Munvård till äldre : En enkätstudie till vårdpersonal på särskilt boende / Oral care for elderly : A questionnaire study to nursing personnel at nursing homes

<p>Antalet äldre ökar i Sverige. Åldrandet medför ökad risk att drabbas av sjukdomar vilket kan påverka munhälsan negativt. Äldre med stort vård- och omsorgsbehov flyttar ofta till särskilt boende. Många av dem behöver hjälp med munhygienen. Munvården till äldre i särskilt boende är ofta bristfällig med dålig munhälsa som följd. Syftet var att beskriva vårdpersonalens attityder och beteenden avseende den egna samt de äldres munhälsa och munvård, samt undersöka om munvårdsutbildning kan relateras till munvårdsinsatser på särskilt boende. En tvärsnittsstudie med kvantitativ ansats genomfördes och datainsamlingen gjordes med hjälp av ett frågeformulär. Resultatet visade att de flesta respondenterna ansåg munhälsan som viktig. Majoriteten tyckte att de boende bör få hjälp med munvård två gånger per dag men få hjälpte de äldre med munvård så ofta. Munhygienen ansågs vara den svåraste delen i omvårdnadsarbetet. Flertalet hade fått munvårdsutbildning och få önskade mer kunskaper. Munvården upplevdes fungera bra på avdelningarna och sågs som en del av god omvårdnad. Flera faktorer, som att de äldre gjorde motstånd och tidsbrist, uppgavs som svårigheter. Konklusionen var att attityden till munvård var positiv, men hindrande faktorer som motstånd från de äldre och tidsbrist bidrog till att den prioriterades lågt. Endast en liten del av vårdpersonalen önskade mer munvårdsutbildning.</p>

<p>Tandhygienistyrket grundades i början av 1900-talet och idag finns tandhygienister i över trettio länder och det är ett kvinnodominerat yrke. Den första tandhygienistutbildningen startade i Sverige 1968. År 1991 blev tandhygienistyrket en legitimerad profession. <em>Syftet </em>var att undersöka vilka kunskaper patienter i åldersgruppen 50 - 65 år har om legitimerade tandhygienisters yrkesroll samt patienternas attityder till tandhygienistbehandling. Frågeställningarna var "vilken kunskap har patienter i åldersgruppen 50 - 65 år om legitimerade tandhygienisters yrkesroll?" och "vilka attityder har patienter i åldersgruppen 50 - 65 år till tandhygienistbehandling?" Projektet genomfördes som en intervjustudie med kvalitativ metod. <em>Resultatet </em>visade att respondenterna lyfte fram tandhygienisternas förebyggande arbete inom tandvården och vikten av regelbundna besök för att i möjligaste mån undvika reparativa åtgärder. De hade relativt goda kunskaper om de arbetsuppgifter och behandlingar tandhygienister utför och respondenterna lyfte fram att tandhygienistbehandling har haft en positiv påverkan på deras munhälsa. Respondenterna hade som gemensamt önskemål att tandvården kallade dem till besöken istället för att själva sköta besöksintervallen. Med den information som kom fram genom denna studie är en <em>slutsats </em>att tandhygienistprofessionen och dess förebyggande uppgifter bör synliggöras mer i samhället genom till exempel mer utåtriktad verksamhet.</p>

Incorporating psychological theory into the model of diffusion of innovations in healthcare

Evidence-based medicine calls for the use of current best evidence (together with individual clinical expertise). Guidelines aim to distil such evidence, yet clinical practice often fails to follow guidelines, for multiple reasons that are still not well understood. One under-researched aspect of the gap between guidelines and practice is psychology. My literature review found that the application of psychology to implementation research has been limited, and such research is not well integrated into wider implementation research. In this study, I sought to a) systematically apply psychological theory to understand the different psychological processes in the stages of adoption described in the diffusion of innovations model; b) collect and analyse data to explore and test this new, psychologically-enhanced model of guideline adoption; and c) improve and extend my model in the light of my empirical data. Having populated my proposed framework with potentially relevant psychological theories based on my literature review, I undertook a first assessment of the validity and added value of this proposed theoretical framework through a case study of the implementation of guidance on universal offering of HIV testing in hospitals serving populations meeting the criteria for high HIV prevalence in the catchment population, interviewing 20 healthcare professionals across two sites. My findings broadly supported my proposed theoretical approach, and illustrated relevant psychological theories for different stages of adoption. My findings support two provisional conclusions. First, that there is potential to improve the effectiveness of efforts to implement guidelines by augmenting the widely-used innovation adoption model with specific psychological theories. Second, that policymakers would do well to shift from viewing the health system as a complicated entity that policy can control and direct is misconceived; I recommend shifting to a perspective of the health system as a complex system, and rethinking the role of policy from that perspective.

The role of culture in children's sex-typed preferences for colours, toys, and affordances : a systems theory approach

Children's sex-typed preferences for colours and toys are well-established, and often function as markers of sex-typicality in research on the development of sex-typed behaviour. However, children's sex-typed colour and toy preferences have not been tested cross-culturally, or in remote unindustrialised cultural settings. The present thesis tested children's preferences for sex-typed toys in four cultural settings: Shipibo villages in the Lake Imiria region of the Peruvian Amazon; kastom villages in the mountains of Tanna Island in Vanuatu in the South Pacific; children attending school in Lenakel town on Tanna Island; and in a large industrialised city in Australia. It also tested children's colour preferences in three of these cultures. It was hypothesised that colour and toy preferences would show some similarities across cultures, and further, that similarities in toy preferences across cultures would be explained by the different types of play afforded by the toys. Results suggested that colour preferences, specifically, a sex difference in preference for pink, are specific to industrialised cultures. Results further suggested that some sex differences in toy preferences replicate in different cultures, and that the relationship between toy preferences and children's preferences for play affordances is a potentially important area for further research. The present thesis also provided two demonstrations of how new statistical methods, adapted from complex and dynamic systems theory, could be applied to the cross-cultural dataset. A machine learning method suggested that sex, more than culture, affects children's sex-typed toy preferences. A multistate dynamic method further suggested that sex, more than culture, affects the dynamics of children's toy choices.

Motiverande samtal, ”tell-show-do”-metoden och positiv förstärkning var pedagogiska strategier, som respondenterna tog upp.</p><p><strong><em>Konklusionen</em></strong> är att samtliga respondenter använder pedagogiska strategier, men de visste inte vilken teori som låg till grund för de pedagogiska strategierna. Utifrån svaren konstaterades att den humanistiska traditionen låg till grund för respondenternas arbetssätt. Tandhygienistens kunskap om pedagogiska strategier och teorier är begränsad. Human behaviour and energy demand : How behavioural science can be used to reduceenergy demand in the residential sector

The threat of human induced climate change is imminent. The reason is an everyincreasing demand for energy and products, producing more and more greenhousegas emissions. Everybody needs to take responsibility now. The estimations are thatwith 2% annual energy savings from residential households 12TWh and 3.3 billionmetric tonnes of CO2 can be saved per year. Greenely, a startup from KIC InnoEnergy,wants to engage residential households to change their energy behaviour athome. They combine a smartphone application with the smart meter infrastructureto reduce households energy demand. Changing behaviour is complicated and researchprior to this thesis revealed that information and economic incentives aloneare not sufficient.A simple and proven technique to change behaviour is Nudging. A gentle pushin the right direction while leaving the freedom of choice. A popular example is aprinted fly in the men’s urinal. It nudges them to aim at the fly. The cleaning costswere reduced by 80% at the Schiphol Airport Amsterdam.Without application usage change is impossible for Greenely. Their main contacttool to households is a smartphone application. The smartphone market is vast andcompetition between applications is strong. Therefore the system outline needs toprecede Nudging for ongoing engagement and long term change. To achieve thatGamification practices are implemented. It is the incorporation of game design intonon-gaming contexts to achieve engagement through motivation and fun.This master thesis is done in cooperation with Greenely and focuses on residentialdemand reduction schemas, Nudging and Gamification. The aim is to improvetheir actual application and create an outline for an improved version that promoteslong term behaviour change. The result incorporates the most suitable features fromthe relevant topics and enables long term change.
