La prise en charge des personnes vivant avec le VIH âgées de 60 ans et plus en Ile-de-France : représentations et perspectives

Les personnes âgées infectées par le VIH ne sont plus des exceptions. Elles vieillissent avec cette maladie devenue chronique et, de surcroît, se découvrent de nouvelles pathologies liées à un phénomène de vieillissement prématuré. La question de leurs représentations pour les années à venir se pose en termes d’accompagnement et de lieux de vie. Par ailleurs, le regard des services et des institutions susceptibles de les accueillir ne sont pas nécessairement en phase avec ces représentations. L’expression de ces perspectives s’est concrétisée par une enquête qualitative auprès de patients âgés de 60 ans et plus, de la file active du COREVIH Ile-de-France Centre et par des entretiens avec des professionnels du secteur sanitaire et médico-social. Une cartographie des situations de vie et des logiques en place a permis d’identifier les profils pertinents pour répondre à la problématique posée. Dans ce contexte, les paroles recueillies montrent que celles-ci se projettent dans une situation de vie en perte d’autonomie, à l’exception étonnamment des plus âgées. Les personnes immigrées souhaitent plutôt une vie en institution pour personnes âgées, les HSH (hommes ayant des rapports sexuels avec des hommes) optent pour un maintien à domicile ou des résidences services haut de gamme, les personnes hétérosexuelles sont plus partagées mais envisagent plus de rester à domicile. Néanmoins, la poursuite d’une vie dans l’âge, à domicile ou en institution, s’accompagne de craintes fortes. Les pratiques discriminatoires en lien avec la pathologie VIH ou l’orientation sexuelle, la divulgation du secret en institution et le refus d’admission lié au VIH sont très souvent exprimés. Ces craintes engendrent de véritables angoisses. Le regard des professionnels est encore réducteur, les tabous persistent, et le coût engendré par l’accueil d’une personne âgée vivant avec le VIH peut constituer un frein. La question d’adapter ou de créer de nouveaux établissements se pose alors pour les rendre plus justes et bienveillants à l’égard de ces personnes âgées. / Elderly HIV-positive people are no longer the exception. People are aging with this now chronic disease and moreover discover new diseases, aged-related premature. The question arises of their representations of the coming years in terms of accompaniment and living place. In addition, the view of the services and the institutions for elderly are not necessarily in phase with these representations. The expression of these prospects was concretized by a qualitative survey near 60 years old patients or older, of the active file of the COREVIH Ile-de-France Centre and by interviews with health-care and social-services professionals. A cartography of living situations and logics in place allowed to identify the relevant profiles to this study. In this context, replies show that they see themselves living in situations with loss of autonomy, except the oldest. The immigrant people rather wish a life in a nursing home, the MSM (men who have sex with men) choose home care or exlusive residence and heterosexuals people are shared but prefer to remain at home. Nevertheless, the continuing of a life in the age, at home or in institutions for elderly, comes along with strong fears. Discriminatory practices related to the disease or sexual orientation, disclosure of their HIV status in senior housing facility, rejection based on the review of the medical records, are frequently expressed. These fears generate true anguishes. The point of view of health-care and social-services professionals is still reducing, the taboos persist, and the cost represented by a elederly HIV-positive people can be a significant barrier. The issue is whether to adapt or create new facilities so that they are fairer and more compassionate for these seniors.

Spiritual care for the sick and its organization in three hospitals The aim of this thesis is to describe and compare the organization of spiritual care at Sisters of Mercy of St. Borromeo Hospital, University Hospital in Motol and Psychiatric Hospital Bohnice. The first part presents theoretical knowledge in the area of spiritual needs of patients and the provision of spiritual care. The practical part analyzes the provision of spiritual care in the selected hospitals. The practical part is based on information which was given in interviews with spiritual care providers in selected hospitals. Their statements are divided into topics allowing comparison. This thesis describes the development of spiritual care in the institution, the organization of a team of spiritual care providers, education and type of employment. It examines nature and availability of spiritual care, cooperation with the health departments. The purpose of this thesis is to highlight the state of providing spiritual care in hospitals in Prague and to induce a discussion on the topic. Inkluderande undervisning och evidensbaserade strategier i fyra danska grundskolor

AbstractTitle: Inclusive education and evidence based strategies in four Danish schoolsAuthor: Margareta AnderssonDanish schools are supposed to include 96% of all students. They are also supposed to use evidence based strategies to make inclusion happen. This case study consist of a survey based on Mitchells (2014) metastudy and four interviews. The purpose of the study was to investigate to what degree Mitchell’s inclusive strategies were used. Another purpose was to investigate what other strategies were used and how the schools documented effective strategies. The results of the interviews were analyzed by theories of special education, the three perspectives, the compensative, the critical and the dilemma perspective (Nilholm, 2007). The results of the survey (n=31) show that teachers use all of the evidence based strategies included in the survey for including pupils with special needs. The interviews show that many of Mitchells strategies were considered effective, especially collaborative learning strategies. Other important factors were the support of pupil care teams and the personnel’s positive relations to the pupils. The teachers in this study worked informed by evidence not evidence based. Documentation about strategies used at the schools are kept in individual educational plans. The results show that teachers meet several dilemmas in working with inclusive strategies. As educational goals sometimes conflict, because of the context, dilemmas occur as to the degree of individualization versus taking part in class activities, what goal to focus on, inclusive or academic goals. Collaboration with parents is another dilemma, concerning the ethics about openness about a child’s or a group of children’s difficulties. Another dilemma is that information about effective strategies at an individual or group level is not generalized at school level. This study can contribute to the discussion on how schools try to resolve the dilemma that inclusion means. The challenge of making schools an inclusive environment remains. The use of cell demodulated electronic targeted anesthesia to control dental operative pain in pediatric patients

Indiana University-Purdue University Indianapolis (IUPUI) / The pain-controlling effects of a recently introduced electronic dental anesthesia device (CEDETA) were compared with those of local anesthesia in this study. Procedures performed involved full-coverage stainless steel crowns on maxillary primary molars, some of which required indirect pulp therapy and pulpotomies. A total of 55 children, aged 6 years to 10 1/2 years, were randomly selected to have treatment done with CEDETA or local anesthetic. Eight of these patients were treated with both CEDETA and local anesthetic at different appointments. At various times during each procedure, the patient and operator rated the patient's level of discomfort using a 6-point Visual Analog Scale. For each of the five evaluation steps, no significant differences existed in discomfort ratings between the CEDETA and local anesthetic methods for the group of eight patients or for the entire group. Operator ratings of patient discomfort did not vary significantly between the two methods of anesthesia for each of the evaluation steps, except at the step of maximum output or after injection, when the CEDETA group as a whole had significantly lower operator-rated pain. In general, patients tended to rate their perceptions of pain higher than those of the operator. Although the operator and patients in this study found CEDETA to be as effective as local anesthetic for controlling dental operative pain, a number of factors must be considered when deciding to use this type of electronic dental anesthesia. A substantial monetary investment is required to purchase the CEDETA device and the disposable electrodes and batteries to power the unit. There is an increased operating expense for each procedure done when using CEDETA, because of the additional time needed for the operator, staff, and patients to become familiar with the use of the device. Additional setup and break-down time is also needed when using CEDETA as opposed to local anesthetic.

The demographic prognoses predict a growing proporsion of seniors in population and it could imply much greater demand for social services in the future. Therefore, it is necessary to discuss this question today. In my thesis, I use evaluation research methods to try answer if the current system is sustainable.

The text tries to bring a different point of view on what citizenship is and what it means, other than political or legal interpretation. It searches for pre-political and pre-legal basis of citizenship able to protect its rights and freedoms as defined by, for example, the Charter of Fundamental Rights and Freedoms or as legally guaranteed in democracies. The theoretical part of the text is based on treatises of Czechoslovak dissidents who examined the phenomenon of citizenship, being themselves citizens of illiberal regime. The treatises originate mainly in the 1970s and look at the citizen through the eyes of the dissent of "normalised" Czechoslovakia. The view is broadened by further interviews focusing on the topic of citizenship with three more dissidents. The goal is to find what citizenship should carry within, what it should mean in order to be able to defend its space, the space which officially belongs to it in democratic conditions.

The main reason that leads me to the elaboration of this work is particularly ambiguous and rather one-sided definition of hospice palliative care in the Czech Republic in terms of health care. In the analysis and evaluation, which is the main part of the paper I will draw on already existing documents and data that relate to the topic immediately. Another integral part of the consultation and discussion with experts from the ranks of both health and social services in providing care face barriers that currently has inadequate definition of the operation itself. Hypertension Management Through Community Outreach Services for Inmates Released From Jail

Hypertension (HTN) is the most common chronic disease among jail inmates. Many inmates treated for HTN while incarcerated in the Fairfax County Jail do not continue treatment when they return to their communities. Factors that contribute to discontinuing HTN management once the inmate returns to the community include homelessness, low income, and lack of access to care. The purpose of this quality improvement project was to educate inmates with HTN about community-based outreach services for HTN management and continuity of care while in the community. The outcome measured was the number of inmates who returned to jail reporting use of a community-based clinic for follow-up HTN care after their last release from jail. The project was conducted in 2 phases during a 6-month period. A pre-HTN survey questionnaire measuring HTN history and lifestyle was administered on initial incarceration. A post-HTN survey was completed when the inmates return to the jail during the 6-month period and measured adherence to post jail follow up HTN care. The findings of this quality improvement project indicate that both inmates who returned to jail in Phase 2 of the project followed up their HTN care in the community after release from jail. This project shows promise as a first step in the process of social change in planning discharge for inmates with HTN at the time of incarceration.

Syftet med detta examensarbete är att undersöka förskollärares uppfattningar om omsorg om andra som ett demokratiskt värde som barnen ska utveckla. För att granska detta så har jag intervjuat fem stycken förskollärare från två olika förskolor. Jag analyserade min data med en fenomenografisk metodansats. Jag gjorde också en teoretisk analys utifrån mitt resultat med hjälp av utvecklingspedagogiken. Resultatet visar att förskollärare uppfattar att omsorg om andra innefattar empati, respekt och att vara omtänksam genom att hjälpa varandra. Det är också på detta sätt som förskollärarna beskriver att omsorg om andra gestaltas. Förskollärarna nämner framförallt tre strategier i sitt arbete för att barnen ska utveckla kunskap om omsorg om andra. Det första är att skapa trygghet, det andra att hjälpa barnen till reflektion. Det tredje innefattar förskollärarnas eget reflektions, dokumentations och utvärderingsarbete. / The aim of this study is to investigate preschool teachers' perceptions of caring for others as a democratic value that children should develop. To find out this, I have interviewed five preschool teachers from two different preschools. I analyzed my data with a phenomenographic method approach. I also did a theoretical analysis based on my results using the development pedagogy. The result shows that preschool teachers perceive that caring for others involves empathy, respect and being caring by helping each other. It is also in this way that the preschool teachers describe that care about others forms. The preschool teachers mainly mention three strategies in their work for the children to develop knowledge about caring for others. The first is to create security, the second to help the children to reflect. The third includes preschool teachers' own reflection, documentation and evaluation work.
