Caring for children at hospital with palliative care needs arouses different reactions in nurses and depends on what prior knowledge and experience nurses have. To besides shift focus from to care children palliative to care curative is further a strain and an emotional changeover. The aim of this study was to illuminate nurse’s perceptions about caring for children at hospital with palliative care needs, and how they shift focus of care between children with palliative care needs and children with curative care needs. The study has a qualitative approach and data were collected with interviews from two focus groups. The interviews were performed in two different hospitals in the middle of Sweden, and analyzed with a manifest content analysis. Five themes were created from the analysis: create confidence and individualized relations, difference in meeting of children with unlike care needs, acquired experience and ability to reflect, need of support, supervision and distance, difficulties to enforce a balance in changing between different care need. The conclusion was that nurses who shift between caring for children with a palliative care or curative care needs are exposed to psychological strain which may lead to psychological ill- health. This is important to prevent through for example continuous supervision which may contribute to the possibility for nurses to obtain more time for reflection and regain new strength. / Sjuksköterskor som arbetar på en barnavdelning möter ofta barn med ett palliativt vårdbehov, vilket väcker olika reaktioner hos sjuksköterskorna beroende på tidigare kunskap och erfarenhet. Att dessutom skifta fokus från att vårda barn palliativt till att vårda kurativt är ytterligare en påfrestande och känslomässig omställning. Syftet med denna studie var att belysa sjuksköterskors uppfattningar om hur det är att vårda barn på barnavdelning i sluten vård med palliativt vårdbehov samt hur det är att växelvis vårda barn med palliativt vårdbehov gentemot barn med kurativt vårdbehov. Studien har en kvalitativ ansats och data har samlats in via intervjuer i två fokusgrupper. Intervjuerna genomfördes på två olika sjukhus i mellersta Sverige och analyserades med manifest innehållsanalys. Utifrån analysen skapades fem teman: skapa förtroende och att individanpassa relationen, skillnader i bemötande av barn med olika vårdbehov, förvärvad erfarenhet och förmåga att reflektera, behov av stöd, handledning och distans, svårigheter att upprätthålla en balans i växlingen mellan olika vårdbehov. Slutsatser som drogs var att sjuksköterskor som växelvis vårdar barn med palliativt eller kurativt vårdbehov utsätts för psykiska påfrestningar, vilket kan leda till psykisk ohälsa. Detta är viktigt att förebygga genom att sjuksköterskorna får till exempel kontinuerlig handledning vilket ger tid till reflektion och möjlighet att inhämta nya krafter.

Parents, employment, gender and well-being: a time use study

Transformations in the economy have led to changes in employment practices that can create a mismatch between parents’ work schedules and family routines. At the same time, approaches to child-rearing have become more time-intensive, with expectations of increased parental involvement in all aspects of children’s lives compared to previous generations. Mothers are subject to a more intensified maternal role and for fathers, the provider role is no longer sufficient. There are strong social pressures for more active participation in children’s lives and to nurture greater emotional connectivity in the parent-child relationship. These transformations in parenting and employment practices have contributed to a situation where parents now report increased levels of time pressure and greater dissatisfaction with the balance between work and the rest of their lives. The purpose of this study was to develop a broader understanding of how mothers and fathers with school-age children allocate their time, how it varies by household composition, season of the year, and work schedule, and how time use is related to subjective well-being. Given the role that leisure may play in creating a more satisfactory work-life balance, special attention was given to the amount of time available for leisure, with whom this time was spent and the relationship to quality of life. The gender relations perspective provided a theoretical framework since role expectations and experiences of parenthood differ for mothers and fathers. By considering individual, interpersonal, institutional and socio-historical levels of influence, patterns of behaviour may be better understood within the Canadian social context. This study is a secondary analysis of the 2005 Canadian General Social Survey, Cycle 19. Using a sub-sample of 2,062 parents of school-age children (ages 5-17 years), patterns of time use and perceptions of quality of life were assessed and compared by gender according to household composition, season of the school year, work schedule and flexible work option. Work schedules were categorized as traditional (daytime, no weekends), non-standard (evening, weekend and rotating shifts) and irregular (unpredictable, with “on call”, casual, or other irregular patterns). Gender inequality in the allocation of time to important life spheres remained substantial even when faced with very complex challenges in coordinating employment arrangements, family routines, and the school year schedule. With the exception of single fathers, men spent more time on employment-related activities than women regardless of work schedule, while women continued to perform greater amounts of domestic and child care activities. Combined workloads of paid and unpaid labour were significantly different only among single mothers and fathers. For married or cohabitating parents, the distribution of paid and unpaid labour was asymmetrical but the combined workload was not significantly different. Fathers continued to be privileged with greater amounts of leisure time. This was especially noticeable for men with non-standard work schedules and fathers of teenage children. Seasonal differences in time use indicated that women’s routines were more linked to the school year than men’s because of their greater child care responsibilities. When irregular or non-standard schedules were in place that could create more optimal conditions for reconsidering time allocation, parents still followed activity patterns that reproduced traditional gender roles. For women, non-standard schedules were the most detrimental to quality of life, whereas for men irregular schedules decreased well-being. Flexible schedules also perpetuated inequality in the distribution of paid work, unpaid work and leisure although quality of life actually improved for women. This improved quality of life may be attributable to more time spent on activities with physical health benefits or increased options for daily schedules, but it also leads to questions about how much control mothers actually have over their time and whether they recognize or care to challenge the inequalities that persist in the distribution of labour and leisure. Quality of life was diminished by conditions that contributed to a deviation from traditional role expectations. Some of these included not having a partner, women’s work schedules that conflicted with other family members, and for men, having an irregular and unpredictable work schedule. Since these conditions are characteristic of a sizable minority of Canadians parents, their experiences should not be dismissed but rather given greater attention. Additionally, future discussions of work-life integration should consider access to leisure since time for leisure was shown to contribute to parents’ quality of life. The gender relations perspective proved to be helpful in interpreting and understanding the dynamics of time use and behaviour. The four levels of interaction were highly interconnected, but changing institutional conditions such as employment schedules did not lead to greater equality. Instead, non-traditional work schedules widened the gender gap, particularly for child care and domestic activities. Dominant parenting practices were shown to be so deeply imbedded that stereotypical patterns remained a prevailing force guiding men’s and women’s daily activities. The GSS data, despite some limitations, provided considerable insight into the effect of parents’ employment and gender on time use and well-being. The findings of this study underscore the relevance of considering multiple levels of influence when assessing parenting practices, gendered behaviour, and quality of life for employed parents.

Osedliga verser och smutsiga barn : Barnavårdens praktik och begreppsanvändning under 1929-1937 / Immoral verses and dirty children

Social work is a profession where documentation about people’s behavior and life circumstances is common. In the beginning of the essay we ask ourselves, if these descriptions about people can be problematic? To explore this, we studied social documentation, from the past. We used a historical source because history can help usunderstand the social work that is being conducted today. The aim of our study was to findout how early welfare work defined and described deviant behavior in child care issues, and how the child care agency handled these issues. Out method was a document analysis off the children’s care protocols in Kalmar, from 1929 to 1937. We present our results along with two illustrative case descriptions. Our theoretical approaches are Howard S Becker and Erwing Goffman´s theories of deviation. Their conclusion is that deviation is created by society, not by individuals or their actions. Since cases of children’s neglect and cases with deviant children were common and well documented we focused our empirical presentation on what was included in these terms. Children’s neglect cases focused on parent’s inability to provide the child with proper food, clothes, housing and similar factors. We also found that they made a distinction between mothers and fathers responsibility in these cases. Regarding the deviant children they also made a distinction between the sexes, identifying different behaviors deviant for girls andboys. Though some factors, such as being a illegitimate child and being unreliable is a definition used on both sexes. Since, according to our theories, deviation is created by our society our conclusion is that both children’s neglect and problem children can be seen as a result of societies expectations, and we present examples of how that can be understood in our analyze. In our final discussion we discuss how the definition of deviant behavior is relevant today. We discuss our findings in relation to BBIC, a Swedish child protection investigation guide. Our conclusion is that we still create deviations through documentation.

Swedish child-care institutions - day nurseries, kindergartens - did not until the 1930s become a concern of the Government. In 1943 the Swedish Riksdag for the first time passed a bill that gave child-care institutions a Government subsidy. This thesis deals with the Government's and the parliamentary commissions' attitudes to child-care institutions. Which type of institution ought to receive a subsidy? And for what reasons? The main argument for child-care institutions has always been that they could stimulate a sound development, for the child's own good and for society's. From the 1930s and into the 1950s most participants in the child-care debate stated that the kindergarten or part-time institutions for the pre-school child from the age of three and upwards was the preferable type. Day nurseries for children, even infants, of families were both parents had to work might be necessary but were to be seen as an emergency solution. From the mid-60s the attitu-de changed. Step by step full-time day nurseries became the institutions that were given priority by the Government. This change in attitude presupposes that the notion of the child changed as well. But it did not change in a vacuum. Borrowing an explanatory model from sociologist Johan Asplund, the thesis treats the child as a "figure of thought", placed between a super-structural discourse on child-care and society's basic, material conditions. Important changes at the level of discourse have been the attitude to modern, industrial society, e.g. the necessity of learning to live and work in a society which is complex, highly specialized and in constant change, and the debate on women's emancipation. At the level of material conditions, the most conspicious change is that more and more women have entered the labour market. The changing notion of the child can be understood as the effect of an influence from discourse and base on the "figure of thought". At the same time, the "figure of thought" in-fluenced the discourse. Thus, a child-care system for the benefit of child and woman and labour market could be established, and harmony could be created, at least in the discourse.

Fundação de Amparo à Pesquisa do Estado do Rio de Janeiro / O objetivo principal desta Dissertação foi avaliar as relações entre a Violência Física entre Parceiros Íntimos (VFPI) nos primeiros seis meses após o parto e a utilização de serviços de saúde entre crianças menores de seis meses de idade. Para estudar o construto utilização de serviços de saúde utilizou-se o momento de início do acompanhamento e o número de consultas da criança em Unidades Básicas de Saúde (UBS). Adicionalmente, estimou-se a prevalência de VFPI nos primeiros seis meses após o parto entre mães de crianças desta faixa etária assistidas nas UBS do Rio de Janeiro. As informações que subjazem a pesquisa originaram-se de um estudo transversal realizado em 27 UBS do Município do Rio de Janeiro, entre junho e setembro de 2007. A população de estudo foi selecionada por meio de amostragem por conglomerado em dois estágios. As UBS unidades primárias de amostragem foram amostradas com probabilidade de seleção proporcional ao volume de consultas pediátricas realizadas conduzindo a uma amostra geograficamente representativa do município. As crianças unidades secundárias de amostragem foram selecionadas de forma sistemática, obedecendo à ordem de saída das consultas. A amostra incluiu 927 crianças nos primeiros seis meses de vida cujas mães relataram ter companheiro na ocasião da entrevista dentre aquelas que buscaram consulta pediátrica ou de puericultura. As informações foram obtidas por meio de entrevista com a mãe da criança utilizando-se um questionário estruturado, contendo escalas previamente validadas, como a Revised Conflict Tatics Scales (CTS2) para a mensuração da VFPI. O artigo inicial apresenta a prevalência de VFPI nos primeiros seis meses após o parto na população estudada e em certos subgrupos de acordo com características sociodemográficas e de saúde de mães e bebês. Elevadas frequências de violência conjugal foram evidenciadas, em especial entre mães em situação socioeconômica desfavorável e que apresentavam falhas no cuidado pré-natal, na amamentação e na utilização do serviço de saúde. Os outros dois artigos apontam que a VFPI após o parto apresenta um sério risco ao acompanhamento regular da criança nos serviços de saúde. O segundo artigo revelou que a VFPI é um fator de risco independente para o início tardio do acompanhamento da criança em UBS em mulheres que possuíam emprego informal ou não trabalhavam e entre aquelas que não haviam realizado um adequado acompanhamento pré-natal. Já o terceiro artigo mostrou que a VFPI aumenta o risco de crianças filhas de mães que não exerciam trabalho remunerado após o parto terem um número de consultas aquém do esperado para a idade nos seus primeiros seis meses de vida. Espera-se que a divulgação dos resultados desta Dissertação possa contribuir para aumentar a sensibilização de profissionais de saúde e dos planejadores de políticas públicas do Setor Saúde para a importância de ações de combate à violência e, assim, colaborar para a promoção da saúde no seu sentido mais amplo.

Dos diferentes significados sociais do “ser criança” aos contextos gerenciais na organização da atenção à saúde da criança em serviços de atenção primária

Orientador: Elen Rose Lodeiro Castanheira / Resumo: A atenção à saúde da criança está entre as ações mais tradicionais e bem estruturadas na Atenção Primária à Saúde (APS) no Brasil, entretanto deve-se reconhecer que as relações entre as necessidades sociais de saúde desse grupo e as práticas a ele dirigidas foram construídas de diferentes maneiras ao longo da história. O reconhecimento desse processo mostra-se importante para identificar quais necessidades sociais de saúde estão sendo respondidas pelas práticas atuais. Apesar dos grandes avanços alcançados, como a redução das taxas de mortalidade infantil, indicadores de atenção à saúde da criança apontam a necessidade de melhorias, como nas taxas de internações por causas evitáveis pela APS, no crescente número de afecções originadas no período perinatal e na atenção ao parto. Avaliar a qualidade da organização da atenção, a partir do projeto contemporâneo para a saúde desse segmento, permite julgar a base material do projeto social em execução pelas práticas. O presente trabalho tem por objetivos: reconhecer a construção histórica do significado social de “ser criança” no Brasil e avaliar a capacidade de resposta dos serviços de APS às necessidades sociais de saúde das crianças. Os estudos foram apresentados na forma de 3 artigos. Parte de um ensaio alicerçado em revisão bibliográfica e na análise de sua correlação com as práticas de saúde pública voltadas à criança, privilegiando-se as ações desenvolvidas na APS (Artigo 1). However, it must be acknowledged that the child's health needs and practices directed at children have been constructed differently. Recognition of this construction process is important in identifying what needs are being replicated by current practices. Indicators of child care show poor performance, as in the rates of hospitalizations for sensitive conditions by PHC and the increasing number of conditions originating in the perinatal period. Evaluating the quality of the organization of child health care today allows us to judge the material basis of the social project being implemented by the practices. The objective of the study is: to recognize the historical construction of the social meaning of being a child in Brazil; and to assess the responsiveness of PHC services to the health needs of children. Three articles were presented. The first is an essay based on a bibliographical review and the analysis of its correlation with the health practices/public of the child, with a focus on PHC actions (Article 1).
