Unintentional childhood injuries in the home: the perceptions of first time mothers on the risk factors in Delft, Cape Town.

Injuries continue to be a significant health problem in children. Injuries put more children in hospital than any other single cause. Injuries to children are due to falls, burns, poisoning, crushing, suffocation, traffic incidents and other causes both intentional and unintentional. Preschool children most commonly experience injuries in the home where they spend most of their time. The Western Cape Department of Health has decided to focus on the prevention of unintentional injuries to preschool children in and around the home. It is necessary to first determine the risk factors to these injuries before embarking on safety promotion and injury prevention programmes. To date there have been no studies documented in South Africa looking at the perceptions of the risk factors associated with household unintentional childhood injuries of first time mothers. This study therefore attempted to explore the perceptions of risk factors of first time mothers in Delft. Delft was chosen because it is a low-income area (which is recognised as being a risk factor to unintentional childhood injuries) and it is one of the pilot sites for the injury prevention programme of the Western Cape Department of Health.

I media får vi stort sett bare høre om førstnevnte i form av oppslag om at ulykken skyldes menneskelig svikt. Årsaksforholdene tar gjerne lengre tid å kartlegge og har heller ikke elementer av helt eller syndebukk i seg, og er således mindre interessante å skrive om. I forebyggende sammenheng er det likevel gjerne her det er mest å hente. I erkjennelsen av at mennesket er og blir feilbarlig, bør man nettopp gripe tak i disse bakenforliggende årsakene, gjerne kalt latente feil, som også kan medvirke til å avverge at et tilløp får lov å utvikle seg til en ulykke. I denne oppgaven blir det gjort rede for bakenforliggende årsaker til en alvorlig samferdselsulykke på Vestlandet i 1999, nemlig hurtigbåten MS Sleipners forlis. Det blir også vist hvordan ulykken kunne vært unngått, eller i det minste at skadeomfanget kunne vært vesentlig redusert, dersom det hadde vært større fokus på å avdekke slike latente forhold. Media mainly tell us the result of the first one, namely that “the cause of the accident was human error”. Usually it takes more time to establish an understanding of the underlying causes. This investigation does not point out a hero, neither a scapegoat. Therefore the real causes are less interesting to the media. Still, when it comes to accident prevention, this is where one should make an effort. By recognizing the fact that humans are eligible to fail, it is here one should try to take preventive measures. By recognizing these latent conditions one may succeed to avoid potentially dangerous situations from developing to serious accidents. In this paper the underlying causes related to a serious accident that struck public transportation in Western Norway will be presented. The accident is the shipwreck of the high speed craft MS Sleipner in 1999. It will also be shown how this accident could have been prevented, or at least how the extent of the damage could have been substantially reduced, if only latent conditions had been paid a little more attention to. To make the general public understand the importance of uncovering such latent conditions, not to say make sure that they who are responsible take action to remove these conditions, it is vital that the media put these questions on the agenda.</p>

Estimating the continuous risk of accidents occurring in the South African mining industry

Thesis (MEng)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: Statistics from mining accidents expose that the potential for injury or death to employees from occupational accidents is relatively high. This study attempts to contribute to the on-going efforts to improve occupational safety in the mining industry by creating a model capable of predicting the continuous risk of occupational accidents occurring. Model inputs include the time of day, time into shift, temperatures, humidity, rainfall and production rate. The approach includes using an Artificial Neural Network (ANN) to identify patterns between the input attributes and to predict the continuous risk of accidents occurring. As a predecessor to the development of the model, a comprehensive literature study was conducted. The objectives of the study were to understand occupational safety, explore various forecasting techniques and identify contributing factors that influence the occurrence of accidents and in so doing recognise any gaps in the current knowledge. Another objective was to quantify the contributing factors identified, as well as detect the sensitivity amongst these factors and in so doing deliver a groundwork for the present model. After the literature was studied, the model design and construction was performed as well as the model training and validation. The training and validation took the form of a case study with data from a platinum mine near Rustenburg in South Africa. The data was split into three sections, namely, underground, engineering and other. Then the model was trained and validated separately for the three sections on a yearly basis. This resulted in meaningful correlation between the predicted continuous risk and actual accidents as well as the majority of the actual accidents only occurring while the continuous risk was estimated to be above 80%. However, the underground section has so many accidents, that the risk is permanently very high. Yet, the engineering and other sections produced results useful for managerial decisions. / AFRIKAANSE OPSOMMING: Mynbou ongeluk statistieke dui aan dat die potensiaal vir besering of dood as gevolg van beroepsongelukke relatief hoog is. Die studie poog om by te dra tot die voortdurende verbetering van beroepsveiligheid in die mynbedryf deur middel van ’n model wat die risiko van beroepsongelukke voorspel. Die model vereis die tyd, tyd verstreke in die skof, temperatuur, humiditeit, reënval en produksie tydens die ongeluk as inset. Die benadering tot hierdie model maak gebruik van ’n Kunsmatige Neurale Netwerk (KNN) om patrone tussen die insette te erken en om die risiko van ’n voorval te beraam. As ’n voorloper tot die model ontwikkeling, is ’n omvattende literatuurstudie onderneem. Die doelwitte van die literatuur studie was om beroepsveiligheid beter te verstaan, verskeie voorspellings tegnieke te ondersoek en kennis van bydraende faktore wat lei tot voorvalle te ondersoek. Nog ’n doelwit sluit die kwantifisering in van geidentifiseerde bydraende faktore, asook die opsporing van die sensitiwiteit tussen hierdie faktore en hierdeur ’n fondasie vir die voorgestelde model te skep. Na afloop van die literatuurstudie is die model ontwikkel, opgelei en gevalideer. Die opleiding en validasie is deur middel van ’n gevallestudie in ’n platinummyn naby Rustenburg in Suid Afrika gedoen. Die data is verdeel in drie afdelings, d.i. ondergronds, ingenieurswese en ander. Die model is vir elke afdeling apart opgelei en gevalideer op ’n jaarlikse basis. Hierdie het gelei tot ’n betekenisvolle korrelasie tussen die voorspelde risiko en die werklike ongelukke met die meerderheid van die werklike ongevalle wat voorgekom het terwyl die risiko 80% oorskry het. In die ondergrondse afdeling is so baie voorvalle waarneem dat die risiko permanent hoog is. Die ander afdelings het wel resultate verskaf wat sinvol gebruik kan word in bestuursbesluite.

Investments in the prevention and knowledge on these accidents bring advantages for the companies, workers and society. And when detaching the itinerary accident, they are deficiencies of the sources of official information, where the sub-registrations dominate, impeding a reliable analysis. Also damages provoked by the accidents of traffic are one of the main problems of public health in world level, requesting combined efforts for prevention executes and maintainable. This work has as main objective to investigate possible relationships among socioeconomic and population indicators with the occurrence of the itinerary accidents, with data attachés in level of Capitals and Brazilian States (UF), seeking to subsidize united politics of safety of traffic and of work. The used methodology will be through risings, treatment and verification of consistence of the data with analyses in the capitals and Units of Federation (UF), Using tool of statistical analysis for 2005. The results can be useful, could supply subsidies for the target of actions in the extent of the system of transports, in way to minimize the imposed risks the quality of life of the population, described through the accidents of traffic. / O homem precisa do trabalho para satisfazer suas necessidades e a preocupação com a segurança e saúde dos trabalhadores constitui um dos problemas mais importantes e atuais, provocando interesse aos empregadores, agentes do governo e principalmente os empregados. Investimentos na prevenção e conhecimentos sobre estes acidentes trazem vantagens para as empresas, trabalhadores e sociedade. E ao destacar o acidente de trajeto, encontram-se deficiências das fontes de informações oficiais, onde os sub-registros dominam, impedindo uma análise confiável. Também danos provocados pelos acidentes de trânsito são um dos principais problemas de saúde pública em nível mundial, requerendo esforços combinados para prevenção efetiva e sustentável. Este trabalho tem como objetivo principal investigar possíveis relações entre indicadores socioeconômicos e populacionais com a ocorrência dos acidentes de trajeto, com dados agregados em nível de Unidades da Federação (UF) e as capitais brasileiras (CB), visando subsidiar políticas conjuntas de segurança de trânsito e de trabalho. A metodologia utilizada será através de levantamentos, tratamento e verificação de consistência dos dados com análises nas capitais e Unidades da Federação (UF), utilizando ferramenta de análise estatística, para o ano de 2005. Os resultados poderão ser úteis, fornecendo subsídios para o direcionamento de ações no âmbito do sistema de transportes, de modo a minimizar os riscos impostos a qualidade de vida da população, descrito através dos acidentes de trânsito.

Climat d'unités de travail et accidents de travail d'un centre hospitalier rôle médiateur de la violence psychologique

Depuis quelques décennies, les approches traditionnelles en matière de prévention des accidents de travail ne permettent plus de diminuer la fréquence de survenue de ces derniers au-delà d'un certain seuil. Devant cette impasse, les liens unissant le climat de travail (CT) à la prévalence d'accidents de travail (AT) ont été explorés par de nombreuses études. Bien que ces résultats permettent certaines avancées dans le domaine de la prévention, la compréhension des mécanismes sous-jacents à cette relation complexe demeure encore superficielle. La littérature du domaine suggère que d'autres variables, comme la violence psychologique au travail (VPT), puissent jouer un rôle de médiateur dans la relation observée entre le CT et les AT. Afin de vérifier cette possibilité, 2595 travailleurs francophones d'un centre hospitalier du Québec ont complété un questionnaire permettant de répondre aux deux principaux objectifs de l'étude: (1) examiner le lien entre les dimensions du CT et le nombre d'AT sur chaque unité de travail; (2) vérifier l'existence d'un effet de médiation partiel de la fréquence de comportements de VPT subis ou observés sur la relation unissant le CT et la fréquence d'AT en milieu hospitalier. Les banques de données du centre hospitalier ont permis de recenser les AT afin de compléter la base de données. Afin de vérifier ces objectifs, des analyses corrélationnelles et de médiation par la méthode de rééchantillonnage suggérées par MacKinnon, Lockwood et Williams (2004) ont été effectuées. Les implications de cette étude pour la recherche et la pratique sont ensuite discutées.

The impact of transformational leadership on the delivery of safety and productivity excellence at Impala Platinum

The underground hard rock mining industry in South Africa is faced with the challenge of simultaneously delivering both safety and productivity excellence. Frontline supervisors need to manage the inherent trade-off between safety and productivity to achieve excellence in both. Previous research suggests that supervisors with a transformational leadership style have delivered safety and productivity in separate studies. This study examined the relationship between the transformational leadership style of underground mining supervisors at Impala and the simultaneous delivery of both safety and productivity. It also investigated the mediating effect of group safety climate on the relationship between transformational leadership and injury rate, and the mediating effect of group cohesiveness on the relationship between transformational leadership and productivity. The relationship between transformational leadership and the perception of leader effectiveness was also investigated in the study context. The research design was a case study using mixed methods in the form of a sequential explanatory design. In the first quantitative main phase of the study, survey questionnaires were completed by respondents to determine the leadership style of the mine overseers who were the unit of analysis. Data was also collected using survey questionnaires for group safety climate and group cohesiveness. The injury rate and productivity data for the mine overseer sections was recorded for a one year period prior to the survey. In the second qualitative phase of the study selected mine overseers were interviewed and a focus group of mine overseers was conducted. Also, underground observations were carried out and documents were scrutinised. The focus of the qualitative research was to interpret and explain the results that were obtained in the quantitative first phase of the study. The results indicate that transformational leadership is related to the perception of supervisors’ effectiveness. This relationship is partially mediated by group safety climate and group cohesiveness. The relationship between transformational leadership and the objective measures of injury vi rate and productivity were not supported in the quantitative results. This may indicate that these measures were too narrow to determine supervisors’ leadership effectiveness, and/or were contaminated by confounding variables as was suggested in the qualitative phase of the study. The qualitative findings indicated that supervisors’ perception was that transformational leadership style is effective in delivering safety and productivity excellence. The overall conclusion of this study is that in the context of Impala or similar operations, that the effective supervisor should employ the full range of leadership. This behaviour includes maintaining discipline and using contingent reward to motivate the achievement of goals. The effective supervisor also uses the transformational leadership style giving meaning to work and creating a feeling of team membership. Transformational leadership inspires the diverse workforce to deliver safety and productivity excellence in the difficult and risky mining conditions. Furthermore, supervisors’ behaviour is greatly influenced by management’s priorities. / Business Management / D. B. L.

Today, there are few studies about risks and accidents at work with harvesters, but each study creates a better basis to prevent future accidents to occur. The purpose of this study was to examine machine driver's perception of risks and accidents at work with harvesters Komatsu 901, 901TX. The focus was on the drivers of the model Komatsu 901 and 901TX, a common thinning and final felling machine in Sweden. Results show that there are deficiencies that are recurring on multiple machines. These deficiencies also have a connection to the actual accidents. Half of the drivers felt that there were defects (flaws) around the handle and the footboard and only one driver had access to the alarm box. The drivers believe the current structure of harvester can be improved but is not always taken seriously when they make their requests for improvements to reduce risks.
