Estudo fitoquímico e atividades biológicas preliminares de extratos de Polygonum Acre (Polygonaceae) H.B.K. e Synadenium Carinatum (Euphorbiaceae) Boiss /

As espécies Polygonum acre e Synadenium carinatum são de fácil cultivo, estando largamente distribuídas no Brasil, e possuindo um alto índice de utilização na medicina popular. Entre os usos populares da espécie Polygonum acre podem ser citados: anti-séptico, antiinflamatório, hipotensor, anti-hemorroidal, diurético, vermicida e anti-diarréico. A espécie Synadenium carinatum é popularmente utilizada para o tratamento de cânceres. No entanto, não existem estudos científicos que comprovem esses efeitos, nem informações sobre a segurança de utilização dessas drogas pelos seres humanos além de haver poucos estudos fitoquímicos destas espécies. Neste trabalho foram realizados os estudos fitoquímico, microbiológico, a busca de atividades biológicas dos extratos dessas plantas e a investigação de aspectos relativos à segurança de utilização. Os testes fitoquímicos indicaram a presença de flavonóides, taninos, saponinas, mono, sesqui e diterpenos, e derivados cinâmicos nas espécies P. acre e S. carinatum, enquanto que apenas a espécie P. acre respondeu positivamente quanto à presença de proantocianidinas condensadas e leucoantocianidinas. Na avaliação da atividade antimicrobiana através do método de difusão em ágar e da técnica de Concentração Inibitória Mínima, a espécie P. acre apresentou atividade antimicrobiana nas concentrações de 300 mg/mL, enquanto a atividade antimicrobiana para os extratos de S. carinatum não foi evidenciada. Nos testes de toxicidade aguda, pode-se observar que na dose de 2 g/kg, o extrato etanólico 70% de P. acre apresentou taxa de mortalidade de 50%, enquanto o extrato etanólico 70% de S. carinatum não apresentou toxicidade ... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Although there are thousands of plant species used in traditional medicines in the world, it is estimated that only 1% are known by scientific studies with demonstrated therapeutic value. The species Polygonum acre and Synadenium carinatum are easy to culture, being widely distributed in Brazil, and having a high rate of use in folk medicine. Among the popular uses of the species Polygonum acre may be cited anti-septic, anti-inflammatory, hypotensive, anti-hemorrhoid, diuretic, vermifuge, and anti-diarrheal. The Synadenium carinatum species is popularly used in the treatment against cancer. However, there are no scientific studies that show these effects, no information about the safe use of these drugs by human beings and there are few phytochemical studies of this species. In this work, the phytochemical study and microbiological quality control, the search for biological activities of the extracts of plants and the investigation of aspects related to safe use were done. The phytochemical tests indicated the presence of flavonoids, tannins, saponins, mono, sesqui and diterpenes, and cinnamic derivatives in P. acre and S. carinatum species, while only the species P. acre responded positively about the presence of condensed proanthocyanidins and leucoanthocianidins. In the evaluation of antimicrobial activity by the ágar diffusion method and Minimum Inhibitory Concentration technique, the P. acre species had antimicrobial activity at concentrations of 300 mg / mL, while the antimicrobial activity for the extracts of S. carinatum was not demonstrated. In the tests of acute toxicity the ethanol extract 70% of P. acre at a dose of 2 g/kg showed 50% of mortality while the ethanol extract 70% of S. carinatum has no toxicity. Investigation into Field Impairment Tests and an evaluation of their validity and reliability as clinical tests of drug-related impairment of driving ability

Background: Drug use among the driving population is a major hazard to road safety and has been the subject of widespread research worldwide. In an attempt to detect and appropriately prosecute “drug-drivers”, the UK has made legislative changes and has introduced Field Impairment Tests (FIT) in the Railway and Transport Safety Act 2003. These FIT, which are identical to the Standardised Field Sobriety Tests (SFST), were devised in the USA in the 1970s to identify clinical signs of impairment due to alcohol intoxication, but were not designed or intended to identify drug-related driving impairment. Concerns have been expressed that FIT are too difficult for their stated function, and are also inappropriate tests, since although they have been validated for alcohol effects they have never been validated for the effects of drugs. This thesis has sought to clarify matters and has questioned the validity of FIT by testing two opposing hypotheses – 1) FIT are reliable and valid tests of drug-related impairment to drive - and drug-free individuals perform well on all tests. 2) FIT are not reliable and valid tests of drug-related impairment to drive - and are too difficult for some groups of drug-free individuals to perform. Methods: A questionnaire was designed and a postal survey was undertaken of 960 Forensic Medical Examiners (FMEs) who were asked to give their opinion on the Field Impairment Tests (FIT). The responses of the FMEs were analysed in detail (chapter 3). FIT were then carried out on three separate groups of 100 subjects in police custody who were all known to have used no drugs for a period of at least 8 hours prior to the testing process (chapter 4). Group A subjects were opiate dependent; Group B were subjects who received legally prescribed methadone; Group C individuals denied any form of drug use. All study groups were simultaneously examined using conventional psychomotor tests, and the results were compared using detailed statistical analysis with logistic regression and summative scores. Results: Returned FME questionnaires showed 63% of FMEs considered the tests “about right” but a significant number (p<0.0001) of 33% of FMEs stated FIT were “too difficult”. The studies on subjects in custody clearly showed 82% of group A; 44% of group B; and 19% of group C were unable to complete FIT satisfactorily, although only 12% of group A; 2% of group B; and 3% of group C were unable to successfully perform conventional psychomotor and cognitive tests. These findings clearly supported the assertions held by 33% of FMEs surveyed that FIT were too difficult for their stated purpose and that poor performance in FIT could not be regarded as definitive evidence of drug-related impairment in driving ability. VII Conclusions: The results of the research studies conducted have provided very strong support in favour of hypothesis 2) FIT are not reliable and valid tests of drug-related impairment to drive - and are too difficult for some groups of drug-free individuals to perform. In an attempt to overcome the problems in respect of FIT, specific proposals have been offered including the introduction of a new battery of more relevant clinical tests of impairment (CTI); a change in the method and manner in which the proposed new tests are applied; and possible legislative and administrative measures which might be introduced to more appropriately and effectively tackle this on-going hazard to road safety.

The Caledonian face test: A new test of face discrimination

yes / This study aimed to develop a clinical test of face perception which is applicable to a wide range of patients and can capture normal variability. The Caledonian face test utilises synthetic faces which combine simplicity with sufficient realism to permit individual identification. Face discrimination thresholds (i.e. minimum difference between faces required for accurate discrimination) were determined in an "odd-one-out" task. The difference between faces was controlled by an adaptive QUEST procedure. A broad range of face discrimination sensitivity was determined from a group (N=52) of young adults (mean 5.75%; SD 1.18; range 3.33-8.84%). The test is fast (3-4min), repeatable (test-re-test r2=0.795) and demonstrates a significant inversion effect. The potential to identify impairments of face discrimination was evaluated by testing LM who reported a lifelong difficulty with face perception. While LM's impairment for two established face tests was close to the criterion for significance (Z-scores of -2.20 and -2.27) for the Caledonian face test, her Z-score was -7.26, implying a more than threefold higher sensitivity. The new face test provides a quantifiable and repeatable assessment of face discrimination ability. The enhanced sensitivity suggests that the Caledonian face test may be capable of detecting more subtle impairments of face perception than available tests. / None

Title: Interrater reliability of assessment set of clinical features of patients with multiple sclerosis Objectives: The aim of this thesis is to verify interrater reliability of the assessment set of clinical features of patients with multiple sclerosis which purpose is to evaluate sensitivly and comprehensivly the stage of the patients' clinical condition. Methods: According to the inclusion criteria, the patients with MS were selected by an independent neurologist who determined the EDSS score and duration of the disease. After, patients were evaluated by the assessment set by two independent physiotherapists. The assessment set of clinical features incluedes Low-Contrast Letter Acuity Test which tests contrast vision, Nine Hole Peg Test investiges fine motor skills, Timed 25 - Foot Walk evaluates walking speed over a distance of 7,5 m, Paced Auditory Serial Addition assesses cognitive function, Motricity Index tests muscle strength, Modified Ashworth Scale spasticity, Berg balance Scale equilibrium. Doenças como AVC e Epilepsia estão constantemente associadas à manifestação de comprometimentos cognitivos e demência, tendo como característica principal os déficits de memória. Atualmente,o instrumento Mini Exame do Estado Mental (MEEM) é o teste de rastreio de comprometimento cognitivo mais utilizado no mundo, individualmente ou como parte integrante em baterias de avaliação neuropsicológica. Devido afalta de instrumentos específicos pararastreiode comprometimento cognitivo de indivíduos com sintomas de demênciavítimas de AVC e/ou Epilepsia em Angola, África, fez se necessária aadaptação evalidação de instrumentos confiáveis.O presente estudoteve por objetivo a avaliação clínicada versão brasileira do instrumento MEEM adaptado por Brucki, para uso na população adulta de Angola.Trata-se de um estudo clínico comparado, envolvendo três populações. Foram incluídos neste estudo 85 indivíduos acometidos por AVC, 40 por Epilepsia e como grupo de comparação, 87 indivíduos de 20 a 65 anos de idade, supostamente saudáveis. A escolaridade mínima para inclusão do estudo foi definida em 4 anos de ensino formal. O presente estudo foi desenvolvido em parceria com o Serviço de Neurologia do Hospital Américo Boavida e com a Faculdade de Medicina da Universidade Agostinho Neto (FMUAN), ambos localizados ao norte da cidade de Luanda, capital de Angola, África.Toda a amostra foi submetida à coleta de dados sócio demográficos e avaliação neuropsicológica, através do instrumento MEEM. O instrumento consistiu no rastreio de comprometimento cognitivo, contemplando funções como orientação temporal e espacial, memória, cálculo, linguagem e capacidade construtivavisual.No presente estudo, encontramos dados que apontam um desempenho significativamente pior das funções cognitivas em pacientes acometidos por AVC quando comparados a pacientes com Epilepsia e grupo dossaudáveis. Esses achados confirmam a validade do instrumento MEEM, em função das características apresentadas pela doença, sendo possível discriminar com clareza os grupos AVC, Epilepsia e Controles Saudáveis. Os dados obtidos indicam uma boa qualidade da versão na língua portuguesa angolana, sendo a consistência interna da versão adaptada com ? de Cronbach = 0,736, semelhante aos dados de referência. Este estudo pioneiro preenche uma importante lacuna na produção de conhecimento em Angola, abrindo um leque de possibilidades para outros trabalhos científicos no país, facilitando a aplicação dos resultados tanto na área acadêmica-científica, quanto na clínica. Currently, the instrument Mini Mental State Examination (MMSE) is the screening test of cognitive impairment most used in the world, individually or as an integrated part in neuropsychological assessment battery (Melo & Barbosa, 2015). Due to the lack of specific instruments for screening cognitive impairment of individuals with dementia symptoms victims of stroke and / or epilepsy in Angola, Africa, it was necessary to adapt and validate reliable instruments. The presentestudy had as objective the clinical evaluationof the brazilian version of the MMSE instrument adapted by Brucki et al. (2003), for use in the adult population of Angola, Africa. It is a comparative clinical study involving three populations. This studyincluded 85 individuals affected by stroke, 40 by Epilepsy, and as a comparison group, 87 individuals, 20 to 65 years of age, who were supposed to be healthy. The minimum schooling for inclusion of the study was defined in 4 years of formal education. Thepresent study was developed in partnership with the Neurology Service of the Américo Boavida Hospital and the Faculty of Medicine of the Agostinho Neto University (FMUAN), both located north of the city of Luanda, the capital of Angola, Africa. All the sample was submitted to the collection of sociodemographic data and neuropsychological evaluation, through the MMSE instrument. The instrument consisted in the screening of cognitive impairment, including functions such as temporal and spatial orientation, memory, calculus, language and visual constructive capacity.In the presente study, we found data that indicate a significantly worse performance of cognitive functions in patients with stroke when compared to patients with epilepsy and healthy group. These findings confirm the validity of theinstrument MMSE in, due to the characteristics of the disease, being possible to discriminate with clarity the groups Stroke, Epilepsy and Healthy Controls. The data obtained indicate a good quality of the Angolan Portuguese language version, being the internal consistency of the adapted versionwhit the Cronbach\'s alpha = 0.736, similar to the reference data.This pioneering study fills animportant gap in knowledge production in Angola, opening up a range of possibilities for other scientific papers in the country, facilitating the application of the results both in the academic and scientific area, as at the clinic. It is necessary that health professional take ownership of more knowledge, so that they can propose appropriate interventions that assist in health and welfare rights of the Angolan population.

Quando este medo, ou a ansiedade gerada por ele, interpÃe-se entre o paciente e cuidados de saÃde, torna-se urgente a adoÃÃo de medidas que visem aboli-lo ou minimizÃ-lo. Ainda hoje, muitos pacientes nÃo procuram o atendimento odontolÃgico em decorrÃncia do medo e da ansiedade. O Ãxido nitroso, como meio de sedaÃÃo, vem sendo usado no mundo todo com ampla margem de sucesso. O objetivo deste trabalho foi uma avaliaÃÃo comparativa entre os parÃmetros cardiovasculares e saturaÃÃo de oxigÃnio de dois grupos de 20 pacientes, classificados como ASA I e II, selecionados para cirurgia de extraÃÃo de terceiros molares inclusos. No grupo de estudo (n=20), os pacientes foram submetidos à cirurgia sob anestesia local, associada à sedaÃÃo com Ãxido nitroso (70%) e oxigÃnio (30%); no grupo controle (n=20), os pacientes foram submetidos ao mesmo tipo de procedimento cirÃrgico, com a utilizaÃÃo Ãnica da anestesia local. Os resultados mostraram que a sedaÃÃo com o Ãxido nitroso, mesmo na concentraÃÃo mÃxima permitida â 70% â à um procedimento seguro e efetivo. Os parÃmetros cardiovasculares, do grupo sob sedaÃÃo, mostraram uma estabilidade hemodinÃmica maior que a observada no grupo que recebeu somente a anestesia local, nÃo tendo sido observado em nenhum dos grupos, qualquer episÃdio de dessaturaÃÃo ou alteraÃÃes eletrocardiogrÃficas de significado clÃnico, todos os procedimentos foram finalizados com sucesso e nenhum evento adverso foi observado / Fear is a fundamental human emotion. It is fear that triggers physiological responses that prepare the body for action against real or imaginary threats, allowing the individual to decide whether to fight or flee. When fear, or resulting anxiety, interferes with the provision of healthcare causing patients to delay or avoid care, it is necessary to take measures to eliminate or minimize this feeling. Even today, patients avoid dental care due to fear or anxiety. To those, classified as âphobicâ, are added patients with special needs and non-cooperative children. In these cases, hypnotic and anxiolytic drugs have been used, as well as general anesthesia. As an alternative to the use of such drugs, the mixture of nitrous oxide and oxygen, also called conscious sedation or inhalation analgesia, have been used throughout the world with a high degree of success. The objective of this study was to perform a comparative analysis of cardiovascular parameters and pulse oximetry values in two groups of 20 patients, classified as ASA I and II according to the American Society of Anesthesiologists (ASA) classification, who were referred for surgical removal of impacted third molars. Patients in the experimental group (n=20) underwent surgery under local anesthesia with sedation consisting of 70% nitrous oxide and 30% oxygen, while patients in the control group (n=20) underwent the same surgical procedure but only under local anesthesia. The results demonstrated that sedation with 70% nitrous oxide is a safe and effective procedure. The cardiovascular parameters in the group given sedation showed more hemodynamic stability than those observed in the group that only received local anesthesia. In none of the groups, desaturation episodes or electrocardiographic changes of clinical significance were observed
