Bond strength evaluation of two resin cements with two adhesives and analysis of mode of failure

Indiana University-Purdue University Indianapolis (IUPUI) / Cementing of indirect restorations with resin cements generally requires the pre-treatment of dentin with an adhesive. When dual-cured or chemical-cured resin cements are used with these single-step adhesives, incompatibility issues exist. This has resulted in manufacturers making chemical changes in their products. Kerr Dental markets a new resin cement, Nexus Third generation (NX3), which utilizes a proprietary redox system different from the second generation of composite luting agent (NX2). The aim of this study was to evaluate microtensile bond strength and mode of failure of NX3 and NX2 with two different adhesive systems (total-etch and self-etch) after 1 week and after 3 months of storage. Methods: Sixty-four non-carious teeth were sectioned to expose the dentin using a low-speed saw. Dentin surfaces were ground with 320-grit SiC paper. The adhesives Optibond Solo Plus (SOL), and Optibond All In One (AIO) were applied, and resin cements (NX2, NX3) were used to lute 4-mm composite discs to the treated dentin surfaces. Microtensile bond strength was determined at 1 week (IM) and after 3 months (3MON) of storage using a universal testing machine (MTS). All specimens were examined under the stereomicroscope to determine the mode of failure. Random specimens from each failure group were examined using scanning electron microscopy. Statistical Analysis: Comparisons between the treatment combinations for differences in microtensile bond strength were performed using Weibull-distribution survival analysis. Comparisons between the treatment combinations for differences in the failure mode were performed using Fisher’s Exact tests. The group NX3 SOL IM (30.5 MPa) had significantly higher bond strength than NX3 SOL 3MON (13.4 MPa); NX3 AIO IM (11.3MPa); NX3 AIO 3MON (8.2 MPa; NX2 AIO 3MON (5.8 MPa); NX2 SOL IM (6.3 MPa), and NX2 SOL 3MON (3.2 MPa). The group NX2 AIO IM (19.3 MPa) was not significantly different from NX3 SOL IM. The group NX2 SOL 3MON and group NX2 SOL IM had a significantly higher percentage of teeth with mixed failure than all of the other groups. None of the other groups had significantly different failure mode. The group NX3 SOL IM had 90-percent beam survival beyond 17 MPa, and NX2 AIO IM had 50 percent of beams surviving beyond 17 MPa, a better performance. For all the other groups, more than 50 percent of beams failed below 17 MPa. Results show high evidence of degradation for all groups considered in this investigation. The use of these types of cement adhesive combinations in clinical situations should be used with this understanding.

Influência da contaminação por saliva durante a realização de procedimentos adesivos / Influence of saliva contamination during adhesive procediments

O objetivo deste estudo foi avaliar a influência da contaminação com saliva em diferentes momentos do procedimento operatório, bem como diferentes tratamentos desta contaminação. Este estudo foi conduzido utilizando estudo de resistência adesiva (RA), através de ensaio de microtração, com avaliação do padrão de fratura. Foram utilizados 120 molares humanos (n=10). Os dois substratos do teste foram obtidos a partir da mesma unidade amostral. Os grupos experimentais para ambos os substratos foram: sem contaminação (controle G1 e G13); contaminação antes do condicionamento ácido (CAC), com lavagem e secagem (G2 e G14); secagem apenas (G3 e G15); contaminação após o condicionamento ácido (CAC), com lavagem e secagem (G4 e G16); secagem apenas (G5 e G17); CDC e recondicionamento para repetição da técnica adesiva (G6 e G18); contaminação entre as duas camadas de adesivo (CEA), com lavagem e secagem (G7 e G19); secagem apenas (G8 e G20); CEA e recondicionamento (G9 e G21); contaminação após a aplicação final do sistema adesivo (CFA), com lavagem e secagem (G10 e G22); secagem apenas (G11 e G23); CFA e recondicionamento (G12 e G24). O adesivo utilizado foi o Adper Single Bond 2 (SB2) e a resina composta Z-250 (3M) para a realização do teste de microtração. Os resultados foram submetidos ao teste de ANOVA demonstrando que existe influencia da contaminação com saliva na RA de esmalte (p=0,007) e dentina (p=0,001). Para o esmalte, apenas o momento da contaminação originou resultados estatisticamente significantes (p<0,001). Contudo, a interação entre os fatores de variação em dentina mostrou-se estatisticamente significante (p<0,001). Dessa forma, concluímos que a contaminação do campo operatório com saliva durante a utilização de sistema adesivo do tipo condicione-e-lave reduz a adesão em esmalte e em dentina somente em condições específicas; o tratamento do contaminante mais indicado para recuperar a RA perdida está diretamente relacionado com o momento do procedimento adesivo em que a contaminação ocorre. / This study evaluated the influence of saliva contamination during distinct steps of the operatory procedure and treatments used to counteract its effects on the microtensile bond strength. 120 human molars were used (n=10) and both substrates were obtained from the same teeth. ANOVA results showed a statistically significant difference when saliva contaminated adhesive procedures in enamel (p=0.007) and dentin (p=0.001). In enamel, the moments when contamination occurred during the operative procedure showed statistical difference (p<0.001), while in dentin the interaction between the factors - moments and treatments - showed to be statistically significant (p<0.001). Based on the obtained results it was licit to conclude that saliva contamination during etch-and-rinse adhesive application decreases adhesion to enamel and dentin in specific experimental conditions; the most indicated treatment to counteract saliva influence is directly related to the moment of the operatory procedure that the contamination occurred.

Tratamento de superfície e adesão em materiais vitrocerâmicos experimentais para aplicação odontológica / Surface treatment and bonding in experimental glass-ceramic materials for dental application

Materiais cerâmicos têm sido utilizados rotineiramente na clínica restauradora. A capacidade de adesão à sistemas adesivos de cimentação é um dos fatores críticos para o sucesso clínico. O objetivo do presente estudo foi avaliar as propriedades de adesão à um sistema de cimentação adesiva de dois materiais vitrocerâmicos experimentais, sendo um reforçado por dissilicato de lítio (D) e outro por metassilicato de lítio (M) para aplicação odontológica. Para tal, objetivou-se determinar a influência de diferentes tratamentos de superfície na resistência de união a um sistema de cimentação adesiva através de ensaios de microtração e caracterizar as alterações morfológicas decorrentes dos diferentes tratamentos de superfície testados. Três tratamentos de superfícies para cada material experimental foram selecionados: 1- jateamento com partículas de Al2O3 (J), 2- ataque ácido com ácido hidrofluorídrico a 10% por 20 segundos (20) e 3- ataque ácido com ácido hidrofluorídrico a 10% por 60 segundos (60). Um grupo do material comercial IPS e.max CAD (Ivoclar-Vivadent) foi utilizado como controle, seguindo as instruções do fabricante para o tratamento de superfície. As alterações morfológicas na superfície das cerâmicas foram analisadas por microscopia eletrônica de varredura (MEV) e demonstraram que os materiais experimentais são ácido sensíveis e sofreram modificações topográficas com o jateamento. Para os ensaios de microtração, os blocos das cerâmicas experimental e comercial tiveram suas superfícies condicionadas e silanizadas com o Agente Silano Relyx Ceramic Primer (3M ESPE), e posteriormente foram cimentados a blocos de resina composta Filtek Z100 (3M ESPE) com o cimento resinoso RelyX ARC (3M ESPE). Tais conjuntos foram seccionados, originando espécimes em forma de palito, com 1mm2 de área de secção transversal e submetidos ao ensaio de microtração. A análise estatística dos dados foi realizada por meio do teste de O teste utilizado foi o de Análise de Variância a um critério (One-Way ANOVA), adotando nível de significância de &#x3B1;=5%, O teste de Bonferroni foi utilizado para comparações múltiplas, demonstrando que os grupos D20 e M60 apresentaram melhores resultados de resistência de união. D20 não apresentou diferença estatística em relação ao grupo controle e M60 demonstrou resistência de união significantemente maior que o material comercial do grupo controle; levando, portanto, à conclusão de que os materiais experimentais testados apresentam características adequadas de adesão ao sistema de cimentação adesiva utilizado. / Ceramic materials have been routinely used in restorative clinic. The adhesion capacity to adhesive cementation systems is a critical factor for clinical success. The main objective of the present study was to evaluate the adhesion properties to an adhesive cementation system of two experimental vitroceramic materials, a reinforced by lithium dissilicate (D) and another by lithium metasilicate (M) for dental application. For this purpose, the objective was to determine the influence of different surface treatments on bond strength to an adhesive cementation system through microtensile tests and characterize morphological changes resulting from the different surface treatments tested. The test used was the analysis of variance to a criterion (One-Way ANOVA), adopting a significance level of &#x3B1; = 5%, the Bonferroni test was used for multiple comparisons, demonstrating that the D20 and M60 showed best results of bond strength. D20 showed no statistical difference in relation to the control group and M60 demonstrated significantly bond streght better than the commercial material of the control group; leading to the conclusion that the experimental material tested has appropriate characteristics of adhesion to the cementation system used.

Relationship between caries-affected dentin mineral density and microtensile bond strength

Objective: To determine the relationship between mineral density and microtensile bond strength of caries-affected dentin (CAD). Methods: Sixty-three extracted human molars with carious lesions and nine extracted sound human molars are collected and flattened to expose the dentin. Caries is removed using Caries Detector (Kuraray Medical, Tokyo, Japan) leaving a firm light pink stained dentin and then bonded with RBC (Z100, shade T, 3M ESPE, Germany) using an etch-and-rinse 3-step adhesive system (Optibond FL,Kerr, Orange, CA, USA). The bonded teeth are stored overnight at 37ºC before vertically sectioned with diamond saw blades (IsoMet 1000, Buehler Ltd., Lake Bluff, IL, USA). The sticks are then trimmed into dumbbell-shaped specimens with a cross-sectional area of 0.5 mm2 and a gauge length of 1 mm. Tensile testing is performed at a crosshead speed of 1 mm/min (Zwick Materials Testing Machine Z2.5/TN1S, Zwick, Ulm, Germany). X-ray microtomography was used to examine the fractured specimens (Micro-CAT II, Siemens Preclinical Solutions, Knoxville, TN) at maximum resolution of 27 microns. The mean mineral densities at the resin-dentin interface of the bonded specimens are calculated using a custom BMD (bone mineral density) analyzer software (Iowa City, IA, USA). Mineral density (image intensity) will be plotted against uTBS (MPa) to determine correlation between these two properties based on Spearman rank correlation test at 0.05 level of statistical significance. Results:There was a statistically significant relationship between µTBS and image intensity (p< 0.0001). However, the correlation coefficient was weak (0.31). Significant effect of the failure mode on the image intensity and the µTBS were observed (p < 0.0001). No significant difference in the mean image intensity was found between the 2 levels (p = 0.6519) and 3 levels of dye staining (p = 0.2531). Intra- and inter-examiner reliability was near perfect (0.99; 0.98) for mineral density measurements. Conclusion: Within the limits imposed in the experimental design, we concluded that the degree of mineralzation of CAD has an influence on its failure mode and µTBS. Positive increasing relationship was also found between CAD's failure mode and its µTBS. Additionally, no significant relationship was found between levels of dye staining and the degree of mineralization.

Microtensile bond strength of resin-dentin bonds following application of a chemical collagen cross-linker using different dentin bonding systems

Introduction: The stabilization of dentinal collagen fibers against enzymatic degradation by the use of biocompatible cross-linker agents is of clinical importance for effective dentin bonding to surpass the test of time. Objective: The present study aims to evaluate and compare the effect of the application of two versions of a desensitizer solution to sound coronal dentin, on the microtensile bond strength (μTBS) of the resin-sound coronal dentin using 4th and 6th generation dentin bonding systems. Materials and Methods: Extracted human third molars were collected from an unidentified bank of teeth followed by IRB approval. A flat surface of all 12 teeth was prepared utilizing a water-cooled high-speed diamond disc, leaving an entire hard sound dentinal area for testing. Subsequently, according to the assigned group, specimens followed specific manufacturer’s instructions for application of dentin bonding systems: specimens were subdivided into 6 groups (n=20). Group 1 (G1) First positive control group. Specimens received an application of a 4th generation dentin bonding system (DBS). Group 2 (G2) Second positive control group. Specimens received an application of a 6th generation DBS. Group 3 (G3) Specimens were exposed to Gluma Desensitizer agent, blot-dried and followed by application of a 4th generation DBS. Group 4 (G4) Specimens were exposed to Gluma Desensitizer agent, blot-dried and followed by application of a 6th generation DBS. Group 5 (G5) Specimens were exposed to Gluma Desensitizer PowerGel agent, blot-dried and followed by application of a 4th generation DBS. Group 6 (G6) Specimens were be exposed to Gluma Desensitizer PowerGel agent, blot-dried and received an application of a 6th generation DBS. After application of the adhesive systems, all specimens were restored using a microhybrid resin composite. The root portion was sectioned 1mm below the CEJ, and discarded. All specimens were thermocycled at 5-55 Cº for 7000 cycles on distilled water. Then each restored tooth was sectioned perpendicular to the bonding interface into 1mm x 1mm x 8mm beams with a slow speed diamond wafering blade under thorough irrigation. Then specimens were subjected to μTBS testing at a crosshead speed of 1mm/min. Subsequently; specimens were subjected to fracture analysis and SEM evaluation of the different failure’s mode of the involved surfaces. Statistical analysis was performed by usingone- way ANOVA, two-way ANOVA and Fisher’s PLSD test (p<0.05). Results: For the first aim of the study and after obtaining the μTBS in MPa: Group G1: 15.50 ± 6.28, Group G2: 13.06 ± 11.53, Group G3: 19.20 ± 9.43, Group G4: 12.76 ± 4.61, Group G5: 14.38 ± 5.95, Group G6: 18.54 ± 9.49. Statistical analysis showed that there is no significant influence of variables on the μTBS (Welch ANOVA [F (5,114) =2.21, p=0.057]). Treatment with Gluma desensitizing agent and Gluma desensitizing PowerGel has no significant influence on the bond strength. For the second aim of the study and to analyze group differences for type of fracture data was first recoded into two groups: (1) Adhesive failure and (2) Cohesive failure. Group differences were analyzed by type of fracture using a Fisher’s exact test. Remoção seletiva da lesão de cárie com laser de Er:YAG e biomodificação da dentina com quitosana / Selective caries removal using Er:YAG laser and dentin biomodification with chitosan

O objetivo deste estudo foi avaliar o efeito da remoção seletiva da lesão de cárie utilizando laser de Er:YAG, seguida da biomodificação da dentina com quitosana. Os objetivos específicos foram: 1) Avaliar os elementos químicos e a morfologia da superfície dentinária, bem como, a microdureza da subsuperfície (experimento 1) e 2) Avaliar a resistência de união da interface resina/dentina e sua morfologia após 24h, 6 e 12 meses de envelhecimento dos espécimes, a perda de massa seca e a resistência à degradação do colágeno (experimento 2). Lesões de cárie em dentina foram criadas pelo método de ciclagem do pH (14 dias) em 208 espécimes bovinos. Os espécimes foram divididos de acordo com método de remoção seletiva da lesão de cárie: broca (em baixa rotação) ou laser de Er:YAG (250 mJ/4Hz). Os espécimes receberam ácido fosfórico a 35%, e foram subdivididos de acordo com a biomodificação da dentina: sem biomodificação ou quitosana a 2,5%. No experimento 1, os espécimes foram submetidos às análises de Espectroscopia Eletrônica de Análise Química (EDS) e Microscopia Eletrônica de Varredura (MEV) da superfície e microdureza da subsuperfície dentinária. No experimento 2, os espécimes foram submetidos a resistência de união à microtração e MEV da interface adesiva após 24h, 6 e 12 meses de armazenamento em água, avaliação da perda de massa seca e liberação de hidroxiprolina (HYP). Os dados foram analisados por meio de testes paramétricos e não paramétricos (=0.05). A remoção seletiva da lesão de com laser de Er: YAG promoveu aumento da microdureza da dentina afetada (p=0,03) e redução na quantidade de Ca (0<0,0001) e P (0<0,0001). A biomodificação com quitosana não influenciou a microdureza (0,0796) e a porcentagem atômica dos elementos Ca (p=0,2269) e P (p=0,8842) presentes na dentina residual. A análise em MEV mostrou alterações morfológicas na superfície de dentina (p>0,05). Após 24h, os maiores valores de resistência de união foram obtidos para a broca em comparação ao laser Er:YAG (p<0,001). Após 6 meses, os métodos foram semelhantes (p=0,432) e após 12 meses a resistência de união à dentina irradiada foi significativamente maior do que à dentina tratada com broca (p=0,025). A biomodificação com quitosana aumentou significativamente a resistência de união à dentina após 6 (p=0,011) e 12 meses (p<0,001). A análise em MEV mostrou que a biomodificação com quitosana preservou a interface adesiva após 12 meses de armazenamento. A perda de massa seca (p>0,664; p>0,263) e a liberação de hidroxiprolina (p>0,6347; p>0,4274) não foram influenciadas, respectivamente, pelo método de remoção da cárie ou pela biomodificação dentinária. A remoção seletiva da dentina cariada com o laser de Er: YAG aumentou a microdureza da dentina afetada por cárie, alterando sua morfologia superficial e composição química. A biomodificação com quitosana não influenciou a composição estrutural e química da dentina residual. Após 24h de armazenamento em gua, a resistência de união da resina composta à dentina foi afetada pelo laser de Er:YAG. No entanto, a resistência de união à dentina irradiada foi mantida, enquanto a dentina tratada com broca apresentou redução contínua. A biomodificação com quitosana, independente do método de remoção utilizado, preservou a resistência de união e a interface adesiva ao longo de 12 meses. O laser de Er:YAG e a biomodificação com quitosana não influenciaram na perda de massa seca e na liberação de hidroxiprolina / The aim of this study was to evaluate the effect of Er:YAG laser for selective removal of carious lesion, followed by biomodification with chitosan. The specific objectives were: 1) To evaluate the chemical elements and the morphology of the dentin surface, as well as, the subsurface microhardness (Experiment 1). 2) To evaluate the microtensile bond strength (TBS) and adhesive interface after 24h, 6 and 12 months of water storage, the dry mass loss and dentin collagen degradation (Experiment 2). Dentinal caries lesions were created in 208 bovine specimens by pH-cycling method (14 days). Specimens were divided according to the carious removal method: bur (low-speed handpiece) or Er:YAG laser (250 mJ/4Hz). Specimens were treated with 35% phosphoric acid and were subdivided according to dentin biomodification: without chitosan (control) and 2.5% chitosan. For the experiment 1, specimens were submitted to Energy Dispersive X-Ray Spectroscopy (EDS) and Scanning Electron Microscopy (SEM) and subsurface microhardness of the dentin. For the experiment 2, specimens were submitted to microtensile bond strength and SEM analysis of the adhesive interface after 24h, 6 and 12 months of storage in water, evaluation of dry mass loss and hydroxyproline release (HYP). Data were analyzed by parametric and non-parametric tests (=0.05). The selective caries removal with Er:YAG laser increased the microhardness value of caries-affected dentin (p=0.03) and reduced the amounts of Ca (0<0.0001) and P (0<0.0001). The biomodification with chitosan did not influence the microhardness (0.0796) and atomic percentage of Ca (p=0.2269) and P (p=0.8842) of residual dentin. SEM analysis showed morphological changes on dentin surface (p>0.05). After 24h, the highest bond strength values were found for bur compared to Er:YAG laser (p<0.001). After 6 months, methods were similar (p=0.432) and after 12 months, the highest bond strength values were found for Er:YAG laser-irradiated dentin (p=0.025). Biomodification with chitosan significantly increased bond strength to dentin after 6 (p=0.011) and 12 months (p<0.001). SEM analysis showed that the biomodification with chitosan preserved the adhesive interface after 12-month storage. The dry mass loss (p>0.664; p>0.263) and HYP release (p>0.6347; p>0.4274) were not influenced, respectively, by the carious removal method or by dentin biomodification. The selective removal of carious dentin with Er:YAG laser increased microhardness of residual caries-affected dentin changing its surface morphology and chemical composition. The biomodification with chitosan did not influence the structural and chemical composition of residual dentin. After 24 hours of water storage, the composite resin bond strength to dentin was affected by Er:YAG laser. However, the bond strength to irradiated-dentin was maintained, while the bur-treated dentin showed a continuous reduction. Biomodification with chitosan, regardless of removal method used, preserved the bond strength and adhesive interface over 12 months. The dry mass and the hydroxyproline release were not influenced

O objetivo do presente estudo foi avaliar o efeito da biomodificação com carbodiimida e quitosana fosforilada na dentina afetada por cárie. A amostra foi composta por 75 espécimes de dentina bovina submetidos à indução de lesão de cárie pelo método de ciclagem do pH (8h em solução desmineralizante e 16h em solução remineralizante) durante 14 dias. Foi realizada a remoção seletiva da lesão de cárie com broca em baixa rotação. Os espécimes foram divididos de acordo com a biomodificação da dentina: sem biomodificação (controle), carbodiimida (EDC) e quitosana fosforilada (P-chi). Os espécimes foram condicionados com ácido fosfórico 35%, e tratados com o agente biomodificador correspondente. Os espécimes foram restaurados com adesivo (Single Bond Universal - 3M ESPE) e resina composta (Z250 - 3M ESPE). Trinta espécimes foram submetidos à análise de microdureza da subsuperfície dentinária em quatro momentos: dentina hígida, dentina afetada por cárie, após a remoção seletiva da lesão de cárie (dentina residual) e, após a restauração dos espécimes. Os 36 espécimes restantes foram submetidos a ensaio de resistência de união à microtração (&mu;TBS) realizado após 24 h e 6 meses de armazenamento em água e microscopia eletrônica de varredura (MEV). Os dados foram analisados por meio de testes paramétricos e não paramétricos (&alpha;=0,05). A biomodificação da dentina residual afetada por cárie com EDC e P-chi não alterou a microdureza da subsuperfície dentinária (p=0,141). Na análise de &mu;TBS, não foram encontradas diferenças significativas entre os grupos 24 h e após 6 meses de armazenamento. Ao longo do tempo, o grupo controle (p<0,001) teve redução da &mu;TBS, o grupo EDC (p=0,099) foi capaz de manter a &mu;TBS e o grupo P-chi (p=0,005) promoveu aumento da &mu;TBS. A análise de MEV mostrou que os biomodificadores preservaram a interface adesiva após 6 meses de armazenamento. A biomodificação com carbodiimida e quitosana fosforilada não influenciou a microdureza da subsuperfície da dentina afetada por cárie, porém foi capaz de inibir a degradação da interface adesiva, mantendo a resistência de união da resina composta à dentina afetada após 6 meses de armazenamento em água / The aim of this study was to evaluate in vitro the effect of biomodification with carbodiimide and phosphorylated chitosan on caries affected dentin. The sample was composed by 75 bovine incisors, submitted to caries induction by pH cycling (8h in demineralizing solution and 16h in remineralizing solution) for 14 days. The selective caries removal was performed with bur at low-speed handpiece. Specimens were divided according to the dentin biomodification: no biomodification (control), carbodiimide (EDC) and phosphorylated chitosan (P-chi). All specimens were conditioned with 35% phosphoric acid and then treated with the correspondent biomodificator. Specimens were restored with an adhesive system (Single Bond Universal - 3M ESPE) and composite resin (Z250 - 3M ESPE). Thirty specimens were submitted to subsurface microhardness test in four periods of time: on sound dentin, on caries-affected dentin, after the selective removal of caries (residual caries-affected dentin) and after the restoration. The remaining 36 specimens were submitted to microtensile bond strength test (&mu;TBS) after 24 h and 6 months of water storage and to scanning electron microscopy analysis (SEM). Data were statistically analyzed by parametric and non-parametric tests (&alpha;=0.05). The biomodification of residual caries-affected dentin with EDC and P-chi did not change the subsurface microhardness (p=0.141). In the &mu;TBS test, there were no significant difference between groups at 24 h and 6 months of water storage. Over time, the control group (p<0.001) had a decreased on the &mu;TBS, the EDC group (p=0.099) was capable of maintained &mu;TBS and the P-chi group (p=0.005) increased in &mu;TBS. SEM analysis showed that biomodification preserved the adhesive interface after 6 months of water storage. The biomodification with carbodiimide and phosphorylated chitosan did not influence the subsurface microhardness of caries-affected dentin, however, it was capable of inhibit degradation of the adhesive interface, maintaining the microtensile bond strength of the composite resin to caries affected dentin after 6 months of water storage
