevention measures. Traffic accident data is collected on a macroscopic level by local police authorities throughout the UK. This data provides a description of accident related factors on a macroscopic level which does not allow for a complete understanding of the interaction between the various road users or the influence of errors made by active road users. Traffic accident data collected on a microscopic level analysis of real world accident data, explaining why and how an accident occurred, can further contribute to a data driven approach to provide safety measures. This data allows for a better understanding of the interaction of factors for all road users within an accident that is not possible with other data collection methods. In the first part of the thesis, a literature review presents relevant research in traffic accident analysis and accident causation research, afterwards three accident causation models used to understand behaviour and factors leading to traffic accidents are introduced. A comparison study of these accident causation coding models that classify road user error was carried out to determine a model that would be best suited to code the accident data according to the thesis aims. Latent class cluster analyses were made of two separate datasets, the UK On the Spot (OTS) in-depth accident investigation study and the STATS19 national accident database. A comparison between microscopic (in-depth) accident data and macroscopic (national) accident data was carried out. This analysis allowed for the interactions between all relevant factors for the road users involved in the accident to be grouped into specific accident segmentations based on the cluster analysis results. First, all of the cases that were collected by the OTS team between the years 2000 to 2003 were analysed. Results suggested that for single vehicle accidents males and females typically made failures related to detection and execution issues, whereas male road users made diagnosis failures with speed as a particularly important factor. In terms of the multiple vehicle accidents the interactions between the first two road users and the subsequent accident sequence were demonstrated. A cluster analysis of all two vehicle accidents in Great Britain in the year 2005 and recorded within the STATS19 accident database was carried out as a comparison to the multiple vehicle accident OTS data. This analysis demonstrated the necessity of in-depth accident causation data in interpreting accident scenarios, as the resulting accident clusters did not provide significant differences between the groups to usefully segment the crash population. Relevant human factors were not coded for these cases and the level of detail in the accident cases did not allow for a discussion of countermeasure implications. An analysis of 428 Powered Two Wheeler accidents that were collected by the OTS team between the years 2000 to 2010 was carried out. Results identified 7 specific scenarios, the main types of which identified two particular looked but did not see accidents and two types of single vehicle PTW accidents. In cases where the PTW lost control, diagnosis failures were more common, for road users other than the PTW rider, detection issues were of particular relevance. In these cases the interaction between all relevant road users was interpreted in relation to one another. The subsequent study analysed 248 Pedestrian accidents that were collected by the OTS team between the years 2000 to 2010. Results identified scenarios related to pedestrians as being in a hurry and making detection errors, impairment due to alcohol, and young children playing in the roadside. For accidents that were initiated by the other road user s behaviour pedestrians were either struck after an accident had already occurred or due to the manoeuvre that a road user was making, older pedestrians were over-represented in this accident type. This thesis concludes by discussing how (1) microscopic in-depth accident data is needed to understand accident mechanisms, (2) a data mining approach using latent class clustering can benefit the understanding of failure mechanisms, (3) accident causation analysis is necessary to understand the types of failures that road users make and (4) accident scenario development helps quantify accidents and allows for accident countermeasure implication discussion. The original contribution to knowledge is the demonstration that when relevant data is available there is a possibility to understand the interactions that are occurring between road users before the crash, that is not possible otherwise. This contribution has been demonstrated by highlighting how latent class cluster analysis combined with accident causation data allows for relevant interactions between road users to be observed. Finally implications for this work and future considerations are outlined.

Contextualization of a physiotherapy clinical practice guideline for stroke rehabilitation in Kenya

Philosophiae Doctor - PhD (Physiotherapy) / Stroke is the third leading cause of death and disability worldwide. Eighty five per cent of strokes occur in developing countries, and it is estimated that the prevalence will increase in future. Evidence based rehabilitation programs inherent in clinical practice guidelines has the potential to improves functional activities, and participation. However Kenya does not have this guideline. Most clinical guidelines are developed in the western world, and reflect developed world healthcare systems and resources that are not always appropriate to developing nations. Likewise, guidelines are costly to produce. Kenya lacks the resources and the expertise for de novo guideline development. It is therefore appropriate and cost effective to contextualise the available high quality recommendations.

A escola é espaço favorável para ações de prevenção destes acidentes, mas estudos desta natureza são raros. Esta pesquisa planeja, implementa e avalia ação educativa de prevenção de acidente infantil doméstico em uma sala da 2ª série do Ensino Fundamental da rede municipal de ensino de Marília-SP. Participam 23 alunos, formando o grupo que recebe a ação educativa (GA), seus pais/responsáveis e a professora da classe. Dezessete alunos da 2ª série de outra escola compõem o grupo que não recebe a ação educativa (GNA), bem como seus pais. Utilizam-se impressos pré elaborados (roteiros de entrevista, cartas de apresentação, termos de consentimento, roteiro da ação educativa e check-list de local/ modo de armazenamento de produtos tóxicos na residência), equipamentos (gravador, pilhas e filmadora) e materiais (sucatas, folhetos de supermercados, etc.). No GA realiza entrevista inicial com alunos e professora, entrevista estruturada com alunos, desenvolve a ação educativa em sala de aula e realiza nova entrevista com escolares e seus pais/responsáveis. No GNA realiza entrevista estruturada com alunos em dois momentos, com o mesmo roteiro do GA, em intervalo de vinte e um dias, além de entrevista semi-estruturada com seus responsáveis após a segunda entrevista. Os alunos dos dois grupos produzem um texto sobre Acidente doméstico antes das duas entrevistas estruturadas. A ação educativa aborda os acidentes domésticos, especificamente intoxicação / The home childhood accidents have a high incidence, with a perspective of reduction through education, planned according the target people reality and needs. Schools are the most adequate place for prevention of accidents actions, but studies for this purpose are rare. This research plans, implements and evaluates education actions for prevent home childhood accidents in a second year classroom, form the city public system of schools, at Marilia, State of São Paulo. There were 23 students that took place on receiving education actions, such as their parents and teacher. Seventeen second year students of another public school, such as their parents, compound the control group, not receiving any education action. It was used pre-elaborated printings (interview protocol, introducing letters, agreement statements, education action scheme and checklists for home risks), equipments (tape-recorder, videorecorder, batteries) and materials (garbage ones, market folders). In the action receiving group (AG), it was made an initial interview with students and teacher, structures interview with students, develop the education action in classroom, and interviews post-action with students and parents. In the non-action group (NAG), there were made two interviews, with the same model of the other group, with a gap of 21 days and the parents’ structured interviews after the second students’ interview. O método para o desenvolvimento do produto foi baseado na suspensoterapia, após sua fabricação foram realizados testes com o produto, e aplicado um questionário aos fisioterapeutas que realizaram os testes. Os exercícios em suspensão demonstraram ser uma alternativa útil em substituição a técnicas tradicionais, sobretudo nos estágios iniciais do tratamento de pacientes portadores de acidente vascular encefálico em que há necessidade de mobilização passiva e ativo-assistida. Com a gravidade e o atrito eliminados, os pacientes conseguiram realizar ou esboçar movimento com maior facilidade, e possibilitando ainda trabalho dos pacientes que não têm consciência corporal ou não respondem a estímulos. No que diz respeito ao tratamento manual e comparando com a utilização do equipamento diminui a apreensão e a fadiga tanto para o paciente quanto para o fisioterapeuta, e demonstrou ser um meio prático e seguro para auxiliar a mobilização precoce. / Objectives: This work aims at presenting a proposal of a product/equipment, which represents a possibility of helping and benefiting treatment and qualification of medical intervention on the carriers of encephalic vascular accident who present as sequel small degree of muscular strenghth or who are not conscious of the body and or not respond to stimuli. In order to try to reach a better quality of life to the user and to the professional who is going to handle it. The method of the development of the product was based on the suspention therapy, after its manufacturing tests were made with the product, and a questionnare was applied to the physiotherapists who performed the tests. The exercices in suspension showed to be a useful alternative to the replacement of traditional techniques, especially in the early stages of treatment of the patients carrying encephalic vascular accident in which there is the need of passive mobilization and active assistance. By eliminating gravity and attrition, the patients managed to realize or rough movements more easily and even enabled working on the patients who do not have conscience of the body or do not respond to stimuli. When it comes to the manual treatment in comparasion with the usage of the equipment it reduces the apprehension and fatigue both to the patient and the physiotherapists, and it showed to be a practical and safe means of helping an early mobilization.

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / O acidente do trabalho, ocorrÃncia freqÃente na vida do trabalhador, Ã, desde os primÃrdios dos tempos, uma preocupaÃÃo para aqueles que lutam para sobreviver. A presente pesquisa dedicou-se ao estudo dos acidentes do trabalho fatais ocorridos com eletricitÃrios ligados direta ou indiretamente à concessionÃria de energia elÃtrica do Estado do Cearà no perÃodo de 1998 a 2004. Os dados foram obtidos de 3 fontes. Dispostos em tabelas e analisados obtendo-se como resultado que 33 trabalhadores do setor elÃtrico, todos do sexo masculino, foram vÃtimas de acidentes do trabalho fatais; sendo que destes somente 13 haviam sido notificados com tal, a mÃdia de idade foi de 33anos; 60% eram casados; 76% tinham filhos; 70% das vÃtimas fatais eram eletricistas; 48% das vÃtimas tinham menos de 1 ano de serviÃo; 15% eram sindicalizados; 82% eram trabalhadores de empresas terceirizadas; 66% das vÃtimas morreu em decorrÃncia de choque elÃtrico, sendo a mÃdia tensÃo a mais freqÃente; os MMSS e as queimaduras foram os locais e as lesÃes mais encontradas; o ano de 1999 registrou o maior nÃmero de acidentes fatais e 68% aconteceram no horÃrio comercial; 91% dos acidentes aconteceram no Interior do Estado; 83% aconteceram em vias pÃblicas. Encontrou-se 21% de concordÃncia entre as 3 fontes pesquisadas e 52% entre 2 fontes. Concluiu-se a ocorrÃncia de um aumento acentuado do nÃmero de acidentes do trabalho fatais no setor elÃtrico no perÃodo de 1998 a 2002, o qual corresponde ao inÃcio da privatizaÃÃo da concessionÃria de energia elÃtrica do CearÃ.
