Aspectos facilitadores e dificultadores do trabalho do enfermeiro em cargos gerenciais em hospital / Facilitating and hindering aspects of the work of nurses in hospital management positions

A nova configuração do papel dos hospitais, na perspectiva da construção de redes de atenção à saúde, tem implicações para o trabalho do enfermeiro com cargo gerencial no tocante às suas ações/decisões administrativas, assistenciais e de ensino. Nesse sentido, este estudo teve o objetivo de identificar os aspectos facilitadores e dificultadores do trabalho do enfermeiro em cargos gerenciais, em um hospital público, de urgência, do interior paulista. Para tanto, foi realizado um estudo descritivo de abordagem quantitativa de dados qualitativos, utilizando a Técnica do Incidente Crítico, para o levantamento das percepções e atitudes em relação ao objeto de investigação. O estudo foi desenvolvido em uma instituição hospitalar de ensino, pública, de nível terciário, situada no nordeste do Estado de São Paulo, Brasil. Participaram 15 enfermeiros em cargos gerenciais que atuavam no referido cargo há pelo menos um ano, sendo excluídos aqueles que se encontravam ausentes do local de trabalho à época da coleta dos dados, em decorrência de afastamentos legais ou por não ter sido possível realizar a entrevista após cinco agendamentos cancelados. A coleta dos dados ocorreu por meio de entrevista semiestruturada individual. Foram relatados incidentes críticos, que se constituíram em 42 situações, das quais 33,3% foram positivas e 66,7% negativas; repercutindo em 57 comportamentos, sendo 84,2% positivos e 15,8% negativos e 74 consequências, sendo 41,9% positivas e 58,1% negativas. Os dados da análise de conteúdo foram agrupados por similaridade de conteúdo. Consideram-se aspectos facilitadores situação/comportamento/consequência com referências predominantemente positivas, as categorias, a saber: interação: equipe, paciente, família; gerenciar a unidade de trabalho; questionar a implantação do Grupo Gestor e comunicar-se. Em contrapartida, foram considerados aspectos dificultadores situação/comportamento/consequência com referências predominantemente negativas, as categorias: estrutura organizacional; gestão de infraestrutura e gestão de pessoas. Cabe destacar que esses resultados podem subsidiar o trabalho do enfermeiro em cargo gerencial e, também, dos próprios gestores do hospital, uma vez que os aspectos dificultadores do trabalho do enfermeiro em cargo gerencial dizem respeito, em sua maioria, a questões de pouca governabilidade para esse profissional. Assim, evidencia-se a necessidade de maior aproximação dos gestores e enfermeiros com cargo gerencial, a fim de, juntos, solucionarem questões que favoreçam o processo de cuidar e de coordenar o trabalho. É inegável a clareza que os participantes trouxeram sobre sua responsabilidade profissional, visto que, embora os incidentes tenham tido referências predominantemente negativas, os comportamentos apresentados tiveram referências predominantemente positivas, evidenciando os esforços que esses profissionais despendem para superar as dificuldades vivenciadas e a importância considerada em poder compartilhar decisões e ações a serem realizadas / The new configuration of the role of hospitals, with a view to building health care networks, has implications for the work of nurses in management positions in relation to their administrative, helth care and teaching actions/decisions. This study aimed to identify the facilitating and hindering aspects of the work of nurses in management positions in a public emergency hospital in the interior of the state of São Paulo. A descriptive study with quantitative approach of the qualitative data was performed, using the Critical Incident Technique, to survey the perceptions and attitudes in relation to the researched object. The study was developed in a public tertiary-level teaching hospital, located in the northeast region of São Paulo state, Brazil. In total, 15 nurses in management positions, who worked in the function for at least one year, participated, excluding those who were absent from the workplace at the time of data collection as a result of legal absences or because it was not possible to conduct the interview after five appointments canceled. Data collection was performed through individual semi-structured interview. Critical incidents consisting of 42 cases were reported, of which 33.3% were positive and 66.7% negative, culminating in 57 behaviors, of which 84.2% positive and 15.8% negative, and 74 consequences, 41.9% positive and 58.1% negative. Content analysis data were grouped by similarity of content. Situation, behavior and consequence with predominantly positive references were considered as facilitating aspects, with the following categories: interaction- team, patient, family; to manage the work unit; to question the implementation of the Management Group and to communicate. On the other hand, situation, behavior and consequence with predominantly negative references were considered hindering aspects, with the following categories: organizational structure; infrastructure management and personnel management. It is highligted that these results may support the work of nurses in management positions as well as hospital managers, once the hindering aspects of the work of nurses in management positions are related mostly to issues of poor governance to this professional. Thus, the study evidences the need for closer alignment of managers and nurses with management position, in order to commonly solve issues that favor the care process and work coordination. It is undeniable the clarity that participants have about their professional liability, since although the incidents have had predominantly negative references, behaviors presented had mainly positive references, showing the efforts that these professionals expend to overcome the difficulties experienced and the importance considered in sharing decisions and actions to be taken

Occupational health hazards encountered by nurses at Letsholathebe II memorial hospital in Maun, Botswana

Thesis (MPH.) -- University of Limpopo, 2015 / Nurses are an integral component of the health care delivery system and they encounter occupational health problems classified as biological, chemical, physical, and psychosocial hazards. Nurses also face health hazards such as Hepatitis B, Acquired Immune Deficiency Syndrome, tuberculosis, cytotoxic drugs, anesthetic agents, needle stick injury, back pain, and stress. At Letsholathebe II Memorial Hospital in Maun, nurses and other health professionals face occupational health and safety risks at the workplace. Aim and Objectives The aim of the study was to identify the occupational health hazards encountered by nurses at Letsholathebe II Memorial Hospital in Maun, Botswana. The objectives were to identify occupational health hazards at Letsholathebe II Memorial Hospital; determine organic and inorganic disorders caused by occupational health hazards; determine coping mechanisms of nurses towards occupational health hazards and the compliance of nurses to written protocols that address occupational health hazards. Research Method and Design A quantitative descriptive cross-sectional method was adopted. The population comprised 200 nurses employed at Letsholathebe II Memorial Hospital. Simple random sampling was used to select 132 nurses who participated in the study. A self-administered questionnaire was used for data collection. Descriptive and inferential statistics were used for data analysis. Results The study revealed health hazards namely; back aches, frequent headaches, and persistent tiredness; mercury, solvents and anaesthetic gases; HIV, streptococcus, staphylococcus, Hepatitis B and measles. Nurses also reported fatigue, loss of sleep due to stress, anxiety and persistent tiredness. Conclusion The study concluded that nurses at Letsholathebe ll Memorial hospital experienced physical, chemical, biological and psychological health hazards. Recommendations The study recommends that nurses should have access to OHS information, that OHS awareness should be created at Letsholathebe II Memorial Hospital.

Public relations practices within selected public hospitals in KwaZulu-Natal

Submitted in fulfillment of the requirement for the Masters Degree of Technology (Public Relations Management), Department of Public Relations Management, Durban University of Technology, Durban, South Africa, 2015. / In South Africa, the media coverage and the general perceptions of the public about public hospitals have been pervasively negative and this has resulted in the overshadowing of the good work being done in hospitals. In an effort to overcome these negative perceptions, the Kwazulu-Natal Department of Health has introduced the practice of public relations in public hospitals since 2001. This effort is in line with the National Health Act 2004 (Act No 61 of 2003). The act is described by the Health Systems Trust to rest heavily on the Constitution which, amongst other things, requires the State to take reasonable legislative and other measures to progressively achieve the right of access to health care services, and reproductive health care, within its available resources. The rationale behind the introduction of public relations was that, by managing and influencing the public’s perceptions, public relations professionals in public hospitals would initiate a sequence of behaviours that would contribute towards the achievement of the hospitals’ objectives. However, are relevant stakeholders aware of the presence, the role and the functions of public relations in public hospitals? Based on the above, this study seeks to evaluate public relations practices within selected public hospitals in Kwazulu-Natal, South Africa. The study is qualitative, cross-sectional and descriptive in nature. Data will be collected through observation, questionnaires and interviews. The findings revealed that a lot more needs to be done by both the Department of Health in KwaZulu-Natal and hospital CEOs in order to benefit greatly from the practice of public relations in public hospitals as public relations practitioners are under-utilized. Issues of budget constraints, lack of public relations understanding by hospital CEOs are some of the factors impacting negatively the function of public relations in hospitals. It is more of a technical role that public relations practitioners play in public hospitals. This study came to the realisation that not enough attention is given to the practice of public relations in public hospitals. One of the major recommendations is that public relations practitioners should participate in the strategic management processes and be part of the planning process and they should use public relations strategies to build harmony between the hospital and all its external and internal publics. For public hospitals to realise the need and value from the function of public relations, they have to appreciate that the practice of public relations has grown over the years and they have to employ effective public relations practitioners in order to accomplish their goals.

Abordagem do erro em unidades de terapia intensiva paulistanas / Approach to error in brazilian intensive care units

A prática da medicina vem mudando rapidamente. Nos últimos 20 anos os profissionais de saúde tem se preocupado cada vez mais com os erros que ocorrem durante o processo de cuidado dos pacientes enquanto trabalham duramente para preveni-los e mitigá-los. Tão importante quanto o erro em si é a maneira como o evento adverso é discutido e a maneira como os profissionais envolvidos no erro são abordados. O objetivo da pesquisa foi aplicar um questionário sobre erro e abordagem do erro. O instrumento foi originalmente publicado em língua inglesa e validado em português pelo método de Brislin. Responderam ao questionário 161 profissionais de saúde (enfermeiros, técnicos de enfermagem, médicos e fisioterapeutas) provenientes de 19 diferentes unidades de terapia intensiva adulto públicas e privadas da cidade de São Paulo (Estado de São Paulo - Brasil).Os resultados revelaram que profissionais do sexo masculino são mais propensos a reconhecer que cometem erros em relação a profissionais do sexo feminino com uma razão de chances de 0,21 (0,07-0,65); que profissionais oriundos de unidades públicas reportam com mais frequência que ameaça de processos {RC 0,23 (0,11 - 0,48)}, ameaça à estabilidade no emprego {RC 0,49 (0,24 - 0,99)} e personalidade de outros membros da equipe {RC 0,22 (0,09 - 0,51)} são motivos para que os erros não sejam discutidos tampouco abordados adequadamente. Estas informações levantam possíveis oportunidades para aprofundar a discussão e o tratamento de eventos adversos em unidades de terapia intensiva / The practice of medicine is changing quickly. In the last 20 years, health professionals have increasingly worried about errors that occur during the process of patient care while working hard for its prevention and mitigation.As important as the error itself is the way each adverse event is discussed as well as each professional involved in an error is approached .The goal of the research was applying a survey about error and its approach. The tool was originally published in english and afterwards validated in portuguese by the Brislin method. The survey was answered by 161 health professionals (nurses, nurse technicians, intensive care physicians and physiotherapists) coming from 19 different adult intensive care units both public and private in the city of São Paulo (São Paulo - Brazil). The results revealed that male professionals are more likely to recognize having made an error than female professionals with a odds ratio of 0.21 (0.07-0.65). It also showed that professionals coming from public units report more often that the threat of litigation {RC 0.23 (0.11 - 0.48)}, threat of unemployment {RC 0,49 (0,24 - 0,99) } and other team members personality {RC 0.22} (0.09 - 0.51) are reasons for problems not being discussed or addressed.
