Análise tridimensional comparativa de reabilitações para a região posterior da mandíbula, por meio do método de elementos finitos / Three-dimensional comparative analysis of rehabilitations for the posterior mandible, with the Finite Element Method

A reabilitação da região posterior da mandíbula com implantes dentários é complexa, especialmente em casos de reabsorção óssea moderada a severa. Embora existam diversas possibilidades de tratamento para essa região, os implantes curtos e a lateralização tem se destacado como alternativas viáveis e de menor tempo de tratamento. O objetivo deste trabalho foi avaliar, comparativamente, o risco mecânico dos dois tratamentos mencionados, com especial atenção ao risco de perda óssea e de fratura do parafuso. Para tanto, foram processadas tomografias de mandíbulas e realizada engenharia reversa de implantes e componentes protéticos para reconstrução de modelos tridimensionais, a fim de simular o comportamento biomecânico de próteses parciais fixas de três elementos, suportadas por dois implantes, utilizando simulações com o Método dos Elementos Finitos. Os modelos de implantes utilizados foram baseados nos implantes MK III (Nobel Biocare), com 5 e 4 mm de diâmetro por 7 de comprimento, representando implantes curtos, e com 4 e 3.75 mm de diâmetro por 15 de comprimento, representando implantes sob tratamento de lateralização do nervo alveolar inferior. Modelos distintos foram confeccionados para representar implantes tratados em um estágio (carga imediata) e dois estágios (implantes osseointegrados). Todos os modelos receberam simulação prévia de pré-tensão, referente a tensão gerada pelo torque do parafuso. Um padrão de carga oclusal axial e outro com carga oclusal oblíqua em 45% foram simulados, resultando em 16 diferentes simulações. A análise dos resultados revelou que o risco mecânico para perda óssea na região periimplantar, quando analisados implantes colocados em dois estágios, é maior para tratamentos com implantes curtos. Entretanto, para implantes colocados em um estágio, o maior risco de perda óssea foi para os implantes colocados com lateralização do nervo alveolar inferior. Quanto aos resultados dos parafusos, embora não se observou claras tendências quando simuladas cargas axiais, sob cargas oblíquas, os implantes de maior diâmetro e menor comprimento tiveram o menor risco à fratura do parafuso. / The rehabilitation of the posterior region of the mandible with dental implants is complex, especially in cases of moderate to severe bone loss. While there are several treatment options for this region, the use of short implants and lateralization has emerged as viable alternatives with less treatment time. The objective of this study was to comparatively evaluate the mechanical risk of both treatments mentioned, with special attention to the risk of bone loss and screw fracture. For this, CT scans of mandibles were processed and implants and prosthetic components were reverse engineered for reconstruction of three dimensional models to simulate the biomechanical behavior of fixed partial dentures of three elements, supported by two implants, using simulations with the Finite Element Method. The models of implants were based on MK III implants (Nobel Biocare) with 5 and 4 mm in diameter by 7 mm in length, representing short implants, and 4 and 3.75 mm in diameter by 15 mm long, representing implants used in treatments with lateralization of the inferior alveolar nerve. Several models were modeled to represent implants treated in one stage (immediate load) and two stages (osseointegrated implants). All models were simulated with pre-stress concerning the stresses generated by the torque of the screw. Axial and oblique oclusal loads at 45% were simulated, resulting in 16 different simulations. The results showed that the mechanical risk for bone loss, when osseointegrated implants were analyzed, is greater for treatments with short implants. However, for implants placed in one stage, the increased risk of bone loss was for implants placed with lateralization of the inferior alveolar nerve. Concerning the results of screws, there were no clear trends when axial loads were simulated, however, under oblique loads, the implants of larger diameter and shorter length had the lowest risk of screw fracture.

Avaliação de linguagem por ressonância magnética funcional em indivíduos com epilepsia do lobo temporal secundária a esclerose mesial temporal / FMRI language lateralization in patients with epilepsy secondary to mesial temporal sclerosis

Introdução: A lateralização atípica de linguagem (LAL) é mais frequentemente observada em indivíduos com epilepsia no hemisfério esquerdo. Pacientes com epilepsia secundária a esclerose mesial temporal (EMT) apresentam alta frequência de LAL. A presença de EMTE, diretamente ou por efeitos à distância, perturbaria a determinação típica de lateralidade de linguagem (LL). Estudos de LL com RMf, em populações homogêneas de pacientes com EMT são escassos. Os padrões regionais de reorganização, nesta população, assim como os fatores clínicos e funcionais associados à LAL são pouco conhecidos. Os objetivos deste estudo foram (1) Validar paradigma de linguagem em RMf capaz de ativar áreas frontais e temporais de linguagem (2) Avaliar LL regional, em população homogênea de pacientes com EMT e verificar o papel de variáveis clínicas e da atividade epileptiforme na determinação de LAL. Foram avaliados 46 pacientes com EMT unilateral (24 EMTE e 22 EMTD) e 24 controles saudáveis, destros. Todos os participantes foram submetidos a RMf de linguagem com paradigma de nomeação responsiva à leitura (NRL) desenvolvido em língua portuguesa. O paradigma foi eficaz em ativar áreas de linguagem. A correlação entre IL-RMf e IL-Wada foi estatisticamente significativa no GFM (0,387; p < 0,01); GFI (0,594, p < 0,001); GTS (0,357, p < 0,05); GTM (0,509, p < 0,001) e GTI (0,489, p < 0,001). Os pacientes foram classificados em discordantes e concordantes (EEG/RM), após monitorização prolongada por vídeo-EEG. As classificações individuais de LL do teste de Wada e RMf foram concordantes em 84%(GFM), 86% (GFI), 74%(GTS), 83%(GTM) e 86%(GTI) dos casos. A frequência de LAL variou de 17 a 33%, para EMTE; 14 a 17%, para EMTD e de 0 a 10%, no grupo controle. Os grupos EMTEc e EMTEd apresentaram menores valores de IL-RMf, em relação ao grupo controle no GFM e GFI (p <= 0.05). Adicionalmente, o grupo EMTEd apresentou menor IL-RMf no GTM (p <= 0.01) e GTI (p <= 0.05). O grupo EMTDd apresentou menor IL-RMf no GFM; GFI; GTM e GTI (p <= 0.05). A ocorrência de mais de 5 descargas/hora no período interictal associou-se a LAL no GFM (p <= 0.01). Início da epilepsia, maior tempo de epilepsia e presença de IPI não foram associadas a LAL. Concluiu-se que a presença de EMT associou-se a LAL nas regiões frontais e temporais, sugerindo reorganização de linguagem envolvendo amplas regiões cerebrais, distantes da zona epileptogênica. O achado de LAL observada no grupo EMTDd sugere que a atividade epileptiforme temporal esquerda pode influenciar a reorganização de linguagem, independentemente da presença de lesão estrutural à esquerda. A maior frequência de descargas à esquerda associou-se a LAL / Atypical language lateralization (ALL) is associated with left hemisphere epilepsy, especially in epilepsy secondary to left mesial temporal sclerosis (MTS). Recent findings indicate that MTS, directly or by an at-distance effect may disrupt typical language lateralization (LL). Language-fMRI investigations in homogeneous MTS populations are scarce. Frontal and temporal language reorganization patterns in this population remain incompletely understood, as well as clinical factors associated with ALL. Our objectives were (1) To validate an fMRI paradigm effective to activate frontal and temporal language areas (2) To evaluate regional LL patterns in a homogeneous population of MTS patients and to investigate clinical factors associated with ALL. We studied forty-six right-handed patients (24 left MTS and 22 right MTS) and 24 controls. Patients and controls were similar in age and gender. Left and right MTS groups were comparable in terms of clinical variables. All participants underwent a language fMRI with reading response-naming paradigm (RRN) task developed in Brazilian Portuguese. The task effectively activated frontal and temporal language areas, with a high concordance with the Wada test. All patients were classified as concordant and discordant after video-EEG monitoring. Correlation between Wada test and fMRI LIs was significant in MFG (0.387; p < 0.01); IFG (0.594, p < 0.001); STG (0.357, p < 0.05); MTG (0.509, p < 0.001) e ITG (0.489, p < 0.001). Individual LI classifications with Wada test and fMRI were concordant in 84%(MFG), 86% (IFG), 74%(STG), 83%(MTG) e 86%(ITG). The frequency of ALL was 17-33%, in left MTS; 14-17%, in right MTS and 0-10%, in controls. Both concordant and discordant-left MTS patients presented lower LI than controls in MFG and IFG (p<=0.05). Additionally, discordant left MTS group presented lower LI in MTG (p<=0.01) and ITG (p<=0.05). Discordant right MTS group presented lower LI in MFG; IFG; MTG and ITG (p <= 0.05). Interictal left-sided spike frequency (< 5/h) was associated with lower LI (p <= 0.01) in MTG. In conclusion, MTS was associated with frontal and temporal language reorganization. Presence of ALL in discordant RMTS group suggests that left temporal epileptic activity may influence language reorganization, even in the absence of a left structural lesion. Higher left-sided spike frequency in MTS was associated with a left-right shift of speech representation

Funtional Near Infrared Spectroscopy Study of Language, Joint Attention and Motor Skills

Near infrared spectroscopy (NIRS) is an emerging non-invasive optical neuro imaging technique that monitors the hemodynamic response to brain activation with ms-scale temporal resolution and sub-cm spatial resolution. The overall goal of my dissertation was to develop and apply NIRS towards investigation of neurological response to language, joint attention and planning and execution of motor skills in healthy adults. Language studies were performed to investigate the hemodynamic response, synchrony and dominance feature of the frontal and fronto-temporal cortex of healthy adults in response to language reception and expression. The mathematical model developed based on granger causality explicated the directional flow of information during the processing of language stimuli by the fronto-temporal cortex. Joint attention and planning/ execution of motor skill studies were performed to investigate the hemodynamic response, synchrony and dominance feature of the frontal cortex of healthy adults and in children (5-8 years old) with autism (for joint attention studies) and individuals with cerebral palsy (for planning/execution of motor skills studies). The joint attention studies on healthy adults showed differences in activation as well as intensity and phase dependent connectivity in the frontal cortex during joint attention in comparison to rest. The joint attention studies on typically developing children showed differences in frontal cortical activation in comparison to that in children with autism. The planning and execution of motor skills studies on healthy adults and individuals with cerebral palsy (CP) showed difference in the frontal cortical dominance, that is, bilateral and ipsilateral dominance, respectively. The planning and execution of motor skills studies also demonstrated the plastic and learning behavior of brain wherein correlation was found between the relative change in total hemoglobin in the frontal cortex and the kinematics of the activity performed by the participants. Thus, during my dissertation the NIRS neuroimaging technique was successfully implemented to investigate the neurological response of language, joint attention and planning and execution of motor skills in healthy adults as well as preliminarily on children with autism and individuals with cerebral palsy. These NIRS studies have long-term potential for the design of early stage interventions in children with autism and customized rehabilitation in individuals with cerebral palsy.

Dans le cadre de cette thèse, nous investiguons la capacité de chaque hémisphère cérébral à utiliser l’information visuelle disponible lors de la reconnaissance de mots. Il est généralement convenu que l’hémisphère gauche (HG) est mieux outillé pour la lecture que l’hémisphère droit (HD). De fait, les mécanismes visuoperceptifs utilisés en reconnaissance de mots se situent principalement dans l’HG (Cohen, Martinaud, Lemer et al., 2003). Puisque les lecteurs normaux utilisent optimalement des fréquences spatiales moyennes (environ 2,5 - 3 cycles par degré d’angle visuel) pour reconnaître les lettres, il est possible que l’HG les traite mieux que l’HD (Fiset, Gosselin, Blais et Arguin, 2006). Par ailleurs, les études portant sur la latéralisation hémisphérique utilisent habituellement un paradigme de présentation en périphérie visuelle. Il a été proposé que l’effet de l’excentricité visuelle sur la reconnaissance de mots soit inégal entre les hémichamps. Notamment, la première lettre est celle qui porte habituellement le plus d’information pour l’identification d’un mot. C’est aussi la plus excentrique lorsque le mot est présenté à l’hémichamp visuel gauche (HVG), ce qui peut nuire à son identification indépendamment des capacités de lecture de l’HD. L’objectif de la première étude est de déterminer le spectre de fréquences spatiales utilisé par l’HG et l’HD en reconnaissance de mots. Celui de la deuxième étude est d’explorer les biais créés par l’excentricité et la valeur informative des lettres lors de présentation en champs divisés. Premièrement, nous découvrons que le spectre de fréquences spatiales utilisé par les deux hémisphères en reconnaissance de mots est globalement similaire, même si l’HG requière moins d’information visuelle que l’HD pour atteindre le même niveau de performance. Étonnament toutefois, l’HD utilise de plus hautes fréquences spatiales pour identifier des mots plus longs. Deuxièmement, lors de présentation à l’HVG, nous trouvons que la 1re lettre, c’est à dire la plus excentrique, est parmi les mieux identifiées même lorsqu’elle a une plus grande valeur informative. Ceci est à l’encontre de l’hypothèse voulant que l’excentricité des lettres exerce un biais négatif pour les mots présentés à l’HVG. De façon intéressante, nos résultats suggèrent la présence d’une stratégie de traitement spécifique au lexique. / In this thesis, we investigate the cerebral hemispheres’ ability to use the available visual information for word recognition in lateral periphery. It is generally acknowledged that the left hemisphere (LH) is more able at reading than the right (RH). Accordingly, the visuoperceptual mechanisms of the brain for word recognition are primarily localized in the LH (Cohen, Martinaud, Lemer et al., 2003). As normal readers use medium spatial frequencies (about 2,5 – 3 cycles per degree of visual angle) to recognize words, it is possible that the LH is better tuned for processing these spatial frequencies than the RH (Fiset, Gosselin, Blais et Arguin, 2006). Furthermore, studies concerned with reading abilities in the cerebral hemispheres commonly present words in visual periphery. However, the effect of visual eccentricity on word recognition is thought to be unequal between hemifields, notably because the first letter in a word usually carries the most information for its accurate identification. It is also the most eccentric letter when a word is presented in the LVF, which may cause a negative bias for the identification of words presented to the LVF regardless of the actual reading capacities of the RH. The main objective of the first study is to determine the spatial frequency tuning functions of the LH and RH for word recognition. The goal of our second study is to explore letter identification biases for words presented to the left and right visual fields as a function of eccentricity by varying the information value of letter positions. Firstly, we discover that the spatial frequency tuning of both hemispheres is globally similar, even though the LH requires less visual information than the RH to reach the same level of performance. Surprisingly however, the RH requires higher spatial frequencies to identify longer words. Secondly, we find that for LVF displays, the first letter, i.e. the most eccentric, is among the most accurately identified, even when it has a greater information value. This argues against the hypothesis that letter eccentricity exerts a negative bias for words presented to the LVF. Interestingly, our findings also suggest a lexical-specific processing strategy.

Localisation et caractérisation du déroulement de la crise d'épilepsie temporale / Localization and characterization of the seizure development of temporal lobe epilepsy

L’électroencéphalogramme (EEG) est un examen incontournable pour le diagnostic, la définition des structures cérébrales responsables de l’origine de crises et la classification des épilepsies. Cependant les enregistrements recueillis à la surface du scalp sont très perturbés par des artefacts et du bruit, ce qui complique considérablement l’interprétation clinique ou l’analyse automatique.Ce travail a pour objectif d’extraire des descripteurs des signaux d’EEG de surface qui peuvent conduire à la caractérisation de la dynamique spatio-temporelle des crises partielles du lobe temporal. Les estimateurs de relations inter-voies appliqués sont les méthodes linéaires paramétriques symétriques et non symétriques telles que l’inter-spectre (S), la cohérence (C), la Directed Transfert Function (DTF) ou la Partial Directed Coherence (PDC). Les relations sont estimées sur des EEG réels contenant une crise. La détection de fortes relations inter-voies est exploitée pour latéraliser puis caractériser la crise. Toutes les méthodes sont appliquées sur des signaux EEG bruts et prétraités. Une étape de prétraitement basée sur la séparation et classification de sources et le débruitage est mise en œuvre afin d’éliminer les artefacts et le bruit avec une perte minimale d’information en diminuant le risque de fausses détections de relations de connectivité inter-signaux. Les résultats obtenus sur 51 crises montrent que le prétraitement améliore la détection et le taux de bonnes latéralisations. Une méthode de couplage entre l’IS et les méthodes paramétriques directives (PDC et DTF) permet d’améliorer la caractérisation des crises / The electroencephalogram (EEG) is the essential clinical examination for the diagnosis, the definition of brain structures responsible of seizures and epilepsy classification. However, the signals collected on the surface of the scalp are very disturbed by artifacts and noise, which complicates the clinical interpretation or the automatic analysis. This work aims to extract descriptors of surface EEG signals that can lead to the spatio-temporal characterization of the temporal lobe seizures. The inter-channel relationship estimators applied are parametric linear methods, such as cross-spectrum (S), coherence (C), Directed Transfer Function (DTF) or Partial Directed Coherence (PDC). Relations are estimated on real EEG recordings containing a crisis. The detection of strong inter-channel relationships is exploited in order to lateralize and to characterize seizures. All methods are applied to raw and preprocessed EEG signals. A preprocessing step, based on the separation and classification of sources and denoising is implemented to remove artifacts and noise with a minimal loss of information by reducing the risk of false detections of inter-signal connectivity relationships. The results on 51 crises show that a signal preprocessing improves the detection and the rate of correct lateralization. A coupling method between S and directivity parametric methods (PDC and DTF) improves the characterization of crises

Visuo-spatial attention in reading

Visuell-räumliche Aufmerksamkeit spielt eine Schlüsselrolle in der Wortverarbeitung und der Augenbewegungskontrolle beim Lesen. Dennoch bleibt die räumliche und zeitliche Dynamik von Aufmerksamkeitsbereitstellung innerhalb einer Fixation ungeklärt. Die vorliegende Dissertation nutzt drei Ansätze in der Ko-Registrierung von Augenbewegungen und EEG um diese Forschungslücke zu untersuchen und direkte Einblicke in die Aufmerksamkeitsverteilung zwischen Fovea und Parafovea, ihre Anpassung an Verarbeitungsansprüche und Sakkaden sowie ihre Auswirkungen auf Wortverarbeitung zu liefern. In Studie 1 wurde das Probe Paradigma als Maß zur Abbildung räumlicher Aufmerksamkeit ohne Augenbewegungen implementiert. Die vergrößerte, Probe-bezogene N1 zwischen Wortpräsentationen deutet auf eine Bereitstellung und Umverteilung zusätzlicher Ressourcen zur simultanen fovealen und parafovealen Wortverarbeitung hin. Studie 2 untersuchte den Preview Benefit und seine Modulation durch Sakkaden und Verarbeitungsschwierigkeit und zielte darauf ab, das elektrophysiologische Korrelat als indirektes Maß für parafoveale Aufmerksamkeit in ERPs und FRPs zu etablieren. Es zeigte sich ein Effekt des orthographischen Previews auf die N1. Interaktionen mit Lesemodus und Verarbeitungsschwierigkeit implizieren, dass dem Preview Effekt Aufmerksamkeitseffekte zugrunde liegen. In Studie 3 wurden zwei Datensätze hinsichtlich der Lateralisierung von Oszillationen im Alpha-Band untersucht um die in Studie 2 generierten Hypothesen zu unterstützen. Alpha war im sakkadischen Lesen stärker rechts-lateralisiert als in RSVP und eine stärkere Lateralisierung sagte eine kürzere Fixationsdauer vorher, was die Rolle von parafovealer Aufmerksamkeit und ihre Verbindung zu Sakkaden betont. Trotz der Einschränkungen der drei Ansätze stellt die Kombination von Augenbewegungen, ERPs, FRPs und EEG-Oszillationen geeignete Maße für Aufmerksamkeitsprozesse in der Wortverarbeitung dar, die traditionelle Forschungsmethoden ergänzen. / Visuo-spatial attention is a key contributor to word processing and oculomotor control in reading. Yet, the spatial and temporal dynamics of attention allocation within a fixation remain unclear. The present dissertation uses three approaches in the co-registration of eye movements and EEG to investigate this gap and provide direct, online insights into attention distribution across fovea and parafovea, its adaptation to processing load and saccadic behavior, as well as its effects on word processing. In Study 1, the probe paradigm was implemented as a mapping tool of spatial attention adaptation in the absence of eye movements, yielding ERPs. The enhanced probe-related N1 in-between trials indicates a recruitment and redistribution of additional resources to achieve simultaneous foveal and parafoveal word-recognition. Study 2 tested the preview benefit and its modulation by saccades and foveal load, thereby aiming at establishing its electrophysiological correlate as an indirect index of parafoveal attention allocation in ERPs and FRPs. There was an effect of orthographic preview on the N1. Interactions with reading mode and foveal load indicate underlying attention effects. In Study 3, two datasets were reanalyzed with regard to the lateralization of oscillatory activity in the alpha-band in order to directly support the assumptions generated in Study 2. Alpha was more strongly right-lateralized in saccadic reading compared to RSVP, and moment-to-moment lateralization predicted shorter subsequent fixation duration, emphasizing the role of parafoveal attention allocation and its relation to saccades. Despite the limitations of the three approaches at this point, the combination of eye movements, ERPs, FRPs, and EEG oscillations provides suitable online markers of attention processes in word recognition that complement traditional research methods.

De façon intéressante, nos résultats suggèrent la présence d’une stratégie de traitement spécifique au lexique. / In this thesis, we investigate the cerebral hemispheres’ ability to use the available visual information for word recognition in lateral periphery. It is generally acknowledged that the left hemisphere (LH) is more able at reading than the right (RH). Accordingly, the visuoperceptual mechanisms of the brain for word recognition are primarily localized in the LH (Cohen, Martinaud, Lemer et al., 2003). As normal readers use medium spatial frequencies (about 2,5 – 3 cycles per degree of visual angle) to recognize words, it is possible that the LH is better tuned for processing these spatial frequencies than the RH (Fiset, Gosselin, Blais et Arguin, 2006). Furthermore, studies concerned with reading abilities in the cerebral hemispheres commonly present words in visual periphery. However, the effect of visual eccentricity on word recognition is thought to be unequal between hemifields, notably because the first letter in a word usually carries the most information for its accurate identification. It is also the most eccentric letter when a word is presented in the LVF, which may cause a negative bias for the identification of words presented to the LVF regardless of the actual reading capacities of the RH. The main objective of the first study is to determine the spatial frequency tuning functions of the LH and RH for word recognition. The goal of our second study is to explore letter identification biases for words presented to the left and right visual fields as a function of eccentricity by varying the information value of letter positions. Firstly, we discover that the spatial frequency tuning of both hemispheres is globally similar, even though the LH requires less visual information than the RH to reach the same level of performance. Surprisingly however, the RH requires higher spatial frequencies to identify longer words. Secondly, we find that for LVF displays, the first letter, i.e. the most eccentric, is among the most accurately identified, even when it has a greater information value. This argues against the hypothesis that letter eccentricity exerts a negative bias for words presented to the LVF. Interestingly, our findings also suggest a lexical-specific processing strategy.

This thesis is concerned with gender-related differences of cognitive predispositions. Findings could be utilized predominantly in human resource management, pedagogy and psychology. The research was proceeded as a case study of 45 undergraduate students. Individual degree of masculinity/femininity (gender) was indicated by the Minnesota Multiphasic Personality Inventory (MMPI), while cognitive preferences were measured and described by Hermann Brain Dominance Instrument (HBDI). Another data were obtained by means of qualitative research methods, such as participant observation and semi-structured interviews. Besides, in general features were surveyed hand-lateral-preference, professional orientation and attention capability. All probands were divided into 6 categories according to gender and degree of MF. Synthesis of MMPI and HBDI results showed significant congruence between the gender-categories and a dominant preference of the HBDI{\crq}s cognitive styles: Masculine men {--} a dominant preference of A-mode (analytical thinking). Feminine men {--} a dominant preference of D-mode (imaginative thinking). Masculine women {--} a dominant preference of B-mode (sequential thinking). Feminine women {--} a dominant preference of C-mode (interpersonal thinking). This findings were in accordance with a professional orientation of the probands (regardless of the field of study they have choosen). Right-brain dominant students (mostly feminine men) were not well-adjusted to a classical passive teaching metods.
