La contamination bactérienne des concentrés plaquettaires (CPs) représente le risque le plus fréquent d'infection transmise par transfusion. Malgré les progrès techniques accomplis dans le dépistage des agents infectieux, le risque de transmission de ces agents par transfusion ne peut encore être considéré comme totalement maîtrisé. Nous avons pu développer une méthode de détection par la technique de MALDI-TOF. Notre protocole permet une détection dans un délai de 8 heures à de faibles concentrations bactériennes.Nous avons évalué les performances d’un automate d’hémoculture, le VersaTREK, par rapport au Bactec, qui est fréquemment utilisé par les centres de transfusion. Nos résultats démontrent son intérêt dans la détection de la contamination bactérienne des CPs. Afin de caractériser l’origine de la contamination bactérienne, nous avons comparé, dans une étude rétrospective, les espèces bactériennes identifiées dans les hémocultures des patients de l’hôpital de la Timone, à celles identifiées dans des poches de CPs contaminés. Les espèces responsables de la contamination des CPs sont essentiellement issues de la flore cutanée. Elles sont donc différentes des espèces responsables d’épisodes de bactériémies.Ainsi, ce travail propose de nouvelles méthodes de détection de la contamination bactérienne. La mise en œuvre de ces nouvelles stratégies de diagnostic couplées à une meilleure connaissance de la prévalence et de l’origine de la contamination bactérienne des CPs pourrait permettre d’améliorer davantage la prévention et la réduction de la morbi-mortalité induite par une transfusion de CPs. / Bacterial contamination of platelet concentrates (PCs) is the most common risk of transfusion-transmitted infection. Despite better control of the elements of the technical progress achieved in the detection of infectious agents, the risk of transmission of these agents by transfusion cannot yet be considered as totally mastered. We have developed a method for detecting PCs contamination by MALDI-TOF. To the best of our knowledge, this is the first time that MALDI-TOF is used in the detection of microbial contamination in a complex medium such as PCs. Our protocol allows detection within 8 hours at low bacterial concentrations. In a second step, we evaluated the performance of a blood culture machine, VersaTREK®, compared to Bactec®, frequently used by transfusion centers. Our results demonstrate its interest in the detection of bacterial contamination of PCs. In order to characterize the origin of bacterial contamination of PCs, we compared in a retrospective study, bacterial species identified in patients' blood cultures treated at the Timone hospital, with those identified in infected PC units. The species responsible for the contamination of the PCs are mainly resulting from the cutaneous flora and are therefore different from the species responsible for episodes of bacteremia.Thus, this work proposes new methods for detecting bacterial contamination. The implementation of these new diagnostic strategies coupled with a better knowledge of the prevalence and origin of bacterial contamination of PCs could further improve the prevention and reduction of both morbidity and mortality induced by blood transfusion of PCs.

A transfusão de sangue é um procedimento benéfico, mas não isento de risco, e por essa razão medidas devem ser tomadas para aumentar a sua segurança, entre elas a adoção de boas práticas transfusionais e as notificações dos eventos adversos, que compõe as atividades de hemovigilância. O objetivo do estudo foi avaliar o impacto da implantação de uma equipe transfusional e da busca ativa por reações transfusionais nas atividades de hemovigilância e na segurança transfusional em um Núcleo de Hemoterapia regional, localizado em um hospital geral. Foi realizado um estudo comparativo entre dois períodos: antes e após a implantação da equipe transfusional. Observou-se um aumento de 9,04 vezes no número de notificações de reações transfusionais, elevando de duas notificações em 1.677 transfusões realizadas (0,12%) no período de 22 meses antes da implantação, para dez notificações em 928 transfusões (1,08%, p<0,01) no período de 12 meses após, revertendo a subnotificação existente. As RFNH foram as reações transfusionais mais frequentemente encontradas, havendo aumento significativo nas notificações, de uma RFNH antes para seis após a implantação da equipe transfusional (p<0,01). Observou-se também um aumento na segurança transfusional, corrigindo falhas na identificação de amostras e requisições de transfusão, na verificação dos sinais vitais e identificação dos pacientes, minimizando assim o risco de reações graves por troca de amostras/pacientes. O estudo mostrou que ter um profissional acompanhando o ato transfusional desde a coleta da amostra até o término da transfusão, traz mais segurança para o receptor da transfusão, ao permitir que os procedimentos estabelecidos sejam seguidos, e aumenta as notificações de eventos adversos, favorecendo o reconhecimento das reações transfusionais e fornecendo subsídios para a adoção de estratégias para sua prevenção e tratamento. Ao final do estudo foram elaborados cartazes e panfletos para orientação de profissionais e pacientes, respectivamente, sobre a transfusão de sangue. / Blood transfusion can be unavoidable and beneficial but has inherent risks. Hemovigilance actions, therefore, should be undertaken to increase its safety, including the adoption of better blood transfusion initiatives and adverse event reporting. The purpose of this study was to evaluate how the introduction of a transfusion practitioner team to monitor transfusion outcomes affects the hemovigilance system and the transfusion safety in a regional hemotherapy center based in a general hospital. We performed a comparison of two clinical periods: before and after the introduction of a transfusion practitioner team. An increase was observed from 2 event reports in 1677 transfusions (0.12%) in a 22-month period to 10 event reports in 928 transfusions (1.08%, p<0.01) in a 12-month period, demonstrating a 9.04-fold increase in transfusion reaction reports, drastically reducing the frequency of underreporting. Febrile non-hemolytic reactions were the most frequent adverse event reported, with a significant increase from 1 to 6 events reported (p <0.01) after the involvement of the transfusion team. There was also an improvement in transfusion safety practices via the correction of misidentification of patient pretransfusion samples and transfusion requests, the avoidance of failure to verify vital signs, and improvement of patient identification at the bedside, all of which are critical to minimize the risk of transfusion error. The study showed that having a professional team oversee the transfusion process, from sample collection to blood administration, brings more safety to the patient by allowing established procedures to be followed. The adoption of this practice increases reporting of adverse events through the early recognition of transfusion reactions, expediting their treatment and enhancing the prevention of future events. At the end of the study posters and educational packets were created to provide guidance about blood transfusion to leading professionals and patients.

A hemovigilância é considerada uma importante ferramenta para a segurança transfusional. Os primeiros sistemas nacionais de hemovigilância, no mundo, foram implantados na década de 1990. No Brasil, o Sistema Nacional de Hemovigilância (SNH), que é coordenado pela Agência Nacional de Vigilância Sanitária (Anvisa), iniciou sua implantação, em 2002, com foco nas reações transfusionais (RT), em uma rede restrita de hospitais. No final de 2006, a possibilidade de notificar RT foi estendida aos demais serviços de saúde e os gestores de saúde da esfera estadual também foram inseridos nesse processo. O estado de São Paulo (SP), onde funciona um subsistema do SNH (SNH-SP), colaborou com 41% do total de RT notificadas, no período de 2002 a 2014, segundo a Anvisa. Contudo, apesar de decorridos cerca de 10 anos, o SNH-SP nunca foi avaliado. O objetivo desse trabalho foi avaliar a implantação do SNH-SP, a fim de obter subsídios para sua melhoria. Trata-se de um estudo avaliativo, utilizando-se metodologia de avaliação baseada no Updated Guidelines for Evaluating Public Health Systems do Centers for Disease Control and Prevention (CDC). Foram utilizadas duas bases de dados secundários: das transfusões e das notificações das RT. Foram também utilizados três questionários concebidos para identificar fatores que contribuem ou não com a adesão e com a implantação do SHN-SP, pelos hospitais, serviços de hemoterapia (SH) e pela vigilância sanitária (Visa). Eles foram respondidos por 81 hospitais, 61 SH e 22 das 28 regionais de Visa. Verificou-se que o SNH-SP tem adesão crescente dos hospitais, especialmente os de maior porte, embora a aceitabilidade destes seja baixa, quando comparada com a dos SH. Foram realizadas 12.182.981 transfusões em 947 serviços de saúde, entre 2008 a 2015, mais de 55% delas, de concentrado de hemácias. O número de notificações de RT aumenta a cada ano, mas há subnotificação geral e de alguns tipos específicos de RT. Foram notificadas 23.942 RT, por 353 serviços de saúde. As 23.734 RT ocorridas nesse período se caracterizaram, predominantemente, como imediatas (97,96%), dos tipos febril não hemolítica (51,81%) e alérgica (38,20%). Apresentaram gravidade leve (87,62%), contudo, houve 35 (0,15%) óbitos. Concentraram-se nos maiores de 50 anos (51,49%). A incidência de RT nesse período variou entre 3,44 e 4,55/1.000, sendo mais elevada nas transfusões de concentrados de granulócitos, chegando a 53,06 RT/1.000. O SNH-SP tem gestão centralizada e tanto ele quanto o sistema de informação utilizado para notificação ainda são desconhecidos de parte dos hospitais. Há déficit de pessoal para trabalhar na área, tanto nos hospitais quanto nas Visa. Há falta de integração com outros sistemas de saúde afins, sugerindo a subnotificação de doenças transmitidas pelo sangue no SNH-SP, como as hepatites B e C. A estruturação do SNH apresenta simplicidade, porém, seu funcionamento atual, em SP, prejudica a avaliação desse atributo. Ao se analisar as RT definidas como eventos sentinela, a oportunidade e a qualidade dos dados foram consideradas insatisfatórias. O SNH-SP foi considerado útil, mas ajustes são recomendados para seu efetivo funcionamento. / Haemovigilance is considered an important tool for transfusion safety. The first national hemovigilance systems in the world were implemented in the 1990s. The Brazilian Hemovigilance System (BHS), which is managed by Agência Nacional de Vigilância Sanitária (Anvisa), began in 2002, focusing on the transfusion reactions (TR) in a limited hospital network. At the end of 2006, the possibility of reporting TR was extended to all health services and state health managers were also included in this process. The state of São Paulo (SP), where a BHS subsystem (BHS-SP) operates, contributed with 41% of the total reported RTs, from 2002 to 2014, according to Anvisa. However, after about 10 years, the BHS-SP has never been evaluated. The objective of this study was to evaluate the implementation of the BHS-SP, in order to obtain inputs for its improvement. This is an evaluative study, using an evaluation methodology based on the Updated Guidelines for Evaluating Public Health Systems of the Centers for Disease Control and Prevention (CDC). The organizational structure of the BHS demonstrates simplicity, but its current functioning in SP impairs the evaluation of this attribute. When RTs defined as sentinel events were analyzed, the timeliness and the quality of the data were considered unsatisfactory. The BHS-SP was considered useful, but adjustments are recommended for its effective functioning.

A pathogen-reduction system for platelet components would be a useful method since it reduces the risk of bacterial, protozoa, viral and white blood cell contamination. The Intercept Blood System method (IBS) for platelets, destroys DNA and RNA and was validated against the routine method in order to reduce pathogen transmission risk during transfusion. The validation of IBS, the trombocyte count for100 buffy coat concentrates from 2007 were compared to values for 100 buffy coat concentrates from 2006 that had been treated with gamma-radiation. Akademiska sjukhuset in Uppsala has a requirement that 75% of the platelet concentrates contain at least 300*10 9 platelets per unit. Šiame darbe nagrinėjamos aktualios mūsų šalyje saugaus donorinio kraujo ir kraujo produktų surinkimo problemos. Per kraujo perpylimą plintančios ligos yra didžiausia kraujo tarnybų problema visame pasaulyje. Ši problema yra tiesiogiai proporcinga kraujo donorų bendruomenėje paplitusioms infekcijoms. Atsižvelgiant į pagrindinius saugaus kraujo užtikrinimo kriterijus darbe rekomenduojamos konkrečios galimos priemonės infekcinių ligų prevencijai.
Darbo tikslas: Palyginti ir įvertinti infekcinių ligų žymenų paplitimą tarp atlygintinų ir neatlygintinų kraujo donorų. Darbe keliami uždaviniai yra šie: išsiaiškinti motyvaciją kraujo donorų tarpe; išsiaiškinti infekcinių ligų ��ymenų paplitimą tarp atlygintinų ir neatlygintinų kraujo donorų; pasiūlyti galimas priemones infekcinių ligų plitimo prevencijai kraujo donorų tarpe.
Tyrimas buvo atliktas 2007 metų liepos – rugpjūčio mėnesiais viešojoje įstaigoje Nacionalinis kraujo centras, atsitiktinai pasirenkant 500 respondentų - kraujo donorų - imtį. Metodikos tinkamumui įsitikinti, tyrimui buvo sudaryta anketa – klausimynas. Statistinė gautų duomenų analizė buvo atliekama personaliniu kompiuteriu, panaudojant statistinės analizės SPSS programinį paketą versija “16.0” ir statistinę programą “5.0”. Statistinių duomenų reikšmingumas buvo įvertintas pagal χ2 kriterijų. Skirtumai tarp atitinkamų rodiklių laikomi reikšmingais, kai klaidos tikimybė p≤0,05 (kai p>0,05 – rezultatų skirtumai statistiškai nereikšmingi).

The diseases spreading through blood transfusion is the biggest problem of Blood Transfusion Services in the world. The problem is directly proportionate to the prevalence of infection markers among blood donors. Taking into consideration the criteria of safe blood assurance definite possible measures are recommended to prevent the spread of infection diseases.
The aim of the study is to compare and assess infection markers prevalence among payable and non-remunerated donors. The objectives of this study are the following: to determine donors motivation, viral infections prevalence among payable and non-remunerated donors, to give recommendations for the prevention of infection markers prevalence in the donors community.
Methods. The study has been accomplished during July – August 2007 in the National Blood Centre, randomly choosing 500 respondents – the donors of blood. To evaluate the appropriation of the method a questionnaire was introduced. The statistical analysis of the results was made by personal computer using SPSS programme package version 16.0 and the statistical programme 5.0. The significance of the statistical data was evaluated according χ² criteria. The difference between the indicators is statistically significant when the error probability is p ≤ 0.

Robust Post-donation Blood Screening under Limited InformationEl-Amine, Hadi 10 June 2016 (has links)
Blood products are essential components of any healthcare system, and their safety, in terms of being free of transfusion-transmittable infections, is crucial. While the Food and Drug Administration (FDA) in the United States requires all blood donations to be tested for a set of infections, it does not dictate which particular tests should be used by blood collection centers. Multiple FDA-licensed blood screening tests are available for each infection, but all screening tests are imperfectly reliable and have different costs. In addition, infection prevalence rates and several donor characteristics are uncertain, while surveillance methods are highly resource- and time-intensive. Therefore, only limited information is available to budget-constrained blood collection centers that need to devise a post-donation blood screening scheme so as to minimize the risk of an infectious donation being released into the blood supply. Our focus is on "robust" screening schemes under limited information. Toward this goal, we consider various objectives, and characterize structural properties of the optimal solutions under each objective. This allows us to gain insight and to develop efficient algorithms. Our research shows that using the proposed optimization-based approaches provides robust solutions with significantly lower expected infection risk compared to other testing schemes that satisfy the FDA requirements. INTRODUÇÃO: No Brasil, os doadores de sangue são submetidos à triagem clinica antes da doação através de um questionário padronizado que segue recomendações do Ministério da Saúde. Apesar de não ser obrigatório, os serviços de hemoterapia brasileiros costumam perguntar aos candidatos sobre o número de parceiros sexuais nos doze meses que precederam aquela doação de sangue. Os candidatos que referem um número de parceiros acima do limite permitido em cada hemocentro são recusados na triagem clínica pré-doação. Este estudo analisa as características demográficas, o número de parceiros heterossexuais e marcadores sorológicos em 689.868 doações de três hemocentros brasileiros, participantes do REDS-II, entre 1 de julho de 2007 a 31 de dezembro de 2009. MÉTODOS: Os doadores foram classificados de acordo com o número máximo declarado de parceiros sexuais nos últimos doze meses permitidos em cada hemocentro. Os valores de corte para Belo Horizonte, Recife e São Paulo são dois, três e seis parceiros, respectivamente. Foram realizados os testes de qui-quadrado e regressão logística a fim de examinar associações entre características demográficas, número de parceiros sexuais em doze meses e taxas de marcadores sorológicos individuais e globais positivas para o vírus da imunodeficiência adquirida (HIV), vírus linfotrópico humano (HTLV) tipo 1 e 2, hepatite B, hepatite C e sífilis. RESULTADOS: Doadores de primeira vez, jovens e com maior nível educacional foram associados a maior número de parceiros sexuais recentes, assim como o gênero em São Paulo e Recife (p < 0, 001). Marcadores sorológicos globais, para HIV e sífilis foram associados com maior número de parceiros em São Paulo e Recife (p < 0, 001), mas não em Belo Horizonte. Na análise de regressão logística, o número de parceiros sexuais foi associado com marcadores sorológicos positivos [razão de chance ajustada (AOR) 1,2-1,5], especialmente no HIV (AOR 1,9-4,4). Em conclusão, o número de parceiros sexuais nos doze meses antes da doação de sangue, foi associado com positividade para HIV e taxas globais de marcadores sorológicos para doenças transmissíveis por transfusão. A associação não foi consistente entre os centros, tornando difícil definir um valor de corte uniforme para todos os hemocentros brasileiros. Estes achados corroboram que o uso da informação dos contatos heterossexuais recentes é um importante critério de inaptidão e de melhora na segurança transfusional no Brasil / INTRODUCTION: In Brazil, blood donors undergo medical screening before donation through a standardized questionnaire that follows recommendations from the Ministry of Health. Although not required, most of the Brazilian blood centers routinely ask candidates about the number of sexual partners in the twelve months preceding that blood donation. Candidates who refer a number of partners over the limit allowed in each blood center are refused at the predonation clinic screening. This study analyzes the demographic characteristics, the number of heterosexual partners and serological markers in 689,868 donations from three Brazilian blood center, participants in the REDS-II, from 1 July 2007 to December 31, 2009. METHODS: Donors were classified according to the stated maximum of the number of sexual partners in the last twelve months, allowed at each blood center. The cutoff values for Belo Horizonte, Recife and Sao Paulo are two, three and six partners, respectively. We conducted the chi-square and logistic regression to examine associations between demographic characteristics, number of sexual partners in twelve months and rates of individual and global serological markers positive for human immunodeficiency virus (HIV), human lymphotropic virus (HTLV) type 1 and 2, hepatitis B, hepatitis C and syphilis. RESULTS: First time donors, young and better educated were associated with increased number of recent sexual partners, as well as gender in São Paulo and Recife (p < 0.001). Global serological markers for HIV and syphilis were associated with greater number of partners in Sao Paulo and Recife (p < 0.001), but not in Belo Horizonte. In logistic regression analysis, the number of sexual partners was associated with positive serological markers [adjusted odds ratio (AOR) 1.2 to 1.5], especially HIV (AOR 1.9 to 4.4). In conclusion, the number of sexual partners in the twelve months before blood donation was associated with HIV positivity and overall rates of serologic markers for transfusion-transmissible diseases. The association was not consistent among the Brazilian blood centers, making it difficult to set a uniform cut off value for all blood banks in Brazil.
