Atendimento pré-hospitalar: histórico do papel do enfermeiro e os desafios ético-legais / Pre-hospital care: nurse´s insertion historical and the ethical-legal challenges

Trata-se de um estudo descritivo, exploratório, de natureza histórico-social e ético-legal, desenvolvido a partir da análise documental como parte do método histórico. As bases de dados nacionais e internacionais pesquisadas foram CINAHL, LILACS, MEDLINE, PeriEnf e PubMed, com os descritores: enfermagem, atendimento pré-hospitalar e aspectos ético-legais, publicados nos últimos 10 anos. O método histórico estuda os fatos sociais e suas relações, a partir de documentos, surgidos de fontes, contextos e momentos distintos, e envolve a reconstrução do conhecimento e novas interpretações dos fatos. A escassez de literatura específica, sobre os aspectos ético-legais no atendimento pré-hospitalar (APH) e a inserção do enfermeiro nesta modalidade de serviço, motivou a realização deste estudo. Igualmente, a experiência profissional como enfermeiro e a vivência de situações críticas no Grupo de Resgate e Atendimento de Urgências (GRAU - RESGATE 193), despertaram o interesse pelo tema e as bases para a incursão no campo do Direito. Com este estudo pretendeu-se fazer um resgate histórico do surgimento do APH e a inserção do enfermeiro com os objetivos de levantar a legislação e as funções do enfermeiro no atendimento pré-hospitalar; identificar os desafios éticos e legais do enfermeiro no exercício profissional nesse atendimento; e, traçar o tipo de enfermeiro profissional necessário para o atendimento pré-hospitalar / This is a descriptive, exploratory study of historical-social and ethical-legal nature, developed through a documentary analysis as part of the historical method. National and international data basis were searched at CINAHL, LILACS, MEDLINE, PeriEnf e PubMed utilizing the following descriptors: nursing, pre-hospital care, and ethical-legal aspects, published in the last ten years. The historical method studies social facts and its relations, from documents found in sources, contexts and distinct moments, and involve the knowledge reconstruction and new interpretation of facts. Lack of specific literature on ethical-legal aspects within pre-hospital care and the nurse´s insertion into this kind of service, has motivated this study. Also, the professional experience as nurse and having lived critical situations within the Rescue Group for Urgent Care (GRAU-Resgate 193), has created the interest on the theme and the basis for this into the Law field. This study aimed at making an historical rescue about the emergence of pre-hospital care and the nurse´s insertion in it with the objectives: to survey the legislation and the nurses´ functions in this service; to identify the nurses´ ethical and legal challenges at professional practice; and, to outline the kind of nurse needed for pre-hospital care

La cuestión básica de esta investigación es esclarecer cuáles han sido los procesos de continuidad y transformación que han constituido la identidad profesional de los/as enfermeros/as en España entre 1956 y 1976. Esta investigación guiada por el paradigma crítico social y las perspectivas teóricas postestructuralistas y postfeministas, plantea un análisis de discurso de carácter genealógico de los manuales de moral profesional dirigidos a la formación de los/as Ayudantes Técnicos Sanitarios y, de entrevistas a profesionales que durante este período fueron estudiantes, profesoras/es y enfermeros/as.La constitución de la identidad profesional se ha formado por discursos y relaciones de poder dominantes, como los discursos de género, técnico y biopatológico, y relaciones de poder disciplinario, pastoral y de autogobierno; y transformadores como el paso de un discurso de moral cristiana a un discurso laico y social, y la emergencia de discursos de concienciación profesional y resistencia. Todo ello permite apuntar una serie de implicaciones para la actualidad profesional, relativas al cómo somos y al porqué somos como somos.

Motivation for carrying out this study is due, firstly, to a liking for history and, secondly, to the nonexistence of records on Nursing in one of the institutions with the most teaching tradition in the area of health and nursing in the state of Rio Grande do Norte. The objectives of the study were to analyze the historical development of Nursing at the HUOL and its association with the teaching of Nursing at the UFRN; describe the historical development of the Hospital referred to; and to establish a relationship between the development of nursing at the HUOL and the teaching of Nursing at the UFRN. Empirical investigation was carried out based on the study of historical documents such as reports, minutes, letters, by-laws, decrees and administrative directives, as well as photographs and interviews with people who lived through this history or who kept vivid memories of it. From this research it can be gathered that Nursing at the HUOL was at first closely identified with the empirical stage of the profession. Its development is a result of the institutionalization of teaching whose starting point is an authorization for running the Nursing Aid School of Natal in 1955. Since then, gradually, teaching has enabled those who practiced nursing at that institution to become professionals through a partnership between the Nursing Aid School and the Nursing Department at the Hospital whose administration had been in charge of a professor for many years. Upon the creation of the undergraduate program in Nursing in 1973, nursing at the HUOL underwent a new transformation process with new nurses being hired. Likewise, the creation of post-graduate specialization and master s degree programs in 1982 and 1996, respectively, opened the way to the growth of the academic qualifications of nurses at that institution. Therefore, it must be asserted that Nursing at the HUOL has, over the years, gone through a continuous process of qualification of its members and in such trajectory the teaching of Nursing that is carried out at different levels at the UFRN stands as a hallmark / Trata-se de um estudo de enfoque hist?rico com abordagem qualitativa, tendo como principal prop?sito investigar a trajet?ria da Enfermagem do Hospital Universit?rio Onofre Lopes (HUOL) e sua rela??o com o ensino de Enfermagem da Universidade Federal do Rio Grande do Norte (UFRN). The History of the Baylor University School of Nursing, 1909-1950

This study traces the development of the Baylor University School of Nursing from its beginning in 1909 through the establishment of the baccalaureate nursing program in 1950. Primary data including official records of the School of Nursing, minutes of the Baylor University Board of Trustees, reports of the School of Nursing to accrediting agencies, and interviews of former students and deans were examined using the historical research techniques of external and internal criticism. A review of the literature that is relevant to the development of nursing education is presented in Chapter II. Chapter III presents the events in the development of the Baylor University School of Nursing. Chapter IV discusses the accreditation criteria which influenced the development of the School of Nursing. Chapter V discusses the curriculum, teaching methods, and faculty qualifications. Chapter VI discusses the people who were the most influential in the development of the School of Nursing. Implications of the study include the recognition that nursing education and nursing service have differing priorities. Conflicts between the needs of patients for care and the needs of students for education arose when the Superintendent of Nurses was responsible for both areas. Usually the needs of patients for care took priority over the needs of students resulting in long hours and less than optimum learning conditions for students. External factors played a major role in the development of nursing education by determining the needs of society for nurses and thus for nursing education. Accreditation criteria established by state and national agencies determined the standards by which the School of Nursing was judged. National accreditation had more impact on the School since higher standards were required. The leadership of the School played a significant role in the development of the professional nursing program. Leaders who had educational preparation and vision for the future of nursing education with an educational institution rather than a service institution worked to establish an improved educational system in nursing.

Trata-se de um estudo de natureza histórico-social, cujo objeto é as estratégias de luta das enfermeiras obstétricas pela ocupação do espaço hospitalar de assistência ao parto durante o processo de implantação da assistência humanizada na Maternidade Leila Diniz. A delimitação temporal do estudo abrange o período de 1996 a 1998. Os objetivos da pesquisa são: Descrever as circunstâncias da inserção das enfermeiras obstétricas na assistência ao parto humanizado na Maternidade Leila Diniz; Analisar as estratégias de luta utilizadas pelas enfermeiras obstétricas para implantar as práticas humanizadas de assistência ao parto na Maternidade Leila Diniz; e Discutir os efeitos das estratégias de luta utilizadas pelas enfermeiras obstétricas para implantar as práticas humanizadas de assistência ao parto na Maternidade Leila Diniz. O estudo apóia-se teoricamente nos conceitos desenvolvidos pelo sociólogo Pierre Bourdieu e utilizou o método da história oral temática. Na análise, houve a articulação de documentos escritos e depoimentos orais. Os resultados da pesquisa evidenciam que a enfermeira obstétrica participou como agente estratégico da Secretaria Municipal de Saúde do Rio de Janeiro com o objetivo de divulgar e implantar o modelo humanizado na Maternidade Leila Diniz; desenvolveu estratégias de luta simbólica eficientes para a ocupação do campo pela enfermeira obstétrica e neste processo de reconfiguração da assistência ao parto na Maternidade Leila Diniz, ganhou autonomia no campo obstétrico hospitalar. / This is a social historic study, wich the object in the strategies of obstetrics nursing fight for the space that occupation on hospital of childbirth assistance during the implantation of process assistance on Leila Diniz maternity. Nowadays the status of a medical nurse represents a fairly actual topic. It is a profession where many changes had to be undertaken during a few years beginning with scholars education up to a graduation of a nurse and their responsibilities. A nurse not only plays the role of a caretaker and a helper but is also an individually thinking professional who has received a proper education and has his or her competence bound by the jurisdiction. The theoretical part focuses on the status of a nurse from the sociological viewpoint, on its development from a historical perspective, on changes taking place in the present and stretching to the future and on the trends we might expect in this profession. The practical part contains information obtained by the research which focused on the perception of the status of a nurse by patients, nurses and physicians. The aim of this graduation research was to question nurses in the Czech Republic about their contentment with their current status and explore their future visions, furthermore to obtain opinions on the topic from both patients and physicians. The H1 hypothesis was: nurses are not content with their current social status, was proved correct. The H2 hypothesis was: the ideas of a nurse about his or her future status in the society are positive was proved correct. The H3 hypothesis was: the assessment of the status of a nurse by a patient is negative, wasn´t correct. The H4 hypothesis: physicians do not follow the current status of a nurse, correct. The H5 hypothesis was: physicians do not display any interest in following the status of a nurse in the future was correct.
