Objetivos: avaliar a percepção do estudante sobre sua aprendizagem em habilidades médicas nos primeiros quatro anos do curso de medicina. Método: Estudo descritivo, avaliativo e transversal. Por meio de um instrumento (INSTRUMENTO DE AUTOAVALIAÇÃO DAS HABILIDADES MÉDICAS - IAAHM), elaborado pelos autores, que abrange quatro dimensões do processo de comunicação, cognitivo, interpessoal, autogerenciamento/automonitoramento e Técnico, por meio de método quantitativo, e qualitativamente por entrevistas com roteiro semi estruturado, com base na "fenomenologia semiótica". Resultados: Todos os estudantes 4º ano de medicina, 105, responderam ao instrumento de autoavaliação e 20 foram entrevistados. Os dados coletados mostram que as habilidades foram adquiridass, com destaque Cognitiva e Interpessoal. A confiabilidade do instrumento medida pelo teste de Cronbach foi de 0,83. Houve diferença significativa entre as dimensões e os melhores resultados foram para Cognitiva (4,14) e Interpessoal (4,11). Os estudantes relataram que o treinamento no Eixo é um diferencial na sua formação e sentem-se preparado para o internato. Ressaltou a aquisição de habilidades técnicas principalmente às normas de biossegurança, estruturação do prontuário médico, elaboração da anamnese, exame físico e a semiotécnica. Conclusões: Os estudantes avaliaram positivamente o módulo educacional de habilidades nos primeiros quatro anos e sentem-se preparados para o internato. Reconhece a Psicologia integrada ao Eixo Habilidades como instrumento facilitador do aprendizado associada ao conhecimento da prática médica / Abstract: Abstract Introduction: In the current context of medical practices' role in contemporary society, it was concluded there is a need for important modifications on professional medical education concerning technical and relationship requirements in order to establish a satisfactory scope of medical practices. Objectives: To evaluate the student's perception of his/her learning medical skills in the first four years of the medicine course. Method: A descriptive, appraising, cross-sectional study was carried out using a MEDICAL SKILLS SELF-ASSESSMENT TOOL (MSSAT), which was developed by the authors of this study, including four dimensions from the communication process such as Cognitive, Interpersonal, Self-management/Self-monitoring by means of quantitative evaluations, and also qualitatively, through a semi-structured interview guide. Results: Based upon the "semiotic phenomenology" criteria, interviews were carried out with 20 medical students; whereas the self-assessment tool was answered by 105 4th-year students of medicine. From data collected it was concluded that students have developed abilities, especially Cognitive and Interpersonal ones. Cronbach's alpha coefficient for reliability was 0.83, which is considered a very good result. Furthermore, there was a significant difference between the dimensions and the best results were for the Cognitive (4.14) and Interpersonal (4.05) ones. The students reported that the differential Axis training is very positive to their future career's profile, feeling prepared for the internship period. Emphasis was given to technical abilities acquisition regarding biosafety rules, medical records structuring, anamnesis development, physical examination process, and semio-technical vocabulary. Conclusions: The educational skills module was positively evaluated by the students during the first four years, helping them get prepared for the internship period. It was also recognized that Psychology integrated to the Abilities Axis is an effective tool to facilitate the acquisition and use of knowledge required for medical practices / Mestrado / Ensino em Saúde / Mestra em Clínica Médica

Challenges Encountered by 0ne-year Diploma student midwives in acquiring clinical skills at selected hospitals in Limpopo Province

Thesis (MPH.) -- University of Limpopo, 2016. / Introduction and purpose: Midwives are the backbone of maternal and child health—the output of their action affect quality of life of mother and child. The purpose of the study was to describe the challenges encountered by one-year diploma student midwives while acquiring clinical skills at selected hospitals in Limpopo Province. Research design and method: A quantitative, descriptive cross-sectional research method was used in this study. The study population comprised all one-year diploma student midwives in selected hospitals in Limpopo Province, namely, Dilokong Hospital, Jane Furse Memorial Hospital, Mokopane Hospital, Philadelphia Hospital, Siloam Hospital and St Rita’s Hospital. The whole study population was used as the total population was small. Data were collected using a self-administered questionnaire. Informed consent was received from the participants as was ethical approval from the relevant authorities. Data were analyzed using SPSS version 22 with the aid of a statistician. Descriptive statistics were used to analyze and describe the data. Data were presented in tables and bar graphs.Findings: Respondents in this study indicated that they encountered the following challenges during their acquisition of clinical skills at the selected hospitals in the Limpopo Province: inadequate resources; inadequate mentoring. Male participants reported feelings of being more competent than their female counterparts. There was no relationship between acquisition of skills and marital status, and no correlation between age and acquisition of skills.

Clinical practicum should provide students with the opportunity to combine cognitive, psychomotor and affective skills to develop competencies to prepare the qualifying practitioner for demands of pre-hospital emergency care in the real world. The aim of this study was to explore the multidimensional aspects of the clinical practicum experience of ALS paramedic students in the Western Cape through the lens of a qualitative research design. The study involved exploring the experience of paramedic students in the clinical practice placement. In addition, the views of paramedic graduates were also explored, to ascertain whether the placement experience adequately prepared them for practice. An analysis of the clinical practice documents utilised in the training of ALS paramedic students was conducted with particular reference to identify correlations with relevant literature. In addition, it was necessary to identify how clinical practice manifested itself, not only in what students and graduates express but also how it was coordinated and conveyed in a professional work related context, against the backdrop of the work integrated learning framework. Using a case study design, focus group interviews were conducted with final year EMC students from CPUT and CCA students from the Western Cape CEC. This was followed with one on one interview with paramedic graduates. Further to this, clinical practice documents were analysed to triangulate the data. This study revealed that the clinical practice learning objectives for paramedic students were not adequately achieved and paramedic graduates felt ill prepared for independent practice. These discrepancies were attributed to the complex issues both in theory and practice. The study provided a number of recommendations for improving the clinical training experience, such that paramedic graduates who enter the complex and challenging world of EMC are better prepared. / M

Fulfilledby nursing using standard protocol mean to identify patients needs according their complaints, clinical findings observed or assesment of those showing potencial risks to a serious condition. The present study aim to analyze nursing skills to do the job of managing urgency and emergency obstetrics at the health country facilities ( emergency room) of Goiânia, Goiás. The specifc goals was : identify the trainning of this nurses about risk classification; describethe nursing care spent by the nurses to a pregnant womens at risck; and identify the nurses perspectives about necessary skills to do the job.This is a descriptive study with a qualitative approach, whose data were collected through semi-structured interviews with 19 nurses in december 2012 and subjected to content analysis, thematic analysis mode from the analysis and interpretation of data, identified two themes: the formation and development of the job in risk classification, which were subdivided into four thematic categories: professional development, host, clinical judgment and decision making, which originated the powers to act in the classification of risk in emergency and obstetric emergency. The study revealed that the competences for performance in risk classification in urgent and emergency obstetric nurses ' perspective interviewed participants of this study, are directly linked to professional experience and the development of the work in this area. in addition, the application of these skills are conditioned, intrinsically, the technical knowledge of specific obstetric care activities, such as prenatal care, host of the woman, whether in ready unit clinical care or birth injury, clinical judgment with decision-making and additional professional development. / A classificação de risco é uma tecnologia que intervém diretamente no acesso das pessoas aos serviços de urgência e emergência, reorganiza o atendimento, reduz filas e melhora a resolutividade e a agilidade no atendimento. Executada por enfermeiros com utilização de protocolos pré-estabelecidos, consiste em identificar as pessoas que necessitam de tratamento de urgência ou emergência de acordo com as suas queixas, quadro clínico, sofrimento e potencial de risco e agravamento. Este estudo tem por objetivo geral: analisar as competências para a atuação na classificação de risco em urgência e emergência obstétrica na perspectiva de enfermeiros de unidades de pronto atendimento do município de Goiânia, Goiás. Objetivos específicos: identificar a formação em classificação de risco dos enfermeiros; descrever o atendimento do enfermeiro à gestante na classificação de risco e identificar as competências para a atuação na classificação de risco em urgência e emergência obstétrica na perspectiva dos enfermeiros. Trata-se de um estudo descritivo com abordagem qualitativa, cujos dados foram coletados por meio de entrevista semi-estruturada com 19 enfermeiros em dezembro de 2012 e submetidos à análise de conteúdo, na modalidade análise temática. A partir da análise e interpretação dos dados, identificou-se dois temas: a formação e o desenvolvimento do trabalho em classificação de risco, que foram subdivididos em quatro categorias temáticas: desenvolvimento profissional, acolhimento, julgamento clínico e tomada de decisão, que originaram as competências para atuação na classificação de risco em urgência e emergência obstétrica. O estudo revelou que as competências para atuação em classificação de risco em urgência e emergência obstétrica, na perspectiva dos enfermeiros entrevistados participantes deste estudo, estão diretamente ligadas à experiência profissional e ao desenvolvimento do trabalho nessa área. Além disso, a aplicação dessas competências encontram-se condicionadas, intrinsecamente, ao conhecimento técnico de atividades específicas da atenção obstétrica, como o pré-natal, acolhimento da mulher, seja em unidade de pronto atendimento clínico ou obstétrico, ao julgamento clínico com tomada de decisão e ao desenvolvimento profissional complementar.

Some of them can be traced back to the creation of the Unified Health System (SUS) and, afterwards, to the creation of the National Curricular Guidelines for the Medical Course. Therefore, as recommended by the Guidelines, there was, starting in 2006, an amplification of the practical scenarios, which resulted in the insertion of the student in the secondary level of attention to urgencies in a Unity of First Care (UPA). The UPA occupies an important role in the Urgency Service Network, which was established by the federal government in July 2011. The Network was created with the objective of qualifying the services for the Brazilian citizen in a situation of urgency in any of the gateways to the SUS, with emphasis in prevention, treatment and recover of the main causes of morbimortality in the Brazilian population: Trauma, Cerebral Vascular Accident and Acute Myocardial Infarction. This insertion of FCMS students in the UPA occurred in 2010, with fifth year interns of the pediatric internship. In 2011, the same interns, now in their sixth year, started to attend the clinical medicine internship in the same unit. Objectives: Evaluate the clinical medicine internship at the UPA in the eyes of the undergraduates and offer resources to the urgencies curriculum of the FCMS. Materials and Methods: The scenario was evaluated through the analysis of data pertaining to the services provided in the unit during the year of 2011 and the services conducted by students during May of 2012. The internship was evaluated using a semi-structured questionnaire applied to the interns of the year 2012. A study was also done to verify how urgency-related themes were presented to the students of the years one through four with basis on online material available on the Moodle platform of the FCMS in the year 2012. Results: The results showed that the students were pleased by many factors, like the practical learning and practical training; the large volume of patients, which allows contact with the most prevalent diseases of the community; and the constant presence of the teacher. The most attention-worthy occurrences were the large number of patients who requested a medical certificate to justify their absence from work, the cases of low back pain, the cases of thoracic pain of possible isquemic origin and the cases of respiratory diseases. Among the negative points cited were the duration of the internship, which was considered short; the demand for medical certificates; the distance between the unity and the school; and the small number of service rooms in some occasions. Their suggestions were an increased course load, more service rooms and more theoretical classes. The data of the dissertation, alongside the theoretical background research for its execution, allowed the elaboration of the Matrices of Competence to be developed throughout the internship. Conclusion: The internship was approved by this group of students. The scenario at the UPA can be an alternative for the teaching of urgencies. The elaboration of the matrices alongside the identification of learning opportunities throughout the course can propitiate a longitudinal approach to teaching / O curso de Medicina da Faculdade de Ciências Médicas e da Saúde (FCMS) da PUC-SP de Sorocaba passou por mudanças em seu modelo curricular, motivadas por vários fatores. Alguns podem ser remetidos à criação do Sistema Único de Saúde (SUS) chegando até às Diretrizes Curriculares Nacionais para o curso de Medicina (DCN). Conforme recomendado pelas diretrizes, ocorreu, a partir de 2006, uma ampliação dos cenários da prática, o que resultou na inserção do aluno no nível secundário de atenção às urgências, em uma Unidade de Pronto Atendimento (UPA). A UPA ocupa papel importante na Rede de Atenção às Urgências, instituída pelo governo Federal em julho de 2011. A Rede foi criada com o objetivo de qualificar o atendimento ao cidadão brasileiro em situação de urgência, em todas as portas de entrada do SUS, enfatizando a prevenção, tratamento e recuperação das principais causas de morbimortalidade na população brasileira: trauma, acidente vascular encefálico (AVE) e infarto agudo do miocárdio (IAM). Essa inserção dos alunos da FCMS na UPA ocorreu em 2010, com os internos do quinto ano, no estágio de pediatria. Em 2011, os mesmos internos, agora no sexto ano, começaram a frequentar a mesma unidade, no estágio de clinica médica. Objetivos: Avaliar o e estágio na UPA, vinculado à área de clinica médica, sob a perspectiva discente e oferecer subsídios para o currículo de urgências da FCMS. Material e métodos: O cenário foi avaliado através dos dados dos atendimentos realizados na unidade durante o ano de 2011 e dos atendimentos realizados pelos alunos em maio de 2012. O estagio foi avaliado através de um questionário semi-estruturado aplicado aos alunos do internato do ano de 2012. Foi feito também um estudo para verificar como os temas relacionados à urgência foram oferecidos aos alunos do 1º ao 4º ano, em material disponível na plataforma Moodle da FCMS, durante o ano de 2012. Resultados: Os resultados mostraram uma avaliação positiva pelos alunos por vários motivos, dentre eles: ser um cenário com aprendizagem e treinamento práticos, com grande volume de pacientes que permitem o contato com as doenças prevalentes na comunidade, além do acompanhamento contínuo do professor. Entre os atendimentos que chamaram a atenção, destacaram-se o grande número de pacientes que solicitam atestados para justificar falta no trabalho, os casos de lombalgia, os casos de dor torácica de possível origem isquêmica e os casos de doenças respiratórias. Entre os pontos negativos foi citada a duração do estágio, que foi considerada curta; a demanda por atestados; a distância da unidade em relação à faculdade; e a falta de salas para atendimento em algumas oportunidades. Como sugestões, foram pedidos aumento de carga horária, mais salas para atendimento e aulas teóricas. Os dados da pesquisa, associados aos referenciais teóricos buscados para a execução do trabalho, permitiram a elaboração de matrizes de competências a serem desenvolvidas durante o estágio. Conclusão: O estágio foi aprovado por essa turma de alunos. O aprendizado na UPA pode ser considerado mais uma alternativa para o ensino de urgência. A elaboração das matrizes com a identificação de oportunidades de aprendizado durante a graduação propicia a longitudinalidade do ensino

The usage of Mini-Cex is an additional formative and summative evaluation option that assesses pre or post graduates students´ medical expertise. Objectives: Evaluate clinical residents competence on performing urodynamic exames; quantify performance development after feedback attainment, quantify technical improvement quality of urodynamic examination by the resident doctor ; quantify the time spent in applying the Mini-Cex by the observer; detect and quantify medical disabilities during urodynamic examination, evaluate the reliability and internal consistency of the Mini exercise Clinical Evaluative ( Mini-Cex ) for the urodynamic examination. Methods: A quantitative and qualitative study conducted with graduated students in urology during the performance of urodynamic examination, through the Mini-Cex instrument application, which is based on a Likert Scale of 9 categories, which grades 1 to 3 refer to unsatisfactory performance ; 4 to 6 to satisfactory performance and 7 to 9 the exemplary performance . This instrument allowed residents' performance assessment in achieving the urodynamic examination by the researcher on the following questions: on interviewing, physical examination, clinical reasoning skills, humanistic and professional qualities, orientation, organization/efficiency and overall, clinical competence. At the end of the consultancy the researcher conducted a feedback to each student pointing their failures and successes in achieving the urodynamic testing, setting up formative assessment. Results: The findings recommend the usage of the Mini-Cex in performing urodynamic studies (Cronbach's alpha between 0.8 and 0.9) and performing feedback advantages on development of residents´ learning / A avaliação de competências clínicas constitui etapa essencial na formação do estudante de Medicina e deve ser feita pelo professor, por meio da observação direta do desempenho em situação real. O uso do Miniexercício Clínico Avaliativo (Mini-Cex) é uma opção complementar de avaliação formativa e somativa, que avalia as competências clínicas do estudantes pré ou pós-graduados. Objetivos: Avaliar a competência clínica, de médicos residentes, na realização do exame urodinâmico; quantificar a evolução do seu desempenho após a realização do feedback; quantificar a melhora da qualidade técnica do exame urodinâmico pelo médico residente; quantificar o tempo gasto na aplicação do Mini-Cex pelo observador; detectar e quantificar as deficiências médicas durante o exame urodinâmico; avaliar a confiabilidade e a consistência interna do Mini-Cex para o exame urodinâmico. Material e Método: Estudo quanti-qualitativo realizado com estudantes de pós-graduação em Urologia durante a realização do exame urodinâmico, por meio da aplicação do instrumento Mini-Cex, que é baseado em uma Escala de Likert de 9 categorias, em que as notas de 1 a 3 referem-se a um desempenho insatisfatório; de 4 a 6 a um desempenho satisfatório e de 7 a 9 a um desempenho exemplar. Este instrumento possibilitou a avaliação do desempenho dos residentes na realização do exame urodinâmico por parte do pesquisador nos seguintes quesitos: habilidades na entrevista, habilidades no exame físico, qualidades humanísticas/profissionalismo, raciocínio clínico, habilidades de orientação, organização/eficiência e competência clínica geral. Ao final da consulta, o pesquisador realizou um feedback com cada estudante, apontando suas falhas e acertos na realização do exame urodinâmico, configurando a avaliação formativa. Resultados: Os achados recomendam o uso do Mini-Cex na realização de exames urodinâmicos (Alpha de Cronbach entre 0,8 e 0,9) e vantagens da realização do feedback na evolução do aprendizado de médicos residentes

Measuring the quantity and quality of midwifery support of women during labour and childbirth : the development and testing of the 'Supportive Midwifery in Labour Instrument'

The thesis describes the development and testing of a new computer based systematic observation instrument designed to facilitate the recording and measurement of the quantity and quality of midwifery intrapartum support. The content of the systematic observation instrument, the ‘SMILI’ (Supportive Midwifery in Labour Instrument), was based on a comprehensive review of the literature. The instrument was found to be valid and reliable in a series of studies. The feasibility and usability of the SMILI was extensively tested in the clinical setting in four maternity units in Scotland, UK. One hundred and five hours of direct observation of forty nine labour episodes were undertaken by four trained midwife observers. The clinical study demonstrated that the study and the instrument were feasible, usable and successful in measuring the quantity and quality of midwifery intrapartum support. The data collected has provided significant new information about the support given by midwives in the National Health Service of Scotland, UK. Continuous one to one support was the norm, with 92% of the observed midwives in the room for more than 80% of the observation period. Emotional support, including rapport building, encouragement and praise, was the most frequently recorded category of support.

Background: The Private Nursing School was founded in 1999 in Durban when the first nursing students started the Nursing program. In 2006 there was an upgrade to the Private Nursing School when a clinical skills laboratory was introduced to enhance the students’ clinical skills. The clinical skills laboratory was furnished with the equipment needed for demonstrations, role-plays and lectures to take place, and a need arose to evaluate the laboratory. Purpose: The purpose of this study was to conduct a process evaluation to evaluate the clinical skills laboratory at the Private Nursing School with regard to the quality of equipment, satisfaction of the students and the efficiency of clinical facilitators. Methodology: A process evaluation was conducted of the clinical laboratory, including a quantitative survey to evaluate the students’ satisfaction (n=97), a quantitative audit of the quality of the equipment, a review of the utilisation of the clinical skills laboratory and semi-structured interviews with the four clinical facilitators. Students from two years were purposively selected for the study as they made the most use of the clinical skills laboratory. Survey tools were developed by the researcher. Findings from the surveys and audits were described and Chi-square test and Mann-Whitney to compare satisfaction and beliefs of the students. Information from the clinical facilitator interviews were analysed for common themes on usage and satisfaction. Results: The students and facilitators reported overall satisfaction with the running of the clinical skills laboratory. The findings of the evaluation showed that the clinical laboratory was reaching the target groups with all students making use of the clinical skills laboratory for the purpose of viewing demonstrations, subjecting skills to assessment or practising skills. The students felt that learning did take place in the clinical skills laboratory with most of the students (94, 96.2%) reporting that knowledge was gained from demonstrations and practice. Almost two thirds of the students (59.8%) felt that the support from the clinical facilitators was beneficial to them. Some quality issues in terms of equipment were identified where damaged equipment could not be used by the support staff and facilitators stating that there was a need to improve the simulation experience for the students so that they can gain the much-needed practical and theoretical knowledge required for their stipulated course. Conclusion: The clinical skills laboratory is functioning at a level that is satisfactory to the nursing students and the clinical facilitators. Future research should be conducted regarding the impact and outcomes of the clinical skills laboratory training on students’ ability to function in the clinical setting. / Thesis (M.N.)-University of KwaZulu-Natal, Durban, 2013.

The purpose of this study was to explore nurse educators‟ perception of Objective Structured Clinical Evaluation (OSCE) as summative assessment for students‟ clinical competence ,with the aim of recommending alternatives to the current situation. The rationale behind the study is that the researcher became aware of concerns raised by nurse educators regarding the appropriateness of OSCE as summative assessment of students clinical competence and differences of opinion on the matter. An explorative, descriptive, contextual, qualitative research design was chosen. The sample included nurse educators actively involved in OSCE as summative assessment of nursing students in the provincial nursing college for at least 5 years or more. Data was collected by means of two (2) focus group interview sessions with fourteen (14) participants. During data analysis it became clear that data saturation had not been reached and a third focus group interview was conducted with seven (7) participants from the original sample who were available. The focus of the interviews was to explore participants‟ perception of Objective Structured Clinical Evaluation as summative assessment for students‟ clinical competence. The data was independently analyzed by the researcher as well as an independent co-coder according to the principles for content analysis as described by Creswell (2008:251). Three (3) themes and nine (9) sub-themes emerged from the data collected from participants. The first theme described the value of OSCE as summative assessment method for students‟ clinical competence. Theme two described the obstacles in using OSCE as summative assessment for students‟ clinical competence. Theme three described the suggested alternatives. The collected data was discussed and integrated with relevant national and international literature to culminate in three (3) conclusive statements.
