Etiological and molecular profile of pathogens causing clinical mastitis, and antimicrobial use in dairy herds / Perfil etiológico e molecular de patógenos causadores de mastite clínica, e uso de antimicrobianos em rebanhos leiteiros

The general objectives of this thesis were: (i) to determine the etiological and molecular profile of clinical mastitis (CM) in 20 dairy herds of Southeast, Brazil; and (ii) to quantify antimicrobial used for treatment of CM in the study population. To achieve this goals, four studies were performed. In the Study 1, we characterized the pathogen frequency and severity of CM in dairy herds. In addition, we determined the incidence rate of clinical mastitis (IRCM) and its association with the following herd-level descriptors: bulk milk somatic cell count (BMSCC), bulk milk total bacterial count (BMTBC), herd size (number of lactating cows), milk yield, housing system and season. The association between herd-level descriptors and IRCM were determined by two groups of mixed regression models: one based on the overall IRCM, and five based on the following specific-pathogen groups: contagious, other Gram-positive, Gram-negative, other (composed of yeast and Prototheca spp), and negative culture. A total of 5,957 quarter-cases of CM were recorded and the most frequently isolated pathogens were Escherichia coli (6.6% of total cultures), Streptococcus uberis (6.1%), and Streptococcus agalactiae (5.9%). The majority of CM cases were mild (60.3%), while 34.1% were moderate and 5.6% severe. Overall, the IRCM was 9.7 quarter-cases per 10,000 quarter-days at risk (QDAR), and the only herd-level parameter associated with overall IRCM was BMSCC, in which the highest IRCM was observed for herds with BMSCC >600.000 × 103 cells/mL. In the models evaluating the specific-pathogen groups, IRCM with isolation of major contagious pathogens was associated with BMSCC, milk yield and housing system. For the evaluation of other Gram-positive pathogens, the IRCM was higher in the rainy season of 2015 in comparison with the other seasonal categories. In addition, for the model evaluating the Gram-negative group, the IRCM was highest in herds with BMTBC >30 × 103 cfu/mL. The Study 2 aimed to characterize the treatment profile and quantify the antimicrobial consumption for treatment of CM in dairy herds; and to determine the association of antimicrobial use (AMU) and the same herd-level descriptors as described in the Study 1. Data on treatment practices and AMU were obtained from 19 dairy herds for a period of 12 months per herd. The AMU for treatment of CM was quantified monthly in units of defined daily dose (DDD) and expressed as antimicrobial treatment incidence (ATI; number of DDD per 1,000 lactating cows-day). The overall monthly mean ATI was 17.7 DDD per 1,000 lactating cow-days (15.4 for intramammary compounds, and 2.2 for systematically administered antimicrobials). Among intramammary drugs, aminoglycosides had the highest ATI (11.7 DDD per 1,000 lactating cow-days), while for systematically administrated antimicrobials, fluoroquinolones (4.2 DDD per 1,000 lactating cow-days) were the most frequently used antimicrobials. Herd size and BMSCC were positively associated with ATI. In addition, herd-level ATI was higher in freestall herds than in compost bedded-pack barns. In the Study 3, we determined the phylogeny of E. coli strains isolated from CM in dairy cows and the association of most frequent phylogroups with antimicrobial susceptibility. A total of 100 E. coli isolates recovered from CM cases described in the Study 1 were categorized according to their phylogenetic group using a quadruplex PCR method; antimicrobial susceptibility pattern was also evaluated. Most isolates were assigned to phylogenetic group A (52%), followed by B1 (38%), B2 (2%), C (4%), D (3%), and E (1%). Resistant isolates were observed for all evaluated antimicrobials. Overall, more than 96% of E. coli isolates were resistant to ampicillin, and more than 23% were resistant to cephalothin, sulphadimethoxine or tetracycline. High levels of resistance (>70%) were also found to erythromycin, oxacillin, penicillin, penicillin associated with novobiocin, and pirlimycin. In contrary, high susceptibility was observed to ceftiofur (96.8%) among E. coli isolates. Difference in the antimicrobial susceptibility among phylogenetic groups was observed only for cephalothin, in which E. coli strains belonging to the phylogroup A were inhibited at lower antimicrobial concentrations than strains assigned to the phylogroup B1. In Study 4, we evaluated the genotypic diversity among Strep. agalactiae and Strep. uberis isolates recovered from CM in dairy cows; in addition, the study evaluated the association of genotypes clustered by genetic similarity with antimicrobial susceptibility pattern. Isolates were subtyped using randomly amplified polymorphic DNA (RAPD) analysis. A great genotypic diversity was found for both Strep. agalactiae (45 subtypes out of 89 isolates) and Strep. uberis (56 subtypes out of 88 isolates). For evaluation of antimicrobial susceptibility, subtypes of Strep. agalactiae were clustered into three groups (Ia, Ib and II), while Strep. uberis subtypes were clustered into two groups (I and II) according to their genetic similarity. Overall, Strep. agalactiae isolates showed high susceptibility to most antimicrobials, except to tetracycline and erythromycin. Differences in the antimicrobial susceptibility among clusters of Strep. agalactiae were observed for ampicillin, ceftiofur, erythromycin, pirlimycin, sulphadimethoxine and tetracycline. In contrary, Strep. uberis isolates were categorized as resistant to most antimicrobials, except to cephalothin and penicillin+novobiocin. No differences were observed among clusters for all antimicrobials in the analysis of Strep. uberis. In conclusion, the results of this thesis indicated a high IRCM in the evaluated herds, and although environmental pathogens were the most common cause of CM in these herds, contagious pathogens such as Strep. agalactiae and Staph. aureus, are still a concern in some dairy herds of Brazil. Furthermore, high frequencies of AMU and off-label protocols were observed among the evaluated herds. The non-judicious use of antimicrobials can become a risk factor for the development of antimicrobial resistance, which was even observed for isolates belonging to the three most prevalent bacterial species identified from CM cases in our study (E. coli, Strep. agalactiae and Strep. uberis). Finally, because there were some herd-level descriptors associated with the IRCM and AMU in our study, there may be opportunity for management strategies aiming to improve the control of CM in dairy herds of southeastern Brazil. / Os objetivos gerais desta tese foram: (i) determinar o perfil etiológico e molecular da mastite clínica (MC) em 20 rebanhos leiteiros do Sudeste do Brasil; e, (ii) quantificar os antimicrobianos usados para tratamento da MC na população estudada. Para alcançar esses objetivos, quatro estudos foram realizados. No Estudo 1, foi caracterizada a frequência de patógenos causadores de MC e a gravidade das infecções nos rebanhos leiteiros. Além disso, foi determinada a taxa de incidência de mastite clínica (TIMC) e sua associação com as seguintes variáveis em nível de rebanho: contagem de células somáticas em leite de tanque (CCSLT), contagem bacteriana total em leite de tanque (CBTLT), tamanho (número de vacas em lactação), produção de leite, sistema de alojamento e estação do ano. A associação entre as variáveis em nível de rebanho e a TIMC foi determinada por dois grupos de modelos de regressão logística multivariada: um baseado na TIMC geral, e cinco baseados nos seguintes grupos específicos de patógenos: contagiosos, outros Gram-positivos, Gram-negativos, outros patógenos (composto de leveduras e Prototheca spp.), e cultura negativa. Um total de 5.957 casos de MC em nível de quarto mamário foi registrado e os patógenos mais prevalentes foram Escherichia coli (6,6% de todas as culturas), Streptococcus uberis (6,1%), e Streptococcus agalactiae (5,9%). A maioria dos casos de MC foi de gravidade leve (60,3%), enquanto 34,1% dos casos foram moderados e 5,6% foram graves. A TIMC geral foi de 9,7 casos por 10.000 quartos-dia em risco (QDR), e o único parâmetro em nível de rebanho associado com a TIMC geral foi a CCSLT, em que a TIMC mais alta foi observada em rebanhos com CCSLT >600.000 × 103 células/mL. Nos modelos que avaliaram os grupos específicos de patógenos, a TIMC de patógenos contagiosos foi associada com a CCSLT, produção de leite e sistema de alojamento. Na avaliação de outros patógenos Gram-positivos, a TIMC foi maior na estação chuvosa de 2015 em comparação com as outras categorias referentes à estação do ano. Adicionalmente, para o modelo avaliando o grupo de patógenos Gram-negativos, a TIMC foi mais alta em rebanhos com CBTLT >30.000 × 103 ufc/mL. O Estudo 2 teve como objetivo caracterizar o perfil de tratamento e o consumo de antimicrobianos em rebanhos leiteiros; e determinar a associação de uso de antimicrobianos (UAM) e as mesmas variáveis em nível de rebanho descritas no Estudo 1. Dados sobre as práticas terapêuticas e UAM foram obtidos de 19 rebanhos leiteiros durante um período de 12 meses por rebanho. A frequência de UAM para tratamento da MC foi quantificada mensalmente em unidades de doses definidas diárias (DDD) e expressa como incidência de tratamento antimicrobiano (ITA: número de DDD por 1.000 vacas em lactação-dia). A média de ITA mensal foi de 17,7 DDD por 1.000 vacas em lactação-dia (15,4 para compostos intramamários, e 2,2 para compostos sistêmicos). Entre os produtos intramamários, os aminoglicosídeos tiveram a ITA mais alta (11,7 DDD por 1.000 vacas em lactação-dia), enquanto que para os compostos administrados pela via sistêmica, as fluoroquinolonas (4,2 DDD por 1.000 vacas em lactação-dia) foram os antimicrobianos mais frequentemente usados. O tamanho do rebanho e CCSLT foram positivamente associados com a ITA. Além disso, a ITA foi mais alta em rebanhos com freestall do que em rebanhos com sistema tipo compost barn. No Estudo 3, determinou-se a filogenia de cepas de E. coli isoladas de casos de MC em vacas leiteiras, e a associação dos filogrupos mais frequentes com a susceptibilidade aos antimicrobianos. Um total de 100 isolados de E. coli identificados nos casos de MC descritos no Estudo 1 foram categorizados de acordo com os grupos filogenéticos por meio de um método de PCR quadruplex; o perfil de susceptibilidade aos antimicrobianos também foi avaliado. A maioria dos isolados pertenceram ao grupo A (52%), seguido dos grupos B1 (38%), B2 (2%), C (4%), D (3%), e E (1%). Foram encontrados isolados resistentes para todos os antimicrobianos avaliados. De forma geral, mais de 96% dos isolados de E. coli foram resistentes a ampicilina, e mais de 23% foram resistentes a cefalotina, sulfadimetoxina ou tetraciclina. Altos níveis de resistência (>70%) foram encontrados também para eritromicina, oxacilina, penicilina e penicilina associada a novobiocina. Ao contrário, foi observado alta susceptibilidade ao ceftiofur (96.8%) entre os isolados de E. coli. Diferenças na susceptibilidade entre os grupos filogenéticos foi observada apenas para a cefalotina, em que os isolados de E. coli pertencentes ao filogrupo A foram inibidos em concentrações de antimicrobianas mais baixas que isolados pertencentes ao filogrupo B1. No Estudo 4, avaliou-se a diversidade genotípica entre isolados de Strep. agalactiae e Strep. uberis identificados em casos de MC em vacas leiteiras; adicionalmente, o estudo avaliou a associação dos genótipos agrupados de acordo com a similaridade genética com o perfil de susceptibilidade aos antimicrobianos. Os isolados foram genotipados por meio do método de amplificação randômica de DNA polimórfico (RAPD). Grande diversidade genotípica foi observada tanto para o Strep. agalactiae (45 subtipos de 89 isolados) quanto para Strep. uberis (56 subtipos de 89 isolados). Para a avaliação de susceptibilidade aos antimicrobianos, os subtipos de Strep. agalactiae foram agrupados em três clusters (Ia, Ib e II), enquanto que os subtipos de Strep. uberis foram agrupados em dois clusters (I e II) de acordo com a similaridade genética. De forma geral, os isolados de Strep. agalactiae apresentaram alta susceptibilidade à maioria dos antimicrobianos, exceto para tetraciclina e eritromicina. Diferenças na susceptibilidade aos antimicrobianos entre os clusters de Strep. agalactiae foram observadas para ampicilina, ceftiofur, eritromicina, pirlimicina, sulfadimetoxina e tetraciclina. Por outro lado, os isolados de Strep. uberis foram resistentes à maioria dos antimicrobianos, exceto para cefalotina e penicilina + novobiocina. Não foram encontradas diferenças entre os clusters para todos os antimicrobianos na análise de Strep. uberis. Em conclusão, os resultados desta tese indicaram alta TIMC nos rebanhos avaliados, e apesar de os patógenos ambientais serem a causa mais comum de MC nestes rebanhos, patógenos contagiosos como Strep. agalactiae e Staph. aureus, ainda são uma preocupação em alguns rebanhos do Brasil. Além disso, observaram-se altas frequências de UAM e de terapias não recomendadas em bula entre os rebanhos avaliados. O uso não judicioso de antimicrobianos pode se tornar um fator de risco para o desenvolvimento da resistência bacteriana aos antimicrobianos, o que foi inclusive observado para isolados pertencentes as três espécies bacterianas mais prevalentes nos casos de MC no nosso estudo (E. coli, Strep. agalactiae e Strep. uberis). Finalmente, pelo fato de algumas variáveis em nível de rebanho terem sido associadas com a TIMC e com o UAM em nosso estudo, é possível que hajam oportunidades para implementação de estratégias de manejo com o objetivo de melhorar o controle da MC em rebanhos leiteiros do sudeste do Brasil.

Multiresistente Enterobakterien bei neugeborenen Milchviehkälbern in Sachsen

Einleitung: Das Auftreten von multiresistenten Bakterien in der Bevölkerung und in Kran-kenhäusern sowie in der Tierhaltung hat in den letzten Jahrzehnten stark zugenommen. Die weltweite Zunahme multiresistenter gramnegativer Bakterien, insbesondere Entero-bakterien wie Klebsiella (K.) pneumoniae und Escherichia (E.) coli, gibt Anlass zu wach-sender Besorgnis und ist Gegenstand zahlreicher Studien. Ziele der Untersuchungen: Im Rahmen der vorliegenden Studie sollte die Prävalenz von Extended-Spectrum-Beta-Lactamase (ESBL)-produzierenden Enterobakterien (ESBL-E) bei Milchkälbern untersucht und Risikofaktoren für deren Auftreten unter Verwendung von Daten zu Antibiotikaeinsatz, Betriebshygiene und Tiergesundheit identifiziert werden. Tiere, Material und Methoden: Zehn Betriebe mit einem Median von 781 Milchkühen (319-1701) nahmen an der Studie Teil. Die Betriebe wurden zweimal im Abstand von 7-11 Monaten besucht und Kotproben von jeweils 10 neugeborenen Kälbern gesammelt. Alle untersuchten Kälber waren jünger als zwei Wochen mit einem Durchschnittsalter von 6,8 (±3,9) Tagen. Die Kotproben wurden 1:10 verdünnt und im Doppelansatz auf Brilli-anceTM ESBL-Agar plattiert. Nach 24 Stunden bei 37 °C wurden die Kolonien gezählt und die Gesamtanzahl der koloniebildenden Einheiten (cfu)/ml berechnet. Die Bakterien-spezies wurde biochemisch identifiziert. Die ESBL-Produktion wurde mittels MICRONAUT-S β-Lactamase-Platten phänotypisch bestätigt. Zusätzlich wurden weitere Resistenztest mit der VITEK® 2 Technologie durchgeführt. Die Bestimmung der Phy-logruppen der E. coli-Isolate und das Screening auf bla-Gene wurde mittels PCR durch-geführt. Der Hygienestatus der Betriebe wurde mit Hilfe eines standardisierten Fragebo-gens erfasst und bewertet und Daten zu Tiergesundheit und Antibiotikaeinsatz wurden über Tier-Scoring und das Herdemanagementprogramm gesammelt. Ergebnisse: ESBL-E konnten in allen Betrieben und 96,5 % der Kotproben nachgewiesen werden. Der dominierende Anteil der ESBL-produzierenden Isolate waren E. coli (92,9 %), gefolgt von Enterobacter (E.) cloacae (5,1 %) und K. pneumoniae subsp. pneumoniae (2,0 %). Die Mehrheit der E. coli-Isolate wurde eindeutig der Phylogruppe C zugeordnet (25,0 %), gefolgt von den Phylogruppen A (15,2 %) und E (14,1 %). Die CTX-M-Gruppe 1 wurde am häufigsten nachgewiesen (80,4 %). Neben der Resistenz gegenüber Penicillinen und Cephalosporinen war die Mehrheit der Isolate zusätzlich ge-genüber einer oder mehrerer weiterer Substanzklassen resistent, wobei ein hoher Anteil gegenüber Fluorchinolonen resistent war. 52,5 % der Isolate wurden außerdem als drei-fach multiresistente gramnegative Bakterien (3MRGN) gemäß der Kommission für Kran-kenhaushygiene und Infektionsprävention charakterisiert. Keines der Isolate war 4MRGN, d.h. keines zeigte eine Carbapenem-Resistenz. Penicilline wurden bei Kälbern in den meisten Betrieben am häufigsten verabreicht und stellten auf Herdenebene in al-len Betrieben die vorherrschende Substanzklasse dar. Insgesamt war die Anzahl der Kälber, die vor der Probenahme behandelt wurden, eher gering (11,7 %). Analysen der Daten zum Betriebsmanagement ergaben Schwächen bei der Biosicherheit und der Rei-nigung und Desinfektion. Neben Beta-Laktam-Antibiotika als den am häufigsten verwen-deten Antibiotika konnten keine weiteren Risikofaktoren identifiziert werden. Schlussfolgerungen: Die Prävalenz von ESBL-E in unserer Studie war außergewöhnlich hoch. Obwohl die Prävalenz mit zunehmendem Alter der Rinder nachgewiesenermaßen abnimmt, sollten unsere Ergebnisse Anlass zur Entwicklung von Strategien sein, die den Eintrag von ESBL-E in das Kälberaufzuchtsystem frühzeitig verhindern. Dies können beispielsweise der verantwortungsbewusste Einsatz antibiotischer Trockensteller und ei-ne sorgfältige Hygiene in Abkalbeboxen und Kälberställen sein. The in-creasing occurrence of multidrug-resistant gram-negative bacteria, especially enterobac-teria such as Klebsiella (K.) pneumoniae and Escherichia (E.) coli, is of growing concern and has been subject to many studies worldwide. Study aims: We studied the prevalence of extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae in dairy calves as part of a routine health check protocol. In addition, data regarding antimicrobial use (AMU), farm hygiene, and farm manage-ment were collected in order to identify possible risks for ESBL occurrence. Animals, material and methods: Ten farms participated in the study with a median of 781 milking cows (319-1701). All calves investigated were younger than two weeks with an average age of 6.8 (±3.9) days. The farms were visited and samples were collected twice at an interval of 7-11 months. Faecal samples diluted 1:10, were plated onto Bril-lianceTM ESBL agar in duplicates. After 24 hours at 37 °C, colonies were counted and to-tal colony forming units (cfu)/ml calculated. Bacteria species were identified biochemical-ly. ESBL-production was phenotypically confirmed using the MICRONAUT-S β-Lactamases system. Additionally, antimicrobial susceptibility was tested using VITEK® 2 technology. Phylotyping of E. coli isolates and screening for bla genes was performed by PCR. The hygienic status of the farms was recorded and rated using a standardized questionnaire developed for dairy cattle and data on animal health and antimicrobial treatment were collected through animal scoring and the herd management program. Results: ESBL-producing enterobacteria were detected on all farms and 96.5 % of calves investigated shed ESBL-positive bacteria. Of all ESBL-producing isolates, the majority were E. coli (92.9 %), followed by Enterobacter cloacae (5.1 %) and Klebsiella pneu-moniae subsp. pneumoniae (2.0 %). The majority of E. coli isolates was clearly assigned to phylogroup C (25.0 %), followed by phylogroups A (15.2 %) and E (14.1 %). CTX-M group 1 was most frequently detected (80.4 %). Besides resistance to penicillins and cephalosporins, the majority of isolates was also resistant to one or more antibiotic clas-ses, with a high proportion being resistant against fluoroqinolones. 52.5 % of isolates were further characterised as threefold multidrug resistant gram-negative bacteria (3MDR-GNB) according to the German Commission for Hospital Hygiene and Infection Prevention. None of the isolates were 4MDR-GNB, i.e. none revealed carbapenem-resistance. Penicillins were the most frequently administered antibiotics to calves on most farms and were the predominant substance class at herd level on all farms. Over-all, the number of calves treated prior to sampling was rather low (11.7 %). Analyses of data regarding the farm management identified weaknesses in biosecurity and cleaning and disinfection. Besides beta-lactam antibiotics being the most commonly used antibiot-ics no other risk factors could be identified. Conclusions: The prevalence of ESBL-carriers in dairy calves in our study was exceptional-ly high. Although ESBL-E-prevalence has been described to decrease with increasing age of cattle, our findings should be motivation to develop strategies to prevent the entry of ESBL-E into the calf rearing system at an early stage such as prudent use of antimi-crobials during drying off and diligent hygiene in calving pens and calf housing. Le traitement sélectif (TS) des vaches laitières au tarissement (où seuls les quartiers ou les vaches infectées sont traités avec des antimicrobiens) constitue une alternative potentielle au traitement universel (TU, où tous les quartiers de toutes les vaches reçoivent des antimicrobiens, quel que soit leur statut infectieux), pour une utilisation plus judicieuse des antimicrobiens. L'objectif de cette thèse était d’apporter plus de lumière sur les décisions de traitement antimicrobien ciblant les quartiers ou vaches infecté(e)s au tarissement. Différents devis et méthodologies ont été utilisés pour répondre à cet objectif. Un essai contrôlé randomisé a été conçu et 569 vaches (2,251 quartiers) provenant de 9 troupeaux laitiers du Québec avec un comptage de cellules somatiques (CCS) du réservoir <250 000 cellules/mL ont été systématiquement enrôlées et réparties au hasard dans 4 groupes : 1) traitement antimicrobien seul pour tous les quartiers ; 2) traitement antimicrobien combiné avec un scellant interne à trayon pour tous les quartiers ; 3) traitement antimicrobien sélectif seul basé sur les résultats de la culture bactériologique du lait sur Petrifilm® ; et 4) traitement antimicrobien sélectif combiné avec un scellant interne à trayon basé sur les résultats de la culture du lait sur Petrifilm®. Dans les groupes de TS, les quartiers non infectés n'ont reçu qu'un scellant interne à trayon. Aucune différence significative n'a été détectée entre le TS par quartier et le TU des vaches laitières au tarissement, en termes d'élimination des infections intramammaires (IIM) et de prévention de nouvelles IIM pendant la période de tarissement, de risque d'un premier cas de mammite clinique (MC), de production laitière moyenne quotidienne et de CCS au cours des 120 premiers jours de la lactation suivante. Un TS reposant sur les résultats d'une culture de lait de quartier sur Petrifilm® au tarissement a permis de réduire l'utilisation d'antimicrobiens de 52% (IC à 95%: 39 – 64) par rapport à un TU. En plus de cet essai contrôlé randomisé, la culture du lait par quartier à l'aide de Petrifilm® a été comparée à l'historique du CCS par une estimation bayésienne de leur précision pour identifier les quartiers ou les vaches qui devraient être traités avec des antimicrobiens dans des protocoles de TS au tarissement. Compte tenu de la disponibilité des données de CCS, de la facilité d'utilisation du dernier test de CCS pré-tarissement et de la valeur prédictive négative élevée qui pourrait être obtenue, les producteurs pourraient envisager d'utiliser uniquement le dernier test de CCS pré-tarissement comme outil potentiel pour identifier les vaches qui devraient être traitées avec des antimicrobiens au tarissement. Le dernier test de CCS pré-tarissement peut être utilisé seul ou en combinaison avec la culture de lait par quartier sur Petrifilm® sur les vaches avec un CCS élevé pour identifier encore plus spécifiquement les quartiers qui doivent être traités. L'ajout d'une culture de lait par quartier à la ferme sur Petrifilm® pour les vaches identifiées comme infectées à l'aide des données du CCS améliorerait la précision du test (principalement la valeur prédictive positive) et réduirait davantage l'utilisation d'antimicrobiens. Également, une revue systématique et une série de méta-analyses ont été menées pour étudier l'efficacité du TS par rapport au TU, afin de guider les décideurs et les utilisateurs qui s'engagent dans une utilisation plus efficace et judicieuse des antimicrobiens au moment du tarissement. Treize articles représentant 12 essais contrôlés, randomisés ou non, étaient disponibles pour les analyses. Le TS a permis de réduire de 66% (IC à 95%: 49 – 80) l'utilisation d'antimicrobiens au moment du tarissement. Les résultats appuient fortement l'idée que le TS réduirait l'utilisation d'antimicrobiens au moment du tarissement, sans effet négatif sur la santé du pis ou la production laitière au cours des premiers mois de la lactation subséquente, si, et seulement si, les scellant internes à trayons sont utilisés pour les quartiers non traités avec des antimicrobiens. Enfin, le suivi de l'utilisation d'un scellant interne à trayon a été effectué pour déterminer la proportion de quartiers qui ont conservé le bouchon de scellant jusqu’à la première traite après le vêlage et la persistance de résidus de scellant dans le lait après le vêlage. Un bouchon de scellant était présent jusqu'à la première traite pour 83% des quartiers, et nous pourrions émettre l'hypothèse que la perte du bouchon s'est produite près du vêlage secondaire à la tétée ou pour une autre raison (ex., la pression hydrostatique du lait), étant donné que les associations observées entre la présence ou non d'un bouchon de scellant observable et les chances de nouvelles IIM étaient relativement faibles. Les résidus de scellant pouvaient être observés dans le lait jusqu'à 12 jours après le vêlage, quoique 75% des quartiers n’excrétaient plus de scellant au bout de 5 jours en lait. / Selective dry cow therapy (SDCT, in which only infected quarters or cows are treated with antimicrobials) represents an alternative to blanket dry cow therapy (BDCT, in which all quarters of all cows at dry off are treated with antimicrobials, regardless of their infection status), for a more judicious use of antimicrobials. The objective of this thesis was to shed more light on targeted antimicrobial treatment decisions of infected quarters or cows at dry-off. Different study designs and methodologies were used to meet this objective. A randomized controlled trial was designed and a total of 569 cows (2,251 quarters) from 9 dairy herds in Québec with bulk tank somatic cell count (SCC) <250,000 cells/mL were systematically enrolled and randomly allocated to 4 groups: 1) antimicrobial treatment alone of all quarters; 2) antimicrobial treatment combined with an internal teat sealant (ITS) of all quarters; 3) selective antimicrobial treatment alone based on milk bacteriological culture results on Petrifilm®; and 4) selective antimicrobial treatment combined with an ITS based on milk culture results on Petrifilm®. In the selective antimicrobial treatment groups, uninfected quarters received only an ITS. No significant differences were detected between quarter-based selective and blanket dry cow therapies, in terms of elimination of intramammary infections (IMI) and prevention of new IMI during the dry period, risk of a first case of clinical mastitis (CM), daily average milk yield and somatic cell count in the first 120 days of the subsequent lactation. A selective antimicrobial treatment relying on results of quarter milk culture using Petrifilm® at dry off enabled a reduction in antimicrobial use of 52% (95% CI: 39 – 64) as compared to blanket dry cow treatment. In addition to this randomized controlled trial, quarter milk culture using Petrifilm® was compared with SCC history through a Bayesian estimation of diagnostic accuracy to identify quarters or cows that should be treated with antimicrobials in selective treatment protocols at dry off. Considering the availability of SCC data, the easiness of using just the last Dairy Herd Improvement (DHI) test before dry off, and the high negative predictive value that could be achieved, producers may consider using just the last DHI test before dry off results as a potential tool to identify cows that should be treated with antimicrobials at dry off. The last SCC test before 7 dry off may be used alone or in combination with quarter-level on-farm Petrifilm® milk culture on high SCC cows to more specifically identify quarters that need to be treated. Adding quarter-level milk culture using Petrifilm® to cows identified as unhealthy using cow-level SCC data could improve the test accuracy (mainly the positive predictive value) and further reduced the use of antimicrobials. Also, a systematic review and a series of meta-analyses were conducted to investigate the efficacy of SDCT compared with BDCT, to guide decision-makers and users to engage in a more effective and judicious use of antimicrobials at dry-off. Thirteen articles representing 12 controlled trials, randomized or not, were available for analyses. SDCT reduced the use of antimicrobials at dry off by 66% (95% CI: 49 – 80). Evidences strongly support that SDCT would reduce the use of antimicrobials at dry off, without any detrimental effect on udder health or milk production during the first months of the subsequent lactation, if, and only if, ITS are used for healthy quarters untreated with antimicrobials. Finally, a follow up on the use of ITS was performed to determine the proportion of quarters that had retained the sealant plug until the first milking after calving and the persistence of ITS residues in milk after calving. A sealant plug was present at first milking after calving for 83% of the quarters, and we could hypothesize that the loss of the plug occurred closely around calving due to suckling or for another reason (e.g., milk hydrostatic pressure), since the observed associations between the presence or not of an observable sealant plug and the odds of new IMI were relatively small. The sealant residues could be observed in milk up to 12 days in milk, although 75% of the quarters had expelled the last ITS residues by 5 days in milk.
