As principais formas de apresentação clínica são hemorragia digestiva alta (HDA) e esplenomegalia. Há pouca informação sobre a evolução dessa doença em crianças. O objetivo desse estudo foi avaliar a evolução clínica, laboratorial, ultra-sonográfica e endoscópica de pacientes com TVP na FEP. Foi realizado um estudo descritivo e longitudinal de 51 pacientes com TVP sem doença hepática crônica associada acompanhados no Ambulatório de Hepatologia Pediátrica do Hospital de Clínicas da Faculdade de Ciências Médicas da Universidade Estadual de Campinas entre janeiro de 1986 e junho de 2006. Elaborou-se uma ficha de coleta de dados, a partir dos prontuários dos pacientes incluídos, que constou dos seguintes itens: identificação e avaliações clínica, laboratorial, ultra-sonográfica e endoscópica. Da avaliação laboratorial, foram obtidos os resultados dos seguintes exames: eritrograma, leucograma, contagem de plaquetas, RNI e R. Foram selecionados os resultados dos primeiros e últimos exames realizados, e os que foram coletados próximo ao ultra-som de abdome em que havia o resultado da dimensão do baço. A análise estatística foi descritiva para os indicadores clínicos e, para análise das variáveis, foram empregados os testes de McNemar, Mann-Whitney e coeficiente de correlação de Spearman. A análise de sobrevida dos pacientes foi realizada pelo método de Kaplan-Meier. Foi adotado nível de significância de 5%. HDA e esplenomegalia foram as principais formas de apresentação clínica que justificaram o encaminhamento para investigação, com freqüência de 66% e 22%, respectivamente. As medianas da idade do início dos sintomas foi de 3,7 anos e do tempo de seguimento foi de 4,8 anos. O fator de risco (FR) associado à TVP não foi identificado na maioria dos casos. Quando identificado, o mais freqüente foi o cateterismo umbilical (27,5%). A freqüência da HDA foi alta na admissão (66%) e na evolução (78%). A esplenomegalia já estava presente ao exame físico em 83,7% dos casos na admissão. A maioria dos pacientes apresentava níveis séricos de aminotransferases normais e alta freqüência de anemia, leucopenia e plaquetopenia. Com a evolução, observaram-se melhora da anemia e persistência da leucopenia e da plaquetopenia. A comparação entre a dimensão esplênica e a contagem de leucócitos, plaquetas e valores de R mostrou diferença estatística, sendo inversamente proporcional para o número de leucócitos e de plaquetas e diretamente proporcional para o R. Cinqüenta pacientes foram submetidos à endoscopia digestiva. Ao diagnóstico, 88% dos pacientes apresentavam varizes de esôfago (VE), 50% varizes gástricas (VG) e 22% gastropatia da hipertensão portal (GHP). As últimas endoscopias evidenciaram VE em 73%, VG em 60% e GHP em 41% dos pacientes. Escleroterapia e/ou ligadura de varizes esofágicas foram realizadas em 41 pacientes, sendo a maioria dos procedimentos realizados no primeiro semestre após o início da terapia endoscópica (p<0,001). Biliopatia portal foi diagnosticada em 4 pacientes. Três pacientes foram submetidos a shunt porto-sistêmico. Evolução para óbito ocorreu em 3 pacientes. Em conclusão, HDA, esplenomegalia e hiperesplenismo devem alertar o médico para o diagnóstico de TVP. A maioria dos casos permanece sem FR conhecido. O sangramento digestivo ocorreu na maior parte dos pacientes e motivou múltiplas transfusões. A doença apresenta baixa mortalidade, mas alta morbidade / Abstract: The aim of this study was to analyze clinical, laboratory, ultrasonographic and endoscopic data of PVT in children. We comprised 51 children who had PVT diagnosed between January 1986 and January 2006. All of them were selected from the record of patients admitted to the Hepatology Pediatric Outpatient Clinic of the University of Campinas Teaching Hospital (Unicamp), city of Campinas, state of São Paulo, Brazil. The information obtained from the patients¿ charts included their clinical, laboratory, ultrasonographic and endoscopic data. The following values were included in the laboratory evaluation: hemoglobin, white blood cell and platelet counts, INR and R values. The first and last results of these exams were selected, as well as those exams performed next to an abdominal ultrasonography in which the spleen dimension was available. The clinical data were obtained from descriptive statistical analysis. The Mann-Whitney and McNemar tests and Spearman¿s rank correlation coefficient were used for variable data. Kaplan-Meier¿s method was used in order to determine patient¿s life expectancy. A p-value of <0.05 was considered signif icant. Initial manifestations included gastrointestinal bleeding and splenomegaly, seen in 66% and 22% of the patients, respectively, with a median age of 3.7 years old. The patients were followed for a median time of 4.8 years. The etiologic factor for PVT was not available in most patients and umbilical catheterization was identified in 27.5% of all cases. Sixty percent of the patients presented with GI bleeding at the first medical evaluation and 78% of them had presented GI bleeding at the last one. Splenomegaly was the most frequent sign at physical examination, occurring in 83.7% of the children at presentation. Most patients had normal liver enzymes, but low levels of hemoglobin, leukocytes and platelet counts were commonly seen. The last evaluation showed persistent leucopenia and low platelet count. The correlation between the spleen dimension and the white blood cell count, platelet count and R was statistically significant. Fifty patients underwent upper gastrointestinal endoscopy. At first evaluation, 88% of the patients presented with esophageal varices, 50% with gastric varices and 22% with portal hypertensive gastropathy. The last endoscopies showed esophageal varices in 73%, gastric varices in 60% e portal hypertensive gastropathy in 41%. Forty-one patients underwent sclerotherapy and/or esophageal ligation, most of whom during the first 6-month period after they were initiated (p < 0.001). Portal biliopathy occurred in four patients. Shunt surgery was performed in three patients. Three patients died. In conclusion: Gastrointestinal bleeding, splenomegaly and hypersplenism should alert physicians to PVT. The etiologic factor related to PVT remains uncertain in most cases. The majority of patients experienced gastrointestinal bleeding and many blood transfusions. É definida como pressão arterial pulmonar média (PAPM) acima de 25 mmHg, resistência vascular pulmonar (RVP) acima de 240 dinas.s.cm?? e a pressão de oclusão da artéria pulmonar (POAP) normal, menor de 15 mmHg. A associação de hipertensão pulmonar com cirrose e hipertensão portal é pouco diagnosticada, sendo a sua prevalência estimada entre 2 - 6%. A mortalidade é diretamente proporcional à medida da pressão arterial pulmonar média e da resistência vascular pulmonar. Alguns estudos sugerem que a pressão da artéria pulmonar média pré-operatória seja um preditor independente para mortalidade, e muitos centros consideram que pressão arterial pulmonar média acima de 50 mmHg seja uma contraindicação absoluta para o transplante de fígado (TF). Objetivo: Analisar a prevalência, as complicações e a sobrevida dos pacientes portadores de HPP submetidos a transplante de fígado. Método: Foram analisados de modo retrospectivo, prontuários e o banco de dados da Unidade de Transplante Hepático do Hospital de Clínicas da Universidade Estadual de Campinas dos pacientes submetidos a transplante de fígado, num período de 21 anos. Dividiu-se a população em dois grupos de acordo com as medidas da pressão da artéria pulmonar média: PAPM ? 25 mmHg e PAPM > 25 mmHg. Foram estudados os exames pré-operatórios, as variáveis hemodinâmicas, bem como as complicações intraoperatórias, eventos relacionados e a sobrevida. Resultado: Foram encontrados 50 casos com PAPM > 25 mmHg e apenas um caso preencheu os critérios diagnósticos de PAPM > 25 mmHg, RVP > 240 dinas.s.cm-5 e POAP < 15 mmHg , de acordo com a European Respiratory Society Task Force on Pulmonary Hepatic Vascular Disorders (2004), e dois casos de PAPM > 50 mmHg que tiveram os procedimentos suspensos, portanto a nossa prevalência da HPP em pacientes submetidos a TF foi de 0,25%. As complicações intraoperatórias foram semelhantes nos dois grupos. A sobrevida no grupo PAPM > 25 mmHg foi menor que no grupo PAPM ? 25 mmHg. Conclusão: A prevalência da HPP observada foi menor que a descrita na literatura, ou seja, 50 (12,9%) pacientes apresentaram PAPM > 25 mmHg, sendo que destes, 49 (12,6%) apresentaram resistência vascular pulmonar < 240 dinas.s.cm-5, portanto sem HPP, e provavelmente tratava-se de circulação hiperdinâmica e/ou sobrecarga de volume. O fato de o paciente apresentar pressão arterial pulmonar média maior que 25 mmHg, não influenciou na sobrevida do enxerto / Abstract: Portopulmonary hypertension (PPH) is characterized by the development of pulmonary artery hypertension associated with portal hypertension, with or without liver disease. It is defined as a mean pulmonary artery pressure (MPAP) above 25 mmHg, pulmonary vascular resistance (PVR) above 240 dynes.s.cm-5 and the pulmonary capillary wedge pressure (PCWP) normal under 15 mmHg. The association of pulmonary hypertension with cirrhosis and portal hypertension is underdiagnosed, with an estimated prevalence of two to 6%. The mortality is directly proportional to the measured mean pulmonary artery pressure and pulmonary vascular resistance. Some studies suggest that the mean pulmonary artery pressure preoperatively is an independent predictor for mortality, and many centers consider that the mean pulmonary artery pressure above 50 mmHg is one absolute contraindication for liver transplantation (LT). Objective: To analyze the PPH prevalence, complications and survival in patients undergoing liver transplantation. Method: We analyzed retrospectively, records and data base from the Liver Transplantation Unit of the Clinical Hospital of UNICAMP of patients undergoing liver transplantation, a 21 year period. The population was separated into two groups according to the measurements of mean pulmonary artery pressure: MPAP ? 25 mmHg and MPAP > 25 mmHg. We studied the preoperative examinations, hemodynamic variables, as well as intraoperative complications, related events to PPH and survival. Result: We found 50 cases with MPAP > 25 mmHg, and one case met the diagnostic criteria for MPAP > 25 mmHg, PVR > 240 dynes.s.cm-5 and PCWP < 15 mmHg, according to European Respiratory Society Task Force on Pulmonary Hepatic Vascular Disorders (2004), and two cases of MPAP > 50 mmHg who had the procedure canceled, so our PPH prevalence in patients undergoing LT was 0,25%. The intraoperative complications were similar in both groups. The survival in group MPAP > 25 mmHg was lower than in group MPAP ? 25 mmHg. Conclusion: The prevalence rate observed was lower than described in literature, that is, 50 (12,9%) patients showed MPAP > 25 mmHg, and 49 (12,6%) of these patients had pulmonary vascular resistance < 240 dynes.s.cm-5 and therefore no PPH. This was probably due to hyperdynamic circulation and/or volume overload. The need for reliable and exact information that are needed to run a company is still growing. This fact leads to finding new ways to obtain, transform, but especially deliver these information. One of the most efficient way is to integrate Business Intelligence into company's information system that leads to the usage of analytical portals. This thesis's target is to acquaint the readers with the topic of analytical portals and conceivable method to evaluate analytical portals and tools designed to build them. This thesis consists of two general sections, theoretical and practical. The theoretical section consists of the explanation of the term analytical portal and the ways a company can benefit from usage of analytical portals. The criteria and the conceivable method to evaluate analytical portals and the tools designed to build them are specified further in this section. The practical section contains description and evaluation of certain tool designed to build analytical portals (SAS Portal). The evaluation is based on the criteria described in the theoretical section and data used in the exhibition of the SAS Portal are purely exemplificative.

In current BI projects, the main emphasis is not put on the process of working with data anymore. The main focus is on working with information -- on technologies and applications that enable to put the data into context and that make possible for the users to get access to information which they use during a decision process. The theme of the thesis is the analysis of possible ways the portal technologies that are incorporated in portal products can be used for working with information from BI systems. The aim of the thesis is to analyze the possibilities of enterprise portal from the viewpoint of its usefulness as a part of company's BI solution. Another objective is to evaluate the situation of the portal products market and to analyze the potentialities of the Microsoft Sharepoint 2010 portal product. The thesis is divided into two logical parts. The first part contains definitions of the terms connected to BI and enterprise portals. It also deals with general evaluation of the potentialities of portal technologies for BI and with evaluation of the portal products market situation. The second part focuses on Sharepoint 2010 -- the latest portal product from Microsoft. This part contains Sharepoint 2010 functionality evaluation considering the individual versions of this product. Further, the position of Sharepoint 2010 in Microsoft's current BI strategy is analyzed. The potentialities of the product as a part of Microsoft BI solution are evaluated. A company that considers buying a portal product can use the thesis as a source of general information about portal technologies and their usefulness in a company's BI. The thesis also offers up-to-date information about the current situation of the portal products' market. If the company considers buying Sharepoint 2010 portal product, the thesis is a source of detailed information which can contribute to a quick and high-quality decision on the suitability and availability of this solution.

Ibyggbranschen i Sverige idag strävas det efter att fortsätta utvecklingen av eneffektivisering gällande miljö, kvalitet och styrning.Samhället och byggbranchen har utvecklats och digitaliserats enormt de senasteåren, dock har vissa mindre delområden avstannat och det är bland annatdigitala metoder för arbetsordrar och registrering bland hantverkare.  Studien som är gjord syftar därför till att undersöka ifall det går atteffektivisera och digitalisera metoder gällande arbetsordrar och registrering.Uppsatsen syftar till att undersöka ifall det går att effektivisera styrningen,miljöpåverkan, kvaliteten och ekonomiska delar bland byggföretag med hjälp aven mobil applikationslösning.I studien har jag valt att använda mig utav teori gällande miljö, kvalitet,styrning och finansiering där jag fördjupat mig i delar som är påverkningsbarmed hjälp av ett digitalt verktyg i form av en mobil applikation och dessfunktioner.För ett införande av applikationslösningen krävs det en invänjning och enmotivation ifrån hantverkarna eftersom att det innebär en stor förändringjämfört med den manuella metoden som använts under en längre tidsperiod. Detframgår att arbetsbelastningen för vissa delar i arbetet kan minska blandtjänstemännen vid  ett fullständigtinförande av den mobila portalen eftersom att vissa led försvinner när detgäller bland annat arbetsordrar och registrering. Jag kan konstatera att enförbättrad och förenklad version av applikationslösningen krävs för att få ettfullständigt införande bland hantverkarna. / In Sweden there is a continuing effort to develop a more effective construction industry with a focus on environment, quality and control.When compared to the industrial sector it has been acknowledged that the construction industry has lagged behind the current environmental and quality requirements and standards. The construction industry has been developed and digitalized tremendously over the last few years; however some small businesses have not been able to follow.A couple of examples of this are the digital methods for work orders and registrations among craftsmen.In this study I will look at the possibilities of digitalization and the methods to produce work orders and registrations more effectively.This thesis aims to examine if it is possible to improve the control, quality and economical aspects of construction companies by using a mobile phone application.I have chosen to use theory concerning environment, quality, control and financial where I have studied the aspects which are influenced by a digital device in form of a mobile application and its functions.There is opposing consensus among the officials who are affected by the mobile phone application.It appears that the workload can be reduce clearly and the possibility to improve the control using the application. I can state that the mobile application must be simplified to get a total usage among the craftsmen.

Die Fakultät für Elektrotechnik und Informationstechnik der TU Chemnitz beabsichtigt, ein Internetportal für den Fachbereich aufzubauen. Zielgruppe des Portals sind in erster Linie Schülerinnen und Schüler, die sich für Technik und Naturwissenschaften interessieren. Mit Hilfe des Portals soll ihnen der Gegenstand der Elektrotechnik und Informationstechnik näher gebracht werden. Die Aufgabe der Bachelor-Arbeit besteht in der theoretischen Konzeption eines solchen Portals. Dazu wird zunächst der Begriff des Internetportals genauer beleuchtet. Es wird geklärt, welche Funktionen ein Portal erfüllt, wodurch es sich von herkömmlichen Internetseiten unterscheidet und wie es kategorisiert werden kann. Anschließend folgt eine Analyse bereits bestehender Schülerportale u.a. nach verschiedenen Kriterien der Benutzerfreundlichkeit. Die Grundlagen für diese Untersuchung werden vorab mit der Erläuterung des Verfahrens der Usability-Analyse und der Usability-Kriterien gelegt. Basierend auf den Erkenntnissen der Analyse sowie der Portaldefinition und den Usability-Kriterien erfolgt die theoretische Konzeption des Portals. Neben der Festlegung und Erläuterung zur Umsetzung der Portalfunktionen werden Vorschläge zur inhaltlichen Gestaltung und der Navigationsstruktur gegeben. Darüber hinaus werden verschiedene Maßnahmen erläutert, die dem Portal einen hohen Grad an Benutzerfreundlichkeit verleihen und damit zu dessen Erfolg beitragen sollen. Abgerundet wird die Arbeit mittels eines grafischen Entwurfs des Portals. / The faculty of Electrical Engineering and Information Technology at Chemnitz University of Technology intends to establish an internet portal for the department. The portal primarily addresses pupils who are interested in technology and natural sciences. It is aimed at giving them an understanding of the subjects of electrical engineering and information technology. The task of this bachelor's thesis consists in the theoretical conception of such a portal. Therefore, the term “internet portal” will be examined first. This will be done by clarifying which functions a portal fulfils, how it differs from a conventional website and how it can be categorised. Second, already existing portals for pupils will be examined, for instance for their usability. The basis for that will be laid down in advance by the explanation of the method of a usability-analysis and of the usability-criteria. The theoretical conception of the portal results from the findings of the analysis as well as the portal functions and the usability-criteria. Besides the determination of the functions of the portal which is to be created, and the explanation of their implementation, proposals for the content and the navigation structure will be given. Furthermore, different measures for the increase of the usability and the handicapped friendliness will be explained. The thesis will be completed by a graphic design of the portal.

An institutional focus on student learning and attainment : a case study of Tallahassee Community College

The overarching goals of this study were twofold: first, to learn how community colleges are preparing students to flourish and contribute to a changing and global economy, and second, how they address those students that do not make it through the community college. Within this context, the researcher elected to perform a case study of Tallahassee Community College (TCC), a college identified as focused on improving student success and empowering students to reach their goals; and, committed to sharing data and information across and throughout the institution. To achieve its ultimate goals, this study had four specific purposes. First, it provided an in-depth account of the development and implementation of the organizational vision and student success (learning and attainment) focus at TCC. Second, the study examined how the use of data provided transparency and redirected the focus of TCC. Third, it examined how TCC’s student learning portal is used by students, faculty, staff, administrators, and the board of trustees. Finally, it considered how the findings may inform policy makers, institutional leaders, and researchers about key factors impacting a student success focus. Taken in concert, the study was designed to provide a contextual framework for implementing an organizational vision focused on improving student success. The study is a qualitative research designed case study informed by quantitative data. The study included TCC serving as the unit of analysis for the entire case, and the student learning portal serving as the unit of analysis for an embedded case study. Qualitative methods employed included documents, archival records, interviews, focus groups, and direct observations. Multiple approaches were used to present the case study including a chronology of key historical events and a categorical or topical format based on an inductive analysis of data.
