A Web Portal for Cognitive Behavioral Therapy Treatment

Clinical psychologists currently struggle to organize information about patients during a cognitive behavioral therapy (CBT) treatment. During a CBT treatment, the patients will have to fill in paper forms between therapy sessions. These forms contain information about how the patients are feeling and the different tasks that they have been assigned as homework. Some problems with the paper forms are that they can easily be forgotten, lost, destroyed, etc. Before this project, Zeeds App could be used instead of the paper forms but the psychologists could not review the homework results from the app. Our solution to this problem is a web portal that is connected to Zeeds App where psychologists can oversee their patient's homework results. Since psychologists can review the answers to the forms before therapy sessions the sessions can be more focused. / Psykologer har för tillfället problem med att organisera information om deras patienter under en kognitiv beteendeterapi. Under en kognitiv beteendeterapi kommer patienten behöva fylla i pappersformulär mellan möten med sin psykolog. Dessa formulär innehåller information om hur patienterna mår och de olika uppgifter som de har fått att utföra som hemläxa. Några problem med dessa pappersformulär är att de är lätt att glömma bort formulären, tappa bort formulären, eller att formulären blir förstörda osv. Innan det här projektet kunde Zeeds App användas istället för pappersformulären men det fanns ingen möjlighet för psykologer att se resultaten från appen. Våran lösning till det här problemet är en webbportal som är kopplad till Zeeds App där psykologer kan se deras patienters resultat för hemläxan. Eftersom psykologer kan se resultaten från hemläxan innan möten kan mötena bli mer fokuserade.

A study designed to investigate stakeholder reaction to a proposed internet portal designed to manage student accommodation at a major language school chain in the UK

No / The company, a market-leading language school chain with five schools in the UK, struggles in the successful provision of UK homestay accommodation for its students. Although a complex problem, it is caused mainly by the students’ pre-arrival expectations of their accommodation quality, and a complicated and only partially-adequate booking process, making for an inefficient process. A proposed solution to the problem is an accommodation internet portal. This paper describes a study that investigated stakeholder reaction to the proposed solution with the aim of enabling the company’s management to understand what is necessary to achieve stakeholder ‘buy in’ and thus benefit from the Portal’s efficiencies, and ultimately to decide on whether to sponsor the Portal’s development. Focus groups and interviews were used to investigate the participants’ understanding of stakeholder theory, to categorise them as stakeholders, to elicit their reactions to the proposed Portal idea, and to gather suggestions on its features and the potential issues its introduction might cause. Despite there being a range of views regarding the introduction of the Portal, the results showed that if correctly implemented the Portal’s radical transformation of the accommodation booking process would lead to significant organisational efficiencies. The conclusion recommends that for the company to benefit from the potential efficiencies of a portal, its management introduce a hybrid portal where users can select the level of engagement they have with it, that the company consults its stakeholders on the design of the Portal, and that the change process is managed carefully and effectively.

The value of a Patient Access Portal in primary care: a cross-sectional survey of 62,486 registered users in the UK

Yes / In England, primary care patients have access to Patient Access Portals (PAPs), enabling them to book appointments, request repeat medication prescriptions, send/receive messages and review their medical records. Few studies have elicited user views and value of PAPs, especially in a publicly funded primary care setting. This study aimed to elicit the value users of PAPs place on online access to medical records and linked services. Secondary data analysis of the completed electronic survey (available 2 May 2015–27 June 2015) distributed via the EMIS PAP to all its registered users. EMIS designed the survey; responses were voluntary. There were 62,486 responders (95.7% self-completed). The PAP was mainly used for medication requests (86.3%) and online appointment bookings (78.4%), and, to a lesser extent, medical record viewing (18.3%) and messaging (9.5%). The majority (70%) reported a positive impact from using it. One in five rated it as their favourite online service second only to online banking. Almost three out of four responders stated that availability of online access would influence their move to another practice. Nonetheless, responders were reluctant to award a high monetary value to it. These findings correlated with the number of long-term conditions. The majority of users place a relatively high value, but not monetary value, on the PAP and report a positive impact from using it. The potential for PAPs to enhance patient experience, especially for those with long-term conditions, appears to be largely untapped. Research exploring the reasons for non-use is also required.

Using ontologies to semantify a Web information portal

Ontology, an explicit specification of a shared conceptualisation, captures knowledge about a specific domain of interest. The realisation of ontologies, revolutionised the way data stored in relational databases is accessed and manipulated through ontology and database integration. When integrating ontologies with relational databases, several choices exist regarding aspects such as database implementation, ontology language features, and mappings. However, it is unclear which aspects are relevant and when they affect specific choices. This imposes difficulties in deciding which choices to make and their implications on ontology and database integration solutions. Within this study, a decision-making tool that guides users when selecting a technology and developing a solution that integrates ontologies with relational databases is developed. A theory analysis is conducted to determine current status of technologies that integrate ontologies with databases. Furthermore, a theoretical study is conducted to determine important features affecting ontology and database integration, ontology language features, and choices that one needs to make given each technology. Based on the building blocks stated above, an artifact-building approach is used to develop the decision-making tool, and this tool is verified through a proof-of-concept to prove the usefulness thereof. Contribuição da ultra-sonografia no diagnóstico da forma hepato-esplênica da esquistossomose mansônica / Ultrasonography in the diagnosis of the hepatosplenic form of schistosomiasis mansoni

O trabalho discute a utilidade da ultra-sonografia, no diagnóstico da forma hepato-esplênica da esquistossomose mansônica. O autor estudou, pela ultra-sonografia, 103 pacientes com esquistossomose hepato-esplênica comprovada e observou os sinais encontrados no fígado, vias biliares, baço e circulação portal. Foi determinada a freqüência dos diferentes sinais presentes no grupo de pacientes esquistossomóticos e feita correlação com as alterações anátomo-patológicas da doença. Os valores dos calibres das veias esplênica, porta e mesentêrica superior dos nossos pacientes esquistossomóticos foram comparados aos encontrados em 50 pacientes sem anormalidades hepáticas (grupo-controle). As alterações encontradas no fígado incluíram espessamento da parede da veia porta e seus ramos, nódulos parenquimatosos, atrofia do lobo direito e hipertrofia do lobo esquerdo. O espessamento da parede dos vasos portais correlaciona-se a fibrose peri-portal encontrada no exame anátomo-patológico e foi sinal muito freqüente. A atrofia do lobo direito e a hipertrofia do lobo esquerdo devem estar relacionadas ao hiperfluxo pela veia esplênica. Os nódulos parenquimatosos representam os granulomas que, pelas suas pequenas dimensões, são raramente visualizados pela ultra-sonografia. A vesícula biliar apresentou espessamento de sua parede em número significativo dos pacientes estudados. O baço encontrou-se aumentado de volume em todos os pacientes, atingindo geralmente grandes proporções. Em pequeno número de casos observou-se padrão multinodular que foi atribuído aos nódulos sideróticos encontrados ocasionalmente na esquistossomose. A média dos calibres da veia porta, veia esplênica e veia mesentérica superior, nos pacientes esquistossomóticos, foi estatisticamente superior à dos pacientes do grupo-controle. O aumento do calibre foi mais acentuado para a veia esplênica do que para a veia mesentérica superior. Os vasos colaterais estudados foram a veia umbilical, a veia gástrica esquerda e os vasos gástricos curtos, não apresentando características particulares para esta doença. A trombose da veia porta foi encontrada em raros casos. Os principais sinais sonográficos da esquistossomose hepato-esplênica incluem fibrose peri-portal, hipertrofia do lobo esquerdo, espessamento da parede da vesícula biliar, nódulos hepáticos, padrão multinodular do baço e aumento do calibre dos vasos portais. Sinais sonográficos freqüentes porém inespecíficos, incluem esplenomegalia e vasos colaterais. O autor considera o exame ultra-sonográfico método importante no diagnóstico não invasivo da esquistossomose hepato-esplênica / The paper discusses the usefulness of ultrasonography in the diagnosis of the hepatosplenic form of schistosomiasis mansoni. The author studied, sonographically, 103 patients with comproved hepatosplenic schistosomiasis and observed the signs found in the liver, biliary tract, spleen, and portal system. The frequency of the different signs present in the group of schistosomotic patients had been determined and the correlation with the anatomopathological alterations of the disease was done. The values of the diameter of the splenic, portal and superior mesenteric veins in the schistosomotic patients was compared with the ones found in 50 patients without hepatic abnormalities. The alterations found in the liver includes the thickening on the wall of the portal vein and its branches; atrophy of the right lobe and hypertrophy on the left lobe and hepatic nodules. The thickening of the wall of the portal vessels is correlated to the periportal fibrosis found in the anatomopathologic examination and is a very frequent sign. The atrophy of the right lobe and the hypertrophy of the left lobe is most probably related to the higher vascular flow through the splenic vein. The hepatic nodules represent granulomas that, due to their small dimentions, are seldom visualized by ultrasonography. The thickening of the gallbladder wall, was found in a significant number of the studied patients. The spleen presented an enlarged volume in all patients, reaching generally big proportions. In a small number of cases a multinodular pattern was observed and it has been atributed to the siderhotic nodules occasionally found in schistosomiasis. The average of the diameters of the portal, splenic and superior mesenteric veins of the patients with schistosomiasis was statistically higher than the ones found in the control group. The enlargement of the diameter was more accentuated in the splenic vein rather than in the superior mesenteric vein. The collateral vessels studied were the umbilical vein, the left gastric vein and the short gastric vessels, not presenting particular characteristics for this disease. Thrombosis of the portal vein was rarely found. The main sonographic signs of the hepatosplenic schistosomiasis include periportal fibrosis, hypertrophy of the left lobe, thickening of the wall of the gallbladder, granulomas, multinodular pattern of the spleen and enlargement of the diameter of the portal vessels. Frequent sonographical signs, but nonspecific, include splenomegaly and collateral vessels. The author considers the ultrasonographic examination an important method in the non invasive diagnosis of this disease
