Mechanisms of MAP kinase signaling to transcriptional regulators

Teoh, Peik Lin January 2012 (has links)
The MAPK pathway is important in various biological functions. It is also important in regulating processes associated with gene transcription via different mechanisms such as by phosphorylation of transcription factors, coactivators/corepressors and histone modifier complexes. H3K4 methylation is highly associated with active transcription. Deposition of this mark is catalysed by SET-domain methyltransferases which consists of a WAR complex (WDR5, ASH2L and RBBP5), a catalytic SET-domain protein and other subunits. However, potential links between ERK MAPK signaling and H3K4 methylation in gene expression are not well understood. Thus, the aim of this study was to probe the potential links between these two pathways towards gene-regulatory networks. This study attempted to elucidate their direct functional interaction by studying whether components of the SETD1A complex could be phosphorylated upon ERK activation. Our results showed that the core components of SETD1A complex were not phosphorylated in vivo and in vitro by ERK. Importantly, we reported that at least two splicing variants of RBBP5 exist. ERK-dependent stabilization of exogenous RBBP5 was observed but the mechanism underlying this is unknown. Surprisingly, we found that WAR complex depletion increased the pre-mRNA expression of immediate-early (IE) genes which did not necessarily reflect changes in their mRNA levels. In addition, this occurred in an H3K4me3-independent manner. This regulation is likely to be posttranscriptional that involves pre-mRNA processing events. First, we noticed a decrease of transcription initiation in WAR complex-depleted cells upon ERK activation. Second, depletion of the WAR complex affects the splicing efficiency of FOS and EGR1. Third, RBBP5 occupancy was observed and was significantly reduced upon siRNA-mediated RBBP5 depletion at the coding region and the 3' end of FOS gene. Therefore, we propose that the WAR complex regulates the pre-mRNA processing of IE genes through an interaction between RBBP5 and a splicing factor that has yet to be identified.

The perceived impact of an emergency department immediate reporting service: An exploratory survey

Snaith, Beverly, Hardy, Maryann L. January 2013 (has links)
No / Immediate reporting, commonly referred to as a ‘hot reporting’, has been advocated as a method of effectively supporting clinical decision making. However, its implementation nationally has been limited with poor understanding of its value in practice. A cross sectional attitudinal survey was distributed to emergency department clinicians (medical and nursing staff) and radiographers to explore perceptions of an immediate reporting service in terms of its influence on professional role and autonomy, patient care and service quality. A total of 87 (n = 87/155; 56.1%) completed questionnaires were returned. The findings suggest that significant support for immediate reporting exists. Immediate reporting is believed to improve service quality, reduce clinical errors and provide opportunity for image interpretation skills development. However, responses were not consistent across clinical professions and staff grades. The immediate reporting of emergency department images is perceived to benefit patient, emergency department clinicians and hospital organisation.

The aim of this study was to evaluate, in a short period of time, the success rate of immediate loading of single-tooth implants: immediate versus non-immediate implantation. Seventy-one patients were examined, totalizing 91 implants, which 48 implants were placed in private practice office and 43 were placed in dental school, between December 2001 and December 2003. Sixty-one implants were placed in fresh extraction sites, and 28 were placed in healed sites. Criteria to evaluate success rate were previously described by Albrektsson and Zarb (1993). After follow-up evaluation, 87 implants fulfilled the success rate criteria (95,6%). Four implants were lost, and among these, three were placed in fresh extraction sites (success rate 95,2%), and one placed in healed sites (success rate 96,4%). Rates of success between both groups was not statistically significant (p=0,76, Qui-square test). Within the limitation of the present study, it could be concluded that single-tooth osteointegrated implants submitted to immediate loading, in immediate and non-immediate implantation, represent a therapeutic alternative with high predictability, compared to the conventional protocol.

Immediate and subsequent effects of fixed-time food presentations on automatically maintained mouthing.

Simmons, Jason N. 12 1900 (has links)
Several studies have demonstrated that fixed-time (FT) schedules of stimulus delivery can function to reduce a variety of behaviors. The purpose of this study was to evaluate the immediate and subsequent effects of FT food deliveries on mouthing. In Phase 1, a preference assessment showed that caramel popcorn, chocolate cookies and pretzels were highly preferred food items. Thus, providing the basis for use of food items during treatment. In Phase 2, a functional analysis showed that mouthing was a nonsocially maintained problem behavior. Phase 3 demonstrated the use of FT schedules of food deliveries as treatment for nonsocially maintained mouthing. Results indicated that FT schedules of food significantly reduced mouthing. In addition, levels of mouthing observed during post-FT observations were reliably lower than pre-FT observations. Treatment effects, operative mechanisms responsible for the treatment effects and the experimental arrangement used to investigate varying FT schedules are discussed.

Fentanyl sublingual spray for breakthrough cancer pain in patients receiving transdermal fentanyl

Alberts, David S, Smith, Christina Cognata, Parikh, Neha, Rauck, Richard L 10 1900 (has links)
Aim: To investigate the relationship between effective fentanyl sublingual spray (FSS) doses for breakthrough cancer pain (BTCP) and around-the-clock (ATC) transdermal fentanyl patch (TFP). Methods: Adults tolerating ATC opioids received open-label FSS for 26 days, followed by a 26-day double-blind phase for patients achieving an effective dose (100-1600 mu g). Results: Out of 50 patients on ATC TFP at baseline, 32 (64%) achieved an effective dose. FSS effective dose moderately correlated with mean TFP dose (r = 0.4; p = 0.03). Patient satisfaction increased during the study. Common adverse events included nausea (9%) and peripheral edema (9%). Conclusion: FSS can be safely titrated to an effective dose for BTCP in patients receiving ATC TFP as chronic cancer pain medication. ClinicalTrials.gov identifier: NCT00538850
