As aminotransferases são rotineiramente empregadas para o seguimento da doença e do tratamento antiviral. A prática da utilização destas enzimas tem demonstrado que os pacientes com insuficiência renal crônica em hemodiálise apresentam níveis pouco elevados de transaminases. Para determinar um melhor ponto de corte ( cutoff ) dos níveis de alanina aminotransferase na identificação de pacientes em hemodiálise com anti-HCV positivo, 202 (123 homens e 79 mulheres) pacientes foram estudados durante 6 meses. Os níveis de alanina aminotransferase foram medidos mensalmente. O valor convencional de cutoff , pelo método, foi de 33 UI/mL para homens e 27 UI/mL para mulheres. O melhor ponto de corte foi obtido através de uma curva ROC (receiver operating characteristic). A prevalência de anti-HCV positivo foi de 7,5%. O tempo médio em hemodiálise foi maior nos pacientes anti- HCV positivos comparado ao grupo com anti-HCV negativo com média de 57 meses (7 a 248) e de 31 meses (6 a 114 meses), respectivamente (p<0,0001) . A média de alanina aminotransferase foi de 24,83 &#1048690; 6,36 UI/mL e 21,67 &#1048690; 6,2 UI/mL em homens e mulheres respectivamente para o grupo com anti-HCV positivo. A média de alanina aminotransferase nos pacientes com anti-HCV negativo foi de 16,5 &#1048690; 6,38 UI/mL e 13,17 &#1048690; 5,02 UI/mL para homens e mulheres respectivamente. Quando comparada, a média de alanina aminotransferase foi significantemente maior (p < 0,0001) entre os pacientes com anti-HCV positivo do que aqueles com anti-HCV negativo. O melhor ponto-de-corte é aquele que revela a maior sensibilidade e maior especificidade, correspondendo ao ponto mais elevado e desviado para a esquerda na curva ROC. O nosso valor de cutoff para a alanina aminotransferase teve uma sensibilidade e especificidade de 66,67 e 75,40%, respectivamente, quando se usa 0,60 do limite convencional de normalidade na identificação de hepatite viral. Sugere-se que os limites de normalidade para a alanina aminotranferases sejam reduzidos para os pacientes com insuficiência renal crônica em hemodiálise

Inhibition of Human Immunodeficiency virus replication through small RNA-induced gene silencing of HIV-1 Tat specific factor 1

Although treatments exist that can substantially alleviate the morbidity and mortality associated with HIV, there is still a need for improved anti-­‐HIV treatments that reduce toxicities and administration frequency and mediate sustained inhibition of viral replication. Given the high mutability and variability of the virus, a strategy that is garnering increasing focus is the targeting of host factors that the virus requires to replicate, so-­‐called HIV-­‐dependency factors (HDFs). It is hoped this will reduce the emergence of viral drug resistance. A number of genome-­‐wide screens have been performed to identify HDFs, although many remain to be validated, particularly in relevant cells lines. An objective of this thesis was to validate three host factors as HDFs, in both TZM-­‐bl reporter and T cell-­‐derived cell lines, and to examine their potential as anti-­‐HIV-­‐1 therapeutic targets through exploitation of the cellular gene silencing pathway, RNA interference (RNAi). These were HIV-­‐1 Tat specific factor 1 (HTATSF1), DEAD (Asp-­‐Glu-­‐Ala-­‐Asp) box polypeptide 3, X-­‐ linked (DDX3X) and SWI/SNF related, matrix associated, actin dependent regulator of chromatin, subfamily b, member 1 (SMARCB1), selected because they had been previously implicated in HIV-­‐ 1 pathogenesis. The well-­‐characterised HDF, PC4 and SFRS1 interacting protein 1 (PSIP1)/lens epithelium-­‐derived growth factor (LEDGF)/p75, was included in the study as a positive control. Cassettes expressing short hairpin RNAs (shRNAs) targeting the four host proteins were generated, although shRNAs did not suppress endogenous ddx3x mRNA levels. The ability of shRNAs to inhibit HIV-­‐1 replication in the reporter cell line, TZM-­‐bl, was examined. These HeLa-­‐ derived cells are permissive for R5-­‐tropic HIV-­‐1 infection and contain an integrated luciferase gene driven by the viral promoter. shRNAs mediated a dose-­‐dependent inhibition of luciferase activity in cells infected with a HIV-­‐1 subtype B molecular clone and, although production of the viral protein p24 was unaltered, infectious particle production was decreased in cells treated with a shRNA suppressing HTATSF1. Little effect was observed with a shRNA targeting SMARCB1, suggesting that this may not function as an HDF under these conditions. No effect on infectious particle production was seen with the shRNA targeting PSIP1, which was a result of the long half-­‐ life of this protein, highlighting a limitation of using such reporter systems for HDF validation. Importantly, shRNAs were not associated with any cytotoxic effects in TZM-­‐bl cells. Whether HTATSF1 is a potential therapeutic target was interrogated further in the more relevant T cell-­‐derived SupT1 cell line. Lentiviruses were used to generate populations where >90% had one copy of the integrated shRNA expression cassette. Replication of the subtype B molecular clone p81A-­‐4 was significantly inhibited in the shH1-­‐expressing SupT1 cell line, which targets HTATSF1, for over 14 days post-­‐infection, although inhibition was not as pronounced asthat observed in the shP1-­‐expressing SupT1 cell line, which targets PSIP1. In contrast to a previous report, no change in the ratio of unspliced to singly-­‐ or multiply-­‐spliced HIV-­‐1 transcripts were detected in shH1-­‐expressing SupT1 cells, suggesting that HTATSF1 does not function as a splicing cofactor in this system. A slight rebound in p24 levels at 14 days post-­‐infection was accompanied by increased HTATSF1 expression and a decrease in the percentage of cells with transgene expression in the population. In addition, there was a slight decrease in shH1-­‐derived guide strand expression, but no change in transcription rates of the htatsf1 gene, suggesting that cells within the population with shH1 expression and HTATSF1 suppression may have a growth disadvantage. Thus, although this work demonstrates for the first time that HTATSF1 functions as an HDF in T cell-­‐derived SupT1 cells, it may not constitute a viable therapeutic target. A second objective of this thesis was to examine the feasibility of transcriptional gene silencing (TGS) of HDFs as an anti-­‐HIV strategy. TGS is a small RNA-­‐induced gene silencing pathway that operates through chromatin remodelling with the potential to mediate long-­‐term silencing of gene expression. Thus, its application may reduce the frequency of drug administration and associated toxicities. Short interfering RNAs (siRNAs) targeting the htatsf1 promoter were able to reduce target mRNA expression, which was accompanied by decreased htatsf1 transcription rates in HEK293T cells, suggesting silencing via a TGS mechanism. The htatsf1 silencing inhibited infectious HIV-­‐1 particle production from TZM-­‐bl cells. This work provides proof of principle that TGS induction at a HDF may inhibit HIV-­‐1 replication. siRNAs targeting the ddx3x promoter did not induce TGS. To examine whether gene susceptibility to TGS may be influenced by promoter architectures, 49 promoter features were examined for enrichment in genes at which small RNA-­‐induced TGS has been reported. Initially, the TGS group was compared to a random set of 2,000 promoters and then all other promoters in the genome. To control for gene activation, two further analyses were performed comparing the TGS group features to those from promoters active in the THP-­‐1 cell line and housekeeping genes. Whilst difficult to ascribe differences between the TGS group and the control groups to anything beyond a variation in the proportion of active genes within each group, there was enrichment for certain promoter features that are independent of activity; the TGS group was characterised by broad transcription start regions, high CpG content and a single expression profile. Moreover, the fraction of promoters with reported non-­‐coding RNA overlap was greater in the TGS group than the control groups. Thus, there is some evidence that a number of promoter features are associated with TGS susceptibility. It is hoped this novel analysis will facilitate selection of future TGS targets, including HDFs. In summary, the work presented in this thesis paves the way for development of improved anti-­‐HIV therapies involving HDF-­‐targeted TGS-­‐based gene therapies that mediate sustained inhibition of the virus.

Desde os primórdios da medicina, substâncias químicas derivadas de animais, vegetais e microrganismos têm sido usadas no tratamento de diversas doenças. Dentre estas, os produtos derivados de vegetais têm dominado as farmacopeias por milhares de anos, fornecendo uma fonte inesgotável de recursos medicinais. O metabolismo secundário geralmente é definido como o responsável pela grande variedade de substâncias químicas apresentadas pelas plantas, porém específicos para alguns grupos. Aos flavonoides atribuem-se importantes funções fisiológicas e ecológicas para o vegetal, como proteção contra a radiação e atração de polinizadores. Para os seres humanos, substâncias dessa classe de metabólitos possuem ação medicinal atuando principalmente como agentes anti-inflamatórios, antioxidantes, antitumorais, hepatoprotetores, antimicrobianos e antivirais. O segundo maior gênero de Euphorbiaceae é Croton L., com aproximadamente 1300 espécies arbóreas, arbustivas ou herbáceas, distribuídas em zonas tropicais e subtropicais do Novo e Velho Mundo. Em praticamente todos os ecossistemas brasileiros encontram-se representantes de Croton. Croton cajucara é uma espécie em testes clínicos, tendo indicações terapêuticas para diabetes, distúrbios hepáticos e renais, anti-inflamatório, antimicrobiano, entre outras. Croton sphaerogynus Baill. por emergir no mesmo clado que C. cajucara, torna-se promissora na prospecção de substâncias bioativas. Este trabalho teve como objetivo investigar a composição fenólica e o potencial biológico de substâncias fenólicas presentes nos extratos foliares de C. sphaerogynus por meio de ensaios antioxidantes, antimicrobianos e anti-HIV-1. Como principais resultados, C. sphaerogynus se caracterizou como uma espécie produtora exclusivamente de flavonóis, além de ser grande produtora de diterpenos. Dois flavonoides foram isolados, quercetina 3,7-dimetil éter e campferol 3-metil éter. Quanto às atividades biológicas, a subfração composta majoritariamente por quercetina 3,7-dimetil éter apresentou alto potencial antioxidante e a maior atividade anti-HIV-1 entre os extratos e outras subfrações testadas. A fração hexânica, rica em diterpenos, foi a que apresentou atividade antimicrobiana promissora. Croton cajucara is an example of species in clinical trials, having therapeutic indications for diabetes, hepatic and renal disorders, anti-inflammatory, antimicrobial, among others. A adesão ao tratamento antirretroviral por crianças e adolescentes com HIV/Aids / Antirretroviral treatment adherence of children and adolescents with HIV/Aids

Introdução: A manutenção da boa adesão aos medicamentos antirretrovirais é considerada fundamental para a terapia de pacientes com HIV/Aids, pois falhas no seguimento da prescrição podem levar ao comprometimento de toda a terapia, desenvolvimento de resistência viral e consequente redução nas opções de tratamento. Nas crianças, a adesão plena torna-se mais relevante se forem considerados os diferentes graus de maturidade do sistema imunológico no momento da infecção pelo HIV, tornando, assim, fundamental a avaliação da adesão nesta população. Objetivos: Analisar a taxa de adesão ao tratamento antirretroviral em crianças e adolescentes com HIV/Aids. Métodos: Este estudo transversal, aninhado à coorte de crianças e adolescentes com HIV/Aids atendidas no ambulatório do Instituto da Criança, analisou 101 pacientes. Os dados dos pacientes foram obtidos por meio de questionários sobre as características sociodemográficas e clínicas, sobre a aderência, além de escalas de expectativa de auto-eficácia. A taxa de adesão foi calculada classificando como aderentes aqueles pacientes que tomaram 95 por cento ou mais das doses referidas.  A análise descritiva (média, desvio-padrão, mediana e proporções) foi realizada para caracterizar a população do estudo e o teste de Wilcoxon foi utilizado para comparação das médias da escala de auto-eficácia dos pais/cuidadores e pacientes acima de 11 anos. Resultados: A taxa de adesão ao tratamento antirretroviral foi de 93,1 por cento (IC = [88,7;94,4]). Houve 7 pacientes que apresentaram má adesão. Os medicamentos antirretrovirais mais utilizados foram lopinavir (51,5 por cento ), lamivudina (43,6 por cento ) e Biovir® (associação de lamivudina e zidovudina) (28,7 por cento ). Conclusão: Alta taxa de adesão ao tratamento antirretroviral por crianças e adolescentes pode ser atingida, independentemente de seu perfil sociodemográfico e/ou 95 por cento de seus cuidadores, por meio do cuidado que a equipe multiproffisional dedica aos pacientes / Introduction: Maintaining good adherence to antiretroviral drugs is considered critical to drug therapy of patients with HIV/Aids, because prescription noncompliance can compromise the entire therapy, lead to the development of viral resistance and consequent reduction in treatment options. In children, full adherence becomes more relevant if different degrees of maturity of the immune system at the time of HIV infection are considered, thereby making the assessment of adherence required in this population. Objectives: To analyze adherence rate to antiretroviral treatment in children and adolescents with HIVAids. Methods: This cross-sectional study nested in a cohort of children and adolescents with HIVAids followed up at the Instituto da Criança, assessed 101 patients. Patient data were obtained by means of questionnaires on sociodemographic and clinical characteristics, adherence and self-efficacy expectancy scales. The adherence rate was calculated by classifying as adherents those patients who had taken 95 per cent or more of these doses. The descriptive analysis (mean, standard deviation, median and proportions) was performed to characterize the study population and the Wilcoxon test was used for the correlation between the scale of self-efficacy of parents/caregivers and patients over 11 years old. Results: The adherence rate to antiretroviral treatment was 93.1 per cent (CI = [88.7;94.4]). There were 7 patients who have poor adherence. Antiretroviral drugs frequently used were lopinavir (51.5 per cent ), lamivudine (43.6 per cent ) and Biovir 95 per cent TM (lamivudine plus zidovudine) (28.7 per cent ). Conclusions: High rate of adherence to antiretroviral treatment for children and adolescents can be met, regardless of their demographic profile and/or of their caregivers, through the care that the multiprofessional team dedicated to patients

Estudo in vitro dos potenciais antioxidante, antimicrobiano e anti-HIV de extratos de Hyptis lacustris A. St.-Hil. ex Benth (Lamiaceae) / In vitro study of the antioxidant, antimicrobial and anti-HIV potential of Hyptis lacustris A. St.-Hil. ex Benth. (Lamiaceae) extracts

Hyptis Jacq., Lamiaceae, subtribo Hyptidinae, ocorre nas regiões semiáridas tropicais da América e da África. Compreendendo cerca de 144 espécies, e é composto por ervas ou arbustos com caule quadrangular e estames epipétalos. Conhecido por ser fortemente aromático e comumente utilizado na medicina tradicional para o tratamento de infecções gastrointestinais, cólicas, dores, perturbações da pele, malária e como repelente de mosquitos. Este trabalho propõe uma avaliação dos potenciais antioxidante, antimicrobiano e anti-HIV-1 de diferentes extratos e fases produzidas a partir de folhas e eixos caulinares de H. lacustres e a realização de triagem fitoquímica dos mesmos. Para tanto, fragmentos de caule e folhas foram secos em estufa e triturados; o pó obtido foi submetido à maceração em etanol por sete dias, à temperatura ambiente. O extrato bruto hidroetanólico foi liofilizado e, parte do extrato, particionada com hexano, diclorometano e acetato de etila. Após as fases serem secas, foi realizada a caracterização das substâncias por Cromatografia Gasosa acoplada a Espectrometria de Massas (CG-EM) e Cromatografia Líquida de Alta Eficiência (CLAE-DAD). As frações obtidas foram identificadas com o auxílio de técnicas de Ressonância Magnética Nuclear (RMN) de 1H, 13C e DEPT 135, sendo isolados e identificados três diterpenoides abietanos: sugiol, horminona e 7-O-acetilhorminona. Os diterpenoides abietanos de duas frações foram identificados com técnicas de RMN e CG-EM, uma fração constituída por uma mistura de ácidos graxos com o diterpenoide abietano 6,7-dehidroferruginol e uma fração constituída por dois diterpenoides abietanos (8&beta;-hidroxi-9(11),13-abietadien-12-one e ferruginol) e ácidos graxos. A avaliação do potencial antioxidante foi realizada pelos métodos de sequestro dos radicais DPPH e ABTS, atividade redutora de ferro (III), atividade quelante de ferro e ORAC (Oxygen radical absorbance capacity). A fase de acetato de etila foi a mais promissora nos ensaios realizados, com valores de EC50 de 16,73 &mu;g mL-1 para DPPH, 5,18 &mu;g mL-1 para ABTS, 44,75 &mu;g mL-1 para quelante, 5,35 &mu;g mL-1 para FRAP e 0,81 &mu;g mL-1 para o ORAC. Para o teste de potencial antimicrobiano utilizou-se o método de microdiluição em caldo dos inóculos de Pseudomonas aeruginosa, Escherichia coli e Bacillus subtilis. A fase hexânica foi a mais promissora contra B. subtilis (MIC50 de 8,87 &mu;g mL-1) com o sugiol e a horminona apresentando MIC50 de 190,92 e 81,97 &mu;M respectivamente. Os ensaios para a avaliação do potencial anti-HIV-1 foram realizados por meio de método colorimétrico com o uso do Kit Elisa RT (Roche), sendo a fase hidrometanólica a mais promissora, MIC50 de 230 &mu;g mL-1 / Hyptis Jacq. belongs to Lamiaceae, subtribe Hyptidinae, occurring in the semiarid tropical regions of America and Africa. Comprising 144 species, this genus has herbs or shrubs with squared stems and epipetals stamens. Hyptis is heavily fragrant and used in popular medicine for the treatment of gastrointestinal infections, cramps, pains, skins diseases, malaria and mosquito repellent. The aims of this research was evaluate the antioxidant, antimicrobial and anti-HIV potential of different extracts and phases produced with leaves and stems of H. lacustris, and performe their phytochemical screening. Fragments of stems and leaves were dried and powdered; the powder was macerated in ethanol 70% for seven days in room temperature. The ethanol extract was freeze-dried and part of it was partitioned with hexane, dichloromethane and ethyl acetate. Phases were chemical characterized by the use of gas chromatography coupled to a mass spectrometer (GC-MS) and high-performance liquid chromatography coupled to a diode array detector (HPLC-DAD). The obtained fractions were analyzed by nuclear magnetic resonance (NMR) of 1H, 13C and DEPT 135, and were identified three isolated abietane diterpenes: sugiol, horminone and 7-O-acetylhorminone. The abietane diterpenoids components of two fractions were identified by NMR and GC-MS techniques: one fraction as a mixture of fatty acids and 6,7-dehydroferruginol; and the other a mixture of fatty acids and two abietane diterpenoids (8&beta;-hydroxy-9(11),13-abietadien-12-one and ferruginol). The antioxidant potential was evaluated using the following methods: DPPH and ABTS radical scavenging, iron reducing ability, iron chelating activity and ORAC (Oxygen Radical Absorbance Capacity). The ethyl acetate phase was the most promising in all assays, EC50 of 16.73 &mu;g mL-1 for DPPH, 5.18 &mu;g mL-1 for ABTS, 44.75 &mu;g mL-1 for iron chelating, 5.35 &mu;g mL-1 for iron reducing and 0.81 &mu;g mL-1 for ORAC. For the antimicrobial assay was used the broth microdilution method with Pseudomonas aeruginosa, Escherichia coli and Bacillus subtilis. The hexane phase was the most promising against B. subtilis (MIC50 8.87 &mu;g mL-1). Sugiol and horminone also had activity against B. subtillis, MIC50 190.92 and 81.97 ?M, respectively. A colorimetric assay kit produced by Roche was used to evaluate the anti-HIV potential, being the hydromethanolic phase the most promising among all phases, presenting MIC50 of 230 &mu;g mL-1

A adesão ao tratamento antirretroviral por crianças e adolescentes com HIV/Aids / Antirretroviral treatment adherence of children and adolescents with HIV/Aids

Introdução: A manutenção da boa adesão aos medicamentos antirretrovirais é considerada fundamental para a terapia de pacientes com HIV/Aids, pois falhas no seguimento da prescrição podem levar ao comprometimento de toda a terapia, desenvolvimento de resistência viral e consequente redução nas opções de tratamento. Nas crianças, a adesão plena torna-se mais relevante se forem considerados os diferentes graus de maturidade do sistema imunológico no momento da infecção pelo HIV, tornando, assim, fundamental a avaliação da adesão nesta população. Objetivos: Analisar a taxa de adesão ao tratamento antirretroviral em crianças e adolescentes com HIV/Aids. Métodos: Este estudo transversal, aninhado à coorte de crianças e adolescentes com HIV/Aids atendidas no ambulatório do Instituto da Criança, analisou 101 pacientes. Os dados dos pacientes foram obtidos por meio de questionários sobre as características sociodemográficas e clínicas, sobre a aderência, além de escalas de expectativa de auto-eficácia. A taxa de adesão foi calculada classificando como aderentes aqueles pacientes que tomaram 95 por cento ou mais das doses referidas.  A análise descritiva (média, desvio-padrão, mediana e proporções) foi realizada para caracterizar a população do estudo e o teste de Wilcoxon foi utilizado para comparação das médias da escala de auto-eficácia dos pais/cuidadores e pacientes acima de 11 anos. Resultados: A taxa de adesão ao tratamento antirretroviral foi de 93,1 por cento (IC = [88,7;94,4]). Houve 7 pacientes que apresentaram má adesão. Os medicamentos antirretrovirais mais utilizados foram lopinavir (51,5 por cento ), lamivudina (43,6 por cento ) e Biovir® (associação de lamivudina e zidovudina) (28,7 por cento ). Conclusão: Alta taxa de adesão ao tratamento antirretroviral por crianças e adolescentes pode ser atingida, independentemente de seu perfil sociodemográfico e/ou 95 por cento de seus cuidadores, por meio do cuidado que a equipe multiproffisional dedica aos pacientes / Introduction: Maintaining good adherence to antiretroviral drugs is considered critical to drug therapy of patients with HIV/Aids, because prescription noncompliance can compromise the entire therapy, lead to the development of viral resistance and consequent reduction in treatment options. In children, full adherence becomes more relevant if different degrees of maturity of the immune system at the time of HIV infection are considered, thereby making the assessment of adherence required in this population. Objectives: To analyze adherence rate to antiretroviral treatment in children and adolescents with HIVAids. Methods: This cross-sectional study nested in a cohort of children and adolescents with HIVAids followed up at the Instituto da Criança, assessed 101 patients. Patient data were obtained by means of questionnaires on sociodemographic and clinical characteristics, adherence and self-efficacy expectancy scales. The adherence rate was calculated by classifying as adherents those patients who had taken 95 per cent or more of these doses. The descriptive analysis (mean, standard deviation, median and proportions) was performed to characterize the study population and the Wilcoxon test was used for the correlation between the scale of self-efficacy of parents/caregivers and patients over 11 years old. Results: The adherence rate to antiretroviral treatment was 93.1 per cent (CI = [88.7;94.4]). There were 7 patients who have poor adherence. Antiretroviral drugs frequently used were lopinavir (51.5 per cent ), lamivudine (43.6 per cent ) and Biovir 95 per cent TM (lamivudine plus zidovudine) (28.7 per cent ). Conclusions: High rate of adherence to antiretroviral treatment for children and adolescents can be met, regardless of their demographic profile and/or of their caregivers, through the care that the multiprofessional team dedicated to patients
