Zdravotní gramotnost obyvatel domova pro seniory / Health literacy of the residents of home for eldery

The diploma thesis deals with the health literacy of the residents living in the homes for the elderly. The aim was to find out what is the seniors' level of health literacy, whether they are interested in raising it and whether they are able to participate in the care of their person and make decision about their health. The second objective of the thesis was to describe the role of the nurse in providing health literacy to the clients of the home for the elderly. In the theoretical part of the diploma thesis, we were dealing first with the concept of old age and aging for a comprehensive overview, and we summarized the concepts of health and illness in old age and issues of geriatric nursing and nursing care in homes for the elderly. We analyzed in detail individual areas of health literacy, the acquired knowledge was applied in the research section. The objectives were achieved through a qualitative research in the form of semi-structured interviews. The set for research was chosen on purpose. In the set, there were clients of retirement homes and nurses that works in these homes. The research part focused on the identification of seniors' health literacy took place in two stages, a cognitive function test was first performed with the selected seniors, and it was then followed by an in-depth interview with cognitively intact seniors. The data were processed using text coloring. In the research focused on seniors, there were created 4 categories with 9 subcategories and in the nurse's research, there were 3 categories with 7 subcategories. The research shows that the level of health literacy of the elderly population differs depending on the specific area of health literacy, but it can be summed up that the seniors generally have insufficient health literacy due to the abandonment of initiative and responsibility in the care of their person and decision-making about their health status to the medical staff. In order to increase their health literacy, education is needed especially in the effect of lifestyle on health, the preventive behaviour, the causes and the possible complications of the diseases and the side effects of the used drugs. In the second part of the research we found that the role of a nurse in the home for the elderly is not only irreplaceable in increasing the health literacy of seniors, but very time and psychologically demanding, therefore, it always does not reach the qualities that the nurses themselves would like.

A Comparison of the Needs of the Elderly and Delivery of Services as Perceived by Directors of and Participants in Senior Centers and Nutrition Sites

This study was a comparison of the perceptions of directors of and participants in senior centers with regard to needs of the elderly and delivery of services to them. The sample consisted of a random selection of 200 of the 741 centers in the state of Texas. Responses were received from 111 center directors and 609 elderly participants. An instrument that used a Likert-type scale to measure the importance of needs and frequency of delivery of services was devised to survey needs in the areas of health, nutrition, recreation, education, and transportation. No statistically significant differences were found between perceptions of directors and participants with regard to needs, delivery of services, or the differences between needs and actual delivery of services at the centers. When participants were grouped on the basis of gender, age, and ethnicity, males and minority groups rated both needs and delivery higher than did females and the majority ethnic group. The sixty to seventy age group rated education needs significantly higher than did the older age groups.

„Arzneimittelinteraktionen und potentiell inadäquate Medikation (PIM) auf einer onkologischen Station“

Drug Interactions and potentially inappropriate medications at an oncology ward Objectives: this study aimed to quantify the prevalence of clinically significant interactions and potentially inappropriate medication (PIM) use through involving a pharmacist among the cancer patients at an oncology ward and an oncology day- unit. Materials and Methods: Prospective study in patients taking more than 5 drugs who had been admitted to Harzklinikum, Wernigerode, Germany between August 2016 and February 2017. The pharmacist conducts a complete comprehensive medication review including over-the-counter drugs and herbal medications. Besides, she took into consideration the intake of grapefruit juice. This information together with the information in the patient's medical history permits identifying critical drug-drug interactions using the mediQ interaction analysis program as well as PIMs using the Beers, Forta, Priscus and STOPP lists. Results: One hundred and eighty-five cancer patients (mean age ± SD = 70 ± 11 years) were included in the study. The interaction analysis program identified 177 potentially interactions. These interactions were evaluated by the pharmacist and 34 interactions for 31 patients (17 %) were considered clinically significant or critical. After the pharmacist interventions, these interactions were resolved in 51 percent. 123 patients aged over 65 years old were enrolled in the study for PIM. By using the four lists (Beers, Forta, Priscus, STOPP) 52 PIMs at 41 elderly persons (33%) were identifies. 11 recommendations in 10 elderly patients (8 %) were made by the pharmacist and result in 55 % of the cases in a prescription change. Conclusion: the use of an interaction analysis program and the lists of inappropriate medications allowed the pharmacist to identify clinically relevant interactions and PIMs and result in prescription change in agreement with the oncologist.

Topic of the diploma thesis is "The family as a subject of social work in Czech Republic and abroad. Study of factors affecting the stability of marriage". The theoretical part deals with changing the form of contemporary families. The aim of this thesis is to take a look at the family as a subject of social work; get acquainted with the current functioning of the family; to check what the difference is between the Czech Republic and Russia, and also to find out what factors affecting the stability of marriage and why we are constantly talking about the crisis of the family. The content of the practical part was the qualitative analysis of data, which were obtained with the help of prepared questionnaire. Diploma thesis may serve as theme and inspiration for social work with young and older families, and may be the basis for social assistance and consultancy.

Eldery who seeks this CRAs by their own initiatives or are accompanied by technical team due to prosecutor complaints and subsequent request of visit from Judiciary of the São Paulo. This dissertation intends to understand the processes experienced by elderly and forms of violence little visualized. I have considered elderly do not get mobility between social classes and do not increase their power of consumption and elderly that integrate a group which grows sharply, sometimes on account of omission of the State, or due to omission of family to look after them, because of the family don´t provide them proper care, this circumstances generate numerous abuses. For development of this research, We have adopted methodological procedure of Oral History of life for men and women 60 to 80 years monitored by CRAS, specifically elderly whose income tax from zero to a minimum wage, including retirees, pensioners and people who receive social assistance BPC (LOAS). L’état de santé des personnes âgées se dégrade, les niveaux de dépendance s’accroissent, induisant de profondes conséquences sur l’organisation sanitaire en France. Les fractures du col du fémur illustrent ce défi au regard de leur fort impact sur la qualité de vie et sur la morbi-mortalité des patients. Notre étude quantitative et qualitative, (Jan. 2012 - Jan. 2013) au sein du principal service de chirurgie orthopédique d’Alsace, a permis de retracer le parcours de soin de 107 patients âgés de 75 ans et plus. Notre principale conclusion est que le patient n’a que très peu de place dans le processus décisionnel de son parcours de soin. Cela est d’autant plus dommageable que cette fracture revêt une double signification: physique (besoin d’assistance accrue) et symbolique (dépendance anticipée). Notre étude interactionniste permet de dégager des pistes d’actions opérationnelles dans les domaines sanitaires, politiques et éthiques afin de limiter les effets délétères de cette pathologie. / In a context of depressed health-conditions for elderly people, the levels of dependency are rising. It induces deep consequences for the entire health care organisation. For instance, the fractures of the femoral neck (FFN) represent a healthcare challenge regarding their strong impact on patients’ quality of life and on their morbity-mortality. We conducted a qualitative and quantitative study (Jan. 2012- Jan. 2013) within the main department of orthopedic surgery in Alsace in order to track the healthcare paths of 107 patients of 75 years-old or more. Our main conclusion is that patients are poorly involved in the decisions for their care. This is all the more harmful because the FFN is in fact dual. It is both a physical (higher assistance needed) and a symbolic (earlier dependency) breakage. Our interactionist study identifies areas for pragmatic actions in health-care, public policies and ethic that could contribute to absorb / reduce the pernicious effects of this pathology.

Efeito do treinamento resistido na press?o arterial e capacidade funcional de idosas hipertensas

Objectives: To determine the effect of two resistance training intensities on resting blood pressure and the effect of resistance training on functional capacity in elderly women with systemic arterial hypertension, analyzing these variables before and after eight weeks of intervention. Methods: Patients underwent eight weeks of resistance training, with a frequency of three times per week on alternate days, in the afternoon. The exercises performed were: leg press, bench press, knee extension, lat pull-down, knee flexion, shoulder abduction, standing cable hip abduction and biceps curl. Results: It was found that patients who underwent training with moderate resistance, showed a reduction on resting values of diastolic blood pressure (DBP) p<0.03 and of mean arterial pressure (MAP) p<0.03. Patients who underwent mild resistance training showed reduction in resting values of MAP (p<0.03) and a tendency to decrease in DBP (p<0.06). With regard to functional capacity, the results showed significant increase in the strength of arms and legs, agility and aerobic endurance (p<0.001) and maintaining flexibility (p>0.05). Conclusion: The data indicated that both mild and moderate resistance training, even when started in old age, promoted cardiovascular benefits and also improve the functional capacity of hypertensive older women. / Os resultados dos estudos sobre a intensidade ideal do treinamento resistido para redu??o dos n?veis press?ricos de repouso, assim como, desse tipo de treinamento sobre o aumento da capacidade funcional de idosas hipertensas ainda s?o obscuros, uma vez que as poucas investiga??es realizadas geralmente analisam indiv?duos jovens normotensos, sendo a literatura carente de informa??o precisa em sujeitos idosos hipertensos. Objetivos: Verificar a repercuss?o de duas intensidades de treinamento resistido sobre a press?o arterial de repouso al?m do efeito do treinamento resistido na capacidade funcional de idosas portadoras da HAS (hipertens?o arterial sist?mica) analisando estas vari?veis antes e ap?s oito semanas de interven??o. M?todos: As pacientes realizaram oito semanas de treinamento resistido, com freq??ncia de tr?s vezes por semana em dias alternados, no per?odo vespertino. Os exerc?cios realizados foram respectivamente: leg press, supino reto, extens?o de joelhos puxada frontal, flex?o de joelhos, abdu??o de membros superiores, abdu??o unilateral de quadril e rosca direta com barra. Resultados: Verificou-se que as pacientes que realizaram treinamento com resist?ncia moderada, apresentaram redu??o tanto nos valores de repouso da press?o arterial diast?lica (PAD) p<0,03 como na press?o arterial m?dia (PAM) p<0,03. As pacientes que realizaram treinamento leve apresentaram redu??o nos valores de repouso da PAM (p<0,03) e tend?ncia ? redu??o na PAD (p<0,06). Quanto ? capacidade funcional, os resultados mostraram aumento significativo da for?a de membros superiores e inferiores, agilidade e endurance aer?bica (p<0,001) e manuten??o da flexibilidade (p>0,05). Conclus?o: Os dados mostraram que tanto o treinamento resistido moderado quanto o leve, mesmo quando iniciados na terceira idade, promoveram benef?cios cardiovasculares e tamb?m na capacidade funcional de idosas hipertensas.
