Sífilis congênita: fatores de risco em gestantes admitidas nas maternidades de Maceió/AL e área Metropolitana e avaliação dos critérios diagnósticos adotados no Brasil

Entre maio/2007 e setembro/2008, realizou-se um estudo de validação fase III de Sackett, a partir dos casos diagnosticados como SC pelos critérios do MS, avaliando o valor preditivo positivo (VPP) e um estudo caso-controle, a partir de mães VDRL positivo. Incluiu gestantes admitidas em 15 maternidades de Maceió/região metropolitana/AL (09 municípios) e seus conceptos nativivos, natimortos e abortos tardios. No caso-controle, definiu-se CASO: neonato, aborto ou natimorto definido pelos critérios do MS e diagnóstico comprovado por FTA-ABs IgM ou histopatológico do cordão umbilical; CONTROLES: nascimentos subsequentes ao caso, mesmo hospital, aborto, neonato ou natimorto, VDRL negativo, inclusive na gestação. À admissão, as gestantes realizavam VDRL e teste rápido de HIV, e os neonatos de mães VDRL positivas, exames clínicos e complementares, incluindo exames PADRÃO DE REFERÊNCIA para confirmação: histopatológico do cordão umbilical e FTA-ABS-IGM. A amostra para análise, casos selecionados pelos critérios do MS, confirmados pelo padrão-referência, correspondeu a 80 casos SC confirmados, dentre 195 casos presumidos e 696 controles. Foram admitidas (partos/curetagens) 35.156 gestantes, 390 (11,1/1.000 gestantes) VDRL positivo e 195 (5,5/1.000 gestantes) preencheram pelo menos um dos critérios do MS; 83,1% gestantes positivas (162/195) enquadraram-se no critério epidemiológico (critério 1). Investigando neonatos usando evidencias sorológica + evidência clinica ou radiológica ou liquórica (critério 3) detectou-se 81/195 (41,5%) neonatos com SC; 20/195 abortos ou natimortos enquadraram-se no critério 4. O critério 1 apresentou VPP de 47,5% (IC 95% 34,8-60,6). Aumentando para 51,3% (IC 95% 41,9-60,6) ao excluir gestantes FTA-Abs negativo. Para o critério 3, o VPP foi 53,1%, (IC 95% 41,7- 64,1), aumentando para 64,2% (IC95% 51,5-75,3) para as gestantes FTA-Abs positivo. O VPP para o critério 4 foi 73,7% (IC 95% 48,6-89,9). Das 245 gestantes admitidas, com VDRL positivo, 18 (7,3%) excluídas, 02 (0,8%) recusas, 30 (12,2%) receberam alta hospitalar antes do resultado dos exames e não localizadas. Dos 195 (79,6%) casos investigados: 10 (5,1%) abortos, 10 (5,1%) natimortos, 175 (89,8%) nativivos; 81/175 (46,3%) VDRL positivo e alteração clínica e/ou liquórica e/ou radiológica de SC; 50/168 (29,6%) confirmados pelo FTA-ABs IgM, e 50/180 (27,8%) pelo histopatológico do cordão, 80/195 casos (41%) com um destes positivo. Realizou-se analise univariada por blocos de variáveis e análise multivariada. O modelo final da associação entre exposição materna a sífilis e SC no neonato contemplou: perda de neonato (OR 4,18 p=0, 000), perda de filho ao final da gestação (OR 4,15 p=0, 000), uso de drogas pelo parceiro (OR 3,49 p=0, 000), ter mais de um parceiro no último ano (1,96 p=0, 058) solteira/viúva/separada (OR 1,70 p= 0, 047). O baixo VPP desses critérios permite questionar sua utilidade no cenário atual da doença, principalmente em regiões de baixa prevalência. Gestantes com perda de neonatos ou filhos ao final da gestação e com mais de um parceiro no último ano apresentaram maior chance de ter filhos com SC. / This study aimed to evaluate the diagnostic criteria of Congenital Syphilis (CS) adopted by the Ministry of Health/Brazil (MH), by determining the percentage of infected neonates amongst those considered positive by the MH criteria, and to identify the factors associated with vertical transmission. A validation (Phase III) and a case-control design were used to investigate the former and the latter objective, respectively. In the case-control study, a CASE was defined as: neonate, abortion or stillborn who met the MH criteria for CS and had the diagnostic confirmed by FTA-ABs IgM or by the histopathological examination of the umbilical cord. CONTROLS were: neonate, abortion or stillborn of the consecutive birth (in the same hospital) following that of the case, with negative VDRL. In the validation study the Positive predictive value (PPV) of CS diagnosed with the MH criteria was estimated. Data was collected between May/2007 and September/2008. Pregnant women admitted in 15 maternity hospitals in Maceió/Metropolitan area/AL (09 cities) and their living child, abortion or stillborn were included in the study. After admission all pregnant women were tested for VDRL and HIV, and the neonates of mothers VDRL positive underwent physical examination and diagnostic testing including those considered as REFERENCE STANDARD for confirmation of CS: histopathology of the umbilical cord and FTA-ABS-IGM. The sample was composed of 80 confirmed cases of CS (cases selected by the MH criteria and confirmed by the standard reference), out of 195 presumed cases, and 696 controls. A total of 35156 pregnant women were admitted (births/curettage), 390 (11.1/1,000 pregnant women) being VDRL positive and 195 (5.5/1,000 pregnant women) fulfilling at least one of the MH criteria; 83.1% pregnant women with a positive VDRL (162/195) met the epidemiological criterion (criterion 1). Investigating neonates presenting serological evidence + clinical or radiological or cerebral spinal fluid evidence (criterion 3), 81/195 (41.5%) neonates with CS were detected; 20/195 abortion or stillborn met criteria 4. Criteria 1 presented a PPV of 47.5% (95%-CI: 34.8-60.6), increasing to 51.3% (95%-CI: 41.9- 60.6) when excluding pregnant women with negative FTA-Abs. For criterion 3, the PPV was 53.1%, (95%-CI: 41.7-64.1), increasing to 64.2% (95%_CI: 51.5-75.3) when just FTA-Abs positive pregnant women were considered. The PPV for criteria 4 was 73.7% (95%-CI: 48.6- 89.9). Of the 245 pregnant women admitted with positive VDRL, 18 (7.3%) were excluded, 02 (0.8%) refused to participate and 30 (12.2%) were discharged from the hospital before the laboratory tests results and were lost to follow-up. Of 195 (79. 6%) investigated cases there were: 10 (5.1%) abortions, 10 (5.1%) stillborns and 175 (89.8%) neonates; 81/175 (46,3%) neonates had VDRL positive and/or clinical and/or cerebral spinal fluid and/or radiological CS alteration; 50/168 (29,6%) were confirmed by FTA-ABs IgM, and 50/180 (27,8%) by the histopathological exam. A total of 80/195 (41%) conceptus had at least one of these exams positive. In the casecontrol study the following factors related to the mothers were independently associated with CS were: loss of neonate (OR 4,18 p=0, 000), loss of child at the end of pregnancy (OR 4,15 p=0, 000), use of drugs by the partner (OR 3,49 p=0, 000), having more than one partner the year before (1,96 p=0, 058) single/widow/separated (OR 1,70 p= 0, 047). The low PPV of the MH criteria allows questioning its utility at the current scenario of the disease, especially in regions with low prevalence. Pregnant women with loss of neonate or loss of child at the end of the pregnancy, as well as more than one partner during the last year showed a greater risk of having children with CS.

Segundo a International Conference on Harmonization (ICH \2013 Conferência Internacional de Harmonização), as autoridades regulatórias são instâncias dotadas de poder de regulamentação que, conforme as normas de Boas Práticas Clínicas, incluem as autoridades que revisam os dados clínicos e as autoridades que realizam a inspeção. Devido à complexidade do processo regulatório no Brasil é necessário estar atento às modificações na legislação e normas para submissão dos projetos de pesquisa às autoridades reguladoras em um curto prazo de tempo, para cumprir o planejamento dos períodos de duração dos estudos clínicos. O presente trabalho tem o objetivo de desenvolver e disponibilizar um sistema para gerenciar os processos regulatórios do Laboratório de Pesquisa Clínica em DST e Aids do Instituto de Pesquisa Clinica Evandro Chagas, da Fundação Oswaldo Cruz, atendendo à regulamentação nacional e internacional. Uma vez que processo é um conjunto de ações ordenadas e integradas para um fim produtivo específico, ao final do qual serão gerados produtos e/ou serviços e/ou informações, foi realizado o mapeamento das atividades desenvolvidas no dia a dia do setor de assuntos regulatórios do centro para obtenção das aprovações regulatórias das pesquisas em DST e Aids, bem como do processo administrativo para ingresso da equipe de trabalho nos protocolos Para a representação gráfica dos processos foi usada a notação de modelagem de processos de negócios, Business Process Model Notation (BPMN), que é um padrão definido pelo Governo Federal e já utilizada pela Fundação Oswaldo Cruz. O software BizAgi® foi usado para modelagem dos processos regulatórios. Como resultado desse trabalho foi desenvolvido um sistema para armazenar em um único local todas as informações regulatórias das pesquisas, incluindo também um módulo com informações sobre a equipe de trabalho autorizada a participar das pesquisas. Foi desenvolvido um sistema para controle do fluxo dos documentos regulatórios e de pessoal do Laboratório de Pesquisa Clínica em DST e Aids que permite ainda à equipe o acesso para consulta aos documentos regulatórios. According to the International Conference on Harmonization (ICH), Regulatory Authorities are instances endowed with regulatory power that, a ccording to the standards of Good Clinical Practices, include the authorities who review the clinical data, and the authorities conducting the inspec tion. Due to the complexity of the regulatory process in Brazil is crucial to be aware of changes in le gislation and regulations for the submission of research projects to regulatory authorities in a sh ort period of time, to comply with the clinical studies scheduled time periods. This study aims to develop and make available a database to manage the regulatory processes of Laboratório de Pesquisa Clínic a em DST/Aids at Instituto de Pesquisa Evandro Chagas (IPEC), Fundação Oswaldo Cruz (Fiocruz) , according to national and international regula tions. As process is a set of coordinated and integrated actions to a specific productive result, which will eventually yield products/services/information, a mapping of the da ily activities of the Laboratory Regulatory Affairs to obtain regulatory approvals for research on STD and Aids, as well as the administrative procedures for including the stu dy team in the protocols. For the process’ graphical representation modeling business processes’ notation wa s used, the Business Process Model Notation (BPMN), which is a st andard defined by the Federal Government, and already used by the Oswaldo Cruz Founda tion. The BizAgi ® so ftware was used for modeling of the regulatory processes. As a result of this work, we developed a system to store in one place all the regulatory information of the research, including a new module of information about the study team authorized to partic ipate in the research. We developed a system for controlling the flow of regul atory documents and study personnel at the Laboratory of Clinical Research on Aids and STDs, which also allows the study team’s access to the regulatory documents. It is hoped that ot her research laboratories at IPEC and other Fiocruz units are able to use this instrument as well.

This master’s thesis is based on the idea to extend a point-to-point 10 Gbps Ethernet cryptoto a multi-point 10 Gbps Ethernet crypto. The multi-point extension is targeted for a fictiveproduct called Krypet. Extending Krypet to multi-point can be seen as creating a distributed Ethernet switch wherethe ports are Krypet units and the information passed between the ports is encrypted. Problems that occur when this extension is designed are identified and analysed. Some problemexamples are address lookup, session changes and packet switching. Different solutions to the identified problems are presented and these are compared anddiscussed. Requirements on the whole multi-point extension are determined and an architecturebased on the solutions is designed. The architecture is implemented and the resultof the implementation is discussed. The implementation successfully extends the system tomulti-point support and satisfies most of the determined requirements.

Especificamente foram analisados os arranjos institucionais introduzidos pela SMS para aquisição de compras de medicamentos, através das modalidades de licitação, no período de 2002 a 2006, pela Secretaria Municipal de Saúde (SMS) município da Bahia, identificando as mudanças ocorridas, a partir da introdução do sistema eletrônico de compras (Pregão Eletrônico). A análise das informações empíricas demonstrou que a TCT possui variáveis importantes ao estudo do objeto de pesquisa, pois, seus pressupostos de racionalidade limitada, incerteza e oportunismo podem levar a falhas na aplicação das regras estabelecidas pela legislação, que trata do assunto, provocando práticas oportunistas e ineficiência no processo de compras de medicamentos. Desse modo, a pesquisa conclui que houve mudanças positivas tanto na relação entre os setores internos da SMS, por onde transitam os processos, como na relação entre esses setores e as empresas fornecedoras de medicamentos, embora, alguns ajustes tornam-se necessários para que se obtenham melhores resultados no processo de compras de medicamentos, em particular, criação de mecanismos de incentivos e monitoramento. / This work argues the transactions between the public sector and the private sector, under the optics of the New Institutional Economy, using the Theory of Costs of Transaction (TCT) that it deals with the costs to draw, to write, to negotiate and to guarantee that the agreements between the agents are fulfilled. Specifically the institutional arrangements introduced by the SMS for acquisition of medicine purchases had been analyzed, through the modalities of limitation, in the period of 2002 the 2006, for the City Department of Health (SMS) state of Bahia, identifying the occurred changes, from the introduction of the electronic system of purchases (Auction Electronic ). The analysis of the empirical information demonstrated that the TCT possess important variables to the study of the research object, therefore, its presuppositions of limited rationality, uncertainty and opportunism can take the imperfections in the application of the rules established for the legislation that deals with the subject, provoking practical opportunists and inefficiency in the process of medicine purchases. In this manner, the research concludes that it had positive changes in such a way in the relation enters the internal sectors of the SMS, for where the processes transit, as in the relation between these sectors and the supplying medicine companies, even so, some adjustments become necessary so that if it gets better resulted in the process of medicine purchases, in particular, creation of mechanisms of incentives and monitoriment.

OBJECTIVE: To estimate the prevalence and identify risk factors for infection by N. gonorrhoeae and co-positivity with C. trachomatis in sexually active adolescents and young adults living in a midsize city of Goias and describe the socio-demographic and behavioral profile of the population seen at the Family Health Program (PSF). METHODOLOGY: The study was designed as a prevalence study in 651 adolescents and young adults, not pregnant, 15 to 24 years were enrolled in the Family Health Program (FHP) in Inhumas, Goiás were excluded pregnant or postpartum teens in antibiotics or menstruating. The diagnosis of gonococcal infection and infection by C. trachomatis was performed using the Polymerase Chain Reaction (PCR) in endocervical specimens using the Roche Amplicor kit for N. gonorrhoeae and C. trachomatis. The socio-demographic and sexual behavior were obtained through a questionnaire as an interview. Univariate analysis was performed to identify potential risk factors associated with infection. Endocervical swab samples were collected from 322 sexually active patients to dtect the DNA of N. gonorrhoeae and C. trachomatis. RESULTS: The mean age of 428 sexually active adolescents and young adults was 18.6 (SD = 2.7), with 64.3% single. Inconsistent use of condoms was observed in 78.0% of girls and 11.9% never used it. The first sexual intercourse occurred before the age of fifteen in 23.4%. Pregnancy before age 20 occurred in 90% and 23.5% of them before age 15. The prevalence of N. gonorrhoeae was 3.4% (95% CI 1.8 to 6.2) in 322 patients from whom samples were collected endocervical swab. Co-infection with chlamydia was 27.3%. Univariate analysis showed that the risk factor associated with infection by N.gonorrhoeae was not using condoms. CONCLUSIONS: Young women and adolescents in this study exhibited various behaviors considered at risk for STDs and those who reported not using condoms had a higher risk of being infected by the gonococcus. OBJETIVO: Estimar a prevalência, identificar os fatores de risco para a infecção por N. gonorrhoeae e a co-positividade com C. trachomatis em adolescentes e jovens sexualmente ativas residentes em uma cidade de médio porte do estado de Goiás e descrever o perfil sócio demográfico e comportamental da população atendida no Programa de Saúde da Família (PSF). METODOLOGIA: A investigação foi delineada como um estudo de prevalência em 651 adolescentes e jovens, não grávidas, de 15 a 24 anos, cadastradas no Programa de Saúde da Família (PSF) em Inhumas, Goiás. Foram excluídas adolescentes grávidas ou no puerpério, em uso de antibióticos ou menstruadas. O diagnóstico da infecção gonocócica e da infecção por C. trachomatis foi realizado empregando a Reação de Cadeia de Polimerase (PCR) em amostras endocervicais utilizando o kit Amplicor Roche para N. gonorrhoeae e C. trachomatis. Os dados sócio-demográficos e de comportamento sexual foram obtidos através de questionário, na forma de entrevista. Análise univariada foi realizada para identificar potenciais fatores de risco associados à infecção. Amostras de swab endocervical foram colhidas de 322 pacientes sexualmente ativas para a detecção do DNA da N. gonorrhoeae e da C. trachomatis. RESULTADOS: A média de idade das 428 adolescentes e jovens sexualmente ativas foi de 18,6 (dp=2,7), sendo 64,3% solteiras. O uso inconsistente de preservativo foi observado em 78,0% das meninas e 11,9 % nunca o usaram. A sexarca ocorreu antes dos quinze anos em 23,4%. A gravidez ocorreu antes dos 20 anos em 90% delas e em 23,5% antes dos 15 anos. A prevalência de N. gonorrhoeae foi de 3,4% (IC 95%1,8 - 6,2) em 322 pacientes das quais foram obtidas amostras de swab endocervical. A co-infecção com clamídia foi 27,3%. A análise univariada mostrou que o fator de risco associado à infecção por N. gonorrhoeae foi o não uso de preservativos. CONCLUSÕES: As jovens e adolescentes do presente estudo apresentaram vários comportamentos considerados de risco para DST e aquelas que referiram não usar preservativos apresentaram um risco maior de estarem infectados pelo gonococo.

Although most infections are asymptomatic it can cause severe reproductive sequels to women. Few studies exist about prevalence of these infection in Brazil and fewer in adolescent population. OBJECTIVES: To determine the prevalence of Neisseria gonorrhoeae genital infection among female sexually active adolescents, in Distrito Sanitário Noroeste from Goiânia, Brazil and to identify demographic and behavioral profiles associated to gonococcal infection. METHODS: We performed a cross-sectional study among 427 sexually active female adolescents (15-19 years), random selected at Distrito Sanitário Noroeste and served by the Health Family Program. It were excluded pregnant adolescents, pos-partum, using antibiotics or on period. The gonococcal infection was determined by using polimerase chain reaction (PCR) of Amplicor Roche kit for N. gonorrhoeae and C. trachomatis applied to endocervical swab specimens. Sociodemographic and behavioral data were assessed by face-to-face questionnaire. RESULTS: The mean age of 427 random selected adolescents was 17.2 ± 1.3 years, most female were single (67.9%). The mean age of menarch was 12.4 years and at first sexual intercourse was 15 years. 35,1% had the first sexual intercourse before 15 years and 16,2% reported more than 3 partners in life. About 80% of the participants reported inconsistent condom use. Among those with gonococcal infection, 22,2% also had chlamydial infection. The prevalence of gonococcal infection was 2,1% (95% CI 1,0 4,1). CONCLUSIONS: Adolescents studied presented risk sexual behaviors to sexually transmitted diseases. These results demonstrate the need for preventive medicine programs for adolescents females with ongoing STI prevention activities. / INTRODUÇÃO: Os adolescentes constituem o grupo de maior risco para a infecção genital por Neisseria gonorrhoeae. Embora a maioria das infecções sejam assintomáticas podem causar graves seqüelas reprodutivas nas mulheres. Existem poucos estudos sobre a prevalência desta infecção no Brasil e menos ainda neste grupo populacional. OBJETIVOS: Determinar a prevalência da infecção genital por N. gonorrhoeae em adolescentes sexualmente ativas residentes no Distrito Sanitário Noroeste do município de Goiânia e identificar o perfil sócio-demográfico e comportamental das adolescentes estudadas. METODOLOGIA: Foi realizado um estudo de corte transversal em 427 adolescentes do gênero feminino com idade entre 15 e 19 anos, sexualmente ativas, selecionadas aleatoriamente no Distrito Sanitário Noroeste e atendidas pelo Programa de Saúde da Família. Foram excluídas adolescentes grávidas ou no puerpério, em uso de antibióticos ou menstruadas. O diagnóstico da infecção gonocócica foi realizado empregando a Reação em Cadeia da Polimerase (PCR) em amostras endocervicais utilizando o kit Amplicor Roche para N. gonorrhoeae e C. trachomatis. Foram também obtidas informações sóciodemográficas e de comportamento sexual através de questionário na forma de entrevista. RESULTADOS: A média de idade das 427 adolescentes selecionadas foi de 17,2 ± 1,3 anos, sendo a maioria delas solteira (67,9%). A média de idade da primeira relação sexual foi de 15 anos. 35,1% das adolescentes tiveram a primeira relação sexual antes dos 15 anos e 16,2% relataram mais que três parceiros durante a vida. Cerca de 80% relataram uso inconsistente de preservativo. Dentre aquelas com infecção gonocócica, 22,2% tinham também infecção clamidial. A prevalência encontrada para a infecção gonocócica foi de 2,1% (IC 95% 1,0 4,1). CONCLUSÕES: As adolescentes estudadas apresentaram comportamentos sexuais de risco para a aquisição de DST. Estes dados mostram a necessidade de programas de medicina preventiva direcionados à adolescentes, principalmente atividades de prevenção de DST.

Os caminhoneiros de estrada constituem um grupo de grande mobilidade geográfica em função do trabalho – o transporte de cargas, que exige vários dias até meses fora de suas casas e longe da família. A solidão a que se vêem condicionados torna alguns destes profissionais vulneráveis a manterem relacionamentos sexuais no transcurso das viagens. Assim, este estudo tem por objetivo investigar os fatores de vulnerabilidade entre os caminhoneiros de estrada para o risco de contaminação com o HIV/aids. A metodologia utilizada foi o método quali-quantitativo que teve como eixo norteador a pesquisa – ação e o método da problematização de Paulo Freire. A coleta de dados foi realizada no período de janeiro a fevereiro de 2003, através de uma entrevista semi-estruturada. Foram entrevistados neste período 50 caminhoneiros de estrada presentes nas cooperativas que serviram de campo para este estudo. A análise dos dados quantitativos foi feita com base na estatística descritiva e os dados qualitativos através do método de Bardin (1979). Os resultados encontrados apontam que os caminhoneiros têm conhecimentos sobre vários aspectos da transmissão da aids, mas muitos se envolvem em relacionamentos sexuais na estrada sem o uso do preservativo. Por outro lado, o uso de álcool e anfetaminas (para reduzir o sono) faz parte do cotidiano de alguns destes profissionais, constituindo fatores de vulnerabilidade, uma vez que sob o efeito da bebida nos pontos de paradas para descanso alguns se envolvem com prostitutas. Finalmente, concluímos que os comportamentos observados entre os caminhoneiros estão fundamentados em princípios socialmente construídos com base na cultura machista, levando muitos a se envolverem no sexo sem proteção ou a aventuras como forma de mostrar sua virilidade a qualquer custo, tornando-os, deste modo, vulneráveis a DST/aids. / The truck drivers constitute a group of great geographical mobility through the cargo transport that requires many days and until months far from their family and home too. The loneliness that they are conditioned, lead some these professionals to vulnerability and to keep sexual relations during their journeys through the highway. Thus, this study aims to investigate the vulnerability factors among truck drivers to the risk of HIV/AIDS’ contamination. The methodology utilized in this study was the quanti-qualitative method that had as the reference the research-action and the Paulo Freire’s problematization method. The data were collected in the months of January to February 2003 through the semi-structured interview. Fifty truck drivers were interviewed in the cooperatives that served as place to this study. The analysis of the quantitative data was based on descriptive statistic and the qualitative data through the Bardin’s method (1979). The results indicate that the truck drivers have knowledge about the several aspects of AIDS’ transmission, but many of them involve to sexual relations in the highway without use of condom. On the other hand, the use of alcohol and amphetamine (to reduce the sleep) is present in the daily life of some them and constitute vulnerability factors in the stop points to rest where the use of alcoholic beverages facilitates the sexual relations together whores. At last, we can conclude that the watched behaviors among the truck drivers are founded in socially built principles based on the culture of masculinity leading many of them to involve in sexual relations or just adventure without use of condom to show their virility all time, becoming them so vulnerable to STD/AIDS.

Sensibilizadas com as questões relativas à sexualidade, DST, aids e drogas, voltadas principalmente para pessoas em detenção em sistema penitenciário, procuramos desenvolver esta pesquisa-ação com 49 mulheres detentas de três cadeias públicas femininas do interior paulista, objetivando levantar com elas seus problemas frente à sexualidade, DST, aids e drogas, trabalhando a seguir, um programa educativo sobre estes temas, possibilitando-lhes, conhecimentos e habilidades nesta área, bem como, preparando-as para serem agentes multiplicadores. Coletamos os dados através de entrevistas individuais gravadas, a partir de questionário aberto e com questões norteadoras as quais nos permitiram qualitativamente verificar o significado positivo que as mulheres dão à vida, à família, sublimando a maternidade. Sentem profunda tristeza pela situação em que se encontram, sendo que a maioria está presa devido ao uso e tráfico de drogas. Praticam qualquer tipo de sexo, mais preferem o vaginal. São promíscuas e algumas bissexuais. Atribuem às DST-aids, a desinformação do povo e possuem grande interesse em saber e participar de atividades educativas no interior das cadeias sobre estas temáticas. Algumas revelam presença de DST em sua vida, referindo-se ao uso inadequado do preservativo, afirmando ser difícil negociar sexo seguro com o parceiro. Demonstram conhecimento simples às vezes ingênuo sobre sexualidade, sexo, DST-aids e drogas e não desenvolvem a consciência para a mudança de comportamento. Depreendemos então, que estas mulheres não têm preparo para lidarem com a sua sexualidade e contra às DST-aids e drogas. / Thinking on issues related to sexuality, STD, AIDS and drugs regarding people confined in prisons, the author developed a research-action with 49 women confined in three public feminine prisons at the state of São Paulo, Brazil aiming at understanding their problems regarding sexuality, STD, AIDS and drugs and afterwards at implementing an educational program on these topics as well as preparing them to disseminate this knowledge. The author collected data through individual interviews, starting from an open questionnaire with guiding questions that enabled her to quantitatively verify the positive meanings that these women give to life, family and maternity. These women showed their sorrow regarding their situation, especially because the majority of them were arrested due to drug trafficking and use. They are used to have sex of different types but prefer vaginal sex. They are promiscuous and some are bisexual. They consider the lack of information the main cause of STD-AIDS and are really interested to participate in educational activities about this theme. Some of them point out the inadequate use of condoms as the cause of STD in their lives, affirming that it is difficulty to talk about safe sex with their partners. They show simple and ingenuous knowledge on sexuality, sex, STD-AIDS and drugs and are not willing to change their behavior. In sum, the author concluded that these women are not prepared to deal with their sexuality and STD-AIDS and drugs.

Pregnancy and sexually transmitted diseases become a problem at this stage of life. Teenage pregnancy today is considered a public health problem causing social and psychological problems Objective: Evaluate health education interventions on teen pregnancy and sexually transmitted Diseases in two schools in the city of Patos ¿ PB. Methodology: intervention study with non-probability sample of convenience with 108 students in the 8th and 9th year of two schools, selected according to the inclusion criteria, being a particular and another Hall of the city of Patos (PB). 04 were held workshops in each theme and students, identified by codenames were assessed before and after the workshops. Descriptive analysis was performed of all variables of the study, the Chi-square test, McNemar and signals. Results: It is observed that the sample is homogeneous with respect to distribution by sex, grade and color. The Chi-square test, the existence of a significant association between schools and income (p < 0.05) with an income equivalent to the minimum wage. For those who study in State school. With respect to religion, in particular school prevails the Catholic religion (p < 0.05). It is observed, the McNemar test a change of knowledge about some contraceptive methods both in private school and in the non-private School (p < 0.05). Conclusion: Due to the increase of knowledge is an effectiveness of interventions implemented, understanding the need for a continuum of interventions. It is expected that these students become multipliers in health and in this way contribute to better results in these health indices. / Introdução: A sexualidade é muito importante na fase da adolescência. A gravidez e as doenças sexualmente transmissíveis tornam-se, então, um problema nesta fase da vida. A gravidez na adolescência hoje é considerada um problema de saúde pública por acarretar problemas sociais e psicológicos. Objetivo: Avaliar as Intervenções de Educação em Saúde sobre Gravidez na Adolescência e Doenças Sexualmente Transmissíveis em duas Escolas do Município de Patos¿PB. Metodologia: Estudo de intervenção com amostra não probabilística por conveniência com 108 alunos do 8º e 9º ano, de duas escolas, selecionadas de acordo com os critérios de inclusão, sendo uma particular e outra municipal da cidade de Patos (PB). Foram realizadas 04 oficinas de cada tema e os alunos, identificados por codinomes, foram avaliados antes e após a realização das oficinas. Foi realizada a análise descritiva de todas as variáveis do estudo, os testes de Quiquadrado, McNemar e dos sinais. Resultados: Observa-se que a amostra é homogênea com relação a distribuição por sexo, ano escolar e cor. Apreende-se, pelo teste de Qui-quadrado, a existência de uma associação significativa entre escolas e renda (p<0,05), predominando uma renda equivalente a um salário mínimo. Para quem estuda em escola estadual. Com relação a religião, na escola particular prevalece a religião católica (p<0,05). Observa-se, pelo teste de McNemar uma mudança de conhecimento sobre alguns métodos contraceptivos tanto na escola particular quanto na escola estadual (p<0,05).Conclusão: Devido ao aumento de conhecimento percebe-se uma efetividade das intervenções aplicadas, entendendo a necessidade de uma continuidade dessa intervenções. Espera-se que esses alunos tornem-se multiplicadores em saúde e dessa forma contribua para obter melhores resultados nesses índices de saúde.

The co-emergence of Spanish as a second language and individual differences : a dynamical systems theory perspective

Dynamical Systems Theory (DST) (De Bot, Lowie, & Vespoor 2007; Larsen-Freeman 1997, 2007; Larsen-Freeman & Cameron 2008; Dörnyei 2009; and van Lier 2000) represents a scientific paradigm shift derived from the fields of physics, engineering and theoretical mathematics that attempts to solve real-world scenarios that do not respond to scientific reductionism, otherwise known as ‘analysis’. The purpose of this dissertation is to (re)frame foreign language learning/use as a dynamical process that that involves interplay among what Dörnyei (2009) terms the language, the agent and the environment. More specifically, this dissertation presents a quasi-experimental, psycholinguistic study that looks at the interface between language (in this case the talk that resulted from NS-NNS interactions) and agent (as defined by a set of personal traits, or Individual Differences [IDs], including motivation, attitudes, personality and aptitude) in order to answer the research question: Do IDs vary in conjunction with language learning/use, and if so, how? Eight tutored Spanish learners were followed over the course of 16 weeks during which time they participated in 8 chat sessions with a native Spanish-speaker. Their ID profiles were measured immediately before and after each session and sessions with significant pre- to post-session ID shifts were analyzed to determine to what extent such shifts correlated with certain types of talk and/or think-aloud sequences. Results indicated that all participants’ pre- and post-interactional ID profiles fluctuated measurably and significantly, even within the span of a single interaction. Moreover, those sessions with significantly positive ID shifts were qualitatively different in terms of language-related episodes (LREs), conversation management/pragmatic markers, and metacognition from those with significantly negative ID shifts. Other unexpected findings revealed, for example, that LREs (especially NS-initiated LREs) negatively impacted motivations and attitudes and, therefore, the language-learning process itself. Taken together, the results of this study indicate that the agent’s IDs and their (inter)language co-emerge; that is to say, they evolve simultaneously and in response to one another. Moreover, this study suggests that DST can indeed be quasi-experimentally applied to the study of SLA, thus necessitating further development in DST-oriented methodologies and research questions. / text
