Dieta hiperlipídica com ou sem adição de sal modula diferentemente a produção de peptídeo natriurético atrial e a expressão de renina em camundongos / Diets rich in saturated fat and/or salt differentially modulate atrial natriuretic peptide and renin expression in C57Bl/6 mice

Fundação de Amparo à Pesquisa do Estado do Rio de Janeiro / Estudo dos efeitos de uma dieta rica em sal e / ou gordura saturada em grânulos de peptídeo natriurético atrial (ANP), hipertensão, expressão da renina e ultraestrutura cardíaca em camundongos C57Bl / 6. Camundongos machos adultos jovens foram separados em quatro grupos (n = 12) e alimentados com uma das seguintes dietas por 9 semanas: dieta padrão para roedores (Grupo P), dieta hiperlipídica (Grupo HL), dieta hipersódica (Grupo HS) e dieta hiperlipídica e hipersódica simultaneamente (HL-HS). Foram examinados: alterações no ANP sérico, ultra-estrutura dos cardiomiócitos produtores de ANP, estrutura do ventrículo esquerdo, pressão arterial sanguínea, expressão da renina no rim, taxa de filtração glomerular (TFG), eficiência alimentar, parâmetros lipídicos e glicídicos. Os animais alimentados com dieta hiperlipídica mostraram um pequeno aumento na produção de ANP, discreta hipertrofia ventricular esquerda, aumento da eficiência alimentar, dislipidemia e hiperglicemia. Animais alimentados com dieta hipersódica tiveram um grande aumento na produção de grânulos de ANP e correspondente elevação do seu nível sérico, hipertrofia ventricular esquerda, hipertensão arterial, diminuição dos níveis de renina e aumento da TFG. A combinação das duas dietas (HL-HS) teve um efeito aditivo. A ingestão de uma dieta com alto teor de sal e lipídeos induz alterações ultraestruturais dos cardiomiócitos, aumento da produção de ANP e elevação de seu nível sérico e reduz a quantidade de renina no rim. / To study the effects of a diet rich in salt and/or saturated fat on atrial natriuretic peptide (ANP)-granules, hypertension, renin expression and cardiac structure in C57Bl/6 mice. Young adult male mice were separated into four groups (n=12) and fed one of the following for 9 weeks: standard chow/normal-salt (SC-NS), high-fat chow/normal salt (HF-NS), standard chow/high-salt (SC-HS) and high-fat chow/high-salt (HF-HS). Alterations in the serum ANP, ultrastructural analysis of cardiomyocytes that produce ANP, structural analysis of the left ventricle, blood pressure, renin expression, glomerular filtration rate (GFR), feed efficiency and lipid and glucose parameters were examined. The HF-NS diet showed a small increase in ANP production and left ventricular hypertrophy, increased food efficiency and abnormal lipid and glucose parameters. The SC-HS diet showed a large increase in ANP granules in myocytes and corresponding elevation in ANP serum levels, left ventricular hypertrophy, hypertension, decrease of renin levels and increase in GFR. The combination of the two diets (HF-HS) had an additive effect. The incorporation of a high-fat high-salt diet induced ultrastructural changes in cardiomyocytes, increased the production of ANP and increased its serum level, and reduced the amount of renin in the kidney.

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / A distensÃo mecÃnica do Ãtrio direito (DA) aumenta a motilidade gÃstrica em ratos anestesiados (Palheta et al., 2010). Resolvemos avaliar o efeito da DA sobre o esvaziamento gÃstrico (EG) de lÃquido em ratos acordados e as eventuais vias neuro-humorais relacionadas ao fenÃmeno. Utilizamos ratos albinos machos (n=361, 250-280g) que receberam um balÃo de silicone posicionado no Ãtrio direito. Decorridos 24h, monitoramos a pressÃo venosa central (PVC), freqÃÃncia cardÃaca (FC) e a pressÃo arterial mÃdia (PAM) e apÃs os 20-min iniciais os animais foram aleatoriamente prÃ-tratados com: Salina (S, 0,1 ml/100g, i.v.), Atropina (A, 0,5 mg/kg, i.v.), Guanetidina (GT, 10 mg/kg, i.p.), HexametÃnio (H, 10mg/kg, i.v.), L-NAME (3mg/kg, i.v.), L-Arg (100mg/kg, i.v.) + L-NAME (3 mg/kg, i.v.), Azul de metileno (MB, 3 mg/kg, i.v.), Glibenclamida (GB, 1 mg/kg, i.p.) ou Glibenclamida + DiazÃxido (3mg/kg, i.v.) [GB + D], Dexametasona (DEX, 1mg/kg, i.p.), Anantin (ANT, 5 &#61549;g, i.v.) ou Atosibana (AT, 40 &#61549; g/kg/h, i.v.). AlÃm disto, em grupos separados realizamos 72h antes da DA Ã vagotomia (V), ou esplancnotomia+ gangliectomia celÃaca (SC) ou denervaÃÃo cardÃaca aferente com capsaicina (ACD). Em outro conjunto de animais realizamos aurilectomia direita uma semana antes da DA (AX). Em seguida ao tratamento farmacolÃgico realizamos protocolo de falsa DA (controle) ou DA com 50&#61549;L do balÃo intra-atrial durante 5min. Decorridos 20 min. da DA, os ratos foram alimentados por via oral com soluÃÃo teste e, apÃs 10-min sacrificados para estudo do EG. AlÃm disto, determinamos os nÃveis plasmÃticos de ocitocina (OT), PeptÃdeo NatriurÃtico Atrial (ANP) e corticosterona (CORT). Para verificaÃÃo da atividade neuronal avaliamos a expressÃo da proteÃna Fos e OT nas regiÃes hipotalÃmicas do nÃcleo paraventricular (PVN) ou supra-Ãptico (SON). Comparado ao controle, a DA diminuiu o EG (p <0,05). AlÃm disso, a DA aumentou a PVC e a FC. A DA diminuiu o EG (p<0,05) nos grupos S, A, GT, L-arginina + L-NAME, MB e GB+D. JÃ o prÃ-tratamento com H, L-NAME, GB, DEX, ANT ou AT, bem como a SV, SC, ACD ou AX preveniu o efeito do DA sobre o EG. AlÃm disso, a DA aumentou os nÃveis plasmÃticos de OT e CORT, mas nÃo alterou o de ANP. Apesar da DA aumentar o nÃmero de neurÃnios imunorreativos para c-fos nas regiÃes parvocellular medial e posterior do PVN, nÃo observamos tal achado nas regiÃes magnocellular do PVN ou do SON, tambÃm nÃo houve diferenÃa significativa para o nÃmero de neurÃnios imunorreativos para Fos-OT apÃs DA. Portanto a diminuiÃÃo do EG de lÃquidos apÃs a DA em ratos acordados depende de uma via aferente cardÃaca mediada por receptores de baixa pressÃo, sendo que tanto neurÃnios simpÃticos como parassimpÃticos participam da cascata mediada pela OT, ANP e NO atravÃs de canais para K+-ATP dependentes. / Right atrium mechanical stretch (AS) increases gastric motility in anesthetized rats. We aimed to study the effect of AS on gastric emptying (GE) in awake rats and the related neuro-humoral pathways ivolved. Male albino rats (N=361, 250-280g) had a silicone balloon inserted in the right atrium. After 24-h, the central venous pressure (CVP), heart rate (HR) and the mean arterial pressure (MAP) were monitored and after the initial 20-min, animals were randomly pre-treated with: saline (S, 0.1 ml/100g, i.v.), Atropine (A, 0.5mg/kg, i.v.), Guanethidine (GT, 10mg/kg, i.p.), hexamethonium (H, 10mg/kg, i.v.), L-NAME (3mg/kg, i.v.), L-Arginine (100mg/kg, i.v.)+L-NAME (3mg/kg, i.v.), Methylene Blue (MB, 3mg/kg, i.v.), Glibenclamide (GB, 1mg/kg, i.p) or Glibenclamide+Diazoxide (3mg/kg, i.v.) [GB+D], Dexamethazone (DEX, 1mg/kg, i.p.), Anantin (ANT, 5&#61549;g, i.v.) or Atosiban (AT, 40&#61549;g/kg/h, i.v.). Besides, in a separate group, we realized vagotomy (V), or splanchnotomy + celiac gangliectomy (SC) or afferent cardiac denervation with capsaicin (ACD) 72h before AS. In another set of animals, we realized right appendectomy (AX) one week before AS. Next, AS with saline zero (sham) or 50&#61549;L was performed during 5min. In this group, rats were gavage fed with a test meal 20-min after AS and euthanized 10-min afterwards to study GE. Moreover we determined plasmatic levels of Ocytocin (OT), Atrial Natriuretic Peptide (ANP) and Corticosterone (CORT), and determined the neuronal activity in the paraventricular (PVN) or supraoptic (SON) hypothalamic regions by measuring expressed double-labeled c-fos-OT. Comparing to Sham, AS decreased GE (p<0.05). Besides, AS increased CVP and HR. AS decreased GE (p <0.05) in S, A, GT, L-Arginine+L-NAME, MB and GB+D groups. However pre-treatment with, H, L-NAME, GB, DEX, ANT or AT, as well as SV, SC, ACD or AX prevented the effect of AS on GE. AS increased OT and CORT plasmatic levels, but did not alter ANP. In spite of AS increasing the number of c-fos expressing neurons in the parvocellular region, we did not observe this finding in the magnocellular regions of PVN or SON. Besides, AS did not alter the number of fos-OT double-labeled neurons. Therefore the decrease of GE of fluids after AS in awake rats depends on an afferent pathway mediated by cardiac low pressure receptors, and both sympathetic and parasympathetic neurons participate in the cascade mediated by OT, ANP and NO through K +-ATP dependent channels.

Impacto da terapia hormonal com baixa dose oral ou não oral sobre fatores de risco cardiovascular na menopausa

Durante a transição menopausal e a pós-menopausa cerca de 75% das mulheres apresentam sintomas de hipoestrogenismo, tais como fogachos. O emprego de terapia hormonal (TH) para alívio dos sintomas da menopausa está bem estabelecido, mas seus efeitos cardiovasculares (CV) permanecem controversos. Dados de estudos recentes indicam a presença de duas populações distintas quanto aos efeitos CV da TH. Essa diferenciação estaria relacionada principalmente com a idade e o tempo de pós-menopausa. Evidências sugerem também que a presença de fatores de risco cardiovascular antes do início do TH, ou de uma associação de fatores de risco, podem ser determinantes dos efeitos CV do TH. Dose de medicação, via de administração e o tipo de progestogênio utilizado em associação com estrogênio para TH também vem sendo estudados como possíveis fatores relacionados ao impacto CV do TH. O presente trabalho é composto por: 1) Ensaio clínico randomizado, comparando os efeitos da via oral baixa dose e via não oral sobre variáveis relacionadas com risco CV em uma população de mulheres saudáveis na pósmenopausa recente; 2) Ensaio clínico randomizado, onde foram avaliados os efeitos da adição de progesterona natural micronizada ao estrogênio não oral durante TH em mulheres na pós-menopausa recente; e 3) Revisão sistemática e meta-análise, onde foram sistematicamente buscados todos os artigos com TH baixa dose que avaliassem os efeitos desta terapia sobre variáveis relacionadas com risco cardiovascular: peso, índice de massa corporal, pressão arterial, proteína C reativa e lipídios. Desenvolvemos ensaio clínico randomizado, cross-over, com objetivo de avaliar os efeitos de dois tipos de tratamento hormonal na menopausa: oral baixa dose, estradiol 1 mg e drospirenona 2 mg diário e não oral, estradiol 17 β gel 1.5 mg (ou nasal 300 mcg) diário e progesterona micronizada vaginal, 200 mg, 14 dias por mês, sobre peptídeo natriurético atrial, variáveis relacionadas com inflamação e função endotelial, perfil antropométrico e metabólico em mulheres na pós-menopausa recente e sem doença clínica evidente. 101 mulheres na pós-menopausa foram alocadas aleatoriamente para iniciar o TH por um dos dois grupos de tratamento: via oral baixa dose (n=50) ou via não oral (n=51). Todas as pacientes utilizaram ambos os TH de forma seqüencial. Após o primeiro período de 2 a 3 meses de TH a paciente passava para o segundo tratamento, sem período de washout. A avaliação laboratorial foi realizada antes e após cada um dos tratamentos. A amostra do estudo foi composta por mulheres com média etária de 51 ± 3 anos e tempo de amenorréia de 22 ± 10 meses. Oitenta e seis pacientes concluíram o estudo. Peso e índice de massa corporal não se modificaram, enquanto que a circunferência da cintura reduziu de forma similar em ambos os grupos de tratamento. Colesterol total e LDL-C reduziram após ambos os TH, e triglicerídeos reduziram somente após a TH não oral. Insulina e glicemia de jejum não se modificaram. Não foram observadas modificações nos níveis de fibrinogênio, fator von Willebrand (FvW) e proteína C reativa (PCR) após TH oral. Após TH não oral, observou-se redução significativa de fibrinogênio e FvW. Níveis de PCR não se modificaram. Houve redução do número de pacientes no maior tertil de PCR (alto risco CV) após TH não oral. Essas pacientes passaram a integrar os grupos de risco intermediário e baixo. Níveis de peptídeo natriurético atrial (PNA) mantiveram-se inalterados após os ambos os TH. Não houve modificações significativas na pressão arterial e esta não se correlacionou com valores de PNA. Realizamos análise adicional do TH não oral, quanto às diferenças entre a via nasal e a percutânea e quanto aos efeitos da adição de progesterona natural micronizada ao estrogênio. Não houve diferenças significativas para todas as variáveis estudadas entre a via nasal e a via percutânea. A adição de progesterona natural micronizada não modificou os efeitos metabólicos e CV do estrogênio não oral. Foi realizada busca sistemática de todos os artigos que incluíssem como TH estrogênio baixa dose e avaliassem os efeitos deste tratamento sobre as variáveis de interesse: peso, índice de massa corporal, pressão arterial, proteína C reativa e lipídeos. Foram consultadas as bases MEDLINE, Cochrane CENTRAL, EMBASE. Foram revisadas todas as referências dos artigos de interesse e revisões e metaanálises no assunto, em busca de artigos relevantes. Após exclusão dos artigos em duplicata, 8610 artigos foram revisados. Destes, 28 artigos foram selecionados para meta-análise. Desta análise foi possível concluir que pacientes em uso de TH baixa dose apresentaram em média menor peso corporal, colesterol total e LDL-C do que não usuárias. A TH baixa dose não apresentou efeitos deletérios sobre demais variáveis estudadas. Em conclusão, ambos os TH apresentaram efeitos neutros ou benéficos sobre variáveis relacionadas com risco CV em uma população de mulheres na pósmenopausa recente e sem evidência de doença CV. A adição de progesterona natural micronizada não modificou os efeitos do estrogênio não oral. Os resultados da metaanálise sobre TH baixa dose e variáveis relacionadas com risco CV também permitem concluir que a TH baixa dose não exerceu efeitos deletérios sobre lipídeos e pressão arterial, e foi observado um possível efeito benéfico deste tratamento sobre o peso corporal. / During the menopausal transition and postmenopause about 75% of women have symptoms of hypoestrogenism symptoms such as hot flushes. The use of hormone therapy (HT) for relief of menopausal symptoms is well established, but its cardiovascular effects (CV) remain controversial. Data from more recent studies suggest the presence of two distinct populations regarding the cardiovascular effects of HT. This differentiation is related mainly to age and time after menopause. Evidence also suggests that the presence of cardiovascular risk factors before the onset of HT, or a combination of risk factors may be determinants of CV effects of HT. Medication dose, route of administration and type of progestin used in combination with estrogen for HT has also been studied as possible factors related to the CV impact of HT. This work consists of: 1) Randomized clinical trial, comparing the effects of low dose oral and non-oral route of variables related to CV risk in a population of healthy women in early postmenopausal; 2) A randomized clinical trial, which we assessed the effects of the addition of natural micronized progesterone to non-oral estrogen for HT in women in early postmenopausal; and 3) systematic review and meta-analysis, which were systematically searched all items with low-dose HT to assess the effects of this therapy on variables related to cardiovascular risk: weight, body mass index, blood pressure, C-reactive protein and lipids. A cross-over, randomized clinical trial was designed in order to evaluate the effects of two types of HT: low dose oral treatment, estradiol oral 1 mg and drospirenone 2 mg, by day and non-oral treatment, estradiol 1.5 mg 17 β gel by percutaneous route (or nasal route 300 mcg) by day and vaginal micronized progesterone, 200 mg/d, 14 days by month on atrial natriuretic peptide, variables associated with inflammation and endothelial function, anthropometric and metabolic variables on early and healthy postmenopausal women. One hundred one women were randomly allocated to start with one of the treatments: low dose oral treatment (n=50) or non-oral treatment (n=51). At the end the first three months period, the patients were crossed over without washout for an additional three months. Laboratory evaluated were carried before and after oral and non-oral HT. The sample of the study included postmenopausal women with a mean age of 51 years and duration of amenorrhea of 22±10 months. Eighty-six patients completed the study. Weight and body mass index remained unchanged, while the waist circumference decreased similarly in both treatment groups. Total cholesterol and LDL-cholesterol reduced after both the HT and triglycerides reduced only after nonoral HT. Insulin and fasting glucose did not change. No changes were observed in the levels of fibrinogen, von Willebrand factor (vWF) and C-reactive protein (CRP) after oral HT. After non-oral HT, there was a significant reduction of fibrinogen and vWF. CRP levels did not change. There was a reduction in the number of patients in the highest tertile of CRP (high CV risk) after non-oral HT. These patients have joined the groups of intermediate and low risk. Levels of atrial natriuretic peptide, ANP, were unchanged after both HT. There were no significant changes on blood pressure and did not correlate with values of ANP. We performed additional analysis of nonoral HT, for the differences between nasal and percutaneous and about the effects of addition of natural micronized progesterone to estrogen. There were no significant differences for all the variables studied between the nasal and percutaneously. The addition of micronized natural progesterone did not modify the metabolic and CV effects of non-oral estrogen. Systematic search of all articles that include as TH low dose estrogen and evaluate the effects of this treatment on the variables of interest was taken: weight, body mass index, blood pressure, C-reactive protein and lipids. The MEDLINE, Cochrane CENTRAL, EMBASE databases were consulted. All references of interest and reviews and meta-analyzes on the subject, in search of relevant articles were reviewed. After removing duplicate articles, 8610 articles were reviewed. Of these, 28 articles were selected for meta-analysis. From this analysis it was concluded that patients using low-dose TH had on average lower body weight, total cholesterol and LDL-C than non-users. The TH low dose showed no deleterious effects on other variables. In conclusion, low-dose oral and non-oral treatments had neutral or beneficial effects on variables related to CV risk in a population of women in early post menopausal and without evidence of CV disease. The addition of micronized natural progesterone did not modify the effects of non-oral estrogen. The results of the metaanalysis of low dose and TH variables related CV risk also showed that the TH low dose did not exert deleterious effects on lipids and blood pressure, and a possible beneficial effect of this treatment on body weight was observed.

Desta análise foi possível concluir que pacientes em uso de TH baixa dose apresentaram em média menor peso corporal, colesterol total e LDL-C do que não usuárias. A TH baixa dose não apresentou efeitos deletérios sobre demais variáveis estudadas. Em conclusão, ambos os TH apresentaram efeitos neutros ou benéficos sobre variáveis relacionadas com risco CV em uma população de mulheres na pósmenopausa recente e sem evidência de doença CV. A adição de progesterona natural micronizada não modificou os efeitos do estrogênio não oral. Os resultados da metaanálise sobre TH baixa dose e variáveis relacionadas com risco CV também permitem concluir que a TH baixa dose não exerceu efeitos deletérios sobre lipídeos e pressão arterial, e foi observado um possível efeito benéfico deste tratamento sobre o peso corporal. / During the menopausal transition and postmenopause about 75% of women have symptoms of hypoestrogenism symptoms such as hot flushes. The use of hormone therapy (HT) for relief of menopausal symptoms is well established, but its cardiovascular effects (CV) remain controversial. Data from more recent studies suggest the presence of two distinct populations regarding the cardiovascular effects of HT. This differentiation is related mainly to age and time after menopause. Evidence also suggests that the presence of cardiovascular risk factors before the onset of HT, or a combination of risk factors may be determinants of CV effects of HT. Medication dose, route of administration and type of progestin used in combination with estrogen for HT has also been studied as possible factors related to the CV impact of HT. This work consists of: 1) Randomized clinical trial, comparing the effects of low dose oral and non-oral route of variables related to CV risk in a population of healthy women in early postmenopausal; 2) A randomized clinical trial, which we assessed the effects of the addition of natural micronized progesterone to non-oral estrogen for HT in women in early postmenopausal; and 3) systematic review and meta-analysis, which were systematically searched all items with low-dose HT to assess the effects of this therapy on variables related to cardiovascular risk: weight, body mass index, blood pressure, C-reactive protein and lipids. A cross-over, randomized clinical trial was designed in order to evaluate the effects of two types of HT: low dose oral treatment, estradiol oral 1 mg and drospirenone 2 mg, by day and non-oral treatment, estradiol 1.5 mg 17 β gel by percutaneous route (or nasal route 300 mcg) by day and vaginal micronized progesterone, 200 mg/d, 14 days by month on atrial natriuretic peptide, variables associated with inflammation and endothelial function, anthropometric and metabolic variables on early and healthy postmenopausal women. One hundred one women were randomly allocated to start with one of the treatments: low dose oral treatment (n=50) or non-oral treatment (n=51). At the end the first three months period, the patients were crossed over without washout for an additional three months. Laboratory evaluated were carried before and after oral and non-oral HT. The sample of the study included postmenopausal women with a mean age of 51 years and duration of amenorrhea of 22±10 months. Eighty-six patients completed the study. Weight and body mass index remained unchanged, while the waist circumference decreased similarly in both treatment groups. Total cholesterol and LDL-cholesterol reduced after both the HT and triglycerides reduced only after nonoral HT. Insulin and fasting glucose did not change. No changes were observed in the levels of fibrinogen, von Willebrand factor (vWF) and C-reactive protein (CRP) after oral HT. After non-oral HT, there was a significant reduction of fibrinogen and vWF. CRP levels did not change. There was a reduction in the number of patients in the highest tertile of CRP (high CV risk) after non-oral HT. These patients have joined the groups of intermediate and low risk. Levels of atrial natriuretic peptide, ANP, were unchanged after both HT. There were no significant changes on blood pressure and did not correlate with values of ANP. We performed additional analysis of nonoral HT, for the differences between nasal and percutaneous and about the effects of addition of natural micronized progesterone to estrogen. There were no significant differences for all the variables studied between the nasal and percutaneously. The addition of micronized natural progesterone did not modify the metabolic and CV effects of non-oral estrogen. Systematic search of all articles that include as TH low dose estrogen and evaluate the effects of this treatment on the variables of interest was taken: weight, body mass index, blood pressure, C-reactive protein and lipids. The MEDLINE, Cochrane CENTRAL, EMBASE databases were consulted. All references of interest and reviews and meta-analyzes on the subject, in search of relevant articles were reviewed. After removing duplicate articles, 8610 articles were reviewed. Of these, 28 articles were selected for meta-analysis. From this analysis it was concluded that patients using low-dose TH had on average lower body weight, total cholesterol and LDL-C than non-users. The TH low dose showed no deleterious effects on other variables. In conclusion, low-dose oral and non-oral treatments had neutral or beneficial effects on variables related to CV risk in a population of women in early post menopausal and without evidence of CV disease. Impacto da terapia hormonal com baixa dose oral ou não oral sobre fatores de risco cardiovascular na menopausa

Durante a transição menopausal e a pós-menopausa cerca de 75% das mulheres apresentam sintomas de hipoestrogenismo, tais como fogachos. O emprego de terapia hormonal (TH) para alívio dos sintomas da menopausa está bem estabelecido, mas seus efeitos cardiovasculares (CV) permanecem controversos. Dados de estudos recentes indicam a presença de duas populações distintas quanto aos efeitos CV da TH. Essa diferenciação estaria relacionada principalmente com a idade e o tempo de pós-menopausa. Evidências sugerem também que a presença de fatores de risco cardiovascular antes do início do TH, ou de uma associação de fatores de risco, podem ser determinantes dos efeitos CV do TH. Dose de medicação, via de administração e o tipo de progestogênio utilizado em associação com estrogênio para TH também vem sendo estudados como possíveis fatores relacionados ao impacto CV do TH. O presente trabalho é composto por: 1) Ensaio clínico randomizado, comparando os efeitos da via oral baixa dose e via não oral sobre variáveis relacionadas com risco CV em uma população de mulheres saudáveis na pósmenopausa recente; 2) Ensaio clínico randomizado, onde foram avaliados os efeitos da adição de progesterona natural micronizada ao estrogênio não oral durante TH em mulheres na pós-menopausa recente; e 3) Revisão sistemática e meta-análise, onde foram sistematicamente buscados todos os artigos com TH baixa dose que avaliassem os efeitos desta terapia sobre variáveis relacionadas com risco cardiovascular: peso, índice de massa corporal, pressão arterial, proteína C reativa e lipídios. Desenvolvemos ensaio clínico randomizado, cross-over, com objetivo de avaliar os efeitos de dois tipos de tratamento hormonal na menopausa: oral baixa dose, estradiol 1 mg e drospirenona 2 mg diário e não oral, estradiol 17 β gel 1.5 mg (ou nasal 300 mcg) diário e progesterona micronizada vaginal, 200 mg, 14 dias por mês, sobre peptídeo natriurético atrial, variáveis relacionadas com inflamação e função endotelial, perfil antropométrico e metabólico em mulheres na pós-menopausa recente e sem doença clínica evidente. 101 mulheres na pós-menopausa foram alocadas aleatoriamente para iniciar o TH por um dos dois grupos de tratamento: via oral baixa dose (n=50) ou via não oral (n=51). Todas as pacientes utilizaram ambos os TH de forma seqüencial. Após o primeiro período de 2 a 3 meses de TH a paciente passava para o segundo tratamento, sem período de washout. A avaliação laboratorial foi realizada antes e após cada um dos tratamentos. A amostra do estudo foi composta por mulheres com média etária de 51 ± 3 anos e tempo de amenorréia de 22 ± 10 meses. Oitenta e seis pacientes concluíram o estudo. Peso e índice de massa corporal não se modificaram, enquanto que a circunferência da cintura reduziu de forma similar em ambos os grupos de tratamento. Colesterol total e LDL-C reduziram após ambos os TH, e triglicerídeos reduziram somente após a TH não oral. Insulina e glicemia de jejum não se modificaram. Não foram observadas modificações nos níveis de fibrinogênio, fator von Willebrand (FvW) e proteína C reativa (PCR) após TH oral. Após TH não oral, observou-se redução significativa de fibrinogênio e FvW. Níveis de PCR não se modificaram. Houve redução do número de pacientes no maior tertil de PCR (alto risco CV) após TH não oral. Essas pacientes passaram a integrar os grupos de risco intermediário e baixo. Níveis de peptídeo natriurético atrial (PNA) mantiveram-se inalterados após os ambos os TH. Não houve modificações significativas na pressão arterial e esta não se correlacionou com valores de PNA. Realizamos análise adicional do TH não oral, quanto às diferenças entre a via nasal e a percutânea e quanto aos efeitos da adição de progesterona natural micronizada ao estrogênio. Não houve diferenças significativas para todas as variáveis estudadas entre a via nasal e a via percutânea. A adição de progesterona natural micronizada não modificou os efeitos metabólicos e CV do estrogênio não oral. Foi realizada busca sistemática de todos os artigos que incluíssem como TH estrogênio baixa dose e avaliassem os efeitos deste tratamento sobre as variáveis de interesse: peso, índice de massa corporal, pressão arterial, proteína C reativa e lipídeos. Foram consultadas as bases MEDLINE, Cochrane CENTRAL, EMBASE. Foram revisadas todas as referências dos artigos de interesse e revisões e metaanálises no assunto, em busca de artigos relevantes. Após exclusão dos artigos em duplicata, 8610 artigos foram revisados. Destes, 28 artigos foram selecionados para meta-análise. Biomarqueurs des états septiques sévères : vers de nouvelles stratégies thérapeutiques individualisées / Biomarkers in severe sepsis : toward new individualized therapeutic strategies

En dépit de nombreux essais thérapeutiques, les syndromes septiques sont la première cause de mortalité en service de soins intensifs. La population septique étant très hétérogène, une meilleure caractérisation des patients serait essentielle afin de mieux individualiser et cibler les thérapeutiques potentiellement bénéfiques. Une approche multiparamétrique de l’utilisation des biomarqueurs est une alternative qui viserait à appréhender la situation de manière plus globale. Notre travail s’inscrit dans ce contexte au travers de l’étude plus spécifique de la défaillance des systèmes cardio-vasculaire et immunitaire. Au-delà de la confirmation de l’intérêt des biomarqueurs présentement étudiés (prohormones cardio-vasculaires et PD-1) dans la prédiction de la mortalité et du risque d’infections nosocomiales, nos résultats apportent des éléments nouveaux. Nous avons montré que (1) la sur-expression des molécules PD-1 est associée à l’énergie leucocytaire, (2) un même biomarqueur peut apporter une information différente au cours du temps, (3) l’information apportée par l’analyse simultanée de deux biomarqueurs est supérieure à celle de la somme de leurs valeurs individuelles, et (4) l’expression dynamique d’un biomarqueur est meilleure que son expression à un temps donné. Au total, notre travail illustre l’intérêt potentiel d’un panel de biomarqueurs pour mieux appréhender la complexité des états septiques et leur rapide évolution. Il reste néanmoins à développer des outils biostatistiques capables de donner au clinicien une vision globale en temps réel des processus en cours. Cela constituera une étape clé pour mieux stratifier et cibler les prochains essais cliniques dans le domaine. / Septic syndromes remain the leading cause of death in the intensive care units despite numerous clinical trials. Septic patients constitute a very heterogeneous population. Therefore improved characterisation of patients in order to better target and personalize potential new therapeutics is highly desirable. A multiparametric biomarker-based approach could be an attractive alternative to obtain a global view of the pathophysiologic situation. In this context, we worked specifically on cardio-vascular and immune dysfunctions. We first confirmed the predictive value of biomarkers for mortality or nosocomial infections, and showed new elements. We observed that (1) PD-1 overexpression is associated with leukocyte anergy, (2) one biomarker could give different information over time, (3) information provided by the association of two biomarkers is more interesting than the addition of their individual values, and (4) dynamic expression of one biomarker is more informative than its expression at a given time point. Finally, our results illustrate the potential interest of biomarker panels to improve our understanding of the septic syndrome complexity and to reflect their rapid evolution. Consequently, next step will depend on our capacity to develop biostatistic tools that enable clinicians to get, in real time, a global view of the process over time. This key step is likely necessary to decrease the heterogeneity of septic patient population in order to better stratify and target next clinical trials in the field.

Die Regulation der ANP-Freisetzung bei Herzinsuffizienz

HINTERGRUND: Im Zustand der Herzinsuffizienz sind die Plasmaspiegel des atrialen natriuretischen Peptids (ANP) erhöht. ANP wird durch Herzmuskelzellen bei atrialer (und dadurch kardiomyozytärer) Dehnung freigesetzt. Eine Reihe von Plasmafaktoren, welche bei der Herzinsuffizienz aktiviert sind, bewirken ebenfalls eine ANP-Freisetzung durch die Interaktion mit G(alpha-q)-gekoppelten Rezeptoren und der daraus resultierenden Aktivierung der Proteinkinase C (PKC). Ziel der vorliegenden Studie war es, im Vergleich von normalen und insuffizienten Rattenherzen die dehnungsinduzierte ANP-Sekretion sowie die Rolle der PKC bei der basalen ANP-Freisetzung zu analysieren. METHODIK: Durch Anlegen eines infrarenalen aortokavalen Shunts (4 Wochen) wurde eine volumeninduzierte Herzinsuffizienz in Wistar Ratten erzeugt. Die ANP-Freisetzung wurde am isoliert, retrograd perfundierten Herzmodell analysiert. Durch Umstellen auf anterograde Perfusion (Perfusionsdruck: 10mmHg) wurde eine atriale De! hnung induziert. Die Aktivierung der PKC fand durch Zugabe von PMA (Phorbol-12-Myristat-13-Azetat) zum Perfusat im Vergleich zu Vehikel statt. Alle Versuchsreihen wurden mit Herzen von shunt- sowie scheinoperierten Tieren durchgeführt. ERGEBNISSE UND DISKUSSION: Nach Shuntoperation war die ANP-Basalfreisetzung deutlich gesteigert und könnte die erhöhten ANP-Plasmaspiegel bei Patienten mit Herzinsuffizienz erklären. Dagegen wurde durch atriale Dehnung (als adäquaten Freisetzungsreiz am gesunden Herzen) keine weitere Steigerung der ANP-Freisetzung in der Shuntgruppe bewirkt. Die ANP-Freisetzung am insuffizienten Herzen ist im Vergleich zur normalen Herzfunktion somit verändert. Unter PKC-Stimulation sank die Freisetzung von ANP in der Shuntgruppe erheblich ab, wogegen es in der Kontrollgruppe zu keiner signifikanten Änderung der ANP-Sekretion kam. Damit wurde der Verdacht erhärtet, dass die Proteinkinase C in die abweichende Regulation der ANP-Freisetzung bei Herzinsuffizie! nz involviert ist. / BJECTIVE: Plasma levels of atrial natriuretic peptide (ANP) are markedly increased in congestive heart failure. ANP has been shown to be released by cardiomyocytes during atrial (and thereby cardiomyocytical) stretch. Several factors that are activated in heart failure as well enhance ANP release through the interaction with G(alpha-q)-coupled receptors and the consequent activation of proteinkinase C (PKC). The goal of this study was to analyse stretch induced ANP secretion and to investigate the involvement of PKC in the regulation of ANP release in heart failure compared to normal hearts. METHODS: Volume overload induced heart failure was produced by an infrarenal aortocaval shunt (4 weeks) in Wistar rats. ANP release was analysed in an isolated retrogradly perfused heart preparation. Atrial stretch was induced by switching to anterograd perfusion (perfusion pressure: 10mmHg). For PKC activation PMA (phorbol 12 myristate 13 acetate) was added to the perfusate and compar! ed to vehicle treatment. All experiments were performed with shunt and sham operated rats. RESULTS AND CONCLUSIONS: After shunt operation ANP baseline release was considerably augmented and could thus explain elevated ANP plasma levels in heart failure patients. In contrast, atrial stretch as an adequate secretion stimulus in control hearts did not further enhance ANP release in hearts of shunt operated animals. The ANP secretion in heart failure therefore seems to differ from the ANP secretion in the healthy state. Stimulation of PKC markedly decreased ANP release in the shunt group, whereas it did not have any significant effect on ANP release in the control group. In conclusion, the role of the proteinkinase C in ANP release differs between normal and failing hearts and seems to be involved in the deviating regulation of ANP release in states of heart failure.

GREBP, un nouveau facteur de transcription contrôlant l’expression de la guanylate cyclase A, récepteur de l’ANP, via l’élément de réponse au cGMP

La découverte du système des peptides natriurétiques (NP), au début des années 80, fut une découverte majeure qui révéla le rôle endocrinien du cœur. Les connaissances sur la relaxation vasculaire, la diurèse et la natriurèse provoquées par ce système ont évolué vers un niveau de complexité insoupçonné à cette époque. Nous savons à présent que les NP sont impliqués dans plusieurs autres mécanismes dont la prolifération cellulaire, l’apoptose, l’inhibition du système rénine-angiotensine-aldostérone (RAAS) et le métabolisme des adipocytes. Le métabolisme des lipides est maintenant devenu une cible de choix dans la lutte contre l’obésité. Cette condition aux proportions pandémiques est un facteur de risque majeur dans l’apparition de l’hypertension et du syndrome métabolique (MetS). La compréhension des mécanismes et des défauts de la voie des NP pourrait avoir un impact positif sur le contrôle du MetS et de l’hypertension. L’expression du récepteur des peptides natriuretiques de type 1 (NPR1/GCA) est contrôlée par plusieurs agents incluant son propre ligand, le peptide natriurétique de l’oreillette (ANP). La découverte d’une boucle de retro-inhibition, dans les années 90, a été un événement majeur dans le domaine des NP. En effet, suite à une stimulation à l’ANP, le NPR1/GCA peut inhiber l’activité transcriptionnelle de son propre gène par un mécanisme dépendant du cGMP. Notre groupe a identifié un élément cis-régulateur responsable de cette sensibilité au cGMP et mon projet consistait à identifier la ou les protéine(s) liant cet élément de réponse au cGMP (cGMP-RE). Nous avons identifié un clone liant le cGMP-RE en utilisant la technique du simple hybride chez la levure et une banque d’ADN complémentaire (ADNc) de rein humain. Ce clone provient d’un ADNc de 1083-bp dont le gène est localisé sur le chromosome 1 humain (1p33.36) et codant pour une protéine dont la fonction était inconnue jusqu’ici. Nous avons nommé cette nouvelle protéine GREBP en raison de sa fonction de cGMP Response Element Binding Protein. Des essais de liaison à l’ADN ont montré que cette protéine possède une affinité 18 fois plus élevée pour le cGMP-RE que le contrôle, tandis que des expériences de retard sur gel (EMSA) ont confirmé la spécificité des interactions protéine-ADN. De plus, l’immuno-précipitation de la chromatine (ChIP) a prouvé que GREBP lie le cGMP-RE dans des conditions physiologiques. La liaison de GREBP au cGMP-RE inhibe l’expression du gène rapporteur luciférase sous contrôle du promoteur de npr1/gca. L’inhibition de GREBP à l’aide d’ARN interférant active le promoteur de npr1/gca. Dans les cellules NCI-H295R, l’ANP stimule l’expression de grebp de 60% après seulement 3 heures et inhibe l’expression de npr1/gca de 30%. GREBP est une protéine nucléaire surtout exprimée dans le cœur et ayant le facteur eIF3F comme partenaire. Les variations nucléotidiques du gène sont plus fréquentes chez les patients hypertendus que chez des patients normotendus ou hypertendus souffrant de MetS. Nous rapportons ici l’existence d’un gène spécifique à l’humain qui agit comme répresseur transcriptionnel de npr1/gca et potentiellement impliqué dans le développement de l’hypertension. / The natriuretic peptide (NP) system was a milestone discovery that revealed the endocrine role of the heart for the first time in the early 1980s. From its vasodilatory, natriuretic and diuretic actions, knowledge about this system has evolved to a degree of complexity unsuspected at that time. Now, through cGMP generation, NPs are involved in several other mechanisms, such as cell proliferation, apoptosis, renin-angiotensine-aldosterone system (RAAS) inhibition, and fat cell function. The latter point is of growing interest in lipid metabolism and has become an important issue in the fight against obesity. This pandemic condition is one of the main risk factors leading to hypertension development and metabolic syndrome (MetS) progression. Thus, understanding, at least in part, the lipid mobilization pathways controlled by NPs could have a positive impact in MetS management. As with hypertension, identifying defects in signaling pathways will certainly help to identify mechanisms implicated in lost sensitivity of the NP system. Natriuretic peptide receptor 1 (npr1/gca) expression is controlled by several agents including its own ligand, the atrial natriuretic peptide (ANP). A major finding in NPs field occured in the mid-90s when a mechanism involving a retro-inhibition loop was described. Indeed, after ANP stimulation, NPR1/GCA down-regulates the transcriptional activity of its gene via a cGMP-dependent mechanism. Since our group previously identified a cis-acting element responsible for this cGMP sensitivity, I proceeded to explore novel putative protein binding to the cGMP-response element (cGMP-RE). Using the yeast-one-hybrid technique with a human kidney cDNA library, we identified a strongly positive clone able to bind cGMP-RE. The clone was derived from a 1083-bp long cDNA of a gene of yet unknown function localized on human chromosome 1 (1p33.36). We named this new protein GREBP for cGMP-Response Element-Binding Protein. DNA-binding assays showed 18-fold higher cGMP-RE-binding capacity than the controls while electromobility shift assay (EMSA) indicated a specific binding for the cGMP-RE and chromatin immuno-precipitation (ChIP) confirmed the binding of GREBP to the element under physiological conditions. By acting on cGMP-RE, GREBP inhibited the activity of a luciferase-coupled NPR1 promoter construct. In H295R cells, ANP heightened GREBP expression by 60% after just 3 hours of treatment while inhibiting npr1/gca expression by 30%. Silencing GREBP with specific small interfering RNA increased the activity of the luciferase-coupled NPR1/GCA promoter and NPR1/GCA mRNA levels. GREBP is a nuclear protein mainly expressed in the heart and has the eIF3F factor as partner. Its nucleotide variations are more frequent in non-obese hypertensive patients than normotensive subjects or hypertensive patients suffering from MetS. We report here the existence of a human specific gene acting as a transcriptional repressor of npr1/gca gene that could be implicated in hypertension development.

Hemos inducido en ratas wistar dos modelos de hipertensión arterial (HTA): hipertensión vasculorrenal 2 riñones-1 clip (2R-1C) e hipertensión por déficits crónico de óxido nítrico (NO). La angiotensina II (AII) participa en el desarrollo y mantenimiento de la hipertensión vasculorrenal y por déficit crónico de NO. La administración crónica de losartán, aún previniendo el desarrollo de la hipertensión por déficit crónico de NO, no confiere protección completa frente a las alteraciones hemodinámicas que dicha hipertensión conlleva. Los bloqueantes de los receptores de la AII, saralasina y losartán, poseen efectos hemodinámicos similares, aunque de distinta potencia, sobre los grupos de HTA. En condiciones normales, los efectos hemodinámicos del péptido natriurético auricular (PNA) no son modulados por los receptores de AII. La HTA potencia los efectos hemodinámicos del PNA por un efecto en el que intervienen los receptores de AII. / We have induced in Wistar rats two models of experimental hypertension: two kidney-one clip hypertension (2K-1C) and hypertension induced by chronic inhibition of nitric oxide production (L-NAME-induced hypertension). In both, angiotensin II (AII) participates in the development and maintenance of the hypertension. Chronic administration of losartan prevents the development of the L-NAME-induced hypertension, although it does not confer complete protection to the hemodynamic alterations that this hypertension causes. The AII receptor antagonists, saralasin and losartan, have similar hemodynamic effects on the hypertensive groups, although in different degrees. Moreover, the hemodynamic effects of the atrial natriuretic peptide (ANP) under normal conditions are not modulated by the AII receptor. However, hypertension increases the hemodynamic effects of the ANP by an effect partially due to the AII receptors

GREBP, un nouveau facteur de transcription contrôlant l’expression de la guanylate cyclase A, récepteur de l’ANP, via l’élément de réponse au cGMP

La découverte du système des peptides natriurétiques (NP), au début des années 80, fut une découverte majeure qui révéla le rôle endocrinien du cœur. Les connaissances sur la relaxation vasculaire, la diurèse et la natriurèse provoquées par ce système ont évolué vers un niveau de complexité insoupçonné à cette époque. Nous savons à présent que les NP sont impliqués dans plusieurs autres mécanismes dont la prolifération cellulaire, l’apoptose, l’inhibition du système rénine-angiotensine-aldostérone (RAAS) et le métabolisme des adipocytes. Le métabolisme des lipides est maintenant devenu une cible de choix dans la lutte contre l’obésité. Cette condition aux proportions pandémiques est un facteur de risque majeur dans l’apparition de l’hypertension et du syndrome métabolique (MetS). La compréhension des mécanismes et des défauts de la voie des NP pourrait avoir un impact positif sur le contrôle du MetS et de l’hypertension. L’expression du récepteur des peptides natriuretiques de type 1 (NPR1/GCA) est contrôlée par plusieurs agents incluant son propre ligand, le peptide natriurétique de l’oreillette (ANP). La découverte d’une boucle de retro-inhibition, dans les années 90, a été un événement majeur dans le domaine des NP. En effet, suite à une stimulation à l’ANP, le NPR1/GCA peut inhiber l’activité transcriptionnelle de son propre gène par un mécanisme dépendant du cGMP. Notre groupe a identifié un élément cis-régulateur responsable de cette sensibilité au cGMP et mon projet consistait à identifier la ou les protéine(s) liant cet élément de réponse au cGMP (cGMP-RE). Nous avons identifié un clone liant le cGMP-RE en utilisant la technique du simple hybride chez la levure et une banque d’ADN complémentaire (ADNc) de rein humain. Ce clone provient d’un ADNc de 1083-bp dont le gène est localisé sur le chromosome 1 humain (1p33.36) et codant pour une protéine dont la fonction était inconnue jusqu’ici. Nous avons nommé cette nouvelle protéine GREBP en raison de sa fonction de cGMP Response Element Binding Protein. Des essais de liaison à l’ADN ont montré que cette protéine possède une affinité 18 fois plus élevée pour le cGMP-RE que le contrôle, tandis que des expériences de retard sur gel (EMSA) ont confirmé la spécificité des interactions protéine-ADN. De plus, l’immuno-précipitation de la chromatine (ChIP) a prouvé que GREBP lie le cGMP-RE dans des conditions physiologiques. La liaison de GREBP au cGMP-RE inhibe l’expression du gène rapporteur luciférase sous contrôle du promoteur de npr1/gca. L’inhibition de GREBP à l’aide d’ARN interférant active le promoteur de npr1/gca. Dans les cellules NCI-H295R, l’ANP stimule l’expression de grebp de 60% après seulement 3 heures et inhibe l’expression de npr1/gca de 30%. GREBP est une protéine nucléaire surtout exprimée dans le cœur et ayant le facteur eIF3F comme partenaire. Les variations nucléotidiques du gène sont plus fréquentes chez les patients hypertendus que chez des patients normotendus ou hypertendus souffrant de MetS.
