Using the International Classification of Functioning, Disability and Health (ICF) to enhance healthcare communication : an action research project with an acute stroke serviceTempest, Stephanie Elaine January 2014 (has links)
Background: Effective communication is key to team working in healthcare. It can be negatively impacted upon by existing cultures, logistical challenges, role confusion, and a lack of collaborative approaches to practice. Clinical guidelines recommend using the International Classification of Functioning, Disability and Health (ICF) to aid communication within stroke teams. Yet no empirical evidence exists on the process or outcomes of such implementation. Aims: This project aimed to explore ways the ICF could be used with an acute stroke service and identify key learning from the implementation process. Methods: Using an action research framework, iterative cycles were used within exploratory, innovatory and reflective phases. Content analysis was used to map patient notes’ entries to ICF categories. Thematic analysis was undertaken, using a model of immersion and crystallisation, on data generated via interview and focus group, e-mail communications, minutes from meetings, field notes and a reflective diary. Descriptive statistics were used to analyse quantitative questionnaire data. Data from all sources were combined to determine key findings. Findings: Participants chose to develop an ICF-based team transfer of care report with an ICF glossary to aid completion. Five overall themes were determined; the need to: (1) adopt the ICF in ways that met local service needs; and (2) adapt the ICF language and format. Once implemented, the ICF: (3) fostered communication within and beyond the stroke team; (4) promoted holistic thinking; and (5) helped to clarify team roles. Conclusions: These are the first empirical findings within stroke services that demonstrate how to make the ICF a clinical reality. Participants needed to adapt and own the ICF to adopt it. When implemented, it enabled specific team communication challenges to be overcome. The use of action research to implement the ICF has facilitated sustained change and improvements to communication, thus benefiting patient care.

need for change and making knowledge an essential tool. In this context, knowledge
management has been an essential factor for organizations. Another relevant change are the
organization of work models that present in the productive organization help knowledge
management. This occurs in all segments, including the poultry sector. This dissertation
addresses this question by analyzing the relationship between knowledge management based
on the Terra model and the organization of work in a company in the poultry sector. To do it,
there was a literature, to support models of Knowledge Management and Organization of
Work. The methodology used was classified as applied nature, quantitative and qualitative
approach and descriptive and exploratory objective, using the case study. Data collection was
conducted through on-site observation and questionnaire. For data analysis we used
descriptive statistics, factor analysis, qualitative data analysis and explanations of the
observations when appropriate. The results showed that the company uses researched
practices of the organization of the Taylorist-Fordist work model, considering the prescriptive
features and control in making the product, but with a degree of flexibility in their social
relations work. Knowledge management practices are concentrated in positions of middle
management, as the strategies and information systems. The organizational structure, human
resources policies and measurement results are not focused on knowledge management needs
improvement. The result arising from factor analysis to identify the relationship between
knowledge management and the organization of work submitted eleven factors, where five of
these contains correlation of the two concepts. The first pointed to a relationship between the
integration of work and culture, some strategies and HR policies. The second identified that
there is information system functions and measuring results by interfering in workers pay.
The third showed that the organizational structure influences interfere in training and in
quality control. The fourth pointed out that the pace of work influence on team building. And
the fifth showed an interaction of prescriptive features with the goals set by the company.
Thus, it was concluded that the way the organization of labor used in the company exerts
great influence on knowledge management from the simplest activities and firmer
construction. It is recommended that are subsequently carried out research with the company's
managers and in other production units and establish a comparison to determine if the results
are present throughout the organization. / Com os avanços tecnológicos e transformações do mercado, as organizações tem se
deparado com a necessidade de mudanças e tornando conhecimento uma ferramenta
essencial. Nesse contexto, a gestão do conhecimento tem se mostrado um fator
imprescindível para as organizações. Outro ponto relevante de mudança são os modelos
de organização do trabalho que presentes na organização produtiva auxiliam a gestão do
conhecimento. Isso ocorre em todos os segmentos, inclusive no setor avícola. Esta
dissertação aborda esta questão ao analisar as relações existentes entre a gestão do
conhecimento com base no modelo Terra e a organização do trabalho em uma empresa
do setor avícola. Para realizá-la, fez-se um levantamento bibliográfico, para
fundamentar os modelos de Gestão do Conhecimento e Organização do Trabalho. A
metodologia utilizada foi classificada com de natureza aplicada, abordagem quantiqualitativa
e objetivo descritivo e exploratório, utilizando o estudo de caso. A coleta de
dados se realizou mediante observação in loco e a aplicação de questionário. Para a
análise dos dados utilizou-se da estatística descritiva, uma análise fatorial, a análise
qualitativa dos dados e explanações das observações quando cabíveis. Os resultados
apresentaram que a empresa pesquisada utiliza práticas do modelo de organização do
trabalho Taylorista-Fordista, considerando as características prescritivas e de controle
na realização do produto, mas com certo grau de flexibilidade em suas relações sociais
do trabalho. As práticas de gestão do conhecimento concentram-se nos cargos de média
gerência, como as estratégias e os sistemas de informação. A estrutura organizacional,
políticas de Recursos Humanos e mensuração de resultados não estão focadas com a
Gestão do Conhecimento necessita de melhorias. O resultado decorrente da Análise
Fatorial para identificar as relações entre a gestão do conhecimento e a organização do
trabalho apresentou 11 fatores, onde cinco destes contém correlação dos dois conceitos.
O primeiro apontou para uma relação entre a integração do trabalho com a cultura,
estratégias e algumas políticas de Recursos Humanos. O segundo identificou que há
funções do Sistema de Informações e da mensuração dos resultados interferindo na
remuneração dos trabalhadores. O terceiro mostrou que a estrutura organizacional
interfere nos treinamentos e no controle da qualidade. O quarto apontou que o ritmo de
trabalho influencia na formação de equipes. E o quinto apresentou uma interação das
características prescritivas com as metas estabelecidas pela empresa. Assim, concluiu-se
que a forma como a organização do trabalho utilizado na empresa exerce grande
influencia na gestão do conhecimento desde as atividades mais simples às de construção
mais firme. Recomenda-se que sejam realizadas posteriormente pesquisas com os
gestores da empresa e nas demais unidades produtivas e estabelecer um comparativo
para identificar se os resultados obtidos estão presentes em toda a organização.

Background: Considering the growing popularity of gig organisations and the socioeconomic situation enabling precarity in this type of firms, there is the need to investigate potential gains and losses of this type of work contract from the organisational side. Aim: The aim is to understand the benefits and losses platform organisations may face by offering precarious work conditions. Apart from economic influence, as on direct labour costs and staff turnover, there are expected to be cultural and social capital losses. Methodology: A qualitative study approach was chosen to discover new details and access the background of the employees. Six food deliverers from three different app-based food delivery companies in Sweden were interviewed to gain a comprehensive understanding of the problem. Findings: The empirical findings indicated that apart from decreasing direct labour costs and high turnover, offering precarious working conditions may deprive organisations of access to a qualified workforce and knowledge pool that the employees bring.

O percurso desta pesquisa perpassa o problema da produção de habitação e sua relação com a indústria da construção civil, suas diferentes estratégias de acumulação, formas de industrialização e de organização do trabalho. Diante da explosão da produção do setor nos anos 2000, busca entender a dinâmica produtiva nos atuais canteiros de obra através do estudo de três empresas construtoras. A análise das condições de produção atuais à luz de sua evolução histórica acaba por pontuar contradições, possibilidades e limites contidos na dinâmica cíclica de desenvolvimento da indústria da construção. / This dissertation outlines the development of the civil construction process in housing production in Brazil throughout the New Republic. It seeks to understand, historically, how the accumulation process in the sector has occurred in view of the advance of productive forces and the conflict between capital and work. The civil construction industry has been examined taking into consideration the particularities of the productive process, materialised in the work sites and characterised by the variety and flexibility of its constraints. This research comprises of the issue of housing production and its relation with the civil construction industry, its diverse accumulation strategies, as well as ways of industrialisation and of work organisation. Bearing in mind the explosion of the production in the sector in the 2000\'s years, it seeks to understand the production dynamics in the existing construction work sites through the study of three main construction companies. The study of the current conditions of the production in the light of its historical evolution results in pointing out contradictions, possibilities and limits contained in the cyclic dynamics of the development of the construction industry.

La presente tesis propone una interpretación de la organización de la producción textil en la Tercera Dinastía de Ur (ca. 2100-2000 a.n.e.) en Mesopotamia. Se centra en las relaciones de género, la división sexual del trabajo, la jerarquización y el estatus para explicar las categorías laborales y los grupos de trabajo especializados. La evidencia utilizada procede esencialmente de textos sumerios de Ur III publicados entre 1972 y 2010. De entre ellos se han seleccionado 100 textos relacionados con la producción de tejidos que se presentan en transliteración y traducción al castellano. / This dissertation proposes an interpretation of how textile production was organised during the Third Dynasty of Ur (ca. 2100-2000 BCE) in Mesopotamia. We concentrate on gender relationships, the sexual division of labour, hierarchy and status to explain job categories and specialised working groups. The sources used are basically Sumerian texts from the Ur III period, published between 1972 and 2010. Among them, we have selected 100 texts related specifically to textile production. All are presented in transliteration and translation into Spanish in this dissertation.

I samband med Covid-19-pandemin har en snabb omställning skett i det svenska arbetslivet och många har övergått från arbete på kontor till arbete hemifrån enligt restriktioner (Folkhälsomyndigheten 2020). Studiens syftet är att öka förståelsen för offentligt anställdas upplevelse av hemarbete i relation till psykosocial hälsa och utifrån det belysa vad som kan tas i beaktning vid arbetsorganisering efter pandemin. Således utgår studien ifrån två frågeställningar där den första berör upplevelsen av hemarbete medan den andra berör en tänkbar framtid i arbetets utformning. Studiens empiri har formats genom semistrukturerade intervjuer, vidare bygger studien på ett fenomenologiskt förhållningssätt där intervjuerna väglett den teori som lyfts upp. Empirin har vidare analyserats med hjälp av olika teorier och begrepp som främst antar ett socialpsykologiskt perspektiv. Resultatet visar att upplevelserna av hemarbete är både positiva och negativa i förhållande till den psykosociala hälsan. Upplevelserna är i stora drag entydiga även om viss variation beroende av individuella faktorer återfinns. Avsaknaden av sociala relationer för både arbetet och välbefinnandet tillsammans med ökad integrering av arbete och fritid har varit de tyngre betonade resultaten. Samtidigt har självständiga arbetsuppgifter visat sig fungera bättre i hemmiljö och medarbetarna upplever en ökad effektivitet och minskad restid som två starka fördelar. / Due to the Covid-19 pandemic the working life has changed dramatically in Sweden where people are restricted to work in their original workplace and instead instructed to work from home (Folkhälsomyndigheten 2020). Through semi structured interviews this study aims to understand employees' experience of home based telework in the public sector, mainly in relation to their psychosocial health, and based on that shed light on how work could be organized post Covid-19. The study utilizes two questions to answer the purpose, where the first one focuses on the employees' experience of home based telework while the second one focuses on how future work could be organized. The empiric is based on a phenomenological approach where the interviews have guided the theory, and it has been further analyzed with the help of different social psychological perspectives. The main result shows that the experience of home based telework has both positive and negative characteristics relative to psychosocial health. The experience is broadly unambiguous although some variation depending on individual factors have been found. The absence of social relations for both work and wellbeing together with an increased integration of work and leisure have been the most strongly emphasized results. Simultaneously, independent work tasks have proven to operate better in the home environment and employees experience increased efficiency and reduced travel time as two major benefits.

Innovare verso un modello per Intensità delle Cure (IdC) offre agli ospedali importanti potenzialità di miglioramento, tanto a livello di qualità delle cure quanto sul piano organizzativo e gestionale. L’introduzione del modello IdC può tuttavia implicare sostanziali modifiche nell’organizzazione sociale del lavoro clinico, modificando relazioni, confini e identità professionali. La tesi esamina i reciproci effetti tra il modello IdC e l’organizzazione sociale del lavoro ospedaliero. Il disegno di ricerca è organizzato in tre fasi: una estensiva analisi documentale, uno studio esplorativo basato su interviste qualitative semi-strutturate, uno caso di studio etnografico in profondità condotto in un ospedale recentemente organizzato secondo il modello IdC. I risultati sono organizzati in tre studi. Il primo mostra come il cambiamento IdC sia stato diversamente interpretato dai manager ospedalieri e dai clinici in prima linea con i pazienti, costituendo così due discordanti ‘narrative di cambiamento’. Il secondo studio si focalizza sugli ostacoli alla creazione di team multidisciplinari negli ospedali IdC. Il terzo studio esamina l’impatto del modello IdC su relazioni e confini professionali tra medici e infermieri. Oltre a contribuire alle teorie socio-psicologiche riguardo a confini e identità professionali, la tesi propone riflessioni concrete su come colmare il divario tra programmi innovativi formali e pratiche di cura quotidiane. / The Patient-Centred Model (PCM) is described as an attempt to redesign the hospitals around the needs of the patients, thus contributing to costs reduction, increased efficiency, and improved care. However, the introduction of the PCM may have a profound impact on the social organisation of work, changing lines of demarcation, challenging well established inter-/intra-professional relationships, and prompting the development of new roles and modes of working. This thesis explores the mutual effects between the new organisational model and the pre-existent social organisation of hospital work. The research design is organised in three phases: an extensive document analysis; an interview study; an in-depth ethnographic case study conducted for over one year in a PCM hospital. The findings are organised in three studies. The first shows that the PCM was interpreted differently by hospital managers and by frontline clinicians, thus giving rise to two divergent narratives of change. The second study focuses on the boundaries to collaboration and care integration in newly created hospital teams within PCM hospitals. The third study looks at the impact of the PCM on the medical-nursing boundary. The thesis contributes to management learning and practice by providing recommendations on how to accompany complex innovations, comprising of both their expected and unexpected consequences. It also enriches academic debates on professional boundaries, relations, and identities in healthcare.
