The evaluation of strategies used to reduce maternal mortality in the Fezile Dabi District (Free State Province)

The purpose of this study was to describe and evaluate the strategies used to reduce maternal mortality of the Fezile Dabi District in the Free State Province, South Africa. A quantitative descriptive study was conducted at three hospitals to describe and evaluate the strategies. Data collection was done by using the Maternal and Neonatal Programme Effort Index (MNPI) self-administered questionnaire. The study population comprised of 71 (midwives (52), doctors (13), management-operational managers (6). The overall response rate for the three hospitals was (43.5%). The study indicated that programme efforts for maternal health are similar across the three hospitals, however, there are particular areas in each hospital that need more attention. The ratings of all the strategies generally showed weak to partial effort. The findings revealed general poor performance under the following strategies: postpartum care, training arrangements, information, education, communication and hospital protocols. La qualité des soins obstétricaux en milieu rural malien : une étude exploratoire

Afin d’améliorer la santé maternelle, il est essentiel de mettre en œuvre des interventions qui agissent sur la période intrapartum, interventions qui consistent essentiellement en l’amélioration de l’accès aux soins obstétricaux et néonataux d’urgence (SONU). Néanmoins, plusieurs pays qui se sont tournés vers ce type d’interventions rencontrent d’importants défis au niveau de leur implantation et les taux élevés de mortalité maternelle persistent. Cette étude vise à identifier des caractéristiques de structure et de processus dans les centres de santé communautaire (CSCOM) où des décès maternels ont eu lieu et de comparer ces caractéristiques avec celles de CSCOM où aucun décès maternel n’a eu lieu. Nous avons opté pour une démarche exploratoire comparative des groupes de CSCOM qui diffèrent de par les issues des parturientes qui y ont été traitées ainsi que par leur configuration en termes de ressources humaines. La population à l’étude correspond à des CSCOM de la région de Kayes dans lesquels ont été prises en charge des femmes qui ont fait face à des urgences obstétricales. Les ressources humaines, les ressources matérielles, les ressources en pharmacie et les connaissances des prestataires de soins de ces établissements constituent les principaux objets à l’étude. L’analyse de nos données montre qu’il existe de grandes disparités au niveau des ressources disponibles dans les CSCOM, ressources pourtant nécessaires à une prise en charge adéquate des urgences obstétricales. Bien qu’aucune caractéristique des CSCOM ne puisse être associée aux issues défavorables des femmes qui y ont été traitées, l’accumulation de déficits au niveau des ressources matérielles et humaines ainsi que le faible niveau de connaissances de certaines catégories de personnel laissent penser que la prise en charge des parturientes est déficiente au point de menacer leur survie. / To improve maternal health, it is essential to implement interventions that act on the intrapartum period, interventions that consist essentially in improving access to obstetric and neonatal emergency care (EmOC). Nevertheless, several countries that have turned to such interventions encounter significant challenges in their implementation and the high rates of maternal mortality persist. This study aims to identify characteristics of structure and process in community health centers (CSCOM) where maternal deaths have occurred and to compare these characteristics with those of CSCOM where no maternal deaths have occurred. We opted for an exploratory comparative approach of CSCOM with patients with different outcomes and different human resources configuration. The study population corresponds to CSCOMs of Kayes region in which obstetric emergencies were managed. The main focus of this study is on human resources, availability of equipment, supply and medicine, and on the level of knowledge of health care providers in these health centers. Our data analysis revealed that there are large disparities in resources available at the CSCOM level. Yet, these resources are needed in order to adequately manage obstetric emergencies. Although none of the CSCOM characteristic can be associated with adverse outcomes, the accumulation of deficits in human and material resources as well as the lack of knowledge of certain categories of staff suggests that the poor quality of obstetric care may threaten the parturient survival.

Violência por parceiro íntimo e morbidade materna grave / Intimate partner violence and severe maternal morbidity.

Violência por parceiro íntimo (VPI) e morbidade materna grave constituem-se em importantes agravos à saúde sexual e reprodutiva feminina e representam formas de expressão das desigualdades de gênero. De acordo com os critérios clínicos, laboratoriais e de manejo relativos à morbidade materna grave, adotados pela Organização Mundial de Saúde (OMS) para a definição de condições potencialmente ameaçadoras da vida materna (CPAV), este estudo teve por objetivo analisar a associação entre VPI na gravidez atual e ocorrência de CPAV entre mulheres atendidas em maternidades públicas da Grande São Paulo. Gestantes e puérperas que constituíram a população de estudo (N=446) foram divididas em dois grupos distintos: 1) que desenvolveram CPAV durante o ciclo gravídico puerperal atual, definidas como casos (n= 109); e 2) que não apresentaram qualquer tipo de intercorrência clínica, laboratorial ou de manejo no mesmo ciclo, definidas como controles (n=337). Respeitando-se os preceitos da ética em pesquisa com seres humanos, os casos e os controles foram selecionados por meio de visitas diárias aos locais de estudo entre novembro de 2010 e junho de 2011; próximo da alta hospitalar, entrevistas estruturadas foram conduzidas para investigação retrospectiva de VPI durante a gravidez atual, por meio de questionário adaptado do Estudo Multipaíses da OMS sobre Saúde da Mulher e Violência Doméstica. A relação entre a variável resposta (CPAV), a variável de exposição (VPI) e demais variáveis independentes, foi avaliada por meio de proporções, testes qui-quadrado ou exato de Fisher e pelo modelo de regressão logística múltiplo. Identificou-se prevalência de \"near miss\" materno de 5,62/1.000NV, ou seja, 0,56% e, Razão de Resultados Maternos Severos de 6,37/1.000NV. Considerando-se a tipificação da violência, observou-se prevalência de 12,7% de violência psicológica; 7,6% de violência física e 1,6% de violência sexual durante a gestação atual entre casos e controles. A despeito da ausência de significância estatística entre a exposição à VPI na gestação atual, relatada por 13% da amostra, e a ocorrência do desfecho CPAV, verificou-se que tanto as gestantes expostas à VPI quanto as mulheres que desenvolveram CPAV, apresentam fatores associados às condições sociodemográficas e reprodutivas desfavoráveis. Concluiu-se sobre a importância do monitoramento de casos CPAV que, assim como o rastreamento rotineiro da VPI entre gestantes, deve ser incluído no processo de trabalho dos enfermeiros. Isto é importante para promover a qualificação da atenção à saúde materna. / Intimate partner violence (IPV) and severe maternal morbidity represents importants women´s sexual and reproductive health issues, as well as an expression means of gender inequalities. According to clinical, laboratory-based and management-based criteria concerning severe acute maternal morbidity adopted by World Health Organization (WHO) to define potentially life-threatening maternal condition, this study aims to analyze the association between IPV in current pregnancy and potentially life-threatening condition among women cared in Great São Paulo public hospitals. Pregnant and postpartum women who constituted the study population (N=446) were categorized into two distinct groups: 1) who developed potentially life- threatening condition during current pregnancy, childbirth or postpartum, called cases (n= 109); and 2) women who did not attended to any clinical, or laboratory- based and management-based criteria, called controls (n=337). According to the ethical standards of human research, cases and controls were selected through daily visits at study settings during November 2010 and June 2011; near discharge from hospital, the structured interviews were conducted to investigate the prospective relationship among IPV during current pregnancy through a questionnaire adapted from Who Multi-country Study on Women\'s Health and Domestic Violence Against Women. The relationship between response variable (potentially life-threatening maternal condition), the exposition variable (IPV) and the others independents variables was assessed by proportions, chi square test or Fisher\'s exact test and multiple regression logistic. The maternal near miss prevalence identified was 5,62/1.000 live births, or 0,56%. The Severe Maternal Outcome Ratio was 6,37/1.000 live births. Considering the violence´s types, the study have found 12,7% of psychological, 7,6% of physical and 1,6% of sexual injuries during current pregnancy among cases and controls. Despite the absence of statistical significance between VPI exposition in current pregnancy, which was related by 13% of the total sample, and the potentially life-threatening maternal outcome, it was verified that both pregnant women exposed to IPV as those who developed potentially life-threatening conditions, showed factors associated with sociodemographic and reproductive unfavorable conditions. In conclusion, it is recommended potentially life-threatening condition continued audit, as well as the IPV routine screening among pregnant women should be included in the nurse´s working process. It`s important to improve the quality of maternal care health system.

Maternal mortality, in turn, is defined as the death of women of childbearing age (15 - 49 years) during pregnancy or within 42 days after delivery. Objective: To analyze maternal mortality in the state of Bahia in 2010, according to differences in race/sk in color. Methods: Descriptive and ecological study of multiple groups, of the maternal mortality in the 49 most populous cities in Bahia in 2010, according to race / skin color. Secondary data available in Health information systems (SIS), the mortality information system (MIS); Information System on Live Births (SINASC); Hospital information system (HIS) of the department of the SUS (SUS Date) and socioeconomic data from the Brazilian Institute of Geography and Statistics (IBGE) were used. To analyze the association between the independent and dependent variables, we used the logistic regression model of Poisson through of software STATA version 10 and R version 2.15.2. In spatial data was used ARCGIS application 10.0 Results: In 2010, Bahia, 209 444 hosp italizations of women of childbearing age were recorded. In the 49 municipalities with the highest population density 118,773 admissions occurred. Of these, 57,173 (48.1%) occurred in the black population, 6,938 (5.8%) in the white population; 54,551 (45.9%) without race / skin color information. The total number of maternal deaths in MIS aged 15- 49 years accounted for 154 deaths, with 85 of these deaths occurred in the 49 municipalities (55.2%). The Maternal Mortality Ratio (MMR) in Bahia was 72.5 / 100,000 live births (LB) as in the cities studied was 71.9 / 100,000 (LB). In the bivariate and multivariate analyzes of association was observed that the number of maternal deaths was positively associated with the proportion of black population, since, as the proportion of blacks increased 5%, the risk of maternal death increased to 25.2 % (p <0.0278) and 26.6% (p <0.0366), respectively. In the bivariate analysis of HDI and Gini index also associated, but were not statistically significant. The multivariate analysis showed that there was increased risk of death even when adjusted for variable water rate (WR), and this result was statistically significant. Conclusion: The analyzed data revealed higher incidence and prevalence of maternal mortality in the black population. Underreporting related to completing the race / skin color variable records hinders a more precise analysis of morbidity and mortality and represents a gap due to the effectiveness of health interventions for vulnerable groups of women. The red uction of maternal morbidity and mortality should be a universal and emergency commitment. Investments in humanization and universalization of quality maternal health care constitute a significant affirmative action against exclusion and social injustice. / Introdu??o : De acordo com a organiza??o Mundial de Sa?de, a morbidade materna ? compreendida como a ocorr?ncia de complica??o durante a gesta??o, parto, ou puerp?rio que, se n?o tratadas podem complicar e levar ? morte. Para an?lise da associa??o entre as vari?veis independentes e dependentes utilizou- se o modelo de regress?o log?stica de Poisson atrav?s do software STATA vers?o 10 e R vers?o 2.15.2. Na espacializa??o dos dados foi usado aplicativo ARCGIS 10.0 Resultados : Em 2010, na Bahia, foram registrados 209.444 internamentos de mulheres em idade f?rtil. Nos 49 munic?pios com maior densidade populacional ocorreram 118.773 internamentos. Destes, 57.173 (48,1%) ocorreram na popula??o negra, 6.938 (5,8%) na popula??o branca; 54.551(45,9%) sem informa??o da ra?a/cor da pele. O total de ?bitos maternos registrados no SIM na faixa et?ria de 15 - 49 anos correspondeu a 154 ?bitos, sendo que 85 destes ?bitos ocorreram nos 49 munic?pios (55,2%). A Raz?o de Mortalidade Materna (RMM) na Bahia foi 72,5/100.000 Nascidos vivos (NV) enquanto nos munic?pios estudados foi 71,9/100.000(NV). Nas an?lises de associa??o bivariada e multivariada observou- se que o n?mero de mortes maternas associou- se positivamente com propor??o de popula??o negra, j? que, ? medida que a propor??o de popula??o negra aumentou 5%, o risco de morte materna aumentou para a 25.2% (p < 0.0278) e 26.6%(p<0.0366) respectivamente. Na an?lise bivariada ?ndice de Gini e IDH tamb?m se associaram, mas n?o foram estatisticamente significantes. Na an?lise multivariada foi observado que houve aumento no risco de morte mesmo quando ajustado pela vari?vel taxa de ?gua (TXAG), sendo esse resultado es tatisticamente significante. Conclus?o : Os dados analisados revelaram maior incid?ncia e preval?ncia de morbimortalidade materna na popula??o negra. A subnotifica??o de registros relacionados ao preenchimento da vari?vel ra?a/cor da pele dificulta uma an?lise mais precisa da morbimortalidade e representa uma lacuna face ? efetividade das a??es em sa?de para grupos de mulheres mais vulner?veis. A redu??o da morbidade e mortalidade materna deve ser um compromisso universal e emergencial. Investimentos em humaniza??o e universaliza??o da qualidade da assist?ncia ? sa?de materna constituem uma significativa a??o afirmativa contra a exclus?o e a injusti?a social.

Métodos: foram realizadas duas abordagens: a primeira, uma análise secundária do banco de dados da Pesquisa Nacional de Demografia e Saúde (PNDS) brasileira de 2006, através de um questionário para avaliar a ocorrência de complicações maternas. Utilizou-se a definição pragmática em que qualquer mulher relatando a ocorrência de eclâmpsia, histerectomia, transfusão de sangue ou internação em unidade de terapia intensiva era considerada como um caso de near miss. Associações entre os extremos da idade reprodutiva (adolescentes e mulheres com 35 a 49 anos) e morbidade materna grave foram avaliados. Para a análise foram realizadas proporções e razões das complicações e intervenções relacionadas definidas como near miss e o risco estimado para morbidade materna. A segunda abordagem foi à análise secundária do estudo de corte transversal multicêntrica, implementado em 27 unidades obstétricas de referência nas cinco regiões geográficas do Brasil e criado para identificação dos casos de morbidade materna grave. Realizou-se vigilância prospectiva das mulheres internadas durante o período de 12 meses. Foram analisados os dados relativos à ocorrência de MM, NM e CPAV na faixa etária da adolescência (10 a 19 anos) e nas gestantes mais velhas (com 35 anos ou mais) usando como referencia mulheres entre 20 e 34 anos. Calcularam-se as razões de prevalência (RP) e os intervalos de confiança a 95%, ajustados por conglomerados para as variáveis maternas epidemiológicas, obstétricas, clínicas e de demoras. Realizou-se análise múltipla por regressão de Poisson. Resultados: Na primeira abordagem foram encontradas 6833 gestações sendo, 73,7% em mulheres entre 20 e 34 anos, 17,9% em mulheres com 35 anos ou mais e 8,4% em adolescentes. A única variável diferente entre os grupos foi à necessidade de transfusão sanguínea, mais prevalente entre os controles. Houve tendência de aumento nas razões de NM com o aumento da idade materna. O único fator de risco identificado foi menor escolaridade entre as mulheres com 35 anos ou mais. Na segunda abordagem foram encontrados 17% de adolescentes e 17% de mulheres com 35 anos ou mais. As prevalências de NM e MM foram maiores entre as mulheres com 35 anos ou mais. NM e MM aumentaram com o aumento da idade da mulher, mas também foram muito elevados no extremo inferior da idade, ou seja, entre as adolescentes com 10 a 14 anos. Na análise multivariada, a adolescência não foi um fator de risco independente, enquanto a idade de 35 anos ou mais o foi (RP 1.25; 1.07-1.45). Conclusões: NM e MM aumentaram com o aumento da idade maternal, com exceção do grupo de adolescentes entre 10 e 14 anos, que também apresentou maiores riscos de NM e MM / Abstract: Objective: to assess the prevalence and factors associated with severe maternal morbidity (SMM), considering the definitions: potentially life-threatening conditions (PTLC), maternal near miss (MNM) and maternal mortality (MM) in women at the extremes of reproductive age. Methods: Two methodological approaches were programmed: the first was a secondary analysis of the 2006 Brazilian Demographic Health Survey (DHS) database that was conducted through a previously validated questionnaire to evaluate the occurrence of maternal complications. We used a pragmatic definition in which any woman reporting the occurrence of eclampsia, hysterectomy, blood transfusion, or hospitalization in the intensive care unit was considered as a case of near miss. Associations between the extremes of reproductive age (adolescents and women aged 35 to 49 years) and SMM were assessed. For this analysis proportions and ratios related to complications and interventions defined as near miss and the estimated risk for maternal morbidity were performed. The second approach was a secondary analysis of a cross-sectional multicenter study, implemented in 27 referral obstetric units in all five geographic regions of Brazil, created to identify cases of SMM, using prospective surveillance of hospitalized women during twelve months. We analyzed the data on the occurrence of MM, MNM and PTLC in teenagers (10 to 19 years) and in older pregnant women (aged 35 or more) using as reference women between 20 and 34 years. We calculated prevalence ratios (PR) with confidence intervals of 95% adjusted for conglomerates. Maternal variables, epidemiological, obstetric, and clinical and delays were considered. Statistical Analysis was performed by multiple Poisson regression. Results: In the first approach 6833 pregnancies were considered, 73.7% in women aged 20-34 years, 17.9% in women 35 years or older and 8.4% in adolescents. The only difference between groups was the need for blood transfusion, more prevalent among controls. There was a tendency of increase in the ratios of MNM with increasing maternal age. The only risk factor identified was lower education among women aged 35 years or more. In the second approach we found 17% of adolescents and also 17% of women aged 35 or more. The prevalence of MNM and MM was higher among women with 35 years or more. MNM and MM increased with increasing age, but were also very high at the lowest end of the age, among adolescents aged 10-14 years. In multivariate analysis. Adolescence was not an independent risk factor while age of 35 years or more was (PR 1.25; CI 1:07 to 1:45). As mais recentes estimativas sugerem que o número de mulheres que morrem de complicações durante a gestação e parto caiu 47% de 1990 a 2010. Para alcançar a redução planejada de 75% até 2015, a taxa anual deveria cair em 5,5%. Porém, um pouco além da metade do que seria necessário foi conseguido. Para que as metas sejam atingidas, intervenções efetivas são identificadas como necessárias. Além de melhorias com investimentos estruturais locais, uma das bases para a organização de ações efetivas é a obtenção de dados fidedignos e o desenvolvimento de sistemas de informação em saúde. Até um passado recente, não havia padronização de conceitos para morbidade materna grave. Em 2009, condições ameaçadoras da vida foram definidas pela Organização Mundial da Saúde (OMS). Como resultado, critérios clínicos, laboratoriais e de manejo específicos orientam a identificação de casos de near miss. A criação de uma rede de vigilância no Brasil utilizando os novos critérios de near miss poderia trazer um panorama desta condição em um país de média renda e população heterogênea. Com tais informações, novas estratégias para melhoria da assistência à saúde materna e perinatal poderiam ser desenvolvidas, com potencial redução real de mortes. Objetivos: Avaliar a homogeneidade amostral dos casos incluídos na Rede, validar os critérios de near miss da OMS, desenvolver um modelo de predição de mortalidade baseado na gravidade dos casos e, a partir deste modelo, avaliar o desempenho dos centros da Rede na prestação de cuidados obstétricos. Métodos: A Rede Nacional de Vigilância de Morbidade Materna Grave foi um estudo de corte transversal multicêntrico implantado em 27 hospitais entre todas as regiões do país, com coleta prospectiva de dados pelo período de um ano, utilizando os novos critérios de near miss da OMS. A partir de 9555 casos incluídos no período, foram calculados os coeficientes de correlação intraclasse (ICC) e efeitos do desenho por cluster das variáveis do estudo. Também, testes de acurácia diagnóstica foram utilizados para validar dos critérios da OMS. Um modelo de regressão logística bivariada foi utilizado para avaliar a correlação entre os critérios de near miss e variáveis distais na ocorrência de morte e desenvolver uma ferramenta para predição de mortalidade, o Maternal Severity Index (MSI). A partir deste modelo, foi analisada a razão entre as mortes observadas e aquelas esperadas de acordo com a gravidade dos casos e avaliados os níveis de desempenhos dos centros na prestação de cuidado obstétrico. Conclusão: Os ICC para as variáveis de desfecho da Rede são considerados pequenos, o que indica adequada heterogeneidade amostral. Seus valores podem ser utilizados para o cálculo do tamanho amostral de estudos futuros na área. O uso dos critérios de near miss da OMS para identificação de casos de morbidade materna grave foi validado. O índice de gravidade materna (MSI) pode ser utilizado como ferramenta para predição de mortalidade e avaliação de desempenho e adequação de cuidado em instituições que prestem atendimento a mulheres com condições ameaçadoras a vida / Abstract: Introduction: Maternal health is one of the focuses of the United Nations' Millennium Development Goals for 2015. The most recent estimates suggest that the number of women who died from complications during pregnancy and childbirth fell 47% from 1990 to 2010. To achieve the planned reduction of 75% by 2015, the annual rate should fall by 5.5%. However, a little more than half of what is required has been obtained. To achieve the goals, effective interventions are identified as necessary. In addition to improvements in local structural investments, one of the bases for the organization of effective actions is obtaning reliable data and the development of health information systems. Until recently, there was no standardazed concept for severe maternal morbidity. In 2009, the World Health Organization (WHO) defined life-threatening conditions. As a result, specific clinical, laboratory and management criteria guide the identification of near miss cases. The development of a surveillance network in Brazil using the new criteria for maternal near miss could bring an overview of this condition in a middle-income country with heterogeneous population. With such information, new strategies to improve maternal and perinatal care could be developed, with potential real reduction of deaths. Objectives: To evaluate the homogeneity of the sample included in the network, to validate the WHO near miss criteria, to develop a predictive model of mortality based on severity of cases and, from this model, to evaluate the performance of the Network facilities in providing obstetric care. Methods: The Brazilian Network for Surveillance of Severe Maternal Morbidity was a multicenter cross-sectional study implemented in 27 hospitals from all regions of the country, with prospective data collection for a one year period, using the new WHO maternal near miss criteria. From 9555 cases included in the period, the intraclass correlation coefficients (ICC) and cluster design effects for the variables of the study were calculated. Also, diagnostic accuracy tests were used to validate the WHO criteria. A bivariate logistic regression model was used to evaluate the correlation between the near miss criteria and distal variables in the occurrence of death and to develop a tool for prediction of mortality, the Maternal Severity Index (MSI). From this model, the ratio between the number of observed and expected deaths was analyzed according to the severity of cases and the performance levels of the centers in providing obstetric care was assessed. Conclusion: The ICC for the outcome variables of the network are considered small, what indicates adequate sample heterogeneity. Their values can be used to calculate the sample size of further studies in the area. The use WHO maternal near miss criteria to identify cases of severe maternal morbidity has been validated. Reproducing Injustice: The Unusual Case for Latinx Birthing Parent Mortality and Its Sociological Factors: Literature Review

For birthing parents, the quality of care falls flat in the United States relative to other developed countries as rates of pregnancy-related deaths (PRD) continue to rise. California has in recent years made extraordinary progress in decreasing birthing parent mortality across the board. Yet, health disparities remain between race/ethnicities as Black birthing parents die at three to four times the rate of white birthing parents. In comparison to white Americans marginalized ethnic/racial groups in the United States have less access to quality care, experience and receive lower quality of health care and have less access to quality care, with few exceptions. Intersecting factors such as education, socioeconomic status and acculturation are investigated. Implicit bias, or racism is often overlooked within the medical professions that has real implications on the above-mentioned sociological factors and in turn birthing mortality rates. Institutional and grassroot methods to bring attention to these factors should be referenced for a more intentional approach to solving this social problem historically engrained in our medical institutions.

Maternal mortality in Sweden

Every year about half a million women die from complications of pregnancy, parturition and puerperium, most of which are preventable. The purpose of this thesis was to chart the distribution and decline in maternal mortality in Sweden between 1751 and 1980, and furthermore to characterize positive (predisposing) factors and negative (protective) factors of maternal mortality. Maternal mortality declined from 900 to 6.6 per 100,000 live births in these 230 years. Maternal deaths accounted for 10070 of all female deaths in the reproductive ages between 1781 and 1785, but only 0.2.0/0 between 1976 and 1980. However, in the 19th century 40-450/0 of the female deaths in the most active childbearing ages were maternal deaths. The children left motherless had an extremely high mortality. Indirect maternal deaths and puerperal sepsis accounted for the bulk of maternal deaths in the rural areas. Only a minority of maternal deaths occurred in lying-in hospitals. Midwifery services in rural areas and antiseptic techniques were most effective in preventing maternal deaths during the late 19th century. The changing distribution ofage and parity amongst the parturients had a definite impact on the mortality decline, enhanced by time, contributing to 500/0 of the mortality decline over the last 15 years. The expontential decline of cause-specific mortality and case fatality rates during the last 40 years is furthermore explained by the emergence ofmodern medicine - antibiotics, antenatal and obstetric care. The earlier serious problem of illegal abortions was eradicated by legislation and changes in hospital practice. The maternal mortality decline has levelled out during the 1970s, the relative importance of embolism as a cause of death is increasing. Advanced age and intercurrent disease are the most difficult risk factors to overcome. To conclude, this study indicates that the reason why maternal mortality has declined faster than otherhealth indices is that the major part of the maternal deaths can be prevented by medical technology, including family planning, antenatal and obstetric care. This experience should be of interest to developing countries where high rates of maternal mortality prevails.

La qualité des soins obstétricaux en milieu rural malien : une étude exploratoire

Afin d’améliorer la santé maternelle, il est essentiel de mettre en œuvre des interventions qui agissent sur la période intrapartum, interventions qui consistent essentiellement en l’amélioration de l’accès aux soins obstétricaux et néonataux d’urgence (SONU). Néanmoins, plusieurs pays qui se sont tournés vers ce type d’interventions rencontrent d’importants défis au niveau de leur implantation et les taux élevés de mortalité maternelle persistent. Cette étude vise à identifier des caractéristiques de structure et de processus dans les centres de santé communautaire (CSCOM) où des décès maternels ont eu lieu et de comparer ces caractéristiques avec celles de CSCOM où aucun décès maternel n’a eu lieu. Nous avons opté pour une démarche exploratoire comparative des groupes de CSCOM qui diffèrent de par les issues des parturientes qui y ont été traitées ainsi que par leur configuration en termes de ressources humaines. La population à l’étude correspond à des CSCOM de la région de Kayes dans lesquels ont été prises en charge des femmes qui ont fait face à des urgences obstétricales. Les ressources humaines, les ressources matérielles, les ressources en pharmacie et les connaissances des prestataires de soins de ces établissements constituent les principaux objets à l’étude. L’analyse de nos données montre qu’il existe de grandes disparités au niveau des ressources disponibles dans les CSCOM, ressources pourtant nécessaires à une prise en charge adéquate des urgences obstétricales. Bien qu’aucune caractéristique des CSCOM ne puisse être associée aux issues défavorables des femmes qui y ont été traitées, l’accumulation de déficits au niveau des ressources matérielles et humaines ainsi que le faible niveau de connaissances de certaines catégories de personnel laissent penser que la prise en charge des parturientes est déficiente au point de menacer leur survie. / To improve maternal health, it is essential to implement interventions that act on the intrapartum period, interventions that consist essentially in improving access to obstetric and neonatal emergency care (EmOC). Nevertheless, several countries that have turned to such interventions encounter significant challenges in their implementation and the high rates of maternal mortality persist. This study aims to identify characteristics of structure and process in community health centers (CSCOM) where maternal deaths have occurred and to compare these characteristics with those of CSCOM where no maternal deaths have occurred. We opted for an exploratory comparative approach of CSCOM with patients with different outcomes and different human resources configuration. The study population corresponds to CSCOMs of Kayes region in which obstetric emergencies were managed. The main focus of this study is on human resources, availability of equipment, supply and medicine, and on the level of knowledge of health care providers in these health centers. Our data analysis revealed that there are large disparities in resources available at the CSCOM level. Yet, these resources are needed in order to adequately manage obstetric emergencies. Although none of the CSCOM characteristic can be associated with adverse outcomes, the accumulation of deficits in human and material resources as well as the lack of knowledge of certain categories of staff suggests that the poor quality of obstetric care may threaten the parturient survival.

THE ROLE OF FAMILY PLANNING IN REDUCING MATERNAL MORTALITY IN BANGLADESH

The main objective of the study is to analyze the role of family planning program in reducing maternal mortality in Bangladesh. A conceptual framework has been developed in which family planning is shown to be integrated in reducing maternal mortality. This study found that the risk factors of maternal mortality e.g. unwanted pregnancy, high parity, and early and old age at child birth still prevail in Bangladesh. It is hypothesized that the prevalence of these factors can be substantially reduced by a proper practice of family planning. There is a high level of unmet need for family planning Bangladesh, and its removal will substantially help in reducing maternal mortality in the country. The risk factors of maternal mortality are strongly associated with lack of family planning practice and other socio-economic and demographic background characteristics of women. By using data from the Bangladesh Demographic and Health Survey (BDHS) of 2004 and the Bangladesh Maternal Health and Maternal Mortality Survey (BMMS) of 2001 the study has analyzed the relationship of the risk factors of maternal mortality, namely wantedness of pregnancy, age at child birth, parity and birth interval with various socio-demographic factors. The analysis has shown that use status of family planning is influenced by the risk factors of maternal mortality. Wantedness of pregnancy has been found to be significantly related with age at birth, parity and birth interval. It has been also found that the risk factors of maternal mortality also affect on antenatal care. The study has identified some policy implications regarding family planning and maternal mortality, and has made appropriate recommendations. One of the major aspects of the strategies to reduce maternal mortality through family planning is to provide family planning services to all women, regardless of any group affiliation. Fulfilment of unmet for family planning has been recommended as an important strategy to reduce maternal mortality in the country. It addition, it is also recommended to raise the age at marriage and child birth, to space births and to limit family size by empowering women through education.
