Bio-inspired structured composites for load-bearing bone graft substitution

Natural composites, in particular nacre, often combine high strength and toughness thanks to highly ordered architectures and controlled geometries of the reinforcement components. However, combining strength, toughness and resorbability in synthetic materials remains a challenge in particular in the field of bone graft substitutes. In the present study, calcium phosphate-(CaP-)based materials with designed architectures inspired from natural composite materials were achieved. CaP platelets obtained by precipitation in organic medium were first aligned in chitosan matrices by solvent casting in ambient conditions. Efficient strengthening was obtained with 15 vol-% ceramic, reaching cortical bone strength (150 MPa) and preserving good ductility (5 % deformation). In a weak magnetic field, high spatial arrangement without percolation was maintained up to 20 vol-%. With directional freezing, good alignment of the platelets could be pushed up to 50 vol-%. In parallel, in situ recrystallization of CaP blocks in hydrothermal conditions led to hierarchical structures. The strength and the work-of-fracture were enhanced (300%) thanks to a change of failure mode.

Dendritische Glykopolymere und deren Polyelektrolytkomplexe als effiziente Drug-Delivery-Systeme für die verzögerte Wirkstofffreisetzung aus Calciumphosphatzement

Das multiple Myelom ist eine seltene maligne Knochenerkrankung bei insbesondere älteren Menschen. Dabei vermehren sich im Knochenmark in hohem Maße unkontrolliert entartete Plasmazellen. Diese Myelomzellen unterdrücken einerseits die Bildung von normalen Plasmazellen, andererseits wird das Gleichgewicht zwischen Knochenaufbau und –abbau empfindlich gestört, woraus eine erhöhte Knochenresorption resultiert. Neben den bisher angewandten Chemo- und Strahlentherapien gewinnen innovative Medikamente, wie Proteasominhibitoren und Bisphosphonate, in der Therapie an Bedeutung. Diese Medikamente reduzieren das Myelomzellwachstum und wirken hemmend auf den Knochenabbau. Durch das Auffüllen von durch Resorptionsprozesse geschädigten Knochendefekten mit wirkstoffbeladenen Calciumphosphatzementen (CPC) wird nicht nur der Knochen stabilisiert, sondern im Vergleich zur herkömmlichen oralen oder intravenösen Medikamentenverabreichung eine gezielte Freisetzung des Wirkstoffes direkt am Wirkort in wesentlich reduzierten Dosen ermöglicht. Durch die Kombination des Knochenzementes mit anderen effizienten Drug-Delivery-Systemen (DDS), wie z. B. Polymeren, kann eine optimale Anpassung der Wirkstofffreisetzung ermöglicht werden. Insbesondere haben sich bereits dendritische Polymere aufgrund ihrer globularen Struktur und Vielzahl an peripheren Funktionalitäten als besonders geeignete Wirkstoffträgersysteme herausgestellt. Bei der Anwendung im physiologischen System spielt insbesondere die Biokompatibilität dieser polymeren DDS eine entscheidende Rolle. Durch Modifizierung der peripheren Gruppen mit biokompatiblen Einheiten, wie Oligosacchariden oder Aminosäuren, kann die physiologische Verträglichkeit signifikant erhöht werden. Für die Behandlung des multiplen Myeloms am Knochen sollte in dieser Arbeit ein geeignetes dendritisches DDS auf Basis von hochverzweigtem Polyethylenimin (PEI) synthetisiert und charakterisiert werden. Das DDS sollte dabei verschiedene Anforderungen, wie eine hohe Wasserlöslichkeit und Biokompatibilität, erfüllen. Weiterhin sollten die mechanischen Eigenschaften des CPC nicht negativ beeinflusst werden und der Wirkstoff sollte effektiv vom DDS aufgenommen und kontrolliert aus dem generierten Komposit (Wirkstoff/DDS/CPC) freigesetzt werden. In der sogenannten N-Carboxyanhydrid (NCA)-Polymerisation wurden am PEI(5) (5 ≙ Mw 5 kDa) benzylgeschützte Polyglutaminsäure bzw. Polyasparaginsäureketten aufgepfropft. Durch hydrolytische Abspaltung der Schutzgruppen an den PBLG-Ketten von PEI(5)-PBLG-346 und PEI(5)-PBLA-346 erfolgte die Generierung der wasserlöslichen DDS PEI(5)-PGlu-346 und PEI(5)-PAsp-346. Die Charakterisierung der synthetisierten Kern-Schale-Architekturen PEI(5)-PBLG-346, PEI(5)-PBLA-346, PEI(5)-PGlu-346 und PEI(5)-PAsp-346 zeigte, dass nur wenige lange Polyaminosäureketten an wenigen primären und sekundären Aminogruppen des PEI(5) aufgebaut wurden. Aufgrund der noch freien primären und sekundären Aminogruppen am PEI(5) und den peripheren Aminogruppen an den Polyaminosäureketten wurden durch die Anbindung von Maltose- bzw. Laktoseeinheiten Kern-Schale-Architekturen mit einer binären Doppelschalenstrukturen erzeugt. Im Gegensatz zu reiner Polyglutaminsäure zeigten die mit Glutaminsäure modifizierten Polymerstrukturen PEI(5)-PGlu-346 und PEI(5)-PGlu-346-Mal interessante strukturelle Eigenschaften in wässriger Umgebung. Aufgrund des pH-abhängigen Ladungszustandes resultiert bei reinen Polyglutaminsäureketten normalerweise der typische Helix-Coil-Übergang. Dabei findet eine Konformationsumwandlung der α-helikalen Struktur zur ungeordneten Sekundärstruktur statt. Im Falle der PEI(5)-PGlu-346- und PEI(5) PGlu-346-Mal-Copolymere wurde jedoch keine α-helikale Konformation bei niedrigem pH-Wert nachgewiesen. Die PGlu-Ketten der wasserlöslichen Kern-Schale-Architekturen bildeten sowohl im sauren, als auch im basischen pH-Wertbereich eine ungeordnete Sekundärstruktur aus. Zusätzlich konnte nachgewiesen werden, dass die Kern-Schale-Architekturen in Abhängigkeit vom pH-Wert als isolierte Makromoleküle bzw. Aggregate mit unterschiedlich lang gestreckten Peptidketten vorliegen. Die Ursache dafür sind nicht-kovalente, intra- und intermolekular wirkende Kräfte. Zur Beurteilung der Kern-Schale-Architekturen als geeignete DDS wurde die Komplexierung des Proteasominhibitors Bortezomib (BZM) in die reinen Copolymere PEI(5)-PGlu-346, PEI(5)-PGlu-346-Mal und PEI(25)-Mal B (25 ≙ Mw 25 kDa, ohne Polyglutaminsäureketten) sowie deren Polyelektrolytkomplexe untersucht. Dabei wurden Copolymer/BZM- bzw. PEK/BZM-Komplexe in verschiedenen Verhältnissen hergestellt und die Komplexierungskapazität durch zeitabhängige Ultrafiltration UV/Vis-spektroskopisch ermittelt. Im Vergleich zu den glutaminsäuremodifizierten Copolymeren wurde durch PEI(25)-Mal B etwa doppelt so viel Wirkstoff in verschiedenen wässrigen Systemen aufgenommen. Der Grund dafür ist der größere PEI-Kern und die dementsprechend höhere Anzahl an peripheren Aminogruppen mit gebundenen Maltoseeinheiten. Die PEK zeigten im Vergleich zu den Copolymeren keine Verbesserung der Komplexierungskapazität. Um eine effektive Wirkstofffreisetzung für eine dosierte Langzeittherapie aus dem Kompositmaterial zu erhalten, ist eine stark verzögerte Freisetzung des Copolymers bzw. PEK selbst aus dem CPC notwendig. In Abhängigkeit von der Konzentration wurde für PEI(25)-Mal B eine geringere Freisetzung aus dem Copolymer/CPC- und PEK/CPC ermittelt. Aufgrund der nanoskaligen Dimension der polymeren Strukturen wird die Diffusion durch das offene CPC-Porensystem erschwert. Für die PEI(5)-PGlu-346, PEI(5)-PGlu-346-Mal und die zugehörigen PEK wurde hingegen keine messbare Freisetzung aus dem CPC nachgewiesen. Die Glutaminsäureeinheiten können Calciumionen komplexieren und beeinflussen dadurch die Keimbildung und das Wachstum der CaP-Phase. Die Copolymerstrukturen werden somit in den CPC integriert und können nur durch Abbau des schwerlöslichen Zementes freigesetzt werden. Bei den Untersuchungen der BZM-Freisetzung aus den BZM/Copolymer/CPC- und BZM/PEK/CPC-Kompositen kristallisierte sich BZM/PEI(5)-PGlu-346-Mal/CPC als effektivstes DDS heraus. Im Vergleich zum reinen BZM in CPC wurde nach 24 h nur etwa die Hälfte des Wirkstoffes aus dem Komposit freigesetzt. Weiterhin steigerte sich die Freisetzungsrate über den gesamten Zeitraum von 14 Tagen auf nur etwa 60 %. Aus dem BZM/CPC-Komposit wurden nach 14 Tagen mehr als 75 % BZM freigesetzt. In Kooperation mit der Arbeitsgruppe von Prof. Michael Gelinsky vom Zentrum für Translationale Knochen-, Gelenk- und Weichgewebeforschung (TU Dresden) wurde keine signifikante Änderung der Druckfestigkeit des CPC durch die Integration der glutaminsäuremodifizierten Copolymere festgestellt. Weiterhin wurde in in vitro-Untersuchungen mit osteogen stimulierten humanen mesenchymalen Stammzellen (hMSC) kein entscheidender Einfluss der in dieser Arbeit hergestellten PEI(5)-PGlu-346- und PEI(5)-PGlu-346-Mal-Copolymere auf die Proliferation der Zellen beobachtet. Zudem war bei beiden Copolymeren eine osteogene Differenzierung der hMSC zu knochenbildenden Osteoblasten nachweisbar, wobei PEI(5)-PGlu-346-Mal die Entwicklung der Stammzellen zu knochenbildenden Zellen sogar zu fördern scheint. Durch die Kombination von hochverzweigtem PEI mit Polyglutaminsäure und Maltose wurde in dieser Arbeit ein innovatives DDS für die kontrollierte und effektiv verzögerte Freisetzung von BZM aus CPC erzeugt, welches die einleitend erwähnten Anforderungen erfüllt. Das Copolymersystem weist eine hohe Biokompatibilität auf, ohne die mechanischen Eigenschaften des CPC zu verändern. Diese Arbeit hat daher einen entscheidenden Beitrag im Bereich der Wirkstofffreisetzung aus festen Materialien geliefert und bildet die Grundlage für zukünftige polymere DDS in CPC.

Avaliação histomorfométrica da neoformação óssea de enxertos de fosfato de cálcio amorfo e de hidroxiapatita em alvéolos dentais de ratos / Histomorphometric evaluation of bone formation of amorphous calcium phosphate and hydroxyapatite grafts in dental alveoli of rats

Apesar do caráter cada vez mais preventivo, a Odontologia ainda lida com a realidade terapêutica de reabilitação de uma população cujo mais grave problema de saúde bucal é a ausência dental. A implantodontia apareceu como uma opção para essas reabilitações, porém, não raro, se vê diante de reabsorções ósseas que inviabilizam a instalação direta de implantes. A utilização de enxertos ósseos é o grande recurso para essas áreas. Os enxertos autógenos são uma excelente opção, mas vêm acompanhados de alguns inconvenientes como maior morbidade e risco de infecção, maior tempo de recuperação e maior custo. Esses dificultadores têm contribuído para o desenvolvimento de materiais alternativos. Um desses materiais é o fosfato de cálcio amorfo (FCA), cuja forma cristalina é a hidroxiapatita (HA). Nesse estudo utilizou-se de uma inédita apresentação do mesmo, em forma filamentosa, para avaliar a neoformação óssea na reparação alveolar. A hidroxiapatita em grânulos foi utilizada como referência comparativa. Foram utilizados 44 animais da espécie Rattus Novergicus albinus, linhagem Wistar, machos e adultos. Estes foram submetidos à exodontia do incisivo superior cuja loja alveolar foi imediatamente preenchida com um biomaterial, a saber: FCA, HA e FCAHA (mistura de 50%-50% de cada um deles). Ao término de 28 dias foram sacrificados e seus alvéolos analisados em microscopia de luz convencional quantificando a formação de coágulo, a neoformação óssea, a deposição de tecido osteóide e a presença de vasos no tecido de reparação. As células típicas do tecido ósseo se mostraram presentes. Osteoblastos e osteoclastos na superfície das trabéculas e profusão de fibroblastos no tecido ósseo imaturo, ocupando os espaços intertrabeculares. A deposição de tecido osteóide e de tecido ósseo neoformado apresentou-se em todos os grupos, porém com diferenças em relação à sua distribuição. O grupo controle apresentou reparação óssea compatível com o clássico descrito em literatura, ou seja, de maneira centrípeta. Também foi evidenciada a presença de um coágulo central ou paracentral em relação ao alvéolo. No grupo FCA a quantidade de tecido ósseo neoformado era mais evidente do que nos demais. Já no grupo HA a presença dos cristais sugeriu uma concorrência espacial com a formação tecidual. No grupo FCAHA a deposição tecidual mostrou-se nitidamente irregular. Nos grupos onde foi utilizado o FCA a deposição de tecido ósseo neoformado foi superior àqueles onde esse biomaterial não estava presente. Nosso estudo sugere que o FCA tem uma característica osteoindutora mais evidente do que a HA. Justifica-se por, dentro do período estudado, verificar-se o maior depósito de tecido neoformado e a menor quantidade de tecido osteóide quando os comparamos com os outros grupos. Concluímos que o FCA apresentado na forma de filamentos comportou-se in vivo com características comparáveis a de outros materiais biocompatíveis, mostrando boa resposta osteoindutora, e uma reparação e neoformação mais rápida que a da HA / Despite it increasingly preventive character, dentistry also deals with the reality of therapeutic rehabilitation of a population whose most serious oral health problem is dental absence. Dental implant appeared as an option for these rehabilitations, however, often, is faced with bone resorption that prevent direct implant placement. The use of bone grafts is a great resource for those areas. Autogenous grafts are an excellent option, but comes accompanied by some drawbacks as higher morbidity and risk of infection, longer recovery time and higher cost. These have contributed to hindering the development of alternative materials. One such material is amorphous calcium phosphate (ACP), which is the crystalline form hydroxyapatite (HA). In this study, we used an unprecedented presentation of the same in filamentous form, to evaluate new bone formation in the alveolar repair. The hydroxyapatite granules was used as a comparative reference. 44 animals were used species Rattus Novergicus Albinus, Wistar, adult males. These were subjected to extraction of the upper incisor whose cellular store was immediately filled with a biomaterial, namely: ACP, and ACPHA (mixture of 50% -50% of each). At the end of 28 days they were sacrificed and their alveoli analyzed by conventional light microscopy quantifying clot formation, bone formation, deposition of osteoid tissue and the presence of vessels in tissue repair. The cells typical of bone tissue showed present. Osteoblasts and osteoclasts on the surface of trabecular and profusion of fibroblasts in the immature bone tissue, occupying the intertrabecular spaces. The deposition of osteoid tissue and newly formed bone appeared in all groups, but with differences regarding their distribution. The control group exhibited bone repair compatible with the classic described in the literature, centripetal manner. In addition, it revealed the presence of a central clot or para in relation to the alveolus. ACP group in the amount of newly formed bone was more evident than in others. Already in the HA group the presence of crystals suggested a space competition with tissue formation. The group ACPHA tissue deposition was shown to be clearly irregular. In groups which was used ACP deposition of newly formed bone was higher than those where this biomaterial were not present. Our study suggests that the ACP has clearer osteoinductive characteristic of the HA. Justified by, within the study period, check it the largest deposit of newly formed tissue and the least amount of osteoid tissue when compared with the other groups. We conclude that the ACP presented in the form of filaments behaved in vivo comparable characteristics of other biocompatible materials, showing good osteoinductive response, a faster repair and neogenesis the HA

Efeito in situ de uma goma de mascar contendo caseína fosfopeptídea - fosfato de cálcio amorfo (CPP-ACP) na erosão dentária / In situ effect of chewing gum containing casein phosphopeptide - amorphous calcium phosphate on dental erosion

O presente trabalho avaliou o efeito in situ de uma goma de mascar comercialmente disponível contendo caseína fosfopeptídea - fosfato de cálcio amorfo (CPP-ACP) na erosão dentária. Para a primeira etapa do estudo (capacidade remineralizadora) utilizaram-se 72 blocos de esmalte humano selecionados pela dureza de superfície (SHi) e erodidos in vitro pela imersão em Coca Cola®, pH 2,4 por 3 min (avaliação da dureza - SHd). Os blocos foram randomizados entre os grupos: GI Trident Fresh® (sem CPP-ACP), GII controle (sem chiclete) e GIII Trident Total® (com CPPACP). Doze voluntários utilizaram dispositivos intrabucais palatinos por 24 h em 3 fases cruzadas. Nas fases de GI e GIII os voluntários mascaram um chiclete (30 min) e em todas as fases após 2h, a dureza foi avaliada (SHf1). Os blocos foram reposicionados e os dispositivos usados por mais 22 h (+ 3 ciclos de mastigação de chiclete - GI e GIII). A dureza foi reavaliada (SHf2) para cálculo do percentual de recuperação de dureza (%SHR) após 2 e 24h. Na segunda etapa do estudo (ciclagem erosiva) 48 blocos de esmalte humano hígidos foram aleatorizados entre os grupos (GI, GII e GIII) e 8 voluntários utilizaram dispositivos intrabucais palatinos em fases cruzadas de 7 dias cada (washout de 7 dias). O protocolo de ciclagem erosiva foi de 4 imersões diárias do dispositivo intrabucal em 150 ml de Coca Cola® durante 5 min. Nos grupos I e III após cada desafio erosivo e reinserção do dispositivo na cavidade bucal, os voluntários mascaram um chiclete durante 30 min. A alteração da superfície do esmalte foi mensurada por perfilometria (&#x3BC;m). Os dados foram submetidos à ANOVA (2 critérios - etapa 1; 1 critério - etapa 2) e teste Tukey (&#x3B1;=0,05). Os resultados da recuperação de dureza demonstraram haver diferença significativa entre os grupos e os tempos (p<0,05). O Trident Total® (2h = 50,0%; 24h = 95,9%) promoveu maior recuperação de dureza que o Trident Fresh® (2h= 30,0%; 24h= 71,1%) e o grupo controle (2h = 15,7%; 24h = 40,9%). No desafio erosivo prolongado, o Trident Total® (5,2± 2,8 &#x3BC;m) e o Trident Fresh® (3,8 ± 1,5 &#x3BC;m) reduziram significativamente o desgaste dentário em relação ao grupo controle (6,8 ± 3,5 &#x3BC;m), no entanto, não houve diferença significativa entre os chicletes (p>0,05). Conclui-se que o efeito remineralizador da saliva foi maior após utilização de goma de mascar, sendo significativamente aumentado em função do período de remineralização e pela presença de CPP-ACP na goma de mascar. Contudo, para o protocolo de desafio erosivo adotado, apesar da goma de mascar convencional apresentar efeito na inibição do desgaste erosivo, o comportamento da goma de mascar com incorporação de CPP-ACP foi similar. / This study evaluated the in situ effect of a commercial chewing gum containing casein phosphopeptide - amorphous calcium phosphate (CPP-ACP) on dental erosion. On the first stage (remineralizing effect) 72 human enamel blocks, which were selected by surface hardness (SHi) and eroded in vitro by immersion in cola drink, pH 2,4 for 3 minutes (hardness evaluation - SHd) were used. Blocks were randomized into 3 groups: GI Trident Fresh® (conventional gum, without CPP-ACP), GII control (no gum) and GIII Trident Total® (with CPP-ACP). Twelve volunteers wore intraoral palatal devices for 24 h in 3 crossover phases. In phases of GI and GIII volunteers chewed a gum (30 min) and in all phases after 2h, the surface hardness was evaluated (SHf1). The blocks were reinserted and the devices used for additional 22h (+ 3 cycles of chewing gum - GI e GIII). The surface hardness was reassessed (SHf2) to calculate the percentage of surface hardness recovery (%SHR) after 2 and 24h. In the second stage (erosive cycling) 48 healthy human enamel blocks were randomized between groups (GI, GII and GIII) and 8 volunteers wore intraoral palatal devices in crossover phases of 7 days each (washout of 7 days). The cycling protocol consisted of 4 daily immersions of the intraoral device into 150 ml of cola drink for 5 min. In groups I and III after each erosive challenge and oral device reinsertion into oral cavity, the volunteers chewed a gum for 30 min. The enamel surface alterations were measured by profilometry (&#x3BC;m). Data were analyzed by Anova (Two way - stage 1, One way - stage 2) and Turkeys test (&#x3B1;=0,05). The results of percentage of surface hardness recovery showed significant differences for factors groups and time (p<0.05). Trident Total® (2h = 50.0%; 24h = 95.9%) showed higher percentage of surface hardness recovery than the Trident Fresh® (2h = 30.0%; 24h = 71.1%) and control group (2h = 15.7% 24h = 40.9%). In the prolonged erosive challenge, Trident Total® (5.2 ± 2.8 &#x3BC;m) and Trident Fresh® (3.8 ± 1.5 µm) significantly reduced tooth wear compared to control group (6.8 ± 3, 5 µm). However, there was no significant difference between chewing gums. It is concluded that the saliva remineralizing effect increased after the use of conventional chewing gum and was enhanced by prolonged period of remineralization and by the presence of CPPACP in the chewing gum. However for the adopted erosive challenge, despite the conventional chewing gum being able to diminish erosive wear, the chewing gum containing CPP-ACP showed similar effect.

Avaliação de novos tratamentos preventivos da erosão e abrasão do esmalte e da dentina / Evaluation of new preventive treatments for erosion and abrasion of enamel and dentin

Este trabalho, conduzido na forma de 2 subprojetos, avaliou: 1) o potencial preventivo de pastas à base de hidroxiapatita (nanopartículas de fosfato de cálcio) sobre a erosão e a abrasão do esmalte e dentina bovinos in vitro e in situ e 2) o efeito de um bochecho com solução de lactato de cálcio antes da escovação com dentifrício fluoretado sobre a erosão associada ou não à abrasão do esmalte e dentina bovinos in vitro e in situ. Na fase in vitro do 1o subprojeto, submeteram-se blocos de esmalte e dentina bovinos, por 5 dias, à erosão (coca-cola, pH 2,6, 4x/dia, 90s cada) + abrasão (escova elétrica + solução do dentifrício sem flúor, 10s cada, 2x/dia) e os tratamentos foram realizados após a abrasão, através da aplicação das seguintes pastas sobre os blocos (3 min, 2x/dia): 10% HAP, 10% HAP + 0,2% NaF, 20% HAP, 20% HAP + 0,2% NaF, 20% HAP + 2% NaF, placebo, 0,2% NaF, 2% NaF, MI paste, MI paste plus e controle. Na fase in situ, (4 fases, 5 dias/cada), 12 voluntários utilizaram dispositivo intrabucal palatino contendo 4 blocos de dentina e 4 de esmalte bovinos divididos nas condições: erosão e erosão+abrasão. Em cada fase foi feito tratamento com uma das seguintes pastas: 10% HAP, 10% HAP + 0,2% NaF, MI paste plus ou Placebo. O protocolo erosivo e abrasivo foi semelhante ao in vitro. No 2o subprojeto, a fase in vitro foi semelhante ao subprojeto 1, porém apresentou as condições erosão e erosão+abrasão. O estudo compreendeu os seguintes tratamentos: BCa + DF, BCa + DP, BP + DF e BP + DP. Após erosão, os blocos foram imersos em água deionizada (BP) ou solução de lactato de cálcio 150 mM (BCa; 1 min, sob agitação). Na condição erosão apenas, a solução de dentifrício fluoretado (DF) ou placebo (DP) foi pipetada sobre as amostras. A fase in situ foi semelhante ao experimento in vitro, porém com 15 voluntários e escovação por 15 s. A variável de resposta para os 2 subprojetos foi a análise de desgaste, avaliada por perfilometria (&#x3BC;m) e para a fase in situ do 1o subprojeto foi realizada também a análise por MEV-EDS. Os dados foram analisados pelos testes ANOVA/Tukey, Kruskal-Wallis/Dunn ou ANOVA a dois critérios/Bonferroni (p<0,05). In vitro, para o esmalte, as pastas à base de hidroxiapatita (10 e 20%), independentemente da presença do F, foram efetivas na prevenção da erosão associada à abrasão. Para a dentina, o efeito protetor foi encontrado apenas para as pastas contendo F, maior para aquelas com concentração mais elevada. In situ, apenas a pasta contendo 10% HAP foi efetiva para reduzir a erosão em esmalte quando comparada à pasta placebo. Nas demais condições testadas nenhuma das pastas utilizadas diferiu significativamente da placebo. Para o segundo subprojeto, o BCa + DF teve um efeito protetor apenas para a erosão de esmalte e dentina in vitro. Para a erosão associada à abrasão in vitro, assim como no estudo in situ, não houve diferença significativa entre os tratamentos. / This study, conducted in the form of two subprojects, evaluated: 1) the preventive potential of hydroxyapatite-based (calcium phosphate nanoparticles) pastes on erosion and abrasion of bovine enamel and dentin in vitro e in situ and 2) the effect of rinsing with a solution of calcium lactate before brushing with fluoride toothpaste on erosion associated or not with abrasion of bovine enamel and dentin in vitro e in situ. In the in vitro phase of the 1st subproject, bovine enamel and dentin blocks were submitted to erosion (coca-cola, pH 2.6, 4x/day, 90s each) + abrasion (electric toothbrush + fluoride-free toothpaste slurry, 10s each, 2x/day) for 5 days. Treatments were performed after abrasion, by applying the following pastes on the blocks (3 min, 2x/day): 10% HAP, 10% HAP + 0.2% NaF, 20% HAP, 20% HAP + 0.2% NaF, 20% HAP + 2% NaF, placebo, 0.2% NaF, 2% NaF, MI paste, MI paste plus and control. In the in situ phase (4 phases, 5 days each), 12 volunteers used intraoral palatal appliance containing four blocks of dentin and four blocks enamel divided into conditions erosion and erosion+abrasion. In each phase, treatment was done with one of the following pastes: 10% HAP, 10% HAP + 0.2% NaF, MI paste plus or Placebo. The erosive and abrasive protocol was similar to the in vitro phase. In the 2nd subproject, the in vitro phase was similar to subproject 1, but presented both conditions erosion and erosion + abrasion. The study included the following treatments: BCa + DF, BCa + DP, BP + DF e BP + DP. After erosion, the blocks were immersed in deionized water (BP) or calcium lactate solution 150 mM (BCa; 1 min, stirring). For the condition erosion only, the slurry of fluoride (DF) or placebo (DP) dentifrice was pipetted on the samples. The in situ phase was similar to the in vitro experiment, but 15 volunteers were included and brushing was conducted for 15 s. The response variable for the two subprojects was wear, evaluated by profilometry (&#x3BC;m). For the in situ phase of the 1st subproject, SEM-EDS was also performed. Data were analyzed by ANOVA/Tukey, Kruskal-Wallis/Dunn or two-way ANOVA/Bonferroni tests (p <0.05). In vitro , for enamel, 10 and 20% hydroxyapatitebased pastes, regardless the presence of F, were effective in preventing erosion associated with abrasion. For dentin, the protective effect was found only for Fcontaining paste and the effect was higher for those with higher F concentration. In situ, only pastes containing 10% HAP were effective in reducing enamel erosion as compared to the placebo paste. For the other tested conditions, none of the pastes used significantly differed from placebo. For the second subproject, the BCa + DF had a protective effect only for enamel and dentin erosion in vitro . For erosion associated with abrasion in vitro , as well as for the in situ study, no significant differences were detected among the treatments.

O objetivo do presente estudo foi quantificar, in vitro, a permeabilidade da dentina tratada com dois agentes anti-hiperestésicos à base de oxalato e um à base de fosfato de cálcio, por meio de testes de condutividade hidráulica e de espectroscopia de impedância eletroquímica e, ainda, determinar se espectroscopia de impedância eletroquímica pode ser considerada um método alternativo para avaliar a capacidade obstrutiva de agentes antihiperestésicos. Para o teste de condutividade hidráulica foram utilizados vinte quatro discos de dentina, obtidos de terceiros molares humanos extraídos, que foram divididos em três grupos de oito espécimes, correspondentes aos três materiais testados: gel Experimental, Sensi Kill e BisBlock. Outros dezoito discos foram selecionados para o teste de espectroscopia de impedância eletroquímica, sendo divididos em três grupos de seis espécimes, correspondentes aos três agentes anti-hiperestésicos citados anteriormente. Para ambos os testes as medidas foram realizadas nas seguintes condições: na presença de smear layer; após condicionamento com ácido fosfórico a 37% e após a aplicação dos materiais na superfície oclusal dos discos de dentina. Com base na análise estatística dos resultados, verificou-se, por meio dos dois métodos utilizados, que o gel Experimental e o BisBlock apresentaram efeitos estatisticamente semelhantes na redução da permeabilidade e foram significantemente mais eficientes que o Sensi Kill. Pode-se concluir que os agentes anti-hiperestésicos Experimental e BisBlock mostraram-se efetivos na redução da permeabilidade dentinária, pelos dois métodos de avaliação, e que o método de espectroscopia de impedância eletroquímica pode ser considerado um método alternativo para a avaliação da capacidade obstrutiva de agentes anti-hiperestésicos. / The aim of the present study was to quantify in vitro the dentin permeability of dentin treated with two potassium oxalate dentin desensitizers and one calcium phosphate agent through hydraulic conductance (Lp) and eletrochemical impedance spectroscopy (EIS) tests and, yet, to determine whether EIS may be considered a suitable alternative method to evaluate the occlusive capacity of the dentin desensitizers. For the hydraulic conductance, twenty-four dentin discs were obtained from extracted human third molars, which were divided into three groups of eight specimens, corresponding to the materials to be tested: Experimental gel, Sensi Kill and BisBlock. Other eighteen dentin discs were selected for theEIS and divided into three groups of six specimens corresponding to the three dentin desensitizers mentioned before. For both tests, the following conditions were measured: in the presence of smear layer, after 37% phosphoric acid etching and after the application of the tested materials in the occlusal surfaces of dentin discs. Based upon the statistical analysis of the results, it was demonstrated by both methods that Experimental gel and BisBlock performed statistically similarly in reducing dentin permeability and were significantly more efficient that Sensi Kill. It can be concluded that the dentin desensitizers Experimental e BisBlock were demonstrated to be effective in the reduction of dentin permeability by both experimental methods and that EIS may be considered as an alternative method for the evaluation of the obstructive capacity of dentin desensitizers.

Efeito in situ da utilização de uma goma de mascar com caseína fosfopeptídea fosfato de cálcio amorfo (CPP-ACP) previamente ao desafio erosivo inicial / Effect of CPP-ACP chewing gum previously to erosive demineralization

O efeito protetor da caseína fosfopeptídea fosfato de cálcio amorfo (CPP-ACP) contra a erosão dentária é controverso. Este estudo in situ teve como objetivo investigar a capacidade de uma goma de mascar com CPP-ACP em prevenir uma única desmineralização erosiva. Blocos de esmalte bovino (120) selecionados pela dureza superficial inicial foram divididos aleatoriamente entre os grupos: GI - goma de mascar com CPP-ACP, GII - goma de mascar sem CPP-ACP e GIII - controle negativo para avaliação do efeito protetor sem estimulação salivar (sem goma de mascar). Dezenove voluntários participaram do estudo durante três fases cruzadas de 2 h cada. Nas fases de GI e GII os voluntários usaram dispositivos intrabucais palatinos contendo 2 blocos de esmalte, durante 120 minutos e mascaram uma unidade da goma de mascar correspondente ao grupo nos últimos 30 minutos. No grupo controle os voluntários usaram o dispositivo intrabucal por 2h, sem uso de goma de mascar. Em cada fase, imediatamente após a utilização, os dispositivos intrabucais foram imersos em refrigerante tipo cola durante 5 minutos para promover a desmineralização erosiva. A dureza superficial final foi mensurada e os valores foram utilizados para o cálculo do percentual de perda de dureza. Os dados foram analisados por ANOVA de medidas repetidas e teste Tukey (&#x3B1; = 5%). Menor perda de dureza do esmalte foi encontrada após a utilização de goma de mascar com (GI - 32,7%) e sem (GII - 33,5%) CPP-ACP relação ao efeito salivar sem estimulação (GIII - 39,8%) (p <0,05). Não houve diferença entre GI e GII (p> 0,05). Os resultados sugerem que a utilização de goma de mascar imediatamente antes de uma desmineralização erosiva é capaz de diminuir a perda de dureza do esmalte. No entanto, a presença de CPP-ACP na goma de mascar não foi capaz de melhorar este efeito. / The erosion-protective effect of CPP-ACP is controversial. This in situ study aimed to investigate the ability of CPP-ACP chewing gum to prevent a single event of erosive demineralization. Bovine enamel blocks (120), after selection (initial surface hardness) were randomly assigned to groups: GI-chewing gum with CPP-ACP, GIIchewing gum without CPP-ACP and Control group-salivary effect without stimulation (no gum). Nineteen volunteers participated on this study during 3 crossover phases of 2 h. On GI and GII, the volunteers wore intraoral palatal devices for 120 min and chewed a unit of the corresponding chewing gum on the last 30 min. On Control group the volunteer wore the appliance for 2 h, without chewing gum. On each phase immediately after the intraoral use, devices were extra orally immersed in cola drink for 5 minutes to promote erosive demineralization. The percentage of surface hardness loss was calculated. The data were analyzed by Repeated Measures ANOVA and Turkeys test. Less enamel hardness loss was found after the use of chewing gum with (GI-32.7 %) and without (GII-33.5%) CPP-ACP when compared to salivary effect without stimulation (control- 39.8%) (p<0.05). There was no difference between GI and GII (p>0.05). The results suggest that the use of chewing gum immediately before an erosive demineralization is able to diminish the enamel hardness loss. However, the presence of CPP-ACP in the chewing gum cannot enhance this protective effect.

Étude des propriétés physico-chimiques et biologiques de ciments biomédicaux à base de carbonate de calcium : apport du procédé de co-broyage / Study of physico-chemical and biological properties of biomedical calcium carbonate based cements : contribution of the co-grinding process

L'implantation de matériaux pour reconstruction osseuse par des techniques chirurgicales peu invasives nécessite des substituts osseux synthétiques, résorbables, injectables et radioopaques. C'est pourquoi le contrôle des propriétés de ces matériaux est primordial. Dans ce contexte, ce travail s'intéresse à la formulation de deux ciments, l'un uniquement à base de carbonate de calcium, le second composé d'un mélange de carbonate de calcium et de phosphate de calcium en quantités égales. Le traitement des phases solides pulvérulentes de ces deux ciments par les procédés de broyage et de co-broyage a été étudié. Ces procédés permettent entre autres de diminuer la taille moyenne des particules. Un mélange intime et homogène entre les constituants de la phase solide est obtenu grâce au co-broyage et les propriétés des ciments sont très significativement améliorées. Le temps de prise est diminué et l'injectabilité de la pâte est fortement augmentée (facteur 100). Cette dernière propriété a pu être évaluée grâce à la mise au point d'un dispositif et d'un protocole de mesure adaptés à un analyseur de texture. Dans le but de visualiser par radiographie aux rayons X l'introduction du ciment injectable dans le site osseux à réparer, du strontium a été introduit en tant qu'agent de contraste radio-opacifiant. Deux voies d'ajout à la formulation du ciment ont été étudiées : la première sous forme de SrCO3 dans la phase solide, la seconde sous forme de SrCl2 dans la phase liquide. Les études réalisées montrent que le co-broyage de la phase solide contenant du SrCO3 est intéressant pour homogénéiser la dispersion de l'agent de contraste et ainsi optimiser la quantité de strontium à incorporer pour atteindre le niveau de radio-opacité requis par la norme en vigueur. De plus, il a été observé que l'ajout de SrCl2 dans la phase liquide rend la pâte plus visqueuse et diminue son injectabilité. Par ailleurs, l'étude de la dissolution de ces ciments à pH physiologique a révélé une libération lente et prolongée du strontium. Enfin, des tests cellulaires in-vitro ont été réalisés sur ces ciments ; ils mettent en évidence l'excellent comportement de cellules ostéoprogénitrices vis-à-vis de ces formulations de ciment ainsi que l'intérêt d'utiliser le sel de SrCO3 plutôt que de SrCl2. La dernière partie de ce travail concerne l'étude de la cristallisation de l'aragonite, variété polymorphe du carbonate de calcium, en présence d'ions phosphate, connus pour inhiber la cristallisation du CaCO3. Grâce à une modélisation à l'aide de la technique de croissance cristalline à composition constante permettant de se placer dans des conditions proches de celles de la prise du ciment uniquement à base de carbonate de calcium in-vivo, il a été montré que la présence d'ions phosphate, même en très faible quantité (concentration &lt; 0,5 µM) diminue à la fois la vitesse de germination et la vitesse de croissance cristalline de l'aragonite. L'ensemble de ces travaux contribue à l'optimisation des propriétés de ces ciments biomédicaux et à mieux appréhender leur comportement que ce soit au moment de leur implantation in-vivo ou de leur évolution et suivi post-opératoires. D'un point de vue fondamental, ces travaux pluridisciplinaires menés dans des conditions modèles in-vitro mais également dans le cadre d'une expérimentation in-vivo ont mis en évidence l'intérêt de confronter ces deux approches pour identifier et comprendre les phénomènes et les réactions impliqués lors de la prise des ciments à base de carbonate de calcium in-vitro et in-vivo. / Implantation of bone substitute materials using minimally invasive surgical techniques requires specific properties for the material including resorbability, injectability and adequate radio-opacity. The control of such properties of the material is of prime importance to meet a surgeon's requirements. In this context, this study deals with two different mineral cements: the first one is only composed of calcium carbonate phases and the second one is a mixture of equal amount of calcium phosphate and calcium carbonate phases. An original methodology involving complementary analytical techniques was implemented to thoroughly investigate the grinding mechanism of separated or mixed reactive powders constituting the solid phase and its effects on cement reactivity and properties. We show that co-grinding the solid phase decreases the mean size of the particles and favours both a homogeneous mixing and good contact between the components, leading to a decrease in the setting time. We also set two original protocols designed to evaluate paste injectability and phase separation during paste extrusion. Co-grinding leads to synergistic positive effects on cement injectability and radio-opacity. It allows maintaining a low and constant load during the extrusion of paste, which composition remains constant. Moreover, the cement's mechanical properties can be enhanced by lowering the L/S ratio because of the lower plastic limit. To be able to follow in situ the injection of the bone cement using X-ray radiography, strontium has been introduced as a contrast agent in the cement composition. Two different routes have been investigated: SrCO3 has been added to the solid phase or SrCl2 has been dissolved in the liquid phase. We show that co-grinding process permits to homogenise strontium distribution in the cement allowing us to optimise the minimum amount of strontium to add into the cement paste to reach the radio-opacity required by ISO 9917-1 standard. Moreover, adding SrCl2 in the liquid phase makes the cement paste more viscous and diminishes its injectability. Release tests performed on Sr-loaded cements show a sustained release of strontium at 37°C and pH 7.4. Finally, in-vitro cell tests have shown the excellent behaviour of osteoprogenitor cells, especially on cements including SrCO3. The last part of this work deals with the study of the crystallization of aragonite CaCO3 in the presence of phosphate ions, naturally present in biological fluids, to better understand the setting ability of calcium carbonate cements in-vivo. Using the constant composition crystal growth technique, we show that the presence of phosphate ions, even in very low amount (concentration &lt; 0.5 µM) diminishes both the nucleation and the crystal growth rates of aragonite. This work contributes to the optimization of the properties of calcium carbonate-based cements and a better understanding and control of their behaviours during implantation and their evolution in-vivo. From a fundamental point of view, this multidisciplinary work performed in model conditions in-vitro and completed by preliminary in-vivo experiments have underlined the interest in combining these two approaches to identify and understand the phenomena and the chemical reactions involved during the setting of biomedical cements.

Isolatings were performed from rhizosphere of vegetables and soil after harvest of cultivated species. The vast majority of the strains showed biochemical profiles identical to Pseudomonas (fluorescent group). While profiles identical to Burkholderia, Stenotrophomonas and Acinetobacter constituted other groups of representatives. Characterization of isolates took into account the capacity for synthesis of compounds possibly involved in mechanisms to stimulate plant growth, namely: biofilms, siderophores, acyl homoserine lactones (AHLs), indole-3-acetic acid (IAA) and indole related compounds, as well as solubilization of calcium phosphate. A total of 94 isolates were evaluated in the greenhouse for their ability to promote the growth of cabbage (Brassica oleracea var. acephala) in pots containing unsterilized loam soil. In addition, consortia were evaluated between components of the collection and rhizobia for the same purpose. In addition, it compared the effects of application of certain producing IAA isolates and the application of synthetic IAA on the growth of cabbage. Laboratory experiments showed that in vitro, 100% of the isolates formed biofilm, 71% synthesized siderophores, 31% produced AHLs, 95% produced IAA and 74% solubilized P. However, bioassays in the greenhouse showed that only 12% of the isolates induced significant responses to promote the growth of cabbage. Of these, a subgroup composed of 10 strains of rhizobacteria given increments in growth parameters of both the biomass and the root system of vegetables. Biofilms, siderophores, IAA and solubilization of P were common characteristics of the subgroup. No synergistic effect was observed among isolates in consortia, although it was found that the two strains of rhizobia consortium promoted the growth of inoculated plants. The isolates R1132 and S311 were higher than the application of synthetic IAA. In-vitro degradation of calcium phosphate bone substitutes : Coupled monitoring of the evolution of mechanical, microstructural and physico-chemical properties of DCPD and β-TCP samples / Dégradation in-vitro de substituts osseux à base de phosphates de calcium : Suivi couplé de l’évolution des propriétés mécaniques, microstructurales et physico-chimiques d’échantillons de DCPD et β-TCP

Ce travail de thèse a eu comme objectif la mise en place et la validation d’une méthodologie expérimentale pour le suivi de l’évolution in-vitro de substituts osseux à base de phosphates de calcium. Du phosphate dicalcique dihydraté (DCPD, CaHPO4·2H2O) et du phosphate tricalcique bêta (β-TCP, β-Ca3(PO4)2) ont été choisis comme matériaux modèles de deux grandes classes de substituts osseux: les “biosolubles” (sujets à dissolution après implantation) et les “biorésorbables” (sujets à résorption cellulaire après implantation). Pour l’étude des phénomènes de dissolution et de reprécipitation observés lorsque les phosphates de calcium sont plongés en solution, ces matériaux ont été produits sous forme d’échantillons microporeux (60% de porosité pour le DCPD, 75% pour le β-TCP) et soumis à des tests de dissolution in-vitro en conditions statiques ou dynamiques (sans ou avec renouvellement du liquide) dans différentes solutions tamponnées à pH physiologique (TRIS et PBS) et pour des durées s’étalant entre 30 minutes et 2 mois. L’analyse des propriétés physico-chimiques, microstructurales et mécaniques des échantillons avant et après immersion a permis d’évaluer l’influence du type de milieu et des conditions de test choisies sur l’évolution des échantillons. Une attention particulière a été prêtée à la caractérisation mécanique: la technique de micro-indentation instrumentée sphérique a été préférée à autres essais plus conventionnels. Cette technique permet d’évaluer plusieurs paramètres tels que la dureté et le module de Young de façon quasi non-destructive et à une échelle locale. En conséquence, l’utilisation de la micro-indentation s’est avérée d’une grande aide pour le suivi des caractéristiques d’échantillons dégradés qui présentaient un gradient de propriétés entre la surface (où le processus de dégradation commence) et le cœur (sujet à des changements sur plus long terme). La dernière partie de cette étude a été dédiée à l’étude du deuxième phénomène qui entraine la résorption de substituts osseux in-vivo, à savoir la résorption cellulaire. Pour cela des essais cellulaires avec des cellules précurseurs d’ostéoclastes ont été réalisés sur des échantillons denses de β-TCP pur ou dopé avec 5% molaire de magnésium. Les résultats des tests ont confirmé la cytocompatibilité des deux types de β-TCP, mais ont également mis en avant une difficulté d’activation des ostéoclastes. Deux des causes possibles seraient liées à la topographie de surface des échantillons et au relargage des ions calcium suite à la dissolution du matériau. / The present Ph.D. thesis work was aimed to establish and assess an experimental methodology to monitor the in-vitro evolution of calcium phosphate (CaP) bone substitutes. Dicalcium phosphate dihydrate and beta-tricalcium phosphate were chosen as model of two main classes of bone substitutes: “biosoluble” ones (which undergo dissolution after implantation) and “bioresorbable” ones (which undergo cellular resorption after implantation). In order to study the dissolution and precipitation phenomena, which take place once CaPs are immersed in solution, these materials were produced in the form of micro-porous samples (60% of porosity for DCPD, 75% for β-TCP) and used for dissolution tests in-vitro in static and dynamic conditions (without or with liquid renewal) in different buffered solutions at physiologic pH (TRIS and PBS) and for periods of time ranging between 30 minutes and 2 months. The analysis of the physico-chemical, microstructural and mechanical properties of the samples before and after immersion allowed to evaluate the influence of the chosen medium and immersion conditions on the evolution of the specimens. Particular attention was paid to the mechanical characterisation: instrumented spherical micro-indentation was preferred to other more conventional tests. This technique enables the evaluation of several parameters such as the hardness and the Young’s modulus in a quasi-non-destructive way and on a local scale. As a consequence, the use of micro-indentation proved to be of great help for monitoring the characteristics of the degraded specimens, which presented a gradient of properties between the surface (where the degradation process starts) and the core (subject to changes on a longer period). The last part of this work was focused on the study of the second main phenomenon, which takes part in the in-vivo resorption of bone substitutes, that is to say the cellular resorption. For this purpose, cellular tests with osteoclast-precursor cells were carried out on dense samples made of pure and magnesium-doped β-TCP (5 mol.% of Mg). The addition of magnesium was aimed to modify the properties of the material and, as a consequence, the cellular behavior. The results confirmed the cytocompatibility of both types of β-TCP, but they also showed a difficult activation of osteoclasts. Two of the possible causes would be linked to the topography of the surface of the specimens and to the release of calcium ions due to the dissolution of the material.
