Outpatient antibiotic use for acute respiratory tract infections: seasonal trends, and implications for care & quality

Acute respiratory tract infection (ARI) is the most common reason for outpatient medical visits in the United States, and frequently results in treatment with an antibiotic. Most ARIs have a viral etiology, thus antibiotic therapy will have little clinical benefit in these cases. It is estimated as much as one-half of all antibiotic utilization for ARI in outpatient settings is inappropriate. Importantly, this misuse is thought to be the primary driver of antibiotic resistance development among bacteria. Antibiotic resistant infections cause an estimated 2 million illnesses and 23,000 deaths annually in the United States, and associated costs exceed $30 billion. Despite our current understanding of relevant predictors of appropriate antibiotic use, less is known regarding seasonality. This dissertation explores effects of seasonality on antibiotic prescribing in three aspects: 1) seasonal variation in appropriateness of prescribing, and antibiotics prescribed among common ARIs; 2) specific predictors of observed seasonal variation in prescribing practices; and 3) implications of seasonality for guideline concordance & quality of care. Findings generated herein, could help inform interventions designed to promote more judicious use of antibiotics in healthcare.

Community acquired Acinetobacter baumannii in pediatric patients under 1 year old with a clinical diagnosis of whooping cough in Lima, Peru

Objective: This study aimed to determine the prevalence of A. baumannii in children aged less than 1 year admitted with a clinical diagnosis of whooping cough. Results: A total of 225 nasopharyngeal samples from children under 1 year old hospitalized with clinical diagnosis of whooping cough were studied from January 2010 to July 2012. The presence of A. baumannii was detected in 20.89% (47/225) of the nasopharyngeal swab samples. Among the 47 patients with A. baumannii: 5 were diagnosed with A. baumannii monoinfection, 17 co-infection with bacteria, 7 co-infection with virus and 18 co-infection with bacteria + virus. It was observed that 51.6% (116/225) were children between 29 days and 3 months old, this same group had the highest overall prevalence with 53.3%. The most common co-infecting pathogens were Bordetella pertussis in 55.3%, Adenovirus in 42.6% and Mycoplasma pneumoniae in 23.4%. / Revisión por pares

Negotiating gender identity and authority in the plays of Penina Muhando and Ari Katini Mwachofi

What are the visions of gender identity that emerge in contemporary Swahili women`s writing? How are gender relations negotiated? How are the attendant notions of `femininity´ and `masculinity´ defined? How does gender identity implicate issues of power, agency and authority? These and other questions I intend to discuss for three plays by Tanzanian and Kenyan women authors: Heshima Yangu (1974) and Nguzo Mama (1982) by Penina Muhando and Mama Ee (1987) by Ari Katini Mwachofi. The theoretical focus of my analyses is stimulated by the interdisciplinary dialogue between feminist theory, cultural studies, and narrative poetics on narrative identity, in particular literary configurations of gender identities and relations. This collaboration proceeds from the premise that any verbal or performative expression of identity already intertwines narrative and identity by representing an individual subjectivity, a `life story´ (see for example Lieblich & Josselson 1984). Narratives are a way `of making sense´ out of seemingly incoherent experiences, and even the lived life is a `storied life´(Ochberg 1984), a telling or performance of a story.

Analysen visar hur tre olika filmer representerar staden Tokyos stadsliv. Hur deras representation skiljer sig från varandra, och hur de är lika varandra. Två scener har valts ut från varje film. Fokus är att se vilken roll stadslivet spelar i filmerna men också för att analysera stadslivet. Bakgrundskaraktärerna har en central roll, hur de agerar och vilken mening de har i skildringen. Resultatet visar att bakgrundskaraktärerna är en väsentlig del av stadsbildningen, vilket i sin tur används som en spegling av huvudkaraktärernas känslor och mentala tillstånd.

Cooking fuels and children respiratory health: Evidence from Nigeria

Household air pollution (HAP) has been recognized as one of the lethal causes of millions of premature deaths every year, victimizing mainly children and women. Literature suggests that transition to modern cooking fuels such as electricity, biogas from the conventional ones, for instance, fuelwood, coal, can reduce HAP, thus minimize the likelihood of respiratory health problems among household members. This study explores whether cooking fuels has an impact on children's respiratory health in the context of Nigeria, and in particular, whether modern cooking fuels can be a solution to this problem. By using the children recode of the survey data collected by the Demographic and Health Survey (DHS) for the year 2018, this study finds support to the claim that modern cooking fuels can reduce the probability of Acute Respiratory Infections (ARI) symptoms among children aged below 5 years in Nigeria. This probability is also found contingent on the age, gender, and education of the household heads. However, with regards to individual fuel types, the result seems inconclusive in some instances, mostly due to a lack of observations. The policy implication is that to reduce ARI among children, households should be encouraged to adopt modern cooking fuels, and this should follow with increasing education and empowerment opportunities for women in the household.
