No. of bitstreams: 1 Poleti_FulviaSteluti_M.pdf: 1819218 bytes, checksum: 77c46d89bafe4a055ffe4648c8992b76 (MD5) Previous issue date: 2005 / Resumo: Este estudo teve como objetivo avaliar os aspectos fonoaudiológicos em indivíduos com a Síndrome de Gilles de la Tourette (SGLT), caracterizando o desempenho fonoaudiológico da linguagem oral e escrita de 10 sujeitos que apresentam como diagnóstico a Síndrome de Gilles de la Tourette (GI ¿ grupo experimental) e comparando estes achados fonoaudiológicos com 10 sujeitos que apresentam desempenho esperado para idade e escolaridade (GII ¿ grupo controle). O processo diagnóstico fonoaudiológico verificou a comunicação oral e escrita dos sujeitos, tanto quanto as habilidades lingüísticas (fonologia, sintaxe, semântica e pragmática) como a fala (articulação das palavras, fluência verbal, voz e motricidade oral), por meio de avaliação fonoaudiológica clínica, provas e testes complementares. Os resultados foram: as crianças com a SGLT apresentaram comprometimentos quanto aos aspectos fonoaudiológicos. Na avaliação clínica, 50% das crianças com a SGLT apresentaram alterações quanto às habilidades fonológicas e 40% nas habilidades sintáticas, sendo que as alterações fonológicas incluíram dessonorização de obstruintes e substituição de líquida lateral. As alterações quanto à consciência fonológica ocorreram nas provas de rima, aliteração, manipulação e transposição silábica e fonêmica. Os achados foram bastante significativos quando comparados entre os grupos nas provas que envolvem leitura e escrita. Na leitura, verificou-se que o GI é diferente do GII, e na escrita esta diferença é altamente significativa. Foram constatadas alterações em raciocínio lógico - matemático em todas as crianças do GI e, quando comparadas com o GII, os resultados foram significativos. Na avaliação da fluência, os resultados foram significativos quando comparados o GI e o GII, sendo que o total de disfluências gagas apresentadas pelo GI foi altamente significativo em relação ao GII, assim como o fluxo de palavras e sílabas por minuto e a porcentagem de disfluências gagas. A avaliação da motricidade oral e a avaliação de voz mostraram total semelhança entre os grupos / Abstract: This study had as a goal, to evaluate the phonoaudiological features in individuals with the Sindrome de Gilles de la Tourette (SGLT), delineating the phonoaudiological performance of the oral and written language of 10 people who show as a diagnosis the Sindrome de Gilles de la Tourette (GI ¿ experimental group) and comparing these phonoaudiological findings with 10 people who show an expected performance according to their ages and school education (GII ¿ control group). The phonoaudiological diagnosis process checked the oral and written communication of people, as much as the linguistic abilities (phonoaudiology, syntax, semantics and pragmatism) like speech (word pronunciation, oral fluency, voice and oral motricity), by a clinical phonoaudiological evaluation and additional exams and tests. The results were: children with SGLT showed implications referring to phonoaudiological features. In the clinical evaluation 50% of children with SGLT show alterations as to phonoaudiological abilities and 40% in syntactic(al) abilities, taking into account that phonoaudiological alterations include dissonority of obstructors and replacing of side liquid. Alterations referring to the phonoaudilogical perception occur in the tests of rhyme, alliteration, handling and syllabic and phonemic conversion. Findings were very significant when compared between the groups in exams that involve (include) reading and writing. On reading was noted that GI is different from GII and on writing this difference is very significant. Alterations were evidenced in logical math thought in all children of GI and when compared to GlI, results were significants. In fluency evaluation results were significant when compared GI and GII, considering that the total of stuttering disfluencies showed by GI were highly significant in relation to GII, like the flood of words and syllabes per minute and the percentage of stuttering disfluencies. Nous proposons un cadre statistique pour analyser les anomalies morphologiques et organisationnelles altérant l'anatomie des circuits neuronaux chez les patients atteints du syndrome de Gilles de la Tourette. Les composants de chaque circuit (matière grise et blanche) sont représentés comme des maillages 3D et intégrés dans un seul complexe. Cela permet d'étudier leur organisation et surtout la connectivité structurelle. La méthode proposée est basée sur une approche statistique appelée construction d'atlas qui résulte en un template, capturant les invariants de la population, et en déformations template-vers-sujets, décrivant la variabilité morphologique. Premièrement, nous intégrons la construction d'atlas dans un cadre bayésien qui permet d'estimer automatiquement des paramètres autrement fixés. Deuxièmement, nous réduisons les ressources de calcul nécessaires au traitement de faisceaux de fibres en définissant un schéma d'approximation basée sur un nouveau modèle appelé courants pondérés. Troisièmement, nous décrivons un nouveau modèle de déformation pour la construction d'atlas qui permet de capturer les variations morphologiques et organisationnelles. On montre l'efficacité de la méthode par comparaison de deux groupes de 44 patients et 22 témoins. Les résultats soulignent des anomalies sur la forme des structures de la matière grise et sur la connectivité structurelle. / We propose a statistical framework to analyse morphological and organisational anomalies altering the anatomy of neural circuits in patients with Gilles de la Tourette syndrome. The components of each circuit, from both white and grey matter, are represented as 3D meshes and integrated in a single shape complex. This allows to study their organisation and in particular the structural connectivity. The proposed methodology is based on a statistical approach called atlas construction which results in a template complex, capturing the invariants of the population and in template-to-subject deformations, describing the morphological variability. First, we embed the atlas construction in a Bayesian framework which allows to automatically estimate important parameters otherwise fixed by the user. Second, we reduce the important computational resources required to process fiber bundles by defining an approximation scheme based on a new computational model called weighted currents. Third, we describe a new deformation scheme for the atlas construction which permits to model variations in structural connectivity and at the same time to capture global anatomical changes. We show the effectiveness of the proposed method by comparing two groups of 44 patients and 22 controls respectively. Results highlight anomalies about both the shape of grey matter structures and structural connectivity.

<p>Syftet med denna uppsats har varit att i en litteraturstudie ge en allmän beskrivning av Tourettes syndrom och övriga ticstillstånd, samt att granska förhållandena för barn med TS och tics i skolan och vilka förslag som i litteraturen ges på anpassad skolgång för dessa barn. Studien är en kunskapsöversikt med följande frågeställningar: Hur beskrivs Tourettes syndrom och tics i vald litteratur? Vilka problem kan ses hos barn med Tourettes syndrom och tics i skolan enligt vald litteratur? samt Vilka förslag ges i vald litteratur på anpassad skolgång eller pedagogiska insatser för barn med Tourettes syndrom eller ticsbesvär? Dessa frågeställningar har utgjort kategorier i struktureringen av materialet via meningskategorisering. Tourettes syndrom är en neuropsykiatrisk störning vars bakomliggande orsaker troligtvis är ärftlighet. Syndromet förekommer hos ungefär en halv procent av alla skolbarn som då besväras av en kombination av motoriska och vokala tics. Dessa kan ta sig uttryck som ryckningar, grimaser, blinkningar, harklingar, grymtande och skall men kan också innebära exempelvis upprepande av egna eller andras ord och fraser, tvångsmässigt uttalande av obsceniteter och socialt oacceptabla gester eller handlingar. I skolan kan barn med TS ha kognitiva problem, svårigheter att förstå och ta emot muntlig information, kontrollera impulser och ouppmärksamhet på lektioner. Retningar, mobbing, utstötning och dålig självkänsla förekommer oftare hos dessa barn jämfört med andra. I analysen används socialkonstruktionistiska och diskursanalytiska tankegångar.</p>
