Soft tissue profile changes in patients treated with non-extraction versus second premolar extraction protocols - using the Damon system

Magister Scientiae Dentium - MSc(Dent) (Orthodontics) / Orthodontic treatment has the ability to improve the aesthetics and the function of patients. In order to create space, orthodontic treatment often requires removal of teeth. The most common teeth removed for orthodontic treatment are the premolars. It has become popular to remove second premolars in certain cases where the soft tissue profile should not be altered. The Damon self-ligating orthodontic system is renowned for not requiring dental extractions in the majority of cases. The effect of extractions on the soft tissue profile of patients, in conjunction with using the Damon system, has therefore not been researched. It is important to understand the effect that orthodontic treatment and extractions can have on the soft tissue profile of patients. This effect can accurately be determined by making use of the soft tissue cephalometric analysis, developed by Dr Reed A. Holdaway in 1983.

Distalização da arcada dentária superior ancorada em mini implantes na crista infrazigomática : estudo clínico e análise de elementos finitos /

A correção da classe II por meio da distalização de toda arcada dentária superior, sem exodontias e ancorada em mini implantes, já foi descrita como relatos de casos isolados. Objetivo: este estudo tem como objetivo avaliar os efeitos da distalização da arcada dentária superior ancorada em mini implante extra alveolar na crista infra zigomática, por meio de análise de elementos finitos (AEF) e clinicamente, meio de tomografia computadorizada de feixe cônico (TCFC), em dois diferentes grupos. Material e Método: Para análise de elementos finitos, foi construído um modelo CAD maxilar, com braquetes em toda a arcada, fio retangular de aço .019x.025” e mini implante na crista infrazigomática, a 10mm do arco de nivelamento. Ganchos estavam localizados entre caninos e primeiro pré-molares em diferentes alturas de 2mm (Grupo1), 10mm (Grupo 2) e 12mm (Grupo 3). Foi aplicada uma força de retração de 2,5N, bilateralmente. Para achados clínicos, dois diferentes grupos foram avaliados. Grupo aparelho autoligado estético (Grupo ALE) foi composto por 15 mini implantes instalados na crista infrazigomática de 10 pacientes (37a03m DP±9.62) que receberam braquetes autoligado estético. Grupo aço inoxidável (Groupo AI) foi composto por 15 mini implantes instalados na crista infrazigomática de 10 pacientes (32a46m DP±16.73) que receberam braquetes metálicos. Objective: The aim of this study was to evaluated the effects of maxillary dentition distalization with extra-alveolar miniscrews placed in infrazygomatic crest (IZC) area, using finite element method (FEM) and clinically, using cone beam computed tomography (CBCT), comparing two different groups. Methods: For finite element analysis, a maxillary CAD model was constructed with full mouth brackets, a 0.019×0.025-in stainless steel archwire and a miniscrew at infrazygomatic crest, 10mm from the archwire. The power arm was located between canine and first premolar and had different height levels of 2mm (Group 1), 10mm (Group 2) and 12mm (Group 3). A retraction force of 2.5N was applied bilaterally. For clinical responses, two different groups were evaluated. Reabertura dos espaços de extrações nos diferentes protocolos de tratamento ortodôntico / Extraction space reopening in different orthodontic treatment protocols

O objetivo deste trabalho foi comparar a frequência e a quantidade da reabertura dos espaços das extrações de primeiros pré-molares em longo prazo nas más oclusões de Classe II tratadas com o protocolo de extração de dois pré-molares superiores, com aquelas tratadas com extrações de quatro pré-molares e com casos de Classe I tratados com extrações de quatro pré-molares. Para isso foi selecionada do arquivo da Disciplina de Ortodontia da Faculdade de Odontologia de Bauru USP, uma amostra composta pelas documentações ortodônticas de 105 pacientes com más oclusões de Classe I e Classe II completa. Em seguida a amostra foi dividida em 3 grupos proporcionais entre os gêneros e as idades ao início do tratamento: grupo 1- constituído por 33 pacientes com má oclusão de Classe II completa tratados com extrações de dois primeiros pré-molares superiores; grupo 2- constituído por 34 pacientes com má oclusão de Classe II completa tratados com extrações de quatro primeiros pré-molares e grupo 3- constituído por 38 pacientes com má oclusão de Classe I tratados com extrações de quatro primeiros pré-molares. As fichas cadastrais e clínicas dos pacientes foram utilizadas para obtenção do gênero e das idades ao início e fim do tratamento, assim como para obtenção dos tempos de tratamento e pós-tratamento. O índice oclusal PAR foi utilizado para avaliar a severidade inicial da má oclusão e a qualidade oclusal ao final do tratamento mensurado nos modelos de gesso das fases correspondentes. A frequência e a quantidade de reabertura dos espaços das extrações dos primeiros pré-molares foram mensuradas com paquímetro digital nos modelos finais e de pós-tratamento, com média de 9,79 anos após o término do tratamento. A comparação entre os grupos das variáveis estudadas foi feita por meio dos testes qui-quadrado e ANOVA. Os grupos foram proporcionais entre os gêneros e à quantidade de locais das extrações que não possuíam pontos de contato ao final do tratamento. Foram compatíveis quanto às idades ao início e fim do tratamento, PAR inicial e PAR final, assim como quanto aos tempos de tratamento e de pós-tratamento. A frequência e a quantidade de reabertura dos espaços das extrações de primeiros pré-molares não apresentaram diferença estatisticamente significante entre os três grupos estudados, concluindo que os protocolos de tratamento estudados apresentam semelhança em relação à recidiva dos espaços das extrações. / The aim of this retrospective study was to compare the frequency and quantity of reopening extraction spaces in long term of Class II malocclusion treated with two premolar extraction protocol, with those treated with four premolar extractions and Class I malocclusion cases treated with extractions of four premolars. The sample, from files of the Department of Orthodontics, School of Dentistry of Bauru - USP, comprised 105 subjects presenting complete Class I and Class II malocclusion. This sample was divided into three groups in proportion between genders and ages: group 1 - comprising 33 patients presenting complete Class II malocclusion treated with two premolar extraction protocol, group 2 - consisting 34 patients presenting complete Class II malocclusion treated with extraction of four first premolars and group 3 - comprising 38 patients presenting Class I malocclusion treated with extraction of four first premolars. The registration and clinical forms were used to obtain the gender and age at beginning and end of treatment, so as to obtain the time of treatment and post-treatment. The PAR occlusal index was used to assess the initial malocclusion severity and occlusal quality at the end of treatment measured on dental casts. The frequency and amount of extraction spaces were measured with a digital caliper rule in the final and post-treatment dental casts, with an average of 9.79 years after treatment. Comparison of variables between groups was performed by chi-square and ANOVA tests. There were no significant differences between groups variables considered in this study and it was concluded that these treatment protocols show similarity in recurrence of extraction spaces in long term.

The contribution of tools that enable three-dimensional analysis has been gradually incorporated into dental research. Objective: This study evaluated the crown inclination and angulation of patients in orthodontic treatment using digital models and cone beam computed tomography. Methodology: With the three-dimensional cephalometric software was created cephalometric analysis to evaluate the crown angulation and inclination in digital models of 26 patients, except second and third molars. For the results of crown angulation and inclination based on Camper and Frankfurt plans were required cone beam computed tomography images. Student's t- test was used to compare the intra and inter-group averages and Pearson's correlation coefficient used to evaluate the results between the groups Camper, Frankfurt and occlusal. Results: The analysis of the reproducibility of the method did not reveal a statistically significant difference. In assessing intra-group before and after orthodontic treatment the lower lateral incisors, the lower right first molar, the canine, and the first and second premolars presented statistically significant differences in crown inclinations. The upper right first molar and the lower left molar, the lower second premolar, the lower right first premolar, and the lower lateral incisors presented statistically significant differences in angulation. In the intergroup evaluation, post treatment and prescriptions, all of the results of the crown inclination and angulation presented statistically significant differences except the crown angulation of the second superior premolars and the first inferior molars. The study shows that between the Camper-occlusal, the Camper-Frankfurt and the Frankfurt-occlusal planes, the measures of the crown inclination for incisors and canine teeth did not present a correlation and other correlations were weak or inverse. The results of the crown angulation presented a strong correlation, except for central inferior incisors and upper lateral incisors, which demonstrated a moderate correlation to the Camper-occlusal and Frankfurt-occlusal planes. Conclusion: The crown angulation and inclination can be determined by means of digital models and it is evident that there is a need for additional bend to obtain crown angulation and inclination with values close to those recommended in preset brackets. The measures of the crown angulation and inclination presented correlations when evaluated by the Camper, Frankfurt and occlusal planes with the exception of the crown inclination of the incisors and canine teeth. / Introdução: A incorporação de novas alternativas e métodos aplicáveis à ortodontia para a análise da angulação e inclinação coronária constitui um verdadeiro desafio. A contribuição de ferramentas que permitem análises tridimensionais tem sido gradativamente incorporada à pesquisa odontológica. Objetivo: O presente estudo avaliou a inclinação e a angulação coronária em pacientes tratados ortodonticamente usando modelos digitais e tomografia computadorizada de feixe cônico. Metodologia: Com o módulo de cefalometria tridimensional do Software VistaDent 3D foi criado análise cefalométrica para avaliar a inclinação e angulação coronária em modelos digitais de 26 pacientes, exceto segundos e terceiros molares. Para obter os resultados de angulação e inclinação coronária com base nos planos de Camper e Frankfurt, foram necessárias documentações de pacientes com imagens tomográficas finais. Foi utilizado o teste t-Student para comparar as médias intra e intergrupo e o coeficiente de correlação de Pearson para avaliar os resultados entre os grupos Camper, Frankfurt e oclusal. Resultados: A análise da reprodutibilidade do método não revelou diferença estatisticamente significante. Quanto à avaliação intragrupo, antes e depois do tratamento ortodôntico, os incisivos laterais inferiores, primeiro molar inferior direito, os caninos, primeiros e segundos pré-molares apresentaram diferenças estatisticamente significantes para inclinações. Primeiro molar superior direito e inferior esquerdo, segundos pré-molares inferiores, primeiro premolar inferior direito e incisivos laterais inferiores apresentaram diferenças estatisticamente significantes para angulações. Quanto à avaliação intergrupo, depois do tratamento ortodontico e prescrições, todos os resultados de inclinação e angulação apresentaram diferenças estatisticamente significantes, exceto a angulação dos segundos pré-molares superiores e primeiros molares inferiores. O estudo aponta que entre os planos Camper-oclusal, Camper-Frankfurt e Frankfurt-oclusal as medidas de inclinação coronária para incisivos e caninos não apresentaram correlação, outros correlação fraca ou inversa. Os resultados da angulação coronária apresentaram forte correlação, exceto incisivos centrais inferiores e laterais superiores que demonstraram moderada correlação para Camper-oclusal e Frankfurt-oclusal. Conclusão: A angulação e inclinação coronária podem ser mensuradas por meio de modelos digitais e os resultados sugerem a necessidade de dobras adicionais no fio ortodôntico para obtenção de angulação e inclinação coronária com valores próximos aos preconizados em braquetes pré-ajustados. As medidas de angulação e inclinação coronárias apresentaram correlacionadas quando avaliadas pelos planos de Camper, Frankfurt e oclusal, exceto a inclinação dos incisivos e caninos.

Condicionamento do esmalte com óxido de alumínio associado ao ácido fosfórico na colagem indireta de braquetes / Enamel conditioning with aluminum oxide associated to phosphoric acid in indirect bonding of brackets

O objetivo deste estudo foi comparar a resistência de união e a interface de falha da colagem de braquetes colados pela técnica indireta após o condicionamento do esmalte com jato de óxido de alumínio de 27, 50 e 90 m seguido da aplicação de ácido fosfórico a 37%; e avaliar quantitativa e qualitativamente as alterações da superfície do esmalte após os diferentes tipos de condicionamento. A amostra foi constituída por 104 pré-molares superiores. Na primeira parte da pesquisa 80 dentes foram divididos aleatoriamente em 4 grupos (n=20) que receberam diferentes condicionamentos das faces linguais: G1 (controle)- ácido fosfórico a 37%; G2, G3 e G4- jato de óxido de alumínio de 27, 50 e 90 m, respectivamente, antes da aplicação de ácido fosfórico. Braquetes linguais foram colados indiretamente com o mesmo material e metodologia nos 4 grupos. A força máxima de cisalhamento requerida para descolar os braquetes foi determinada por meio de uma máquina de ensaios mecânicos universal, e a interface de falha na colagem, por meio do Índice de Resina Remanescente (ARI). Para a avaliação quantitativa dos efeitos provocados no esmalte pelos diferentes condicionamentos, 20 dentes foram divididos aleatoriamente em 4 grupos, e suas faces linguais foram condicionadas de acordo com o protocolo proposto anteriormente. A rugosidade do esmalte (Ra) antes e após o seu condicionamento foi determinada por meio de um microscópio a laser confocal. Na avaliação qualitativa foram utilizados 4 dentes, que foram condicionados segundo protocolo descrito anteriormente. O teste ANOVA de um critério foi utilizado para comparar a resistência adesiva e o incremento de rugosidade entre os grupos. As diferenças entre as pontuações do ARI foram avaliadas com o teste de Kruskal-Wallis. Foi adotado nível de significância de 5% em todas as avaliações estatísticas. Não foram observadas diferenças estatisticamente significantes entre os quatro tipos de condicionamento de esmalte quando avaliou-se as médias de resistência de união e o incremento de rugosidade. Também não foram verificadas diferenças estatisticamente significativas entre as pontuações do ARI. O resultado da avaliação qualitativa evidenciou padrões diferentes de condicionamento entre o esmalte condicionado com ácido fosfórico e o esmalte condicionado com óxido de alumínio associado ao ácido. No esmalte condicionado com ácido, observaram-se leves irregularidades correspondentes com o padrão tipo III de Silverstone; enquanto no esmalte condicionado com jato de óxido de alumínio de 27, 50 e 90 m mais ácido fosfórico verificaram-se padrões similares de condicionamento, com superfícies de textura áspera bastante uniforme, presença de múltiplas elevações com diferentes alturas e bordas arredondadas, semelhantes ao padrão tipo II de Silverstone. Assim, concluiu-se que o condicionamento do esmalte com óxido de alumínio associado ao ácido não incrementou a resistência de união de braquetes colados indiretamente, não modificou a quantidade de resina remanescente, nem aumentou a rugosidade do esmalte. No entanto, produz um padrão de condicionamento diferente ao obtido com o ácido. / The purpose of this study was to compare the bond strength and the bond failure location of indirectly bonded brackets after enamel conditioning with 27, 50 and 90 m aluminum oxide followed by application of 37% phosphoric acid; and to analyze quantitatively and qualitatively the alterations on the enamel surface after the different conditionings. The sample consisted of 104 superior premolars. In the first stage of this research, 80 teeth were randomly divided into 4 groups (n=20) that received different conditionings at the lingual faces: G1 (control)- 37% phosphoric acid, G2, G3 and G4- 27, 50 and 90 m aluminum oxide, respectively, before applying phosphoric acid. The lingual brackets were indirectly bonded using the same material and methodology in the 4 groups. The maximum shear bond strength required to debond the brackets was determined using a mechanical testing machine, and the bond failure location through the Adhesive Remnant Index (ARI). For the quantitative analysis of the effects caused in the enamel due to the different conditionings, 20 teeth were randomly divided into 4 groups, and their lingual faces were conditioned according to the previously described protocol. The enamel roughness (Ra) before and after conditioning was measured using a confocal laser scanning microscope. In the qualitative analysis 4 teeth that were conditioned according to the previously described protocol were used. One-way ANOVA analysis was used to compare the bond strength and the roughness increment between the groups. The differences among the ARI scores were analyzed with the Kruskal-Wallis test. A 5% significance level was adopted for all statistical analysis. No statistical differences were observed among the 4 types of enamel conditionings when the mean shear bond strength and roughness increase were compared. There were not also statistically significant differences between the ARI scores. The results of the qualitative analysis showed different conditioning patterns between the enamel prepared with phosphoric acid and the enamel treated with aluminum oxide associated to phosphoric acid. In the acid-etching enamel, slight irregularities corresponding to Silverstones type III pattern were found; while in the enamel conditioned with aluminum oxide of 27, 50 and 90 m followed by phosphoric acid, similar conditioning patterns were found, with rough surfaces highly uniform, presence of multiple elevations of different heights and rounded edges, similar to Silverstones type II pattern. In conclusion, the enamel conditioning with aluminum oxide associated to phosphoric acid did not increase the bond strength of indirectly bonded brackets; it did not modify the quantity of remnant resin nor increased the enamel roughness. However, it produced a conditioning pattern different to the one obtained with acid.

Influence d’un rince-bouche fluoré sur la corrosion galvanique entre un fil NiTi ou un fil CuNiTi et différents boîtiers orthodontiques : incidence sur les propriétés mécaniques des fils

Objectif : il a été rapporté que l’utilisation d’agents prophylactiques fluorés pouvait favoriser la corrosion galvanique au sein des alliages de titane. L’objectif de la présente étude était d’évaluer l’effet d’un rince-bouche fluoré sur les propriétés mécaniques de fils en nickel-titane (NiTi) et de fils en cuivre-nickel-titane (CuNiTi) lorsque ces derniers sont couplés à des boîtiers de compositions différentes (boîtiers de marques Smartclip, Clarity, et Sprint). Matériels et Méthodes : 90 segments de fils en NiTi et 90 segments de fils en CuNiTi ont été chacun couplés à 2 boîtiers de chaque marque. Chaque assemblage fil-boîtiers a été par la suite incubé pendant 3 heures à 37°C, soit dans une solution de fluore neutre (Fluorinse™ 0,05% NaF), soit dans une solution de salive artificielle (solution contrôle). Suite à l’incubation, les échantillons étaient nettoyés avec de l’eau déshydrogénée, les fils séparés des boîtiers et montés sur un support pour subir un test de pliage en trois points en milieu humide (salive artificielle) à 37°C. Les modules d’élasticité ainsi que les limites conventionnelles d’élasticité en activation et en désactivation ont été mesurés et comparés. Des analyses de Variance (ANOVA) et des comparaisons post-hoc avec la correction de Bonferronni ont été utilisées pour comparer les groupes entre eux (α = 0,05). Résultats : L’utilisation d’un rince-bouche fluoré a produit une réduction du module d’élasticité et de la limite conventionnelle d’élasticité en activation et en désactivation pour les fils en NiTi ; cependant, cet effet a été modulé par le type de boîtier auquel le fil a été couplé. Les propriétés mécaniques de fils en CuNiTi n’ont pas été affectées par le fluor, ou par le type de boîtier utilisé. Conclusions : L’utilisation d’un rince-bouche fluoré modifie les propriétés mécaniques des fils en NiTi seulement. Cet effet est modulé par le boîtier auquel le fil en NiTi est couplé. A la différence des autres études publiées dans la littérature, nos résultats ne nous permettent pas de conclure que la modification des propriétés mécaniques des fils en NiTi entrainerait obligatoirement un allongement de la durée du traitement orthodontique. Mots clés : Fluor, fils nickel-titane, boîtiers orthodontiques, corrosion galvanique, propriétés mécaniques. / Aim: it has been reported that the use of fluoride prophylactic agents can cause galvanic corrosion of the titanium based alloys used in orthodontics. The purpose of the present study was to investigate the effects of a fluoride mouthrinse on the mechanical properties of nickel-titanium (NiTi) and copper–nickel-titanium (CuNiTi) orthodontic archwires that have been coupled with different types of orthodontic brackets (Smartclip, Clarity, and Sprint brackets). Materials and Methods: 90 segments of NiTi and 90 segments of CuNiTi archwires were tested. Every segment was coupled with 2 brackets of each brand. The wire–bracket assembly obtained was incubated, at 37°C for three hours either in a solution of a commercially available mouthwash (Fluorinse™ 0,05% NaF), or in a solution of artificial saliva (control). Following the incubation, the wires were separated from the brackets, rinsed, mounted on a stainless steel support and placed in a waterbath of artificial saliva at 37°C. A 3-point bending test was made to calculate the loading and unloading elastic modulus and yield strength of the wires. Analysis of variance (ANOVA) and post hoc comparisons were made using Bonferronni’s correction to identify the statistically significant differences (α = 0,05). Results: The use of a fluoridated mouthrinse reduced the loading and unloading elastic modulus and yield strength of the NiTi wires. This reduction, however, varies with the type of bracket that was coupled with the wire. The mechanical properties of the CuNiTi wires were not modified by the use of the fluoride rinse or by the type of bracket to which they were coupled. Conclusions : The use of a fluoride mouthrinse alters the mechanical properties of NiTi wires only. This effect varies with the type of bracket that was in contact with the wire. However our results do not allow us to conclude that the alteration of the mechanical properties of the wires would necessarily imply a prolongation of the orthodontic treatment time. Indirect Bonding Method: in vitro Comparison of the Shear Bond Strength between Metallic Orthodontic Brackets and Different Porcelain Surface Preparations

Introduction : La force d’adhésion à l'interface métal-céramique avec les résines auto-polymérisantes destinées au collage indirect des boîtiers orthodontiques n'a pas été évaluée à ce jour et un protocole clinique basé sur la littérature scientifique est inexistant. Objectifs : 1) Comparer la force de cisaillement maximale entre des boîtiers métalliques et des surfaces en porcelaine préparées selon différentes méthodes; 2) Suggérer un protocole clinique efficace et prévisible. Matériel et méthodes : Quatre-vingt-dix disques en leucite (6 groupes; n = 15/groupe) ont été préparés selon 6 combinaisons de traitements de surface : mécaniques (+ / - fraisage pour créer les rugosités) et chimiques (acide fluorhydrique, apprêt, silane). Des bases en résine composite Transbond XT (3M Unitek, Monrovia, California) faites sur mesure ont été collées avec le système de résine adhésive auto-polymérisante Sondhi A + B Rapid Set (3M Unitek, Monrovia, California). Les échantillons ont été préservés (H2O/24hrs), thermocyclés (500 cycles) et testés en cisaillement (Instron, Norwood, Massachusetts). Des mesures d’Index d’adhésif résiduel (IAR) ont été compilées. Des tests ANOVAs ont été réalisés sur les rangs étant donné que les données suivaient une distribution anormale et ont été ajustés selon Tukey. Un Kruskall-Wallis, U-Mann Whitney par comparaison pairée et une analyse de Weibull ont aussi été réalisés. Résultats : Les médianes des groupes varient entre 17.0 MPa (- fraisage + acide fluorhydrique) à 26.7 MPa (- fraisage + acide fluorhydrique + silane). Le fraisage en surface ne semble pas affecter l’adhésion. La combinaison chimique (- fraisage + silane + apprêt) a démontré des forces de cisaillement significativement plus élevées que le traitement avec (- fraisage + acide fluorhydrique), p<0,05, tout en possédant des forces similaires au protocole typiquement suggéré à l’acide fluorhydrique suivi d’une application de silane, l’équivalence de (- fraisage + acide fluorhydrique + silane). Les mesures d’IAR sont significativement plus basses dans le groupe (- fraisage + acide fluorhydrique) en comparaison avec celles des 5 autres groupes, avec p<0,05. Malheureusement, ces 5 groupes ont des taux de fracture élévés de 80 à 100% suite à la décimentation des boîtiers. Conclusion : Toutes les combinaisons de traitement de surface testées offrent une force d’adhésion cliniquement suffisante pour accomplir les mouvements dentaires en orthodontie. Une application de silane suivie d’un apprêt est forte intéressante, car elle est simple à appliquer cliniquement tout en permettant une excellente adhésion. Il faut cependant avertir les patients qu’il y a un risque de fracture des restorations en céramique lorsque vient le moment d’enlever les broches. Si la priorité est de diminuer le risque d’endommager la porcelaine, un mordançage seul à l’acide hydrofluorique sera suffisant. / Background : Bond strength at the metal-ceramic interface of auto-polymerizing resins used in orthodontic indirect bonding has not yet been evaluated and a literature-based clinical protocol is lacking. Goals : 1) To compare shear bond strength (SBS) between metal brackets and differently treated porcelain surfaces; 2) To suggest efficient and predictable chairside approaches. Materials and methods : Ninety leucite discs (6 groups; n=15/group) were prepared following 6 combinations of mechanical (+/- bur roughening) and chemical (hydrofluoric acid, primer, silane) treatments. Metal brackets with custom composite resin Transbond XT (3M Unitek, Monrovia, California) bases were bonded with the adhesive resin system Sondhi A+B Rapid Set (3M Unitek, Monrovia, California). Samples were stored (H2O/24hrs), thermocycled (500 cycles) and tested (Instron, Norwood, Massachusetts). Maximum SBS and adhesive remnant index (ARI) scores were collected for each sample. ANOVAs were performed on ranks since data was not normally distributed, and then adjusted with the post-hoc Tukey method. A Kruskall-Wallis, a Mann Whitney U pairwise comparison and a Weibull analysis were also performed. Results : SBS medians of groups ranged from 17.0 MPa (- bur + hydrofluoric acid) to 26.7MPa (- bur + hydrofluoric acid + silane). Bur roughening did not affect bond strength. The chemical preparation of (- bur + primer + silane) showed statistically significant higher SBS than (- bur + hydrofluoric acid) preparation (p<0,05), while having similar SBS to the popular recommended protocol (- bur + hydrofluoric acid + silane). ARI scores were statistically significant lower in group (- bur + hydrofluoric acid) with p<0,05, while all other 5 groups were not different from each other. Percentage of porcelain damage in these 5 groups were very high at 80-100%. Conclusion : All the tested surface preparations combinations provided clinically adequate adhesion for orthodontic tooth movement. A silane and primer combination for porcelain surface preparation is clinically attractive as it is safe and simple and provides great adhesion for orthodontic tooth movement. It is wise to warn patients that there is a risk of porcelain fractures when debonding brackets. If one prefers to avoid porcelain fracture at all cost, one may treat porcelain with hydrofluoric acid only.

Longitudinal evaluation of sleep-related breathing disorders in an orthodontic population

Introduction: Les troubles respiratoires du sommeil (TRS), qui représentent une préoccupation croissante pour la santé, ont des effets significatifs sur la santé, le comportement et la performance académique chez l’enfant. Les malformations craniofaciales, l’hypertrophie adéno-amygdalienne et l'obésité, représentent des facteurs de risque importants dans le développement de cette condition. Les symptômes des TRS ont été étudiés dans une étude prospective chez les enfants et adolescents durant leur traitement orthodontique dans un milieu universitaire. Cette étude a cherché à décrire la prévalence et les facteurs de risque principaux des TRS, ainsi que l'impact des différentes interventions orthodontiques sur les symptômes TRS. Matériel et méthodes: dans une étude cohorte prospective, un groupe de 168 sujets âgés de 12 à 21 ans ont été soumis, quatre ans après la prise de données initiale, à un examen craniofacial en plus d'être administré des questionnaires qui ont recueilli des données sur la situation socio-démographique, le bruxisme et les troubles d’ATM, le sommeil et le comportement diurne, et les facteurs neuropsychologiques. Résultats: l'indice de masse corporelle a été augmenté mais est demeurée dans la même catégorie aux deux moments de l'enquête. Il ya eu une augmentation du serrement des dents et des symptômes de l'ATM, une diminution de la taille des amygdales, et une augmentation de la somnolence diurne. La prévalence des TRS n'a pas changé entre l’étude initiale et l’étude de suivi. Aucune intervention orthodontique s'est avérée avoir un effet cliniquement significatif sur les voies aériennes supérieures. Conclusions: la prévalence des symptômes TRS était constante par rapport aux valeurs de base pour la population étudiée, mais a augmenté si rapportée à la population générale. Les traitements orthodontiques ne montrent aucun effet sur les TRS. Craniofacial malformations, along with adenotonsillar hypertrophy and obesity, represent important risk factors in the development of this condition. SDB symptoms in children and adolescents followed for orthodontic treatment in a university setting have been investigated in this prospective study. The aims of this study were to describe the prevalence and main risk factors of SDB and the impact of different orthodontic interventions on the SDB symptoms. Materials and methods: in a prospective cohort study, four years following an initial evaluation, a group of 168 subjects aged 12-21 years underwent a craniofacial examination in addition to being administered self-completed questionnaires that collected information on socio-demographic and psychosocial factors, bruxism and temporo-mandibular joint (TMJ) disorders, sleep and daytime behaviour, and neuropsychological factors. Results: Body mass index (BMI) was slightly increased but remained in the same category at the two time points of investigation. There was an increase in tooth clenching and TMJ symptoms, a decrease in tonsils’ size, and an increase in daytime sleepiness. Prevalence of SDB did not change between baseline and follow-up studies. No orthodontic treatment intervention proved to have any clinically significant impact on the upper airway. Conclusions: SDB symptoms prevalence was constant when compared to the baseline values for the studied population, but increased if reported to the general population.
