Profiel van 'n traumaontlonter

Increasing violence in the work environment created a need for an effective intervention to debrief traumatised workers. The aim of this study is to compile a profile of an effective trauma debriefer that can be used as a criterion for the selection of trauma debriefers. The task description of the trauma debriefer is based on the literature studied. The profile of the trauma debriefer was compiled using the characteristics of a self-actualised person, as described by the Humanism, as well as the skills based on Ivey's Microskills Hierarchy Model. Structured, non-directive interviews were conducted with five experts on the training of trauma debriefers. A qualitative analysis was made of the interviews, based on the personality characteristics and skills identified in the literature. A profile of an effective trauma debriefer that can be used as criterion during the selection of trauma debriefers, was compiled. Conclusions and recommendations were made and limitations were identified. / Toenemende geweld in die werksomgewing het gelei tot 'n behoefte aan 'n koste- effektiewe intervensie vir die ontlonting van getraumatiseerde vverkers. Die doei van die navorsing is die samestelling van 'n profiel van 'n effektiewe traumaontlonter, ten einde die profiel as kriterium vir die keuring van traumaontlonters te gebruik. Die taakbeskrywing van 'n traumaontlonter, gebaseer op relevante literatuur, is opgestel. Die profiel van 'n effektiewe traumaontlonter is aan die hand van die persoonlikheidskenmerke van die selfaktualiserende persoon, soos vervat in die Humanism©, en die vaardighede, gebaseer op Ivey se Mikrovaardighede Hierargiese Model, saamgestel. Gerigte, nie-direktiewe onderhoude is met vyf kundiges op die gebied van opleiding van traumaontlonters, gevoer. Mental health education for mothers of children with depression

(Psychiatric Nursing) / In this research an attempt was made to identify manifestations of depression in black children and give mental health education to their mothers in an attempt to alleviate the state of depression in the children. A questionnaire was structured as a tool for collection data. The instrument was based on a standardized depression scale which was used cross-culturally by the World Health Organization during 1983. The non-probability sampling procedure was carried out utilising a purposive sampling method in compiling an experimental and a control group of mothers of depressed children in a quasi-experimental research design. A mental health education programme on depession was developed which was given to mothers in the experimental group. The individual method of teaching was utilised based on adult education principles. At the end of the mental health education session, each mother got a brochure in the language preferred by the mother. Mothers from the control group did not have any discussions on depression. The study was done over the period of three months from 6 August 1988 to 26 October 1988. The analysis of gathered data was computerised using the Mann-Whitney U-test of SPSSX for the independent group and the Wilcoxon matched-pairs signed-ranks test for the dependent groups. The non-parametric statistics of the Z-va1ue and two-tailed probability was utilised. The findings on the comparison. between the experimental group pretest and the control group pretest revealed that there were not any statistical difference in eight of the ten variables of depression i.e. helplessness, anxiety, sleep disturbance, school problems, interpersonal relations, self concept, guilt feelings and the disturbance of perception. The other two variables, suicide and physical functioning, were not included because they showed significant statistical difference prior to experimentation. There was no significant statistical difference between the eight variables of the state of depression in the findings of the control group pretest with the control group post test. That means that if mothers of depressed children do not receive mental health education, the children's state of depression could remain the same. In conclusion, mental health education to mothers of depressed children seems to have a positive effect on a child's emotional stability. It can contribute substantially to diminishing the occurance of childhood depression.

The purpose of this study was to present and understand the experiences of HIV-positive lay counsellors working in Voluntary Counselling and Testing (VCT) settings. Specifically exploring and understanding the utilisation of personal experiences within counselling encounters, the practice of peer counselling within VCT, and the challenges experienced by HIV-positive lay counsellors within VCT settings. This study, employing a qualitative interpretative phenomenological methodology required a small sample of practicing HIV-positive lay counsellors, who were selected and interviewed on their experiences utilising semi-structured interviewing. Data was analysed for meaning units, which were interpreted inductively and hermeneutically, and categorised into super-ordinate themes. Three superordinate themes within the participants’ experiences of providing VCT services were determined, namely: ‘diagnosis and disclosure experiences’, ‘peer counselling’, and ‘challenges’. This research found that the experiences of providing peer counselling depended upon identification with their client’s negative appraisal of their diagnosis experiences. Whether through empathic connections generated through the shared experience of discovering a seropositive status, or through countertransferential reactions induced through their client’s yearning for care and support. This required the counsellor to selfdisclose within counselling encounters in order to provide personal experiences of living with HIV/AIDS. Successful implementation of peer counselling provided recently diagnosed individuals with knowledge surrounding HIV/AIDS, coping skills to manage the daily physiological and psychological challenges, facilitation and adherence to treatment, social assistance, ongoing relationships, inspiring hope, and the creation of positive appraisals. However the informal utilisation of task-shifting within lay healthcare cadres, and the lack of governmental recognition for the emotional labour provided within VCT indicated that HIVpositive lay counsellors require ongoing training, support and remuneration to limit potential occupational stress, resignation, and burnout.

L’exercici físic provoca efectes beneficiosos per a la salut en persones sanes però també amb certes patologies cròniques. L’objectiu d’aquesta tesi fou valorar els efectes i l’aplicabilitat d’un programa de prescripció i planificació d’exercici físic per a la salut a dos centres d’atenció primària per a pacients amb patologia crònica reclutats durant visites rutinàries al seu equip d’atenció primària. El disseny de l’estudi fou d’una intervenció pragmàtica de 18 mesos avaluada seguint les dimensions RE-AIM. Els protocols establerts per a la intervenció, emmarcada en el Programa CAMINEM, han demostrat ser aplicables en els dos centres i els efectes de l’exercici físic pels participants adherits mostraren una tendència favorable, tot i que els resultats no són estadísticament significatius. Aquesta intervenció en prescripció d’exercici físic per a la salut possiblement hagi estat la primera basada en els principis de l’entrenament esportiu i en promoció de salut pública on un professional no sanitari treballà conjuntament amb professionals sanitaris d’atenció primària. / El ejercicio físico provoca efectos beneficiosos para la salud en personas sanas pero también con ciertas patologías crónicas. El estudio fue diseñado como una intervención pragmática de 18 meses evaluada siguiendo las dimensiones RE-AIM. Los protocolos establecidos para la intervención, basada en el Programa CAMINEM, han demostrado ser aplicables en los dos centros y los efectos del ejercicio físico para los participantes adheridos reflejaron una tendencia favorable, a pesar de que los resultados no sean estadísticamente significativos. Esta intervención en prescripción de ejercicio físico para la salud posiblemente haya sido la primera basada en los principios de entrenamiento deportivo y de promoción de salud pública en el cual un profesional no sanitario trabajó conjuntamente con profesionales sanitarios de atención primaria. / Regular exercise has many health benefits for apparently healthy people and also for people suffering determinant chronic diseases. The main aim of this thesis was to assess the effects and feasibility of a health-enhancing exercise prescription and exercise planning in two primary health-care settings for regular patients suffering from chronic conditions attending their health practitioner. The study was developed as a pragmatic-driven trial for 18 months. It was evaluated following the dimensions of the RE-AIM framework. Health-enhancing physical activity (HEPA) intervention procedures, designed within the framework of the Let’s Walk Programme, have been found to be feasible in both centres. Positive trends may be found in health indicators for adhered participants even though results were not statistically significant. This intervention has probably been the first based on exercise training principles and public health promotion in which exercise specialists were working alongside primary care practitioners.

Factors preventing the uptake of HIV counseling and testing (HCT) programmes : the case of the Industrial Development Corporation in Johannesburg, South Africa

Thesis (MPhil)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: HIV counseling and testing (HCT) is a cornerstone of both HIV prevention and care in South Africa, but only one in five South Africans who are aware of HCT services have been tested for HIV and hence the uptake is reportedly low. This study investigated factors that prevent the uptake of HCT programme in the workplace. Specific factors that were looked at include: fear of learning about one‟s HIV status, HIV-stigma and discrimination and knowledge of and attitudes towards HCT. The study employed descriptive survey design; anonymous questionnaires were randomly distributed irrespective of age, gender, marital status, race, educational level, work position and experience. Closed and open-ended easy- to- answer questions which were written in English were asked; and they required fewer instructions. Ethical issues were considered and university guidelines followed. The results of this study showed that a great proportion of participants (93.8%) tested for HIV as compared to (6.2%) who had never tested. Of these, 59.4 % tested because they wanted to know their HIV status and, 43.8% of participants preferred using the workplace HCT programme for convenience; while 50% used private facilities for privacy and confidentiality. The study further established that fear of knowing one‟s HIV status, workplace discrimination, knowledge of and attitudes towards HCT were not associated with workplace HCT programme uptake. The results did however show that both participants who had tested and those who had not tested (68.8%) demonstrated significantly greater AIDS-related stigma. Supportive and collaborative efforts are necessary to create and promote an enabling and conducive environment in order to dispel workplace HIV-related stigma. In addition, it is imperative to develop and implement workplace stigma mitigation strategy putting in place interventions that aim to reduce all forms of stigma, as well as emphasizing on the benefits of testing. / AFRIKAANSE OPSOMMING: MIV/Vigs-voorligting en toetsing is die hoeksteen vir beide die voorkoming en versorging van MIV-pasiënte in Suid-Afrika. Ongelukkig is net ongeveer een uit elke vyf mense bewus van hulle MIV-status. Die doel van hierdie studie is 'n poging om vas te stel waarom so min mense gebruik maak van gratis toetsingsdienste in die werksplek. 'n Beskrywende studie-ontwerp is in hierdie navorsing gebruik met anonieme vraelyste wat ewekansig versprei is onder 'n steekproef waarin geen onderskeid ten opsigte van ras, geslag. opvoedkundige vlak, posisie in die werk en ervaring gemaak is nie. Geslote en oop-einde vrae is gebruik en Engels is as kommunikasiemedium gebruik omdat al die proefpersone dit verstaan het. Resultate van die studie het aangetoon dat beduidend meer mense hulle wel laat toets het teenoor die wat hulle nie laat toets het nie. Die studie het verder bevind dat faktore soos die vrees om status te weet; diskriminasie in die werksplek, kennis van en houding teenoor MIV/Vigs nie geassosieer kan word met die lae opname van vrywillige toetsing in die werksplek nie. Die studie het wel bevind dat diegene wat hulle . laat toets het, beduidend meer stigma in die werksplek ondervind. Ondersteunende dienste is uiters nodig ten einde stigma suksesvol in die werksplek te bestuur. Daar word voorgestel dat daar 'n volledige opleidingsprogram ,in die werksplek van die organisasie wat in die studie gebruik is, ontwikkel moet word ten einde die invloed van stigma tot 'n minimum te beperk.
