Epidemiology and impacts of a leaf spot disease in Veratrum viride (Melanthiaceae)

Fungal phytopathogens can cause disease epidemics in crops, weeds, andpopulations of native plants. To investigate the impact of a foliar phytopathogen on the native herbaceous species, Veratrum viride, a demographic and disease assessment was carried out on two high elevation grassy bald populations on Roan Mountain, Tennessee. A leaf spot disease impacted all plants in both populations, causing widespread premature senescence of leaves and stems. Disease severity increased over the course of the growing season. Based on host disease symptoms and fungal conidia morphology, Pseudocercosporella sublineolata was shown to be the causal pathogen. A study of herbarium specimens showed no evidence that the disease was epidemic in the species and no evidence of an increase in disease prevalence over time. However, the disease was more common in the mid Atlantic and southern Appalachian regions, but rare in New England.

Asthma control in Latin America: the Asthma Insights and Reality in Latin America (AIRLA) survey.

Objectives. The aims of this survey were (1) to assess the quality of asthma treatment and control in Latin America, (2) to determine how closely asthma management guidelines are being followed, and (3) to assess perception, knowledge and attitudes related to asthma in Latin America. Methods. We surveyed a household sample of 2 184 adults or parents of children with asthma in 2003 in 11 countries in Latin America. Respondents were asked about healthcare utilization, symptom severity, activity limitations and medication use. Results. Daytime asthma symptoms were reported by 56% of the respondents, and 51 % reported being awakened by their asthma at night. More than half of those surveyed had been hospitalized, attended a hospital emergency service or made unscheduled emergency visits to other healthcare facilities for asthma during the previous year. Patient perception of asthma control did not match symptom severity, even in patients with severe persistent asthma, 44.7% of whom regarded their disease as being well or completely controlled. Only 2.4% (2.3% adults and 2.6% children) met all criteria for asthma control. Although 37% reported treatment with prescription medications, only 6% were using inhaled corticosteroids. Most adults (79%) and children (68%) in this survey reported that asthma symptoms limited their activities. Absence from school and work was reported by 58% of the children and 31% of adults, respectively. Conclusions. Asthma control in Latin America falls short of goals in international guidelines, and in many aspects asthma care and control in Latin America suffer from the same shortcomings as in other areas of the world. / Revisión por pares

A soja é a cultura de grãos mais importante no Brasil. Na safra de 2010/11 ocupou uma área de 24 milhões de hectares, se destacando como principal oleaginosa produzida e consumida no mundo. Alguns fatores podem limitar a produção da soja, entre eles as doenças. A mancha- alvo (Corynespora cassiicola) é frequente nas em todas as regiões produtoras de soja no Brasil e nos últimos anos tem se atribuído prejuízos crescentes à doença. Os objetivos deste trabalho foram: determinar a faixa ideal de temperatura para o crescimento micelial de diferentes isolados de C. cassiicola; verificar a esporulação em função do meio de cultura; avaliar a germinação de conídios sob diferentes períodos de molhamento foliar; estudar o efeito da temperatura e molhamento foliar nos componentes monocíclicos e realizar estudos de quantificação de danos. Para avaliar o efeito da temperatura no crescimento micelial, diferentes isolados foram mantidos nas temperaturas de 5°C, 10°C, 15°C, 20°C, 25°C, 30°C e 35°C. A temperatura mínima e máxima estimada pela função beta generalizada para o crescimento micelial foi de 6,9°C e 33°C, respectivamente. A esporulação foi estudada nos meios de cultura BDA e Czapek. Os dados obtidos foram comparados por meio do teste Scott-Knott (P<0,05). A produção de conídios foi variável em função do isolado, tanto em meio BDA como Czapek. O ensaio de germinação foi feito em folhas de soja utilizando-se diferentes períodos de molhamento foliar (2, 6, 12, 24, 48 e 72 horas). A contagem de conídios germinados foi feita em microscópio eletrônico de varredura. A germinação de conídios na superfície das folhas foi verificada a partir do período de 6 horas. Para o estudo dos componentes monocíclicos as temperaturas testadas foram 10ºC, 15ºC, 20ºC, 25 ºC e 30ºC e os períodos de molhamento foliar foram 12, 24, 48 e 72 horas. As variáveis quantificadas foram a severidade, o número e o diâmetro das lesões. Os dados foram ajustados às funções beta generalizada e monomolecular. A temperatura mínima, máxima e ótima capazes de promover o desenvolvimento da mancha-alvo foram de 18°C, 32°C e 27°C, respectivamente. O período de molhamemto foliar de 48 horas resultou em maior desenvolvimento da doença. No ensaio de quantificação de danos, cultivares comerciais foram semeadas em duas épocas. Os componentes avaliados em campo foram: a severidade, a área foliar, o indice vegetativo da diferença normalizada (NDVI) e a produtividade (kg ha-1). Com o uso destes dados foram calculadas a área abaixo da curva de progresso da doença (AACPD), a área abaixo da curva de progresso do índice de área foliar (AULIAPC), o indice de área foliar (LAI), o indice de área foliar sadia (HLAI), a duração da área foliar sadia (HAD) e a absorção da área foliar sadia (HAA). Neste ensaio verificou-se que valores de severidade de até 37% no dossel inferior não causam danos no patossistema C.cassicola - soja. As variáveis HLAI e AULAIPC se correlacionaram com a produtividade e podem ser úteis para estudos futuros de quantificação de danos para este patossistema. / Soybean is the most important grain crop in Brazil. The minimum, maximum and optimum temperature able to promote the development of target spot in soybean leaves were 18 ° C, 32 ° C and 27 ° C, respectively. The 48- hour leaf wetness resulted in greater development of the disease. For damage quantification, commercial cultivars were sown in two sowing times. The components evaluated in the field were: severity, leaf area, normalized difference vegetation index (NDVI) and grain yield (kg ha-1). Using these data, it was calculated the area under disease progress curve (AUDPC), area under the leaf area index progress curve (AULIAPC), leaf area index (LAI), healthy leaf area index (HLAI), healthy area duration (HAD) and healthy area absorption area (HAA). The severity values up to 37% in the lower canopy do not cause crop loss in C.cassicola - soybean pathosystem .The variables HLAI and AULAIPC correlated with grain yield and may be useful for future studies of crop loss for this pathosystem.

Experiences of pain and associations between pain, disease severity and individual quality of life in people with motor neuron diseases

Many people with the incurable and often times fatal motor neuron diseases have pain, but there is lack of knowledge about people’s experiences of living with pain. Further, the correlation between pain and their quality of life is not well understood, and previous studies have not used individual quality of life, namely that people with their own words express what quality of life is.   The aim of these studies was to explore the experiences of pain and the association between pain and quality of life in people with MND. Methods: Study I was explorative about the individual experience of pain, while study II was correlational between pain, pain severity, disease severity and IQOL. Study I was qualitative, whereas study II used both qualitative and quantitative analysis. Results and conclusions: People with motor neuron diseases experienced pain to have multiple characteristics and impact. However, the results emphasise that the individual experienced some pain characteristics as difficult and that pain could worsen functions that were already affected by the disease. The experience was also that it could be challenging to manage pain. However, the symptom of pain could pass unnoticed in contacts with healthcare professionals (study I). The three most important areas for individual quality of life in both participants with and without pain were: Social relations, followed by Activities for amusement and relaxations, and Being in the outdoor environment. Individual quality of life was noticed to be good regardless of pain. Pain and pain severity were not found to be associated with satisfaction of individual quality of life in patients with motor neuron diseases, neither was disease severity. The results support previous findings, that strong associations between symptoms of MND and IQoL are not obvious. However, this does not infer that pain in people with MNDs should be neglected and undertreated. On the contrary, it seems to be important for healthcare to pay more attention to pain in people with motor neuron diseases and that pain continuously is measured, individually treated and followed. Regardless of whether persons with MND have pain or not, the results point to the importance of healthcare professionals providing support to not only the patient but also the patient’s family and friends, as well as assisting in various forms of relaxing activities and possibility of being in the outdoor environment.

Asociación entre tiempo de injuria y estatus funcional en sobrevivientes de trauma: analisis exploratorio de crash-2 / Association between functional outcomes and time since injury in trauma survivors: Exploratory Analysis of the CRASH-2 trial

Introducción: Actualmente, los traumatismos son considerados la sexta causa de mortalidad y la quinta causa de discapacidad en todos los grupos etarios. Uno de los factores más importantes en la supervivencia de un paciente traumatizado es el tiempo de injuria. Objetivo: Estimar la asociación entre el tiempo de injuria y el nivel de estatus funcional al alta en pacientes adultos con trauma enrolados en el ensayo CRASH-2 durante los años 2005-2010. Métodos: Estudio de tipo cohorte retrospectiva que corresponde a un análisis secundario de la base de datos del ensayo experimental CRASH-2. Se calculó el riesgo relativo (RR) del tiempo de injuria que se define como el intervalo de tiempo que inicia desde el momento del accidente hasta la el arribo al hospital y estatus funcional mediante un modelo lineal generalizado de la familia Poisson, link log con varianza robusta con un intervalo de confianza al 95% ajustado por edad, sexo, tipo de injuria, accidente cerebro vascular (ACV), traumatismo encéfalo craneano (TEC) y eventos vasooclusivos. Resultados: Se analizó la información de 16,950 pacientes para este estudio. El 66.97% de los pacientes tuvieron un tiempo no prolongado (≤ 3 horas) y el 76.77% presentaron un estatus funcional independiente al alta. Encontramos que los pacientes que tuvieron un tiempo de injuria prolongado tienen un 11.00% más riesgo de presentar un estatus funcional dependiente en comparación a los pacientes con tiempo de injuria no prolongado. Conclusión: Nuestro estudio apoya el rápido transporte de pacientes con trauma ante la relación entre un tiempo de injuria prolongado y el desarrollo de un estatus funcional dependiente. / Introduction: Currently, trauma is considered the sixth global cause of mortality and fifth cause of disability in all age groups. . One of the most important factors in the survival of an injured patient is the injury time. Objective: To estimate the association between injury time and functional status at discharge in adult trauma patients enrolled in the CRASH-2 trial during the years 2005-2010. Methods: Retrospective cohort study corresponding to a secondary analysis of the CRASH-2 experimental trial database. The relative risk (RR) of time of injury (defined as the time interval from the time of the injury to hospital arrival) and functional status was calculated using a generalized linear model of the Poisson family, link log with robust variance with a 95% confidence interval adjusted for age, sex, type of injury, stroke, traumatic brain injury (TBI) and vaso-oclusive events. Results: Data from 16,950 patients were analyzed for this study. The 66.97% of the patients had a non-prolonged injury time (≤ 3 hours) and 76.77% presented an independent functional status at discharge. We found that patients who had a prolonged injury time had a 11.00% higher risk of presenting a dependent functional status compared to patients with a non-prolonged injury time. Other factors associated with dependent functional status are age, type of injury, stroke, TBI, among others. Conclusions: Our study concludes that a prolonged injury time increases the risk of developing a dependent functional status and due to this result, we encourage the rapid transportation of trauma patients to reduce the risk of dependent functional status. / Tesis

Propósito: Evaluar el valor del Vesikari Scoring System (VSS) como herramienta diagnóstica en la predicción de patógenos en niños con gastroenteritis aguda. Métodos: En un estudio retrospectivo, se analizó 247 historias clínicas y registros laboratoriales del Hospital de Emergencias Pediátricas (Lima, Perú), utilizando el diagnóstico de CIE-10 A00-A009 (enfermedades intestinales infecciosas). Se dividió según los patógenos detectados: no específico, viral y bacteriana. Se detectaron las bacterianas por coprocultivo y las virales por inmunoanálisis. Los casos no específicos tenían ambos resultados negativos o no hubo pruebas laboratoriales. Se ha calculado la sensibilidad, especificidad, valores predictivos negativos (VPN) y positivos (VPP), índice de probabilidad (LR) y el área bajo la curva (ROC). Resultado: El área bajo la curva ROC del VSS no fue significativo para determinar la etiología bacteriana ni viral. En el punto de corte de 9 en VSS, se analizó VPP (18.29%) y VPN (89.47%). Los leucocitos en heces tuvieron un área bajo la curva ROC de 0.8831(IC95% 0,79-0,96 y p=0.04) para etiología bacteriana, con una sensibilidad de 80.95%, especificidad de 88.24% y LR- de 0.22. Los leucocitos en heces con un punto de corte de ≥20, tuvo una precisión aceptable para diagnosticar gastroenteritis bacteriana. La prueba de moco en heces tuvo una sensibilidad de 83.33%, especificidad de 82.35% y VPP de 95.89%. Conclusión: El VSS no es una herramienta adecuada para determinar etiología de gastroenteritis viral y bacteriana. Sin embargo, se ha determinado que los leucocitos en heces con un punto de corte de ≥20 leucocitos por campo y moco en heces permite identificar la gastroenteritis aguda bacteriana. / Purpose: To evaluate the Vesikari Scoring System (VSS) as a valuable diagnostic tool for predicting pathogens in children with acute gastroenteritis. Methods: In this retrospective study were analyzed 247 clinical and laboratory records of the Pediatric Emergency Hospital (Lima, Peru), with the diagnosis of ICD-10 A00-A009 (Intestinal Infectious diseases). The cut-off point of 9 in VSS was calculated by analyzing PPV (18,29%) and NPV (89,47%). The fecal leukocyte had a ROC of 0.8831 (IC95% 0.79-0.96 and p=0.04) for bacterial group, with a sensibility of 80,95%, specificity of 88,24% and LR- of 0,22. At a cut-off point of ≥20 fecal leukocytes was an acceptable diagnostic accuracy for bacterial gastroenteritis. The stool mucus for the bacterial group had a sensibility of 83,33%, a specificity of 82,35%, and a PPV of 95,89%. Conclusion: VSS is not an adequate diagnostic tool to differentiate viral from bacterial etiology. Nevertheless, we determine that a test of fecal leukocytes, with a cut-off point of ≥20 leukocytes per field and mucus in stools, allow us to identify the bacterial etiology of gastroenteritis. / Tesis
