Effects of Provider Education on Documentation Compliance in the O.R.Kingdon, Brenda 01 January 2009 (has links)
Knowledge of The Joint Commission's National Patient Safety Goals and an effective provider cooperative practice involving communication and teamwork are essential for the delivery of safe and compliant patient care in the surgical setting. The purpose of this study was to assess the impact of an educational intervention for physicians and nurses designed to increase documentation of compliance with national patient safety standards. As events of noncompliance have impacted patient safety at the hospital where this project was conducted, measures were needed to assess barriers to compliance with standards of practice and to focus educational session plans on identified knowledge-base needs.
The goal of this project involved bringing all surgical team members together for educational sessions on safety standards. Pre-intervention and post-intervention assessments of knowledge were administered to study participants. Additionally, random chart documentation audits were conducted before and after the intervention to assess the effectiveness of the education sessions on documentation compliance with the targeted standards.
Outcomes of this study included improved knowledge of, and compliance with, national patient safety goals. Results may improve safe patient care at this hospital, reduce costs, and create mutual respect and teamwork, all contributing to the successful achievement of the organization's quality improvement goals.

Security of electronic personal health information in a public hospital in South AfricaChuma, Kabelo Given 01 1900 (has links)
The adoption of digital health technologies has dramatically changed the healthcare sector landscape and thus generates new opportunities to collect, capture, store, access and retrieve electronic personal health information (ePHI). With the introduction of digital health technologies and the digitisation of health data, an increasing number of hospitals and peripheral health facilities across the globe are transitioning from a paper-based environment to an electronic or paper-light environment. However, the growing use of digital health technologies within healthcare facilities has caused ePHI to be exposed to a variety of threats such as cyber security threats, human-related threats, technological threats and environmental threats. These threats have the potential to cause harm to hospital systems and severely compromise the integrity and confidentiality of ePHI. Because of the growing number of security threats, many hospitals, both private and public, are struggling to secure ePHI due to a lack of robust data security plans, systems and security control measures. The purpose of this study was to explore the security of electronic personal health information in a public hospital in South Africa. The study was underpinned by the interpretivism paradigm with qualitative data collected through semi-structured interviews with purposively selected IT technicians, network controllers’, administrative clerks and records management clerks, and triangulated with document and system analysis. Audio-recorded interviews were transcribed verbatim. Data was coded and analysed using ATLAS.ti, version 8 software, to generate themes and codes within the data, from which findings were derived. The key results revealed that the public hospital is witnessing a deluge of sophisticated cyber threats such as worm viruses, Trojan horses and shortcut viruses. This is compounded by technological threats such as power and system failure, network connection failure, obsolete computers and operating systems, and outdated hospital systems. However, defensive security measures such as data encryption, windows firewall, antivirus software and security audit log system exist in the public hospital for securing and protecting ePHI against threats and breaches. The study recommended the need to implement Intrusion Protection System (IPS), and constantly update the Windows firewall and antivirus program to protect hospital computers and networks against newly released viruses and other malicious codes. In addition to the use of password and username to control access to ePHI in the public hospital, the study recommends that the hospital should put in place authentication mechanisms such as biometric system and Radio Frequency Identification (RFID) system restrict access to ePHI, as well as to upgrade hospital computers and the Patient Administration and Billing (PAAB) System. In the absence of security policy, there is a need for the hospital to put in place a clear written security policy aimed at protecting ePHI. The study concluded that healthcare organisations should upgrade the security of their information systems to protect ePHI stored in databases against unauthorised access, malicious codes and other cyber-attacks. / Information Science / M. Long-Term Survival and Prognostic Factors in Patients with Acute Decompensated Heart Failure According to Ejection Fraction Findings: A Population-Based Perspective: A Master ThesisColes, Andrew H. 18 August 2014 (has links)
Limited data exists describing the long-term prognosis of patients with acute decompensated heart failure (ADHF) further stratified according to currently recommended ejection fraction (EF) findings. In addition, little is known about the magnitude of, and factors associated with, long-term prognosis for these patients. Based on previously validated and clinically relevant criteria, we defined HF-REF as patients with an EF value ≤40%, HF-PEF was defined as an EF value > 50%, and HF-BREF was defined as patients with an EF value during their index hospitalization between 41 and 49%. The hospital medical records of residents of the Worcester (MA) metropolitan area who were discharged after ADHF from all 11 medical centers in central Massachusetts during the 5 study years of 1995, 2000, 2002, 2004, and 2006 were reviewed. Follow-up was completed through 2011 for all patient cohorts. The average age of this population was 75 years, the majority was white, and 44% were men. Patients with HF-PEF experienced higher post discharge survival rates than patients with either HF-REF or HF-BREF at 1, 2, and 5-years after discharge. Advanced age and lower estimated glomerular filtration rate findings at the time of hospital admission were important predictors of 1-year death rates, irrespective of EF findings. Previously diagnosed chronic obstructive pulmonary disease, chronic kidney disease, and atrial fibrillation were associated with a poor prognosis in patients with PEF and REF whereas a history of diabetes was an important prognostic factor for patients with REF and BREF. In conclusion, although improvements in 1-year post-discharge survival were observed for patients in each of the 3 EF groups examined to varying degrees, the post- 7 discharge prognosis of all patients with ADHF remains guarded. In addition, we observed differences in several prognostic factors between patients with ADHF with varying EF findings, which have implications for more refined treatment and surveillance plans for these patients.

Prescribing cotrimoxazole prophylactic therapy (CPT) before and after an electronic medical record system implementation in two selected hospitals in MalawiGadabu, Oliver Jintha 11 1900 (has links)
Opportunistic infections (OIs) have been identified as a leading cause of poor outcomes in the ARV therapy (ART) programme. In order to reduce OIs, the Malawi, MoH introduced routine prescription of cotrimoxazole preventive therapy (CPT) in 2005. The MoH also started scaling up a point-of-care electronic medical record (EMR) system in 2007 to improve monitoring and evaluation.
This study had the following objectives: i) to quantify prescription of CPT before and after implementing EMR; ii) to compare the difference in CPT prescription before and after implementing EMR.
A historically controlled study design was used to compare CPT prescriptions one year before, and one year after implementation of the EMR at two health facilities.
The data indicated that there was a significant (P <0.001) decrease in CPT prescribing at one health facility and a significant increase in CPT prescription at another. / Health Studies / M.A. (Public Health)

In the last decade, the deployment of Electronic Health Records has increased tremendously in many developed countries. This increasing trend intensifies the need for developing countries like South Africa to implement electronic health record systems in state owned hospitals to facilitate e-referral processes to improve health care delivery.
The aim of this research was to investigate the current process of patient record keeping, management, and the referral process of patients within the same hospital and to other hospitals and based on the findings compile an Electronic Health Record (EHR) framework to facilitate e- referral processes.
This research study was based on a qualitative case study approach. A multiple data collection technique was used which included group interviews, questionnaires, document analysis and informal discussions with the hospital workers. Data were analysed by categorization and thematic approach.
The findings obtained from state hospitals indicated that there is no EHR system which accommodates patient health record systems to facilitate e-referral processes. These findings led to a compilation of the Limpopo Electronic Health Record System (LEHRS) to aid e-referral processes in state hospitals. The increasing need for accurate, reliable, available and accessible EHR will be addressed by the implementation of LEHRS as information will be stored in a central database in a useable format and will be easily accessed. / Computing / M. Understanding Perspectives of Risk AwarenessPark, Byunguk Randon 01 August 2014 (has links)
Research in risk awareness has been relatively neglected in the health informatics literature, which tends largely to examine project managers’ perspectives of risk awareness; very few studies explicitly address the perspectives held by senior executives such as directors. Another limitation evident in the current risk literature is that studies are often based on American data and/or they are restricted to American culture. Both factors highlight the need to examine how senior executives (i.e., directors) who oversee or direct eHealth projects in Canada perceive risk awareness. This research explores and discusses the perspectives of risk awareness (i.e., identification, analysis, and prioritization) held by directors and project managers who implement Canadian eHealth projects. Semi-structured interviews with nine directors and project managers uncovered six key distinctions in these two groups’ awareness of risk. First, all project managers valued transparency over anonymity, whereas directors believed that an anonymous reporting system for communicating risks had merit. Secondly, most directors emphasized the importance of evidence-based planning and decision making when balancing risks and opportunities, an aspect none of the project managers voiced. Thirdly, while project managers noted that the level of risk tolerance may evolve from being risk-averse to risk-neutral, directors believed that risk tolerance evolved toward risk-seeking. Directors also noted the importance of employing risk officers, a view that was not shared by project managers. Directors also believed the risk of too little end-user engagement and change management was the most important risk, whereas project managers ranked it as the least important. Finally, when directors and project managers were asked to identify and define the root cause(s) of eHealth risks, directors identified the complexity of health care industry, while project managers attributed it to political pressure and a lack of resources where eHealth projects are concerned. This research proposes that the varied perspectives of risk awareness held by directors and project managers must be considered and integrated to properly align expectations and build partnerships for successful eHealth project outcomes. Health practitioners collect health data on a daily basis at health facility levels in order to monitor and evaluate the performance of priority national health programmes (District Health Plan 2012:6). Routine data quality for health programmes monitoring need a collective intervention to ensure clear understanding for what data to be collected at primary health care setting. The aim of the study is to explore the understanding of routine health data, determine the use of routine data and feedback mechanism at primary health care clinic setting. Quantitative descriptive research design was used to answer the research question on this research study. Structured data collection questionnaire was used for the study to accomplish the research purpose and reach the study objectives. A total of 400 participants was sampled, and 247 responded. One of the findings was that the understanding of routine health data by Health Practitioners was at 82.6% (% = f/n*100, f= 3242 and n= 3926). / Health Studies
