Avaliação do EuroSCORE como preditor de mortalidade em cirurgia valvar no Instituto do Coração de Pernambuco

Métodos: Foram incluídos no estudo 840 pacientes operados entre 2001 e 2009. Os prontuários continham todas as informações que permitiram calcular o EuroSCORE. O desfecho de interesse foi óbito na internação. Com o objetivo de avaliar a aplicabilidade do EuroSCORE, foi usado o teste não paramétrico de Mann-Whitney. A calibração do modelo foi medida pela comparação da mortalidade observada com a esperada, usando-se o teste de bondade de ajuste de Hosmer- Lemershow. A acurácia do modelo foi avaliada pela curva ROC (receiver operating characteristic curve). Resultados: A comparação entre a mortalidade prevista e a observada, por meio do teste de Hosmer-Lemershow, evidenciou boa capacidade preditiva tanto por grupo de risco (P=0,767), como para cada valor do EuroSCORE Aditivo (P=0,455). Obteve-se uma área sob a curva de ROC de 0,731 (IC95% 0,660 - 0,793), com valor de P<0,001. A mortalidade global prevista foi praticamente idêntica à observada (7,9%). O grupo de baixo risco (EuroSCORE 0-2) era constituído por 345 pacientes, e a mortalidade foi de 3,19%. O grupo de médio risco (EuroSCORE 3-5) compreendeu 364 pacientes, com mortalidade de 7,69%, e o grupo de alto risco (EuroSCORE >6) incluiu 131 pacientes, com mortalidade de 20,6%. A análise de regressão logística permitiu identificar os seguintes fatores de risco para o óbito: idade acima de 60 anos, sexo feminino, operação prévia, endocardite ativa, cirurgia associada à aorta torácica e arteriopatia extracardíaca. Conclusões: O EuroSCORE, um método simples e objetivo, revelou-se um preditor satisfatório de mortalidade operatória e, por ele, foram identificados fatores de risco para o óbito em pacientes submetidos à cirurgia valvar no Instituto do Coração de Pernambuco

Introdução: A estratificação de risco pré-operatória é elemento essencial para a decisão cirúrgica, assim foram desenvolvidos alguns sistemas de pontuações para predizer mortalidade após cirurgia cardíaca em adultos. O EuroSCORE (ES) é um dos mais difundidos mundialmente sendo considerado um bom preditor de mortalidade em pacientes que foram submetidos à cirurgia cardíaca e foi considerado um sistema de pontuação de fácil uso e boa aplicabilidade. Racional: O ES já é usado assistencialmente em nossa instituição, entretanto, não foi realizada uma validação local em uma coorte específica de portadores de valvopatias. Sabemos das importantes diferenças epidemiológicas entre nossa população e pacientes citados na literatura mundial. Objetivos: O objetivo desse estudo é validar o ES como ferramenta preditora de mortalidade após cirurgia valvar. Métodos: Foram incluídos no trabalho 540 pacientes portadores de Valvopatia, com indicação de substituição da função valvar. O período de inclusão foi de fevereiro a dezembro de 2009. Todos os pacientes tiveram o cálculo da mortalidade estimada, baseada no EuroSCORE no pré-operatório, foram seguidos até alta hospitalar ou 30 dias após cirurgia. A capacidade discriminativa do modelo foi calculada utilizando a área sobre a curva receiver operating Characteristic (ROC) e a para o cálculo calibração utilizou-se o teste de Hosmer-Lemeshow (H-L). Resultados: A média etária da população foi de 56 ± 12 anos, 50% do sexo feminino, com etiologia predominante de Doença reumática. As variáveis: endocardite infecciosa, hipertensão pulmonar e o histórico de cirurgia prévia foram mais prevalentes em nossa coorte quando comparada com o banco de dados original do ES. A mortalidade observada global foi de 16% (6% em cirurgias eletivas e 34% em cirurgia de emergência), já a mortalidade predita foi de 6.1%, 8.7% e 4.31% para ES aditivo, ES logístico e ESII, respectivamente. Na avaliação da capacidade discriminativa a área sobre a curva ROC (ASCR) foi considerada boa com valores de 0,81 ; 0,76 ; 0,76 respectivamente para ES II; ES aditivo e logístico. A calibração do modelo foi considerada ideal com P > 0,05 para os modelos de ES. Conclusão: Os modelos do ES foram validados como ferramentas preditoras de risco de mortalidade após cirurgia cardíaca valvar / Introduction: Preoperative risk stratification is essential for surgical decision, and some scoring systems have been developed to predict mortality after cardiac surgery in adults. The European System for Cardiac Operative Risk Evaluation (EuroSCORE), developed in European states, aims to predict 30-day mortality of patients undergoing cardiac surgery. Although already used in our institution we not been performed a local validation. We know the epidemiological difference between Brazilian and European population. Therefore, the aim of this study was to evaluate the validation of the EuroSCORE models in our institution. Methods: Between February 1st 2009 and December 30th 2009, a total of 540 consecutive patients with valvular heart disease and indication for surgical treatment were evaluated before and after this treatment. Patient demographics, risk factors, surgery details, length of hospital stay and 30-day mortality were collected. The EuroSCORE algorithms were calculated according to the published guidelines (http://www.euroscore.org) on the entire patient sample prior to the time of surgery. Performance of the models was assessed by comparing the observed and predicted mortality. The area under the receiver operating characteristic curve (AUCR) evaluated the predictive performance. The calibration was analyzed by Hosmer-Lemeshow goodness-of-fit statistic. Results: The mean age was 56±12 years and 50% of patients were female. The most common etiology of valvular heart disease was Rheumatic disease. Main differences between the present cohort and the original EuroSCORE cohort were: Age, gender, previous cardiac surgery, pulmonary hypertension and active endocarditis. The mean of Aditive and logistic EuroSCORE were 6 ± 3 and 8.66 ± 10.35 respectively. For EuroSCORE II the mean logistic value was 4.2 ± 5.95. Mortality rate of 16% (6% in elective surgery and 34 % in emergency/urgency surgery), with estimated mortalities according to additive, logistic EuroSCORE and EuroSCORE II of 6.1%, 8.7% and 4.31%, respectively. The AUC for additive EuroSCORE was 0.76, for logistic EuroSCORE 0.76, was lower than that for EuroSCORE II 0.81. Conclusion: EuroSCORE models demonstrated good discriminative capacity and calibration in these valvular heart disease patients undergoing cardiac surgery

Outcome after mitral valve surgery for mitral valve regurgitation

Abstract The repair of degenerative mitral valve regurgitation has been shown to be an effective procedure with durable results. The techniques for mitral valve repair are not completely risk-free for late failure, and the identification of factors associated with this increased risk is of clinical relevance as it permits an appropriate selection of patients for whom mitral valve surgery should be offered and by which technique. The European system for cardiac operative risk evaluation score (EuroSCORE) has been used and demonstrated worldwide to be a valid tool for the prediction of immediate postoperative outcome after coronary artery bypass surgery. There are only a few studies which examine its value in heart valve surgery. Mitral valve repair has been shown to be associated with significant improvement in terms of functional capacity, but the late quality of life in these patients has not been adequately assessed, and there is no data on the quality of life of long-term survivors. The study population consisted of two groups of patients operated on at our institution. The first group included 164 patients who underwent isolated or combined mitral valve repair for mitral valve regurgitation during the period 1993 to 2000, while the second group consisted of 207 patients who underwent mitral valve repair (164 patients) or replacement (43 patients) for isolated mitral valve regurgitation during the same time-period. The first study aimed to identify preoperative variables which may have impact on the 30-day postoperative outcome. In the second study, the long-term outcome after mitral valve repair was evaluated in order to identify the risk factors associated with late failures. The third study analyzed quality of life after valve repair and compared it to that of an age- and gender-adjusted Finnish general population. In the fourth study, the validity of EuroSCORE was tested in predicting the immediate and late outcome of patients who had undergone mitral valve repair or replacement for isolated valve regurgitation. Patient age, a history of prior cardiac surgery and NYHA functional class were independent predictors of poor outcome. A residual regurgitation grade of more than one immediately after primary repair and chronic pulmonary disease were independent predictors of mitral valve reoperation. After valve repair, quality of life was similar to the age- and sex-adjusted general Finnish population. Both additive and logistic EuroSCOREs were predictors of the immediate and late outcomes of patients after surgery for mitral valve regurgitation.

El risc de la cirurgia coronària a Catalunya: mètodes i usos de la seva avaluació

L'objectiu d'aquest treball era avaluar la mortalitat hospitalària de la cirurgia coronaria en malalts de la sanitat pública operats en centres de gestió pública i privada de Catalunya, mitjançant l'ús de dues escales de risc (l'EuroSCORE i un model d'àmbit local). S'analitzà també la validesa dels mètodes d'avaluació i es comparà el resultat de la cirurgia sense circulació extracorpòria respecte de la cirurgia amb circulació extracorpòria. Es van incloure tots els malalts consecutius (1.605) sotmesos a una primera intervenció d'empelt coronari aïllat durant dos anys en cinc hospitals. Els resultats indiquen que a Catalunya: 1) La gestió privada del centre s'associa marginalment amb una millor supervivència. 2) L'efectivitat de la cirurgia coronària a millorat en els últims anys. 3) Ambdós instruments d'ajust del risc son útils per a l'avaluació d'aquests resultats. 4) La cirurgia sense circulació extracorpòria s'associa a millors resultats, sobre tot en els pacients amb risc preoperatori baix. / El objetivo de este trabajo era evaluar la mortalidad hospitalaria de la cirugía coronaria de los pacientes de la sanidad pública operados en centros de gestión pública y privada de Cataluña, mediante dos escalas de riesgo (el EuroSCORE y una de ámbito local). Se analizó también la validez de los métodos de evaluación y se comparó el resultado de la cirugía sin circulación extracorporea con el de la cirugía con circulación extracorporea.Se incluyeron los pacientes (1.605) sometidos a una primera intervención de implante aortocoronario aislado durante dos años en cinco centros.Los resultados indican que en Cataluña: La gestión privada se asocia marginalmente a mejor supervivencia. La efectividad de la cirugía coronaria ha mejorado en los últimos años. Ambos instrumentos de ajuste del riesgo resultan útiles para la evaluación de estos resultados. La cirugía sin circulación extracorporea se asocia a mejores resultados, sobre todo en pacientes de riesgo bajo. / The objective of the present study was to evaluate hospital mortality after coronary surgery in patients from the public health system operated on in public and private centers, using two risk scores (the EuroSCORE and a locally derived model). In addition, validity of such evaluating methods was assessed and the results of off-pump and on-pump coronary surgery were compared.All consecutive patients (1.605) undergoing a first isolated coronary by-pass procedure during two years were recruited in five hospitals.The results show that in Catalonia: 1) Private hospital management is associated to a maginal increase in hospital survival. 2) Effectiveness of coronary by-pass surgery has increased compared to previous evaluations. 3) Both risk scores showed complementary properties fort he evaluation of results. 4) Off-pump coronary surgery is associated to better results and the association is grater in low risk patients.

USO DE UM ESCORE DERIVADO DO HEMOGRAMA NA PREDIÇÃO DE RISCO DE PACIENTES SUBMETIDOS À CIRURGIA CARDÍACA COM CIRCULAÇÃO EXTRACORPÓREA / USEFULNESS OF COMPLETE BLOOD COUNT-DERIVED SCORE IN PATIENTS UNDERGOING CARDIAC SURGERY WITH CARDIOPULMONARY BYPASS

Some CBC parameters have been implicated in individual susceptibility to death, both in heart disease and cardiac surgery populations. The cellular elements of blood are widely affected during cardiopulmonary bypass (CPB), technique used in cardiac surgery. A Complete Blood Count called Risk Score (CBC-RS) was calculated from the average of the deviations of the various elements of the CBC and has been previously validated and published. The CBC-RS showed as excellent predictor of death from all causes in large healthy and cardiovascular risk populations. Despite the effect of CPB on the blood cells, there is no acknowledgement from the prior assessment of this score in the surgical setting. The aim of this study was to evaluate the role of CBC-RS in the surgical risk prediction (mortality and morbidity) in patients undergoing cardiac surgery with CPB. For this, it was evaluated a historical cohort of 428 patients undergoing cardiac surgery with CPB. The individual CBC-RS was calculated using the collected blood count of patients preoperatively. Logistic regression and statistical C analyzed the predictive accuracy of this score. The primary endpoint was in-hospital mortality (all-cause) and secondary outcomes included the majors and bleeding complications. In our study, CBC-RS was a predictor of hospital mortality (OR = 1.28 for each score increments, 95% CI = 1123-1458, p <0.001) and secondary outcomes (OR = 1.208, 95% CI = 1.103 to 1.323, p <0.001). The areas under the curve (AUC) was 0.697 (p <0.001) and 0.636 (p <0.001) for both the primary and secondary endpoints, respectively. In multivariate analysis, after adjustment for other risk predictors (EuroSCORE II and CPB time), the CBC-RS remained significant and was the strongest predictor of mortality. Therefore, the CBC-RS proved to be an independent predictor of mortality and surgical complications during hospitalization. It may be a useful tool in risk assessment of patients undergoing cardiac surgery. / Dentre os diversos parâmetros fornecidos pelo hemograma, alguns já foram implicados em aumento da suscetibilidade individual à morte, tanto em pacientes com patologias cardíacas quanto os submetidos à cirurgia cardíaca. Os elementos celulares do sangue são amplamente afetados durante a circulação extracorpórea (CEC), técnica usada nas cirurgias cardíacas. Um escore calculado a partir dos desvios da média dos diversos componentes do hemograma foi previamente validado, publicado e chamado de Complete Blood Count Risk Score (CBC-RS). O CBC-RS se mostrou excelente preditor de morte por todas as causas em grandes populações saudáveis ou com fatores de risco cardiovascular. Apesar do efeito da CEC sobre as células sanguíneas, não se tem conhecimento da avaliação prévia deste escore no contexto cirúrgico. O objetivo do presente trabalho foi avaliar o papel do CBC-RS na predição de risco cirúrgico (mortalidade e morbidade hospitalar) em pacientes submetidos à cirurgia cardíaca com CEC. Para isso, uma coorte histórica de 428 pacientes submetidos à cirurgia cardíaca com CEC foi avaliada. O CBC-RS individual foi calculado utilizando o hemograma coletado dos pacientes no pré-operatório. A acurácia preditora deste escore foi analisada através regressão logística e estatística C. O desfecho primário avaliado foi a mortalidade hospitalar (por todas as causas) e os desfechos secundários incluíram as complicações maiores e sangramento. Em nosso estudo, o CBC-RS foi um preditor de mortalidade hospitalar (OR = 1,28 por cada aumento de pontuação do CBC-RS, IC 95% = 1.123 - 1.458, p <0,001) e dos desfechos secundários (OR = 1,208, IC 95% = 1,103 - 1,323, p <0,001). As áreas sob a curva (AUC) foram 0,697 (p <0,001) e 0,636 (p <0,001) para os desfechos primário e secundário, respectivamente. Na análise multivariada, após ajuste para preditores de risco pré-operatório (EuroSCORE II) e transoperatório (tempo de CEC) conhecidos, o CBC-RS permaneceu significativo e foi o preditor de mortalidade mais forte. Sendo assim, o CBC-RS se mostrou um preditor independente da mortalidade e complicações cirúrgicas no período hospitalar, podendo representar uma ferramenta útil na avaliação de risco de pacientes submetidos à cirurgia cardíaca.

以疾病為導向之醫療風險管理-以心臟冠狀動脈繞道手術為例 / Disease-oriented control of medical risks- analyzed with coronary artery bypass grafting surgery

背景與目的: 每一項疾病都有其潛在的風險,但要有效的降低死亡率及併發症發生率,必須找出關鍵性指標加以改善或預防。我們以心臟冠狀動脈繞道手術之患者為例,希望藉由統計分析的方式,找出造成死亡以及術後併發症最相關的因素,目的不只在預測,而在於防範。 研究對象與方法: 在我們的實驗設計上,風險因子分布在手術前、手術中、以及手術後三個階段,對象是某醫學中心接受心臟冠狀動脈繞道手術的220例患者。分析採用迴歸統計建立模型,其中羅吉斯迴歸中的依變數為死亡率與罹病率,線性迴歸的依變數為加護病房留置天數以及總住院日數。ROC curve亦將被建立,以判斷模型是否能區別病患是否罹病或死亡。所得資料亦計算EuroScore及其ROC曲線面積,並與歷史資料做比較。 結果: 所建立的死亡估計模型的有兩個,預測值都在97%以上,ROC曲線面積亦都超過0.96;併發症估計模型由六個變數所構成,預測率及ROC曲線面積分別為94%和0.984。加護病房留置天數及住院天數估計模型分別由八個及十三個因子來解釋,調整後的R square分別為0.527及0.6。EuroScore對死亡與併發症的預測率,分別為93.7%和82%,ROC曲線面積分別是0.864和0.797,均高於歷史文獻記錄,未來應該廣泛應用。 結論與建議: 經由適當的風險分級和危險因子分析,我們可以找出風險高低的標準和依據,了解影響死亡率與罹病率的關鍵因子是什麼,儘可能的做事前的防範與處置,希望能夠改善結果並提高手術的存活率。 EuroScore是個值得採用的預測工具,可以廣泛應用在死亡率與併發症發生率的估計,但是必須搭配風險因子的改善,才能發揮實際的功效。我們認為,體外循環時間與再次手術是最具有空間來降低死亡率與罹病率的兩個要素,有效率的控制時間、改善造成再手術的前因後果,除了死亡率與併發症發生率的下降外,還可以及早脫離對加護病房照顧的需求並減少留置的天數。
