Análise da função renal em idosos com comprometimento cognitivo leve usuários de lítio em baixa dosagem: um estudo randomizado, duplo cego, placebo-controlado / Analysis of the renal function in elderly with mild cognitive impairment using lithium in low dose: a randomized, double-blind, placebo- controlled study

Introdução: segundo a literatura, sais de lítio podem produzir redução da função renal. A magnitude dessa informação é debatível, uma vez que não há estudo clínico randomizado e controlado entre usuários de lítio, em sua maioria pacientes com depressão ou transtorno bipolar. A possibilidade do uso do lítio para o tratamento da demência de Alzheimer prodrômica reforça a necessidade de maior investigação de efeitos adversos atribuídos ao lítio, especialmente com relação à função renal. Objetivos: avaliar a segurança da utilização do lítio em baixa dosagem com relação à função renal de pacientes idosos. Como objetivos secundários serão avaliadas: a segurança clínica através de exame e questionário específico, as funções tireoidiana, imunológica e o metabolismo glicêmico. Métodos: estudo randomizado e placebo controlado de 2 anos, seguido de fase aberta por mais 2 anos. Foram avaliados 59 idosos com comprometimento cognitivo leve com seguimento mínimo de dois anos (fase controlada). A função renal foi estimada através das fórmulas aMDRD e CKD- EPI, a partir de exames laboratoriais e dados clínicos coletados durante o estudo. As funções tireoidiana, imunológica e glicêmica foram avaliadas respectivamente através de TSH, T4 livre, leucócitos total, neutrófilos, linfócitos, glicemia e insulinemia de jejum, e HOMA-IR. A segurança clínica foi avaliada através de entrevista sistemática realizada a cada 3 meses, utilizando exame físico e a escala UKU para efeitos adversos. Resultados: não houve piora da função renal com o uso do lítio (litemia entre 0,25-0,5 mmol/l) tanto pela aMDRD (p=0,453) como pela CKD-EPI (p=0,181). Houve aumento significativo de neutrófilos (p=0,038) e do TSH (p=0,034). O grupo lítio apresentou incidência significativamente maior de diabetes mellitus (p=0,037) e arritmias (p=0,028), maior ganho de peso (p=0,015), mais sintomas na escala UKU (p=0,045), e maior interferência dos efeitos adversos do lítio em atividades diárias (p<0,001). Houve correlação entre a opinião de médico e do paciente nas interferências das atividades diárias atribuídas aos sintomas adversos (p<0,001). Conclusões: o uso de lítio em baixa dose não alterou a função renal, produziu alterações no sistema imunológico e tireoidiano sem impacto clínico, e foi seguro clinicamente. As razões do aumento de incidência de diabetes e arritmias merecem investigação posterior / Introduction: according to the literature, lithium salts may produce a reduction in kidney function. The magnitude of this information is debatable because there is no randomized and controlled clinical trial among lithium users, being mostly patients with depression or bipolar disorder. The possibility of using lithium for the treatment of prodromal Alzheimer\'s disease dementia increases the need for further investigation of adverse effects attributed to lithium, especially regarding to renal function. Objectives: To evaluate the safety of using low-dose lithium with respect to renal function in elderly patients. Secondary objectives were the evaluation of the clinical safety through specific questionnaire and clinical assessment, and to assess thyroid, immunological and glycemic function. Methods: a randomized and placebo controlled study for 2 years, followed by an open label follow-up of 2 years. We evaluated 59 elderly patients with mild cognitive impairment with accomplishment of at least two years of the controlled phase. Renal function was estimated by the aMDRD and CKD-EPI equation, and by laboratory and clinical data collected during the trial. The thyroid, immunological and glycemic functions were respectively evaluated by TSH, free T4, leukocyte count, neutrophil count, lymphocyte count, fasting plasma glucose and insulin, and the HOMA-IR. The clinical safety was evaluated through systematic examination performed every 3 months, with physical examination, clinical interview and UKU scale for adverse effects. Results: There was no decline of renal function with the use of lithium (litemia between 0.25-0.5 mmol/l) both in the aMDRD (p=0.453) and CKD-EPI (p=0.181) equations. A significant increase of neutrophils (p=0.038) and TSH (p=0.034) were observed. The lithium group showed significantly higher incidence of diabetes mellitus (p=0.037), arrhythmias (p=0.028), weight gain (p=0.015), more symptoms of UKU scale (p=0.045), and greater interference from the adverse effects of lithium during daily activities (p<0.001). There was an observed correlation between the opinion of the attending physician and the patient in respect to the interference in daily activities secondary to the adverse symptoms (p<0.001). Conclusions: The use of lithium in low doses did not result in renal function impairment, produced subtle changes in the immunological system and thyroid function, and was clinically safe for adverse effects. The reasons for the increased incidence of arrhythmias and diabetes mellitus deserve further investigation

Avaliação dos eventos envolvidos na evolução crônica da lesão renal aguda pós isquêmica em ratos com deficiência de vitamina D / Assessment of the events involved in chronic evolution of acute kidney injury in a murine ischemia/reperfusion model after vitamin D deficiency

Na maioria dos países, a incidência e prevalência da doença renal crônica (DRC) vem aumentando ao longo dos anos. Embora tenha havido uma melhora significativa no manejo da DRC com os inibidores do sistema renina-angiotensina-aldosterona, a doença ainda é progressiva, levando a necessidade do surgimento de novas estratégias protetoras. A fibrose renal progressiva está presente na DRC e envolve a participação de várias citocinas, com destaque para o fator Transforming growth factor- beta1 (TGF-beta1). Tem sido demonstrado que a mortalidade de pacientes com DRC está diretamente relacionada à função renal e está associada a riscos tradicionais como cardiovasculares e infecções. Entretanto, esses riscos tradicionais explicam apenas metade das causas de mortalidade nesses pacientes. Evidências crescentes mostram que o status de vitamina D pode ser um fator de risco não tradicional para a evolução da DRC. Tendo em vista o importante papel da vitamina D na manutenção das funções fisiológicas essenciais e a observação da queda dos níveis deste hormônio na DRC, torna-se relevante o estudo da deficiência de vitamina D nos eventos envolvidos na evolução crônica da lesão renal aguda em modelo experimental de isquemia/reperfusão renal. Ratos Wistar foram divididos em quatro grupos: controle, animais que receberam dieta padrão; dVD, animais que receberam dieta depletada em vitamina D; Isq, animais que receberam dieta padrão e foram submetidos ao insulto de isquemia/reperfusão renal bilateral no 28º dia; Isq+dVD, animais que receberam dieta depletada em vitamina D e foram submetidos ao insulto de isquemia/reperfusão bilateral no 28º dia. Ao final dos 90 dias do protocolo, os animais foram submetidos à eutanásia, amostras de sangue, urina e o tecido renal foram coletados para a análise dos mecanismos de lesão renal. Os animais submetidos ao insulto de isquemia/reperfusão renal apresentaram hipertrofia renal, aumento dos níveis de pressão arterial média, colesterol e de PTH plasmático. Além disso, foi observada expansão da área intersticial, aumento do infiltrado de macrófagos/monócitos, da expressão de colágeno IV, fibronectina, vimentina e alfa-actina e redução da expressão da proteína Klotho. A deficiência de vitamina D contribuiu para a elevação dos níveis plasmáticos de PTH e aumento da proteinúria assim como para as alterações túbulo-intersticiais crônicas importantes (fibrose e infiltrado inflamatório do interstício, dilatação e atrofia tubular), aumento da expressão da citocina TGF-beta1 expressão do receptor de vitamina D (VDR) e da proteína Klotho, observados nos animais deficientes em vitamina D submetidos ao insulto de isquemia/reperfusão renal. Portanto, através de vias inflamatórias e com participação do fator de crescimento TGF-beta1 ê um fator agravante para o dano túbulo-intersticial e formação de fibrose intersticial nesse modelo experimental de isquemia/reperfusão renal / In most countries, the incidence and prevalence of chronic kidney disease (CKD) has been increasing over the years. Although there was a significant improvement in the management of CKD with renin-angiotensin-system inhibitors, the disease is still progressive, leading to the need of emergence of new protective strategies. The progressive renal fibrosis is present in CKD and involves the participation of several cytokines, especially the Transforming growth factor-beta1 (TGF-beta1). It has been shown that the mortality of patients with CKD is directly related to renal function, which is associated to traditional risk factors such as cardiovascular diseases and infections. However, these traditional risk factors explain only half of the causes of mortality in these patients. Growing evidence shows that vitamin D status may be a non-traditional risk factor for the progression of CKD. Considering the important role of vitamin D in the maintenance of essential physiological functions and the observation of low levels of this hormone in CKD, the study of vitamin D deficiency in the events involved in chronic evolution of acute kidney injury in an experimental model of ischemia/reperfusion becomes relevant. Wistar rats were divided into four groups : control, animals received a standard diet ; dVD, animals received a vitamin D-depleted diet ; Isq, animals received a standard diet and were subjected to bilateral renal ischemia/reperfusion injury on day 28; Isq +dVD, animals received a vitamin D-depleted diet and were subjected to bilateral renal ischemia/reperfusion injury on day 28 . At the end of the 90 days of the protocol, the animals were euthanized and samples of blood, urine and kidney tissue were collected for analysis of the mechanisms of renal injury. The animals subjected to the insult of ischemia/ reperfusion showed renal hypertrophy, increased levels of mean blood pressure, cholesterol and plasma PTH. Furthermore, expansion of the interstitial area, increased infiltration of macrophages/monocytes, increased expression of collagen IV, fibronectin, vimentin and alpha-actin, and reduced expression of Klotho protein were observed. The vitamin D deficiency contributed to the elevation of plasma PTH levels and increased proteinuria as well as for important chronic tubulo-interstitial changes (fibrosis and inflammatory infiltration of the interstitium, tubular dilation and atrophy), increased expression of cytokine TGF-beta1 vitamin D receptor (VDR) and Klotho protein observed in vitamin D-deficient animals subjected to the insult of renal ischemia/reperfusion. Therefore, through inflammatory pathways and involvement of TGF-beta1 w y aggravating factor in tubulointerstitial damage and formation of interstitial fibrosis in this experimental model of renal ischemia/reperfusion

<p>Akutno bubrežno o&scaron;tećenje je česta i ozbiljna komplikacija nakon hirur&scaron;ke rekonstrukcije abdominalne aorte. Ona produžuje, poskupljuje bolničko lečenje i povećava morbiditet i mortalitet bolesnika. &Scaron;to ranije prepoznavanje bolesnika koji su pod povi&scaron;enim rizikom uz pomoć prediktivnih faktora, kao i identifikovanje akutnog bubrežnog o&scaron;tećenja od velikog je značaja za pravovremeno započinjanje preventivnih i terapijskih mera i smanjenje mortaliteta ovih bolesnika. Cilj istraživanja: Utvrditi incidencu akutnog bubrežnog o&scaron;tećenja, najznačajnije preoperativne i intraoperativne prediktivne faktore za nastanak akutnog bubrežnog o&scaron;tećenja i utvrditi da li serumski cistatin C ranije i preciznije ukazuje na akutno bubrežno o&scaron;tećenje u odnosu na serumski kreatinin 72 sata nakon elektivnih operacija na infrarenalnom segmentu aorte. Metodologija: Na Klinici za anesteziju, intenzivnu terapiju i terapiju bola i Klinici za vaskularnu i transplantacionu hirurgiju Kliničkog centra Vojvodine sprovedeno je prospektivno opservaciono istraživanje u trajanju od 18 meseci, od oktobra 2017. do aprila 2019. godine. U istraživanje je uključeno 140 pacijenata koji su podvrgnuti elektivnom operativnom zahvatu na infrarenalnom segmentu aorte. Preoperativni i intraoperativni potencijalni prediktivni faktori identifikovani su iz medicinske dokumentacije: istorije bolesti, protokoli anestezije, dnevne terapijske liste, liste vitalnih parametara i liste laboratorijskih parametara. Nastanak ABO je potvrđivan pomoću AKIN klasifikacionog sistema. Tokom postoperativnog perioda lečenja od 72 sata obraćana je pažnja na vreme i stepen nastanka ABO. Za statističku obradu podataka kori&scaron;ćen je program SPSS, verzija 21 (IBM SPSS, Čikago, Ilinois). Rezultati su predstavljeni tabelarno i grafički, a statistički značajnim su se smatrale vrednosti nivoa značajnosti p&lt;0,05. Podaci su obrađivani standardnim statističkim testovima. Za dobijanje modela predikcije primenjena je multivarijantna logistička regresija. Rezultati: Incidenca akutnog bubrežnog o&scaron;tećenja nakon elektivnih operacija na infrarenalnom segmentu aorte na Klinici za anesteziju, intenzivnu terapiju i terapiju bola i Klinici za vaskularnu i transplantacionu hirurgiju Kliničkog centra Vojvodine iznosi 28,56%. Prema ROC analizi granična vrednost koncentracije cistatina C od 1,14 mg/l ima najvi&scaron;u senzitivnost (82,5%), i specifičnost (76%) u diferenciranju bolesnika koji će dobiti akutno bubrežno o&scaron;tećenje. Kompletan model predikcije ABO nakon elektivnih operacija na infrarenalnom segmentu aorte sadrži sledeće faktore: prisustvo hronične bubrežne slabosti, preoperativna serumska koncentracija cistatina C &gt;1,14 mg/l, primena koloida u volumenu &gt;500 ml u toku operacije, ukupni volumen nadoknade u intraoperativnom periodu &gt;59 ml/kg. Zaključak: Incidenca akutnog bubrežnog o&scaron;tećenja nakon elektivnih operacija na infrarenalnom segmentu aorte na Klinici za anesteziju, intenzivnu terapiju i terapiju bola i Klinici za vaskularnu i transplantacionu hirurgiju Kliničkog centra Vojvodine je ne&scaron;to vi&scaron;a u odnosu na literaturne podatke. Cistatin C je bolji prediktor i pokazatelj akutnog bubrežnog o&scaron;tećenja u odnosu na kreatinin. Bolesnici koji imaju hroničnu bubrežnu slabost, preoperativni nivo cistatina C&gt;1,14 mg/l, koji tokom operacije dobiju preko 500 ml koloidnih rastvora, i ukupni volumen nadoknade &gt;59 ml/kg imaju veću &scaron;ansu da dobiju akutno o&scaron;tećenje bubrega u postoperativnom periodu.</p> / <p>Acute kidney injury is frequent and serious complication after aortic surgery which increases length of hospital stay, costs, morbidity and mortality. Earlier recognition of patients at risk by predictive factors and identification of acute kidney injury could have important role on right timing of preventive and therapeutic measures and lower mortality of these patients. Aims: to investigate the incidence of acute kidney injury, the most important preoperative and intraoperative predictive factors for acute kidney injury, and check whether cystatin C indicates acute kidney injury earlier and more accurately than creatinine 72 hours after elective infrarenal aortic surgery. Methodology: This prospective observational study was performed at Clinic of Anesthesia, Intensive Care and Pain Therapy and Clinic of Vascular and Transplantation Surgery, Clinical Centre of Vojvodina, during the period of 18 months, from October 2017 till April 2019. It includes 140 adult patient who underwent elective infrarenal aortic surgery. Potential predictive factors were identified out of medical records such as: patient history, anesthesia lists, daily therapeutic lists, vital parameters and laboratory values lists. The occurrence of acute kidney injury was noted according to AKIN criteria. IBM SPSS version 21 (Chicago, Illinois) was used for statistical analysis. The results were presented in tables and graphs, statistical significance was set at p value of less than 0,05. Standard statistical tests were applied. Multivariate logistic regression model was used for potential predictive factors. Results: The incidence of acute kidney injury at Clinic of Anesthesia, Intensive Care and Pain Therapy and Clinic of Vascular and Transplantation Surgery, Clinical Centre of Vojvodina, was 28,56%. The cut off value of cystatin C serum concentration of 1,14 mg/l has the highest sensitivity (82,5%), and specificity (76%) in the differentiation of patients who will develop acute kidney injury. The final model for predicting acute kidney injury in patients who underwent elective infrarenal aortic surgery contains the following variables: presence of chronic kidney failure, preoperative serum concentration of cystatin C&gt;1,14 mg/l, application of colloid solutions in volume&gt;500 ml during the operation and total intravascular fluid replacement volume&gt;59 ml/kg in intraoperative period. Conclusion: The incidence of acute kidney injury at Clinic of Anesthesia, Intensive Care and Pain Therapy and Clinic of Vascular and Transplantation Surgery, Clinical Centre of Vojvodina, is somewhat higher comparing to world literature data. Patients who are more likely to develop acute kidney injury have: chronic kidney failure, preoperative serum concentration of cystatin C&gt;1,14 mg/l and get colloid solutions in volume&gt;500 ml and total intravascular fluid replacement volume&gt;59 ml/kg during intraoperative period.</p>

Cell Death Pathways Drive Necroinflammation during Acute Kidney Injury

Renal tubules represent an intercellular unit and function as a syncytium. When acute tubular necrosis was first visualized to occur through a process of synchronized regulated necrosis (SRN) in handpicked primary renal tubules, it became obvious that SRN actually promotes nephron loss. This realization adds to our current understanding of acute kidney injury (AKI)-chronic kidney disease (CKD) transition and argues for the prevention of AKI episodes to prevent CKD progression. Because SRN is triggered by necroptosis and executed by ferroptosis, 2 recently identified signaling pathways of regulated necrosis, a combination therapy employing necrostatins and ferrostatins may be beneficial for protection against nephron loss. Clinical trials in AKI and during the process of kidney transplantation are now required to prevent SRN. Additionally, necrotic cell death drives autoimmunity and necroinflammation and therefore represents a therapeutic target even for the prevention of antibody-mediated rejection of allografts years after the transplantation process.

L’insuffisance rénale aiguë congestive en chirurgie cardiaque

Background: Every year, more than 2 million people undergo cardiac surgery including 15 000 Canadians (1). Acute kidney injury remain a frequent complication in this setting which can affect up to 39% of patients (2). This complication is associated with a significant increase in the risk of short-term and long-term mortality after cardiac surgery (1). Multiple mechanisms can lead to acute kidney injury in the peri-operative period which complexify prevention and treatment. Among them, multiple clinical factors can result in an increase in venous pressure leading to a state of systemic congestion deleterious to kidney function in addition to other organs. The detection of congestion at the bedside of patients after cardiac surgery could be used to identify patients at risk of developing congestive complications such as congestive acute kidney injury as well as opening possibilities for prevention and treatment. Doppler ultrasound is a non-invasive technology enabling the assessment of blood flow velocity within the venous system. A reduction of systemic venous compliance lead to the appearance of alterations in portal vein flow and intra-renal venous flow. The objectives of the work presented in this thesis were the following: To determine the prevalence and predictive factor associated with the appearance of venous flow alterations during the intra-operative and post-operative period, to determine if their detection is associated with acute kidney injury in the post-operative period and to determine the clinical significance of their detection in the immediate post-operative period. Main results: This thesis is comprised of 3 cohort studies including a total of 1497 ultrasound asessments in 362 patients. Alterations in venous Doppler signals were observed in a subtantial proportion of patients during the per-operative period, from 10.8% to 24.3% depending on the time of assessment and the site assessed. We observed significant correlations between venous Doppler alterations and other clinical markers of congestion including central venous pressure, NT-pro-BNP and fluid balance. Furthermore, we observed that portal flow pulsatility and abnormal patterns of intrarenal venous flow were correlated. Using repeated assessments in a cohort of 145 patients, we observed that portal flow pulsatility and severe alterations in intrarenal venous flow were associated with the subsequent development of acute kidney injury in the post-operative period. A re-analysis of this data suggested that a grading system combining mutliple Doppler assesments at intensive care admission after cardiac surgery including heaptic veins, the portal vein and intrarenal veins may be able to identify patients at risk of developping acute kidney injury with high specificity. Conclusions: In the context of cardiac surgery, Doppler ultrasound can be used to identify alterations in peripheral venous Doppler signals suggestive of a congestion phenomenon and may be able to anticipate complications related to venous congestion such as acute kidney injury. / Contexte : Chaque année, plus de 2 millions de personnes subissent une chirurgie cardiaque, dont 15 000 Canadiens (1). L’insuffisance rénale aiguë demeure une complication fréquente chez les patients subissant une chirurgie cardiaque atteignant une incidence jusqu’à 39 % dans la période postopératoire (2). Cette complication est associée à une augmentation du risque de mortalité à court et long termes. Plusieurs mécanismes peuvent engendrer l’insuffisance rénale aiguë dans la période peropératoire, ce qui complexifie la prévention et le traitement. Parmi ceux-ci, divers facteurs peuvent engendrer une augmentation des pressions veineuses menant à un état de congestion systémique qui affecte la fonction des reins ainsi que celle des autres organes vitaux. La détection de la congestion au chevet des patients durant la période intraopératoire et postopératoire pourrait permettre d’identifier les individus à risque de développer des complications de nature congestive telles que l’insuffisance rénale aiguë ainsi que de mettre en place des stratégies de prévention et de traitement. L’échographie Doppler est une technologie non invasive qui permet d’évaluer la vélocité du sang dans le réseau veineux. La diminution de la compliance veineuse entraine l’apparition d’altérations du flot veineux de la veine porte et des veines intrarénales. Les objectifs des travaux présentés dans cette thèse étaient les suivants : déterminer la prévalence ainsi que les facteurs prédicteurs de l’apparition de ces altérations durant la période peropératoire; déterminer si la détection de ces altérations est en mesure de prédire l’apparition d’insuffisance rénale aiguë dans la période postopératoire; et déterminer quelle est la signification clinique de l’apparition de ces signes dans la période postopératoire immédiate. Résultats principaux : Les travaux contenus dans cette thèse comportent trois études de cohorte comprenant 1497 examens échographiques chez 362 patients. La présence d’altération du flot veineux a été observée chez une proportion substantielle des patients durant la période post-opératoire, allant de 10.8% à 24.3% selon le site intérrogé et le moment où l’examen est effectué. Nous avons observé des associations entre les altérations du flot veineux et les autres marqueurs de congestion incluant la pression veineuse centrale, la mesure du NT-pro-BNP et la balance liquidienne. De plus, nous avons observé que la pulsatilité du flot portal est corrélée aux altérations du signal Doppler dans les veines intrarénales. Grâce à des examens répétées effectuées dans une cohorte de 145 patients, nous avons observé que la pulsatilité du flot portal et la présence d’un profil compatible avec une anomalie sévère du flot intrarénal veineux étaient associées indépendamment avec la survenue subséquente d’insuffisance rénale aiguë durant la période postopératoire. Une réanalyse de ces données nous a permis de constater qu’un système de gradation combinant la présence des altérations du flot veineux à plusieurs sites, incluant les veines hépatiques, la veine porte et les veines intrarénales, au moment de l’admission aux soins intensifs permet d’indentifier les patients qui développeront une insuffisance rénale aiguë avec une spécificité élevée. Conclusions : Dans le contexte de la chirurgie cardiaque, l’échographie Doppler peut être utilisée au chevet afin d’indentifier des altérations du flot veineux périphérique suggestives d’un phénomène de congestion et d’anticiper les complications de nature congestive tel que l’insuffisance rénale aiguë.

Aims: The thesis deals with the biomarkers of early renal injury, namely albuminuria and neutrophil gelatinase-associated lipocalin (NGAL). The aims in the case of albuminuria were the implementation of HPLC method, comparing HPLC with immunoturbidimetric (IT) method and monitoring the relationship to the diagnosis of diabetes mellitus. The aim of urinary NGAL (and eventually other markers) examination was to verify its reliability in the prediction of acute kidney injury (AKI). Methods: We investigated albuminuria in fresh urine samples in the groups of 636 diabetics and 456 nondiabetics using the HPLC method (Agilent 1200, Agilent Technologies, USA) and immunoturbidimetrically (Cobas Integra 400, Roche Diagnostics); we studied the correlations and relationships between albuminuria and glycated hemoglobin HbA1c. We investigated urinary NGAL by chemiluminescent microparticle immunoassay (Architect i4000, Abbott) in children's groups: 1) after renal transplantation (N = 15), 2) with acute or chronic kidney disease (N = 28); and in adult patient's groups: 1) after cardiac surgery (N = 10) and 2) post angiography (N = 41). Hydrodynamic delivery for the study, treatment and prevention of acute kidney injury

Indiana University-Purdue University Indianapolis (IUPUI) / Advancements in human genomics have simultaneously enhanced our basic understanding of the human body and ability to combat debilitating diseases. Historically, research has shown that there have been many hindrances to realizing this medicinal revolution. One hindrance, with particular regard to the kidney, has been our inability to effectively and routinely delivery genes to various loci, without inducing significant injury. However, we have recently developed a method using hydrodynamic fluid delivery that has shown substantial promise in addressing aforesaid issues. We optimized our approach and designed a method that utilizes retrograde renal vein injections to facilitate widespread and persistent plasmid and adenoviral based transgene expression in rat kidneys. Exogenous gene expression extended throughout the cortex and medulla, lasting over 1 month within comparable expression profiles, in various renal cell types without considerably impacting normal organ function. As a proof of its utility we by attempted to prevent ischemic acute kidney injury (AKI), which is a leading cause of morbidity and mortality across among global populations, by altering the mitochondrial proteome. Specifically, our hydrodynamic delivery process facilitated an upregulated expression of mitochondrial enzymes that have been suggested to provide mediation from renal ischemic injury. Remarkably, this protein upregulation significantly enhanced mitochondrial membrane potential activity, comparable to that observed from ischemic preconditioning, and provided protection against moderate ischemia-reperfusion injury, based on serum creatinine and histology analyses. Strikingly, we also determined that hydrodynamic delivery of isotonic fluid alone, given as long as 24 hours after AKI is induced, is similarly capable of blunting the extent of injury. Altogether, these results indicate the development of novel and exciting platform for the future study and management of renal injury.

Chronic kidney disease (CKD) and acute kidney injury (AKI) are major public health problems. It is important to be able to identify those at high risk of adverse outcome, CKD progression and associated cardiovascular disease. The aim of the thesis was to study novel promising biomarkers, their relationship to kidney function, chronic inflammation and/or cardiovascular risk - placental growth factor (PlGF), pregnancy associated plasma protein A (PAPP-A), matrix metalloproteinase 2 (MMP-2), matrix metalloproteinase 9 (MMP-9), soluble receptor for advanced glycation end products (sRAGE), calcium binding protein S100A12 or extracellular newly identified RAGE binding protein (EN-RAGE), and high mobility group box protein-1 (HMGB-1) in patients with renal diseases including CKD, haemodialysis (HD), AKI patients, and healthy controls for comparison. First study revealed that PlGF is elevated in patients with decreased renal function. Second study demonstrated the association of MMP-2 and PAPP-A with proteinuria in patients with CKD. Moreover, serum MMP-2, MMP-9 and PAPP-A levels significantly differed in patients with various nephropathies.
