O objetivo deste trabalho foi comparar radiografias periapicais (RP) com quatro protocolos diferentes de tomografia computadorizada por feixe cônico (TCFC) para o diagnóstico de fraturas alveolares e radiculares. Oitenta incisivos superiores (40 hígidos e 40 com fratura radicular) de cães foram inseridos em 80 alvéolos anteriores (40 hígidos e 40 com fratura alveolar) de 20 maxilas caninas maceradas em posições randomizadas previamente. Cada maxila foi submetida a duas RP com diferentes angulações verticais e a quatro protocolos de TCFC com variações em kV (72 kV ou 80 kV), mA (4 mA ou 8 mA) e número de projeções (400 ou 500). As imagens obtidas foram randomizadas e analisadas por dois radiologistas odontológicos por duas vezes, com um intervalo de duas semanas entre as observações. Valores de sensibilidade, especificidade e acurácia para as análises binárias foram melhores para TCFC do que RP; fraturas radiculares apresentaram melhores resultados corretos de diagnóstico do que fraturas alveolares para RP e TCFC. Para a verificação da acurácia através da área sob a curva (ASC) de característica de operação do receptor (ROC), resultados de TCFC foram melhores do que resultados de RP; fraturas radiculares apresentaram melhores resultados corretos de diagnóstico do que fraturas alveolares para RP e TCFC. Quando a localização da fratura alveolar (porção vestibular ou porção palatina do alvéolo) ou da fratura radicular (terço cervical, médio ou apical) foi levada em consideração para a realização das análises estatísticas, os mesmos apresentaram uma discreta redução. RP com duas angulações verticais diferentes podem ser consideradas um bom método para diagnosticar fraturas radiculares. Quando não é possível diagnosticar fraturas radiculares e alveolares em RP, o próximo exame de escolha é a TCFC. / The purpose of this study was to compare periapical radiograph (PR) and four different protocols of cone-beam computed tomography (CBCT) in the diagnosis of alveolar and root fractures. Eighty incisor teeth (40 higid and 40 with root fracture) from the canine species were inserted in 80 anterior alveolar sockets (40 higid sockets and 40 sockets with alveolar fracture) of 20 macerated canine maxillae in previously randomized positions. Afterwards, each maxilla was submitted to periapical radiograph in two different vertical angulation incidences, and to four different CBCT protocols, with variations in kV values (72 kV or 80 kV), mA values (4 mA or 8 mA) and number of frames (400 ou 500). The images were randomized and posteriorly analyzed by two oral and maxillofacial radiologists two times, with a two-week interval between observations. Sensitivity, specificity and accuracy values for binary analyses were better for CBCT than for PR; root fractures showed better diagnosis results than alveolar fractures for both CBCT and PR. When accuracy was calculated through area under the receiver operating characteristic (ROC) curve, CBCT showed better results than PR; root fractures showed better diagnosis results than alveolar fractures for both CBCT and PR. There was a slight reduction in value results when we considered the alveolar fracture (buccal or palatine portion of alveolus) or root fracture (cervical, median or apical third) location to perform the statistical analyses. Periapical radiograph with two different vertical angulations may be considered an accurate method to detect root fractures. When no fractures are diagnosed in PR and the patient describes pain symptoms, the subsequent exam of choice is CBCT.

and surgically assisted maxillary rapid expansion (ERMAC). A three-dimensional
evaluation was performed with CBT and scanning of the intraoral and gingival
models at the preoperative (T0) and postoperative (T1 and T2) moments in order to
evaluate linear and angular movements with bone and dental changes underlying.
The possible morphological alterations of the related structures were evaluated:
nasopharynx volume, oropharynx volume in relation to ERMAC. The sample
consisted of 15 individuals (8 males and 7 females aged 19 to 54 years) submitted to
MRS. In general, ERMAC produced a significant increase in all linear distances of
the maxilla. There was a greater expansion of the maxilla in the anterior region than
in the posterior one. The ERMAC produced a significant increase of the skeletal
dimensions of the maxilla in the transversal direction. In the airways, there was a
significant increase in all patients and the greatest effects of ERMAC occurred at the
dental level, with the buccal inclination of the first molars and the upper premolars
being observed. / Objetiva-se com este estudo retrospectivo avaliar as altera??es morfol?gicas produzidas ap?s e expans?o r?pida maxilar assistida cirurgicamente (ERMAC). Realizou-se uma avalia??o tridimensional com TCCB e escaneamento dos modelos de gesso e dos dentes intraoral no momento pr?-operat?rio (T0) e p?s-operat?rio
(T1 e T2) a fim de avaliar os movimentos lineares e angulares com as altera??es ?sseas e dent?rias subjacentes. Enhanced Computerized Surgical Planning System in Craniomaxillofacial SurgeryChang, Yu-Bing 2011 May 1900 (has links)
In the field of craniomaxillofacial (CMF) surgery, surgical planning is an important and necessary procedure due to the complex nature of the craniofacial skeleton. Computed tomography (CT) has brought about a revolution in virtual diagnosis, surgical planning and simulation, and evaluation of treatment outcomes. It provides high-quality 3D image and model of skull for Computer-aided surgical planning system (CSPS).
During the planning process, one of the essential steps is to reestablish the dental occlusion. In the first project, a new approach is presented to automatically and efficiently reestablish dental occlusion. It includes two steps. The first step is to initially position the models based on dental curves and a point matching technique. The second step is to reposition the models to the final desired occlusion based on iterative surface-based minimum distance mapping with collision constraints. With linearization of rotation matrix, the alignment is modeled by solving quadratic programming. The simulation was completed on 12 sets of digital dental models. Two sets of dental models were partially edentulous, and another two sets have first premolar extractions for orthodontic treatment. Two validation methods were applied to the articulated models. The results show that using the proposed method, the dental models can be successfully articulated with a small degree of deviations from the occlusion achieved with the gold-standard method.
Low contrast resolution in CBCT image has become its major limitation in building skull model. Intensive hand-segmentation is required to reconstruct the skull model. Thin bone images are particularly affected by this limitation. In the second project, a novel segmentation approach is presented based on wavelet active shape model (WASM) for a particular interest in the outer surface of the anterior wall of maxilla. 19 CBCT datasets are used to conduct two experiments. This model-based segmentation approach is validated and compared with three different segmentation approaches. The results show that the performance of this model-based segmentation approach is better than those of the other approaches. It can achieve 0.25 +/- 0.2mm of surface error distance from the ground truth of the bone surface.
Field of view (FOV) can be reduced in order to reduce unnecessary radiation dose in CBCT. This ROI imaging is common in most of the dentomaxillofacial imaging and orthodontic practices. However, a truncation effect is created due to the truncation of projection images and becomes one of the limitation in CBCT. In the third project, a method for small region of interest (ROI) imaging and reconstruction of the image of ROI in CBCT and two experiments for measurement of dosage are presented. The first experiment shows at least 60% and 70% of radiation dose can be reduced. It also demonstrates that the image quality was still acceptable with little variation of gray by using the traditional truncation correction approach for ROI imaging. The second experiment demonstrates that the images reconstructed by CBCT reconstruction algorithms without truncation correction can be degraded to unacceptable image quality.

Évaluation des effets de l’expansion palatine rapide assistée chirurgicalement (EPRAC) sur les voies aériennes supérieures à l’aide de la tomodensitométrie à faisceau coniqueDrapeau, Audrey 06 1900 (has links)
Introduction : L’expansion palatine rapide assistée chirurgicalement (EPRAC) est une option de traitement privilégiée chez les patients ayant atteint la maturité squelettique et présentant une déficience transverse du maxillaire. L’effet bénéfique de l’EPRAC sur la fonction respiratoire est régulièrement mentionné, toutefois, encore peu d’études ont évalué son impact sur les voies aériennes supérieures. L’objectif de cette étude clinique prospective comparative consistait à évaluer les effets tridimensionnels de l’EPRAC sur la cavité nasale, le nasopharynx et l’oropharynx à l’aide de la tomodensitométrie.
Méthodologie : L’échantillon était constitué de 14 patients (5 hommes, 9 femmes) dont l’âge moyen était de 23,0 ± 1,9 ans (16 ans 4 mois à 39 ans 7 mois). Tous ont été traités avec un appareil d’expansion de type Hyrax collé et l’expansion moyenne a été de 9,82 mm (7,5 - 12,0 mm). Tous ont eu une période de contention d’une année avant le début de tout autre traitement orthodontique. Une évaluation par tomodensitométrie volumique à faisceau conique a été réalisée aux temps T0 (initial), T1 (6 mois post-expansion) et T2 (1an post-expansion) et le volume des fosses nasales, du nasopharynx et de l’oropharynx ainsi que les dimensions de la zone de constriction maximale de l’oropharynx ont été mesurés sur les volumes tridimensionnels obtenus.
Résultats : Les résultats radiologiques ont démontré une augmentation significative du volume des fosses nasales et du nasopharynx ainsi qu’une augmentation de la zone de constriction maximale de l’oropharynx à 6 mois post-expansion. Par la suite, une portion du gain enregistré pour ces trois paramètres était perdue à un an post-EPRAC sans toutefois retourner aux valeurs initiales. Aucun effet significatif sur le volume de l’oropharynx n’a été observé. De plus, aucune corrélation significative entre la quantité d’expansion réalisée et l’ensemble des données radiologiques n’a été observée. L’analyse de la corrélation intra-classe a démontré une excellente fiabilité intra-examinateur.
Conclusions : L’EPRAC entraîne un changement significatif du volume de la cavité nasale et du nasopharynx. L’EPRAC ne modifie pas le volume de l’oropharynx, par contre, un effet significatif sur la zone de constriction maximale de l’oropharynx est noté. Les effets observés n’ont pas de corrélation avec le montant d’activation de la vis d’expansion. / Introduction: Surgically assisted rapid palatal expansion (SARPE) is a treatment of choice for patients who have reached skeletal maturity and present a maxillary transverse deficiency. It is often mentioned that SARPE has the benefit to improve respiratory function, however, only a few research projects have investigated the effects of SARPE on the upper airways. The objective of this clinical prospective comparative study was to evaluate the three-dimensional effects of SARPE on the nasal cavity, the nasopharynx and the oropharynx using computed tomography.
Materials and Methods: The sample consisted of 14 subjects (5 males, 9 females) whose mean age was 23.0 ± 1.9 years (range: 16 y. 4 mo. to 39 y. 7 mo.). All patients were treated using a bonded Hyrax expander and the mean expansion was 9.82 mm (7.5-12.0 mm). A one-year retention period was undertaken before the beginning of any other orthodontic treatment. A cone-beam computed tomography (CBCT) evaluation was performed at T0 (initial), T1 (6 months post-expansion) and T2 (1 year post-expansion), and then the nasal cavity, nasopharyngeal and oropharyngeal volumes and the oropharyngeal minimal cross-sectional area were measured on the three-dimensional volumes that were obtained.
Results: Radiological results have demonstrated a significant increase of the nasal and nasopharyngeal volumes and also an increase of the oropharyngeal minimal cross-sectional area at 6 months post-expansion. At one year post-SARPE, for these three parameters, a part of the gain was lost but did not return to the initial values. No significant effect on oropharyngeal volume was found. No significant correlation between expansion screw activation and radiological parameters were noted. Intra-class correlation analysis showed excellent intra-examiner reliability.
Conclusions: SARPE causes significant changes of the nasal cavity and nasopharyngeal volumes. SARPE does not modify the oropharyngeal volume, but induces significant changes of the oropharyngeal minimal cross-sectional area. The observed effects do not have a correlation with the amount of expansion screw activation.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A projeção de incisivos e expansão dos arcos dentários são uma alternativa valiosa à extração dentária, especialmente quando se considera a estética facial em pacientes adultos. O efeito da projeção ortodôntica dos incisivos inferiores sobre o periodonto é controverso devido às avaliações em exames bidimensionais e os aspectos multi-fatoriais que envolvem as recessões gengivais. O objetivo deste estudo foi comparar as modificações na altura da borda alveolar dos dentes ântero-inferiores de pacientes, que foram submetidos à projeção ortodôntica, com pacientes tratados sem projeção; e correlacionar estas modificações com o grau de inclinação dentária, com as alterações da distância bicanina e com o biotipo gengival. Pacientes adultos com mais de 3 mm de falta de espaço no arco inferior e curva de Spee moderada ou acentuada compuseram o grupo experimental (n=15). O grupo controle (n=7) consistiu de pacientes com bons arcos inferiores, que não necessitavam de grandes movimentos dentários. Estes pacientes foram submetidos a alinhamento e nivelamento dentário até o fio de aço .020". Tomografias computadorizadas de feixe cônico (TCFC) foram obtidas antes do tratamento e ao final da fase de alinhamento e nivelamento. As alturas das bordas alveolares (BA) de incisivos e caninos inferiores foram medidas nas TCFC em reconstruções 3D e comparadas entre os grupos e entre os tempos pelos testes-t de Student não pareado e pareado, respectivamente. As BA foram correlacionadas com o grau de inclinação dentária (IMPA), com a distância intercaninos (DIC) e com o biotipo gengival (BG) pelo teste de correlação de Pearson. Os resultados demonstraram que os caninos inferiores do grupo experimental apresentaram perda óssea significativa (p<0,005), quando comparados com o grupo controle, em média 2,5 mm. As BA dos dentes 43, 33 e 32 ao final do alinhamento e nivelamento eram significativamente maiores do que ao início do tratamento no grupo experimental (p<0,001). Não foram encontradas diferenças significativas entre as medidas iniciais e finais das BA de todos os dentes do grupo controle. Apesar destes resultados, não foram encontradas correlações entre a remodelação da BA e o IMPA, a DIC e o BG. Pode-se concluir que o aumento no comprimento do arco inferior com arcos ortodônticos contínuos aumenta a inclinação dos incisivos inferiores e a DIC. O aumento da DIC parece exercer maior efeito sobre a BA dos caninos inferiores do que a inclinação de incisivos sobre a BA dos incisivos inferiores. No entanto, as modificações da BA não estão associadas ao grau de inclinação dos incisivos, a quantidade de expansão do arco inferior e ao biotipo gengival. / Incisor proclination and dental arch expansion are a valuable choice of treatment instead of extraction, especially when considering facial aesthetics in adult patients. The effect of orthodontic projection of the lower incisors on the periodontium is controversial because of evaluation in two-dimensional exams and the multi factorial aspects of gingival recessions. The aim of this study was to compare alveolar height modifications in the lower arch of patients that were treated with orthodontic proclination, with patients treated without proclination, and correlate these modifications with the degree of dental projection, with the changes in the lower intercanine width and with gingival biotype. Adult patients with lower arch discrepancy of more than 3 mm and moderate or pronounced curve of Spee composed the experimental group (n=15). The control group (n=7) consisted of patients with good lower arches, that did not need large dental movements. These patients had their dental arches aligned and leveled up to a .020 wire. Cone beam computed tomography (CBCT) was taken before treatment and after leveling and alignment. The height of the alveolar border (AB) of the lower incisors and canines were measured on CBCT with 3D reconstruction, and compared between groups, and between times, by paired and independent Students t-test, respectively. The AB were correlated with the degree of dental inclination (IMPA), intercanine width (ICW) and gingival biotype (GB) by the Pearson correlation test. Results showed that the lower canines of the experimental group presented significant bone loss (p<0,005) when compared with the control group, with a mean of 2.5 mm. The AB of teeth 43, 33 and 32 at the end of alignment were significantly greater than at beginning of treatment for the experimental group (p<0,001). No significant differences were found between initial and final AB measurements for all control group teeth. Despite these results, no correlation was found between remodeling of AB and IMPA, ICW and GB. It can be concluded that the increase of lower arch length with continuous orthodontic archwires increases lower incisor inclination and ICW. The increase of the ICW seems to exert more effect over the AB of lower canines, than the incisor inclination over the AB of the incisors. However, AB modifications are not associated with the degree of incisors inclination, neither the amount of lower arch expansion, nor to the gingival biotype.

Previous issue date: 2011-10-19 / This study evaluated the skeletal effects of maxillary protraction using 2D images (lateral radiograph) generated from the cone beam CT (3D images). The sample consisted of 20 children (15 females and 5 males), aged 5,6 -10,7 years who had malocclusion Class III malocclusion. CT was performed before treatment (T1) and after treatment (T2). The treatment was performed by maxillary protraction using the Hyrax expansion appliance associated with individualized face mask with the force of 600 to 800g each side for 14 hours a day. The correction of canine relationship in Class I or Class II in their overcorrection was obtained after 4-7 months of treatment. To check the systematic error and random test was used for paired "t" and the formula of Dahlberg, respectively. The test paired "t" (p <0.05) showed a significant difference between the cephalometric measurements obtained in T1 and T2. In the maxilla there were increases in SNA 2.2°, Nperp A 1.47mm and Co-A 2.58mm. In the mandible, SNB decreased by -0.54° and P-Nperp, -1.45mm, while Co-Gn increased 1.04mm. There was improvement in maxillomandibular relationship ANB 2.74° and Wits 4.23mm. Variables GoGn.SN, Gn.SN, FH.Md, Mx.Md and AFAI demonstrating that there was an increased rotation of the mandible in a clockwise direction. The palatal plane rotated in a counterclockwise direction. It can be concluded that the treatment ofmaxillary protraction in early age has promoted an improvement in maxillomandibular relationship due to a shift of the jaw forward and a displacement of the mandible down and back. / Este estudo avaliou os efeitos esqueléticos da tração reversa da maxila utilizando imagens 2D (telerradiografia lateral) geradas a partir da tomografia de feixe cônico (imagens 3D). A amostra foi composta por 20 crianças (15 do gênero feminino, e 5 do masculino), com idade variando de 5,6 a 10,7 anos que apresentavam má-oclusão de Classe III de Angle. A tomografia foi realizada antes do tratamento (T1) e logo após o tratamento (T2). O tratamento foi realizado por meio da tração reversa da maxila utilizando-se o aparelho expansor Hyrax associado à máscara facial individualizada, com força de 600 a 800g de cada lado, durante 14 horas por dia. A correção da relação de caninos em Classe I ou com sua sobrecorreção em Classe II foi obtida após 4 a 8 meses de tratamento. Para verificar o erro sistemático e casual foi utilizado o teste t pareado e a fórmula de Dahlberg, respectivamente. O teste t pareado (p<0,05) mostrou diferença significante entre as medidas cefalométricas obtidas em T1 e T2. Na maxila houve aumento do SNA 2,2°, A-Nperp 1,47mm e em Co-A 2,58mm. Na mandíbula, SNB diminuiu -0,54° e P-Nperp, -1,45mm, enquanto Co-Gn aumentou 1,04mm. Houve melhora na relação maxilo-mandibular ANB 2,74° e Wits 4,23mm. As variáveis GoGn.SN, Gn.SN, FH.Md, Mx.Md, e AFAI aumentaram demonstrando que houve uma rotação da mandíbula no sentido horário. O plano palatino rotacionou no sentido anti-horário. Pode se concluir que o tratamento de tração reversa da maxila na idade precoce promoveu uma melhora na relação maxilo-mandibular devido a um avanço da maxila e um deslocamento da mandíbula para baixo e para trás.

As BA foram correlacionadas com o grau de inclinação dentária (IMPA), com a distância intercaninos (DIC) e com o biotipo gengival (BG) pelo teste de correlação de Pearson. Os resultados demonstraram que os caninos inferiores do grupo experimental apresentaram perda óssea significativa (p<0,005), quando comparados com o grupo controle, em média 2,5 mm. As BA dos dentes 43, 33 e 32 ao final do alinhamento e nivelamento eram significativamente maiores do que ao início do tratamento no grupo experimental (p<0,001). Não foram encontradas diferenças significativas entre as medidas iniciais e finais das BA de todos os dentes do grupo controle. Apesar destes resultados, não foram encontradas correlações entre a remodelação da BA e o IMPA, a DIC e o BG. Pode-se concluir que o aumento no comprimento do arco inferior com arcos ortodônticos contínuos aumenta a inclinação dos incisivos inferiores e a DIC. O aumento da DIC parece exercer maior efeito sobre a BA dos caninos inferiores do que a inclinação de incisivos sobre a BA dos incisivos inferiores. No entanto, as modificações da BA não estão associadas ao grau de inclinação dos incisivos, a quantidade de expansão do arco inferior e ao biotipo gengival. / Incisor proclination and dental arch expansion are a valuable choice of treatment instead of extraction, especially when considering facial aesthetics in adult patients. The effect of orthodontic projection of the lower incisors on the periodontium is controversial because of evaluation in two-dimensional exams and the multi factorial aspects of gingival recessions. The aim of this study was to compare alveolar height modifications in the lower arch of patients that were treated with orthodontic proclination, with patients treated without proclination, and correlate these modifications with the degree of dental projection, with the changes in the lower intercanine width and with gingival biotype. Adult patients with lower arch discrepancy of more than 3 mm and moderate or pronounced curve of Spee composed the experimental group (n=15). The control group (n=7) consisted of patients with good lower arches, that did not need large dental movements. These patients had their dental arches aligned and leveled up to a .020 wire. Cone beam computed tomography (CBCT) was taken before treatment and after leveling and alignment. The height of the alveolar border (AB) of the lower incisors and canines were measured on CBCT with 3D reconstruction, and compared between groups, and between times, by paired and independent Students t-test, respectively. The AB were correlated with the degree of dental inclination (IMPA), intercanine width (ICW) and gingival biotype (GB) by the Pearson correlation test. Results showed that the lower canines of the experimental group presented significant bone loss (p<0,005) when compared with the control group, with a mean of 2.5 mm. The AB of teeth 43, 33 and 32 at the end of alignment were significantly greater than at beginning of treatment for the experimental group (p<0,001). No significant differences were found between initial and final AB measurements for all control group teeth. Despite these results, no correlation was found between remodeling of AB and IMPA, ICW and GB. It can be concluded that the increase of lower arch length with continuous orthodontic archwires increases lower incisor inclination and ICW. The increase of the ICW seems to exert more effect over the AB of lower canines, than the incisor inclination over the AB of the incisors. However, AB modifications are not associated with the degree of incisors inclination, neither the amount of lower arch expansion, nor to the gingival biotype.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Imagens de tomografia computadorizada (TC) permitem a visualização, sem distorções ou sobreposições, do complexo maxilo-facial, principalmente do osso alveolar. Estudos demonstraram boa reprodutibilidade e precisão da mensuração da altura da borda alveolar, todavia a influência da espessura óssea ainda é pouco descrita. Através da comparação com a mensuração direta, o objetivo deste estudo foi avaliar a precisão, reprodutibilidade e a influência da espessura óssea, na mensuração da altura da borda alveolar em imagens volumétricas e imagens bidimensionais multiplanares em TC de feixe cônico (TCFC) e em TC espiral (TCE). Utilizando 10 mandíbulas secas de humanos, 57 dentes anteriores foram tomografados em equipamentos iCAT (Imaging Science International, Hatfield, PA, EUA) e Brilliance 64 canais (Philips Eletronics, Eindhoven, Holanda), ambos utilizando voxels de 0,25 mm. Através de imagens volumétricas (3D) e imagens bidimensionais (2D) de cortes multiplanares, foi comparada a mensuração da altura da borda alveolar dessas imagens com a mensuração direta nas mandíbulas, feita por vestibular e lingual, por três avaliadores, com o auxílio de um paquímetro, totalizando 114 bordas alveolares medidas. Alta reprodutibilidade intra-avaliador (0,999 a 0,902) e interavaliador (0,998 e 0,868) foi observada através do índice de correlação intraclasse (ICC). Observou-se alta correlação entre a mensuração direta e indireta da altura da borda alveolar em imagens 2D, sendo r=0,923** e 0,916**, e em imagens 3D, com r=0,929** e 0,954*, em TCFC e TCE, respectivamente. Imagens 2D superestimam a altura da borda alveolar em 0,32 e 0,49 mm e imagens 3D em 0,34 e 0,30 mm, em TCFC e TCE respectivamente. Quando o osso alveolar apresenta espessura de no mínimo 0,6 mm a média da diferença entre medidas diretas e indiretas é de 0,16 e 0,28 mm em imagens 2D e de 0,12 e 0,03 mm em imagens 3D para TCFC e TCE respectivamente, sendo que 95% do limite de concordância varia de -0,46 a 0,79 mm e -0,32 a 0,88 mm em imagens 2D, e de -0,64 a 0,67 mm e -0,57 a 0,62 mm em imagens 3D, para TCFC e TCE respectivamente. Quando o osso alveolar é mais fino do que 0,6 mm a TC é imprecisa, pois 95% do limite de concordância variou de -1,74 a 5,42 mm e -1,64 a 5,42 mm em imagens 2D, e de -3,70 a 4,28 mm e -3,49 a 4,25 mm em imagens 3D, para TCFC e TCE respectivamente. Conclui-se que a mensuração da altura da borda alveolar através de imagens tomográficas apresenta alta reprodutibilidade, sendo que quando a borda alveolar apresenta pelo menos 0,6 mm, a precisão da mensuração é alta, todavia quando esta espessura é menor do que 0,6 mm a técnica é imprecisa. / Computerized tomogoraphy (CT) images allow for visualization, with no distortions or superimpostions, of the maxillo-facial complex, especially of the alveolar margins. Studies have demonstrated good reproducibility and precision of the alveolar bone height, however the influence of bone thickness is still scarcely described. The purpose of this study was, by comparing the direct measurement of alveolar bone height, to evaluate the precision, reproducibility and the influence of bone thickness on the same measurement in cone beam CT (CBCT) and spiral CT (SCT) volumetric and biplane images. Ten dry human mandibles, with a total of 57 anterior teeth were tomographed with an iCAT (Imaging Science International, Hatfield, PA, EUA) and a 64 channels Brilliance 64 tomograph (Philips Eletronics, Eindhoven, Holland), both using voxels of 0.25 mm. Alveolar bone height was measured on volumetric (3D) and bidimensional (2D) images of tomograms and compared with direct caliper measurements performed on the dry mandibles, on the buccal and lingual sides, by three raters in a total of 114 measurements. High intra-rater (0.999 a 0.902) and inter-rater (0.998 e 0.868) reproducibility was observed using the intraclass correlation coefficient (ICC). A high correlation was also observed between the direct and indirect measurements of alveolar bone height in 2D (r=0.923** e 0.916**), and 3D images (r=0.929** e 0.954*), for CBCT and SCT, respectively. Two-dimensional images overestimate alveolar bone height in 0.32 and 0.49 mm and 3D images in 0.34 and 0.30 mm, for CBCT and SCT, respectively. When alveolar bone thickness is at least 0.6 mm the mean difference is of 0.16 and 0.28 mm in 2D images and of 0.12 and 0.03 mm in 3D images for CBCT and SCT, respectively, with 95% limits of agreement ranging from -0.46 to 0.79 mm and -0.32 to 0.88 mm in 2D images, and of -0.64 to 0.67 mm and -0.57 to 0.62 mm in 3D images, for CBCT and SCT, respectively. When alveolar bone thickness is smaller than 0.6 mm CT evaluation is inaccurate, since 95% limits of agreement vary from -1.74 to 5.42 mm and -1.64 to 5.42 mm in 2D images, and from -3.70 to 4.28 mm and -3.49 to 4.25 mm on 3D images, for CBCT and SCT, respectively. It can be concluded that measurement of alveolar bone height on CT images presents high reproducibility. When alveolar bone thickness is greater than 0.6 mm measurement precision is high, and when this thickness is less than 0.6 mm the technique is inaccurate

Les ``C-arm'' sont des systémes de radiologie interventionnelle fréquemment utilisés en salle d'opération ou au lit du patient. Des images 3D des structures anatomiques internes peuvent être calculées à partir de multiples radiographies acquises sur un ``C-arm mobile'' et isocentrique décrivant une trajectoire généralement circulaire autour du patient. Pour cela, la géométrie conique d'acquisition de chaque radiographie doit être précisément connue. Malheureusement, les C-arm se déforment en général au cours de la trajectoire. De plus leur motorisation engendre des oscillations non reproductibles. Ils doivent donc être calibrés au cours de l'acquisition. Ma thèse concerne la calibration intrinsèque d'un C-arm à partir de la détection de la projection du collimateur de la source dans les radiographies.Nous avons développé une méthode de détection de la projection des bords linéaires du collimateur. Elle surpasse les méthodes classiques comme le filtre de Canny sur données simulées ou réelles. La précision que nous obtenons sur l'angle et la position (phi,s) des droites est de l'ordre de: phi{RMS}=+/- 0.0045 degrees et s{RMS}=+/- 1.67 pixels. Nous avons évalué nos méthodes et les avons comparés à des méthodes classiques de calibration dans le cadre de la reconstruction 3D. / Mobile isocentric x-ray C-arm systems are an imaging tool used during a variety of interventional and image guided procedures. Three-dimensional images can be produced from multiple projection images of a patient or object as the C-arm rotates around the isocenter provided the C-arm geometry is known. Due to gravity affects and mechanical instabilities the C-arm source and detector geometry undergo significant non-ideal and possibly non reproducible deformation which requires a process of geometric calibration. This research investigates the use of the projection of the slightly closed x-ray tube collimator edges in the image field of view to provide the online intrinsic calibration of C-arm systems.A method of thick straight edge detection has been developed which outperforms the commonly used Canny filter edge detection technique in both simulation and real data investigations. This edge detection technique has exhibited excellent precision in detection of the edge angles and positions, (phi,s), in the presence of simulated C-arm deformation and image noise: phi{RMS} = +/- 0.0045 degrees and s{RMS} = +/- 1.67 pixels. Following this, the C-arm intrinsic calibration, by means of accurate edge detection, has been evaluated in the framework of 3D image reconstruction.
