O significado social da dor pélvica crônica em mulheres com endometriose: abordagem qualitativa por grupos focais / The social significance of chronic pelvic pain in women with endometriosis: qualitative approach by focal groups

Objetivo: As relações sociais são importantes em diversas condições crônicas, com impacto nos resultados terapêuticos. Neste estudo nós relatamos as percepções de mulheres com endometriose e dor pélvica crônica sobre as suas interações sociais. Métodos: Foi realizado um estudo qualitativo sobre 29 mulheres com endometriose e dor pélvica crônica. O estudo foi realizado em um hospital universitário, localizado no sudeste do Brasil, entre fevereiro de 2013 e janeiro de 2014. As mulheres foram divididas em seis grupos focais. As transcrições das entrevistas foram analisadas, segundo a abordagem da teoria fundamentada, e enviadas as categorias emergentes, que foram codificadas utilizando a plataforma WebQDA. Resultados: O isolamento social foi o principal tema emergente. O isolamento social foi percebido como associado à falta de compreensão sobre sintomas de endometriose, à resignação diante dos episódios recorrentes de dor. Evitar a intimidade parceiro, isolamento da família e isolamento dos amigos foram às subcategorias identificadas. Conclusão: Nosso estudo fornece evidências que mulheres com endometriose desenvolvem isolamento social progressivo após o início da dor pélvica crônica. Este achado é importante para a abordagem multidisciplinar da doença / Objective: Social ties have been associated with outcomes of several chronic conditions. In this study we report perceptions of women with endometriosis and chronic pelvic pain about their social interactions. Methods: We conducted a qualitative study on 29 women with chronic pelvic pain and endometriosis. The study was conducted in a university hospital, localized in the southwest of Brazil, between February 2013 and January 2014. Women were enrolled in six focus groups interviews. Transcripts were analysed according to the grounded theory approach and the emerging categories were coded using the WebQDA platform. Results: Social isolation was the main emerging theme. Social isolation was perceived as associated with lack of understanding about endometriosis symptoms, and with resignation in face of recurrent pain episodes. Avoiding partner intimacy, isolation from family and isolation from friends were components of social isolation. Conclusion: Our study provides evidence that women with endometriosis develop progressive social isolation after the onset of chronic pelvic pain. This finding is important for the multidisciplinary management of the disease

Essa dissertação demonstra como a metodologia de pesquisa denominada grupo de foco oferece resultados consistentes para suplementar a avaliação quantitativa realizada pelo Sistema de Bibliotecas da Universidade de São Paulo (SIBi/USP). Tem como base uma pesquisa minuciosa realizada sobre as mais recentes aplicações da metodologia do grupo de foco em diversas áreas do conhecimento, especialmente no campo da Biblioteconomia. Para construir um referencial diversificado neste campo, foram selecionados e descritos experimentos realizados em bibliotecas universitárias nacionais e estrangeiras. É descrita ainda a experiência conduzida por esta pesquisadora junto a algumas bibliotecas integrantes do Sistema de Bibliotecas da Universidade de São Paulo (SIBI/USP). Procura dessa forma mostrar que o grupo de foco pode auxiliar as bibliotecas a incrementarem seus serviços e a melhorar uma competência que é fundamental para os bibliotecários: a arte de ouvir seus usuários. / This dissertation demonstrates how a methodology called focus groups provides solid results to supplement a quantitative evaluation performed by the Sistema de Bibliotecas da Universidade de São Paulo (SIBi/USP). The basis for this dissertation is a detailed research about the most recent uses of the focus groups methodology in different areas of knowledge, especially in the Library Science field. Experiments performed in national and international universities libraries were selected and reported to build a diversified reference in this subject. It is also described one experiment headed by this researcher in some Libraries that constitutes the SIBi/USP. In this way the focus group methodology may help the libraries to improve their services and develop a essential skill for librarians: the art of listen to their users.

Lesbian, gay, bisexual, transgender & questioning young people on the Internet : insights from European focus groups

Introduction: This thesis investigates the experiences of young people who identify as lesbian, gay, bisexual, transgender and who are questioning their sexuality (LGBTQ) on the Internet. Specifically, the project explores how LGBTQ young people use the Internet, how they communicate online, the impact that the Internet has on their life and how they stay safe online. Despite the Internet being an ever-growing aspect of people’s lives and the potential opportunities that it presents for marginalised groups such as LGBTQ young people, there have been a relatively small number of qualitative studies in the area. Methodology: As there has been limited research regarding LGBTQ young people’s use of the Internet, a systematic review of qualitative studies exploring the experiences and views of cyberbullying by children and adolescents in the general population was conducted using Framework Synthesis. Subsequently, an empirical study was completed which involved conducting a secondary analysis, using Framework Analysis methodology, of data collected from focus groups with LGBTQ young people regarding their Internet use. In total, five focus groups were held with forty-one LGBTQ young people recruited across four European countries. Results: A total of eighteen studies were included in the qualitative synthesis exploring children and adolescents’ cyberbullying experiences. Although there was some variation in the quality of the studies, there was clear support for four main themes: Online vs. Traditional Bullying Environment, Risk Factors, Victim’s Experience and Preventative Measures. These themes highlighted both the potential causative factors of cyberbullying as well as how the victim experiences different aspects of the incident such as their initial understanding of the event to the long-term impact of cyberbullying. A number of preventative measures were also suggested, including the need for adults to increase their understanding of technology and cyberbullying in order to enable them to be a viable source of help. Within the empirical study, four main themes emerged from the data: Digital World as Part of Daily Life, In Control of Their Online World, Seeking Connection and Navigating Risk. The latter three main themes also consisted of a number of subthemes. The results indicate that participants have embraced the Internet into their everyday lives and that the LGBTQ population reaps specific benefits as the Internet allows them to overcome or compensate for barriers faced within their offline lives. Participants also reported the need to navigate many risks online, however interestingly they appeared confident in doing so and discussed the variety of ways in which they achieve this. Discussion: The results of the qualitative synthesis provided tentative support for two different theoretical models of cyberbullying, indicating that both an individual process model and an ecological system model are mutually useful ways of understanding this phenomenon. Clinical implications spanned both individual and systemic measures that could be taken to reduce the likelihood of cyberbullying occurring. However, it is also clear that further research, in particular qualitative research, is required to continue to develop our understanding of this topic as a whole. The findings from the empirical project suggest that LGBTQ young people must balance the opportunities provided by the Internet whilst also managing the risks that it poses. The importance of retaining the empowerment for young people on the Internet was clear, especially for young LGBTQ people who may use the Internet as an alternative way of meeting their needs and engaging in developmental tasks such as sexual identity development. However, there is also a need to ensure that these young people are safe online and therefore interventions such as parental education and the development of age appropriate resources are required to promote both empowerment and safety for this population.

Young people's perceptions of novel psychoactive substances

Novel Psychoactive Substances (NPS) also known as "legal highs" replicate the effects of illegal substances such as ecstasy and cocaine. The most common NPS reported are stimulants and synthetic cannabinoids. Despite the Psychoactive Ban (2016) recent reports identified the UK as having the largest market of NPS use anywhere in Europe. These substances have a short history of consumption and consequently little is known about their effects and health implications. Despite this, the sale of NPS is easily achieved through the internet and street dealers. Increased reports of negative health consequences from NPS consumption and research findings highlighting the willingness of young people to consume drugs without knowing what they are, mean it is vital that we investigate young people's understandings and perceptions of them. At present there are very few in-depth qualitative studies on NPS. A series of 7 focus groups with a range of young people (40=N: aged 16- 24 years) across the Merseyside area were carried out. Research sites included colleges, youth groups, supported living accommodations, and youth drug and alcohol services. Focus group interviews explored participants' perceptions of NPS and were followed up with a few semi structured interviews with selected participants. The direction of the study focused on mainly on synthetic cannabinoids which may reflect the age of the study's population. Using thematic analysis informed by a social constructionist perspective, three main themes were identified around stigma and identity, attractive features of NPS and risk. Findings showed that young people's perceptions of these substances were dependent on their level of experience with illegal substances and NPS. A novel finding was that synthetic cannabinoid use is employed in the normalisation of cannabis use. Local, national and policy recommendations are made on how youth and health services in both educational and specialised services could work more closely and effectively with young people NPS. They also identify a need among young people for specific guidelines on how to use the Internet and Print media in relation to previous knowledge and experience.

Construção e validação de um instrumento voltado à satisfação do doador de sangue / Construction and validation of an instrument designed to blood donor\'s satisfaction

Em 2015, no Brasil, a taxa de doação de sangue para o país foi estimada pelo Ministério da Saúde em 18,2 doações por 1.000 habitantes, enquanto em 2014, esta taxa era 18,49. A manutenção dos estoques de sangue no país é um desafio constante nos serviços de hemoterapia e estratégias de captação e fidelização dos doadores são essenciais. Assim, é necessário conhecer as expectativas do doador em relação ao atendimento durante a doação e identificar o que o deixa satisfeito em relação ao serviço prestado. Os questionários usuais de satisfação, encontrados na literatura, voltados a clientes ou pacientes não são diretamente aplicáveis a doadores, uma vez que os doadores não são propriamente conceituados como indivíduos que comparecem à instituição somente para receber um produto ou atendimento, mas participam de um processo em que contribuem oferecendo gratuitamente um bem de natureza material, que é seu próprio sangue. Diante da ausência de instrumentos validados em língua portuguesa, o objetivo do presente estudo é construir um instrumento capaz de avaliar a satisfação do doador de sangue, bem como estudar a validade de construto e consistência interna. O estudo será conduzido em quatro etapas: (1) desenvolvimento do instrumento com base em grupos focais e no instrumento introduzido por Borges et al. (2005), (2) validação de conteúdo, considerando a avaliação do instrumento por um grupo de especialistas, (3) pré-teste do instrumento e (4) aplicação do instrumento para validação em uma amostra de 1.019 doadores de sangue. O instrumento proposto possui 25 itens que caracterizam atributos da satisfação do doador de sangue, divididos em três domínios: acesso/conveniência, aspectos técnicos e aspectos interpessoais. O instrumento apresentou satisfatória consistência interna, quanto ao conjunto de itens. Propõe-se o gráfico desempenho-importância (GDI) como uma ferramenta simples de interpretação dos dados obtidos pelo instrumento, na rotina de monitoramento da qualidade do serviço prestado por bancos de sangue. Considerando os dados obtidos para a amostra de 1.019 doadores, o GDI permitiu identificar os itens que, se melhorados, a satisfação geral do doador tenderá a aumentar, bem como os itens que precisam ser mantidos para a garantia da satisfação do doador. The maintenance of blood stocks in the country is a constant in hemotherapy services and the strategies of donor recruitment and loyalty are essential. Thus, it is necessary to know donor\'s expectations regarding the assistance during the donation and to identify what makes them feel satisfied with the service delivered. The usual satisfaction questionnaires, found in the literature, designed to customers or patients are not directly applicable to donors, once donors are not properly conceived as subjects who attend to the institution only to receive a product or care, actually they take part in a process to which they contribute by offering goods of material nature for free, in this case their own blood. Upon the absence of validated instruments in Portuguese, the aim of this study is to construct an instrument capable of assessing blood donor\'s satisfaction, as well as to study its validity and internal consistence. The study will be conducted in four steps: (1) development of the instrument based on focus groups and on the instrument introduced by Borges et al. (2005), (2) validation of the content, considering the instrument evaluation by a group of experts, (3) instrument pre-test, and (4) application of the instrument to validate a sample of 1,019 blood donors. The instrument proposed has 25 items characterizing the attributes of blood donor\'s satisfaction, divided in three domains: access/convenience, technical aspects, and interpersonal aspects. The instrument showed satisfactory internal consistence in relation to the set of items. Continuity of care and its effect on patients' motivation to initiate and maintain cardiac rehabilitation

Background: Despite national guidance and empirical support for its clinical and cost-effectiveness, cardiac rehabilitation (CR) is underused. Only 44% of patients go to CR, with angioplasty (Percutaneous Coronary Intervention; PCI) patients being least likely to attend (31% of 87,000). Aim: To investigate the relationship between ‘continuity of care’ and patients’ motivation towards CR, through the lens of self-determination theory, and develop a model to inform service design to increase CR uptake and adherence. Methods: A mixed methods approach was used. In Study 1, a theoretical model of continuity of care and motivational antecedents was tested with a cross-sectional sample of 107 PCI patients. To further explore interactions between continuity of care and motivation towards cardiac rehabilitation, a Critical Interpretive Synthesis of the extant literature was used in Study 2, and focus groups were carried out with patients and service providers in Study 3. Results: In Study 1, continuity of care positively predicted patients’ autonomous motivation towards CR, and this was partially mediated by autonomy support. Autonomy support was associated with CR attendance. In Studies 2 and 3, aspects of continuity of care with positive effects on attendance were identified. These included timely, appropriate information provision, relationships bridging CR phases and settings, and continuing management strategies incorporating trusting, warm staff-patient relationships, and positive encouragement and feedback about progress. The SDT constructs of autonomy support, need satisfaction, internalisation and quality of motivation helped to explain positive and negative influences of continuity on attendance. Conclusion: Continuity of care has a positive effect on patients’ motivation towards CR. The most enduring motivation comes from delivering continuity of care in an autonomy-supportive and competence-supportive way. Longitudinal research is needed to compare how need-supportive and need-thwarting aspects of continuity of care affect CR attendance and adherence, and whether these relationships are influenced by need satisfaction and need frustration.

Experiências relacionadas à simulação de tratamentos endodônticos em pré-clínica por alunos de graduação em Odontologia

O objetivo do presente estudo foi avaliar as experiências dos alunos de graduação em Odontologia ao realizar tratamentos endodônticos em atividades de Pré-Clínica e avaliar níveis de confiança e o perfil de ansiedade neste grupo de indivíduos. Alunos de Odontologia que frequentavam a disciplina de Pré-Clínica Odontológica foram convidados a participar do estudo. Após o aceite, os participantes preencheram um formulário elaborado para o estudo, contendo perguntas sobre as diferentes etapas do tratamento endodôntico a serem realizadas em dentes monorradiculares, pré-molares e molares. O grau de autoconfiança para cada etapa foi atribuído, por meio de escala Likert de cinco pontos, variando de “muito pouco confiante” a “muito confiante”. Utilizou-se o inventário IDATE Traço e Estado para determinar o perfil de ansiedade dos participantes. Após a aplicação de todos os instrumentos quantitativos, foram sorteados 18 participantes para integrar de uma análise qualitativa por meio de um grupo focal. Foi realizada análise estatística e inferencial. Os dados obtidos no grupo focal foram compilados, e gerou-se uma nuvem de palavras, para representar graficamente os achados. Um total de 39 participantes integrou o estudo. A maioria deles sentia-se “confiante” para realizar as diferentes etapas do tratamento endodôntico simulado, independentemente do grupo dental. Contudo, observou-se que os participantes sentiam-se menos confiantes para realizar a “radiografia de prova do cone de guta-percha” em molares do que em monorradiculares e pré-molares. Não houve diferença estatisticamente significante nos níveis de confiança para realizar as diferentes etapas de um tratamento endodôntico em um mesmo grupo dental. Participantes que tiveram escores de IDATE Traço/Estado acima do terceiro quartil foram categorizados como “altamente ansiosos”, representando 36,11% e 23,07% para IDATE-Traço e IDATE-Estado, respectivamente. Correlação fraca, porém estatisticamente significativa foi encontrada entre níveis de autoconfiança para realizar etapas específicas do tratamento endodôntico, nos diferentes grupos dentais. O grupo focal trouxe informações quanto às dificuldades encontradas pelos alunos na disciplina de Pré-Clínica, na sua percepção quanto ao desenvolvimento das atividades, e também dos sentimentos e expectativas a serem vivenciadas na transição entre os tratamentos laboratoriais e clínicos. Pode-se concluir que foram observados níveis importantes de ansiedade entre os participantes do estudo, conforme determinado pelo IDATE Traço/Estado. Contudo, parece haver fraca correlação entre a confiança em realizar determinada etapa do tratamento endodôntico em atividade Pré-Clínica e o nível de ansiedade. Torna-se importante discutir e considerar as impressões dos alunos ao se adotar abordagens de ensino e aprendizagem em Endodontia Pré-clínica. / The aim of the present study was to evaluate the self-confidence and experiences of dental students and assess their anxiety levels when performing pre-clinical root canal treatment.Undergraduate students answered the STAI-Trait/State and a questionnaire to assess their confidence level to perform each step of pre-clinical root canal treatment in anterior, bicuspid and molars. Further eighteen randomly selected students participated in a focus group aimed at obtaining qualitative data on perception of conducting endodontic treatment and the learning process necessary to perform it in a pre-clinical environment. Statistical analysis was carried out. A total of 39 students joined the study. The majority of the participants reported feeling "confident" to perform the different steps of the simulated root canal treatment, despite the dental group. However, students felt less confident to perform the master cone X-ray for molars than for anterior and bicuspid teeth. There was no statistical difference in the self-reported confidence to perform different steps of the root canal treatment in the same group of teeth. Students who scored above the 3rd quartile of STAI-Trait/State were classified as "highly anxious", and comprised 36.11% and 23.07% and for STAI-Trait and STAI-State, respectively. A weak correlation was observed for the levels of self-reported confidence for specific steps of the root canal treatment and STAI-Trait/State. The focus group brought information on the difficulties that the students faced in the pre-clinical activities, on the perception for the activities development in the classes, and also on the feelings expected in the transition from pre-clinical to clinical activities. It can be concluded that there were considerable levels of anxiety among the students, as determined by the STAI-Trait/State. However, it might weakly influence their confidence to perform simulated root canal treatments, in pre-clinical endodontics. The current findings suggest it might be important to rearrange consider the students perceptions for determining the learning approaches to be adopted in the Pre-clinical activities in endodontics.

An investigation into service quality in the Supreme Court civil registry in Jamaica

Notwithstanding the strategic importance of service quality to public sector reforms, the Government of Jamaica has given it no consideration in its justice reform project. Neither had anyone previously applied the service quality methodology to service delivery in the Jamaican Supreme Court. This thesis is a study of service quality in the Supreme Court civil registry in Jamaica and of theservices provided to legal practitioners using the registry by clerks, administrators and registrars engaged in that registry. The aim was to identify, investigate andunderstand the perceptions of service quality in the registry. The study was conducted in three stages: A pilot study, a main study andfocus groups' assessments of the findings. The main study used the SERVQUAL instrument, adapted to suit the circumstances of a court's civil registry. The sample frame for the main study was legal practitioners working in the Kingston Metropolitan Area who were users of the civil registry. Survey methodology was used to collect data. Three focus groups of practitioners later evaluated theparticipants' understanding of the items on the questionnaires. The groups supported the findings of the main study and confirmed that the service quality dimensions used in the study represented an accurate interpretation of the servicequality experience of users of the registry. The study also supports the dominant opinion in the literature thatSERVQUAL and SERVPERF are both good measures of overall service quality. The findings were that practitioners in Jamaica experienced poor overall service quality in several service quality dimensions, including the areas that they considered to be most important, and that this dissatisfaction did not vary by gender or how far their place of employment was from the Supreme Court. This work confirms that the Government of Jamaica's programme of civil justice reform is notmeeting the needs of important stakeholders, such as legal practitioners, and that the emphasis of the reforms may be misplaced.

As narrativas de adolescentes sobre gênero em um ambiente virtual / Adolescents’ gender narratives in a virtual environment

A adolescência é um período de redefinição das identidades, incluindo a dimensão de gênero. Nesta pesquisa, procurou-se conhecer as concepções de adolescentes sobre o que é ser homem e ser mulher, a partir de suas próprias narrativas. Participaram 41 adolescentes, com idades entre os 14 e os 15 anos, estudantes de uma escola pública e de uma escola privada de Porto Alegre. Com eles, foram realizados 6 grupos focais online, através do programa de bate-papo MSN: 2 grupos compostos por adolescentes homens, 2 por adolescentes mulheres, e 2 mistos. As narrativas interacionais produzidas pelos grupos foram analisadas, em primeiro lugar, segundo a Análise de Conteúdo temática, que revelou uma predominância dos papéis tradicionais de gênero. Após, um trecho de cada grupo foi microanalisado, evidenciando que a interação fomentada pelos grupos focais promoveu também questionamentos e redefinições dos sentidos comumente atribuídos ao homem e à mulher. / Adolescence implicates redefinition of identities, including the gender dimension. The goal of this research was to identify teenagers’ conceptions about what does it mean to be a man and a woman, through their own narratives. Participated in the study 41 teenagers, between 14 and 15 years of age, who were students of a public school and of a private school, both located in the city of Porto Alegre. Through the chat software MSN, 6 online focus groups were carried out: 2 groups of young men, 2 of young women, and 2 hibrid groups. The narratives in interaction produced by the groups were analyzed, firstly, by using a thematic content analysis, that revealed a predominance of traditional gender roles. Then, a section of the narratives produced by each group was micro-analyzed, showing that the interaction fostered by the focus-groups promoted questioning and the redefinition of the meanings commonly assigned to men and women.
