Bakgrund: I takt med det normala åldrandet försämras kroppens funktion, vilket gör att flera äldre behöver hjälp och stöd med sin dagliga omvårdnad såsom munvård. Studier har visat att munvård hos äldre på särskilt boende är ofta bristfällig, med munhälsoproblem till följd, därför behöver kunskap och attityd kring munhälsa sammanställas. Syfte: Syftet med denna litteraturöversikt är att sammanställa vårdpersonalens kunskap och attityd kring munhälsa på särskilt boende för äldre samt vilka åtgärder som anges kring förbättring av munvård. Metod: Studien genomfördes som en litteraturöversikt. Resultatet är baserad på 15 vetenskapliga artiklar varav n=8 kvantitativ och n=7 kvalitativ. Artikelsökning ansats har genomförts i databaser CINAHL och PubMed. Resultat: Resultatet i denna litteraturöversikt visade kunskapsbrist i munvård bland vårdpersonal samt faktorer som påverkar vårdpersonalens attityd av att utföra god munvård. Munvård utbildningsprogram och munvårdsbedömning visade förbättringar i vårdtagarens munstatus samt ökad vårdpersonalens kunskap kring munvård. Slutsats: Vårdpersonalens kunskap och attityd till munvård var de största bidragande faktorerna till att munvård inte utförs effektivt. Däremot har munvårdsutbildning visat positivt resultat hos vårdtagaren / Background: The normal aging process deteriorates the body`s function, which means that more elderly people need help and support with their daily care such as oral care. Studies have shown that oral care for elderly people in nursing homes is often inadequate, and always result to oral health problems. Because of this, knowledge and attitudes regarding oral health need to be compiled. Aim: The aim of this literature review is to compile nursing staffs` knowledge and attitude towards oral health in nursing homes and what measures have been taken regarding improvement of oral care. Materials and methods: This study was carried out as a literature review. The result is based on a total of 15 scientific articles, n=8 quantitative and n=7 qualitative. These articles were collected from databases CINAHL and PubMed. Results: The results in this study revealed lack of knowledge in oral care among nursing staff in nursing homes and factors affecting nursing staffs' attitudes towards performing good oral care. Educational programs and oral assessment showed significant improvements in the care residents oral status and increased nursing staff's knowledge in oral care. Conclusion: Nursing staffs’ knowledge and attitude to oral care were the main contributing factors to the fact that the oral care was not performed efficiently. On the other hand, oral care education has shown positive improvements in care residents.

Oral Care Assistance at Geriatric Homes in the County of Västerbotten, Sweden : A Questionnaire Interview

The elderly population in Sweden is growing and as a result of that, society is facing different types of challenges. Oral hygiene is important for preventing oral disease but also for the general health (GH). A goal for the Swedish dental society is that every inhabitant should have an equal right to dental-care and oral health (OH) under the same conditions. This is not only a challenge for dental staff but also for medical and healthcare personnel. Our hypothesis is that nursing staff do not possess the education needed regarding oral hygiene, especially not in relation to GH. The opinions regarding support for nursing staff differ between nursing staff and managers. This study was conducted through questionnaire interviews with managers and nursing staff at geriatric homes in three municipalities in the county of Västerbotten. Questions were asked about education and routines, performance of oral care, and OH linked to GH. Response frequencies were compared between municipalities, professions, and response options. A significant lower part (p <0.001) of nursing staff found that non-existing oral care could cause suffering in the form of general disease than in the form of oral disease. The opinion differs between managers and staff in questions about established routines (p <0.05). This study indicates that knowledge about relationships between oral and general health might be inadequate and that education is both wanted and needed among nursing staff. More extensive studies, and studies about how dental professions could support nursing staff in a better way is desirable.

Organisation, practice and experiences of mouth hygiene in stroke unit care: a mixed methods study.

no / Aims and objectives To (1) investigate the organisation, provision and practice of oral care in typical UK stroke units; (2) explore stroke survivors', carers' and healthcare professionals' experiences and perceptions about the barriers and facilitators to receiving and undertaking oral care in stroke units. Background Cerebrovascular disease and oral health are major global health concerns. Little is known about the provision, challenges and practice of oral care in the stroke unit setting, and there are currently no evidence-based practice guidelines. Design Cross-sectional survey of 11 stroke units across Greater Manchester and descriptive qualitative study using focus groups and semi-structured interviews. Methods A self-report questionnaire was used to survey 11 stroke units in Greater Manchester. Data were then collected through two focus groups (n = 10) with healthcare professionals and five semi-structured interviews with stroke survivors and carers. Focus group and interview data were recorded, transcribed verbatim and analysed using framework approach. Results Eleven stroke units in Greater Manchester responded to the survey. Stroke survivors and carers identified a lack of oral care practice and enablement by healthcare professionals. Healthcare professionals identified a lack of formal training to conduct oral care for stroke patients, inconsistency in the delivery of oral care and no set protocols or use of formal oral assessment tools. Conclusion Oral care post-stroke could be improved by increasing healthcare professionals' awareness, understanding and knowledge of the potential health benefits of oral care post-stroke. Further research is required to develop and evaluate the provision of oral care in stroke care to inform evidence-based education and practice.

ABSTRACT  Background: During the past years the authorities and the dental care in the world have begun to realize the importance of maintaining a good oral health of the seniors. The care staff at special residences have an essential role in the daily oral care of the elderly. The oral state of health is also important for the rest of the health. Studies show that oral care is deficient in the elderly and needs to be highlighted as the elderly population is expected to increase.   Aim: The aim of this literature review is to evaluate the potential improvement regarding the staff's work with oral care in elderly care.  Methods: Literature study search was performed with MeSh terms in the database PubMed and Scopus. The following keywords have searched:Dental care for aged, Education, Nursing homes, Nursing staff, Oral health, Oral care.    Results: Several studies have found improvements that are beginning to emerge to develop the oral care of the elderly. These improvements can be achieved by a convenient oral care training that is designed for the elderly. The training programs for the care staff working with the elderly positively affects their attitudes, routines and documentation of the patient’s information, which benefits the elderly oral health.    Conclusion:To achieve good oral care for the elderly, several improvements are needed to be done by the governments involving nursing and working staff´s routines. A little different has been done, but there is significant work still to be done to improve the elderly dental health.

Specialistsjuksköterskor på IVA kan lindra lidande hos patienten som vårdas oralt intuberad genom munvårdsåtgärder : En kvantitativ litteraturstudie

Bakgrund: Patienten som vårdas oralt intuberad på intensivvårdsavdelning får successivt försämrad munhälsa. Endotrakealtuben orsakar muntorrhet, risk för trycksår samt ansamling av bakterier och plack, vilket innebär ett lidande för patienten i form av påverkad munhälsa. Studien fokuserar på specialistsjuksköterskans roll att förebygga och lindra patientens lidande genom att undersöka munvårdsåtgärder som påverkar munhälsa. Syfte: Att sammanställa munvårdsåtgärder som påverkar munhälsa hos patienter som vårdas oralt intuberade. Metod: Kvantitativ litteraturstudie med systematisk ansats. Munvårdsåtgärdernas påverkan på munhälsa har i studierna undersökts genom olika utfallsmått och metoder. Resultatet av munvårdsåtgärdernas påverkan på munhälsa sammanställdes genom kategorierna munskölj, mekanisk rengöring, munvårdsprotokoll och fixation av endotrakealtub, uppdelat på utfallsmåtten bakterier och plack samt munslemhinnans status. Resultat: De munskölj som undersöktes resulterade i varierat positiv, utebliven och negativ påverkan på bakterier och plack samt munslemhinnan. Alla metoder av mekanisk rengöring hade positiv påverkan på munhälsa. Munvårdsprotokoll med individanpassad frekvens och ompositionering av endotrakealtuben hade positiv påverkan på munhälsa. Fixationer av endotrakealtuben påverkade munslemhinnan både positivt och negativt. Slutsats: Munvårdsåtgärder påverkar munhälsa hos patienten som vårdas oralt intuberad med både positiv, utebliven och negativ påverkan. Specialistsjuksköterskan kan förebygga samtlindra patientens lidande genom bedömning av munhälsan och tillämpning av individanpassade munvårdsåtgärder. Eftersom munvårdsåtgärder även kan leda till negativ påverkan på munhälsa är det av stor vikt att specialistsjuksköterskan även utvärderar effekten av tillämpade munvårdsåtgärder. / Background: Patients who is orally intubated in intensive care units gradually deteriorates oral health. Endotracheal tubes causes dry mouth, pressure ulcers, bacteria and plaque, which implies suffering from affected oral health. The study focuses in the specialist nurse's role in preventing and alleviating patients’ suffering by examining measures of oral care whichaffects oral health. Aim: To compile measures of oral care that affect oral health in patients who are orally intubated. Method: Quantitative literature review with systematic approach. The impact of oral care on oral health has in the studies been measured through different outcomes and methods. The result of the oral cares’ impact on oral health were compiled through the categories mouthwash, mechanical cleaning, oral care protocol and endotracheal tube fixation, divided by the outcomes bacteria and plaque and the status of oral mucosa. Results: Mouthwashes resulted in varied positive, absent and negative effects on bacteria and plaque as well as the oral mucosa. All methods of mechanical cleaning had a positive effect on oral health. Oral care protocols with individualized frequency and repositioning of the endotracheal tube had a positive effect on oral health. Endotracheal tube fixations affected the oral mucosa both positively and negatively. Conclusion: Oral care affects oral health of patients who are orally intubated with both positively, absent and negatively effect. Specialist nurses can prevent and alleviate thepatient's suffering with assessment of oral health and individualized oral care. It is of utmost importance that the nurse evaluates the effect since oral care also can result in negative impact on oral health.
