Tratamento endoscópico versus cirúrgico para adenomas de papila: revisão sistemática e metanálises / Endoscopic versus surgical treatment of ampullary adenomas: systematic review and meta-analysisMendonça, Ernesto Quaresma 28 April 2017 (has links)
Objetivos: Avaliar os desfechos da ressecção endoscópica em comparação à cirurgia no tratamento dos adenomas de papila. Métodos: Foi realizada uma revisão sistemática com metanálise de acordo com as recomendações do Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). As bases de dados Medline, Embase, Cochrane, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Scopus and Cumulative Index to Nursing and Allied Health Literature (CINAHL) foram escaneadas. Os estudos incluíram pacientes com adenoma de papila e dados de comparação dos tratamentos endoscópico e cirúrgico para os seguintes desfechos: Ressecção primária completa; Sucesso primário; Recorrência; Sucesso final; e Complicações. A análise foi baseada em modelos de efeito randômico e fixo. Resultados: Cinco estudos de coorte retrospectivo foram selecionados, com um total de 465 pacientes. Todos os estudos tinham dados de ressecção primária completa disponível, mostrando uma diferença favorável ao tratamento cirúrgico (Diferença de riscos = -0,22; Intervalo de confiança de 95% = -0,41 a -0,04). Dados de Sucesso primário também foram identificados em todos os cinco estudos. A análise mostrou que a abordagem cirúrgica supera o tratamento endoscópico neste desfecho (DR = -0,13; IC 95% = -0,24 a -0,02). Dados de recorrência foram encontrados em todos os estudos (465 pacientes), com benefício para o tratamento cirúrgico (DR = 0,12; IC 95% = 0,01 a 0,22). Analisando o desfecho de Sucesso final, disponível em todos os estudos, não encontramos diferença entre as duas abordagens terapêuticas (DR = -0,06; IC 95% = -0,15 a 0,04). Três estudos (251 pacientes) apresentaram dados de complicação, e a análise não mostrou diferença entre os tratamentos endoscópico e cirúrgico (DR = -0,15; IC 95% = -0,53 a 0,23), sem a possibilidade de descartar o viés de seleção para este desfecho. Conclusões: Considerando os desfechos de ressecção primária completa, sucesso primário e recorrência na comparação do tratamento cirúrgico com o tratamento endoscópico para adenomas de papila, a abordagem cirúrgica tem resultados significativamente melhores. Com relação ao sucesso final, não houve diferença entre os dois tratamentos. No desfecho das taxas de complicação, esta revisão sistemática não permite uma conclusão confiável devido à presença de alta heterogeneidade e provável viés de publicação nesta comparação / Objectives: To address the outcomes of endoscopic resection compared to surgery in the treatment of ampullary adenomas. Methods: A systematic review and meta-analysis was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations. Medline, Embase, Cochrane, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Scopus and Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases were scanned. Studies included patients with ampullary adenomas and comparison data considering endoscopic treatment and surgery for the following outcomes: Complete primary resection; Primary success; Reccurence; Final success; and Complications. The analysis were based on both random and fixed effects model. Results: Five retrospective cohort studies were selected, with 465 patients. All five studies had complete primary resection data available, showing a difference that favours surgical treatment (Risk Difference = -0.22, 95% Confidence Interval = -0.41 to -0.04). Primary success data were identified in all five studies too. Analysis showed that surgical approach overcome endoscopic treatment in this outcome (RD = -0.13, 95% CI = -0.24 to -0.02). Recurrence data was found in all studies (465 patients), with benefit for the surgical treatment (RD = 0.12, 95% CI = -0.01 to 0.22). Analyzing the final success outcome, available in all studies, we found no difference between the two therapeutic approaches (RD = -0.06, 95% CI = -0.15 to 0.04). Three studies (251 patients) presented complication data and analysis shown no difference between the approaches (RD = -0.15, 95% CI = -0.53 to 0.23), not discarding the possibility of presence of selection bias for this outcome. Conclusions: Considering complete primary resection, primary success and recurrence outcomes, surgical approach achieves significantly better results. Regarding the final success, there was no difference between the two treatments. Addressing complication data, this systematic review does not allow for a reliable conclusion due to the presence of high heterogeneity and likely publication bias in this comparison

<p>Inferno constitutes a turning point in Strindberg's literary production in that scriptural quotations appear more frequently and a new style emerges. This thesis presents the characteristics of the scriptural quotations appearing in Inferno (1897) and Jacob Wrestles (a fragment following Légendes, written in French and in Swedish in 1898). Comparative, discourse, textual and intertextual approaches are used to define the place and role of scriptural quotations in this literary corpus.</p><p>From a historical point of view, both novels are part of the religious history of late 19th century France, where religion played a more important role than during the scientific, rationalist era characterizing the preceding decades. Strindberg adopts a new style corresponding to the spirit of his time. The art of "quoting the Bible at random" is a rhapsodic style, which appears mainly in Strindberg’s correspondence, in his Occult Diary (writings contemporary with Inferno) and in the work of some French 19th century writers. This style originates, above all, in the occult tradition, but it is also a means of imitating the Bible and identifying with a prophetic figure.</p><p>The research discussed in this dissertation has made it possible to determine, for the first time, what Bible translations are used in the two novels by Strindberg (translations by Ostervald and Martin / Roques). Five different types of rewritings of quotations were found: omissions, cutting of verses, substitutions, typographical changes and inversions. These variations were aimed at harmonising the Biblical text and the Strindbergian text, while removing contextual and theological elements that bothered the writer. The discourse analysis has concentrated on the quotations viewed as reported speech, distinguishing different ways of introducing Biblical verses in the novel. It was found that the narrator's subjectivity is present in the comments leading up to the quotations. The polyphonic character of some quotations has stressed the importance of identification play between the narrator and certain quotations characters such as Christ, Job and the psalmist.</p><p>The intertextual analysis has revealed a large number of similarities in the scriptural quotations in the literary production of Strindberg, Swedenborg and French 19th century literature. It is shown that Inferno contains various quotations that appear in Occult Diary and in other writers’ works, such as those of Swedenborg, Péladan, Zola, Huysmans and Chateaubriand. Jacob Wrestles, on the other hand, does not include as many intertextual elements but instead reassembles many scriptural quotations that were underlined in the Bible translation used for this novel: La Sainte Bible, Ostervald's translation from 1890, which can be found in Blå Tornet (The Strindberg Museum in Stockholm). Strindberg is consequently recycling Biblical material when he writes Inferno, while resorting to the French Bible of Ostervald from 1890 to write Jacob Wrestles.</p><p>The quotations strewn in Inferno constitute a crescendo and reveal the narrator’s unsuccessful attempt at conversion, at the same time forming the structure of a complaint psalm in which the narrator cries out his suffering and awaits liberation. In the French text of Jacob Wrestles, the writer offers a package of scriptural quotations in order to identify the narrator as "a religious man", imploring God's mercy like Moses and Job. In the Swedish text of Jacob Wrestles, a new perspective is introduced as a result of the change in language, the change from Old to New Testament, the new spiritual disposition of the narrator and the sudden intrusion of the writer in the narrator’s space. The role of scriptural quotations in the entire fragment of Jacob Wrestles is a true linguistic, thematic and theological revolution, which accounts for the narrator's extraordinary religious evolution. The misery of the narrator in Inferno allows a ray of Christian hope, which will persist in Strindbergs’s literary production post-Inferno.</p>

Diese Studie vergleicht die politische Kommunikation in Deutschland und Polen. Der Forschungsüberblick macht deutlich, dass international vergleichende Analysen, die etablierte Demokratien und Transformationsländer umfassen, relativ selten durchgeführt werden. Dabei gibt es keine Untersuchungen, die sich mit dem Vergleich der politischen Kommunikation zwischen Deutschland und Polen befassen. Mit der Untersuchung der beiden Länder wird die Dissertation die bestehende Forschungslücke schließen. Die politische Kommunikation wird in dieser Studie aus der Sicht von zwei unterschiedlichen Akteuren – Parteien und Journalisten – gezeigt. --- Kapitel 1: Es ist nicht mehr möglich, die moderne politische Kommunikation als Phänomen zu begreifen, das man auf singuläre nationale Räume beschränken könnte. Vielmehr lässt sich über nationalübergreifende Kommunikationsprozesse sprechen, die sich in den Metathemen Amerikanisierung, Globalisierung und Modernisierung wiederspiegeln. In diesem Zusammenhang ergibt sich die Frage, ob es sich um eine generalisierbare Entwicklung der politischen Kommunikation handelt. Ein Schwerpunkt des Interesses dieser Forschungsarbeit liegt deswegen bei der Frage nach den Gemeinsamkeiten und den Unterschieden in der politischen Kommunikation in beiden Ländern, die durch die Mediatisierungsprozesse beinflusst werden. Vor diesem Hindergrund stellt Kapitel 1 die wichtigsten Trends in der politischen Kommunikation dar. Im nächsten Schritt werden ausgewählte Indikatoren der Mediatisierung (u.a. Professionalisierung, Personalisierung, Negativität, Emotionalisierung), die dann im empirischen Teil getestet werden, erläutert. Gleichzeitig stellt Kapitel 1 eine Übersicht über international vergleichende Analysen, die sich der Untersuchung der politischen Kommunikation widmen, vor. Der Focus wird dann schließlich auf die Studien in Polen und Deutschland gelegt und die Relevanz der vorliegenden Analyse diskutiert. --- Kapitel 2: Die Dissertation zeigt in welchen politischen Rahmen die politische Kommunikation in Deutschland und Polen eingebettet ist. Deswegen wird im Kapitel 2 das politische System der analysierten Länder diskutiert. In diesem Zusammenhang werden drei Elemente des politischen Systems angesprochen: das Parteiensystem, das Wahlsystem und die politische Partizipation. Dadurch wird gezeigt, dass die politische Einbettung spürbare Auswirkungen hat, die den gesellschaftlichen, sozio-ökonomischen und historischen Kontext mitdefinieren. Dies lässt gleichzeitig Unterschiede zwischen einem Transformationsland (Polen) und einer etablierten Demokratie (Deutschland) aufzeigen und Gründe der wichtigsten politischen Tendenzen erklären. --- Kapitel 3: Ebenso wie die Strukturbedingungen des politischen Systems das Handeln politischer Akteure beeinflussen, so wirken die Rahmenbedingungen des Mediensystems auf das Handeln von Journalisten. Aus diesem Grunde wird im Kapitel 3 das Mediensystem in Deutschland und Polen thematisiert. Die Analyse basiert auf der Klassifikation von Hallin &amp; Mancini (2004a). Dabei werden die von den Autoren vorgeschlagenen Indikatoren benutzt, um das Mediensystem in beiden Ländern einzuordnen. Dies ist besonders wichtig, weil die politische Kommunikation immer häufiger von medialer Umgebung abhängig ist. --- Kapitel 4: Die im Kapitel 1, 2 und 3 dargestellten Phänomene und Entwicklungen formen Kapitel 4, das Forschungshypothesen vorstellt. Die formultierten Hypothesen münden in zwei inhaltsanalytischen Untersuchungen, die die politische Kommunikation aus der Perspektive der politischen Parteien und Journalisten präsentieren. Es handelt sich dabei um zwei Fallstudien, die dann im Kapitel 6 und 7 getrennt examiniert werden. --- Kapitel 5: Im nächsten Schritt wird das Forschungsdesign und die Operationalisierung der Hypothesen erläutert. Da die Mediatisierungsprozesse besonders deutlich während der Wahlkampagnen zu ermitteln sind, wird die politische Kommunikation in Deutschland und Polen im Kontext von politischen Kampagnen dargestellt. Dies verspricht auch inhaltlich fokussiertes Material für die Untersuchung. Die erste Fallstudie untersucht die Wahlspots der Parteien und zeigt, inwiefern sich die Wahlwerbung in beiden Ländern unterscheidet. Die zweite Fallstudie bietet die Analyse der Medienberichterstattung, um festzustellen, wie die Journalisten in Deutschland und Polen die Wahlkampagnen darstellen. Die Kompläxität des Geflechtes der auf die politische Kommunikation Einfluss nehmenden Variablen macht den Einsatz komplexer Analyseverfahren erforderlich. Die Wahlspots und die Wahlkampfberichterstattung werden in der Dissertation mit der sozialwissenschaftlichen Methode der Inhaltsanalyse untersucht. Als Methode zur Erhebung sozialer Wirklichkeit ist die Inhaltsanalyse für die Untersuchung besonders geeignet. Ihre Vorteile werden in diesem Kapitel angesprochen. Darüber hinaus werden hier die Codebücher, die für die Analyse der Wahlwerbung und Medienberichterstattung vorbereitet wurden, dargestellt und die Codierungsvorgehensweise präsentiert. Schließlich werden die einzelnen Variablen besprochen und die Ergebnisse der durchgeführten Pretests geliefert. --- Kapitel 6: Im nächsten Schritt werden die Ergebnisse der empirischen Analyse der Wahlspots dargestellt. Die Studie zeigt, wie die politischen Parteien ihre Wahlspots gestalten und inwiefern die Wahlwerbung die Mediatisierungsprozesse wiederspiegelt. Im Kapitel 6 werden die Hypothesen, die im Kapitel 4 formuliert wurden, getestet. Dabei werden die Befunde im Kontext von solchen Aspekten wie u.a. Professionalisierung, Personalisierung, Negativität und Emotionalisierung dargestellt. --- Kapitel 7: Im Kapitel 7 wird die politische Kommunikation aus der Sicht der Journalisten examiniert. Die empirische Auswertung dient dazu, die im Kapitel 4 formulierten Hypothesen zu prüfen. Darüber hinaus wird die Medienberichterstattung in den Prozessen der Mediatisierung dargestellt. Dabei wird u.a. Personalisierung und Negativität der Berichterstattung präsentiert. Es wird auch gezeigt, inwiefern die Medien ihre politische Präferenzen zeigen. --- Kapitel 8: Im letzten Kapitel werden die Ergebnisse der Untersuchung im Licht der Theorie und formulierten Hypothesen diskutiert. Dabei werden auch die Defizite der Studie und potenzielle Barrieren der vergleichenden Studien präsentiert. Kapitel 8 lenkt dann den Blick in die Zukunft und zeigt, welche Aspekte der politischen Kommunikation untersucht werden sollten. Solche internationalen Vergleiche, besonders wenn sie Transformationsländer und etablierte Demokratien umfassen, können dazu dienen, Gemeinsamkeiten in der Dreiecksbeziehung zwischen politischem System, Medien und Wählerschaft zu ermitteln, um so übergreifenden Entwicklungen auf die Spur zu kommen.
