O eixo central da análise consiste em demonstrar como os pressupostos iniciais deste campo tinham uma vertente subversiva em relação a prática médica. Fundamentalmente, questionavam-se os padrões universalizantes da ciência nos quais a medicina moderna se estruturou. Havia o objetivo de refundá-la, a partir de bases que não fosse as ditadas pela racionalidade cientifica moderna. A inserção da Psicologia Médica na UERJ retrata uma trajetória marcada por dificuldades e lutas políticas, que implicaram em uma gradual modificação de seu propósito de origem. Perfil psicológico dos pacientes com distúrbios funcionais colorretoanais: avaliação quantitativa e qualitativa / Psychological profile of patients with colorectal funcional disorders: a quantitative and qualitative approach

Introdução: Os distúrbios funcionais colorretoanais, frequentemente representados por constipação intestinal, incontinência anal e dor anorretal apresentam alta prevalência na população geral. De etiologia multifatorial onde estão envolvidas causas orgânicas, funcionais, culturais e psicológicas. Dados bibliográficos apontam que a depressão, a ansiedade e o estresse estão entre os fatores psicológicos mais presentes nesse grupo de pacientes. Representam um desafio para abordagem gerando impacto significativo na qualidade de vida. Objetivo: Este estudo prospectivo teve como objetivo avaliar o perfil de psicológico de pacientes com distúrbios funcionais colorretais: constipação intestinal crônica (CIC), incontinência anal (IA) e dor anal (DA), utilizando uma versão reduzida, mais aplicáveis do MMPI, o MMPI- IRF (Minnesota Multiphafic Personality Inventory Improved Readability Form). Métodos: Um total de 90 pacientes, sendo 30 de cada grupo sintomático, CIC, AI e DA, respectivamente, e 60 indivíduos controle foram avaliados. Todos os grupos foram igualmente distribuídos por sexo. Os dados foram obtidos através de semi-dirigidas e MMPI-IRF, que consiste de 167 propostas divididas em quatro escalas de validade (\"não posso dizer \", mentira, defensividade) e 10 escalas clínicas (hipocondria, depressão, histeria, desvio- psicopático, masculinidade e feminilidade , paranóia, psicastenia, esquizofrenia, hipomania e introversão-extroversão social). O estudo estatístico incluiu o teste ANOVA, Belferroni e exato de Fisher sendo considerados significativos quando os calores de p<0,05. Resultados: A média de idade os valores foram 48, 52, 49 e 43 anos para o CIC, IA, DA e grupos de controle, respectivamente. O IA se destacou por apresentar menor escolaridade 36% nao concluíram o 1° grau. Quanto aos antecedentes psiquiátricos DA obteve maior porcentagem de pacientes que já estiveram em tratamento psiquiátrico (43%). O início da depressão precedeu o início da queixa funcionais em 36% dos pacientes do CIC, mas em apenas 20% e 23%, respectivamente, IA e pacientes DA. Os escores de depressão, hipocondria e histeria escalas foram significativamente maiores em todos os grupos de pacientes quando comparados aos indivíduos do grupo controle. Conclusão: As elevações nas escalas hipocondria, depressão e histeria são indicativos que os sintomas funcionais colorretoanias são mecanismos de defesa que atuam protegendo os indivíduos da ansiedade e da depressão excessivos. Breast Cancer, Mana'olana/Hope, and the Experience of Native Hawaiian Women

Breast cancer is one of the leading causes of morbidity and mortality among women. A diagnosis of cancer is a stressful event that requires an individual to adapt to new stressors. The purpose of this qualitative study was to better understand the perceptions of mana'olana/hope and living with breast cancer among Native Hawaiian women. The conceptual framework of this phenomenological study was positive psychology. Data collection included in-depth interviews with 5 Native Hawaiian women who had been diagnosed with breast cancer. Data coding and analysis resulted in identification of 8 themes. The themes included: (a) mana'olana /hope is the essence of my being, (b) family strengthens me and gave me mana'olana/hope, (c) my relationship with God and Jesus promoted mana'olana/hope in me, (d) my religious affiliation promoted mana'olana/hope in me, (e) the cancer support group promoted mana'olana/hope in me, (f) the cancer treatment team promoted mana'olana/hope in me, (g) treatment options; Allopathic, Osteopathic, Naturopathic, Alternative medicine, and herbal remedies promoted mana'olana/hope in me, and (h) nature's beauty and the arts promoted mana'olana/hope in me. Results may be used by health psychologists, cancer treatment practitioners, and the field of biobehavioral oncology to support and improve the well-being and health outcomes of women diagnosed with breast cancer.

No. of bitstreams: 0 Previous issue date: 2015 / Este estudo pretendeu investigar como os alunos da graduação em Medicina da Unifesp perceberam o curso “Conhecendo Pessoas – uma ciência, uma arte”, dentro da disciplina de Psicologia Médica, bem como avaliar o seu interesse pelo desenvolvimento de habilidades psicoafetivas e a construção da intersubjetividade. Para tanto, optamos pela abordagem qualitativa, cujas estratégias metodológicas facilitam a compreensão dos fenômenos humanos, especialmente nos aspectos que não podem ser medidos nem quantificados. Os dados empíricos foram obtidos em entrevistas com alunos do 2º, 3º, 4º, 5º e 6º anos da Faculdade de Medicina da Universidade Federal de São Paulo (UNIFESP), usando a técnica de entrevista semidirigida, orientada segundo roteiro de perguntas-chave. As entrevistas foram gravadas e posteriormente transcritas e conferidas. Para a análise do material empírico obtido foi utilizado o método da análise de conteúdo de acordo com a metodologia categorial temática. A análise de conteúdo aconteceu em três etapas: pré-análise, descrição analítica e interpretação inferencial. As categorias temáticas relevantes para o estudo foram: o sentido de conhecer pessoas; intersubjetividade e avaliação do curso “conhecer pessoas”; nestas categorias foram destacadas subcategorias, quais sejam: aproximação do outro, empatia, o humano por trás da máscara, relação dialógica professor-aluno, relação aluno-paciente, a disciplina oferece oportunidade para autoconhecimento, a disciplina como espaço para reflexão e acolhimento, discussão da relação médico-paciente não é a própria relação médico- paciente, o aprender a ser “humano”. Este estudo propiciou o conhecimento sobre a visão predominante dos alunos em relação à experiência de conhecer pessoas, evidenciando que o curso foi um auxiliar na compreensão de si mesmo, do outro, na valorização do relacionamento interpessoal, quer no âmbito pessoal, quer profissional. / This study aimed to investigate how Medicine’students percive the course "Meeting People - a science, an art," within the Medical Psychology discipline and evaluate their interest in developing psychoaffective skills and the construction of intersubjectivity . To this end, we opted for a qualitative approach, whose methodological strategies facilitate the understanding of human phenomena, especially those aspects that can not be measured or quantified. Empirical data were obtained using semi-structured interviews with 2nd, 3rd, 4th, 5th and 6th year medical students of the Faculdade de Medicina da Universidade Federal de São Paulo (UNIFESP), using a key questions script. The interviews were recorded and later transcribed and checked. For the analysis of empirical data obtained was used the method of content analysis according to thematic categorical methodology. Content analysis took place in three stages: pre-analysis, analytical description and inferential interpretation. Relevant themes for the study were: the meaning of meeting people; intersubjectivity and evaluation of the course "Meeting People"; in each of these categories were highlighted subcategories, namely: approaching the other; empathy; the human behind the mask; dialogic relation: teacher-student, student-patient; opportunity for self-knowledge; the discipline as a oportunity for reflection; discussion about the doctor-patient relationship; learning to be "human." This study provided knowledge about the prevailing view of the students in relation to the experience of meeting people, showing that the course was an aid to understand oneself when related to other people, the enhancement of interpersonal relationships whether on a personal or professional level.

A anemia falciforme é a doença hereditária mais comum no Brasil e constitui um problema de saúde pública. Alguns trabalhos buscaram destacar a relevância do enfoque epidemiológico da anemia falciforme entre as mulheres e suas repercussões na saúde reprodutiva dessa população. Mulheres portadoras de anemia falciforme apresentam maior risco de abortamento e complicações durante o parto, fatos que geram repercussões emocionais na gestante. O objetivo desta pesquisa foi conhecer significados das vivências da gravidez relatados por mulheres portadoras de anemia falciforme. Utilizamos o método clínico-qualitativo, tendo sido a amostra fechada por saturação das informações, composta por nove pacientes acompanhadas no Ambulatório de Alto Risco e na enfermaria de patologias obstétricas do Centro de Atenção Integral à Saúde da Mulher (CAISM). Suas idades variaram de 19 a 35 anos de idade, com períodos gestacionais entre 16 e 30 semanas. Foram realizadas entrevistas semidirigidas com questões abertas, gravadas, transcritas e submetidas à análise qualitativa de conteúdo. Os resultados foram categorizados em tópicos, dos quais elegemos dois para discussão neste trabalho: (1) o significado da maternidade parece permitir que o desejo de ser mãe se manifeste mais forte do que sentimentos comuns de medo relacionados à doença e os seus agravamentos; (2) a presença da doença no cotidiano dessas mulheres, as várias complicações e internações foram percebidas de maneira ambivalente, como um ¿mal necessário¿. Como esperado, o apoio de familiares e de profissionais da saúde foram cruciais para que a gestação dessas mulheres fosse enfrentada de forma mais tranqüila. O estudo sugere que apesar da vivência de angústias existenciais e psicológicas que envolvem normalmente a gravidez, das limitações da doença para o desempenho de funções profissionais e sociais e do conhecimento das possibilidades de complicações clínicas para a mãe e/ou para o bebê, estes sentimentos não interferiram na manifestação do desejo do exercício da maternidade / Abstract: The political proposals concerning the health of the black population have a recent history in the Brazilian political scenery with a special focus on the Sickle Cell Anemia National Program. The sickle cell anemia is the most common hereditary disease in Brazil and it is a public health problem. Some works have tried to emphasize the epidemiological aspect of the sickle cell anemia among women and its repercussions for the reproductive health of that population. Women with sickle cell anemia have higher miscarriage risks, besides higher risks of complications during childbirth. These risks have an emotional impact on pregnant women. The objective of this research was to have a better understanding of the experience of pregnant women with sickle cell anemia. We used the clinical-qualitative method; the sample included nine patients who had their follow-up done at the ¿Clinic of High Risk and in the Infirmary of Obstetric Pathologies at the Center of Integral Attention to the Woman's Health¿ (CAISM). Their ages ranged from 19 to 35, and the gestational age ranged from the 16th to the 30th weeks. Data were collected through semidirected interviews with open-ended questions; they were recorded, transcribed and underwent qualitative content analysis. The results were classified in topics, of which we chose two for discussion in this work: (1) the importance of maternity seems to make the desire of being a mother stronger than the fears related to the disease and its aggravations; (2) the presence of the disease in the daily lives of these women, the several complications and hospitalizations were faced with mixed feelings, as a "necessary evil". As expected, the support of relatives and health professionals was crucial for these women to withstand their pregnancy with more tranquility. The study suggests that in spite of the feelings of existential and psychological anguishes that usually involve these kind of pregnancy, and in spite of the professional and social limitations caused by the disease and in spite of the awareness of the possibilities of clinical complications for the mother and/or for the baby, there was still a strong desire of being a mother / Mestrado / Ciencias Biomedicas / Mestre em Tocoginecologia

Family experiences of physical trauma

Trauma is an event during which individuals are confronted with a threat to their own or to someone else’s integrity. If intense fear, horror and helplessness are experienced during the event there may be psychological traumatisation. However, individuals may experience physical trauma and require hospitalisation. The patients’ subjective experiences from the hospitalisations may precipitate further trauma. Although families of patients are not involved in the traumatic event, they may experience their own traumatisation. The individuals and their families experience the trauma on the biological, psychological and social levels. Medical literature is mostly positivistic and there is little qualitative research on the experience of hospitalisation, particularly of family experiences of the intensive care unit (ICU). There is also a paucity of research on psychological experiences in the medical world. The research that has been conducted in psychology is mostly with psychiatrists. The aim of this research is to explain the sense families make of physical trauma using narrative. Narrative is the sense individuals make of experiences across time through telling and re-telling stories. Qualitative research is most suited to explore these subjective experiences of individuals. Social constructionism is one form of qualitative research and a process exploring the world of individuals in the context of culture, history and social interaction. Individuals arrange these stories using myths, symbols and archetypes that will provide coherence to the lived experience. Languaging the experiences facilitates meaning attribution that informs behaviour. Data was collected through photographs taken by the participants and individual interviews were conducted. The co-construction of this text occurred in the context of the researcher as a counsellor, the researcher as a previous physical trauma patient and the family perspectives of the participants. The exploration of the photographs and their sequence are followed by a narrative analysis of the interview texts using storymaps. Narratives were co-created in this context. The participants selected the stories and created coherence by narrating and ordering the sequence of photographs. Since the family language this lived experience, the members explored various selves and their relationships with their worlds. The family was impacted biopsychosocially and is writing an alternate story in the discourse of the medical world that says further rehabilitation is difficult, if not impossible. They have made sense of the physical trauma by searching for unique outcomes and narrating on a temporal framework: stories of their self, relationships with others, their physical self and their physical environments. Transplante cardíaco - o ninho da fênix: um estudo sobre as relações objetais de pacientes em processo de transplante cardíaco / Heart transplant - the phoenix's nest: a study of objects relations in patients under cardiac transplant process

Estudos anteriores demonstram que fatores psicológicos intervêm nos resultados de um transplante cardíaco. Diferem, em seus achados, em função dos paradigmas a que estão vinculados, que incluem ou não o campo intersubjetivo nos quais os fenômenos emergem. Nesta investigação objetivamos descrever o funcionamento mental de pacientes candidatos ao transplante, a partir de suas relações objetais inconscientes, delimitando uma vivência emocional comum nesta situação, bem como, buscamos averiguar se existem diferenças entre os pacientes, em função da realização ou não da cirurgia. Utilizou-se, como procedimento, o Teste de Relações Objetais de H. Phillipson (TRO), aplicado em 63 cardiopatas com indicação ao procedimento. O material clínico proveniente do acompanhamento psicológico destes pacientes também é utilizado para completarmos as observações. Os resultados apontam a presença de indicadores psicopatológicos, de acordo com a classificação de Grassano (1996) para depressão clínica em 60 casos. Em dois casos observaram-se indicadores para psicopatia e um caso para funcionamento psicótico, com repercussões negativas sobre o vínculo com o tratamento. Constataram-se diferenças estatisticamente significantes na performance dos pacientes frente às lâminas BG e C2, mas não podemos afirmar que as dificuldades de ajuste perceptual nestas lâminas, mais freqüentes entre aqueles que não fazem a cirurgia, seja fator de obstáculo ao procedimento. Sugerimos, como hipótese teórica deste trabalho, a presença de refúgios psíquicos, de acordo com Steiner (1997) como estratégia de sobrevivência psíquica, nesta situação, tendo em vista o predomínio, regressivo, de relações de objeto persecutórias, em virtude da extrema ansiedade depressiva. Assim, neste refúgio (ninho da Fênix), o paciente abriga-se da dor da perda (luto pela vida, pelo coração a ser retirado) e da aniquilação (devastação da doença, risco cirúrgico e da imunossupressão), conseguindo enfrentar o processo de transplante cardíaco. Caso contrário, o paciente sucumbe ao quadro depressivo. / Prior studies have shown that psychological factors influence the results not a hearth transplant. The findings are different in function of the paradigm linked to them that may include or not the inter-subjective field from which the phenomena emerge. The purpose of this research was to describe the mental functioning of patients who are candidate to a heart transplant, starting from their unconscious object relations that delimit a shared emotional existence upon that situation, and we also verified if there were differences between patients as to the accomplishment or not of the surgery. The procedure used was the Object Relation Test (ORT) applied over 63 cardiac patients who had indication for that procedure. The clinical material arose from the psychological follow-up performed with those patients was used to complete the observations as well. The results indicate the presence of psychopathological markers, following the Grassano's classification (1996) to the clinical depression in 60 cases. The markers for psychopathy were observed in two cases, and in one case it was observed the marker for psychotic functioning with negative repercussions on the link to the treatment. It was verified statistically significant differences in the patients' performance on the analysis of the BG and C2 plates, but it was impossible to assert that the difficulties in the perceptual adjustment in those plates which were more frequently found plates in patients who were not submitted to the surgery is an obstacle factor to the procedure. The theoretical hypothesis we have suggested in this paper is the presence of psychic refuges, according to Steiner (1997) as strategy for psychic survival to face such situation, having in mind the regressive predominance of the relationships of persecutory object upon an extremely depressive anxiety. Thus, in such refuge (the Phoenix's nest), the patient has a shelter against the pain caused by the loss (mourning the life because one's heart has to be withdrawn), and the annihilation (devastation caused by the disease, surgical risk, and due to the immunosuppression), in order to face the heart transplant process. Otherwise, the patient succumbs to the depressive situation.
