Avaliação de nevos cutâneos e melanomas por ferramenta de análise morfométrica nuclear digital

To evaluate the nuclear morphometric differences between nevi excised in winter or summer; demonstrating the presence of characteristics and irregularities in nuclear morphology caused by solar radiation on nevi. To consider whether there are morphological differences between common acquired nevi, dysplastic nevi and melanomas. To access the potential use of nuclear morphometry to detect cell senescence, apoptosis and other nuclear irregularities of nevi and melanomas in HE stained slides of paraffin embedded specimens.Methodology: Histopathological specimens were subjected to digital image analysis.Results: Through this pilot study, glass slides obtained from biopsies of 30 patients were analyzed; 10 nevi excised in winter, 5 nevi excised in summer, 9 dysplastic nevi and 6 melanomas. Between winter and summer nevi; secondary variables able to perform such a separation were 'variability Area' and % of nuclei with NII higher than 1SD.Conclusion: There are differences of nuclear morphometric characteristics of excised nevi in winter and summer and can be caused by cumulative effects of solar radiation on nevi. There are morphometric differences between common acquired nevi, dysplastic nevi and melanomas. The dysplastic nevi and melanomas were separated by the average area and percentage of nuclei with area above one standard deviation of the mean area. / Objetivos: Avaliar as características dos núcleos celulares de melanócitos através de análise morfométrica nuclear digital e as diferenças morfométricas nucleares entre nevos excisados no inverno e nevos excisados após o verão; para detectar possíveis características e irregularidades na morfometria nuclear provocadas pela radiação solar sobre nevos. Averiguar a possibilidade de identificação de diferenças morfométricas entre nevos adquiridos comuns, nevos displásicos e melanomas e avaliar a possibilidade de detecção de senescência, apoptose e outras irregularidades nucleares em nevos e melanomas por morfometria digital em cortes histológicos corado por HE a partir de blocos de parafina.Metodologia: Os espécimes histopatológicos foram submetidos à análise de imagem digital realizada através de ferramentas de avaliação nuclear digital.Resultados: Neste estudo piloto foram analisados os espécimes de biópsia de 30 pacientes, compostos por: 10 nevos biopsiados no inverno, 5 nevos no verão, 9 nevos displásicos e 6 melanomas malignos. Nos nevos de inverno e nevos de verão; as variáveis capazes de realizar tal separação foram ‘Variabilidade da Área’ e ‘porcentagem de núcleos com NII acima do valor da média+1DP.Conclusão: Foram detectadas diferenças morfométricas nucleares entre nevos excisados no inverno e nevos excisados após o verão, que podem ser provocadas pelos efeitos cumulativos da radiação solar sobre nevos. Foram catacterizadas diferenças morfométricas entre nevos adquiridos comuns, nevos displásicos e melanomas. Os nevos displásicos e melanomas foram separados pelas características de área média e porcentagem de núcleos com área acima de um desvio padrão da média.

Mapeamento digital e avaliação dos padrões globais dermatoscópicos de nevos melanocíticos durante a gestação

Introdução: O conhecimento sobre nevos na gestação é escasso. Existem relatos de crescimento e mudança no padrão dermatoscópico, com formação de novas estruturas e alterações de pigmentação e da arquitetura da rede pigmentar. A maioria dos estudos nesta área são relatos de caso e estudos prospectivos em que foram avaliados poucos nevos por gestante. Objetivo: Analisar mudanças no mapeamento corporal de gestantes e nas estruturas dermatoscópicas dos nevos melanocíticos destas pacientes. Método: Foram avaliados um total de 703 nevos melanocíticos provenientes de 18 gestantes, no primeiro e terceiro trimestres. Foi realizado exame de mapeamento corporal total e dermatoscopia digital e as imagens foram comparadas em relação à formação de novas lesões; mudanças nas estruturas dermatoscópicas (crescimento, mudanças pigmentares, padrão pigmentar, mudanças de rede pigmentar, glóbulos e pontos, novas áreas estruturas vasculares, novas estrias, novas áreas sem estruturas); associação de crescimento de nevos com relação à localização, ao fototipo, ao risco de melanoma, às estruturas dermatoscópicas e à faixa etária; e associação de fototipo e risco de melanoma com estruturas dermatoscópicas. Resultados: Na comparação das imagens do mapeamento corporal, tiveram 44% de pacientes com novas lesões, variando de 1 a 5 lesões por paciente. Todas as pacientes com novas lesões tiveram pelo menos uma das lesões localizada nos membros superiores. Com relação à avaliação dos 703 nevos melanocíticos, 10.4% tiveram hiperpigmentação e 5.8% tiveram hipopigmentação. As mudanças nas estrtuturas dermatoscópicas foram: 23% de mudanças de rede pigmentar, 3.2% de formação de novos vasos, 1.7% de novas estrias, 1% de novas áreas sem estruturas e 55% de crescimento de nevos. Os nevos que cresceram se localizavam com maior frequencia no abdômen (87.1%; p < 0.001), foram mais frequentes nas pacientes que tinham maior risco de melanoma (45% ; p = 0.019), tiveram mais mudanças de rede pigmentar (27.1% ; p = 0.014) e maior formação de glóbulos/pontos (16% ; p < 0.001). Houve uma associação entre formação de estrias e fototipo (p = 0.012), sendo mais frequente nos fototipos II (2.7%), se comparados com fototipos III (1.3%) e IV (0%). Conclusão: Novos nevos melanocíticos podem aparecer na gestante, especialmente nos membros. Um parcela dos nevos pré-existentes se altera na gestação. Crescimento dos nevos parecem ocorrer mais frequentemente em pacientes de alto risco para desenvolver melanoma. O aparecimento de novas estrias é mais frequente em fototipos mais baixos. / Background: Melanocytic nevi might vary in size and number in pregnant women and the differential diagnosis with melanoma may be challenging. Dermoscopy improves the accuracy of pigmented lesions evaluation. Little is known about the dermoscopic changes in melanocytic nevi during pregnancy. Objectives: This study aims to describe changes in total body photography of pregnant women and dermoscopy aspects of their melanocytic nevi. Methods: A total of 703 melanocytic nevi were evaluated from 18 pregnant women, in the first and in the third trimester of gestation. Total body photography and digital dermoscopy were made and the images obtained from the first and third trimesters were compared for new lesions formation; changes in dermoscopic aspects (enlargement, pigmentation changes, pigmentation pattern, network changes, globules and dots, new vascular structures, new streaks, new structureless area); association of nevi enlargement with body location, skin type, risk melanoma, dermoscopy structures and age group; and association of skin type and risk of melanoma with dermoscopy structures. Results: When comparing the total body photography, there were 44% of patients with new lesions, ranging from 1 to 5 new lesions per patient. All patients with new lesions presented at least one of them on the upper limbs. Regarding dermoscopic evaluation of the 703 melanocytic nevi, 10.4% had hyperpigmentation and 5.8% had hypopigmentation. Regarding the dermoscopic structures, there were 23% of network changes, 12.4% new globules/dots, 3.2% of new vessels formation, 1.7% of new streaks, 1.0% of new structureless area, and 55.0% nevi increased in size when comparing the first and third trimesters. These lesions that increased in size were more likely to be seen in the abdomen (87.1%; p<0.001), were more frequent in patients with high risk of melanoma (45.0% ; p=0.019), had more network changes (27.1% ; p=0.014) and new globules and dots formation (16.0% ; p<0.001). An association between streaks formation and skin type was significant (p=0.012) and was more frequent in skin type II (2.7%), when compared to skin type III (1.3%) and IV (0%). Conclusions: New melanocytic nevi may occur in pregnant women, especially on the limbs. A portion of the pre-existing melanocytic nevi changes in pregnancy. Nevi enlargement seem to occur more frequently in high risk melanoma patients. Appearance of new streaks are more frequent in fair skin types.

No. of bitstreams: 0 Previous issue date: 2009-05-25 / Este trabalho tem como objetivo avaliar o efeito do uso tópico da fenitoína na cicatrização de feridas cutâneas. A fenitoína como terapia sistêmica tem ação anticonvulsivante, e com o uso tópico tem ação de acelerar a cicatrização de feridas cutâneas. Neste estudo foram avaliados 100 pacientes cada um com 2 feridas cutâneas resultantes da excisão de lesões de nevos melanocíticos, sendo 50 pacientes com lesões na face e 50 pacientes com lesões em dorso do tórax, totalizando 200 lesões excisadas com punch modificado. Durante o tratamento o mesmo paciente recebia a fenitoína a 0,5% em creme e o creme base (controle), para fazer os curativos diariamente. O seguimento dos pacientes foi realizado no periodo de 7, 14, 21 e 60 dias, sendo avaliados os seguintes parâmetros: sangramento, exsudato, eritema na borda da ferida, infecção, presença de tecido de granulação hipertrófico, tempo de cicatrização, intensidade da epitelização, área da ferida cutânea, o formato e área da cicatriz, as possíveis reações adversas e o resultado cosmético. Os pacientes foram comparados quanto a idade, peso, altura, fototipo, tipo histológico da lesão. A fenitoína apresentou melhor resultado terapêutico e cosmético comparado com o creme base (controle). A fenitoína é uma droga de baixo custo, que acelera a cicatrização de feridas cutâneas. / This study aims to evaluate the effect of topical use of phenytoin in healing of skin wounds. A systemic therapy with phenytoin have anticonvulsant activity, and the topical use action is to accelerate the healing of skin wounds. This study evaluated 100 patients each with 2 wounds resulting from excision of lesions of melanocytic nevi, 50 patients with lesions on the face and 50 whith back injuries in the chest, total of 200 lesions excised with punch modified. During treatment the patient received phenytoin to 0.5% in the cream and cream base (control), to make the dressing daily. The follow up of patients was carried out from 7, 14, 21 and 60 days, where the following parameters were evaluated: bleeding, exudate, erythema at the edge of the wound, infection, presence of hypertrophic granulation tissue, healing time, intensity of epithelialization, the wound area, the shape and area of the scar, possible adverse reactions and cosmetic outcome. The patients were compared in age, weight, height, phototype, histological type of lesion. The phenytoin showed better therapeutic and cosmetic results compared with the cream base (control). Origem e distribuição do plexo lombossacral de javali (Sus scrofa - Linnaeus, 1758)

Objetivou-se estudar a origem e distribuição do plexo lombossacral de Sus scrofa. Foram utilizados 30 exemplares, sendo 13 fêmeas e 17 machos, após serem fixados em solução aquosa de formaldeído a 10%. Os javalis possuem cinco ou seis vértebras lombares e quatro sacrais. O plexo lombossacral origina-se dos segmentos medulares de L2 a S4. O nervo cutâneo femoral lateral de L2 a L4 ramifica-se nos músculos psoas maior e menor, oblíquo interno do abdome, linfonodos subilíacos e pele da região da articulação do joelho. O nervo femoral de L3 a L6 supre os músculos psoas maior, psoas menor, ilíaco, quadríceps femoral, pectíneo, emite nervo safeno que inerva o músculo sartório e fáscia e pele das articulações do joelho e tarso. O nervo obturatório de L3 a L6 distribui-se para os músculos obturador externo, pectíneo, grácil e adutor. Nervo glúteo cranial de L4 a S1 inerva os músculos glúteo médio e piriforme. Nervo glúteo caudal de L6 a S2 emite ramos para o músculo glúteo superficial. Nervo isquiático de L4 a S2 inerva os músculos tensor da fáscia lata, glúteo médio, glúteo profundo, glúteo acessório, gêmeos, quadrado femoral, semitendíneo, semimbranáceo, bíceps femoral. O nervo tibial supre os músculos gastrocnêmico, sóleo, poplíteo e flexores profundos dos dedos na região plantar divide-se em nervos plantares medial e lateral, os quais ramificam em digitais plantares. O nervo fibular comum divide-se em nervos fibular superficial e fibular profundo. O fibular profundo emite ramos para os músculos tibial cranial, fibular terceiro, extensor longos dos dedos, fibular longo, extensor lateral dos dedos e interósseo. O nervo fibular superficial supre a fáscia da articulação do tarso continua como nervos digitais dorsais. O nervo pudendo de S2 a S4 distribui-se para músculos, esfíncter externos do ânus, levantador do ânus, constritor da vulva, isquiocavernoso e bulboesponjoso, emite os ramos dorsal do clitóris e mamários que inervam clitóris e pele da região vulvar respectivamente, o ramo dorsal do pênis distribui-se na glande pênis, prepúcio e escroto. O nervo retal caudal de S2 a S4 distribui-se para os músculos levantadores do ânus, coccígeo e esfíncter externo do ânus. A formação do plexo lombossacral, assim como origens e distribuições dos nervos que o compõem são similares aos descritos na literatura alusiva aos animais domésticos. / It aimed to study the origin and distribution of the lumbosacral plexus of Sus scrofa. 30 specimens were used, with 13 females and 17 males, after being fixed in an aqueous 10% formaldehyde. Boars have five or six lumbar vertebrae and four sacral vertebrae. The lumbosacral plexus originates from the spinal cord segments from L2 to S4. The lateral femoral cutaneous nerve from L2 to L4 branches off to the psoas major and minor muscles, internal oblique of the abdomen, sub-iliac lymph nodes and the knee joint region skin. The femoral nerve L3 to L6 supplies the major psoas muscle, iliac minor psoas, femoral quadriceps, pectineus, emits saphenous nerve that innervates the sartorius muscle and fascia and skin of the knee and tarsus joints. The obturator nerve from L3 to L6 is distributed to the external obturator muscles, pectineus, gracilis and adductor. The gluteal cranial nerve from L4 to S1 innervates the gluteus medius and piriformis muscles. The gluteal caudal nerve from L6 to S2 emits branches to the superficial gluteal muscle. The sciatic nerve from L4 to S2 innervates the tensor muscles of the fascia lata, gluteus medius, deep gluteus, gluteus accessory, twins, femoral square, semitendinosus, semimembranosus, biceps femoris. The tibial nerve supplies the gastrocnemius, soleus, popliteal and the deep finger flexors in the plantar region that is divided into medial and lateral plantar nerves which branch off into digital plantar. The common fibular nerve is divided into superficial nerves and deep fibular. The deep fibular nerve emits branches to the cranial tibial, third fibular, long extensor of fingers, peroneus longus, lateral extensor digitorum and interosseous muscles. The superficial fibular nerve supplies the fascia joint of the tarsos, which continues as dorsal digital nerves. The pudendal nerve from S2 to S4 is distributed to muscles, external anal sphincter, levator ani, constrictor of the vulva, ischiocavernosus and bulbospongiosus, emits the dorsal branches of the clitoris and of the breast, that innervate the clitoris and skin of the vulvar region, respectively. The dorsal branch of the penis is distributed in the glans penis, prepuce and scrotum. The rectal caudal nerve from S2 to S4 is distributed to the levator ani, coccyx and external anal sphincter. The formation of the lumbosacral plexus, as well as sources and distributions of the nerves that form it, are similar to those described in the literature alluding to domestic animals. / Tese (Doutorado)
