Syftet med studien var att undersöka sjuksköterskors upplevelser av att arbeta med känguruvård inom svensk neonatalvård. Tidigare forskning är sparsamt beskriven och inga studier är publicerade som rör svensk omvårdnad utifrån denna metod. Semi-strukturerade intervjuer med sju sjuksköterskor genomfördes på en neonatalavdelning i södra Sverige. Textmaterialet analyserades, efter att ha skrivits ut ordagrant, enligt en kvalitativ innehållsanalys. Analysen resulterade i tre huvudkategorier; att befrämja anknytning, att uppleva inspiration och att uppleva begränsning. Sammanfattningsvis var personalen överens om fördelarna med känguruvård men poängterade också de svårigheter som fanns främst relaterade till arbetsmiljön. / The aim of this study was to describe nurses experiences of working with kangaroo care in Swedish neonatal care. Earlier research is limited regarding nurses experiences of kangaroo care in a Swedish context. Semi-structured interviews with seven nurses have been carried out in a neonatal unit in the southern of Sweden. Verbatim transcripts were produced. The transcriptions were analysed using a qualitative content analysis. The results of this study fall into three main categories; to support attachment, to experience inspiration and to experience limitation. The conclusions were that the nurses described the benefits of kangaroo care. The difficulties they described were related to the environment.

The Effects of Kangaroo Care on The Stress Levels of Parents in The NICU

Background: Parents of NICU patients have high stress levels compared to other populations, which negatively impact their mental health. The kangaroo care (KC) method has been used for its numerous benefits for infants, but not as widely studied for its impacts on parental stress. The purpose of this integrative literature review is to examine and synthesize the existing evidence regarding kangaroo care and its impact on the stress levels of NICU parents. Methods: A literature search was conducted utilizing CINAHL Plus with Full Text, MEDLINE, and APA PsychInfo databases to identify relevant articles. Articles were included if they were published within the last five years (2018-2023), discussed parental subjective and/or objective measures of stress, and implemented a KC intervention in the NICU. A total of seven articles were analyzed and synthesized. Results: Key findings in this review included how KC is implemented and its effects on subjective and objective stress. Findings from all seven articles demonstrate that KC is effective in decreasing cortisol, increasing oxytocin, and decreasing subjective parental stress. Discussion: KC as an intervention for stress reduction for NICU parents showed an overall positive effect on parental stress measures. It may be beneficial for NICU providers to encourage KC not only for the positive benefits on the infant, but for the positive effects it can have on parental stress. However, additional research is necessary in subsequent populations, geographical locations, and with different measures of stress to improve generalizability of the results.

Exploratory study of fathers providing Kangaroo Care in a Neonatal Intensive Care Unit

Yes / Aim and Objectives: To explore fathers' views and experiences of providing Kangaroo Care (KC) to their baby cared for in a Neonatal Intensive Care Unit (NICU). Kangaroo Care has been known to improve the health outcome for preterm, low birth weight and medically vulnerable term infants and achieve the optimal perinatal health wellbeing for parents and infants. Historically, mothers are considered as the dominant KC providers, whereas fathers are spectators and have been overlooked. Little is known about the fathers' perspectives in providing KC in NICUs. Methods: Individual semi-structured interviews were conducted with 10 fathers who delivered KC to their baby when in the NICU. Data were analysed using Braun and Clarke's six-phase thematical framework. The Consolidated Criteria for Reporting Qualitative Research (COREQ) checklist was followed to report this qualitative study. Findings: Fathers in this study identified they were passing a silent language of love and connecting with their baby by the act of KC in a challenging environment. Three themes emerged: ‘Positive psychological connection’, ‘Embracing father-infant Kangaroo Care’ and ‘Challenges to father-infant Kangaroo Care’. Conclusion: The findings of this study show KC enhances the bonding and attachment between fathers and infants. The conceptualisation of the paternal role in caregiving to a newborn is evolving as a contemporary practice. Further research is warranted to confirm or refute the study findings. Policies and facilities should be modified to include father–infant KC within the fields of neonatal care. Relevance to Clinical Practice: It is important for nurses and other health professionals to support and enable fathers to give KC. Fathers providing kangaroo care in neonatal intensive care units

Yes / . Kangaroo care (KC) has been used widely in neonatal care to promote bonding/attachment and neurodevelopment for preterm and term infants. However, current literature suggests that research mainly focuses on infants' and mothers' experiences. The role of fathers in caring for their infant/child is changing and evolving in many countries around the globe. Yet little is known about fathers' experiences of KC in neonatal units. This review, therefore, aims to scope the current evidence of Father-infant KC (Father KC) in Neonatal Intensive Care Units (NICUs). Research question. What impact does KC have upon fathers when their baby is cared for in NIUCs? Search method. A scoping review was conducted and guided by the framework of Arksey and O'Malley (2005). The data sources consisted of Medline, Embase, America Psychological Association (APA) PsycInfo, Emcare, Cochrane Central, Web of Science, Google Scholar and ProQuest. The study inclusion criteria were: (1) studies involving fathers who had experience of KC with their baby whilst in Neonatal Intensive Care Units (NICUs), and other neonatal care settings, such as Special Care Baby Nursery (SCBU), delivery/labour room and postnatal ward; (2) literature published from 2000 to 2020; (3) primary studies including qualitative, quantitative, and mixed methods studies; (4) studies published in English. Results. The total number of studies identified were 13. Seven studies were qualitative and six were quantitative. None were mixed methods. Studies reported several positive KC benefits on fathers, such as reduced stress, promotion of paternal role and enhanced father-infant bond. It was highlighted that KC could be time-consuming for fathers and challenging to practise when balancing work and family life commitments. Conclusion. This review provides evidence that KC practice has health and wellbeing benefits for fathers and infants in NICUs and other relevant neonatal care settings, The findings of this review support the justification to promote Father KC in NICU environments, and guide policies to include father involvement. Implementing Father KC in NICU settings will assist fathers to care and connect with their baby. Further research is needed to explore how to facilitate and evaluate KC education for fathers from diverse s and cultures.

Considera-se que a Inovação Social pode emergir Top-down em que os agentes induzem a inovação externamente de “cima para baixo”, por exemplo: política de governo, empresa ou indústria; ou Bottom-up quando emergem de “baixo para cima” pela sociedade civil, indivíduos ou grupos. Assim, as influências Bottom-up podem garantir o sucesso da inovação e a sua replicação em um estágio mais avançado, dependendo do meio local onde está inserido. André e Abreu (2006) nomeiam essas questões locais por Meio Inovador que depende da Diversidade, Tolerância e Participação para se tornar mais ou menos capaz de inovar. O método canguru é alicerçado dentro da Unidade de Terapia Intensiva Neonatal (UTINeo), que é o foco desse estudo. In the health area, in addition to technological advances and rational accompanying specific innovations in this sector, it is considered necessary with a call to integral human being with subjective values, guided by ethics, respect and solidarity in human attention. In this sense, Attention Humanized the Newborn Low Birth Weight - Kangaroo Mother Care was proposed by a government policy in 1999 in Brazil to qualify the social interactions in the Unified Health System, through changing values and parental participation. Through the built bonding and training of parents and relatives, all become empowered to assist the sick newborn care, which benefits from this link to your best recovery. It is considered that the Social Innovation can emerge Top-down in which agents induce innovation outside of "top-down", for example: government policy, business or industry; Bottom-up or when emerging from "the bottom up" by civil society, individuals or groups. So, the bottom-up influences can ensure the success of innovation and its replication in a more advanced stage and cyclical, depending on the local environment in which it operates. André and Abreu (2006) appoint these local issues by Environment Innovator that depends on the diversity, tolerance and participation to become more or less able to innovate. Kangaroo care is rooted in the l Intensive Care Unit which is the focus of this study. Therefore, it is necessary to understand, in a historical context, the local characteristics of how this method have been adopted in the neonatal ICU and bottom-up influences the environment where IS is considering its challenges and potential. This is a case study, qualitative, sectional with longitudinal perspective in the areas of administration and health. It was chosen as the study site to neonatal intensive care Clinical Hospital, owned by Federal University of Pernambuco. As a result, it is inferred that the Kangaroo method is social innovation in such a process; it is understood that there have been and there are still many challenges to be overcome as part listed in all historical phases, whereas there are also many local potential. How challenging factors are cited: the "rigidity” of public administration, the lack of preparation and priority of managers which most were not hospital management experts, the shortcomings of the public health service network and demotivation of participatory processes team. As potential factors cited were the participatory management stimulation of Kangaroo Care, creativity, sensitivity and interaction of the health team, as well as good acceptance of new external science-based, not the staff accommodation which tends to have risk posture.

The Effects of Kangaroo Care on the Neurodevelopment of Preterm Infants in the Neonatal Intensive Care Unit (NICU)

Preterm birth disrupts the development of the brain and other critical organs of the infant body. Since the brain is one of the last organs to finish developing during pregnancy, the risk for substantial neurological deficits increases as the gestational age decreases. One way to combat these deficits is to reconnect the preterm infant with the mother via skin-to-skin contact, also known as kangaroo care (KC). This intimate touch helps to replicate aspects of the environment that the preterm infant experienced in utero. The purpose of this literature review was to analyze the current literature to better understand the effects that KC may have on facilitating neurodevelopment of preterm infants in Neonatal Intensive Care Units (NICUs). Emphasis was placed on neurophysiologic functioning, autonomic functioning, and neurobehavioral functioning. A database search of CINAHL Plus with Full Text, PsycINFO, MEDLINE, Cochrane Central Register of Controlled Trials, and Health Source: Nursing/Academic Edition was conducted, and a total of six articles were reviewed based on their relevance and application towards this thesis. KC is a low-cost, relatively easy intervention to initiate that can have positive impacts on many aspects of preterm infant growth and maturation. There is limited research regarding the use of KC as an intervention to support neurodevelopment, especially with regards to long-term effects. Existing research supports the use of KC as an intervention to facilitate neurodevelopment in preterm infants in the NICU.

Föräldrars upplevelser av känguruvård på neonatalavdelningar

Bakgrund: Känguruvård är en typ av vård som används hos för tidigt födda eller sjuka barn. Metoden utgörs av tre delar: förlängd hud-mot-hud-kontakt, strävan efter exklusivt födointag av bröstmjölk samt strävan efter en tidig hemgång eller förflyttning till lägre vårdnivå. Kängurumetoden har visat sig ha många positiva effekter varpå metoden idag är rekommenderad att initieras en timme efter födseln. Föräldrarna har en viktig roll i känguruvården vilket gör det relevant att undersöka deras upplevelser av behandlingsmetoden för att den ska kunna optimeras.  Syfte: Syftet var att undersöka föräldrars upplevelser av att vårda sitt barn med känguruvård på neonatalavdelningar. Metod: Kvalitativ, deskriptiv litteraturstudie. Tio artiklar inkluderades i resultatet. Huvudresultat: Resultatet från litteraturstudien delades in i två huvudkategorier; (i) barriärer för att utföra känguruvård och (ii) emotionella upplevelser. Resultatet var övervägande positivt där föräldrarna upplevde ett ökat självförtroende, en märkbar trygghet hos barnet samt en ökad anknytning mellan barn och förälder. Även barriärer för att utföra känguruvården framkom. Dessa utgjordes av sjukhusmiljön, personalens attityder, livet utanför samt hälsan hos mamman.  Slutsats: Föräldrarnas upplevelser av känguruvård var övervägande positiva där de beskrev att behandlingsmetoden var något som stärkte dem emotionellt. Det framkom dock även hinder i samband med utförandet av känguruvård vilket innebär att en utvecklingspotential för behandlingsmetoden finns. I sjukvården och som sjuksköterska är det därför viktigt att lyssna på föräldrarnas upplevelser och åsikter för att uppnå en positiv utveckling inom känguruvården.  Nyckelord: Känguruvård, föräldrar, upplevelser, prematur, hud-mot-hud. / Background: Kangaroo mother care is a method used for premature or sick children. The method contains three parts: extended skin-to-skin contact, an aim for exclusive food intake of breast milk, as well as an aspiration for an early departure home or a transfer to a lower level of care. The kangaroo mother care has indicated several positive effects and is therefore currently recommended to be initiated within an hour post labor. The parents play a crucial part in the kangaroo mother care making it relevant to examine their experiences of the method of treatment to enable optimization of it.  Aim: The aim was to examine the parental experiences by caring for their newborn children with kangaroo mother care in the neonatal intensive care unit. Method: Qualitative, descriptive literature study. Ten articles were included in the result. Main findings: The result from the literature study were divided into two main categories; (i) barriers to execute kangaroo mother care and (ii) emotional experiences. The result was predominantly positive as the parents experienced an increased self-esteem, a noticeable sense of security of the child and an increased attachment bond between child and parent. Barriers to perform kangaroo mother care also emerged. These were constituted by the hospital environment, the attitude of staff, life outside the hospital and the mother’s health. Conclusion: The parental experiences of kangaroo mother care were predominantly positive as they expressed a feeling of an increased emotional strength due to the method of treatment. However, barriers to execute kangaroo mother care were also presented which indicates a potential for further development of the method. Within health care and as a nurse it is therefore important to listen to parents voicing their experiences and opinions in order to achieve a positive development of kangaroo mother care.  Keywords: Kangaroo care, parents, experiences, premature, skin-to-skin.
