Unfolding movement in time and space : defining upper-limb recovery post-stroke / Décomposer le mouvement dans le temps et l’espace : définition de la récupération post-AVC du membre supérieur

Plusieurs champs de recherche ont été combinés - mettant en évidence l'utilité de l'analyse cinématique, non seulement afin d'évaluer le comportement moteur, mais aussi afin de contribuer à la compréhension de la récupération motrice post-AVC. Au travers d'analyses cinématiques, les mouvements du membre supérieur hémiplégique ont été décomposés dans le temps et l'espace, afin d'en extraire l'échelle et les composantes structurelles. Cette décomposition systématique, d'abord connue pour son bien-fondé clinique, nous a permis d'identifier les marqueurs les plus pertinents du contrôle du membre supérieur parétique : i.e., la fluidité, la rectitude et la vitesse. Subséquemment, il a été démontré que i) les changements cinématiques se stabilisent au cours de la phase de rééducation, indiquant potentiellement la nécessité de modifier la stratégie thérapeutique; ii) les patient post-AVC sont capables de percevoir la fluidité du mouvement en réalité virtuelle, cette perception étant meilleure lorsque le feedback visuel ne concerne que le point du travail du membre; iii) l'espace de travail post-AVC n'est pas isotrope pour un patient hémiplégique; iv) chez les patients post-AVC, le niveau de ‘bruit neuromoteur' est augmenté; et v) la cinématique reflète la conséquence des stratégies d'adaptation à l'augmentation du bruit, ces stratégies étant basées sur un compromis entre des modes de contrôle d'erreur en feedforward et en feedback des actions motrices. Ainsi, il peut être conclut que la décomposition du mouvement dans le temps et l'espace est un moyen simple et efficace d'appréhender contrôle moteur chez l'Homme en situation normale et âpres AVC. L'enjeu est maintenant d'implémenter ces méthodes d'analyse cinématique dans les protocoles de rééducation post-AVC quotidienne afin de développer de larges bases de données permettant, à l'aide de méthodes de modélisation, de définir des profils de récupération et ainsi personnaliser de façon optimale la rééducation à chaque patient particulier. / Multiple research fields were combined – highlighting the value of kinematic analysis, not only to evaluate motor behaviour, but also to contribute to the understanding of motor recovery post-stroke. By means of kinematics, hemiplegic upper-limb movements were unfolded in time and space, to extract the scaling and structural components of the movement. This systematic decomposition, first proven to have clinical relevance, allowed us to identify the most pertinent markers of paretic upper-limb control: i.e. smoothness, directness and velocity. Subsequently it was shown that i) change in kinematics levels off over rehabilitation, possibly indicating that treatment may profit from change; ii) people post-stroke are able to perceive movement fluency in virtual realities, whereby simple end-point displays facilitate perception; iii) the workspace post-stroke is heterogeneous; iv) stroke patients have increased levels of neuromotor noise; and v) kinematics reflect the outcome of adaptation strategies to the increased noise in relation to the automaticity of error-corrections on the trade-off between feedforward and feedback based motor control. It may thus be concluded that unfolding the movement in space and time, is a simple and powerful way to define human motor control. The challenge is to implement kinematic analysis in daily post-stroke practice to develop a large database enabling the definition of recovery profiles contributing to provide each individual patient with the right therapy at the right time.

Modelování kreditního rizika protistrany / Counterparty credit risk modelling

Counterparty credit risk is an important type of financial risk. The importance of proper counterparty risk management became most apparent in the wake of the 2008 series of failures of several large banks. Correlation of market factors is an important issue in the calculation of CVA. A notable case of correlation is wrong-way risk which occurs whenever the probability of default of the counterparty is positively correlated with exposure. The basic formulas for CVA and basic counterparty credit risk models do not account for wrong-way risk because its modeling is nontrivial. This thesis aims to answer how well can the impact of wrong-way risk on CVA be approximated with an add-on which only depends on correlation between the price of the underlying asset and the credit spread of the counterparty. The thesis is supplemented by a fully documented implementation of the model in the Mathematica software.

Default contagion modelling and counterparty credit risk

This thesis introduces models for pricing credit default swaps (CDS) and evaluating the counterparty risk when buying a CDS in the over-the-counter (OTC) market from a counterpart subjected to default risk. Rather than assuming that the default of the referencing firm of the CDS is independent of the trading parties in the CDS, this thesis proposes models that capture the default correlation amongst the three parties involved in the trade, namely the referencing firm, the buyer and the seller. We investigate how the counterparty risk that CDS buyers face can be affected by default correlation and how their balance sheet could be influenced by the changes in counterparty risk. The correlation of corporate default events has been frequently observed in credit markets due to the close business relationships of certain firms in the economy. One of the many mathematical approaches to model that correlation is default contagion. We propose an innovative model of default contagion which provides more flexibility by allowing the affected firm to recover from a default contagion event. We give a detailed derivation of the partial differential equations (PDE) for valuing both the CDS and the credit value adjustment (CVA). Numerical techniques are exploited to solve these PDEs. We compare our model against other models from the literature when measuring the CVA of an OTC CDS when the default risk of the referencing firm and the CDS seller is correlated. Further, the model is extended to incorporate economy-wide events that will damage all firms' credit at the same time-this is another kind of default correlation. Advanced numerical techniques are proposed to solve the resulting partial-integro differential equations (PIDE). We focus on investigating the different role of default contagion and economy-wide events have in terms of shaping the default correlation and counterparty risk. We complete the study by extending the model to include bilateral counterparty risk, which considers the default of the buyer and the correlation among the three parties. Again, our extension leads to a higher-dimensional problem that we must tackle with hybrid numerical schemes. The CVA and debit value adjustment (DVA) are analysed in detail and we are able to value the profit and loss to the investor's balance sheet due to CVA and DVA profit and loss under different market circumstances including default contagion.

In this swap transaction the company receives the return on its own stock, whilst paying a fixed or floating interest rate. However, this kind of swap presents wrong-way risk, that is, a positive dependence between the underlying asset and the counterparty’s default probability, which must be considered by dealers when pricing this kind of swap contracts. In this work we propose a model for incorporating dependence between default probabilities and the counterparty’s exposure in the calculation of the CVA for these kind of swaps. We use a Cox process to model default times, given that the stochastic default intensity follows a CIR model, and assuming that the factor driving the underlying stock price and the factor driving the default intensity are jointly given by a bivariate standard Gaussian distribution. We analyze the impact on CVA of incorporating wrong-way risk in this kind of swap transaction with different counterparties, and for different maturities and dependence levels. / Uma forma interessante para uma companhia que pretende assumir uma posição comprada em suas próprias ações ou lançar futuramente um programa de recompra de ações, mas sem precisar dispor de caixa ou ter que contratar um empréstimo, ou então se protegendo de uma eventual alta no preço das ações, é através da contratação de um swap de ações. Neste swap, a companhia fica ativa na variação de sua própria ação enquanto paga uma taxa de juros pré ou pós-fixada. Contudo, este tipo de swap apresenta risco wrong-way, ou seja, existe uma dependência positiva entre a ação subjacente do swap e a probabilidade de default da companhia, o que precisa ser considerado por um banco ao precificar este tipo de swap. Neste trabalho propomos um modelo para incorporar a dependência entre probabilidades de default e a exposição à contraparte no cálculo do CVA para este tipo de swap. Utilizamos um processo de Cox para modelar o instante de ocorrência de default, dado que a intensidade estocástica de default segue um modelo do tipo CIR, e assumindo que o fator aleatório presente na ação subjacente e que o fator aleatório presente na intensidade de default são dados conjuntamente por uma distribuição normal padrão bivariada. Analisamos o impacto no CVA da incorporação do riscowrong-way para este tipo de swap com diferentes contrapartes, e para diferentes prazos de vencimento e níveis de correlação.

Objectives: evaluating the health-related quality of life of family caregivers of patients with sequelae with CVA. Methods: the research was descriptive, transversal type, with a quantitative approach, held at the domicile of patients with sequelae by AVE, in the city of João Pessoa-Paraíba, in the period between the months of February and April 2013. The population of the study consisted of 136 family caregivers of patients affected by stroke. For data collection, there was used a structured instrument, contemplating closed issues relating to variables of the study and the specific measures to assess, respectively, the physical capacity of individuals with sequel by CVA, the overload and health-related quality of life among caregivers: Barthel index, Burden Interview Scale and Short Form-36. The analysis was performed using three techniques: statistical descriptive analysis and exploratory data analysis and Association through the Chi-square test and the T-test and F for comparisons of the averages. The project was submitted to the ethics committee of the Centre for Health Sciences, Federal University of Paraíba Health and approved under protocol number 0279/13 and CAAE: 13778313.3.0000.5188. Results: commitment was evidenced in almost all domains of quality of life of family caregivers, among them, those who were presented with lower average were: Pain (40.16), Mental Health (53.62) and Social Aspects (54.12). Characteristics of individuals with sequel by CVA, which is related with the domains of quality of life, one can find out that those who were below the age of 65, were married and had highest level of schooling, major averages, respectively, in the fields of Social Aspect, Emotional and Mental Health Aspects. Caregivers of people with higher level of dependency to the basic activities of daily living had lower average in the mental health field. Regarding the characteristics of the caregiver, major averages were caregivers under the age of 40 years old and married in the field, Functional Capacity, those who had a profession, on emotional aspects and those who had higher income, in the areas of mental health and vitality. There was also an association between overload and worse health-related quality of life in the areas of functional capacity, physical aspects, emotional aspects and Pain. Conclusion: the results of the study allowed understanding the different dimensions and specificities in the context of the health-related quality of life, evidencing the need for redirection of the look to public spheres for the recognition of the problems faced by the family when individuals are affected by a disabling disease, such as CVA. / Introdução: As sequelas físicas e cognitivas provocadas pelo AVE comprometem a capacidade funcional, a independência e a autonomia da pessoa acometida e acarreta alterações na dinâmica de vida não só do indivíduo acometido, mas também da família, sobretudo na do cuidador principal. Objetivos: Avaliar a qualidade de vida relacionada à saúde de cuidadores familiares de pacientes com sequelas com AVE. Metodologia: A pesquisa foi do tipo descritivo, transversal, com abordagem quantitativa, realizada no domicílio dos pacientes com sequela de AVE, no município de João Pessoa PB, no período de fevereiro a abril de 2013. A população do estudo foi composta por 136 cuidadores familiares de pacientes acometidos por acidente vascular. Para a coleta dos dados, foi utilizado um instrumento estruturado, contemplando questões fechadas relativas às variáveis do estudo e às medidas específicas para avaliar, respectivamente, a capacidade física dos indivíduos com sequela de AVE, a sobrecarga e a qualidade de vida relacionada à saúde entre os cuidadores: índice Barthel, Burden Interview Scale e o Short-Form-36. A análise foi realizada através de três técnicas: estatísticas: Análise Descritiva e Exploratória de Dados e Análise de Associação por meio do Teste de qui-quadrado e o Teste T e F para comparações das médias. Quanto às características dos indivíduos com sequela de AVE, que se relacionaram com os domínios de qualidade de vida, conclui-se que os que tinham idade inferior a 65 anos eram casados e tinham um nível mais alto de escolaridade, obtiveram maiores médias, respectivamente, nos domínios Aspecto sociais , Aspectos emocionais e Saúde mental . Os cuidadores de pessoas com nível mais alto de dependência para as atividades básicas de vida diária tiveram menor média no domínio Saúde mental . No concernente às características do próprio cuidador, as maiores médias foram relativas aos cuidadores com menos de 40 anos e os casados, no domínio Capacidade funcional , os que tinham uma profissão, no domínio Aspectos emocionais , e os que apresentavam maior renda, nos domínios Saúde mental e Vitalidade . Constatou-se também uma associação entre a sobrecarga e pior qualidade de vida relacionada à saúde nos domínios Capacidade funcional , Aspectos físicos , Aspectos emocionais e Dor . Conclusão: Os resultados do estudo permitiram compreender as diferentes dimensões e especificidades no contexto da qualidade de vida relacionada à saúde e evidenciou que as esferas públicas precisam redirecionar o olhar para reconhecer os problemas enfrentados pela família quando indivíduos são acometidos por uma doença incapacitante como o AVE.

An Educational Module on High Blood Pressure Management and Control

An educational module, based on evidence-based practice (EBP) guidelines by the 8th Joint National Committee (JNC 8) and the American College of Cardiology (ACC), was created and implemented to determine if its implementation would impact the knowledge of clinic staff regarding current EBP guidelines about self-management of hypertension (HTN). The module has the potential to contribute to the resolution of patient noncompliance on HTN treatment and management by increasing nursing staff proficiency in knowledge transfer to patients on effective self-management of their health condition. The creation of the module was guided by the concept of need or asset assessment and the theory of planned behavior. The module was composed of 2 short lecture presentations on HTN, current EBP guidelines on lifestyle modifications, and proper blood pressure measurement. The module also included pre- and postlecture surveys to evaluate knowledge and practices of staff, and reiterated the current guidelines and approaches presented in the lectures. Survey data were analyzed using McNemar’s test for paired and binary data. Results showed the agreement of all the staff in recognizing the utility of the module in standardizing their knowledge of current EBP guidelines on lifestyle modifications and blood pressure measurement procedures. The results also showed the enhancement of staff proficiency which might lead to efficient education of patients on effective HTN treatment and management protocol. This pathway has the potential to bring about social change by decreasing the incidence of patient noncompliance and improving patient health.

PCA and CVA biplots : a study of their underlying theory and quality measures

Thesis (MComm)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: The main topics of study in this thesis are the Principal Component Analysis (PCA) and Canonical Variate Analysis (CVA) biplots, with the primary focus falling on the quality measures associated with these biplots. A detailed study of different routes along which PCA and CVA can be derived precedes the study of the PCA biplot and CVA biplot respectively. Different perspectives on PCA and CVA highlight different aspects of the theory that underlie PCA and CVA biplots respectively and so contribute to a more solid understanding of these biplots and their interpretation. PCA is studied via the routes followed by Pearson (1901) and Hotelling (1933). CVA is studied from the perspectives of Linear Discriminant Analysis, Canonical Correlation Analysis as well as a two-step approach introduced in Gower et al. (2011). The close relationship between CVA and Multivariate Analysis of Variance (MANOVA) also receives some attention. An explanation of the construction of the PCA biplot is provided subsequent to the study of PCA. Thereafter follows an in depth investigation of quality measures of the PCA biplot as well as the relationships between these quality measures. Specific attention is given to the effect of standardisation on the PCA biplot and its quality measures. Following the study of CVA is an explanation of the construction of the weighted CVA biplot as well as two different unweighted CVA biplots based on the two-step approach to CVA. Specific attention is given to the effect of accounting for group sizes in the construction of the CVA biplot on the representation of the group structure underlying a data set. It was found that larger groups tend to be better separated from other groups in the weighted CVA biplot than in the corresponding unweighted CVA biplots. Similarly it was found that smaller groups tend to be separated to a greater extent from other groups in the unweighted CVA biplots than in the corresponding weighted CVA biplot. A detailed investigation of previously defined quality measures of the CVA biplot follows the study of the CVA biplot. It was found that the accuracy with which the group centroids of larger groups are approximated in the weighted CVA biplot is usually higher than that in the corresponding unweighted CVA biplots. Three new quality measures that assess that accuracy of the Pythagorean distances in the CVA biplot are also defined. These quality measures assess the accuracy of the Pythagorean distances between the group centroids, the Pythagorean distances between the individual samples and the Pythagorean distances between the individual samples and group centroids in the CVA biplot respectively. / AFRIKAANSE OPSOMMING: Die hoofonderwerpe van studie in hierdie tesis is die Hoofkomponent Analise (HKA) bistipping asook die Kanoniese Veranderlike Analise (KVA) bistipping met die primêre fokus op die kwaliteitsmaatstawwe wat daarmee geassosieer word. ’n Gedetailleerde studie van verskillende roetes waarlangs HKA en KVA afgelei kan word, gaan die studie van die HKA en KVA bistippings respektiewelik vooraf. Daar is gevind dat die akkuraatheid waarmee die groepsgemiddeldes van groter groepe benader word in die geweegde KVA bistipping, gewoonlik hoër is as in die ooreenstemmende ongeweegde KVA bistippings. Drie nuwe kwaliteitsmaatstawe wat die akkuraatheid van die Pythagoras-afstande in die KVA bistipping meet, word gedefinieer. Hierdie kwaliteitsmaatstawe beskryf onderskeidelik die akkuraatheid van die voorstelling van die Pythagoras-afstande tussen die groepsgemiddeldes, die Pythagoras-afstande tussen die individuele observasies en die Pythagoras-afstande tussen die individuele observasies en groepsgemiddeldes in die KVA bistipping.

The connections between values alignment and teamwork : A case study

Background: Research today highlights the importance of values alignment, but all of the research has been conducted on manufacturing companies. We found it interesting to investigate if the same theories are applicable to the knowledge-intense service sector, such as consultancy firms. Furthermore, research has identified a set of key attributes of successful teamwork, which we find closely related to values alignment. Despite this, we found no previous research within this area. Purpose: This study investigates the connections between values alignment and teamwork at a consultancy firm. Method: Our research was conducted as a case study at the department of Management at a consultancy firm. A survey, based on Barrett’s (2008) Seven Level Consciousness, and Cultural Values Assessment models, was carried out. However, we have modified the model to fit Swedish work-related values. We conducted pre-survey, and follow-up survey interviews with employees at the consultancy firm, as well as an interview with a former employee. This triangulation of information gathering techniques allowed us to ensure the validity of our study. Conclusion: We found a set of four clear relations between values alignment and, what previous research has identified as, key success factors of successful teamwork. Commitment, guidance in decision making, quality of outcome and reduced interpersonal conflicts are all outcomes of values alignment, and they are in different ways connected to one or several of commitment to success and shared goals, commitment to team processes and accountability, interpersonal skills, open communication and feedback or interdependencies. / Bakgrund: Dagens forskning belyser vikten av att ha en gemensam värdegrund, men så vitt vi kan se sträcker sig forskningen till att enbart involvera tillverkningsföretag. Vi tyckte att bristen på forskning inom kunskapsintensiva serviceföretag, så som konsultföretag, var intrigerande och bestämde oss för att undersöka huruvida befintliga teorier är applicerbara även på den här sektorn. Forskning har även identifierat ett antal faktorer som kännetecknar framgångsrikt teamwork, vi anser att dessa faktorer är kopplade till att ha en gemensam värdegrund. Trots detta har vi inte hittat någon tidigare forskning inom det här området. Syfte: Den här uppsatsen undersöker sambandet mellan att ha en gemensam värdegrund och teamwork på ett konsultföretag. Vi har dock anpassat enkäten till att stämma överens med svenska arbetsrelaterade värderingar. Förutom detta genomförde vi intervjuer innan enkäten skickades ut, samt uppföljningsintervjuer. Utöver de intervjuerna, gjorde vi även en intervju med en före detta anställd på företaget. Den här trianguleringen gjorde det möjligt för oss att försäkra validiteten för vår undersökning. Slutsats: Vi fann fyra tydliga kopplingar mellan fördelarna med att ha gemensam värdegrund, och vad som genom tidigare forskning har identifierats som framgångsfaktorer för teamwork. The connections between values alignment and teamwork : A case study

<p><strong>Background: </strong>Research today highlights the importance of values alignment, but all of the research has been conducted on manufacturing companies. We found it interesting to investigate if the same theories are applicable to the knowledge-intense service sector, such as consultancy firms. Furthermore, research has identified a set of key attributes of successful teamwork, which we find closely related to values alignment. Despite this, we found no previous research within this area.</p><p><strong>Purpose:</strong> This study investigates the connections between values alignment and teamwork at a consultancy firm.</p><p><strong>Method:</strong> Our research was conducted as a case study at the department of Management at a consultancy firm. A survey, based on Barrett’s (2008) Seven Level Consciousness, and Cultural Values Assessment models, was carried out. However, we have modified the model to fit Swedish work-related values. We conducted pre-survey, and follow-up survey interviews with employees at the consultancy firm, as well as an interview with a former employee.
