Sexual behavior in older adults diagnosed with dementia| Curriculum for caregivers in dementia communities

<p> Behavioral complications, such as Inappropriate Sexual Behavior (ISB), are developed in many cases of dementia. Sex, even for those with dementia, is a primary need, but supporting this need in an appropriate manner may overwhelm caregivers. The purpose of this project was to create a curriculum for Dementia Friendly Community (DFC) residential caregivers on the topic of sexual expression among older adults with dementia. By offering training on sexuality and dementia it is expected that caregivers can increase the DFC's provision of quality of life. This project presented an assessment tool and plan on how to manage ISB, activities relevant to the discussed topics, and curriculum evaluation forms. Expert reviewers offered recommendations to further improve the curriculum's quality.</p>

Increasing the education of nursing staff is a society-wide issue, influenced by continuous development of medicine and medical technologies. The provision of nursing care is rightly demanding. The only way to keep up with new trends is lifelong nursing education. Nursing education in a particular workplace primarily aims at addressing current problems of the department and nurses? needs. The theoretical part deals with the influence of education on the quality of nursing care. We present a structure of education of paramedical professions. We define and clarify terms such as pedagogy, didactics, education and learning. We deal with the specifics of education, didactic principles and principles of adult education. In addition, we specify instructional strategies for nursing staff. For the purposes of this thesis, four objectives were set. In the first one we wanted to find out what the system and process of nursing education in the department looks like. The research showed that educational events are held in all departments. The organizing departments mostly report irregular or monthly frequency. The aim of the second objective was to obtain an opinion on the effectiveness of education in the department. We found disparity between the views of ordinary nurses and head nurses. The third objective was to answer the question of whether the organized educational events are initiated by the nurses? requests. Based on the answers of head nurses and ordinary nurses, we found out that this is not always the case. The last objective was to determine whether nurses implement the knowledge and skills gained to routine nursing care. The questionnaires showed that ordinary nurses try to incorporate the knowledge and skills to routine nursing care. For the purposes of this thesis, the combination of qualitative and quantitative research was selected. In order to carry out the qualitative part of the research, the method of questioning, using the technique of semi-structured interview with open questions was chosen. For the issue of quantitative research, the method of questioning, using the questionnaire technique with closed and semi-closed questions was used. For qualitative research, we set two research questions. In the first research question we wanted to find out what impetus the management uses when planning education in the hospital and the department. We found out that most often it is initiated by head nurses and ward nurses. In the second research question, we examined what obstacles the management sees to the effectiveness of education and implementation of the knowledge and skills gained into practice. Head nurses identify a problem in the actual participation in education in the department. For quantitative research, we set five hypotheses. Hypothesis 1 was: Nursing education in the workplace is an activity organized by the management regardless of the ordinary nurses? requests. The hypothesis was not confirmed. Hypothesis 2 was: Management, unlike ordinary nurses, sees the effectiveness of education in the workplace positively. The hypothesis was not confirmed either. The third hypothesis was: Education is seen as a necessary, routine activity, not as motivating factor for improving the quality of nursing care. The hypothesis was not confirmed. The last hypothesis was: Implementation of the knowledge gained from the educational events into nursing care is monitored by the management. The hypothesis was not confirmed, the implementation of knowledge is monitored, but not to the extent we expected. When evaluating the results we came across other very interesting facts, which we believe deserve more attention. Finally, it is surprising that the organization of educational events does not reflect nurses? shifts. One of the reasons for this might be the fact that some head nurses do not prepare the plan and time schedule for nursing education. We tried to propose the time schedule of nurses education in deparments.

We used a qualitative approach to collect empirical data through focus groups conducted with nursing students from 3rd to 5th year in the Faculty of Health Sciences, Trairi / UFRN in the year 2012. Data analysis occurred through the technique of thematic content analysis constituted of three stages: pre-analysis, exploration of the material, treatment of results, inference and interpretation of the material. Thus, the categories formulated by organizing, classifying and aggregating the the content of the speeches to groups who shared opinions approximations of thought generating a set of categories of analysis designed from the contents. The results present the experiences of nursing students and their reflections, demonstrating that the process of reflection permeates the journey of life. It is inferred that the knowledge acquired in these experiences do not always participate in this dialogue in this formative process, reducing previous experiences in the field of exemplification of everyday situations. We conclude that it is necessary to consolidate innovative pedagogical proposals that allow the continuous dialogue with reality, breaking with the decontextualized teaching process from reality insertion of the university. Análise de implicação profissional: um dispositivo disparador de processos de educação permanente em saúde / Analysis of professional implication: a device triggering process of Permanent Education in Health

Esta tese teve por objetivo geral analisar a implicação profissional com os apoiadores de humanização e os articuladores de Educação Permanente em Saúde (EPS) dos municípios do Departamento Regional de Saúde (DRS) de Araraquara/São Paulo. Trata-se de uma pesquisa-intervenção Socioclínica, de abordagem qualitativa, realizada com os apoiadores de humanização e os articuladores de EPS dos 24 municípios que compõem o DRS em questão. Foram realizadas entrevistas individuais com 07 sujeitos que desenvolviam ambas as funções em seus municípios e 12 entrevistas grupais, sendo 11 compostas pelo apoiador e articulador e 01 composta por um grupo de 06 apoiadores do município. Portanto, 35 apoiadores e articuladores participaram da pesquisa-intervenção em questão. Foram também utilizados enquanto dispositivos analíticos para a produção dos dados desta pesquisa-intervenção: dez encontros mensais com os apoiadores e articuladores; sete encontros de planejamento e análise e o diário de pesquisa. As entrevistas e os encontros com os apoiadores e articuladores foram transcritos e as apreensões feitas nos encontros de planejamento e análise foram redigidos no diário de pesquisa. Todo esse material, bem como outras anotações feitas em diário, foi analisado a partir dos princípios da Socioclínica Institucional, sendo que um deles corresponde aos momentos de restituição, possibilitando um aprofundamento coletivo das análises realizadas. Os resultados foram apresentados a partir das dimensões libidinal, ideológica e organizacional que compõem a implicação profissional. Dessa forma, identificamos e analisamos com os apoiadores e articuladores os atravessamentos que a profissão inicial (enfermeiro, dentista, psicólogo, etc.) exerce sobre o fazer apoio; a contradição em se pensar em um perfil pronto para o desenvolvimento dessa função; os sentimentos de desânimo, pessimismo e otimismo enquanto componentes da dimensão ideológica que também atravessam o fazer apoio e a articulação de EPS; o tempo, o modo de fazer a gestão e o poder político enquanto analisadores da dimensão organizacional dos apoiadores e dos articuladores; e a ausência/presença do desejo, a vontade de agradar os profissionais de saúde, desenvolvendo uma relação harmônica no trabalho enquanto pistas analisáveis da dimensão libidinal dos apoiadores e dos articuladores. Na medida que o trabalho de análise com os apoiadores e articuladores prosseguiu, mudanças nos modos de se pensar e fazer apoio às equipes de saúde e também o pensar sobre o trabalho de apoiar e articular a EPS foi se modificando, fazendo com que esses profissionais entrassem, eles mesmos, em processos de EPS, refletindo e interrogando o próprio trabalho. O pesquisador ao realizar a análise de implicação também se colocou em processo de interrogação de sua prática, experienciando também a EPS em ato. Concluímos que a análise de implicação profissional consiste em um potente dispositivo disparador de processos de EPS / This thesis had as general objective to analyze the professional implication with the humanization supporters and the articulators of Permanent Health Education (PHE) of the municipalities of the Regional Department of Health (RDH) of Araraquara/São Paulo. It is a Socioclinic intervention research, with a qualitative approach, carried out with the humanization supporters and the articulators of PHE of the 24 municipalities that compose the mentioned RDH. Individual interviews were carried out with 07 participants who performed both functions in their municipalities and 12 group interviews, of which 11 were composed of the supporter and articulator and 01 were composed of a group of 06 supporters of the municipality. Therefore, 35 supporters and articulators participated in the intervention research in question. They were also used as analytical devices for the production of the data of this intervention research: ten monthly meetings with the supporters and articulators; seven planning and analysis meetings and the research diary. The interviews and meetings with the supporters and articulators were transcribed and the apprehensions made in the planning and analysis meetings were written in the research diary. All this material, as well as other diary entries, were analyzed based on the principles of Socioclinic Institutional, one of which corresponds to the moments of restitution, allowing a collective deepening of the analyzes carried out. The results were presented from the libidinal, ideological and organizational dimensions that make up the professional implication. In this way, we identify and analyze with the supporters and articulators the crossings that the initial profession (nurse, dentist, psychologist, etc.) exercises on making support; the contradiction in thinking of a ready profile for the development of this function; the feelings of discouragement, pessimism and optimism as components of the ideological dimension that also cross the support and articulation of PHE; time, management and political power as analysts of the organizational dimension of supporters and articulators; and the absence/presence of desire, the willingness to please health professionals, developing a harmonious relationship at work as analysable clues to the libidinal dimension of supporters and articulators. As the work of analysis with the supporters and articulators continued, changes in ways of thinking and supporting health teams and also thinking about the work of supporting and articulating the PHE was changing, causing these professionals to enter, themselves, in processes of PHE, reflecting and questioning the work itself. The researcher, when performing the implication analysis, also put himself in the process of interrogating his practice, also experiencing PHE in action. Esta dissertação registra uma análise de situação da avaliação no ensino técnico em enfermagem com base nas diretrizes curriculares nacionais para o ensino técnico e na concepção de enfermagem como cuidado humano. Os atores reconhecidos e escutados pela pesquisa foram cuidadores (técnicos em enfermagem e enfermeiros), educadores (professores e supervisores de estágio), aprendizes e usuários, o que difere na totalidade dos estudos realizados até então, a maioria abordando apenas a díade professor-alunos ou a triangulação professor-aluno-alunos. A metodologia utilizada foi da abordagem qualitativa, mediante entrevista, análise dos documentos e realização de um grupo focal, a fim de evidenciar a existência de territórios avaliativos, mais que um sentido e prática únicos à avaliação, tendo em vista compreender a teia que a mesma se insere na formação profissional de técnicos em enfermagem. As entrevistas abrangeram educadores, aprendizes, cuidadores e usuários e um grupo focal que foi realizado com educadores. Trabalhar com ensino técnico em enfermagem exige um exercício de pensar e fazer saúde de forma intensa, pois este profissional é formado para permanecer muito tempo com os usuários, acolhendo, interagindo e escutando-os, além de prestar-lhes procedimentos diretos de cuidado em enfermagem e execução da prescrição médica. Avaliar alguém que está aprendendo a cuidar é tarefa complexa e que mobiliza muitos cenários (serviços, políticas e sistemas de saúde) e atores (educadores, aprendizes, cuidadores e usuários). A situação encontrada na análise foi a de que tanto a formação como a avaliação da aprendizagem das práticas laborais estão embasadas na prescrição médica, nas rotinas de enfermagem e na história natural da doença, mas não no cuidado humano. Os aprendizes não compreendem porque a teoria não é possível de ser aplicada na prática e gostariam de mais interpenetração teoria-prática, os educadores desconhecem as rotinas dos serviços e sofrem a descontinuidade dos processos formativos, os cuidadores manifestam o desejo de serem ouvidos nos processos avaliativos e os usuários, comunicação, mas não são desenvolvidas estratégias para vencer entraves a esses desejos e necessidades. The interviews had enclosed educators, apprenticees, nurses on charge and users and a focal group that was carried through with educators. To work with education technician in nursing demands an exercise to think and to make health of intense form, therefore this professional is graduated after spending a lot of time with the users, receiving, interacting and listening to them, besides giving to them to procedures right-handers of care in nursing and execution of the medical lapsing. To evaluate somebody that is learning how to be a responsible nurse is a complex task and that it mobilizes many aspects (services, politics and systems of health) and responsables (educators, apprenticees, people-in-charge and users). The situation found in the analysis was the formation and the evaluation of the learning of practical the labor ones is based in the medical lapsing, the routines of nursing and the natural history of the illness, but not in the human care. Responses of Successful Latina Students to Academic Challenges of Prerequisite Nursing Courses

Nursing schools struggle to prepare enough nurses to meet the health care needs of a diverse and aging population in the United States. Many students do not complete their degrees, contributing to the problem. The purpose of this qualitative study was to explore and describe how successful Latina students experience and respond to the academic challenges of prerequisite nursing courses. The nursing shortage; healthcare needs of Hispanic patients; student outcomes; self-efficacy, achievement goal orientation, and academic help-seeking; and Hispanic cultural values framed the study. Using semistructured private interviews, the researcher explored the lived experiences of 6 successful Latina students when they encountered challenges in prerequisite nursing courses. Seven superordinate themes emerged following interpretative phenomenological analysis of the data: (a) facing academic challenges, (b) recognizing emotional response, (c) seeking help, (d) transcending academic challenges, (e) owning knowledge, (f) persevering, and (g) living out values and beliefs. The hermeneutic approach allowed participants to describe the transition from avoiding to reaching out for help. Conclusions and recommendations include the need for faculty to recognize and respond to the importance of family, relationships, values, and beliefs to academic success in Latina students. Implications for social change include enhanced faculty ability to support the academic endeavors of this population. Improved graduation rates in Latina nursing students will help to address the nursing shortage and the need for a health care workforce that mirrors the population of the United States.

The topic of the thesis Nursing Care of Newborns in the Process of Perioperative Care deals with specific and distinct features of such care from the viewpoint of nursing. The main objective was to specify all tasks of nurses in the process of perioperative care and the procedures they use. Last but not least, we focused on how the nursing care differs with regard to the age of newborns. The selected research method was qualitative research using semi-structured interviews with nurses from perinatology centers. The research covered 10 respondents with various levels of education and lengths of practical experience. Nurses most frequently understand the term of perioperative care as the care provided before, during and after a surgery. Most of them actually perform preparation of the child before the surgery and then they provide postoperative care. Only 2 out of the 10 respondents are directly involved in the surgery procedure and thus accompany the newborns throughout the entire process of perioperative care. The care provided before the surgery most often includes identification of the child, checking of its vital functions, involvement in blood collection, including intravenous cannulation, administration of prescribed medication, preparation of the surgical site and communication with the parents. Children are most frequently accompanied by their mothers and nurses need to communicate with them. Nurses transport the child to the operating room, hand the child over and subsequently take it back after the surgery. They also record all those activities in the medical files and in some cases they check signed informed consents. Intraoperative care consists mainly of monitoring of the newborn, assisting to the physician in airway management - intubation and during the entire surgery procedure. The most common surgical procedures performed in children are hernia, bowel and heart surgeries. The nurses also generally mentioned surgeries of developmental disorders. After the surgery nurses usually move the child to the neonatology intensive care unit which is equipped with a ventilator, incubator and all types of medication. They regularly check and record child´s vital functions, monitor the surgical wound and its proximity, intake and excretion, they provide nutrition etc. Nurses also assess the pain, most frequently using the NIPS scale, and they educate the parents. Post-surgery complications occur only sporadically and they include infections, bleeding or abstinence syndrome after administration of opiates. The collected data have shown only one difference relating to the age of newborns who underwent a surgery. Specifically, certain surgeries, e.g. of necrotic enterocolitis, are performed more frequently on less mature newborns. In general, most of the surgeries are performed on prematurely born neonates and extremely immature neonates. When asked what they would like to change or improve in the perioperative care from the nursing point of view the nurses primarily mentioned more contacts between the mother and child and consistent compliance with aseptic procedures. The responding nurses were mostly content, they praised the highly specialized care and the good cooperation between the nursing team and the medical team which consists of pediatricians and specialists. Results of those teams thus contribute to continually decreasing mortality rate of high-risk and pathological newborns and to better quality of life of those children after the surgery.

Teias da avaliação no ensino-aprendizagem em saúde: uma "análise de situação" da avaliação do desempenho cognitivo e laboral no ensino técnico em enfermagem

Esta dissertação registra uma análise de situação da avaliação no ensino técnico em enfermagem com base nas diretrizes curriculares nacionais para o ensino técnico e na concepção de enfermagem como cuidado humano. Os atores reconhecidos e escutados pela pesquisa foram cuidadores (técnicos em enfermagem e enfermeiros), educadores (professores e supervisores de estágio), aprendizes e usuários, o que difere na totalidade dos estudos realizados até então, a maioria abordando apenas a díade professor-alunos ou a triangulação professor-aluno-alunos. A metodologia utilizada foi da abordagem qualitativa, mediante entrevista, análise dos documentos e realização de um grupo focal, a fim de evidenciar a existência de territórios avaliativos, mais que um sentido e prática únicos à avaliação, tendo em vista compreender a teia que a mesma se insere na formação profissional de técnicos em enfermagem. As entrevistas abrangeram educadores, aprendizes, cuidadores e usuários e um grupo focal que foi realizado com educadores. Trabalhar com ensino técnico em enfermagem exige um exercício de pensar e fazer saúde de forma intensa, pois este profissional é formado para permanecer muito tempo com os usuários, acolhendo, interagindo e escutando-os, além de prestar-lhes procedimentos diretos de cuidado em enfermagem e execução da prescrição médica. Avaliar alguém que está aprendendo a cuidar é tarefa complexa e que mobiliza muitos cenários (serviços, políticas e sistemas de saúde) e atores (educadores, aprendizes, cuidadores e usuários). A situação encontrada na análise foi a de que tanto a formação como a avaliação da aprendizagem das práticas laborais estão embasadas na prescrição médica, nas rotinas de enfermagem e na história natural da doença, mas não no cuidado humano. Os aprendizes não compreendem porque a teoria não é possível de ser aplicada na prática e gostariam de mais interpenetração teoria-prática, os educadores desconhecem as rotinas dos serviços e sofrem a descontinuidade dos processos formativos, os cuidadores manifestam o desejo de serem ouvidos nos processos avaliativos e os usuários, comunicação, mas não são desenvolvidas estratégias para vencer entraves a esses desejos e necessidades. Conclui-se que o tema deveria convocar as escolas e os serviços e saúde onde ocorrem as práticas de ensino, a fim de tecerem a integração de suas ações no processo de formação em saúde, implicando a gestão da educação na saúde no país para além da formulação de diretrizes do ensino e à autorização de cursos. / This text shows an analysis of situation’s evaluation of the nursing technical studies based on national school lines of direction for technicians’ education and concept nursery as well-taken care of human being. The collaborators recognized and listened for the research were responsibles (graduates chiefs-nurses and nurses), educators (teachers and supervisors of apprentice), apprenticees and others users, on an aspect that differs on totality of the (until then) studies carried through, at most approaching only relations between professor-pupils or the triangulation professor-pupil-pupils. The methodology was of the qualitative boarding, by means of interview, analysis of documents and accomplishment of a focal group, in order to evidence the existence of evaluations notes of territories, more than a practical direction and only to the evaluation, in view of understanding the web that the one inserts itself in the professional formation of technician in nursing. The interviews had enclosed educators, apprenticees, nurses on charge and users and a focal group that was carried through with educators. To work with education technician in nursing demands an exercise to think and to make health of intense form, therefore this professional is graduated after spending a lot of time with the users, receiving, interacting and listening to them, besides giving to them to procedures right-handers of care in nursing and execution of the medical lapsing. To evaluate somebody that is learning how to be a responsible nurse is a complex task and that it mobilizes many aspects (services, politics and systems of health) and responsables (educators, apprenticees, people-in-charge and users). The situation found in the analysis was the formation and the evaluation of the learning of practical the labor ones is based in the medical lapsing, the routines of nursing and the natural history of the illness, but not in the human care. The apprenticees don’t understand because the theory is not possible of being applied and they would like more theory-practical interpenetration, the educators are unaware of the services’ routines and suffer the discontinuity from the formative processes, the people-in-reveal the desire to be heard on evaluation processes and the users communication, but strategies are not developed to win impediments to these desires and necessities. It is concluded that the subject would have to convoke schools, services and health where the use of educational theories really occur, in order to weave the integration of its action in the process of formation in health, implying the management of the education in the country’s health process for beyond the formularization of lines of direction of education and to the authorization courses.
