Participaram da pesquisa um total de (n=76) sujeitos, (n=50 turno noturno e n=26 turno diurno), distribuídos nas seguintes categorias: enfermeiros, auxiliares de enfermagem e técnicos de enfermagem, que trabalhavam no Hospital das Clinicas da Universidade Estadual de Campinas. A idade média foi de 42 anos. O horário de trabalho do turno noturno era das 19h00min até às 07h00min do dia seguinte e do turno diurno, das 07h00min às 13h00min ou das 13h00min até às 19h00min. Os instrumentos utilizados para a coleta de dados foram: o diário de sono (coletado durante 30 dias); questionário de informações pessoais; uma balança para pesagem e mensuração da altura para o cálculo do índice de massa corporal (IMC), e para classificar o IMC utilizou-se a tabela de BRAY (1998). Os dados do peso foram coletados em dois momentos caracterizados por peso 1, altura 1 e IMC 1 do registro da Divisão de Segurança e Saúde Ocupacional da UNICAMP, correspondendo ao controle de saúde do trabalho no período de 2003. O segundo momento em 2005, correspondendo ao peso 2, altura 2 e IMC 2. Os resultados da análise dos padrões do sono dos grupos diurno e noturno mostraram dados estatisticamente significantes (Teste de Mann-Whitney) para a latência do sono (p=<0,001), horário de acordar (p=<0,001), duração do sono noturno (p=0,002), qualidade do sono noturno (p=0,009) e presença de cochilos (p=0,001). Constatou-se pelo teste de correlação de Spearman que houve diferença significativa estatisticamente. (r=-0,296; p=0,0366) dado que, conforme aumenta a freqüenta da prática de atividade física, diminui a diferença de peso e IMC. Comparando os resultados do aumento de peso associados com o tempo de trabalho em turno noturno observou-se que não houve diferença significativa pelo teste Mann-Whitney (p=0,302), no entanto, os valores das médias do tempo de trabalho dos grupos de redução e aumento de peso mostraram diferenças (11,18 anos e 13,87 anos respectivamente). Verifcou-se que os sujeitos do grupo do turno noturno apresentaram média de IMC maior em comparação ao grupo diurno com diferenças estatisticamente significantes (teste Mann- Whitney). Entretanto,não se pode afirmar que as alterações de sono estiveram presentes influenciando o ganho de peso no grupo do noturno / Abstract: The main goals of this work were: evaluate the individuals? characteristics of the sleep-wake cycle compare the variation in weight, in the period of two years, among individuals working in the day-shift and in the night-shift; investigate the existence of any relationship between the working time in the night-shift and change in weight and BMI as well as investigate any relationship between change in weight and BMI and the variables of the sleep-wake cycle which in this study are taken as latency, quality of sleep in the night, sleep duration and napping. This research counted on a number of participants of (n=76) people, (n=50 of the night-shift and n=26 of the day-shift), from several distinct professions: nurses, nursing technician and nurses aides that worked in the Hospital das Clínicas da Universidade Estadual de Campinas. The average age of the group was 42 years. The working time of the night-shift was from 19h00min to 07h00min of the following day, and the working time of the day-shift was from 07h00min to 13h00min or from 13h00min to 19h00min. The instrumentation used to collect the date was: the diary of sleep (which was filled in with data for 30 consecutive days); questionnaire on personal information related to life style; a balance to measure weight and height to get data for BMI calculation, and the BRAY (1998) table to classify the BMI. The data related to weight were collected in two distinct points in time; the first instant is represented by weight-1, height-1 and BMI-1 of the records from Occupational Health and Safety Division at UNICAMP, respective to the Labor Health control of the period of 2003. The second instant, in 2005, is represented by weight-2, height-2 and BMI-2. The results of the analysis of the night- shift group and day-shift group sleep patterns lead to data statistically significative (Mann-Whitney test) for the latency of sleep (p=<0,001), wake up time (p=<0,001), night sleep duration (p=0,002), night sleep quality (p=0,009), and occurrence of napping (p=0,001). It was found through the test of correlation of Spearman that there was a statistically significant difference (r=-0,296; p=0,0366), given that the difference in weight and the IMC decrease as it increases the frequency of physical activity. From the comparison among the results of the weight increase associated with time of work in the night-shift, it was noticed that there was not significant difference by means of the test of Mann-Whitney (p=0,302), however, the average time of work of the groups with weight reduction and with weight increase showed differences: (11,18 years and 13,87 years respectively). It was noticed that the individuals from the group of night-shift lead to average IMC higher than the ones from the group of day-shift with statistically significant differences (test of Mann-Whitney). Os objetivos foram: verificar a presença de queixas relativas ao sono, comparar a qualidade subjetiva do sono, identificar o padrão habitual de sono dos sujeitos antes e após a realização da ginástica laboral. Esse estudo foi quantitativo e observacional. Instrumentos utilizados: Ficha de Identificação, Questionário de Sono, Diário do Sono, Índice de Qualidade do Sono de Pittsburgh e Questionário da Ginástica Laboral. O programa de ginástica laboral era composto por exercícios de alongamento, massagem e atividades lúdicas. Foram realizadas quarenta aulas, às 2ª, 4ª e 6ª feiras, durante três meses. As aulas tinham a duração de quinze minutos e cada exercício físico trinta segundos. A população estudada foi constituída por 17 trabalhadores do turno noturno, dez mulheres e sete homens; 41,1% pertenciam à faixa etária de 26 a 30 anos, 58,8% optaram por trabalhar nesse turno e 82,3% não pretendiam mudar de turno de trabalho. Resultados após a realização da ginástica laboral: melhora na qualidade do sono, alteração no padrão do sono, diminuição do tempo de latência para o sono e dos cochilos. A maior parte dos trabalhadores do turno noturno referiu melhora do seu bem-estar diário, do sono e do seu estilo de vida. Concluímos que a ginástica laboral associada a um programa de exercícios físicos pode diminuir os efeitos colaterais advindos do turno noturno / Abstract: The present study analyzed the effects of the stretch break on complaints associated to the sleep of night shift workers in a plastic manufacturing company in Sorocaba-SP. The purposes were: checking the existence of complaints associated to sleep, comparing the sleep subjective quality, identifying the habitual sleep pattern of the subjects before and after the participation in the stretch break program. This study was a quantitative and observational one. Instruments utilized: Identification card, Sleep Questionnaire, Sleep Diary, Pittsburgh Sleep Quality Index, and Stretch Break Questionnaire. The stretch break program was made by stretching exercises, massage and entertainment activities. Forty sessions were performed on Mondays, Wednesdays, and Fridays, for three months. The classes were fifteen minutes long when each exercise lasted for thirty seconds. The population studied was made of 17 night shift workers, 10 women and 7 men; 41,1% belonged to the 26 to 30 year old age bracket, 58,8% chose to work this shift and 82,3% have no intention to change shifts. Results after the workplace exercise sessions: Improvements in the sleep quality, sleep pattern alteration, decrease in the sleep latency time and number of naps. Most of the night shift workers reported an improvement in their daily well-being, sleep quality and life style. Bakgrund: Omvårdnad ska bedrivas på individnivå med fokus på fysisk miljö, socialt stöd och de förmågor patienten själv har. Ambulanspersonal ska kunna upprätthålla en god omvårdnad, både fysik och psykisk, oavsett förutsättningar och arbetsmiljö. Sjuksköterskan har ett ansvar för att en säker och en hälsofrämjande miljö upprätthålls. Syfte: Syftet med föreliggande studie var att beskriva omvårdnaden inom ambulanssjukvården under olika delar av dygnet. Metod: Denna studie är av kvalitativ design, och sammanlagt elva semistrukturerade intervjuer har genomförts med grundutbildade sjuksköterskor samt sjuksköterskor med specialistutbildning mot ambulanssjukvård verksamma inom den prehospitala vården. En kvalitativ innehållsanalys har använts för att analysera det insamlade materialet. Huvudresultat: Omvårdnad inom ambulanssjukvården påverkas både positivt och negativt beroende på tiden av dygnet den utförs. Två teman framkom, vilka var Utföra god omvårdnad och Utföra bristande omvårdnad. Utifrån det första temat framkom det att omvårdnad under dagtid samt i början av arbetspass ansågs vara god, medan det utifrån det andra temat framkom att omvårdnaden under nattetid och i slutet av arbetspassen ansågs kunna bli bristfällig. Slutsats: Under vissa delar av dygnet utför ambulanspersonal bristande omvårdnad, som i sin tur kan leda till risker kring patientsäkerheten. Det är av stor vikt att ambulanspersonal ger en god omvårdnad dygnet runt. Detta för att kunna upprätthålla en god patientsäkerhet samt minimera potentiella risker för patienten. / Background: Nursing care should be conducted on an individual level with a focus on physical environment, social support and the patients own abilities. Ambulance staff should be able to give good care, both physical and mental, regardless preconditions and environment. The prehospital care is performed 24 hours a day. The nurse has a responsibility for ensuring that a health-promoting environment is maintained. Aim: The aim of this study was to describe the ambulance care during different parts of the day. Method: This study has a qualitative approach. A total of eleven semi-structured interviews have been carried out with registered nurses and specialist nurses specializing in ambulance care active within the prehospital care. A qualitative content analysis has been used to analyze the collected material. Main result: The ambulance care was affected both positive and negative due to the time of the day it was performed. Two themes emerged from the result. The first was Perform good care and the second was Perform insufficient care. The first theme included that the ambulance care that was performed during day time and in the beginning of the shift was considered positive, while the second theme included ambulance care that was performed during night time and in the end of the shift was considered negative. Conclusion: During some parts of the day, ambulance staff perform inadequate care, which in turn can lead to patient safety risks. It is important that the ambulance staff provides good nursing 24 hours a day. This in order to maintain good patient safety and minimize potential risks to the patient.

Efeito do trabalho em turnos nos hábitos alimentares : uma revisão sistemática

O trabalho por turnos é definido como aquele realizado fora dos horários considerados “convencionais”, por exemplo: pelo trabalho no turno noturno ou o trabalho de forma contínua, através do revezamento de equipes. Durante as últimas décadas, a proporção de trabalhadores que executam as atividades em escalas de turnos vem aumentando e, com as mudanças nas condições de trabalho, a organização do ambiente social, familiar, dos hábitos de vida e das necessidades básicas desses trabalhadores passou por significativas transformações, que podem causar danos saúde. O desenvolvimento de doenças crônicas não transmissíveis e distúrbios metabólicos em trabalhadores de turnos já é bastante evidenciado na literatura, e os estudos epidemiológicos demonstram associações consistentes entre o trabalho por turnos e a ocorrência de doenças, como obesidade, diabetes e síndrome metabólica. Além da relação entre alterações dos ritmos biológicos, disruptura do sistema circadiano e alterações metabólicas, mudanças comportamentais experienciadas pelos trabalhadores de turnos são apontadas como potenciais fatores de risco adicionais ao desenvolimento de doenças. Dentre as alterações nos hábitos de vida, a alimentação parece ser alterada em decorrência do trabalho por turno. Mudanças nos horários de sono, vigília, da atividade laboral, bem como na disponibilidade de alimentos e tempo para realização das refeiçoes em locais adequados, são fatores determinantes nas escolhas alimentares desses indivíduos. Ainda que estudos voltados à avaliaçao da alimentação de trabalhadores de turnos tenham sido propostos e bastante discutidos na literatura, a real influência do turno de trabalho nos hábitos alimentares é pouco elucidada. Assim, o objetivo desse trabalho foi revisar sistematicamente os resultados de estudos que avaliaram os hábitos alimentares de trabalhadores de turnos, de forma a esclarecer como o turno de trabalho influencia, positiva ou negativamente na alimentação. A sumarização de evidências permite a elaboração de condutas e estratégias nutricionais específicas a esse grupo de risco. Além disso, destaca aspectos importantes a serem considerados no desenvolvimento de futuros estudos, visando contribuir com a qualidade das informações obtidas. / Shift work is defined as the one performed outside the "conventional" hours, for example by working the night shift or working continuously through the team relay. Over the last few decades, the proportion of workers performing turn-based activities has been increasing and, with changes in working conditions, the organization of the social, family, living, and basic needs of these workers has changed significantly which can cause health damage. The development of chronic noncommunicable diseases and metabolic disorders in shift workers is already well documented in the literature, and epidemiological studies demonstrate consistent associations between shift work and the occurrence of diseases such as obesity, diabetes and metabolic syndrome. In addition to the relationship between changes in biological rhythms, disruption of the circadian system and metabolic alterations, behavioral changes experienced by shift workers are indicated as potential additional risk factors for the development of diseases. Among the changes in life habits, food habits seems to be altered as a result of shift work. Changes in sleep schedules, wakefulness, labor activity, as well as food availability and time to make meals in appropriate places, are determining factors in the food choices of these individuals. Although studies aimed at evaluating the feeding of shift workers have been proposed and discussed in the literature, the real influence of work shift on eating habits is little elucidated. Thus, the objective of this study was to systematically review the results of studies that evaluated the eating habits of shift workers, in order to clarify how the work shift influences, positively or negatively in the diet. Summarizing evidence allows the elaboration of specific nutritional strategies and strategies for this risk group. In addition, it highlights important aspects to be considered in the development of future studies, aiming to contribute to the quality of the information obtained.

Effects of shift work on employee retention : an examination of job satisfaction, organizational commitment, and stress-based explanations

Shift work is becoming increasingly prevalent in our society, with 17% of the full-time work force and 36% of the part-time work force working non-standard shifts (Beers, 2000). The goal of this study was to explore the relationships between several shift work variables and retention of employees working in a retail organization that is open 24-hours a day. Results indicated no significant differences between workers in fixed versus mixed shift schedules on job satisfaction or role stress. Contrary to my hypothesis, mixed shift workers reported higher levels of commitment and remained with the organization longer than fixed shift workers. As predicted, night shift workers reported lower levels of job satisfaction and organizational commitment than evening workers. However contrary to predictions, there were no significant differences between morning and night shift workers. Supervisor support did not moderate the relationship between shift work and commitment, satisfaction, or role stress. However is was a significant moderator of the relationship between day versus evening shift and role stress, with individuals working evening shifts and perceiving high levels of supervisor support remaining with the organization the longest.

Sjuksköterskors upplevelser av att arbeta i nattpatrull : är det rena natta att jobba om natta?

Syftet med denna studie var att beskriva sjuksköterskors upplevelser av att arbeta i hemsjukvården och därmed få ökad kunskap och kännedom om villkoren för nattarbete. Studien hade en kvalitativ deskriptiv design och fjorton sjuksköterskor besvarade frågeformulären. Materialet analyserades med hjälp av innehållsanalys. Resultatet utmynnade från ett tema med följande kategorier, nattarbete ett val av praktiska skäl, vidgat perspektiv ger arbetstillfredsställelse och insikten om arbetets komplexitet ger en känsla av otillfredsställelse. Majoriteten av sjuksköterskorna valde att arbeta inom nattpatrullen för att tjäna extra pengar, samt på grund av närheten till arbetet. Samtliga sjuksköterskor påtalade vikten av att ha tillräckligt med tid vid hembesöken. Flertalet män ansåg dock sin kompetens för hög för de arbetsuppgifter de utförde i hemsjukvården. Att de sedan valt att fortsätta arbeta inom nattpatrullen berodde på att de upplevde arbetet som självständigt, fritt och stimulerande. Sjuksköterskorna påpekade att de inte kände sig delaktiga i beslut. Flera påtalade att cheferna inte hade någon förståelse för deras arbetsvillkor. Behovet av hemsjukvård ökar och mer personal kommer att behövas. Att rekrytera och behålla sjuksköterskor inom hemsjukvården är en utmaning. Det är därför av vikt att ta reda på hur vårdpersonal upplever sina arbetsvillkor och vad som kan locka fler att arbeta inom hemsjukvården. / The purpose of this study was to describe nurses' experiences of working in home health care and thus enhance the understanding and knowledge of the conditions of night work. The study had a qualitative descriptive design and fourteen nurses answered the questionnaires. The material was analysed using content analysis. The analysis resulted from a theme with the following categories: night work as a choice for practical reasons, broader perspective gives job satisfaction and the recognition of the complexity of the work gives a sense of dissatisfaction. The majority of the nurses chose to work in night patrol to earn extra money, as well as the close proximity to their work. All the nurses emphasised the importance of having enough time at home visits. Most men, however, considered their skills too advanced for the tasks they performed in home nursing. That they then chose to remain with the night patrol was because they experienced their work as independent, free and stimulating. The nurses said that they felt left out when it came to decisions. Several complained that the managers had no understanding of their working conditions. The need for home care will increase and more staff will be needed. To recruit and retain nurses within home care is a challenge. It is therefore important to find out how health professionals perceive their working conditions and discover what might attract more people to work in home health care.
