Application and Evaluation of Unified Medical Language System Resources to Facilitate Patient Information Acquisition through Enhanced Vocabulary Coverage

Two broad themes of this research are, 1) to develop a generalized framework for studying the process of patient information acquisition and 2) to develop and evaluate automated techniques for identifying domain-specific vocabulary terms contained in, or missing from, a standardized controlled medical vocabulary with emphasis on those terms necessary for representing the canine physical examination. A generalized framework for studying the process of patient information acquisition is addressed by the Patient Information Acquisition Model (PIAM). PIAM illustrates the decision-to-perception chain which links a clinician's decision to collect information, either personally or through another, with the perception of the resulting information. PIAM serves as a framework for a systematic approach to identifying causes of missing or inaccurate information. The vocabulary studies in this research were conducted using free-text with two objectives in mind, 1) develop and evaluate automated techniques for identifying canine physical examination terms contained in the Systematized Nomenclature of Medicine and Veterinary Medicine (SNOMED), version 3.3 and 2) develop and evaluate automated techniques for identifying canine physical examination terms not documented in the 1997 release of the Unified Medical Language System (UMLS). Two lexical matching techniques for identifying SNOMED concepts contained in free-text were evaluated, 1) lexical matching using SNOMED version 3.3 terms alone and 2) Metathesaurus-enhanced lexical matching. Metathesaurus-enhanced lexical matching utilized non-SNOMED terms from the source vocabularies of the Metathesaurus of the Unified Medical Language System to identify SNOMED concepts in free-text using links among synonymous terms contained in the Metathesaurus. Explicit synonym disagreement between the Metathesaurus and its source vocabularies was identified during the Metathesaurus-enhanced lexical matching studies. Explicit synonym disagreement occurs, 1) when terms within a single concept group in a source vocabulary are mapped to multiple Metathesaurus concepts, and 2) when terms from multiple concept groups in a source vocabulary are mapped to a single Metathesaurus concept. Five causes of explicit synonym disagreement between a source vocabulary and the Metathesaurus were identified in this research, 1) errors within a source vocabulary, 2) errors within the Metathesaurus, 3) errors in mapping between the Metathesaurus and a source vocabulary, 4) systematic differences in vocabulary management between the Metathesaurus and a source vocabulary, and 5) differences regarding synonymy among domain experts, based on perspective or context. Three approaches to reconciling differences among domain experts are proposed. First, document which terms are involved. Second, provide a mechanism for selecting either vocabulary-based or Metathesaurus-based synonymy. Third, assign a "basis of synonymy" attribute to each set of synonymous terms in order to identify the perspective or context of synonymy explicitly. The second objective, identifying canine physical examination terms not documented in the 1997 release of the UMLS was accomplished using lexical matching, domain-specific free-text, the Metathesaurus and the SPECIALIST Lexicon. Terms contained in the Metathesaurus and SPECIALIST Lexicon were removed from free-text and the remaining character strings were presented to domain experts along with the original sections of text for manual review. / Ph. Pratiques éducatives des parents d’adolescents présentant un TDAH : perception des parents et des adolescents en lien avec les comportements à risque

La présente étude vise à examiner d’une part le lien entre les pratiques éducatives rapportées par la mère et celles perçues par son adolescent ayant un trouble déficitaire de l’attention avec ou sans hyperactivité (TDAH) et examiner le lien avec l’adoption de comportements à risque chez l’adolescent (délinquance et association aux pairs déviants). Cinq pratiques éducatives ont été étudiées de manière distincte, soit l’engagement parental, les pratiques positives, la discipline inconstante, la faible supervision ainsi que la punition corporelle. Chacune d’entre elles a été mesurée par la mère, son adolescent et une variable de désaccord a ensuite été créée. L’échantillon est composé de 32 dyades mère-adolescent ayant participé cinq ans auparavant au programme Ces Années Incroyables. Les adolescents ont tous un diagnostic de TDAH et sont âgés entre 10 ans et 15 ans. Les résultats démontrent une absence de corrélation entre chacune des pratiques éducatives rapportées par la mère et celles perçues par l’adolescent. La mère tend à avoir une perception plus positive des pratiques éducatives que son adolescent. Les résultats démontrent aussi que la perception de l’adolescent sur les pratiques éducatives est associée à la compréhension des comportements à risque. Finalement, la perception de l’adolescent s’est avérée être un facteur aggravant de la relation entre l’ampleur du désaccord mère adolescent et l’association aux pairs déviants. La discussion explique ces résultats et aborde les importantes implications cliniques. / This study aims to examine the link between educational practices reported by the mother and those perceived by adolescents with attention deficit disorder with or without hyperactivity (ADHD) and also examine the relationship with the adoption of risk behaviors among adolescents (delinquency and association with deviant peers). Five educational practices were examined separately, parental involvement, positive practices, inconsistent discipline, low supervision and corporal punishment. Each of them has been measured by the mother, teenager and a variable of disagreement was created. The sample consisted of 32 mother-teen dyads involved five years ago in the Incredible Years program. Teenagers have all been diagnosed with ADHD and are aged between 10 and 14 years and 11 months. The results show an absence of correlation between each of the educational practices reported by mothers and those perceived by adolescents. Mothers tend to have a more positive perception of parenting practices then heir teenage. The results also show that the perception of adolescents in educational practices is associated with the adoption of risk behaviors. Finally, the perception of adolescents proved to be a precipitating factor in the relationship between the extent of disagreement between teens and mothers and association with deviant peers. The discussion explains these results and discusses important clinical implications.

O campo externo assume dois valores: $ h $ e $ -h $, onde $ h> 0 $. Os sítios associados a valores positivos e negativos do campo externo, formam uma configuração em forma de tabuleiro de xadrez (nós chamamos de {\\it cell-board configuration}), com células retangulares de tamanho $ L_1 \\times L_2 $ sítios, de tal forma que o valor total do campo externo é zero. Como principal resultado, mostramos a presença de uma transição de fase de primeira ordem. A transição de fase existe para $ h <\\frac + \\frac $, onde $ J $ é uma constante de interação. A prova é construida usando o método de {\\it reflection positivity (RP)}. Aplicamos uma desigualdade que é normalmente referida como a estimativa de {\\it chessboard}. The sites associated with positive and negative values of external field form a cell-board configuration with rectangular cells of sides $L_1\\times L_2$ sites, such that the total value of the external field is zero. As a main result, we show the presence of a first-order phase transition. The phase transition holds if $h<\\frac+ \\frac$, where $J$ is an interaction constant. We use the reflection positivity (RP) method. We apply a key inequality which is usually referred to as the chessboard estimate. Furthermore, we prove uniqueness for Gibbs measure in $h>4J$, using a uniqueness condition obtained in terms of disagreement percolation.

The study of the act of disagreeing in daily conversation has been associated to both, Conversation Analysis studies, based on the notion of preference, and to the concept of model of Politeness. There are notable points of convergence in the preference setting and politeness. More specifically, both see disagreement as socially disruptive and, thus, considered as the second dispreferred part or face-threatening-acts. Both approaches emphasize the fact that disagreements should be mitigated or postponed: between, within and across speech turns. Thus, conversational disagreements tend to be temporarily postponed and structurally complex and generally preceded by partial agreements, hesitations, requests for clarification, repetitions of the question, among others. In fact, the degree and the type of strategy used to mitigate face-threatening-acts or dispreferred acts cannot be postulated a priori, but must be supported by empirical data analysis, which are situationally and contextually sensitive of the event of speech in which the act occurs. The critical nature of the analysis carried out is supported by the Systemic Functional Linguistics, which allows the relationship between the microstructure of lexicogrammatical choices with the macrostructure of disagreement and their implications. Following the trend of this research area trend, I adopt an eclectic theoretical base, extracting insights of different approaches, but with special focus on Conversation Analysis, Systemic Functional Linguistics and Critical Linguistics. This research aims to answer the following questions: (a) How are disagreement and mitigation characterized linguistically? (b) Which roles do causality, concession and adversativity perform in this process? (c) What is the function of Appraisal and Modality in relation to these issues? / O objetivo desta pesquisa é o exame do desacordo em um diálogo informal entre um homem e uma mulher para verificar as escolhas léxico-gramaticais que marcam a ocorrência de desacordos, bem como as circunstâncias que cercam essas ocorrências. O estudo do ato de discordar na conversa diária tem sido associado de um lado ao trabalho de Análise da Conversa, com base na noção de preferência, e, de outro lado, ao conceito de face no modelo de polidez. Há notáveis pontos de convergência na definição de preferência e de polidez. Mais especificamente, ambas veem o desacordo como socialmente disruptivo, sendo assim, considerado como segunda parte despreferida ou atos-ameaçadores-de-face. Ambas as abordagens enfatizam o fato de que os desacordos devem ser mitigados ou adiados: entre, dentro e através de turnos. Assim, os desacordos conversacionais tendem a ser temporariamente postergados e estruturalmente complexos e, em geral, prefaciados por acordos parciais, hesitações, pedidos de esclarecimento, repetição da pergunta, entre outros. Seguindo tendência dessa área de pesquisa, adoto uma base teórica eclética, extraindo as intravisões de diferentes abordagens, mas com referência especial para a Análise da Conversa, a Linguística Sistêmico-Funcional e a Linguística Crítica. "Cool my doubt is erased": constructive disagreement and creating a psychologically safe space in multicultural student teamwork

This paper investigates the roles of disagreement and trust in multicultural teamwork on an English-medium master's programme at an Austrian business university. The teamwork project - assigned by the content teacher - took place mostly outside the classroom and simulated business practice both in terms of the tasks and the multicultural context. Each team comprised two Austrian students and two international students, resulting in an English as a lingua franca (ELF) setting. The teams were observed and audio-/video-recorded, with the analysis focusing on an early stage of the project where they laid the groundwork for the team mental models in terms of establishing the team goals, relationships and communicative practices. Additionally, retrospective interviews were conducted at the end of the project with each of the team members and the lecturer to gain emic insights into the project. The findings suggest a symbiotic relationship between disagreement and trust, in which high levels of trust and the construction of a psychologically safe space allow the team members to disagree with and challenge each other without damaging their relationships, leading to better decisions. In turn, these decisions can contribute to a sense of shared success that strengthens the team's joint identity.

The main purpose of this study is to conduct a contrastive analysis on a corpus of Swedish and Spanish family conversations with respect to two adjacency pairs: opinion-agreement/disagreement (OADs) and offer-acceptance/rejection (OARs). On one hand, from a structural perspective, based on the methodology of Conversation Analysis, one of the objectives is to observe how (dis)preferred turns of the OADs and OARs are managed by the interlocutors. On the other hand, from a functional perspective, based on the methodology of Sociocultural Pragmatics, the intention is to study how face is constructed and how politeness is managed by the family members when expressing OADs and OARs. The structural analysis of OADs and OARs shows that the majority of agreements and acceptances follow the rules for preferred turns proposed by orthodox conversation analysts, i.e. they appear directly after the first part of the adjacency pair (opinion or offer), and they are brief and unambiguous. However, the structural analysis also reveals that 70% (Swedish corpus) and 72% (Spanish corpus) of the disagreements as well as 64% (Swedish corpus) and 70% (Spanish corpus) of the rejections have a tendency to not follow the proposed rules for dispreferred turns, i.e. they are not delayed or accompanied by hesitations, justifications, etc. and nor are they evaluated as dispreferred by the participants. This indicates that social perspective, especially face, has to be considered when deciding what is considered (dis)preferred. The functional analysis of the OADs indicates that the majority of the disagreements in both Swedish (68%) and Spanish (79%) corpus are not mitigated, but rather are expressed in a fairly direct manner. Swedes tend to avoid disagreements, and therefore we expected to find a major difference between the two groups. One explanation could be that family members enjoy close relationships, and therefore the Swedes feel free to express their disagreements. As for the impact on the family members face, in both groups, it is both autonomy face and affiliation face that are influenced when OADs are expressed. As for agreement, for example, it is usually autonomy face that is affected. We interpret this as a way for the participants to show that both speakers and listeners have valuable opinions that deserve to be both voiced and commented on. This reveals the more discursive (rather than ritual) nature of OADs. In addition, the functional study of OARs shows that acceptances and rejections in both corpora are expressed using both ritual and attenuating politeness according to the norms required by the situation. Concerning the impact on face, autonomy face has different requirements in the two cultures: in the Swedish conversations, it is important to offer food without insisting several times, and in the Spanish corpus, it is important to offer food more than one or two times, and there is also a tendency to refuse the offer several times before accepting it. Therefore, according to one’s situational role, one has to know how to both give and receive offers, which points to the more ritual nature of OARs. Finally, we want to emphasize that by adding a social perspective to the structural one, we can interpret the meaning of the conversations in a way that provides a broader understanding of what is being said as participants express OADs and OARs.

A theology of disagreement

Even the most casual contemporary observer of Christianity must recognise that the notion of Christian community being identifiable through the mutual love of its members (John 13:35) is difficult to reconcile with the schismatic reality of current ecclesial life, dominated in the public sphere by divisive debates on matters such as gender or sexuality. Given the constant presence of disagreement throughout the church’s history, it remains an ethical subject neglected by scholars. This study examines how New Testament texts might inform Christian approaches to disagreement. It is the first systematic consideration of disagreement as a New Testament theme; it follows, and critiques, the methodological approach of Richard Hays in The Moral Vision of the New Testament. The context is public disagreement among Christians: how the church speaks in public when facing its inevitable disagreements, and what theological and ethical concerns might inform how this speech proceeds. The thesis is in three parts. Part One is an examination of the New Testament in relation to disagreement, following Hays' 'descriptive task'. In Part Two, the 'synthetic' and 'hermeneutical' tasks of Hays' methodology are critiqued and some modifications are proposed; a theology of disagreement that emerges from the New Testament is outlined. Part Three considers some ecclesiological implications of this theology of disagreement. Following Hays' 'pragmatic task', it examines how moral theological insights from the New Testament interact with the life of the contemporary church. Illustrative examples consider the church's public theological witness, its pneumatology, and its liturgy, to demonstrate the need for a Christian ethic to engage with extra-Biblical authority with greater enthusiasm than Hays. The thesis concludes by affirming the particular value of reading the New Testament in pursuit of ethical wisdom, but without excluding insights from tradition, reason and experience. The challenge for the church is identified as a move Towards Loving Disagreement; an integral part of its mission is to disagree Christianly.

The pragmatic particles 'enfin' and 'écoute' in French film and TV dialogue

This thesis investigates the use of the pragmatic particles (PPs) 'enfin' and 'écoute' in French film dialogue, and their translations in British English subtitles. Using a corpus of nine films and eight episodes drawn from two television series – all released in the UK between 2005 and 2015, and equating to approximately twenty-two hours – the study identifies tokens across a much wider range of contexts than has previously been possible using traditional corpora. The main contribution is an analysis of PP functions. The results for 'enfin' show a different functional distribution of the particle to other corpora, with corrective 'enfin' occurring significantly less frequently. The relatively large number of tokens of performative and emotional (or affective) 'enfin' allows for an elaboration of these two categories, and a tendency is observed for 'enfin' to appear as an apparent disagreement mitigator in discussions between peers. With regard to 'écoute', it is argued that écoute1 functions as a face-threat mitigator in unequal relationships and écoute2 as an FTA, although the particle is multifunctional and some tokens exhibit characteristics of both categories. Attention is given to combinations of 'enfin' and 'écoute' with other particles: while there is a clear tendency for disagreement-mitigating 'enfin' to co-occur with 'mais', and for the precision and restrictive subcategories of the corrective to co-occur with 'je veux dire', other previously documented combinations ('enfin bon' and 'ben écoute') are not frequently occurring in the present corpus. The thesis also makes a significant contribution to the field of Audiovisual Translation (AVT). The English subtitles show high rates of omission for both particles consistent with previous research, with disagreement-mitigating 'enfin' particularly vulnerable to omission. However, the analysis reveals a surprising pattern regarding 'écoute': a clear division of labour between ‘look’ (used to translate more confrontational tokens) and ‘listen’ (more conciliatory and socially distant). The study includes an experimental analysis of the subtitles relative to their character limits, demonstrating a potential new approach for researchers wishing to investigate the impact of various subtitling constraints.
