Validação do analisador hematológico automático BC-2800 VET® para realização de hemogramas de muares /

Resumo:Neste estudo foram comparados os valores do hemograma completo realizado em 30 muares com idade entre 4 e 8 anos, obtidos pelo uso de um analisador hematológico automático e pela contagem manual de células sanguíneas. A análise dos resultados revelou que a concordância entre os dois métodos foram considerados bons uma vez que o viés (média das diferenças) foi pequeno, sendo que os intervalos de confiança de 95% para o viés apresentaram-se estreitos e apenas um "outlier" foi observado, para eritrócitos, monócitos e granulócitos (nenhum valor caiu fora dos limites de concordância [viés ± 2 desvios padrão]). Não houve diferença significativa para os valores de eritrócitos, hemoglobina, leucócitos e granulócitos. Os valores médios de monócitos obtidos pelo contador automático de células foram significativamente maiores do que aqueles obtidos pelo método manual. A análise dos resultados revelou ainda um viés positivo para hematócrito e nenhum viés foi detectado para eritrócitos, hemoglobina, leucócitos, granulócitos e linfócitos. Foi observada boa concordância entre os resultados obtidos pelos dois métodos para eritrócitos, hemoglobina, leucócitos, granulócitos, linfócitos e monócitos nessa faixa etária. Podendo o Analisador hematológico automático BC- 2800Vet®, ser utilizado para realização de hemogramas em muares / Abstract:In this study the values of complete blood counts performed in 30 mules aged 4 and 8, obtained by using an automated hematology analyzer and the manual blood counts were compared. The results showed that the correlation between the two methods were considered good since the bias (mean difference) was small, and the confidence intervals of 95 % for the bias showed up narrow and only one "outlier" was observed for erythrocytes, monocytes and granulocytes (no value fell outside the limits of agreement [bias ± 2 standard deviations]). There was no significant difference in the values of erythrocytes, hemoglobin, leukocytes and granulocytes. The average values obtained from monocytes by an automatic cell counter were significantly higher than those obtained by the manual method. The results also showed a positive bias for hematocrit and no bias was detected in erythrocytes, hemoglobin, leukocytes, granulocytes and lymphocytes. Good agreement between the results obtained by the two methods for erythrocytes, hemoglobin, leukocytes, granulocytes, lymphocytes and monocytes was observed in this age group. Evaluation of the ADVIA®60 on highvalue platelets

<p>Platelets are the smallest cells in the blood. They are formed in the bone-marrow and are important for the blood coagulation. Platelet tranfusions are given to patients propyhlactically before an operation but also in therapeutical purpose in connection with bleeding. It’s importent that the quality controls of the platelet concentrates are reliable.</p><p>ADVIA®60 (Bayer HealthCare) is a fully automated cell counter which uses impedance principle to count platelets in blood samples. The purpose of the study was to evaluate this new instrument for use in the blood bank of Akademiska Sjukhuset in Uppsala. The instrument was bought to be used for quality control of platelet concentrates. 30 samples from platelet concentrates, from both apheresis and from buffy coats, were analyzed 10 times each on ADVIA®60 and the coefficient of variation (CV) was calculated for each sample. CV variated from 0,8 % to 2,9 % which is good considering that according to Bayer HealthCare the CV should be < 5 % for thrombocytes on ADVIA®60. The instrument was newly calibrated when the study was performed. Platelet count can also be performed by immunological or optical principles.</p>

[pt] Os analisadores hematológicos são sistemas de medição que permitem identificar elementos sanguíneos, incluindo a contagem diferencial dos cinco tipos de leucócitos encontrados no sangue periférico: neutrófilos, linfócitos, monócitos, eosinófilos e basófilos. Devido à inexistência de materiais de referência certificados ou métodos de referência para a análise diferencial de leucócitos, tem sido habitual avaliar o desempenho de novos analisadores comparando-os com sistemas tradicionalmente disponíveis e bem consolidados. Em 2015, foi lançada uma nova tecnologia portátil e de baixo custo, o modelo DxH500 (Beckman Coulter), baseada na perda axial de luz usando LED azul como emissor, combinada à medição de impedância. O presente trabalho compara o desempenho do DxH500 com um modelo bastante utilizado desde seu lançamento em 2001 para exames laboratoriais em larga escala, o modelo LH750 (Beckman Coulter), baseado na combinação dos princípios de impedância, condutividade e dispersão de luz (VCS). No estudo foram examinadas 310 amostras pareadas. A análise comparativa entre os resultados fornecidos por cada dispositivo indicou uma boa correlação para a caracterização de neutrófilos, linfócitos, monócitos e eosinófilos. Apesar da reduzida correlação observada para a contagem de basófilos, esse resultado não apresenta relevância clínica já que os valores obtidos para as amostras avaliadas foram muito reduzidos, inferiores aos limites de referência. Embora a análise comparativa realizada tenha apontado para um desempenho equivalente utilizando-se tecnologias com princípios diversos, a adequada avaliação dos dispositivos de análise hematológica requer o desenvolvimento de materiais de referência certificados, uma demanda fundamental para a garantia da confiabilidade na quantificação diferencial de leucócitos. / [en] Hematology analyzers are measurement systems that allow the identification of blood elements, including differential counting of the five types of leukocytes found in peripheral blood: neutrophils, lymphocytes, monocytes, eosinophils, and basophils. Because of the lack of certified reference materials or reference methods for differential leukocyte analysis, it has been customary to evaluate the performance of new analyzers by comparing them with traditionally available and well-established systems. In 2015, a new low-cost, portable technology was introduced, the Beckman Coulter DxH500 model, based on the axial loss of light using blue LED as a combined transmitter for impedance measurement. The present work compares the performance of the DxH500 with a widely used model since its launch in 2001 for large-scale laboratory tests, the Beckman Coulter model LH750, based on the combination of impedance, conductivity and scatter (VCS). In the study, we examined 310 paired samples. The comparative analysis between differential leukocyte evaluation results provided by each device indicated a good correlation for the characterization of neutrophils, lymphocytes, monocytes, and eosinophils. Despite the low correlation observed for basophil counts, this result is not clinically relevant since the values obtained for the evaluated samples were very low, lower than the reference limits. Although the comparative analysis carried out pointed to an equivalent performance using technologies with different principles, the adequate evaluation of hematological analysis devices requires the development of certified reference materials, fundamental demand for the reliability of the differential quantification of leukocytes.
